Abstract:Eosinophilic cystitis (EC) is a rare inflammatory disorder. We herein report a case of EC in an asthmatic female patient who had a recent exacerbation with none known allergen. She was administered montelukast sodium orally for four weeks and received complete remission. This medication was successfully discontinued after a three-month follow up period. This case report about successful treatment of an adult EC patient using montelukast sodium may provide a new option for EC patients with allergic history.

Abstract:There is debate among professionals regarding the use of the glycemic index (GI) for meal planning. In type-1 diabetes, there are 4 studies (average duration approximately 4 weeks) comparing high versus low GI diets; none reported improvements in HbA1c, and although 2 reported improvements in fructosamine, 2 reported no differences. In type-2 diabetes, there are 12 studies (average duration approximately 5 weeks); 3 reported improvements in HbA1c and fructosamine, 5 reported no differences in HBA1c, and 3 reported no differences in fructosamine. In adults, there is limited evidence that a low GI diet is beneficial for weight loss or satiety. Three epidemiologic studies reported that a low GI/glycemic load (GL) is associated with a reduced risk of developing diabetes or prevalence of insulin resistance; however, 5 studies report no association between GI/GL and the risk of developing diabetes, fasting insulin or insulin resistance, or adiposity. In general, the total amount of carbohydrate in a meal is the primary meal-planning strategy for people with diabetes. The GI can be used as an adjunct for the fine tuning of postprandial blood glucose responses. Other food/meal-planning interventions have been shown to be more effective than the use of the GI.

Abstract:The Centers for Disease Control and Prevention reports that more than 4 million children have been diagnosed with asthma. Currently, there is no treatment that could prevent the development of asthma or change its natural course over long-term follow-up. However, the disease can be controlled using treatments used in clinical practice. For persistent asthma in children aged 5 years and younger, low doses of inhaled glucocorticosteroids are recommended, as well as the administration of montelukast, a leukotriene receptor antagonist. In addition, montelukast is prescribed to patients with allergic rhinitis as an alternative to or in combination with oral antihistamines or nasal corticosteroids. Leukotrienes are lipid mediators that play a key role in acute and chronic inflammation and allergic diseases. They exhibit their biological effects by binding to specific G-protein-coupled receptors. Each subtype of the leukotriene receptor has unique functions and expression patterns. Leukotrienes play an important role in various allergic diseases, including bronchial asthma and allergic rhinitis. Montelukast is a cysteinyl leukotriene receptor-1 antagonist widely used to suppress the inflammatory response in asthma and allergic rhinitis. This review briefly summarizes the biology of leukotrienes and their receptors, recent developments in the field of antileukotriene drugs and the prospects for their different therapeutic applications. The role of antileukotriene drugs and key indications for the use of montelukast in the complex therapy of asthma and allergic rhinitis in children have been determined.

Abstract:Leukotriene receptor antagonists, such as montelukast (MK), are currently used to treat rhinitis and asthma, but their anti-inflammatory role in eosinophil inflammation is not well understood. The aim of this study is to investigate the effect of MK on an in vitro model of upper-airway eosinophil inflammation by reducing pro-inflammatory cytokines from both nasal mucosa (NM) and polyp (NP) epithelial cells and reducing eosinophil survival primed by epithelial cell secretions. Epithelial cells were stimulated with fetal bovine serum (FBS) with or without MK for 24 hours, and cytokine concentrations in epithelial secretions were measured by ELISA. After incubating peripheral blood eosinophils with epithelial cell-conditioned media (ECM) with or without MK up to 3 days, eosinophil survival was assessed by Trypan blue dye exclusion. Results are expressed as mean±SEM of cytokine concentration (percent of control) or eosinophil survival (percent). Epithelial cell stimulation increased GM-CSF, IL-6, IL-8, and sICAM-1 secretion in both NM and NP. MK had a significant inhibitory effect on FBS-induced GM-CSF, IL-6, and IL-8 secretion, but not sICAM-1, in both NM and NP. MK also showed an inhibitory effect (p<0.05) on ECM-induced eosinophil survival from both NM (from 10(-5)M to 10(-7)M, n=7) and NP (at 10(-5)M, n=7), after 3 days of incubation. These anti-inflammatory effects on epithelial cell cytokine secretion and on eosinophil survival suggest that montelukast may contribute to the reduction of eosinophilic inflammation in upper-airway inflammatory diseases such as rhinitis and nasal polyposis.

