Abstract:Emergency Medicine (EM) has continuously evolved since its origins on the battlefields of eighteenth-century Europe. Adapting to emerging challenges in healthcare, it has, in the past 70 years, developed to become a critical safety net for society. Despite its resilience and many accomplishments, EM still faces significant challenges, including workforce attrition, resource constraints, and the need for ongoing innovation. This paper explores the various adaptations EM has implemented to meet the demands of healthcare systems globally. By examining these factors and challenges, the paper outlines a future roadmap for EM, emphasizing global equity, interdisciplinary collaboration, and sustained investment to ensure that EM remains robust in addressing future healthcare challenges.

Abstract:Introduction: High-income country (HIC) authors are disproportionately represented in authorship bylines compared with those affiliated with low and middle-income countries (LMICs) in global health research. An assessment of authorship representation in the global emergency medicine (GEM) literature is lacking but may inform equitable academic collaborations in this relatively new field. Methods: We conducted a bibliometric analysis of original research articles reporting studies conducted in LMICs from the annual GEM Literature Review from 2016 to 2020. Data extracted included study topic, journal, study country(s) and region, country income classification, author order, country(s) of authors' affiliations and funding sources. We compared the proportion of authors affiliated with each income bracket using Χ2 analysis. We conducted logistic regression to identify factors associated with first or last authorship affiliated with the study country. Results: There were 14 113 authors in 1751 articles. Nearly half (45.5%) of the articles reported work conducted in lower middle-income countries (MICs), 23.6% in upper MICs, 22.5% in low-income countries (LICs). Authors affiliated with HICs were most represented (40.7%); 26.4% were affiliated with lower MICs, 17.4% with upper MICs, 10.3% with LICs and 5.1% with mixed affiliations. Among single-country studies, those without any local authors (8.7%) were most common among those conducted in LICs (14.4%). Only 31.0% of first authors and 21.3% of last authors were affiliated with LIC study countries. Studies in upper MICs (adjusted OR (aOR) 3.6, 95% CI 2.46 to 5.26) and those funded by the study country (aOR 2.94, 95% CI 2.05 to 4.20) had greater odds of having a local first author. Conclusions: There were significant disparities in authorship representation. Authors affiliated with HICs more commonly occupied the most prominent authorship positions. Recognising and addressing power imbalances in international, collaborative emergency medicine (EM) research is warranted. Innovative methods are needed to increase funding opportunities and other support for EM researchers in LMICs, particularly in LICs.

Abstract:OBJECTIVES: Ultrasound (US) has become an indispensable skill for emergency physicians. Growth in the use of US in emergency medicine (EM) has been characterized by practice guidelines, education requirements, and the number of EM US practitioners. Our purpose was to further document the growth of EM US by profiling the breadth, depth, and quality of US-related research presented at EM's most prominent annual research conference: the Society for Academic Emergency Medicine Annual Meeting.METHODS: We reviewed published research abstracts from the annual Society for Academic Emergency Medicine conferences from 1999 to 2015. Abstracts related to US were identified and examined for the number of authors and rigor of the research design. Designs were categorized as experimental, quasiexperimental, and nonexperimental. Abstract submissions were analyzed by the average rate of change over time.RESULTS: From 1999 to 2015, we observed a 10.2% increase in the number of accepted abstracts related to US research. This rate compared to a 3.2% average rate of change for all abstracts in general. The number of unique authors engaged in US research increased at a rate of 26.6%. Of the 602 abstracts identified as US related, only 12% could be considered experimental research.CONCLUSIONS: We observed larger increases in the number of US-related research relative to the total number of abstracts presented at a national conference. The number of investigators engaging in this research has also steadily increased. The research design of these studies was found to be primarily quasiexperimental. To improve the quality of EM's use of point-of-care US, more rigorous research with experimental designs is needed.

Global emergency medicine research priorities: a mapping review

Abstract:Recognizing and prioritizing research areas in emergency care is crucial for generating evidence and advancing research programs, aiming to enhance health outcomes for both individuals and populations. The objective of this review is to document global clinical and nonclinical research priorities. The Emergency Medicine Education and Research by Global Experts network, consisting of 22 sites across six continents, conducted a mapping review of publications on emergency medicine research priorities (2000-2022) across seven databases. We included studies with replicable methodologies for determining research priorities, excluding those limited to individual diseases. Three reviewers independently screened, selected, and categorized results into clinical and nonclinical topics. Discrepancies were resolved by an independent investigator and consensus. Outcomes measures and analysis include descriptive analysis of research priorities grouped into clinical and nonclinical topics, characteristics of publications including countries represented in the author list, target audience (such as researchers or policy makers), participants (e.g. patients), and methods (e.g. Delphi) of priority setting. Among 968 screened papers, 57 publications from all WHO regions were included. Most (36, 63%) had authors from only a single country, primarily in North America and Europe. Patient representatives were included in only 10 (18%). Clinical research priorities clustered into resuscitation, cardiology, central nervous system, emergency medical services, infectious disease, mental health, respiratory disease, and trauma. Distribution was broad in North America and Europe but focused on infectious diseases and resuscitation in Africa and Asia. Eleven nonclinical topics included access to care, health policy, screening/triage, social determinants of health, staffing, technology/simulation, shared decision making, cross-sectoral collaboration, education, patient-centered care, and research networks. Nonclinical topics were broad in Europe and America, focused on access to care and health screening in Africa, and mostly absent in other WHO regions. Published research priorities in emergency medicine are heterogeneous and geographically limited, mostly containing groups of authors from the same country. The majority of publications in global research priority setting stem from Western countries, covering a broad spectrum of clinical and nonclinical topics. Research priorities from Africa and Asia tend to focus on specific issues more prevalent in those regions of the world.

