Abstract:Introduction Sepsis, a condition of global public health concern, is a major cause of morbidity and mortality, especially in patients with underlying HIV infection. This study aims to determine outcomes, aetiology and antibiotic resistance patterns among children with HIV exposure or infection admitted with a clinical presentation suggestive of sepsis who have confirmed bloodstream infections at Arthur Davison Children's Hospital (ADCH) in Ndola, Zambia. Methods and analysis This will be a prospective longitudinal study of 200 children aged <2 years admitted with sepsis at ADCH with two of the following conditions: temperature of 38.0°C, respiratory rate ≥20 breaths per minute and pulse rate ≥90 beats per minute. About 2–5 mL of blood collected from each participant will be inoculated into BACTEC culture bottles and incubated for 5–7 days. Positive cultures will be inoculated onto culture media for subculture followed by species identification followed by antibiotic susceptibility testing. Time-to-event outcomes such as hospital readmission and mortality will be analysed using Kaplan-Meier and Cox proportional hazards. Predictors will be identified using regression methods. All statistical tests will use a 5% significance level with a 95% confidence level. STATA V.16 will be used for statistical analysis. Ethics and dissemination Ethical clearance and approval have been granted by the Tropical Diseases Research Centre Ethics Committee (TDRC-EC 092/07/23). Caregiver consent will be obtained verbally for participants presenting as medical emergencies, and written informed consent will be obtained once stable. Findings from this study will be shared with the Ministry of Health Zambia and will be disseminated to the scientific community through peer-reviewed scientific journals.

Clinical Features of Bloodstream Infection in Children with Haematological Malignancies

Abstract:Purpose: Sepsis and bacteraemia are both referred to as bloodstream infection (BSI). It contributed majorly to the mortality of the children with blood cancer. In order to reveal the clinical features of BSI and improve its outcome, we performed the investigation among the inpatients in haematology/oncology department. Methods: Neutropenia was defined as an absolute neutrophil count (ANC) in peripheral blood of <0.5×10 9 /L. Microbiologically documented infection (MDI) was defined when causative pathogen was isolated from blood. Drug susceptibility test was performed using the VITEK-60 Auto Microbic System and Kirby-Baue disk diffusion method. Relative cytokines in the sera were detected through flow cytometry (FCM). All of the data were analysed with STATA software (version 9.0). Results: The incidence of BSI was 10.73% (161/1500), the rate of MDI for BSI was 43.48% (70/161). There was no statistical significance between non-MDI and MDI groups on the clinical characteristics. Total 79 strains were detected from 70 blood samples. Gram-positive bacteria, gram-negative bacteria and fungi accounted for 55.70% (44/79), 43.04% (34/79) and 1.27% (1/79), respectively. The most common pathogens were Staphylococcus epidermidis (20.25%), Escherichia coli (15.19%) and Klebsiella pneumoniae (15.19%). Staphylococcus showed no/low resistance to linezolid, vancomycin and furantoin. Gram-negative bacteria showed no/low resistance to cefoxitin, imipenem, piperacillin/tazobactam and amikacin. The mortality of BSI was 3.1% (5/161). Conclusions: BSI had no specific signs and symptoms clinically, and gram- positive bacteria were the dominant pathogens. Antibiotics selection according to the susceptibility test was superior to the empirical treatment. Improved prevention, early detection, and advanced therapeutic strategies might decrease the incidence of the BSI and improve its treatment outcome.
Factors influencing predisposition to sepsis in children with cancers and acquired immunodeficiencies unrelated to human immunodeficiency virus infection

