Abstract:Objective To study the feasibility, safety, and efficacy of thoracoscopic surgery in the treatment of pediatric mediastinal neurogenic tumors, and summarize the treatment experiences and surgical skills. Methods A single-center retrospective analysis of 37 patients with pediatric mediastinal neurogenic tumors was conducted. Clinical charactersistics and postoperative complications were all analyzed. Results All the operations were successfully completed. There was no statistically significant difference in tumor diameter between the two groups (p > 0.05). The open surgery group had an average operation time of 96.5 +/- 32.38 min, while the thoracoscopic surgery group had an average operation time of 78.3 +/- 24.51 min (p < 0.05). The thoracoscopic surgery group had significantly lower intraoperative blood loss than the open surgery group (p < 0.05). In addition, the duration of the postoperative thoracic drainage tube was 5.43 +/- 0.76 days in the open surgery group, which was longer than the 2.38 +/- 0.87 days in the thoracoscopic surgery group (p < 0.05). Furthermore, the postoperative length of hospital stay was an average of 10.23 +/- 1.43 days for the open surgery group, longer than for the thoracoscopic surgery group (4.36 +/- 0.87 days) (p < 0.05). Conclusions Thoracoscopic surgery has several advantages in the treatment of pediatric mediastinal neurogenic tumors and is worthy of clinical popularization and application. For giant mediastinal malignant neurogenic tumors, puncture biopsy and adjuvant chemotherapy can be performed before surgery to lessen the tumor volume and enlarge the operation space, which would reduce bleeding and complications.
Transaxillary Minithoracotomy in Intrathoracic Surgery for 316 Infants and Children

Abstract:Objective To introduce the technique of intrathoracic surgery performed through vertical transaxillary minithoracotomy.Methods From March 1989 to March 2001, 316 patients underwent intrathoracic surgery through a vertical transaxillary minithoracotomy. 285 patients suffered from patent ductus arteriosus (PDA), 10 congenital esophageal atresia, 8 congenital pulmonary cysts, 6 congenital emphysema, 1 pulmonary sequestration, 5 mediastinal tumor, and 1 eventration of the diaphragm.Results All of the patients were successfully treated under satisfactory exposure. No operative mortality and severe postoperative complications occurred.Conclusions Intrathoracic surgery performed through a vertical transaxillary minithoracotomy appears to be less invasive, and is a simple, safe, cosmetically acceptable and efficient approach.

Abstract:Objective Retrospectively analysis of our hospital thoracoscopic lobectomy cases in infants and children.This study evaluates the safety and efficacy of thoracoscopic lobectomy in infants and children.Methods Retrospective analysis of our hospital from April 2015 to August 2016,50 consecutive patients were plans to implement thoracoscopic lobectomy,excluding extralobar isolated,lung bullae,lung biopsies.35 males and 15 females,aged 2 months and 13 years,the average (5.94 ±3.94) years,3.25-59.00 kg body weight,average(22.15 ± 12.54) kg.11 cases of prenatal ultrasound to confirm.21 cases have a history of recurrent pneumonia.3 patients had a history of hemoptysis,are leaf-type isolation within the lung.Results 50 patients were successful in 46 cases by thoracoscopic surgery,4 patients underwent thoracotomy,transit rate of 8％.Transfer 4 cases,2 cases of left upper lobe,13 year-old and 15 year old children,preoperative recurrent pneumonia,pleural adhesions.1 case of right lower lobe,right lower pulmonary artery surgery damage the basal segments,bleeding.1 case of left lower lobe,the upper and lower leaf division stunted.VATS 40-300 minutes,an average of 120 minutes.There was no operative mortality,postoperative bleeding reoperation case who,as a transit cases thoracotomy.Lesion distribution right upper lobe in 5 cases,1ease of right middle lower,19 cases of right lower lobe,left upper lobe in 7 cases,18 cases of left lower lobe.Histological examination showed bronchial pulmonary cyst in 4 cases,leaf-type isolation within the lung in 15 cases,cystic adenomatoid malformation in 30 cases(type I 17 eases,type Ⅱ 13 cases).Indwelling chest tube after 2-3 days in hospital after 5-10 days,an average of 7 days.Postoperative follow-up 1-12 months,no recurrence and thoracic collapse,the remaining lung well compensated.Conclusion VATS lobectomy with less trauma,quicker recovery after surgery.However,due to the small chest in children,one lung difficulties,thoracoscopic operation requires a longer learning curve.Preoperative recurrent infections,pleural adhesions,fissure dysplasia will increase the rate of conversion to open.
Applications and Outcomes of Thoracoscopy for Pediatric Solid Tumors