Abstract:Objective To investigate the effects of montelukast(MK),a selective cysteinyl leukotriene receptor 1 antagonist on interleukin(IL)-5 and eotaxin expression as well as serum total immunoglobulin E(IgE) production during MK treatment of mouse acute asthma airway inflammation.Methods Sensitized Balb/c mice were challenged by ovabumin(OVA)to establish the acute asthmatic mo-(del).Twenty-five mg/kg of MK(MK group) or Saline(Saline group)were given by intravenously for 3 days.Cellular infiltration of bronchoalveolar larvage fluid(BALF) were assessed by Wright staining.Production of IL-5 and eotaxin in the lung or BALF and serum IgE were determined by enzyme linked immunosorbent assay(ELISA).The expression of IL-5 and eotaxin mRNA were measured by semi-quantified reverse transcriptase-polymerase chain reaction(RT-PCR).Plasma MK level was determined by liquid chromatography.Results After 24 h OVA challenge,the numbers of total white blood cells,neutrophils and eosinophils(EOS)of BALF were(26.0±18.9)×10~7/L,(5.92±8.09)×10~7/L and(0.74±0.88)×10~7/L in MK treatment group.They were significantly reduced compared with those in Saline group,respectively(P0.05).MK reduced the number of EOS by 80%.The level of IL-5 in BALF and lung tissue were(48.52±14.45) ng/L and(27.40±9.62) ng/g protein in MK group,which significantly declined compared with that in saline group;BALF eotaxin level declined too.Serum IL-5 and total IgE level were also significantly reduced;IL-5 mRNA expression in lung significantly decreased.Eotaxin and its mRNA expression in lung were not decreased significantly.Conclusion MK(exerts) its anti-inflammatory effect mainly through the suppression of IL-5 and IgE production.

METHODS: Asthma model was induced by chronic exposure to ovalbumin (OVA) in C57BL/6 mice. The eosinophils in bronchoalveolar lavage (BAL) fluid and lung tissues were counted, IL-5 level in BAL fluid was measured, and CysLT1 and CysLT2 receptor mRNA expressions were detected by semi-quantitative RT-PCR. RESULTS: Montelukast (6 mg/kg, once per day for 20 d) significantly suppressed the increased eosinophils in BAL fluid and lung tissue, and increased IL-5 level in BAL fluid in OVA challenged mice. OVA challenge increased CysLT1 but decreased CysLT2 receptor mRNA expression. Montelukast inhibited the increased CysLT1 but not the reduced CysLT2 expression after OVA challenge. CONCLUSION: CysLT receptors are modulated immunologically, and montelukast inhibits up-regulation of CysLT1 receptor and airway eosinophilic inflammation in asthmatic mice.
Effect of montelukast on the apoptosis of eosinophil in bronchoalveloar lavage fluid

Abstract:Objective:This study was to investigate the effect of montelukast on the apoptosis of eosinophils(EOS) in bronchoalveolar lavage fluid(BALF) of children with asthma.Methods:42 asthma patients were randomly divided into control group and treatment group according to the asthma severity,21 cases in each group.The control group was given routine treatment,while the treatment group was given 5mg montelukast each day at bedtime on the base of routine treatment.The course of treatment was 4 weeks.Then the apoptosis rates and levels of EOS in two groups before and after therapy were observed to find whether the difference had statistical significance.Results:The level of EOS before therapy had no significant difference between the two groups(P>0.05).The level of EOS after one-week and four-week therapy had statistical significance between the two groups respectively(P<0.05,P<0.01).The percentage of apoptosis of EOS before therapy had no statistical significance between the two groups(P>0.05).The percentages of apoptosis of EOS after one–week and four-week therapy were statistically significant between the two groups(P<0.05,P<0.01).Conclusion:Montelukast can induce eosinophils apoptosis in broncho alveloar lavage fluid and reduce the number of eosinophils in lung tissue in asthma.

