Abstract:<p>The United Nations' Sustainable Development Goal 3.2 aims to reduce under‐5 child mortality to 25 deaths per 1,000 live births by 2030. Child mortality tends to be concentrated in developing regions where information needed to assess achievement of this goal often come from surveys and censuses. In both, women are asked about their birth histories, but with varying degrees of detail. Full birth history (FBH) data contain the reported dates of births and deaths of every surveyed mother's children. In contrast, summary birth history (SBH) data contain only the total number of children born and total number of children who died for each mother. Specialized methods are needed to accommodate this type of data into analyses of child mortality trends. We develop a data augmentation scheme within a Bayesian framework where for SBH data, birth and death dates are introduced as auxiliary variables. Since we specify a full probability model for the data, many of the well‐known biases that exist in this data can be accommodated, along with space‐time smoothing on the underlying mortality rates. We illustrate our approach in a simulation, showing robustness to model misspecification and that uncertainty is reduced when incorporating SBH data over simply analyzing all available FBH data. We also apply our approach to data from the Central region of Malawi and compare with the well‐known Brass method.</p>

A Pseudo-likelihood Approach to Under-5 Mortality Estimation

Abstract:Accurate and precise estimates of the under-5 mortality rate (U5MR) are an important health summary for countries. However, full survival curves allow us to better understand the pattern of mortality in children under five. Modern demographic methods for estimating a full mortality schedule for children have been developed for countries with good vital registration and reliable census data, but perform poorly in many low- and middle-income countries (LMICs). In these countries, the need to utilize nationally representative surveys to estimate the U5MR requires additional care to mitigate potential biases in survey data, acknowledge the survey design, and handle the usual characteristics of survival data, for example, censoring and truncation. In this paper, we develop parametric and non-parametric pseudo-likelihood approaches to estimating child mortality across calendar time from complex survey data. We show that the parametric approach is particularly useful in scenarios where data are sparse and parsimonious models allow efficient estimation. We compare a variety of parametric models to two existing methods for obtaining a full survival curve for children under the age of 5, and argue that a parametric pseudo-likelihood approach is advantageous in LMICs. We apply our proposed approaches to survey data from four LMICs.

Abstract:Accurate estimates of the under-5 mortality rate (U5MR) in a developing world context are a key barometer of the health of a nation. This paper describes new models to analyze survey data on mortality in this context. We are interested in both spatial and temporal description, that is, wishing to estimate U5MR across regions and years, and to investigate the association between the U5MR and spatially-varying covariate surfaces. We illustrate the methodology by producing yearly estimates for subnational areas in Kenya over the period 1980 - 2014 using data from demographic health surveys (DHS). We use a binomial likelihood with fixed effects for the urban/rural stratification to account for the complex survey design. We carry out smoothing using Bayesian hierarchical models with continuous spatial and temporally discrete components. A key component of the model is an offset to adjust for bias due to the effects of HIV epidemics. Substantively, there has been a sharp decline in U5MR in the period 1980 - 2014, but large variability in estimated subnational rates remains. A priority for future research is understanding this variability. Temperature, precipitation and a measure of malaria infection prevalence were candidates for inclusion in the covariate model.

Abstract:Understanding the underlying causes of maternal death across all regions of the world is essential to inform policies and resource allocation to reduce the mortality burden. However, in many countries there exists very little data on the causes of maternal death, and data that do exist do not capture the entire population at risk. In this article, we present a Bayesian hierarchical multinomial model to estimate maternal cause of death distributions globally, regionally, and for all countries worldwide. The framework combines data from various sources to inform estimates, including data from civil registration and vital systems, smaller‐scale surveys and studies, and high‐quality data from confidential enquiries and surveillance systems. The framework accounts for varying data quality and coverage, and allows for situations where one or more causes of death are missing. We illustrate the results of the model on three case‐study countries that have different data availability situations.

Abstract:The maternal mortality ratio (MMR) is defined as the number of maternal deaths in a population per 100,000 live births. Country-specific MMR estimates are published on a regular basis by the United Nations Maternal Mortality Estimation Inter-agency Group (UN MMEIG) to track progress in reducing maternal deaths and to evaluate regional and national performance related to Millennium Development Goal (MDG) 5, which calls for a 75% reduction in the MMR between 1990 and 2015. Until 2014, the UN MMEIG used a multilevel regression model for producing estimates for countries without sufficient data from vital registration systems. While this model worked well in the past to assess MMR levels for countries with limited data, it was deemed unsatisfactory for final MDG 5 reporting for countries where longer time series of observations had become available because by construction, estimated trends in the MMR were covariate-driven only and did not necessarily track data-driven trends. We developed a Bayesian maternal mortality estimation model, which extends upon the UN MMEIG multilevel regression model. The new model assesses data-driven trends through the inclusion of an ARIMA time series model that captures accelerations and decelerations in the rate of change in the MMR. Varying reporting and data quality issues are accounted for in source-specific data models. The revised model provides data-driven estimates of MMR levels and trends and will be used for MDG 5 reporting for all countries.