Abstract:Objective To discuss the treatment and prognosis of malignant solid tumors in children.Methods Over the past 8 years,cases of malignant solid tumor with pulmonary metastasis in children were collected from department of pediatric surgery of Shanghai Xinhua Hospital and Shanghai Children's Medical Center.The results were discussed including classifications,treatment,and prognosis.During 2007-2015,a total of 14 patients(8 male and 6 female) with the age ranging from 1 to 11 years with a median of 5 years were under treatment,including 2 cases of hepatoblastoma,6 cases of nephroblastoma,3 cases of rhabdomyosarcoma,1 case of medulloblastoma,immature teraoma and neuroblastoma separately.Four patients only underwent biopsy,the rest of them underwent thoracoscopic lung metastases resection whose pulmonary metastasis was localized and become 1-2 of peripheral pulmonary nodules after several courses of chemotherapy.Results After thoracoscopic lung metastases resection,malignant tumor cells were detected with the same primary pathology in 13 cases while no active tumor cell was discovered in 1 case.After the surgery,chemotherapy continued according to the plan.Patients were followed up for 14.5 months(1-96 months),2 patinets lost follow-up,and 2 patients died.No surgical related complications or death occurred.Conclusions Children with stage Ⅳ malignant solid tumors still should be treated actively.Resection and pathological examination is helpful in the evaluation of prognosis and making further treatment options in patients whose lung metastases is stable after chemotherapy and removal of primary tumor.Thoracoscopy-a minimally invasive technique,provides an acceptable option for parents.
Video-assisted Thoracoscopic Lobectomy in Children: a Retrospective Single-Center Study with 175 Consecutive Cases

Abstract:Objective To evaluate the safety and efficacy of video-assisted thoracoscopy for lung diseases in infants and children.Methods From September 2014 to June 2016,a retrospective review was conducted for 175 consecutive patients undergoing video-assisted thoracoscopy.Clinical data,surgical procedures,surgical findings and postoperative pathological examinations were analyzed.Results All procedures were successfully completed.There was no single case of severe complication or re-operation.There were 100 boys and 75 girls.The mean age was (18.5 ± 41.6)(2-156) months,mean operative duration (75.0 ± 37.9) (15-120) minutes and mean blood loss (5.0 ± 26.9)(1-300)ml.There were conversions into open thoracotomy (n =7) and open laparotomy (n =1,4.6％).Postoperative pathological examinations revealed lung cystic adenomatoid malformations,i.e.congenital pulmonary airway abnormalities (CPAM,n =105),pulmonary sequestration (n =47),lung tumor (n =9),emphysema (n =6),pulmonary cyst (n =4),pulmonary vascular lymphatic malformation (n =1),pulmonary arteriovenous fistula (n =1) and pulmonary parasitic granuloma (n =1).Conclusions Safe and effective for pulmonary resection,video-assisted thoracoscopy is indicated for treating lung diseases in infants and children.This minimally invasive approach offers less surgical trauma,minor postoperative pains and shorter hospitalization stay.In addition,operative scar is smaller and cosmetic outcomes are better.

