Background:Peripheral tears of the posterior horn of the medial meniscus, known as "ramp lesions," are commonly found in anterior cruciate ligament (ACL)–deficient knees but are frequently missed on routine evaluation.Purpose:To predict the presence of ramp lesions in ACL-deficient knees using machine learning methods with associated risk factors.Study Design:Cohort study (Diagnosis); Level of evidence, 2.Methods:This study included 362 patients who underwent ACL reconstruction between June 2010 and March 2019. The exclusion criteria were combined fractures and multiple ligament injuries, except for medial collateral ligament injuries. Patients were grouped according to the presence of ramp lesions on arthroscopic surgery. Binary logistic regression was used to analyze risk factors including age, sex, body mass index, time from injury to surgery (≥3 or .05). The cut-off values for the presence of ramp lesions on receiver operating characteristic analysis were as follows: medial tibial slope >5.5° (P 5.0° (P 71.3% (P = .033).Conclusion:Steep medial tibial and meniscal slopes, an increased LFC ratio, and higher knee rotatory laxity were observed risk factors for ramp lesions in patients with an ACL injury. The prediction model of this study could be used as a supplementary diagnostic tool for ramp lesions in ACL-injured knees. In general, care should be taken in patients with ramp lesions and its risk factors during ACL reconstruction.

Background:Medial meniscal ramp lesions have recently been an area of interest because of their recognized prevalence in association with anterior cruciate ligament (ACL) ruptures. Anatomically, the medial meniscal ramp is composed of the meniscocapsular ligament in continuity with the semimembranosus muscle and the meniscotibial ligament. Diagnosis of ramp semimembranosus complex (RSC) injuries remains challenging, and their prevalence is likely to be underestimated in comparison with ramp lesions.Purpose:To determine the prevalence of RSC lesions after a complete ACL rupture.Study Design:Cross-sectional study; Level of evidence, 3.Methods:A retrospective database analysis was performed. The cohort consisted of the first 100 patients with complete ACL rupture confirmed by magnetic resonance imaging (MRI) who underwent knee arthroscopy for ACL reconstruction in 2019. The semimembranosus lesions were identified using MRI by 2 independent radiologists specializing in musculoskeletal imaging. The ramp lesions were initially diagnosed using MRI and then confirmed during arthroscopy by an experienced knee surgeon. The magnitude of rotatory instability was recorded using the pivot-shift test. A multivariate analysis was used to determine the lesions associated with the semimembranosus complex.Results:Of 100 patients, 53 showed lesions of the RSC; among them, 30 ramp lesions were confirmed after arthroscopic evaluation, and 40 semimembranosus lesions (23 without ramp lesion and 17 with ramp lesion) were found using MRI. A positive pivot shift was present in 57% of patients with combined RSC injury (P = .04) compared with 36% in patients without an RSC lesion.Conclusion:Lesions of the RSC were found in more than half of ACL ruptures in this retrospective cohort. Rotational instability could be associated with combined ACL and RSC injury.

Anterior Tibial Subluxation of Lateral Compartment is Associated with High-Grade Rotatory Instability for Acute but Not Chronic Anterior Cruciate Ligament Injuries: an Magnetic Resonance Imaging Case-Control Study

Abstract:Purpose: To investigate whether anterior tibial subluxation obtained from magnetic resonance imaging (MRI) could be a predictor of high-grade rotatory instability for anterior cruciate ligament (ACL) injuries, including acute and chronic cases. Methods: From September 2016 to August 2018, we retrospectively investigated 163 patients with ACL injuries who subsequently underwent primaryACL reconstruction. Among them, 30 patients with high-grade rotatory instability (grade II/ III pivot shift) were included in the high-grade group, and their age and sex were matched 1:2 to low-grade cases (3months) phases. Results: The high-grade group had a larger anterior tibial subluxation of lateral compartment (8.1mmvs 5.9 mm; P =.004) than the low-grade group, whereas no significant difference was found in anterior tibial subluxation of medial compartment (P >.05). Moreover, high-grade anterior tibial subluxation of lateral compartment (>= 6mm) was found to be an independent predictor (odds ratio, 12.992; P 1/4 .011) associated with concomitant meniscal tears after ACL injuries. Anterior tibial subluxation of lateral compartment demonstrated statistical significance between the two groupswhen comparing subgroups within 3months but not beyond 3months. Conclusion: InACL-injured patients, high-grade anterior tibial subluxation of lateral compartment (>= 6 mm) could be a unique predictor of high-grade knee rotatory instability for acute but not chronic injuries. Prolonged time from injury to surgery and lateral meniscus tears were risk factors for high-grade rotatory laxity in chronic patients. Level of Evidence: Level III, retrospective prognostic trial.