Abstract:To the Editor: As we have known, Emergency Medicine is an interdisciplinary subject and has many hot spots for research. However, many researchers often worry that their research is too narrowed or not so cutting edge as to be accepted by journals with high impact factor (IFs). Why? One reason perhaps lies in the fact that many of them have difficulty in following the fronts and hot spots in internal Emergency Medicine research. Here, we introduce the use of Essential Science Indicator (USA) tools to analyze the frontier and cutting edge researches in the field of internal Emergency Medicine, and we hope this can help researchers in finding research fronts. The 5 hotspots of internal Emergency Medicine research were “sepsis,” “acute pulmonary,” “trauma,” “shock” and “cardiopulmonary resuscitation.” Yes, sepsis is the hottest subject in Emergency Medicine. Although there are already some large studies published in top medicine journals about Surviving Sepsis Campaign Protocols (SSCP),[123456] SSCP is still the hottest front for years to come. The followed hotspots are acute lung injury; the most attention were paid on the acute respiratory distress syndrome, receptor signaling, and acute lung injury evaluation. Abstract:The year 2020 was not easy for Emergency Medicine (EM) clinicians with the burden of tackling a pandemic. A large focus, rightfully so, was placed on the evolving diagnosis and management of patients with COVID-19 and, as such, the ability of clinicians to remain up to date on key EM pharmacotherapy literature may have been compromised. This article reviews the most important EM pharmacotherapy publications indexed in 2020. A modified Delphi approach was utilized for selected journals to identify the most impactful EM pharmacotherapy studies. A total of fifteen articles, eleven trials and four meta-analyses, were identified. This review provides a summary of each study, along with a commentary on the impact to the EM literature and EM clinician.

Abstract:A key skill for successful clinician educators is the effective dissemination of scholarly innovations and research. Although there are many ways to disseminate scholarship, the most accepted and rewarded form of educational scholarship is publication in peer-reviewed journals. This paper provides direction for emergency medicine (EM) educators interested in publishing their scholarship via traditional peer-reviewed avenues. It builds upon four literature reviews that aggregated recommendations for writing and publishing high-quality quantitative and qualitative research, innovations, and reviews. Based on the findings from these literature reviews, the recommendations were prioritized for importance and relevance to novice clinician educators by a broad community of medical educators. The top items from the expert vetting process were presented to the 2016 Canadian Association of Emergency Physicians (CAEP) Academic Symposium Consensus Conference on Education Scholarship. This community of EM educators identified the highest yield recommendations for junior medical education scholars. This manuscript elaborates upon the top recommendations identified through this consensus-building process.

Abstract:Approaching the end of another year, we are pleased to bring you a further world-spanning collection of articles in this issue of the Journal of Evidence Based Medicine. As well as covering the globe, with contributions from Bahrain, Brazil, Canada, China, England, Malaysia, the Netherlands and the USA we are also able to bring you a variety of types of article and study relevant to evidence based health care. These articles present the findings of new research studies, reviews of previous studies, and discussions of the quality of research. In a controlled trial of preservative-free 0.9% sodium chloride for flushing and locking peripheral intravenous access devices, which recruited nearly 400 patients with gastroenterological or hepatic diseases, the solution was judged to as safe and effective as the more commonly used hepatic solution. Another study, from the USA, found no direct evidence that testing for anaemia in patients with chronic kidney disease improved their morbidity or mortality. A systematic review of five randomised trials, with nearly 400 patients evaluates the effect of ephedrine on intubation conditions one minute after anesthesia induction using propofol and rocuronium. While, another review with more studies (21 trials) and participants (1619 patients with lung cancer) is cautious about drawing conclusions on the effects of the injection of brucea javanica oil emulsion with chemo-radiotherapy, because of the low quality of the studies. Systematic reviews themselves are the material for an article from a team of researchers in Malaysia. They performed a cross sectional study of systematic reviews to examine how the ‘number needed to treat’ statistic was used in these reports. The head of the Public Health Program at the College of Health Sciences in the University of Bahrain, Dr Asokan, writes about the use of an evidence based practice curriculum in the teaching of allied health professionals. Finally, Lelia Duley from the Nottingham Clinical Trials Unit in England describes the importance of public involvement in research. She discussed this topic in a podcast for International Clinical Trials Day earlier this year and we are also able to share with you the words from several authors of Cochrane Reviews that have featured in these Cochrane Evidence Pods more recently. They describe their Cochrane Reviews of vaccines for shingles or herpes zoster,[1] combined pharmacotherapy and behavioural interventions for smoking cessation,[2] garlic for patients with hypertension,[3] and inhaled analgesia for pain management in labour.[4]
International Collaboration in Taiwan Emergency Department Publications: A Social Network Analysis