Abstract:Objective To review the cases of thoracoscopic lobectomy and summarize the experiences of thoracoscopic lobectomy in children.Methods We reviewed all cases of thoracoscopic lobectomy at our hospital from June 2011 to January 2014.The intraoperative findings and postoperative outcomes were analyzed.Results There were 13 males and 8 females with an age range of 4 months to 11 years.The locations were left lower (n =6),left upper (n =3),right lower (n =7),right upper lobe (n =2) and extrapulmonary sequestration (n =3).Ten patients accepted onequng ventilation while the remainder bilateral ventilation.Conversion was required because of adhesion (n =3) and hemorrhage (n =1).In thoracoscopic cases,the range of operative duration was 40-130 min and hemorrhage was estimated at 5-40 ml.The postoperative pathologic examinations confirmed congenital cystic adenomatoid malformation (n =10),intrapulmonary sequestration (n =7),extrapulmonary sequestration (n =3) and fibroblastoma (n =1).For thoracoscopic cases,chest tubes were removed at Days 3-4 and were discharged at Days 5-7 postoperation.There were no postoperative complications.The converted cases were discharged at Days 10-14 postoperation.The short-term prognosis was all fair.Condusions Thoracoscopic lobectomy offers many advantages of miniinvasiveness,better intraoperative exposure,shorter operative duration,minimal postoperative pain,rapid recovery and fewer respiratory complications.It is ideal for children.Differential ventilation,history of pneumonia,individual age,body built and incomplete fissure are the major influencing factors of operative difficulty.

Abstract:Objective To compare the therapeutic efficacies of small incision versus thoracoscopic surgery in the treatment of pediatric mediastinal tumor.Methods From January 2007 to December 2014,92 surgical cases of mediastinal tumor were divided into two groups according to different operative approaches.Group A (n =35) undertook small incision operation while group B (n =57) had thoracoscopy.Operative duration,intraoperative volume of blood loss,postoperative drainage time,hospitalization time and postoperative complications of two groups were recorded and compared.Age,body weight and tumor size were basically similar and had no significant inter-group difference.The average operative duration was shorter in group A than that in group B [(75.89 ± 7.81) vs (87.84 ± 11.88) min],the volume of intraoperative blood loss was similar [(27.17 ± 10.58) vs (28.51 ± 10.51) ml],average postoperative drainage time and hospitalization time of group A were slightly longer than those of group B [(5.40 ± 1.48) vs (4.63 ± 1.47) days,(7.23 ± 1.40) vs (5.81± 1.34) days].All operations were successfully completed.There was no conversion into thoracotomy in group B.Recovery was uneventful and there was no onset of severe complications.Results Conclusions Compared with small incision surgery,thoracoscopy offers the advantages of smaller incision and faster recovery for mediastinal tumors under 5 cm.However,it is more timeconsuming.

Abstract:Thoracoscopic lobectomy offers the benefit of having fewer overall and respiratory complications than thoracotomy. It did not, however, clearly outperform open surgery in terms of operative time, wound infection, or postoperative hospitalization. Background Thoracoscopic lobectomy is always compared with open thoracotomy in congenital lung lesions with no definitive results. This systematic review and meta‐analysis compared the clinical outcomes of thoracoscopic versus thoracotomy lobectomy in children. Methods The electronic databases MEDLINE, EMBASE, and PubMed were searched. A meta‐analysis was used to analyse and compare the operating time, length of hospital stay, and postoperative complications, such as wound infections and respiratory issues, in thoracoscopic and thoracotomy lobectomy. Results Two thousand and ninety five paediatric patients were analysed over 17 studies (1 ambidirectional cohort study and 16 retrospective studies). Of these patients, 903 and 1192 had undergone thoracoscopic lobectomy and thoracotomy, respectively. There was a significant lower incidence of overall and respiratory complications and longer operative time for thoracoscopic lobectomy compared with thoracotomy in children; however, pooling the results for the two methods revealed no significant difference in wound infection or length of hospital stay. Conclusion Thoracoscopic lobectomy offers the benefit of having fewer overall and respiratory complications than thoracotomy. It did not, however, clearly outperform open surgery in terms of operative time, wound infection, or postoperative hospitalization.