Abstract:SOI technology is well suited for high speed digital circuits. However, its history effect due to the floating body poses a major challenge to analog circuits. This paper presents the design of a high-performance IO interface (FlexIOTM [5]) used in Cell Broadband EngineTM processors [1][4], currently in mass production. The design has been scaled across 90nm, 65nm, and 45nm SOI CMOS processes. The parallel interface implemented in 45nm SOI comprises 5 transmit and 5 receive bytes with each link unidirectionally running at 6.4Gb/s achieving BER ≪ 10-20. The paper discusses clocking, IO, and ESD aspects of the design in SOI processes.
Suspected Adverse Drug Reactions Associated With Leukotriene Receptor Antagonists Versus First Line Asthma Medications: A National Registry-Pharmacology Approach

Abstract:Aims. To determine the suspected adverse drug reaction (ADR) profile of leukotriene receptor antagonists (LTRAs: montelukast and zafirlukast) relative to first-line asthma medications short-acting beta agonists (SABA: salbutamol) and inhaled corticosteroid (ICH: beclomethasone) in the United Kingdom. To determine chemical and pharmacological rationale for the suspected ADR signals. Methods. Properties of the asthma medications (pharmacokinetics and pharmacology) were datamined from the chemical database of bioactive molecules with drug-like properties, European molecular Biology laboratory (ChEMBL). Suspected ADR profiles of the asthma medications was curated from the Medicines and Healthcare products Regulatory Authority (MHRA) Yellow Card interactive drug analysis profiles (iDAP) and concatenated to the standardised prescribing levels (Open Prescribing) between 2018-2023. Results. Total ADRs per 100,000 Rx (P < .001) and psychiatric system organ class (SOC) ADRs (P < .001) reached statistical significance. Montelukast exhibited the greatest ADR rate at 15.64 per 100,000 Rx. The low lipophilic ligand efficiency (LLE = 0.15) of montelukast relative to the controls may explain the promiscuity of interactions with off-target G-coupled protein receptors (GPCRs). This included the dopamine signalling axis, which in combination with bioaccumulation in the cerebrospinal fluid (CSF) to achieve Cmax beyond a typical dose can be ascribed to the psychiatric side effects observed. Cardiac ADRs did not reach statistical significance but inhibitory interaction of montelukast with the MAP kinase p38 alpha (a cardiac protective pathway) was identified as a potential rationale for montelukast withdrawal cardiac effects. Conclusion. Relative to the controls, montelukast displays a range of suspected system organ class level ADRs. For psychiatric ADR, montelukast is statistically significant (P < .001). A mechanistic hypothesis is proposed based on polypharmacological interactions in combination with CSF levels attained. This work further supports the close monitoring of montelukast for neuropsychiatric side effects.

Abstract:INTRODUCTION Erythropoietic protoporphyria was generally assumed to be an autosomal dominant disease with variable penetrance. The determination of the ferrochelatase activity and the biological molecular studies have shown that both autosomal dominant and recessive patterns of inheritance are possible. CASE REPORT Is reported the case of a 17 years-old male patient with erythropoietic protoporphyria and porokeratosis. There are some hepatic biochemical abnormalities without cholelithiasia and without pathological change of the liver biopsy. Leucocyte ferrochelatase activity is decreased to 5 p. 100 of the normal mean level. In both the parents, without photosensitivity, the enzyme activity is reduced to 40 p. 100 of the normal values. DISCUSSION The patients with severe ferrochelatase defect have no more important clinical manifestations than in the usual form of erythropoietic protoporphyria. For clarify the exact mode of inheritance, the determination of the ferrochelatase activity and the identification of the mutations in the patient and his parents are necessary. In our patient the porokeratosis should be in relation with the protoporphyrin induced phototoxic reaction which facilitate the emergence of a mutant cellular clone of epithelial cells.
Effects of montelukast on airway inflammation and remodeling in murine model of asthma