Abstract:Objectives: The main objectives of this review are to provide insight into the various factors that affect the risk of sepsis in immunocompromised children and to discuss special issues that should be considered when such patients are enrolled in clinical trials. Strategy: A literature review was conducted, and authoritative references were consulted when appropriate. This was supported by discussion among experts at an international consensus conference on pediatric sepsis. Outcome: The review discusses general issues as they relate to the factors that are associated with a predisposition to sepsis in children with cancers and non-human immunodeficiency virus (HIV)-related acquired immunodeficiencies. The host defects that are associated with specific infections are discussed, and an overview of the indicators of immune dysfunction in the previously well child is presented. Selected examples of patients with non-HIV-related acquired immunodeficiencies, including those with cancer or who have undergone solid-organ or hematopoietic stem-cell transplants are discussed. Special challenges that may affect clinical trials include the altered immune response as this relates to the definition of infection and disease and the assessment of outcomes and the heterogeneity of study populations due to the variable manifestations of immune deficiency states. Summary: Knowledge of the factors that are associated with sepsis in immunocompromised patients is important when such patients are to be entered into clinical trials on sepsis. These factors do not necessarily operate in isolation and may occur concurrently or sequentially. With these considerations in mind, clinical trials involving immunocompromised children can go forward and will very likely lead to significant advances in the care of this understudied population.

Abstract:Abstract Background Secondary hemophagocytic syndrome (SHS) may develop as a result of excessive immune activation, with proliferation of T cells and macrophages. It is often fatal and remains ill-recognized in children, leading to false or delayed diagnosis and suboptimal management. This condition has been described more often in association with virus, but it can be secondary to bacteria, fungi, and parasites. Methods Retrospective, descriptive, and observational study. We systematically reviewed the clinical files of patients diagnosed with SHS at the Instituto Nacional de Pediatría between January 1995 and December 2018; age, gender, clinical features, laboratory results, management, and final outcome were registered. Results 49 cases of SHS were diagnosed, 26 (53%) were male, mean age at diagnosis was 67.5 months (newborn–15.5 years), 47 (96%) presented with fever, 46 (94%) hepatomegaly, 37 (76%) splenomegaly, 28 (57%) lymphadenopathy, 18 (37%) jaundice, and 4 (8%) bleeding. The most common laboratory findings were anemia in 29 (59%) patients, thrombocytopenia in 39 (80%), hypertriglyceridemia in 39 (80%), with isolation of Epstein Barr Virus in 18 (35%), followed by Cytomegalovirus in 2 (4%), Escherichia coli in 2 (4%), Salmonella enterica in 2 (4%), and Leptospira, Brucella abortus, Shigella sonnei, Enterococcus faecium, Enterovirus and congenital Rubella in 1 case each (2%). Intravenous Gamma-globulin therapy was administered to 38 (78%) patients, antimicrobials to 26 (53%), systemic steroids to 46 (94%) and VP16 to 32 (65%). 22 (45%) had full recovery, 6 (12%) patients presented with a recurrent episode of SHS, and 21 (43%) died. Conclusion Due to the rarity of this syndrome, variable clinical presentation, and lack of specificity of the clinical and laboratory findings, the diagnosis of SHS is often delayed. EBV remains an important etiologic agent of this syndrome, identified in 35% of our cases. The mortality rate around the world is reported approximately around 45%, confirmed in our study in 43% of the cases, remarking the importance of prompt diagnosis and treatment to increase the survival in our population. Abstract:Studies have shown increased invasive Group A Streptococcus (GAS) disease, including bloodstream infections (GAS-BSI). However, the epidemiological data of GAS-BSI are limited in children. We aimed to describe GAS-BSI in children in Madrid, over 13 years (2005-2017). Multicenter retrospective cohort study from 16 hospitals from Madrid, Spain. Epidemiology, symptomatology, laboratory, treatment, and outcome of GAS-BSI in children ≤ 16 years were analyzed. 109 cases of GAS-BSI were included, with incidence rate of 4.3 episodes/100,000 children attended at the emergency department/year. We compared incidence between two periods (P1: 2005-June 2011 vs P2: July 2011-2017) and observed a non-significant increase along the study period (annual percentage change: + 6.0% [95%CI: -2.7, + 15.4]; p = 0.163). Median age was 24.1 months (IQR: 14.0-53.7), peaking during the first four years of life (89/109 cases; 81.6%). Primary BSI (46.8%), skin and soft tissue (21.1%), and osteoarticular infections (18.3%) were the most common syndromes. We compared children with primary BSI with those with a known source and observed that the former had shorter hospital stay (7 vs. 13 days; p = 0.003) and received intravenous antibiotics less frequently (72.5% vs. 94.8%; p = 0.001) and for shorter duration of total antibiotic therapy (10 vs. 21 days; p = 0.001). 22% of cases required PICU admission. Factors associated with severity were respiratory distress, pneumonia, thrombocytopenia, and surgery, but in multivariate analysis, only respiratory distress remained significant (adjusted OR:9.23 [95%CI: 2.16-29.41]). Two children (1.8%) died. Conclusion: We observed an increasing, although non-significant, trend of GAS-BSI incidence within the study. Younger children were more frequently involved, and primary BSI was the most common and less severe syndrome. PICU admission was frequent, being respiratory distress the main risk factor. What is known: • In recent decades, several reports have shown a worldwide increase in the incidence of invasive Group A streptococcal disease (GAS), including bloodstream infection (BSI). Recently, there have been a few reports showing an increase in severity as well. • There needs to be more information on the epidemiology in children since most studies predominantly include adults. What is new: • This study, carried out in children with GAS-BSI in Madrid, shows that GAS-BSI affects mostly younger children, with a broad spectrum of manifestations, needing PICU admission frequently. Respiratory distress was the leading risk factor for severity, whereas primary BSI seemed to be less severe. • We observed an increasing, although non-significant, trend of GAS-BSI incidence in recent years (2005-2017).
Pneumocystis carinii Pneumonia in Human Immunodeficiency Virus‐Infected Infants and Children