Abstract:[Objective] To evaluate the application of video-mediastinoscopy(VM)and video assisted thoracoscopy surgery(VATS)in diagnosis on mediastinal small cell cancer.[Method] 20 patients with mediastinal lesions undertook 16 VM biopsies and 4 VATS exploration form July 2000 to March 2006.Three patients with massive thoracic pleural effusion received talc pleurodesis and 1 patient with pericardial effusion received pericardial window after malignancy were confirmed.[Results] All patients recovered smoothly without mortality and severe postoperative complications.Small cell cancer was confirmed in all the 20 patients.One of them was hematogenic and lymphagenic,2 of them were from thymus and other 17 lesions were from lung.Both thoracic pleural effusion and pericardial effusion were well controlled.[Conclusion ] VM and VATS is a mini-invasive,safe and accurate method in diagnosis on mediastinal small cell cancer.It is the best way to make a histological diagnosis on such kind of lesions.Also,effective treatment on thoracic effusion and pericardial effusion could be performed through VATS.
Experience of video-assisted thoracoscopic resection for posterior mediastinal neurogenic tumours: a retrospective analysis of 58 patients.

Abstract:Introduction: The objective of this study was to review the experience of video-assisted thoracoscopic resection of posterior mediastinal neurogenic tumours and to investigate the technical features and difficulties of this thoracoscopic approach. Methods: From May 2001 to June 2011, 58 consecutive patients underwent thoracoscopic resection of posterior mediastinal tumours sequentially in our institution, including 36 males and 22 females. The median age of the patients was 38.7 years. The median tumour size was 4.9 cm. Twenty-four lesions were located at the left side, 33 lesions at the right side and 1 lesion at the bilateral side. All procedures generally required three ports, and intracapsular enucleation was preferred. For bulky tumours, dense adhesion and massive bleeding, conversion to thoracotomy was performed by extending the anterior incision to 6-10 cm. Results: All procedures were successfully performed without death or serious complications occurring. The average operating duration was 127.2 min. The average intraoperative blood loss was 206.4 mL. The average chest tube drainage duration was 2.72 days. The average post-operative stay was 5.19 days. Fifty-three procedures were performed entirely under thoracoscopy to achieve gross total resection. There were five cases (8.6%) of conversion to thoracotomy procedure. Seven patients suffered from post-operative complications, including four Horner syndromes. No local recurrence occurred after an average follow-up of 44.9 months. Conclusion: Video-assisted thoracoscopic resections of the posterior mediastinal tumours were safe and reliable for selected patients with mastered thoracoscopic skills. Intracapsular enucleation was demonstrated to be a safe procedure. For tumours larger than 6 cm and located in the apex, the risk of the operation increased significantly.
Thoracoscopic segmentectomy for congenital and acquired pulmonary disease: a case for lung-sparing surgery

Abstract:Purpose: Over the last 15 years thoracoscopic lobectomy for congenital and acquired lesions has become an accepted modality in pediatric thoracic surgery. There is still debate about the need to perform a complete lobectomy for some of these lesions, and some advocate observation rather than resection, despite possible long-term complications of untreated lesions. High-resolution computed tomography (CT) scans and physical findings at the time of surgery, along with new advanced techniques, now allow for discrete partial anatomic resections, which may preserve normal lung. This study evaluates the feasibility and early results using these techniques in selected cases. Patients and methods: With institutional review board approval, the records of all patients undergoing thoracoscopic lung resection were reviewed. From January 2006 to December 2012, 23 patients, ranging from 1 month to 16 years of age and weighing 3.8-42 kg, underwent thoracoscopy for planned resection. Pathology was congenital cystic lung disease in 19 patients, bronchiectasis in 3 patients, and arteriovenous malformation in 1 patient. In each case findings on CT scan and at the time of surgery warranted consideration of lung-preserving surgery. Procedures were performed through three ports using single lung ventilation and CO2 insufflation to achieve lung collapse. The LigaSure™ device (Covidien, Norwalk, CT) was the primary instrument used to seal and divide the lung parenchyma and seal vessels. Results: All procedures were completed successfully thoracoscopically. An anatomic segmental resection was achieved in 22 of 23 cases. Operative time ranged from 30 to 300 minutes (mean, 120 minutes). Segmental resections included the left upper lobe apical/posterior (n=4), lingula (n=3), left lower lobe superior (n=5), medial or posterior basal (n=3), right middle lobe medial (n=1), right upper lobe apical (n=1), right lower lobe superior (n=4), and posterior basal (n=2). Two patients had more than one segment excised. Chest tubes were left in for 24 hours in 16 cases, 48 hours in 4 cases, and 5 days in 1 case. Hospital stay ranged from 1 to 6 days (mean, 2 days). Follow-up CT scans obtained at 1-6 years (mean, 28 months) show no residual disease in 20 of 21 patients. One patient underwent a nonanatomic resection and had evidence of recurrent congenital pulmonary airway malformation at the 4-year follow-up. This patient underwent a secondary thoracoscopic resection. Conclusions: Thoracoscopic lung-conserving therapy is technically feasible and safe in infants and children. The magnification provided by a thoracoscopic approach makes identification of segmental anatomic planes easier, aiding in safe dissection and resection. Anatomic resection appears to be associated with a low morbidity. It may be appropriate in the case of bilateral or extensive disease or in cases where the diseased tissue is clearly limited to an anatomic segment. Continued long-term follow-up is needed.
Clinical application of thoracoscopic surgery in the treatment of pediatric thymoma

