Abstract:Abstract Background: The dyshomeostasis of trace elements is associated with multiple organ dysfunction in patients with septic shock. However, it remains unclear whether the change of whole blood copper (Cu) and zinc (Zn) levels influence cardiac function in patients with septic shock. Here, we sought to explore relationship between whole blood Cu and Zn and cardiac dysfunction in septic shock. Methods: Between April 2018 and March 2020, septic shock patients with sepsis-induced left ventricular systolic dysfunction (SILVSD, left ventricular ejection fraction, LVEF ＜ 50%) and with no sepsis-induced myocardial dysfunction (non-SIMD, septic shock alone and LVEF＞ 50%) on an intensive care unit (ICU) in south China and healthy controls were prospectively enrolled. whole blood Cu and Zn levels were measured using flame atomic absorption spectrophotometry.Results: 86 patients with septic shock including 41 SILVSD and 45 non-SIMD and 25 healthy controls were studied. Whole blood Cu levels were significantly higher and Zn were significantly lower in SILVSD compared with non-SIMD and controls [Cu, (16.34±1.93) vs. (15.23±2.07) vs. (14.02±1.65) µmol/L, p = 0.009, < 0.001; Zn, (78.45±12.18) vs. (85.07±14.80) vs. (94.90±14.78) µmol/L, p = 0.029, < 0.001]. Both increased whole blood Cu and reduced Zn were associated with lower LVEF ( all p < 0.001) and higher amino-terminal pro-B-type natriuretic peptide (NT-proBNP) (Cu, p = 0.002, Zn, p = 0.001), and had predictive values for SILVSD [Cu, (AUC = 0.666; p = 0.005) ; Zn, (AUC = 0.625; p = 0.039)]. Whole blood Cu levels were increased but Zn were reduced in non-survivors compared with survivors[Cu, (17.20 ±2.25) vs. (14.99 ±1.49) µmol/L, p < 0.001; Zn, (71.17 ±11.98) vs. (87.67±11.30) µmol/L, p < 0.001],. Whole blood Cu and Zn displayed the value of predicting 28-day mortality [Cu (AUC = 0.802, p < 0.001); Zn (AUC = 0.869, p < 0.001)].Conclusions: Whole blood Cu levels were increased in SILVSD patients and positively correlated with the cardiac dysfunction while Whole blood Zn were reduced and negatively associated with the cardiac dysfunction. Moreover, both whole blood Cu and Zn could distinguish between SILVSD and non-SIMD in septic shock patients and predict 28-day mortality.Trial registration: ChiCTR1800015709. Abstract:Purpose: This study aimed to explore relationships between whole blood copper (Cu), zinc (Zn) and Cu/Zn ratio and cardiac dysfunction in patients with septic shock. Conclusion: Findings of the study suggest that whole blood Cu levels and Cu/Zn ratio are increased in SILVSD patients and positively correlated with cardiac dysfunction, while whole blood Zn levels are reduced and negatively associated with cardiac dysfunction. Moreover, both whole blood Cu, Zn and Cu/Zn ratio might distinguish between SILVSD and non-SIMD in septic shock patients and predict 28-day mortality.

Abstract:Left ventricular ejection fraction (LVEF) is inconsistently associated with poor outcomes in patients with sepsis. Newer parameters such as LV longitudinal strain (LVLS), mitral annular plane systolic excursion (MAPSE) and LV longitudinal wall fractional shortening (LV-LWFS) may be more sensitive indicators of LV dysfunction, but are sparsely investigated. Abstract:BackgroundLeft ventricular diastolic dysfunction is common in patients with severe sepsis or septic shock, but the best approach to categorization is unknown. We assessed the association of common measures of diastolic function with clinical outcomes and tested the utility of a simplified definition of diastolic dysfunction against the American Society of Echocardiography (ASE) 2009 definition.MethodsIn this prospective observational study, patients with severe sepsis or septic shock underwent transthoracic echocardiography within 24 h of onset of sepsis (median 4.3 h). We measured echocardiographic parameters of diastolic function and used random forest analysis to assess their association with clinical outcomes (28-day mortality and ICU-free days to day 28) and thereby suggest a simplified definition. We then compared patients categorized by the ASE 2009 definition and our simplified definition.ResultsWe studied 167 patients. The ASE 2009 definition categorized only 35 % of patients. Random forest analysis demonstrated that the left atrial volume index and deceleration time, central to the ASE 2009 definition, were not associated with clinical outcomes. Our simplified definition used only e′ and E/e′, omitting the other measurements. The simplified definition categorized 87 % of patients. Patients categorized by either ASE 2009 or our novel definition had similar clinical outcomes. In both definitions, worsened diastolic function was associated with increased prevalence of ischemic heart disease, diabetes, and hypertension.ConclusionsA novel, simplified definition of diastolic dysfunction categorized more patients with sepsis than ASE 2009 definition. Patients categorized according to the simplified definition did not differ from patients categorized according to the ASE 2009 definition in respect to clinical outcome or comorbidities.