Abstract:Understanding patterns in mortality across subpopulations is essential for local health policy decision making. One of the key challenges of subnational mortality rate estimation is the presence of small populations and zero or near zero death counts. When studying differences between subpopulations, this challenge is compounded as the small populations are further divided along socioeconomic or demographic lines. In this paper, we build on principal component-based Bayesian hierarchical approaches for subnational mortality rate estimation to model correlations across subpopulations. The principal components identify structural differences between subpopulations, and coefficient and error models track the correlations between subpopulations over time. We illustrate the use of the model in a simulation study as well as on county-level sex-specific US mortality data. We find that results from the model are reasonable and that it successfully extracts meaningful patterns in US sex-specific mortality. Additionally, we show that ancillary correlation parameters are a useful tool for studying the convergence and divergence of mortality patterns over time.

Abstract:Many people living in low- and middle-income countries are not covered by civil registration and vital statistics systems. Consequently, a wide variety of other types of data, including many household sample surveys, are used to estimate health and population indicators. In this paper we combine data from sample surveys and demographic surveillance systems to produce small area estimates of child mortality through time. Small area estimates are necessary to understand geographical heterogeneity in health indicators when full-coverage vital statistics are not available. For this endeavor spatio-temporal smoothing is beneficial to alleviate problems of data sparsity. The use of conventional hierarchical models requires careful thought since the survey weights may need to be considered to alleviate bias due to nonrandom sampling and nonresponse. The application that motivated this work is an estimation of child mortality rates in five-year time intervals in regions of Tanzania. Data come from Demographic and Health Surveys conducted over the period 1991-2010 and two demographic surveillance system sites. We derive a variance estimator of under five years child mortality that accounts for the complex survey weighting. For our application, the hierarchical models we consider include random effects for area, time and survey and we compare models using a variety of measures including the conditional predictive ordinate (CPO). The method we propose is implemented via the fast and accurate integrated nested Laplace approximation (INLA).

Abstract:Abstract Reducing the global burden of stillbirths is important to improving child and maternal health. Of interest is understanding patterns in the timing of stillbirths—that is, whether they occur before the onset of labour (antepartum) or during labour (intrapartum)—because stillbirths that occur intrapartum are largely preventable. However, data availability on the timing of stillbirths is highly variable across the world, with low- and middle-income countries generally having few reliable observations. In this paper, we develop a Bayesian penalized splines regression framework to estimate the proportion of stillbirths that are intrapartum for all countries worldwide. The model accounts for known relationships with neonatal mortality, pools information across geographic regions, incorporates different errors based on data attributes, and allows for data-driven temporal trends. A weighting procedure is proposed to account for unrepresentative subnational data. Results suggest that the intrapartum proportion is generally decreasing over time, but progress is slower in some regions, particularly Sub-Saharan Africa.

A prediction model for neonatal mortality in low- and middle-income countries: an analysis of data from population surveillance sites in India, Nepal and Bangladesh

Abstract:Background: In poor settings, where many births and neonatal deaths occur at home, prediction models of neonatal mortality in the general population can aid public-health policy-making. No such models are available in the international literature. We developed and validated a prediction model for neonatal mortality in the general population in India, Nepal and Bangladesh. Methods: Using data (49 632 live births, 1742 neonatal deaths) from rural and urban surveillance sites in South Asia, we developed regression models to predict the risk of neonatal death with characteristics known at (i) the start of pregnancy, (ii) start of delivery and (iii) 5 minutes post partum. We assessed the models' discriminative ability by the area under the receiver operating characteristic curve (AUC), using cross-validation between sites. Results: At the start of pregnancy, predictive ability was moderate {AUC 0.59 [95% confidence interval (CI) 0.58-0.61]} and predictors of neonatal death were low maternal education and economic status, short birth interval, primigravida, and young and advanced maternal age. At the start of delivery, predictive ability was considerably better [AUC 0.73 (95% CI 0.70-0.76)] and prematurity and multiple pregnancy were strong predictors of death. At 5 minutes post partum, predictive ability was good [AUC: 0.85 (95% CI 0.80-0.89)]; very strong predictors were multiple birth, prematurity and a poor condition of the infant at 5 minutes. Conclusions: We developed good performing prediction models for neonatal mortality. Neonatal deaths are highly concentrated in a small group of high-risk infants, even in poor settings in South Asia. Risk assessment, as supported by our models, can be used as a basis for improving community- and facility-based newborn care and prevention strategies in poor settings.