Abstract:Purpose To assess the incidence of ramp lesions in anterior cruciate ligament (ACL) injuries and to compare preoperative knee laxity between the patients with and without ramp lesions by using an electromagnetic measurement system (EMS). Methods Two hundred six patients who underwent primary ACL reconstruction with preoperative EMS measurements were retrospectively enrolled in the present study. The diagnoses of the ramp lesions were made by arthroscopic inspections. The patients with ramp lesions and no other meniscal lesions were allocated to 'ramp group', and the patients without any meniscal lesions were allocated to 'control group'. Before ACL reconstruction under general anaesthesia, the side‐to‐side difference (SSD) in anterior tibial translation (ATT) during Lachman test (mm) and tibial acceleration (m/s2) of posterior tibial reduction during the pivot‐shift test was measured using the EMS. The SSD in tibial internal/external rotation angle (°) at 30, 60 and 90 were further measured using the EMS. The SSD in ATT using KT‐2000 was also measured. Knee laxity measurements were compared between two groups using unpaired Student's t test. Results Ramp lesions were observed in 30 patients (14.7%). Subsequently, 17 patients were allocated to 'ramp group' and 77 patients to 'control group', and there were no statistical differences with regard to background demographics. ATT‐SSD during Lachman test was significantly greater in 'ramp group' (9.1 [95% confidence interval, CI: 5.7–12.5] mm vs. 6.2 [95% CI: 5.1–7.3] mm, p = 0.037). However, SSD in ATT with KT‐2000, tibial acceleration during pivot‐shift test, and SSD in tibial rotational angles were not significantly different between the two groups. Conclusion Presence of ramp lesion was associated with increased anterior knee laxity during Lachman test, suggesting ramp lesions may need to be addressed at the time of ACL reconstruction. Abstract:Background: Medial meniscus ramp tears are present in 22.9–40.8% of anterior cruciate ligament tears. The diagnosis of ramp tears is difficult on MRI, with sensitivity reported around 48%, which has recently emphasized the importance of proper arthroscopic probing for ramp tears. Methods: A prospective evaluation was performed on patients undergoing a single bundle ACL reconstruction to assess patient demographics, posterior tibial slope, posterior cruciate ligament angle, Lachman's exam, and rotational instability to determine secondary findings associated with medial meniscal ramp tears. Results: A total of 96 patients underwent ACL reconstruction, of these, 63 patients had an isolated ACL tear, and 33 patients had an ACL tear with a concomitant medial meniscus ramp tear. A high-grade Lachman's exam and male sex were associated with medial meniscus ramp tears. There were no differences in posterior tibial slope, posterior cruciate ligament angle, or rotational instability between groups. Conclusions: This study found that a high-grade Lachman's exam and male sex were significantly associated with patients with an ACL tear with a concomitant medial meniscus ramp tear. These findings suggest that an ACL tear with concomitant medial meniscus ramp tear may be better diagnosed based upon the clinical evaluation rather than other secondary radiological findings.

ObjectivesTo evaluate the incidence and epidemiological aspects of meniscal ramp injuries in patients undergoing anterior cruciate ligament (ACL) reconstruction, and determine the related risk factors.Methods824 patients who underwent ACL reconstruction were retrospectively analyzed. Patients who presented medial meniscal instability underwent evaluation of the posteromedial compartment of the knee. In case of injury, surgical repair was performed. Potential risk factors associated with injuries were analyzed.ResultsThe overall incidence of ramp injuries in the study was 10.6% (87 injuries in 824 patients). Multivariate analysis using the Chi-Square test demonstrated that the presence of meniscal ramp injuries was significantly associated with the following risk factors: right laterality and chronic injuries. Sex, age and sporting activity were not statistically significant. Football was the most frequent cause of sports-related ramp injuries, accounting for 78.2% of cases. However, it did not prove to be a risk factor. The annual incidence ranged from 4.0% to 20.6%.ConclusionThe incidence of meniscal ramp lesions was 10.6% in ACL reconstruction, being more frequent in chronic injuries. The increasing annual incidence ranged from 4.0% in 2014 to 20.6% in 2019.