Abstract:Objective To investigate the dynamic change of cardiac function in the patients of septic shock and analyze its influence to prognosis.Methods A total of 129 patients in department of emergency medicine,Beijing Chao-Yang Hospital,Capital Medical University,from January 2014 to January 2016 were enrolled in the investigation using mornitoring the hemodynamics including the contour pulse wave mean artery pressure (MAP),cardiac index (CI) and systemic vascular resistance indes (SVRI).Echocardiography was done 7 days laterenrollment of left ventricular eject fraction (LVEF),E and A peak of mitral blood flow velocity,mitral annulus moving speed (E') were detected changes.E/A and E/E'were calculated.According to the 28-day prognosis,all patients were devided into the survival and death group for detecting the risk factors of death.Results In all patients,hemodynamics detected changes showed the features of septic shock with high output and low vascular risistance.The values of CI in the survival group and death group were (4.30±0.71) L/(min.m2) and (4.52±0.91) L/(min· m2),repectively,and the values of SVRI were (1 477±297) dyn·s · cm-5m2 and (1 488±233) dyn·s · cm-5·m2,repectively.There were no significant differences in CI and SVRI were found between the two groups (both P＞0.05).When 7 days after admission in hospital,CI [(3.88±0.51) L/(min· m2)] was lower and SVRI [(2 044±266) dyn·s · cm-5·m2] was higher compared with those at admission in the survival group with significant differences,between the two intervals (all P＜0.05).In the death group,CI decreased to (2.52±0.87) L/(min · m2) and SVRI increased to (3 201±329) dyn·s · cm5·m2 after admission on the 7th day in hospital,significant differences between the two groups (all P＜0.01).There were significantly differenced with LVEF [(69.1±12.5)％ vs.(69.5±11.2)％] and E/E'(8.43±0.59) vs.(8.89±0.64) found between the two groups on the first day in hospital (all P＞0.05).Compared with survival group,LVEF significantly decreased (64.4％±16.3)％ vs.(54.4±17.6)％ and E/E'(8.73±0.67) vs.(9.97±0.55),all P＜0.01] was significantly decreased in the death group.APACHE Ⅱ (OR=1.667,95％CI:1.322-1.863,P=0.001),LVEF (OR=0.809,95％CI:0.612-0.912,P=0.001) and E/E'(OR=1.219,95％CI:1.030-1.501,P=0.006) were detected to be the risk factors by logistic analysis.Conclusions In septic shock patients,both the systolic,and the diastolic cardiac function were found to be significantly abnormal,resulting poor prognosis.
Subclinical Left Ventricular Systolic Dysfunction in Patients with Septic Shock Based on Sepsis-3 Definition: A Speckle-Tracking Echocardiography Study