Abstract:In order to implement disease-specific interventions in young age groups, policy makers in low- and middle-income countries require timely and accurate estimates of age- and cause-specific child mortality. High-quality data is not available in settings where these interventions are most needed, but there is a push to create sample registration systems that collect detailed mortality information. current methods that estimate mortality from this data employ multistage frameworks without rigorous statistical justification that separately estimate all-cause and cause-specific mortality and are not sufficiently adaptable to capture important features of the data. We propose a flexible Bayesian modeling framework to estimate age- and cause-specific child mortality from sample registration data. We provide a theoretical justification for the framework, explore its properties via simulation, and use it to estimate mortality trends using data from the Maternal and Child Health Surveillance System in China.

Abstract:Small area estimates of population are necessary for many epidemiological studies, yet their quality and accuracy are often not assessed. In the United States, small area estimates of population counts are published by the United States Census Bureau (USCB) in the form of the Decennial census counts, Intercensal population projections (PEP), and American Community Survey (ACS) estimates. Although there are significant relationships between these data sources, there are important contrasts in data collection and processing methodologies, such that each set of estimates may be subject to different sources and magnitudes of error. Additionally, these data sources do not report identical small area population counts due to post-survey adjustments specific to each data source. Resulting small area disease/mortality rates may differ depending on which data source is used for population counts (denominator data). To accurately capture annual small area population counts, and associated uncertainties, we present a Bayesian population model (B-Pop), which fuses information from all three USCB sources, accounting for data source specific methodologies and associated errors. The main features of our framework are: 1) a single model integrating multiple data sources, 2) accounting for data source specific data generating mechanisms, and specifically accounting for data source specific errors, and 3) prediction of estimates for years without USCB reported data. We focus our study on the 159 counties of Georgia, and produce estimates for years 2005-2021.

Abstract:Civil registration vital statistics (CRVS) data are used to produce national estimates of maternal mortality, but are often subject to substantial reporting errors due to misclassification of maternal deaths. The accuracy of CRVS systems can be assessed by comparing CRVS-based counts of maternal and non-maternal deaths to those obtained from specialized studies, which are rigorous assessments of maternal mortality for a given country-period. We developed a Bayesian bivariate random walk model to estimate sensitivity and specificity of the reporting on maternal mortality in CRVS data, and associated CRVS adjustment factors. The model was fitted to a global data set of CRVS and specialized study data. Validation exercises suggest that the model performs well in terms of predicting CRVS-based proportions of maternal deaths for country-periods without specialized studies. The new model is used by the UN Maternal Mortality Inter-Agency Group to account for misclassification errors when estimating maternal mortality using CRVS data.

Abstract:Mortality is different across countries, states and regions. Several empirical research works however reveal that mortality trends exhibit a common pattern and show similar structures across populations. The key element in analyzing mortality rate is a time-varying indicator curve. Our main interest lies in validating the existence of the common trends among these curves, the similar gender differences and their variability in location among the curves at the national level. Motivated by the empirical findings, we make the study of estimating and forecasting mortality rates based on a semi-parametric approach, which is applied to multiple curves with the shape-related nonlinear variation. This approach allows us to capture the common features contained in the curve functions and meanwhile provides the possibility to characterize the nonlinear variation via a few deviation parameters. These parameters carry an instructive summary of the time-varying curve functions and can be further used to make a suggestive forecast analysis for countries with barren data sets. In this research the model is illustrated with mortality rates of Japan and China, and extended to incorporate more countries.

Abstract:Civil registration and vital statistics (CRVS) systems should be the primary source of mortality data for governments. Accurate and timely measurement of the completeness of death registration helps inform interventions to improve CRVS systems and to generate reliable mortality indicators. In this work we propose the use of hierarchical Bayesian linear mixed models with Global-Local (GL) priors to estimate the completeness of death registration at global, national and subnational levels. The use of GL priors in this paper is motivated for situations where demographic covariates can explain much of the observed completeness but where unexplained within-country (i.e. by year) and between-country variability also play an important role. The use of our approach can allow institutions improve model parameter estimates and more accurately predict completeness of death registration. Our models are based on a dataset which uses Global Burden of Disease (GBD) death estimates based on the GBD 2019 and comprises 120 countries and 2,748 country-years from 1970-2019 (Demographics Collaborators, 2020). To illustrate the effectiveness of our proposal we consider the completeness of death registration in the departments of Colombia in 2017.