Abstract:Objective To investigate the effects of early treatment of montelukast on the airway inflammation and remodeling in murine asthma model.Methods Twenty-four mice were divided into 3 groups(8 in each group) including group A[ovalbumin(OVA) sensitized/challenged mice],group B(saline sensitized/challenged mice),group C(OVA sensitized/challenged mice treated with montelukast in the early phase).Mice were sensitized on day1 and day14 by OVA and challenged from days 24 to 41 by OVA repeatedly to establish a murine model of asthma characterized by airway inflammation and airway remodeling.Monterlukast(6 mg/kg) were given from the day before OVA challenge in group C.The differential counts of inflammatory cells in bronchoaveolar lavage fluids(BALF),peripheral blood(PB) as well as eosinophils(EOS) count in bone marrow(BM) were calculated.Airway inflammation indices,EOS count,amount of collagen deposition around the bronchus,area of airway smooth muscle,the degree of mucus secretion in the lumens and depth of airway smooth muscle(ASM) in bronchus of different grade were studied by HE,PAS and Masson′s staining.The supernatant collected from BALF,PB,and BM were assayed for IL-5.The BALF were also assayed for INF-γ(levels).Results In repeatedly OVA-challenged mice (group A),EOS counts in BALF,PB and BM [(45.09±4.08) ×10~4/mL,(2.41±0.31)×10~5/mL,(1.51±0.18)×10~6/femur] were increased significantly when compared with group B[(0.08±0.03)×10~4/mL,(0.62±0.09)×10~4/mL,(0.63±0.09)×10~6/femur,all P<0.01],respectively.The IL-5 levels in BALF and PB in group A increased significantly compared with group B [BALF:((57.86±)2.72) pg/mL vs (18.68±2.46) pg/mL,P<0.01;PB:(18.76±0.84) pg/mL vs(12.11±0.51) pg/mL,P<0.01] while IFN-γ in BALF decreased significantly [(36.55±1.71) pg/mL vs(64.53±5.72) pg/mL,P<0.01].EOS counts around bronchus were increased in group A [(912.12±65.97)/mm~2] when compared with group B[(8.79±2.61)/mm~2,P<0.01].Goblet cells hyperplasia [(51.12±3.50)% vs((1.92±)0.67)%,P<0.01 ]and mucus hypersecretion [score:(3.48±0.12) vs(0.17±0.08),P<0.01],airway smooth muscle hypertrophy [(25.99±0.89)μm~2/μm vs(13.06±0.60) μm~2/μm,(P<)0.01],enhanced collagen deposition of reticular basement membrane(RBM) [(29.69±2.02) μm~2/μm vs(5.56±0.35)μm~2/μm,P<0.01] were more significant in group A than those in group B.Treated with Montelukast early resulted in decreased EOS counts [(6.25±0.79)×10~4/mL in BALF,(0.71±0.11)×10~5/mL in PB and(0.77±0.10)×10~6/femur in BM],declined IL-5 levels [(25.96±1.79)pg/mL in BALF,(13.14±0.55) pg/mL in serum] and increased IFN-γ levels [(52.70±2.15)pg/mL in BALF].Montelukast also attenuated the remodeling procedure in the airway described above.Conclusions Prolonged allergen exposure could induce airway structural changes characteristic of excessive collagen deposition.Montelukast,an anti-(inflammatory) agent,has inhibitory activity on airway remodeling.

Abstract:Introduction . One of the antileukotriene drugs for the treatment of bronchial asthma and allergic rhinitis with and without polyposis is montelukast. The presented article presents the results of the analysis of the effectiveness, safety and impact on the quality of life of patients, the use of the singular, in various forms of allergic rhinitis and associated comorbid conditions. Aim of the study . To assess the impact on the quality of life of patients, the effectiveness and safety of the use of montelukast in the complex therapy of patients with allergic rhinitis and comorbid diseases. Materials and methods. A simple, blind, randomized, controlled clinical trial involved 97 patients divided into 4 groups: patients with moderate allergic rhinitis; allergic rhinitis of moderate severity and bronchial asthma; severe allergic rhinitis and bronchial asthma; severe allergic rhinitis, bronchial asthma and polypous rhinosinusitis. In parallel with the comparison of drug tolerability, safety and clinical symptoms (rhinorrhea, nasal congestion, itching, sneezing, bronchopulmonary manifestations), the quality of life was assessed using a special SNOT-22 questionnaire. Results . The use of the antileukotriene drug montelukast in the complex therapy of allergic rhinitis and related comorbid conditions significantly improved the clinical symptoms and quality of life of patients over the entire period of treatment in all groups. There was an increase in the suppression of leukotriene-mediated effects when using montelukast, more in the group of people with allergic rhinitis and bronchial asthma, such as symptoms of the allergic triad, mucus hypersecretion, bronchospasm, eosinophilia, increased vascular permeability, etc. Conclusion . In the treatment of patients with allergic rhinitis and morbid conditions, Singular has demonstrated an inhibitory effect on cysteinyl leukotrienes, high bioavailability, good tolerability and safety.