Abstract:Objective:To explore the safety and effect of video-assisted thoracoscopic resection of pediatric thymoma.Methods:From January 2016 to December 2019, a total of 6 children with thymoma were recruited.There were 5 boys and 1 girl with an age range of (10.1±2.7) years.Five of them were associated with ocular myasthenia gravis.Preoperative computed tomography (CT) indicated that tumor envelope remained intact and there was no obvious invasion of surrounding tissues.Tumor size was 2.3 - 5.1 cm.All of them underwent right thoracoscopic thymomectomy.Results:All 6 cases were cured.Operative duration was (52.5±17.5) min and volume of blood loss (6±4) ml.No wound or intrathoracic infection occurred and there was no postoperative crisis of myasthenia crisis.They were discharged at (4±1) days postoperatively.Postoperative pathological examination confirmed thymoma.According to the WHO International classification of thymic tumors, the clinical types were A (n=3), AB (n=1) and B1 (n=2). And all of them belonged to stage I. During a follow-up period of (25±23) months, there was no recurrence.Conclusions:Thoracoscopic resection is both safe and feasible for pediatric thymoma.And comprehensive managements (including surgery) are required for thymoma complicated with myasthenia gravis.
A Clinical Comparative Study of Thoracoscopic Anatomical Lesion Resection and Lobectomy in the Treatment of Congenital Lung Malformations

Abstract:Background: To explore the safety and feasibility of HuaXi thoracoscopic anatomical lesion resection (HX-TALR) in the treatment of congenital lung malformations (CLMs) in children. Methods: A retrospective review of clinical data was conducted for patients who underwent HX-TALR and thoracoscopic lobectomy (TL) in our hospital from October 2017 to March 2023. Intraoperative and postoperative outcomes were compared between the HX-TALR and TL groups. Results: There were 485 patients in this study, 267 of whom underwent HX-TALR and 218 of whom underwent TL. All patients underwent thoracoscopic surgery without conversion to open surgery. No patients had major complications, including bronchopleural fistula, hemorrhage, atelectasis, recurrence or reoperation. The operative time, intraoperative bleeding volume, cases with thoracic drainage tubes, postoperative hospital stay, and cases with postoperative fever in HX-TALR were greater than those in TL (P < 0.05). Conclusions: HX-TALR is safe, feasible, and retains all normal lung tissue while removing the lesion, which is expected to become the preferable operation for the treatment of CLMs. HX-TALR is a new and technically challenging procedure that needs to be carried out after training. Thoracoscopic repair of oesophageal atresia: experience of 33 patients from two tertiary referral centres.