Abstract:Background: There is a lack of research on the management of ramp lesions associated with anterior cruciate ligament (ACL) injuries. Furthermore, there has been no report of the risk factors for failure of ramp lesion sutures, linked to either the technique used (all-inside suture implant vs suture hook through a posteromedial portal) or the type of lesion (location in the red zone or meniscocapsular junction, longitudinal extension, partial- or full-thickness tear). Purpose: To evaluate the results of arthroscopic repair of ramp lesions and determine the risk factors associated with ramp lesion repair failure, with special focus on their subtype and the suture technique. Study Design: Case-control study; Level of evidence, 3. Methods: All patients who underwent arthroscopic ramp lesion repair in association with ACL reconstruction between November 2015 and January 2018 were evaluated retrospectively. The following parameters were studied: demographics; clinical history; clinical findings including International Knee Documentation Committee score, complications, time from injury to surgery, side-to-side laxity, and pivot shift; and surgical findings including subtype, surgical management, and type and number of sutures. Failure of the ramp lesion repair was defined at secondary arthroscopy. Results: Among the 248 lesions analyzed, 18 (7.3%) failures were documented. The failures occurred in 21.1% of repairs managed with the all-inside device versus 4.3% of sutures managed with the suture hook ( P = .003). Among the 6 factors included in the Cox model, the only one identified as a risk factor for failure was the type of repair ( P = .003), with a risk for the all-inside device that was >5-fold higher than that for the suture hook repair (corresponding hazard ratio, 5.1 [95% CI, 1.8-14.5]). No other complications involving the surgical technique or device were registered. Conclusion: An arthroscopic all-inside technique of meniscal repair of ramp lesions appeared to be safe and effective. It provided excellent healing of the repaired meniscus, with an overall failure rate of 7.3%. The type of suture was associated with failure of the ramp lesion repair, with a significantly higher risk with the all-inside device than with suture hook repair sutures.

Abstract:Background: Nowadays, the anterior cruciate ligament (ACL) injury has been one of the most common diseases of the knee joint. The relationships between the ACL injury and the anatomical structures are still controversial. This study aimed to identify the anatomical risk factors of ACL injury by magnetic resonance imaging (MRI) of the knee. Methods: This was a retrospective study of 125 patients undergoing primary ACL reconstruction between July 2013 and May 2017. Another 125 patients without any organic knee joint injury were served as controls. The shape of intercondylar notch, the intercondylar notch width index, the intercondylar notch height index, the alpha angle, the beta angle, and the medial and lateral tibial plateau slope were measured with MRI and compared. The data were compared by binary logistic regression to find the risk factors. Results: The two groups differed in the proportion of male patients (70.4% vs. 52.0%, chi(2) = 8.911, P = 0.003), but gender was excluded as a risk factor for ACL injury with regression analysis (odds ratio = 1.476, 95% confidence interval [CI]: 0.689-3.160, P = 0.317). The injured group was found to have a smaller notch width index (95% CI = 7.960E-23-2.154E-9, P < 0.001), a larger beta angle (95% CI = 1.311-1.785, P < 0.001), and a larger lateral tibial plateau slope (95% CI = 1.201-1.683, P < 0.001). The cutoff values of notch width index, beta angle, and the lateral tibial plateau slope were 0.252, 38.5 degrees, and 7.5 degrees, respectively. Conclusions: In this study, a narrow intercondylar notch (intercondylar notch width index < 0.252), a larger lateral tibial slope (> 7.5 degrees), and larger beta angle (> 38.5 degrees) might be the factors associated with ACL injury
Steep posterior slope of the medial tibial plateau is associated with ramp lesions of the medial meniscus and a concomitant anterior cruciate ligament injury