Abstract:Introduction . Left ventricular dysfunction is quite common in septic shock. Speckle-tracking echocardiography (STE) is a novel, highly sensitive method for assessing left ventricular function, capable of detecting subclinical myocardial dysfunction, which is not identified with conventional echocardiography. We sought to evaluate subclinical left ventricular systolic function in patients with septic shock using speckle-tracking echocardiography. Methods . From May 2017 to December 2018, patients aged ≥18 years admitted to the intensive care unit with the diagnosis of sepsis and septic shock based on the sepsis-3 definition were included. Patients with other causes of cardiac dysfunction were excluded. Transthoracic echocardiography was performed for all the patients within 24 hours of diagnosis. Left ventricular systolic function was assessed using conventional echocardiography and speckle-tracking echocardiography. Results . Patients with septic shock ( n = 90) (study group) and 37 matched patients with sepsis but no septic shock (control group) were included. Left ventricular ejection fraction (LVEF) by conventional echocardiography showed no significant difference between two groups (58.2 ± 9.9 vs. 58.6 ± 8.3, p = 0.804 ). The global longitudinal strain (GLS) by STE was significantly reduced in patients with septic shock compared with that in the control (−14.6 ± 3.3 vs. −17.1 ± 3.3, p < 0.001 ). Based on the cutoff value of GLS ≥ −15% for the definition of subclinical left ventricular systolic dysfunction, this dysfunction was detected in 50 patients with septic shock (55.6%) and in 6 patients in the control group (16.2%) ( p < 0.05 ). Conclusions . Speckle-tracking echocardiography can detect early subclinical left ventricular systolic dysfunction via the left ventricular global longitudinal strain compared with conventional echocardiographic parameters in patients with septic shock.

Prevalence and Correlates of Left Ventricular Diastolic Dysfunction in A Community-Based Elderly Female Population in China

Abstract:Purpose: Left ventricular diastolic dysfunction (LVDD) is proposed as essential diagnostic index of heart failure with preserved ejection fraction. We aimed to investigate the prevalence and correlates of LVDD in a community-based elderly female population in China. Methods: This cross-sectional survey was performed in 1274 community residents (769 women, aged ≥ 65 years old) who participated in the Shanghai Heart Health Study. Echocardiography was used to evaluate diastolic function with conventional and tissue Doppler imaging (TDI) techniques. Data including demographic parameters, diseases history, blood and urine parameters were obtained to analyze correlates of LVDD. Results: The overall prevalence of LVDD was 39.56%, and increased with age in females. The elderly female residents had higher prevalence of LVDD than male residents (43.56% vs. 33.47%, P < 0.001). In elderly female residents, the waist circumference [odds ratio (OR) 1.039, 95% confidence interval (CI) 1.007-1.073], atrial fibrillation (OR 3.744, 95% CI 1.084-12.937), N-terminal pro-B type natriuretic peptide (NT-proBNP) (OR 2.217, 95% CI 1.305-3.766) and heart rate (OR 0.979, 95% CI 0.966-0.992) were independent correlates of LVDD after adjustment of other confounders, while age (OR 1.093, 95% CI 1.033-1.157), serum urea (OR 1.276, 95% CI 1.072-1.519), urinary albumin to creatinine ratio (UACR) (OR 1.934, 95% CI 1.097-3.408) and low density lipoprotein cholesterol (LDL-C) (OR 0.713, 95% CI 0.546-0.932) were determinants associated with the severity of LVDD. Moreover, the proportion of severe LVDD in the LVDD female subjects significantly increased (P < 0.001) while mild LVDD significantly decreased (P < 0.001) with aging. The level of NT-proBNP elevated with aging regardless of the left ventricular diastolic function status. Conclusions: LVDD is common in this community elderly population, and waist circumference, atrial fibrillation, NT-proBNP and heart rate were independent correlates of LVDD in elderly female residents, providing insight on possible underlying mechanisms for LVDD in elderly population.

Abstract:Abstract Introduction Right ventricle (RV) dilation in combination with elevated central venous pressure (CVP), which is a state of RV congestion, is seen as a sign of RV failure (RVF). On the other hand, RV systolic function is usually assessed by tricuspid annular plane systolic excursion (TAPSE) and fractional area change (FAC). This study aimed to investigate the prevalence and prognostic value of RVF and RV systolic dysfunction (RVSD) in septic patients. Methods Mechanically ventilated sepsis and septic shock patients were included. We collected haemodynamic and echocardiographic parameters as well as prognostic information including mechanical ventilation duration, length of ICU stay and 30-day mortality. RVF was defined as a right and left ventricular end-diastolic area ratio ≥ 0.6 in combination with CVP ≥ 8 mmHg. RVSD was defined as TAPSE < 16 mm or FAC < 35%. Results A total of 215 patients were enrolled in this study, and the patients were divided into 4 groups: patients with normal RV function (normal, n = 101), patients with RVF but without RVSD (RVF only, n = 38), patients with RVSD but without RVF (RVSD only, n = 44), and patients with combined RVF–RVSD (RVF/RVSD, n = 32). The RVF/RVSD group and RVSD only group had a lower cardiac index than the RVF only group and normal groups ( p < 0.05). At 30 days after ICU admission, 50.0% of patients had died in the RVF/RVSD group, which was much higher than the mortality in the RVF only group (13.2%) and normal group (13.9%) ( p < 0.05). In a Cox regression analysis, the presence of RVF/RVSD was independently associated with 30-day mortality (HR 3.004, 95% CI:1.370–6.587, p = 0.006). In contrast, neither the presence of RVF only nor the presence of RVSD only was associated with 30-day mortality (HR 0.951, 95% CI:0.305–2.960, p = 0.931; HR 1.912, 95% CI:0.853–4.287, p = 0.116, respectively). Conclusion The presence of combined RVF–RVSD was associated with 30-day mortality in mechanically ventilated septic patients. Additional studies are needed to confirm and expand this finding.