Recent advances in the management of asthma using leukotriene modifiers

Abstract:Asthma is a chronic inflammatory disease of the airways that affects approximately 100 million people worldwide. In order to reduce symptoms, improve pulmonary function, and decrease morbidity, current treatment guidelines emphasize the importance of controlling the underlying inflammation in patients with asthma. Leukotrienes are leukocyte-generated lipid mediators that promote airway inflammation. Recognition of the importance of leukotrienes in the pathogenesis of asthma has led to the development of leukotriene modifiers, the first new class of drugs for the treatment of asthma to become available in 25 years. Controlled clinical trials with the four currently used leukotriene modifiers (montelukast, zafirlukast, and zileuton in the US and pranlukast in Japan) have established their efficacy in improving pulmonary function, reducing symptoms, decreasing night-time awakenings, and decreasing the need for rescue medications. They exert anti-inflammatory effects that attenuate cellular infiltration and bronchial hyperresponsiveness and complement the anti-inflammatory properties of inhaled corticosteroids. In patients with moderate and severe asthma, they permit tapering of the corticosteroid dose. In patients with exercise-induced asthma, leukotriene modifiers limit the decline in and quicken the recovery of pulmonary functions without the tolerance issues seen with chronic long-acting beta(2)-adrenoceptor agonist use. In patients with aspirin (acetylsalicylic acid)-induced asthma, they improve pulmonary function and shift the dose response curve to the right, reducing the patient's response to aspirin. In patients with seasonal allergic rhinitis, with or without concomitant asthma, they improve nasal, eye, and throat symptoms as well as quality of life. Leukotriene modifiers are generally safe and well tolerated with adverse effect profiles similar to that of placebo. The one safety issue raised with leukotriene modifiers, Churg-Strauss Syndrome, appears to be the unmasking of an already present syndrome that is manifested when the leukotriene modifiers permit corticosteroid doses to be reduced. Although current treatment guidelines recommend their use in patients with mild persistent asthma, these guidelines were developed just as leukotriene modifiers were coming to the market, before much of the clinical efficacy data were published. Because asthma is a heterogeneous disease, the different asthma phenotypes respond differently to therapies; consequently asthma therapy needs to be individualized. Leukotriene modifiers increase the therapeutic options for patients with asthma and, based on recent data, it is expected that future guidelines will describe expanded uses for these agents in clinical circumstances where these drugs are effective.

Abstract:The wingless/integrase‐1 (WNT) pathway involved in the pathogenesis of inflammatory airway diseases has recently generated considerable research interest. Montelukast, a leukotriene receptor antagonist, provides therapeutic benefits in allergic asthma involving eosinophils. We aimed to investigate the role of the WNT pathway in the therapeutic actions of montelukast (MT) in a mixed type of allergic‐acute airway inflammation model induced by ovalbumin (OVA) and lipopolysaccharide (LPS) in mice. Female mice were sensitized with intraperitoneal OVA‐Al(OH)3 administration in the initiation phase and intranasal OVA followed by LPS administration in the challenge phase. The mice were divided into eight groups: control, asthmatic, and control/asthmatic treated with XAV939 (inhibitor of the canonical WNT pathway), LGK‐974 (inhibitor of the secretion of WNT ligands), or MT at different doses. The inhibition of the WNT pathway prevented tracheal 5‐HT and bradykinin hyperreactivity, while only the inhibition of the canonical WNT pathway partially reduced 5‐HT and bradykinin contractions compared to the inflammation group. Therefore, MT treatment hindered 5‐HT and bradykinin hyperreactivity associated with airway inflammation. Furthermore, MT prevented the increases in the phosphorylated GSK‐3β and WNT5A levels, which had been induced by airway inflammation, in a dose‐dependent manner. Conversely, the MT application caused a further increase in the fibronectin levels, while there was no significant alteration in the phosphorylation of the Smad‐2 levels in the isolated lungs of the mice. The MT treatment reversed the increase in the mRNA expression levels of interleukin‐17A. An increase in eosinophil and neutrophil counts was observed in bronchoalveolar lavage fluid samples obtained from the mice in the inflammation group, which was hampered by the MT treatment. The inhibition of the WNT pathway did not alter inflammatory cytokine expression or cell infiltration. The WNT pathway mediated the therapeutic effects of MT due to the inhibition of GSK‐3β phosphorylation as well as the reduction of WNT5A levels in a murine airway inflammation model.