Abstract:BackgroundMedial meniscus (MM) tears are associated with both acute and chronic anterior cruciate ligament (ACL) insufficiency and can lead to degenerative changes in the knee. ACL reconstruction (ACLR) combined with the meniscal repair was reported to result in decreased anterior knee joint laxity with evidence of improved patient-reported outcomes in the long term. However, a subtle tear of the MM posterior segment, also known as a ramp lesion, is difficult to detect on conventional magnetic resonance imaging (MRI) and is frequently missed in ACL-deficient knees. However, there are few studies about the associations between bone geometry and ramp lesion of the MM. This study aimed to compare sagittal medial tibial slope (MTS), medial tibial plateau depth (MTPD), and coronal tibial slope (CTS) between ACL-injured knees with and without ramp lesion of the MM. We hypothesised that patients with ramp lesion of the MM and a concomitant ACL injury have a steeper MTS and shallower MTPD than those without ramp lesion of the MM.MethodsTwenty-seven patients who underwent ACLR (group A), and 15 patients with combined MM repair (group AM) were included in the study. Anterior tibial translation (ATT) was measured under general anaesthesia just before surgery using a knee arthrometer. MRI was performed in the 10°-knee-flexed position. The MTS and MTPD were measured on sagittal view, and the CTS was measured on coronal view. These parameters were compared between the groups. Differences in MRI measurements or patient demographics between the groups were evaluated using the Mann-Whitney U test.ResultsNo significant difference was observed in demographic data and post-operative side-to-side difference in ATT between both groups. Pre-operative ATT was significantly higher in group AM than in group A (P < 0.05), whereas post-operative ATT was similar in both groups. Further, Pre-operative ATT was significantly higher in patients with MTS ≥5.0° than in those with MTS <5.0° (P < 0.05). In groups A and AM, the MTS were 3.6° ± 1.8° and 6.2° ± 2.9°, the MTPD were 2.0 ± 0.5 mm and 2.1 ± 0.6 mm, and the CTS were 2.5° ± 1.8° and 2.4° ± 1.6°, respectively. Patients in group AM had a significantly steeper MTS compared to those in group A (P < 0.01), whereas MTPD and CTS were nearly the same in both groups. When the MTS cut-off value was set at 5.0°, the sensitivity and specificity for ACL injury with concomitant ramp lesion of the MM were 0.73 and 0.76, respectively.ConclusionA steep posterior slope of the medial tibial plateau is a risk factor for ramp lesion of the MM associated with an ACL injury. Especially in patients with MTS ≥5.0°, an occult MM ramp lesion should be strongly suspected, and surgeons should prepare for MM repair in combination with ACLR.
Biomechanical impact of meniscal ramp lesions on knee joint contact characteristics in ACL deficient knees: a cadaveric analysis

Abstract:Purpose: To evaluate the changes in contact characteristics of the tibiofemoral joint resulting from a meniscal ramp lesion in the medial meniscus. Methods: Twelve cadaveric knees (six matched pairs) were subjected to a 600 N axial load using a custom testing jig, which allowed for knee positioning at 0°, 45°, and 90° of flexion without other constraints. The knees were randomly assigned to either a ramp lesion group (n = 6) or a posterior root lesion group (n = 6). Four testing conditions were examined: (1) intact, (2) isolated ramp lesion, (3) isolated posterior root tear of the medial meniscus, and (4) combined ramp lesion and posterior root tear of the medial meniscus. Contact characteristics were evaluated using a flexible pressure sensor, the I-Scan System. Results: Peak contact pressure in isolated ramp lesions (4.15 ± 0.98 MPa, P = 0.206) showed non-significant increases compared to the intact condition (3.86 ± 1.32 MPa). Peak contact pressure in isolated posterior root tears (4.58 ± 1.70 MPa, P = 0.040) and, combined ramp and posterior root lesions (4.67 ± 1.47 MPa, P = 0.003) were significantly higher than that in the intact condition. The knee flexion position significantly affected the medial tibiofemoral joint's contact area, contact pressure, and peak contact pressure (P < 0.001 for all). Conclusion: Isolated ramp lesions did not significantly impact force transmission, contact area, or contact pressure. In contrast, isolated root lesions and combined ramp and posterior root tears of the medial meniscus significantly intensified the changes in contact characteristics in the medial tibiofemoral joint compared to the intact condition. Abstract:Introduction: Ramp lesions, often associated with anterior cruciate ligament (ACL) injuries, exhibit a varied incidence rate of 9–42%, increasing with delayed ACL reconstruction. These lesions, resulting from abnormal tibial movements and semimembranosus muscle contraction, are challenging to diagnose due to their hidden nature in standard magnetic resonance imaging and arthroscopy procedures. Case Report: This report examines the case of a ramp lesion in the context of a multi-ligament injured knee of a 34-year-old male. The patient had a complete ACL, medial collateral ligament, and avulsion of the posterior cruciate ligament with a type 1 ramp lesion. These findings were confirmed by arthroscopy and were treated with arthroscopic reconstruction of the ligament and all inside repair of the ramp lesion. We report the pertinent imaging findings relevant to the ramp lesions. Conclusion: Ramp lesions present a significant diagnostic and treatment challenge in orthopedic practice. Enhanced imaging techniques and a deeper understanding of their pathophysiology are crucial for an accurate diagnosis. Keywords: Ramp, hidden lesion, magnetic resonance imaging, arthroscopy.