Abstract:Background: Emergency medicine (EM) is a growing specialty both clinically and academically. Academic EM development can be measured by number of scientific publications. This study aimed to evaluate the academic international cooperation trend of Taiwan emergency departments (EDs) in the past two decades using social network analysis (SNA). Methods: The study population were publications with first author affiliated with Taiwan EDs and the study duration was publication year before 2021. The enrolled publications were categorized into two groups: Group one, all authors were affiliated with Taiwan (domestic publications) and Group two, authors were affiliated with Taiwan and other countries (international publications). The primary outcome measurement was the degree centrality of Taiwan before 2021. The secondary outcome measurements included the trend in annual publication number of Group one and Group two, the trend in country number of each year in Group two, the top five countries that collaborate with Taiwan, the difference between the median of citation numbers in Group one and Group two, and the difference between the median of author numbers in Group one and Group two. Results: A total 4,363 publications were enrolled, of which 4,046 publications were classified in Group one and 317 publications were in Group two. The annual publication number of both groups increased significantly. The annual country number of collaboration with Taiwan ED publications had also significantly increased. The median of citation number and author number in Group two were both significantly higher than Group one. The top five countries collaborating with Taiwan were the United States, China, Malaysia, Japan, and Australia. Conclusions: Taiwan EDs' growing international collaboration in the past two decades indicated a capacity to conduct multi-country research. International collaboration publications obtained higher citations compared to domestic publications. Researchers should enhance international collaborations for academic advancement.
Developmental trends in academic emergency medicine journals, 2000 to 2019

Abstract:We investigated academic developmental trends in emergency medicine (EM) by analyzing the performance of EM journals. Data from the Journal Citation Reports (JCR) database for EM category journals, including journal titles, language, numbers, and impact factors (IFs) from 2000 to 2019 were collected. The aggregate IFs of EM and 11 other categories (cardiac and cardiovascular systems, clinical neurology, critical care medicine, gastroenterology and hepatology, infectious diseases, general and internal medicine, pediatrics, respiratory system, surgery, toxicology, and urology and nephrology) were collected from 2003 to 2019. The slope of the linear regression was used to evaluate the trend in EM journal IFs and the aggregate IFs of all categories. Pearson’s correlation coefficient was used to evaluate the correlation between EM journals’ IF in 2000 and their IF trend from 2000 to 2019. The EM journal number increased from 8 (all in English) in 2000 to 31 (26 in English) in 2019. In total, 28 EM journals had a positive IF trend since their initial enrollment into the JCR database, and the trend was significant for 18 journals. The correlation of the EM journals’ IF in 2000 and IF trends from 2000 to 2019 was 0.75. The increasing trend of aggregate IF for the EM category was significant. In conclusion, the increased number, language diversity, and IF trend for EM journals indicates that the development of academic EM is a continuous international trend. In the past 20 years, the IF trend increased faster for EM journals with a higher initial IF. The overall performance of EM journals was non-inferior to other medical specialties.

Abstract:Reaching the end of 2011, this issue of the Journal of Evidence Based Medicine contains a rich variety of material from around the world, with articles from England, Malaysia, the Philippines as well as China, and contributions from the authors of Cochrane reviews in Denmark and Australia. We write this editorial at the time of another major earthquake, this time in Van in Turkey, but with a much lower loss of life than other natural disasters we have featured in previous issues of the Journal. It raises once again the importance of reliable evidence to inform those working in disaster risk reduction, planning and response and we are pleased to report that Evidence Aid's First Conference took place in Oxford England in September, with a follow-up meeting during the Cochrane Colloquium in Madrid Spain in October. We hope to bring you a report of the outcome of those meetings in our first issue of 2012. Returning to this issue, Julie Burrett and Daniel Lunn from the University of Oxford in England analyze a large cohort of randomised trials of treatments for childhood leukaemia; examining factors which might have influenced the timing and prominence of the reporting of the findings of the trials. This sits neatly alongside a look back at the joint statement for promoting publication ethics in medical journals in China, which first appeared five years ago.