Abstract:Pneumocystis carinii pneumonia (PCP) is the most common opportunistic infection in adults and children infected with the human immunodeficiency virus (HIV). Without prophylaxis, half of all these children will develop PCP at sometime during their illness. The disease is associated with high mortality and a poor prognosis for long‐term survival in this patient population. In infants and young children, PCP may be a primary infection, compared with reactivation of a latent infection that is usually the case in older children and adults. Clinical features, radiographic findings and diagnostic strategies are similar in children and adults. Although alternative agents are being investigated, trimethoprim‐sulfamethoxazole (TMP‐SMX) and pentamidine remain the standard therapeutic agents. Insufficient data are available to recommend routine adjunctive corticosteroids in children with acquired immunodeficiency syndrome (AIDS), PCP, and significant respiratory disease. Prophylaxis against PCP occurrence or recurrence is indicated for HIV‐infected children and infants under 1 year of age, children with less than 20% T4 helper lymphocytes, those meeting age‐related Centers for Disease Control (CDC) guidelines for prophylaxis, and those with a history of suspected or documented PCP. The CDC recommends intermittent TMP‐SMX for PCP prophylaxis in children with AIDS.

Mortality and morbidity in community-acquired sepsis in European pediatric intensive care units: a prospective cohort study from the European Childhood Life-threatening Infectious Disease Study (EUCLIDS)