Abstract:Background: Despite vigorous efforts to delineate the efficacy of magnetic resonance imaging (MRI) for the diagnosis of meniscal ramp lesions, there is still a great deal of uncertainty regarding its diagnostic performance. Therefore, we conducted a systematic review and meta-analysis to investigate the diagnostic performance of MRI for detecting ramp lesions in anterior cruciate ligament (ACL)-deficient knees. Methods: We performed a systematic search of MEDLINE via PubMed, Scopus, Web of Science, and Embase and included all articles, published before October 20, 2022, comparing the accuracy of MRI with that of arthroscopy as the gold standard for diagnosis of ramp lesions. We performed statistical analysis using Stata and Meta-DiSc software. Quality assessment of the included studies was performed using the QUADAS-2 (Quality Assessment of Diagnostic Accuracy Studies-2) tool. Results: This meta-analysis evaluated 21 diagnostic performance comparisons from 19 original research articles (2,149 patients). The pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and area under the summary receiver operating characteristic (SROC) curve for diagnosing a ramp lesion were 0.70 (95% confidence interval [Cl], 0.66 to 0.73), 0.88 (95% Cl, 0.86 to 0.89), 6.49 (95% Cl, 4.12 to 10.24), 0.36 (95% Cl, 0.28 to 0.46), 24.33 (95% Cl, 12.81 to 46.19), and 0.88, respectively. Meta-regression using different variables yielded the same results. Conclusions: MRI exhibited a DOR of 24.33 and moderate sensitivity, specificity, and accuracy for diagnosing ramp lesions in ACL-deficient knees. However, arthroscopy using a standard anterolateral portal with intercondylar viewing is recommended to confirm a diagnosis of a ramp lesion. Level of Evidence: Diagnostic Level III . Abstract:Purpose The study aimed to estimate the prevalence of ramp lesions among patients undergoing anterior cruciate ligament (ACL) reconstruction and identify risk factors associated with these lesions. Methods A retrospective, multicentre cohort study was conducted using data from the Francophone Arthroscopic Society's registry, including 5359 patients who underwent ACL reconstruction (ACLR) from June 2020 to June 2023. Potential risk factors for ramp lesion such as patient demographics, revision surgery, pivot shift, side‐to‐side anteroposterior laxity, medial collateral ligament (MCL) injury, lateral meniscal tear and the volume of ligament remnant were evaluated using multivariate regression analyses. Abstract:Abstract Objectives The purpose of the present paper was to study isolated meniscal ramp lesions without obvious ACL rupture. Their biomechanical mechanisms were analyzed and their clinical characteristics were reviewed. The clinical effects of an all‐inside horizontal mattress suture for isolated ramp lesions were evaluated. Materials and Methods Twenty isolated meniscal ramp lesion patients without obvious ACL rupture from 2015 to 2017 were retrospectively reviewed. Preoperative MRI showed intact ACL and signs of ramp lesions. These isolated ramp lesions were arthroscopically confirmed and repaired through an all‐inside horizontal mattress suturing method. MRI was performed 3 months postoperatively to assess isolated ramp lesion healing. The Tegner–Lysholm score and the visual analog scale score were recorded preoperatively and at 2 years postoperatively. The Wilcoxon rank sum test was performed to determine statistical significance. Results Arthroscopic exploration confirmed isolated ramp lesions and longitudinal ACL splits or degeneration without obvious ACL rupture. MRI 3 months postoperatively showed healing of the isolated ramp lesions. At 2 years postoperatively, the VAS scores were significantly decreased and the Tegner–Lysholm scores were significantly increased. Knee function, without pain, was restored in all patients, including walking, climing and descending stairs, and squatting. Conclusion Isolated meniscal ramp lesions without obvious ACL rupture may exist because of ACL longitudinal splits or degeneration and can be repaired through anterolateral and anteromedial portals with an all‐inside horizontal‐mattress suturing method.

