Abstract:The glymphatic hypothesis proposes an intriguing mechanism of amyloid-beta clearance. The hypothesis is successful on animal models. However, few studies have been performed in a clinical setting, because of the lack of a clinically feasible and safe technique to quantify the CSF circulation. The majority of the CSF is secreted by the choroid plexus (CP) and the production rate of CSF is closely associated with the blood supply to the CP. Therefore, we sought to measure the blood flow of the CP using arterial spin labeling (ASL) MRI and examine its potential for detecting alterations in CSF generation. Seven healthy volunteers were imaged on a 3T GE MR scanner. ASL images were acquired with 12 labeling-durations (LD) and post-label-delays (PLD) (table 1). Regions of the CP (ROI) were manually defined on 3D high-resolution T1w images (Fig1) and transferred to the ASL image space using the same parameters that coregistered the ASL images to the gray matter (GM) of the T1w images. The mean of the 20% most-highly perfused pixels in the ROIs were used to quantify the blood flow with dynamic ASL models (Fig2). The perfusion of the GM region was quantified as a reference. Because of the fenestrated capillaries of the CP, its blood flow (134.3±32.5ml/100g/min) is almost double the blood flow of GM (69.5±15.4ml/100g/min, p<0.02). Furthermore, the arterial transit time of the CP (1.34±0.09s) is also significant shorter than that of the GM (1.13±0.23s, p<0.05). More interestingly, the apparent T1 of the CP (2.60±0.44s) is significantly longer than that of the GM (1.80±0.16s, p<0.02). Since CSF is the only known fluid with a T1 (4.2s) longer than 2.5s, this finding may indicate that the water of blood (T1=1.8-1.9s) has been secreted into CSF. Our findings suggest that ASL can provide a clinically feasible option to image the blood flow of the CP. The high blood flow and short blood arrival time reflect the unique vascular structure of the CP. The long T1 of the CP may suggest the transport of water from arterial blood to the CSF, which provides a potential method to quantify CSF generation.