Abstract:Background: Sepsis is one of the main reasons for non-elective admission to pediatric intensive care units (PICUs), but little is known about determinants influencing outcome. We characterized children admitted with community-acquired sepsis to European PICUs and studied risk factors for mortality and disability. Methods: Data were collected within the collaborative Seventh Framework Programme (FP7)-funded EUCLIDS study, which is a prospective multicenter cohort study aiming to evaluate genetic determinants of susceptibility and/or severity in sepsis. This report includes 795 children admitted with community-acquired sepsis to 52 PICUs from seven European countries between July 2012 and January 2016. The primary outcome measure was in-hospital death. Secondary outcome measures were PICU-free days censured at day 28, hospital length of stay, and disability. Independent predictors were identified by multivariate regression analysis. Results: Patients most commonly presented clinically with sepsis without a source (n = 278, 35%), meningitis/encephalitis (n = 182, 23%), or pneumonia (n = 149, 19%). Of 428 (54%) patients with confirmed bacterial infection, Neisseria meningitidis (n = 131, 31%) and Streptococcus pneumoniae (n = 78, 18%) were the main pathogens. Mortality was 6% (51/795), increasing to 10% in the presence of septic shock (45/466). Of the survivors, 31% were discharged with disability, including 24% of previously healthy children who survived with disability. Mortality and disability were independently associated with S. pneumoniae infections (mortality OR 4.1, 95% CI 1.1-16.0, P = 0.04; disability OR 5.4, 95% CI 1.8-15.8, P < 0.01) and illness severity as measured by Pediatric Index of Mortality (PIM2) score (mortality OR 2.8, 95% CI 1.3-6.1, P < 0.01; disability OR 3.4, 95% CI 1.8-6.4, P < 0.001). Conclusions: Despite widespread immunization campaigns, invasive bacterial disease remains responsible for substantial morbidity and mortality in critically ill children in high-income countries. Almost one third of sepsis survivors admitted to the PICU were discharged with some disability. More research is required to delineate the long-term outcome of pediatric sepsis and to identify interventional targets. Our findings emphasize the importance of improved early sepsis-recognition programs to address the high burden of disease.

Abstract:Background: Children with sickle cell disease (SCD) are at a high risk of invasive bacterial infections (IBI). Universal penicillin prophylaxis and vaccination, especially against Streptococcus pneumoniae, have deeply changed its epidemiology. Analysis of IBI in children with SCD in a post-13-valent pneumococcal vaccine era is limited. Methods: Twenty-eight pediatric hospitals from 5 European countries retrospectively collected IBI episodes in SCD children aged 1 month to 18 years between 2014 and 2019. IBI was defined as a positive bacterial culture or polymerase chain reaction from a normally sterile fluid: blood, cerebrospinal, joint, or pleural fluid and deep surgical specimen. Results: We recorded 169 IBI episodes. Salmonella spp. was the main isolated bacteria (n = 44, 26%), followed by Streptococcus pneumonia (Sp; n = 31, 18%) and Staphylococcus aureus (n = 20, 12%). Salmonella prevailed in osteoarticular infections and in primary bacteremia (45% and 23% of episodes, respectively) and Sp in meningitis and acute chest syndrome (88% and 50%, respectively). All Sp IBI occurred in children ≤10 years old, including 35% in children 5 to 10 years old. Twenty-seven (17%) children had complications of infection and 3 died: 2 because of Sp, and 1 because of Salmonella. The main risk factors for a severe IBI were a previous IBI and pneumococcal infection (17 Sp/51 cases). Conclusions: In a post-13-valent pneumococcal vaccine era, Salmonella was the leading cause of bacteremia in IBI in children with SCD in Europe. Sp came second, was isolated in children ≤10 years old, and was more likely to cause severe and fatal cases.

Abstract:This study was conducted to investigate the influence of fitness level and gradually increased amounts of exercise on individuals' ratings of perceived exertion (RPE). 20 men served as paid subjects. They were divided into groups of high (>56 ml/kg/min.) and low fitness (<46 ml/kg/min.) according to their maximal oxygen uptake (VO2 max). Participants were required to pedal on a cycle ergometer at a progressively increased workload (every 10 min.) corresponding to 40, 50, 60, 70, and 80% of individual VO2 max values. Heart rates, RPE, and core temperatures were recorded every 5th minute after the initiation of exercising at a specific workload. Analysis indicated that, when controlling for VO2 max values, elevations of heart rate and core temperature were not affected by fitness. However, highly fit individuals perceived themselves under less exertion than did the group low in fitness. Correlations showed that, taking into consideration fitness, there is a stronger relationship between RPE and heart rate and RPE and core temperature for the highly fit individuals than for the less fit.