Abstract:Purpose The purpose of this study was to assess the frequency of medial collateral ligament (MCL), posterior oblique ligament (POL) and anterolateral ligament (ALL) tears and different types of RAMP lesions of patients with verified acute anterior cruciate ligament (ACL) tears by magnetic resonance imaging (MRI). Methods MRI was performed on patients with a clinical diagnosis of acute ACL injury. Patients were eligible for inclusion if they had an initially clinically noted ACL tear confirmed on MRI within 30 days of trauma. Results A total of 146 patients were included in the study, 42 (28.8%) females and 104 (71.2%) males. The mean age at MRI was 27.2 ± 9.4 years, and the mean time from injury to MRI was 15.7 ± 7.8 days. Thirty‐four (23.3%) patients had a complete MCL lesion, 32 (21.9%) had a complete POL lesion and 28 (19.2%) had a complete ALL lesion. One hundred and fourteen patients (78.1%) presented with RAMP lesions, while 20 (13.7%) patients reported other meniscal lesions. The mean medial and lateral tibial slopes were 4.0° ± 2.7° and 4.0° ± 3.1°, respectively. Only 10 (6.8%) patients reported no lesions associated with ACL rupture. The most common injuries were isolated RAMP type 3 (18–12.3%) and isolated RAMP type 1 (17–11.6%). Thirteen (8.9%) patients had a combination of MCL, POL and ALL rupture. Conclusions Isolated lesions of the ACL are extremely rare. In most cases, a single RAMP lesion should be investigated. In the presence of MCL injury, POL injury should always be suspected as well, while nearly 20% of patients present a rupture of the ALL. About one in 10 patients had three lesions (MCL, ALL and POL), and most of them had a combined RAMP lesion. Level of Evidence Level IV.

Abstract:Abstract Background Ramp lesion is defined as a vertical longitudinal detatchment of PHMM due to meniscocapsular ligament tears that is not greater than 2 cm in medio-lateral length resulting in separation of meniscocapsular or meniscotibial ligament. Aim of the Work The study aims to perform a systemic review and meta analysis concerning outcomes of ramp lesion repair during ACL reconstruction. Methodology Systematic review and Meta-analysis on Patients with ramp lesion undergoing ACL reconstruction and ramp lesion treatment with total of 387 studies matched our search terms. After exclusion of duplicates, 37 studies were initially included based on their Abstract. 27 studies were finally included. The average MINORS score of all non-randomised studies was 8.7/16 for non- comparative studies and 16.9/24 for comparative studies, respectively. Results Our results suggest that medial meniscus repair associated with ACL reconstruction may restore meniscal function by adjusting the anteroposterior length of the torn medial meniscus. Our data demonstrate that more than 75% of medial meniscal tears in the ACL-deficient knee occur in the peripheral posterior horn. If we assume that the anatomy as described by Hughston and Eilers is correct, then care should be taken when addressing these tears, because a strong repair is needed to preserve the stability of the knee. Conclusion MRI demonstrated moderate sensitivity and excellent specificity in the diagnosis of ramp lesion in patients with ACL tear. According to the current systematic review, ramp lesions are common in ACL-deficient knees and are often missed through standard anterior portals. Exploration of the meniscocapsular complex via an additional posteromedial portal is recommended, if MRI suggests a lesion in this area or if instability is present at probing. If the posteromedial view still does not reveal any lesion despite significant instability, soft-tissue debridement might uncover hidden lesions (meniscotibial ligament disruptions). Ramp lesion repair leads to a significant improvement of subjective knee scores, regardless of the specific fixation technique. For stable ramp lesions, trephination and abrasion might be equivalent to ramp lesion repair in terms of postoperative stability.