Abstract:Objective This study investigates the prevalence and prognostic impact of diastolic dysfunction (DD) in patients hospitalized with heart failure (HF) with mildly reduced ejection fraction (HFmrEF) in sinus rhythm. Background Data regarding the prognostic impact of DD in patients with HFmrEF is limited. Methods From 2016 to 2022, all patients hospitalized with HFmrEF (i.e., left ventricular ejection fraction 41–49% and signs and/or symptoms of HF) were retrospectively included at one institution. Patients with DD were compared to patients without (i.e., non-DD), further risk stratification was performed according to the severity of DD. The primary endpoint was all-cause mortality at 30 months (interquartile range (IQR) 15–61 months), key secondary endpoint was rehospitalization for worsening HF. Results From a total of 1154 patients (median age 68 years, 68% males) hospitalized with HFmrEF, concomitant DD was present in 72% (grade I: 56%, grade II: 14%, grade III: 2%). Patients with DD were older (71 years vs.65 years; p = 0.001) and presented with higher rates of cardiovascular comorbidities. The presence of DD was not associated with the risk of long-term all-cause mortality (adjusted HR = 0.815; 95% CI 0.612–1.085; p = 0.161) or HF-related rehospitalization (adjusted HR = 0.736; 95% CI 0.442–1.225; p = 0.238). Furthermore, the outcome did not differ in patients with more advanced stages of DD. Conclusion DD is commonly prevalent in patients with HFmrEF, but not associated with long-term prognosis.

Prevalence and outcome of diastolic dysfunction in children with fluid refractory septic shock--a prospective observational study

Abstract:Objectives: Our primary objective was to determine the prevalence and outcome of diastolic dysfunction in children with fluid refractory septic shock. The secondary objective was to determine possible early predictors of diastolic dysfunction. Design: Prospective observational study. Setting: PICU of a tertiary care teaching hospital. Patients: Consecutive children 17 years old or younger with fluid refractory septic shock and not on mechanical ventilation admitted to our ICU from June 2011 to August 2012 were included. Survivors were followed up till 1 year of discharge (July 2013). Interventions: Children were subjected to 2D echocardiography and qualitative cardiac troponin-T test within the first 6 hours of admission. Measurements and main results: A total of 56 children were included. Median age was 7 years (interquartile range, 1.5, 14) and majority (52%) were males. Most common underlying diagnoses were meningitis and pneumonia. The prevalence of diastolic dysfunction was 41.1% (95% CI, 27.8-54.4), and mortality rate was 43% in those with diastolic dysfunction. At 1-year follow-up, residual dysfunction was present in only one of 11 of the survivors (11%). On univariable analysis of possible early predictors of diastolic dysfunction, we observed that these children tended to have higher mean central venous pressure (13 vs 6; p < 0.0001) and greater positivity for cardiac troponin-T (70% vs 36%; p = 0.01) compared with others. Although factors such as duration of illness and diastolic blood pressure were also lower in children with diastolic dysfunction compared with others, the difference was not statistically significant. On multivariable analysis, only the variable central venous pressure remained significant (adjusted odds ratio, 1.6; 95% CI, 1.12-2.14; p = 0.008). Conclusions: Diastolic dysfunction is common in children with fluid refractory septic shock, and immediate outcomes may be poorer in such patients. Increased central venous pressure after initial fluid resuscitation may be an early indicator of diastolic dysfunction and warrant urgent bedside echocardiography to guide further management.