Abstract:York in genetics, as in most other branches of biology, has so far been essentially analytical. A century of dissection of heredity has led to a reasonable picture of that the ‘factor’ disclosed by Mendel’s work is made of and how it carries specificity. The gap created between the factor and the character is progressively hied by well-defined molecules. It seems justified to hope that, in some not too instant future, the structure and properties of such molecules will be sufficiently understood. The main programme for the Mendel bicentenary will probably consist essentially determining how molecular structures recognize each other to form cellular superstructures and how they interact in vivo to coordinate the extraordinarily complex chemistry of the cell. In genetics, this problem is merely that of the chromosomal structure, of its regulation and its reproduction in strict coordination with cellular growth and division. It seems fair to say that, if some analytical information concerning the components of a chromosome is now available, little known about the molecular arrangement of these components. As for the reproduction of chromosomes, this is obviously a very complicated process since it represents a crucial event in the chemistry of the cell and must therefore be a losely connected with a great many other processes. The complexity of this feroblem is exemplified by the following argument: there exists good experimental evidence indicating that DNA does replicate according to the mechanism that Watson & Crick (1953) predicted from their model; furthermore, from the work of Kornberg (1961) and his colleagues, an enzyme is known which, in vitro, is able order and polymerize deoxynucleotides according to the sequence imposed by DNA template; yet if a fragment of bacterial DNA is transferred to a recipient bacterium, by transformation or incomplete conjugation, it appears to be unable to be replicated as such. It is replicated only when it has been integrated, as a result of genetic recombination with one of the DNA structures present in the bacterial cells.
Epidemiology and Clinical Outcomes of Multidrug-resistant, Gram-negative Bloodstream Infections in a European Tertiary Pediatric Hospital During a 12-month Period

Abstract:BACKGROUND: Bloodstream infections caused by multidrug-resistant, Gram-negative (MDRGN) bacteria represent a significant cause of morbidity and mortality. Prompt diagnosis and appropriate empiric treatment are the most important determinants of patient outcome. The objective of our study was to assess the epidemiology and clinical outcome of MDRGN sepsis in a tertiary-care pediatric hospital during a 12-month period.METHODS: It was a retrospective, observational study of MDRGN bacteremia including all patients <18 years of age, hospitalized during 2011, with documented bacteremia caused by Enterobacteriaceae or non-fermentative bacteria.RESULTS: Overall, 136 blood cultures in 119 patients were included. The median age of patients was 1.1 years; 86.3% of patients had an underlying disease. The cumulative incidence of Gram-negative bloodstream infections was 5.4/1000 hospital admissions and the infection rate was 0.65/1000 hospital days. Most frequently isolated strains were Klebsiella pneumoniae, Escherichia coli and Pseudomonas aeruginosa; 67.6% of infections were hospital acquired. The percentage of multidrug-resistant (MDR) organisms among isolated species was 39%. The crude rate of mortality was 16% and sepsis-related mortality was 9.2%. The mortality rate among patients with an antibiotic-resistant isolate was 22.6%. Factors significantly associated with sepsis-related mortality were antibiotic resistance (odds ratio: 4.26, 95% confidence interval: 1.07-16.9) and hospital acquisition of infection (odds ratio: 1.13, 95% confidence interval: 1.05-1.22).CONCLUSIONS: This study demonstrates the high mortality of hospital-acquired MDRGN bacteremia in children. International networks focusing on clinical management and outcomes of MDRGN in children are required. Study of novel antibiotics active against Gram-negative bacteria should include children early in the clinical trial development programs.

Abstract:Infectious complications remain major contributors to adverse outcomes in patients treated for non-communicable disease, particularly in resource limited settings. We performed a 5-year retrospective study of primary bloodstream infections at a dedicated pediatric oncology center in Guatemala. Two hundred and twelve episodes occurring in 194 unique patients qualified for inclusion. Patients required intensive care unit admission in 55% of episodes and death occurred in 24% of episodes. Despite subspecialty support in infectious diseases, poor outcomes, including prolonged hospitalization and mortality, were frequent. Our findings suggest that investments in laboratory and clinical data collection are critical to understanding the contributors to poor outcomes and therefore to improving the quality of bloodstream infection management in resource limited settings.