Abstract:The aim of the present study was to compare the morphometric differences between patients with or without anterior cruciate ligament (ACL) injury, and identify the anatomic risk factors associated with ACL injury in active individuals. The knee joint magnetic resonance images (MRI) of 100 subjects were included in this study. Data from the ACL-injured group (50 patients) and matched controls (50 subjects) were obtained from the same hospital. These data were analyzed by univariable analysis or multivariable conditional logistic regression analysis to examine the effects of the following variables on the risk of suffering ACL injury for the first time: TT-TG distance, medial and lateral tibial slope, intercondylar notch width and depth, femur condylar width, lateral femoral condylar depth, notch width index (NWI), notch shape index (NSI), notch depth index (NDI), and cross-sectional area (CSA). In the univariable analysis, the ACL-injured group had a larger TT-TG distance, increased medial and lateral tibial slope, narrower intercondylar notch width, deeper lateral femoral condylar depth, lesser NWI and NSI, and CSA when compared with the control group (P < 0.05). However, there were no significant between-group differences in intercondylar notch depth (P = 0.174), femur condylar width (P = 0.797), and NDI (P = 0.436). The multivariable analysis revealed that TT-TG distance [odds ratio (OR) = 1.37, 95% CI = 1.04–1.81, P = 0.028], medial tibial slope (OR = 1.30, 95% CI = 1.02–1.66, P = 0.036) and NWI (OR = 0.46, 95% CI = 0.24–0.91, P = 0.025) had significant multivariable associations with the sole independent risk of ACL injury. Larger TT-TG distance, increased MTS, and lesser NWI are independent anatomic risk factors for active individuals with ACL injury. Case-control study; Level of Evidence, III.

Abstract:Importance: The diagnosis of ramp lesions can be problematic, even with arthroscopy, due to their extreme posteromedial position. Consequently, they have been colloquially referred to as the "hidden lesions" of the knee. Undiagnosed and untreated injuries in this knee region may be associated with ongoing dynamic rotational laxity of the knee after anterior cruciate ligament reconstruction and an increased risk of anterior cruciate ligament graft failure. Aim: This study aimed to systematically review the literature to assess the sensitivity, specificity and accuracy of magnetic resonance imaging (MRI) for detecting ramp lesions in children and adolescents with anterior cruciate ligament (ACL)-deficient knees. It was hypothesized that MRI has poor sensitivity for identifying ramp lesions in children and adolescents. Evidence review: A systematic review was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The following search terms were used in the title, abstract and keywords fields: "ramp" or "meniscus" AND "children" or "adolescents." The outcome data extracted from the studies were incidence of ramp in concomitant with ACL lesion, MRI sensitivity, specificity, accuracy and positive and negative predictive values (PPV and NPV). Findings: Of the 387 patients with ACL injury, 90 were reported to have ramp lesions (23.3%). The mean age at the time of diagnosis was 15.3 ± 0.81 years. The mean time from injury to MRI was 116.1 ± 113.5 days, while the mean time from injury to surgery was 172.6 ± 139.1 days. The MRI taken to detect ramp lesions in the paediatric population showed a pooled sensitivity of 50%, specificity of 75%, accuracy of 70%, PPV of 41% and NPV of 79%. Conclusions and relevance: The prevalence of ACL-associated ramp lesions in children and adolescents is similar to that in adult populations. Magnetic resonance imaging has low sensitivity but good specificity for assessing ramp lesions. In the presence of a posteromedial tibial bone bruise or a thin fluid signal separating the posterior horn of the medial meniscus and the posteromedial capsule a ramp lesion should always be suspected.