The synergistic effects of clopidogrel with montelukast may be beneficial for asthma treatment

Abstract:Platelets modulate asthma pathogenesis by forming the platelet-eosinophil aggregation (PEA), which facilitates the activation of eosinophils. Platelets exhibit the purinergic receptor (P2Y12R), which responds to cysteinyl leukotriene E4 (LTE4 ). We have suggested that the combination of an antiplatelet drug (clopidogrel, [Clo]) and montelukast (Mon) would synergistically suppress asthma. BALB/c mice were intraperitoneally sensitized with ovalbumin (OVA) on days 0 and 14 and subsequently challenged on days 28-30 and 42-44. Mice were administered with Clo (10 mg/kg), Mon (10 mg/kg) or both drugs (Clo/Mon) orally 30 minutes before the OVA (1%) challenge on days 42-44. Mice were assayed for airway hyper-responsiveness (AHR) to methacholine and airway inflammation. Clopidogrel and montelukast attenuated the increased AHR; the combined treatment was more effective than a single treatment for total and eosinophil counts (all P < 0.05). Levels of interleukin (IL)-4, IL-5, IL-13, platelet factor 4, eosinophil peroxidase and LTE4 increased in the bronchoalveolar lavage fluid of asthmatic mice, but these levels decreased in mice treated with Clo/Mon (all P < 0.05). Goblet cell hyperplasia decreased in response to Clo/Mon. Mouse platelets and eosinophils were isolated and co-cultured for an in vitro assay with 10 µmol/L adenosine diphosphate (ADP), LTE4 (200 nmol/L), Mon (1 µmol/L), Clo (1 µmol/L) and Clo/Mon (1 µmol/L). Flow cytometry revealed that the increased formation of the PEA (%) was fully mediated by ADP and partly mediated by LTE4 . Clo/Mon reduced ADP-induced PEA formation and P-selectin expression (P < 0.05). In conclusion, Clo/Mon synergistically relieved asthma by inhibiting ADP-mediated PEA formation.
The Efficacy of Montelukast on the Airway Hyperresponsiveness in Children with Asthmatic Bron-chitis

Abstract:Objective To investigate the effect of montelukast on the airway hyperresponsiveness in children with asthmatic bronchitis.Methods Total of 76 children with asthmatic bronchitis admitted to Shanghai Jiaotong University Affiliated Xinhua Hospital from Dec .2013 to Mar.2015 were included in the study,and divided into control group and observation group based on a random number table,38 cases each. The control group was treated with conventional therapy,while the observation group was added with monte-lukast treatment,once per day,4.0 mg per time.The treatment course of both groups was one month.The therapeutic effect of the two groups was compared.Results After treatment,total effective rate of the obser-vation group was higher than the control group [ 97.4%( 37/38 ) vs 84.2%( 32/38 ) ] , the difference was statistically significant ( P <0.05 ) .The asthma, wheeze, cough disappearing time and hospital stay of the observation group were less than the control group [(2.1 ±0.4) d vs (4.4 ±1.2) d,(2.2 ±0.8) d vs (5.2 ±1.8) d,(4.1 ±1.2) d vs (6.2 ±1.8) d,(4.2 ±1.4) d vs (6.8 ±1.3) d],the differences were statistically significant(P<0.01).After treatment,ECP,LTE4,IL-10 of the observation group were lower than the control group [(9.6 ±1.2) μg/L vs (15.5 ±2.8) μg/L,(38 ±8) pg/L vs (80 ±9) pg/L, (9.3 ±1.4) pg/L vs (14.3 ±2.0) pg/L],IL-12 was higher than the control group[(58 ±10) pg/L vs (42 ±8) pg/L],the differences were statistically significant(P<0.05).Conclusion Montelukast sodium has a significant effect for asthmatic bronchitis in children ,which can improve children′s airway hyperrespon-siveness,prevent the further development of the disease to a certain degree,and help improve the quality of life of the children,thus is worthy of the clinical application.