Abstract:Background: Respiratory infections are a common cause of systemic inflammatory response syndrome (SIRS) in pediatric patients. However, literature specifically addressing respiratory viruses that lead to SIRS or sepsis in children is limited, primarily consisting of case reports and lacking data for more comprehensive conclusions. Objectives: This study aims to investigate the relationship between respiratory viral infections and SIRS in children, focusing on virus detection rates, patient profiles, and the incidence of septic shock associated with different respiratory viruses. Methods: A retrospective analysis was conducted from January 2022 to December 2023 on hospitalized children diagnosed with respiratory infections and SIRS. Respiratory viruses were detected through qualitative PCR testing, and procalcitonin levels were measured using an enzyme-linked fluorescent immunoassay. Results: A total of 931 pediatric patients with SIRS were tested for respiratory viruses and/or other pathogens. Among these, 298 (32.0%) were positive for respiratory viruses, with 263 (28.2%) cases attributed solely to respiratory viruses. Human rhinovirus (HRV) had the highest infection rate at 37.3% (98/263), followed by human adenovirus (HAdV) at 24.3% (64/263). Seasonal distribution among the 263 SIRS cases associated with respiratory viral infections showed the highest incidence in spring (39.5%), followed by summer (25.5%), winter (17.9%), and autumn (17.1%). Septic shock rates were highest among cases of influenza A) FluA(, human metapneumovirus (hMP), and co-infections, with incidences of 27.8%, 18.2%, and 22.7%, respectively. There was significant variation in septic shock rates across different viral infections in pediatric patients (P < 0.001). Conclusions: Many hospitalized children with respiratory viral infections develop SIRS, and a subset of these cases can progress to septic shock.

Abstract:Abstract Background Nosocomial Infections (NI) are a frequent and relevant problem. The purpose of this study was to determine the epidemiology of the three most common NI in a Pediatric Intensive Care Unit from a developing country. Methods We performed a prospective study in a single Pediatric Intensive Care Unit during 12 months. Children were assessed for 3 NI: bloodstream infections (BSI), ventilator-associated pneumonia (VAP) and urinary tract infections (UTI), according to Center for Disease Control criteria. Use of devices (endotracheal tube [ETT], central venous catheter [CVC] and urinary catheter [UC]) was recorded. Results Four hundred fourteen patients were admitted; 81 patients (19.5%) developed 85 NIs. Density of incidence of BSI, VAP and UTI was 18.1, 7.9 and 5.1/1000 days of use of CVC, ETT and UC respectively. BSI was more common in children with CVCs than in those without CVCs (20% vs . 4.7%, p < 0.05). Candida spp. was the commonest microorganism in BSI (41%), followed by Coagulase-negative Staphylococcus (17%). Pseudomonas (52%) was the most common germ for VAP and Candida (71%) for UTI. The presence of NI was associated with increased mortality (38.2% vs . 20.4% in children without NI; p < 0.001) and the median length of ICU stay (23 vs . 6 days in children without NI; p < 0.001). Children with NI had longer average hospital stay previous to diagnosis of this condition (12.3 vs . 6 days; p < 0.001). Conclusions One of every 5 children acquires an NI in the PICU. Its presence was associated with increased mortality and length of stay. At the same time a longer stay was associated with an increased risk of developing NI.

Abstract:A new time-averaged frame subtraction technique is introduced for vibration analysis by digital speckle shearing interferometry. The technique permits the enhancement of fringes by subtracting two Bessel fringe patterns at different forcing levels. Compared with the phase-shift method, this method is more efficient and easier to implement for qualitative vibration measurement, providing a means for fast inspection of plate vibration behavior. It is also capable of tracing contours of zero strain and locating antinodes on vibrating plates.