Abstract:With advances in minimally invasive surgery, thoracoscopic repair of oesophageal atresia has become popular in many centres worldwide and indeed has been described as the pinnacle of neonatal surgery. Here, we report our experience in two tertiary referral centres.Thoracoscopic technique was introduced in 2007. Thus, a retrospective review of all patients diagnosed with oesophageal atresia was carried out. Patients who had thoracoscopic repair were included, and those who had open repair due to co-morbidities were excluded. Patient demographics, operative data, complications, and associated anomalies were noted.A total of thirty-three patients underwent thoracoscopic repair during the time period. Thirty-one were successfully repaired thoracoscopically. Two patients had conversions due to intra-operative instability. The mean body weight of the neonates was 2.58 kg. The mean operative time was 146 min. Three patients suffered from minor anastomotic leaks, which healed on conservative management. Seven patients had anastomotic strictures, which responded successfully to endoscopic dilatation. Two patients died in the post-operative period due to pneumonia. One patient had a recurrent fistula 3 months after the primary repair, and he subsequently underwent a successful second repair.In experienced hands, thoracoscopic repair of oesophageal atresia is at least as good as open surgery but with less surgical trauma. Standard of post-operative care contributes significantly to post-operative outcome. Thoracoscopic technique is now our preferred approach.
Clinical analysis of 100 cases of thoracoscopic mediastinal tumor resection

Abstract:Objective:To investigate the curative effect of video -assisted thoracoscopic resection of mediastinal tumor.Methods:One hundred patients under the thoracoscopic mediastinal tumor resection were collected treated by a double lumen endotracheal intubation,intravenous anesthesia,thoracoscopic mediastinal tumor resection.Results:One hundred cases of operating mediastinal tumor patients were all successful,without conversions to open chest operation. The operation time was (75.6 ±5.2)min,and the intraoperative blood loss was (60.10 ±7.8)ml,with the postoper-ative chest drainage time 1 ~4 days,and the postoperative hospitalization time 3 ~7 days.No improvement in 9 cases with myasthenia gravis,2 cases with simple anemia symptoms.Conclusion:Video -assisted thoracoscopic mediastinal tumor resection operation is minimally invasive,safe,and can shorten postoperative recovery time.

Abstract:We present a case of prenatal diagnosis of a mediastinal cyst by ultrasound, confirmed during infancy through computed tomography (CT) and magnetic resonance imaging (MRI). A 6-month-old male infant was admitted to the hospital. A mid-mediastinal cyst was detected through a province-wide fetal ultrasonography screening program. During the infancy stage, the CT scan and the MRI confirmed the finding of a mediastinal cyst. Based on the findings on prenatal ultrasonography and postnatal imaging studies (CT, MRI), a benign mediastinal cyst was suspected. The cyst was below the tracheal carina, and the patient developed symptoms of respiratory distress. Owing to the enlargement trend of the cyst, the respiratory distress it has caused, and the anxiety it caused to the patient's family, a multi-disciplinary conference reached the consensus that surgical resection is the treatment of choice and a carina-approach mediastinal lesion resection via video-assisted thoracoscopic surgery (VATS) was performed. The cyst was removed successfully. Although this report is of a single case and follow-up duration is not long enough, we nevertheless make the first move to improve the pattern to a feasible, reliable, and efficient one, which could be used to treat similar patients.
Diagnosis and Surgical Management of Mediastinal Cystic Lymphangioma.

Abstract:Objective To study the clinical manifestations,diagnostic methods,surgical management and prognosis of patients with mediastinal cystic lymphangioma.Methods Seven patients with mediastinal cystic lymphangioma were analyzed retrospectively.Results Seven patients were operated with complete resection and proved cystic lymphangioma with anatomy,there was no recurrence during long-term follow-up.Conclusion Cystic lymphangioma of mediastnum are benign and could be detected with CT.Surgical resection can achieve effective result and satisfactory prognosis.The minimal invasive technology and video assisted thoracoscopy surgery are of special value in this treatment in the near future.
Surgical resection for congenital lung malformation: Lessons learned from thoracotomy to biportal thoracoscopy under one-lung ventilation