Abstract:‐LV diastolic dysfunction is common in critically ill patients and is associated with poor outcomes. ‐Echocardiography is used to assess LV diastolic function and estimate LV filling pressures. ‐Recent American Society of Echocardiography/European Association of Cardiovascular Imaging guidelines do not account for the specificities of the critically ill patient and are not validated in this population. ‐LV and left atrial speckle tracking indices and artificial intelligence may provide better insight into the classification of DD in the critically ill. Myocardial dysfunction is common in patients admitted to the intensive care unit (ICU). Septic disease frequently results in cardiac dysfunction, and sepsis represents the most common cause of admission and death in the ICU. The association between left ventricular (LV) systolic dysfunction and mortality is not clear for critically ill patients. Conversely, LV diastolic dysfunction (DD) seems increasingly recognized as a factor associated with poor outcomes, not only in sepsis but also more generally in critically ill patients. Despite recent attempts to simplify the diagnosis and grading of DD, this remains relatively complex, with the need to use several echocardiographic parameters. Furthermore, the current guidelines have several intrinsic limitations when applied to the ICU setting. In this manuscript, we discuss the challenges in DD classification when applied to critically ill patients, the importance of left atrial pressure estimates for the management of patients in ICU, and whether the study of cardiac dysfunction spectrum during critical illness may benefit from the integration of left ventricular and left atrial strain data to improve diagnostic accuracy and implications for the treatment and prognosis.

Association of Biochemical Parameters of Renal Function with Left Ventricular Diastolic Dysfunction in a Community-Based Elderly Population in China

Abstract:Purpose: Relationship of left ventricular diastolic dysfunction (LVDD) with parameters that could provide more information than hemodynamic renal indexes has not been clarified. We aimed to explore the association of comprehensive renal parameters with LVDD in a community-based elderly population. Methods: 1166 community residents (aged ≥ 65 years, 694 females) participating the Shanghai Heart Health Study with complete data of renal parameters were investigated. Echocardiography was used to evaluate diastolic function with conventional and tissue Doppler imaging (TDI) techniques. Serum urea, creatinine, urea-to-creatinine ratio, estimated glomerular filtration rate (eGFR), and urinary albumin-to-creatinine ratio (UACR) were analyzed on their association with LVDD. Results: LVDD were diagnosed in 464 (39.8%) subjects out of the 1166 participants. The prevalence of LVDD increased in proportion to increasing values of serum urea (P = 0.038), urea-to-creatinine ratio (P = 0.005) and UACR (P = 0.029), and these three parameters were also found correalted with LVDD in logistic univariate regression analysis. Serum urea, urea-to-creatinine ratio and UACR were negatively correlated to peak early (E) to late (A) diastolic velocities ratio (E/A), and positively correlated to left atrial volume index (LAVI); UACR was also positively correlated to E to peak early (E') diastolic mitral annular velocity ratio (E/E'). After adjustment for confounders, urea-to-creatinine ratio was remianed independently correlated to LVDD [Odds ratio (OR) 2.82, 95% confidence interval (CI) 1.34-5.95, P = 0.006). Serum urea (OR 1.18, 95% CI 1.03-1.34), creatinine (OR 6.53, 95% CI 1.70-25.02), eGFR (OR 0.22, 95% CI 0.07-0.65) and UACR (OR 2.15, 95% CI 1.42-4.24) were all found independent correlates of advanced LVDD. Conclusions: Biochemical parameters of renal function were closely linked with LVDD. This finding described new cardio-renal relationship in the elderly population.
The Value of Serum N-terminal Pro Brain Natriuretic Peptide in Predicting Left Ventricular Diastolic Dysfunction in Septic Shock Patients

