Abstract:Introduction Up to 50.8% of Nepali older adults have depression, and the suicide rates among adults over 80 are nearly quadruple the rate among the general population. Addressing geriatric mental health in Nepal is difficult due to several factors. First, awareness is limited, even among healthcare providers. This can be partially attributed to limited mental health curricula—just 25-92 hours of didactic and 2-4 weeks of clinical training for MBBS degree holders—and specialist care is limited to one psychiatrist per 200,000 people. Second, 85% of people live in rural or remote areas, and 83% of those live in poverty, often lacking health insurance. Finally, stigma against mental health disorders, especially psychosis, is equally rampant among the general population and health providers. Therefore, access to mental health care is limited by financial constraints, lack of awareness, and cultural stigma, causing many to go without it. Traditional healers, called Dhamis or Jhankris, play a vital role in providing care as they are the first line of physical and spiritual care for approximately 85% of the population. Including them in any plan to address the psychiatric needs of older adults is essential. This study aims to develop evidence-based and culturally conscious strategies to improve geriatric mental health outcomes in Nepal. Methods Our approach combines a thorough literature review on geriatric mental health and traditional healers in Nepal with ethnographic data collected in May 2023 by our team, highlighting cultural factors impacting mental health. We use this data to formulate evidence-based strategies, including implementing tailored training modules. Results Multiple studies have quantified the rates of depression and its major risk factor, loneliness, among geriatric populations in Nepal. Rates of depression have been estimated between 11.7% and 50.8%, while rates of loneliness range from 55.6% to 68.7%. Nepali adults >80 years old have a suicide rate of 37.4 per 100,000, compared to 9.77 per 100,00 in the general population. Significant correlates of depression include age, living arrangement, perceived health status, perceived economic satisfaction, and subjective loneliness. Although people in Nepal seek out traditional healers as their first choice when they become ill, most are elderly, and their numbers are diminishing. Current public education curricula neglect traditional healing, causing a knowledge deficit among the younger generation. This, along with a lack of interest and high emigration rates among younger citizens, results in intergenerational loss of health knowledge. There are far more traditional healers than approximately 3,500 Western medicine-trained physicians in Nepal (estimated 400-800,000). After receiving a 2-day HIV/AIDS training, traditional healers played a role in reducing HIV stigma by providing culturally acceptable health education to the community. Including them in psychiatric interventions could be of great help. Medical education in Nepal lacks adequate training to address mental health issues. The World Psychiatric Association and the World Federation of Medical Education have created guidelines for psychiatry education, but they must be fully integrated into Nepal's current medical curriculum. The World Health Organization has developed a task-sharing intervention guide called mhGAP to train medical officers and non-specialized healthcare providers, which has helped improve mental health services in Nepal. Conclusions Our recommended implementation strategy involves developing mental health training modules. Collaboration with NGOs such as Transcultural Psychosocial Organization and AAGP professionals will aid in module development and implementation. Our model includes training traditional healers and medical officers to assess and manage mental health problems among older adults. In conclusion, our target population has an incredibly high need for mental health services. High rates of depression are exacerbated by loneliness, low socioeconomic status, and perceived stress, leading to high rates of suicide among this group of aging adults in isolated and shrinking communities. Further work on this project will address other psychiatric issues, including psychosis and dementia. Our study suggests training approaches proven effective in other contexts to equip traditional healers with skills to address geriatric mental health issues. Additionally, strengthening mental health education during MBBS programs in Nepal can enhance competence in this area among medical graduates.