Abstract:Early identification of high-risk pregnancies can reduce global neonatal mortality rate. Using the most recent Demographic and Health Surveys from 56 low- and middle-income countries, we examined the proportion of mothers with history of neonatal deaths. Logistic regression models were used to assess the association between maternal history of neonatal death and subsequent neonatal mortality. The adjusted models controlled for socioeconomic, child, and pregnancy-related factors. Country-specific analyses were performed to assess heterogeneity in this association across countries. Among the 437,049 live births included in the study, 6910 resulted in neonatal deaths. In general, 22.4% (1549) occurred to mothers with previous history of neonatal death; at the country-level, this proportion ranged from 1.2% (95% confidence interval [CI] 0.0, 2.6) in Dominican Republic to 38.1% (95% CI 26.0, 50.1) in Niger. Maternal history of neonatal death was significantly associated with subsequent neonatal death in both the pooled and the subgroup analyses. In the fully adjusted model, history of neonatal death was associated with 2.1 (95% CI 1.9, 2.4) times higher odds of subsequent neonatal mortality in the pooled analysis. We observed large variation in the associations across countries ranging from fully adjusted odds ratio (FAOR) of 0.4 (95% CI 0.0, 4.0) in Dominican Republic to 16.1 (95% CI 3.6, 42.0) in South Africa. Our study suggests that maternal history of neonatal death could be an effective early identifier of high-risk pregnancies in resource-poor countries. However, country-specific contexts must be considered in national policy discussions.

Abstract:The average age of infant deaths, a10, and the average number of years lived-in the age interval-by those dying between ages 1 and 5, a41, are important quantities allowing the construction of any life table including these ages. In many applications, the direct calculation of these parameters is not possible, so they are estimated using the infant mortality rate-or the death rate from 0 to 1-as a predictor. Existing methods are general approximations that do not consider the full variability in the age patterns of mortality below the age of 5. However, at the same level of mortality, under-five deaths can be more or less concentrated during the first weeks and months of life, thus resulting in very different values of a10 and a41. This article proposes an indirect estimation of these parameters by using a recently developed model of under-five mortality and taking advantage of a new, comprehensive database by detailed age-which is used for validation. The model adapts to a variety of inputs (e.g., rates, probabilities, or the proportion of deaths by sex or for both sexes combined), providing more flexibility for the users and increasing the precision of the estimates. This fresh perspective consolidates a new method that outperforms all previous approaches.
Probabilistic mortality forecasting with varying age-specific survival improvements

Abstract:Many mortality forecasting approaches extrapolate past trends. Their predictions of the future development can be quite precise as long as turning points and/or age-shifts of mortality decline are not present. To account even for such mortality dynamics, we propose a model that combines recently developed ideas in a single framework. It (1) uses rates of mortality improvement to model the aging of mortality decline, and it (2) optionally combines the mortality trends of multiple countries to catch anticipated turning points. We use simulation-based Bayesian inference to estimate and run this model that also provides prediction intervals to quantify forecast uncertainty. Validating mortality forecasts for British and Danish women from 1991 to 2011 suggest that our model can forecast regular and irregular mortality developments and that it can perform at least as well as other widely accepted approaches like, for instance, the Lee-Carter model or the UN Bayesian approach. Moreover, prospective mortality forecasts from 2012 to 2050 suggest gradual increases for British and Danish life expectancy at birth.

Abstract:The under-5 mortality rate (U5MR), a critical health indicator, is typically estimated from household surveys in lower and middle income countries. Spatio-temporal disaggregation of household survey data can lead to highly variable estimates of U5MR, necessitating the usage of smoothing models which borrow information across space and time. The assumptions of common smoothing models may be unrealistic when certain time periods or regions are expected to have shocks in mortality relative to their neighbors, which can lead to oversmoothing of U5MR estimates. In this paper, we develop a spatial and temporal smoothing approach based on Gaussian Markov random field models which incorporate knowledge of these expected shocks in mortality. We demonstrate the potential for these models to improve upon alternatives not incorporating knowledge of expected shocks in a simulation study. We apply these models to estimate U5MR in Rwanda at the national level from 1985-2019, a time period which includes the Rwandan civil war and genocide.