Abstract:Objective To investigate the predictive value of serum N-terminal pro brain natriuretic peptide NT-proBNP in left ventricular diastolic dysfunction(LVDD) in septic shock patients.Methods According to retrospective analysis of clinical data 96 patients with septic shock were divided into LVDD group and non-LVDD group.General clinical data,APACHE scores,NT-proBNP,Troponin I(TNI),creatinine,procalcitonin,D-dimer and lactic acid within the first 24 hours after admission were recorded,and multi-factors logistic regression analysis was conducted to find independent risk factors for left ventricular diastolic dysfunction in septic shock patients Receiver operating characteristic curve ROC was constructed to indicate the predictive value.Results There were no significant differences in general clinical data,procalcitonin,D-dimer and lactic acid between two groups.Compared with non-LVDD group the levels of NT-proBNP [lgNT-proBNP (3.66±0.38) vs.(3.03±0.59),P =0.000],TNI [lgTNI(-1.45±0.86)vs.(-2.36±0.82),P ＜ 0.01] and creatinine [(186.12±124.24)vs.(101.16±57.01),P ＜ 0.01] in LVDD group were significantly higher.It was shown by multi-factors logistic regression analysis that NT-proBNP (OR=8.731,95％CI;1.541-49.466,P=0.014) was an independent risk factor for left ventricular diastolic dysfunction in septic shock patients.The area under the curve AUC of NT-proBNP was 0.813 with the cut-off value of 1 725 pg/μL,sensibility =88.6％,specificity =62.1％.Conclusion NT-proBNP was avaluable indicator in predicting left ventricular diastolic dysfunction in septic shock patients.

Full and simplified assessment of left ventricular diastolic function in covid‐19 patients admitted to ICU: Feasibility, incidence, and association with mortality

Abstract:Purpose Left ventricular diastolic dysfunction (LVDD) is associated with poor outcomes in the intensive care unit (ICU). Nonetheless, precise reporting of LVDD in COVID‐19 patients is currently lacking and assessment could be challenging. Methods We performed an echocardiography study in COVID‐19 patients admitted to ICU with the aim to describe the feasibility of full or simplified LVDD assessment and its incidence. We also evaluated the association of LVDD or of single echocardiographic parameters with hospital mortality. Results Between 06.10.2020 and 18.02.2021, full diastolic assessment was feasible in 74% (n = 26/35) of patients receiving a full echocardiogram study. LVDD incidence was 46% (n = 12/26), while the simplified assessment produced different results (incidence 81%, n = 21/26). Nine patients with normal function on full assessment had LVDD with simplified criteria (grade I = 2; grade II = 3; grade III = 4). Nine patients were hospital‐survivors (39%); the incidence of LVDD (full assessment) was not different between survivors (n = 2/9, 22%) and non‐survivors (n = 10/17, 59%; p = .11). The E/e' ratio lateral was lower in survivors (7.4 [3.6] vs. non‐survivors 10.5 [6.3], p = .03). We also found that s' wave was higher in survivors (average, p = .01). Conclusion In a small single‐center study, assessment of LVDD according to the latest guidelines was feasible in three quarters of COVID‐19 patients. Non‐survivors showed a trend toward greater LVDD incidence; moreover, they had significantly worse s' values (all) and higher E/e' ratio (lateral).
Prognostic value of right ventricular free wall strain in patients with sepsis

Abstract:Background: Right ventricular systolic dysfunction (RVSD) in patients with sepsis is an area of growing interest, but its prognostic significance remains unclear and additional tools are needed to improve our understanding. Right ventricular free wall strain (RV-FWS) is a relatively new parameter to assess RV function. This study aimed to investigate the potential correlation between impaired RV-FWS and prognostic outcomes in patients with sepsis. Methods: We prospectively assessed right ventricular function in patients with sepsis within the initial 24 h of their hospital admission. RV-FWS, right ventricular global strain (RV-GS), fractional area change (FAC), and tricuspid annular plane systolic excursion (TAPSE) were examined. RVSD was defined as impaired RV-FWS. Moreover, the association between RVSD and 30-day mortality rate was assessed. Results: This study included 89 patients. Among them, 27 (30.3%) succumbed to their illness within 30 days. The nonsurviving patients demonstrated significantly lower absolute RV-FWS (−19.7% ± 2.4% vs. −21.1% ± 2.1%, P = 0.008) and RV-GS (−17.7% ± 1.2% vs. −18.4% ± 1.4%, P = 0.032) values than the surviving patients. However, TAPSE and FAC values were not significantly different between the two groups. The optimal cutoff values for RV-FWS, RV-GS, FAC, and TAPSE were −19.0%, −17.9%, 36.5%, and 1.55 cm, respectively. Kaplan–Meier survival curves revealed that patients with impaired RV-FWS and RV-GS demonstrated lower 30-day survival rates, and the predictive performance of RV-FWS (hazard ratio [HR]: 3.97, 95% confidence interval [CI]: 1.85–8.51, P < 0.001) was slightly higher than FAC and TAPSE. However, multivariable Cox regression analysis revealed no association between impaired RV-FWS and mortality outcomes (HR: 1.85, 95% CI: 0.56–6.14, P = 0.316). Conclusions: Impaired RV-FWS is not associated with short-term mortality outcomes, and RV strain imaging is of limited value in assessing the prognosis of sepsis.