Abstract:Infections constitute a major cause of morbidity and mortality in patients undergoing hematopoietic stem cell transplantation (HSCT). In the pediatric population, infection-associated mortality rate ranging from 7% to 17% has been reported, depending on the type of HSCT, being lower in autologous transplants and higher in cases involving unrelated donors. Other factors related to HSCT that influence the clinical outcomes of these patients include the intensity of the conditioning regimen, the development of graft-versus-host disease (GVHD), cytotoxic and immunosuppressive effects of the medications used, the presence of disease activity prior to HSCT, length of hospital stay, and the use of intravascular devices, among many others. All pediatric patients (under 16 years old) undergoing HSCT at the 'Dr. José Eleuterio González' University Hospital over a 55-month period (March 2019 – October 2023) were included. Clinical and epidemiological characteristics of the patients, mortality rate, and risk factors related to transplantation and infectious events (IEs) in these patients were recorded. A total of 75 patients who received HSCT were included, among them, 52 patients (69%) presented IEs. The mortality associated with IEs among these patients was 6% at day 30 and 12% at day 100, compared to 0% and 1.3% respectively in the non-IEs related deaths. The epidemiological characteristics, baseline diagnosis, donors and graft source are described in table 1 . The average number of IEs in the associated mortality cases was 2.8 compared to 1.6 in the survival group. Among the mortality group (8 cases), 67% presented multiple infections and 33% single infections, with a 75% of them caused by viral infections. Regarding HSCT characteristics, in the group with IE-associated mortality, a higher proportion of haploidentical transplants, myeloablative conditioning regimens, lower use of total body irradiation, and a higher proportion of patients with detectable disease by flow cytometry prior to HSCT were observed compared to the survival group. Among non-infectious complications related to HSCT a lower frequency of acute GVHD grade I-II, a higher frequency of acute GVHD grade III-IV in the IE-associated mortality group was identified. Among the risk factors associated with hospital care, the IE-associated mortality group showed a higher use of central venous catheters, mucositis, systemic steroids, and pediatric intensive care unit stay compared to the survival group. Pediatric patients undergoing HSCT present multiple risk factors that increase the incidence of infectious events and contribute to higher mortality rates. It is crucial to identify those factors related to adverse events and death to implement strategies that reduce exposure to these factors and improve the clinical outcomes. Table 1. Abstract:A blood culture (BC) is frequently requested in both patients with a suspected occult bacteremia/invasive infection as well as those with certain focal infections. Few data are available on the characteristics of patients in whom a bacteremia is identified in the Pediatric Emergency Department (PED). A prospective multicenter registry was established by the Spanish Pediatric Emergency Society. Epidemiological data, complementary test results, clinical management, and final outcome were recorded. Data from the first three years of the registry were analyzed. A true bacterial pathogen grew in 932 of 65,169 BCs collected [1.43 %; 95 % confidence interval (CI) 1.34–1.51 %], with 711 of them collected in patients without previously known bacteremia risk factors. Among them, 335 (47.1 %) were younger than 1 year old and 467 (65.7 %) had a normal Pediatric Assessment Triangle (PAT) on admission. Overall, the most frequently isolated bacterial species was Streptococcus pneumoniae (27.3 %; 47.6 % among patients with an altered PAT). The main pathogens were Escherichia coli (40.3 %) and S. agalactiae (35.7 %) among patients younger than 3 months, S. pneumoniae among patients 3–60 months old (40.0 %), and S. aureus (31.9 %) among patients over 60 months of age. Neisseria meningitidis was the leading cause of sepsis in patients older than 3 months. Eight patients died; none of them had a pneumococcal bacteremia and all had abnormal PAT findings on admission. S. pneumoniae is the main cause of bacteremia in patients without bacteremia risk factors who attended Spanish PEDs. Age and general appearance influence the frequency of each bacterial species. General appearance also influences the associated mortality.