Abstract:Background: The purpose of this study is to compare the clinical characteristics and surgical outcomes of thoracotomy and video-assisted thoracoscopic surgery (VATS) in children with congenital lung malformations (CLMs) in a tertiary referring center and to report our modified biportal VATS setting. Methods: This is a single-center retrospective chart review study including children who underwent surgical resection for CLMs between January 2007 and December 2020. Patient characteristics and surgical outcomes were compared between open and thoracoscopy, as well as conventional VATS and biportal VATS. Biportal setting included an anterior utility wound and a camera trocar wound with one-lung ventilation. Results: A total of 100 patients were identified. Twenty patients received thoracotomy, and 80 patients received VATS (67 conventional and 13 biportal VATS). The median age at operation was 0.4 months in the thoracotomy group and 4.7 months in the VATS group. More patients in the thoracotomy group had preoperative symptoms, comorbidities, and emergent operations. The patients who underwent thoracotomy had significantly longer postoperative ICU stays, chest tube durations, hospital stays, and more complications. The pathological analysis revealed 67 congenital pulmonary airway malformations, 27 pulmonary sequestration, 6 hybrid lesions, and one accompanying pleuropulmonary blastoma. Compared to conventional VATS, the ICU stay was shorter in the biportal VATS group, with comparable operative durations, hospital stay and complications. Conclusion: VATS for CLMs is associated with better postoperative recovery and fewer complications. Biportal VATS is also a safe and feasible approach.

Abstract:Background. Mediastinal bronchogenic cysts are rarely diagnosed in adults, hence surgical experience is limited particularly with regard to video-assisted thoracoscopic surgery. In support of the thoracoscopic approach we report our single-center experience in this rare entity. Methods. Between June 1995 and December 2002, a nonselected series of 12 consecutive patients presenting with mediastinal bronchogenic cysts underwent video-assisted thoracoscopic surgery. Six cysts (50%) had been diagnosed 2 to 22 years prior, only three of which became symptomatic. In asymptomatic patients (n = 7) surgery was performed because of increasing cyst size (n = 3), patient's request (n = 3), or suspected metastasis (n = 1). Results. Mediastinal bronchogenic cysts were correctly diagnosed by computed tomography in 83% (10/12) and by magnetic resonance imaging in 100% (9/9). Using a three-trocar technique thoracoscopic surgery was successfully performed in 11 of 12 cases (92%). We noted no signs of acute cyst infection. No serious postoperative complications were observed. In 1 patient conversion to open thoracotomy was necessary due to extensive pleural adhesions. In another case thoracoscopic excision of the cyst wall was incomplete. Patients with thoracoscopic excision were discharged after a median of 5.5 days (range 4 to 14 days). No recurrences or complications were observed during a mean follow-up of 40.5 months. Conclusions. Considering the low conversion and complication rate in our series, video-assisted thoracoscopic surgery should be the primary therapeutic choice among adults with symptomatic mediastinal bronchogenic cysts. Surgical intervention in patients with asymptomatic and uncomplicated cysts appears optional. (C) 2004 by The Society of Thoracic Surgeons.

Abstract:Objective:To summarize the clinical characters and modalities of surgical treatment in children huge thoracic tumors Method:The clinic and pathologic material as well as the of the surgic treating process of the 12 children with huge thoracic tumors were retrospectively reviewes.The therapeutic efficacy and follow up were analysesd.Results:Among four anterior mediastinum tumors,two of them was spindle cell carcinoma.the other two originated from thoracic gland,calld malignant schwannoma and benign teratoma.Four tumors had been complete removed.The radical resection operations also had been completed in 5 cases of posterior mediastinum tumors children.Pathological reports shew 4 of them were neuroblastoma,the other 1 was paraganglioma.In 2 children concerned as suffering lung tumor before operations,1 right pneumonoresection and 1 right middle lower pulmonary lobectomy had been taken.All the pathologically revealed pneumoblastoma.The pathologically of the huge thoracic tumor occupying more than half of the whole thoracic cavity was mesenchymal chondrosarcoma.Conclusion:The therapeutic results of children with huge thoracic tumors are determined by the control of operative indications and surgical approaches.Postoperative management takes part in important role either.