Abstract:OBJECTIVE To investigate the clinical characteristics and risk factors of gastrointestinal related acute abdomen complicated with septic shock .METHODS A total of 82 patients with gastrointestinal related acute abdo-men complicated with septic shock from May 2009 to Dec .2014 were selected .The clinical manifestations and signs of septic shock ,the relationship between the type of pathogenic bacteria and the severity of septic shock , prognosis of surgical and nonoperative treatment in patients and the risk factors for septic shock were observed . RESULTS The main clinical manifestations were apathy ,irritability and pale limbs jueleng ,the main signs were hypotension ,slow heart rate and low urine output ,and body temperature was not the main criteria for determining septic shock .The patients of gram-negative bacteria infection accounted for 64 .63% .The proportion of slight sep-tic shock patients with gram-positive infection was 65 .52% ,the proportion of slight septic shock patients with gram-negative infection was 15 .09% ,and the difference was significant (P<0 .05) .The proportion of severe sep-tic shock patients with gram-positive infection was 13 .79% ,the proportion of severe septic shock patients with gram-negative infection was 60 .38% ,and the difference was significant (P<0 .05) .The total cure rate of surgical treatment was 85 .29% ,the total cure rate of conservative treatment was 35 .71% ,and the difference was signifi-cant (P<0 .05) .Age ,types of primary disease ,duration of disease ,number of organ failure ,chronic diseases history and treatment time were risk factors for septic shock (P<0 .05) .CONCLUSION In the diagnosis and treat-ment of patients with gastrointestinal related acute abdomen complicated with septic shock should pay close atten-tion to the related risk factors and typical clinical manifestations and signs .Once diagnosed ,early anti-shock ,sur-gery and anti infection treatment are necessary .

Abstract:Background This study aimed to investigate the ventricular mechanical relaxation pattern and its clinical influence in patients with ST-segment elevation myocardial infarction (STEMI). Methods Echocardiography was performed to measure mitral and tricuspid diastolic opening times. Left ventricular diastolic mechanical delay (LVMDd) was defined as diastolic filling of the right ventricle earlier than that of the left ventricle, and right ventricular diastolic mechanical delay (RVMDd) was defined as the right ventricular diastolic filling later than left ventricular filling. Results Among 152 patients with STEMI, 100 (65.8%) had LVMDd, and 47 (30.9%) had RVMDd. In-hospital complications were significantly increased in patients with RVMDd (61.6% vs. 41.0%, P = 0.017). Those with RVMDd exhibited significantly lower left ventricular global longitudinal strain (11.7 ± 4.1% vs. 13.2 ± 4.0%, P = 0.035), global work index (913.8 ± 365.9 vs. 1098.9 ± 358.8 mmHg%, P = 0.005) and global constructive work (1218.6 ± 392.8 vs. 1393.7 ± 432.7 mmHg%, P = 0.021). Mitral deceleration time significantly decreased (127.4 ± 33.5 vs. 145.6 ± 41.7 ms, P = 0.012), and the ratio of early mitral inflow to early mitral annular velocity (E/E’) significantly increased [13.0(11.0–20.0) vs. 11.9(9.3–14.3), P = 0.006] in the RVMDd group. Logistic regression analysis showed that age (odds ratio [OR]:0.920; P = 0.001), brain natriuretic peptide level (OR: 1.1002; P = 0.036) and mitral E/E’ (OR: 1.187; P = 0.003) were independently associated with RVMDd. Conclusions Delayed right ventricular filling is related to more severe left ventricular systolic and diastolic dysfunction in STEMI patients. More attention should be paid to patients with RVMDd to prevent adverse events during hospitalization.