Abstract:BACKGROUND There are limited data on respiratory syncytial virus (RSV) infection among children in settings with a high prevalence of human immunodeficiency virus (HIV). We studied the epidemiology of RSV-associated acute lower respiratory tract infection (ALRTI) hospitalizations among HIV-infected and HIV-uninfected children in South Africa. METHODS Children aged <5 years admitted to sentinel surveillance hospitals with physician-diagnosed neonatal sepsis or ALRTI were enrolled. Nasopharyngeal aspirates were tested by multiplex real-time polymerase chain reaction assays for RSV and other viruses. Associations between possible risk factors and severe outcomes for RSV infection among HIV-infected and uninfected children were examined. The relative risk of hospitalization in HIV-infected and HIV-uninfected children was calculated in 1 site with population denominators. RESULTS Of 4489 participants, 4293 (96%) were tested for RSV, of whom 1157 (27%) tested positive. With adjustment for age, HIV-infected children had a 3-5-fold increased risk of hospitalization with RSV-associated ALRTI (2010 relative risk, 5.6; [95% confidence interval (CI), 4.5-6.4]; 2011 relative risk, 3.1 [95% CI, 2.6-3.6]). On multivariable analysis, HIV-infected children with RSV-associated ALRTI had higher odds of death (adjusted odds ratio. 31.1; 95% CI, 5.4-179.8) and hospitalization for >5 days (adjusted odds ratio, 4.0; 95% CI, 1.5-10.6) than HIV-uninfected children. CONCLUSION HIV-infected children have a higher risk of hospitalization with RSV-associated ALRTI and a poorer outcome than HIV-uninfected children. These children should be targeted for interventions aimed at preventing severe RSV disease.

Abstract:Background: Respiratory viruses contribute to the seasonal pattern of invasive pneumococcal disease (IPD), but the impact of viral coinfections on the clinical characteristics and outcomes of patients with IPD have not been well defined. Objective: This study was designed to describe and compare the clinical presentations and outcomes of patients with IPD with or without viral coinfections. Design/methods: Retrospective analyses of records of all children treated at Children's Medical Center Dallas (CMCD) for IPD from July 2005 to June 2008. Viral studies included viral direct fluorescent antibody staining and culture. For comparisons, patients were classified in 3 groups: with positive, negative, and no viral studies performed. Results: A total of 129 patients were admitted to CMCD with IPD during the 3 year study; 57% were male. Ages ranged from 2 months to 18 years (median 25 months) and 48% were <2 years. Viral studies were performed in 82 (63%) patients, and 28 (34%) had positive results. The most common viruses isolated were influenza (7, 25%), rhinoviruses (6, 21%), adenoviruses (6, 21%), and RSV (5, 18%). Peaks of positive viral studies occurred in February and November which coincided with the peak numbers of patients admitted with IPD. Of 6 with adenovirus coinfection, 5 were admitted to Pediatric Intensive Care Unit (PICU). The most common pneumococcal serotypes were 19A (41, 32.5%), 7F (14, 11%), and 23A (13, 10.3%). Pneumonia (42%), bacteremia (22%), and meningitis (17%) were the most common clinical syndromes. There were no differences in duration of fever before admission, maximum temperatures during hospitalization and white blood cell counts, duration of fever and hospitalization between patients with positive and negative viral studies, but there was a trend for patient with positive viral studies to be admitted to PICU more frequently and to have longer PICU stay. Three of the 6 patients who died had documented viral coinfections (2 adenovirus, 1 parainfluenza 3), and all 3 had no underlying conditions. The other 3 patients who died had no viral studies performed. Duration of treatment ranged from 1 to -210 days (median 14), with no differences among the groups. Conclusions: Viral coinfections were common in children with IPD. Future prospective studies should include new PCR assays to characterize better the impact of viral coinfections in the occurrence and outcome of IPD.
