Abstract:Cholesterol gallstone (CG) disease is a worldwide common disease characterized by cholesterol supersaturation in gallbladder bile. Ganoderma lucidum polysaccharide (GLP) has been shown to possess various beneficial effects against metabolic disorders. However, the role and underlying mechanism of GLP in CG formation are still unknown. This study aimed to determine the role of GLP in ameliorating lithogenic diet (LD)-induced CG formation.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to explore the role of Ganoderma lucidum polysaccharide (GLP) in improving the formation of cholesterol gallstones (CG) caused by bile acid metabolism disorders and its underlying mechanisms. Specifically, the study aims to determine whether GLP can reduce the formation of cholesterol gallstones in mice induced by a lithogenic diet (LD) by regulating cholesterol and bile acid metabolism, and to explore whether this process depends on the regulation of the farnesoid X receptor (FXR) signaling pathway. ### Research Background - **Cholesterol Gallstone**: It is a common disease. Approximately 90% of gallstone patients worldwide have cholesterol gallstones. Cholecystectomy is the main treatment for gallstones, but it may lead to complications and affect the patient's quality of life. - **Bile Acid Metabolism**: Bile acids are important products of cholesterol metabolism, and excess cholesterol can be eliminated through bile acid metabolism. The imbalance of bile acid metabolism is closely related to the formation of cholesterol gallstones. - **Farnesoid X Receptor (FXR)**: It is a nuclear receptor, mainly expressed in the intestine and liver, and plays a crucial role in regulating cholesterol and bile acid metabolism. The activation of FXR can inhibit the expression of bile acid - synthesizing enzymes and reduce the synthesis of bile acids. ### Research Objectives - **Explore the Role of GLP**: Study the improvement effect of GLP on the formation of cholesterol gallstones in a mouse model induced by a lithogenic diet. - **Reveal the Underlying Mechanisms**: Explore whether GLP affects cholesterol and bile acid metabolism by regulating the FXR signaling pathway, thereby reducing the formation of cholesterol gallstones. ### Main Findings - **GLP Reduces Gallstone Formation**: GLP significantly reduces the formation rate of gallstones in mice induced by a lithogenic diet and decreases the cholesterol saturation index (CSI) in bile. - **Regulate Bile Acid Metabolism**: GLP increases the total bile acid level in bile, while reducing the levels of FXR - agonist bile acids (such as cholic acid and deoxycholic acid) in the intestine and increasing the level of FXR - antagonist bile acid (such as β - muricholic acid). - **Inhibit FXR Signaling Pathway**: GLP inhibits the negative feedback regulation of bile acid synthesis by down - regulating the FXR - FGF15 signal in the intestine and the FXR - SHP signal in the liver, promotes the synthesis of bile acids, and reduces the synthesis and secretion of cholesterol. - **Improve Intestinal Microbiota**: GLP also improves the imbalance of the intestinal flora, reduces the level of hydrogen sulfide (H2S), a bacterial metabolite in serum, and further inhibits the FXR signaling pathway in the liver. ### Conclusion In summary, GLP effectively reduces the formation of cholesterol gallstones by regulating cholesterol and bile acid metabolism, especially in an FXR - dependent manner. These findings provide a scientific basis for using GLP as a potential strategy for preventing cholesterol gallstone disease.
