Abstract:Background: Mechanisms of menopausal hormone replacement therapy (HRT)-related bleeding, undoubtedly mediated through endometrial steroid receptors, are poorly understood. We aimed to determine the steroid receptor expression in HRT-exposed endometrium in relation to disturbances of bleeding patterns. Methods: Prospective observational study in a tertiary referral menopause clinic in Western Australia. Thirty-eight outpatient endometrial biopsies (seven from women not on HRT, 31 from HRT users) were collected from 21 postmenopausal women during and outside bleeding episodes. Eleven women provided multiple biopsies. We performed an immunohistochemical analysis of endometrial glandular, stromal, epithelial, perivascular and endothelial expression of progesterone receptor (PR), glucocorticoid receptor (GR), androgen receptor (AR), estrogen receptors alpha and beta (ERalpha and ERbeta) and studied their relationship to bleeding patterns. Results: In HRT users, during a bleeding episode, there was a trend (non-significant) towards a decrease in PR and an increase in GR in endometrial glandular cells. No differences were observed in AR and ER expression. Conclusions: We have been unable to demonstrate significant differences in steroid receptor expression in endometrium of women using HRT who report unscheduled bleeding episodes. These observations differ from the endometrial steroid receptor expression observed with normal menstruation and long-term progestogen-only administration, suggesting that different local mechanisms are involved in HRT-related unscheduled bleeding.

Abstract:Uterine bleeding is a common reason why women discontinue menopausal hormone therapy (HT). This systematic review compared bleeding profiles reported in studies for continuous-combined HT approved in North America and Europe for moderate to severe vasomotor symptoms in postmenopausal women with a uterus. Non-head-to-head studies showed that uterine bleeding varies by formulation and administration route, with oral having a better bleeding profile than transdermal formulations. Cumulative amenorrhea over a year ranged from 18 to 61% with oral HT and from 9 to 27% with transdermal HT, as reported for continuous-combined HT containing 17β-estradiol (E2)/progesterone (P4) (56%), E2/norethisterone acetate (NETA) (49%), E2/drospirenone (45%), conjugated equine estrogens/medroxyprogesterone acetate (18-54%), ethinyl estradiol/NETA (31-61%), E2/levonorgestrel patch (16%), and E2/NETA patch (9-27%). Amenorrhea rates and the mean number of bleeding/spotting days improved over time. The oral E2/P4 combination was amongst those with lower bleeding rates and may be an appropriate alternative for millions of women seeking bioidentical HT and/or those who have bleeding concerns with other HT.

Abstract:Post-menopausal bleeding (PMB) accounts for 5% of gynecologic office visits and is the presenting symptom in 90% of women with endometrial cancer, which requires prompt evaluation. The most common etiology of PMB is vaginal or endometrial atrophy and endometrial polyps, while endometrial hyperplasia and carcinoma account for less than 10% of PMB. Transvaginal ultrasonography measurement of an endometrial thickness (EMT) less than or equal to 4 mm has a 99% negative predictive value for endometrial carcinoma. Endometrial sampling is required if EMT >4 mm or persistent bleeding occurs. Further evaluation can be accomplished with saline infusion sonography, magnetic resonance imaging, and hysteroscopy.

Abstract:Objective: To compare the effects of frequently used two different regimens of combined continuous hormone replacement therapy; 0.625 mg conjugated equine estrogen (CEE) + 2.5 mg medroxyprogesterone acetate (MPA) and 1 mg 17beta estradiol (E2) + 0.5 mg norethindrone acetate (NETA), on endometrial histopathology and postmenopausal uterine bleeding. Materials and methods: Two hundred and forty-six outpatient subjects aged 41-57 years were enrolled in the study conducted at the menopause clinic between November 2003 and November 2004. One hundred and thirty-nine patients were assigned to receive 0.625 mg conjugated equine estrogen + 2.5 mg medroxyprogesterone acetate (CEE/MPA), whereas 107 patients were to receive 17beta estradiol + 0.5 mg norethindrone acetate (E2/NETA). Inclusion criteria of the study were: normal values of endometrial thickness at basal evaluation, women with intact uterus, at least 12 months of amenorrhea, normal vaginal smear, bilateral mammography and biochemical blood parameters. All women were questioned every 3 months for vaginal bleeding/spotting. Endometrial sampling was performed by Pipelle catheter in the 12th month of therapy. Results: For the first 3 months, vaginal bleeding/spotting rate for the CEE/MPA group was 38.7%, whereas it was higher (45%) in the E2/NETA group. For the second 3-month period, vaginal bleeding/spotting frequencies were 41.1 and 37.8%, respectively. In the third 3-month period 30.6 and 29.6%, and in the fourth 3-month period, 18.5 and 12.5% of the patients reported vaginal bleeding or spotting. None of the results of endometrial sampling have shown findings of cancer histopathology. Conclusion: Compared to CEE/MPA regimen, E2/NETA therapy has not shown more favorable effects on postmenopausal bleeding abnormalities. Irregular endometrial proliferation was seen more with the E2/NETA regimen.

Abstract:Objective To observe the bleeding endometrium of women during menopause hormone therapy (M HT ) with vaginal ultrasound and explore the cut-off value for curettage during uterine bleeding combining pathological results .Methods From 2009 to 2014 totally 311 post-menopausal women visiting the First Affiliated Hospital of Xi'an Jiaotong University and accepting tibolone were selected as study objects ,and they were followed up for two years .The bleeding endometrial was monitored by transvaginal ultrasound and endometrial specimens were collected for further analysis of cut-off value for curettage .Results There was not significant difference in bleeding endometrial thickness between treatment group and control group (t = 1 .543 ,P > 0 .05 ) .The differences in endometrial thickness for curettage and proportion of endometrial pathological changes between two groups were not significant (t = 0 .443 ,χ2 = 4 .711 ,both P > 0 .05) .When the cut-off value of endometrial thickness was 7 .55mm ,the sensitivity and the specificity were 85 .71％ and 93 .02％ ,respectively (P < 0 .05) .The AUC was 0 .96 (P < 0 .05) .Conclusion There is no obvious influence on bleeding endometrial thickness and pathological change with tibolone treatment .The cut-off value for endometrial thickness may be appropriately adjusted for curettage when irregular bleeding of uterus occurs during therapy .

Abstract:Menopause is characterized by permanent cessation of menstrual periods and is clinically diagnosed after 12 months of complete amenorrhea. It occurs at a median age of 51 years alongside the physiological process of aging, although it can happen at an earlier age for other medical conditions or after surgery (surgical menopause). Due to reduced circulating estrogens and progesterone, the reproductive organs undergo progressive atrophy. This physiologic process of aging is also present at an endometrial level; without the cyclic hormonal actions of the menstrual cycle, the endometrium during menopause becomes atrophic. Postmenopausal bleeding (PMB) is a common gynecologic complaint encountered by the clinician. Endometrial cancer is present in about 10% of patients with PMB. Nevertheless, many other conditions, such as endometrial or cervical polyps, genital atrophy, or non-gynecologic conditions, may also be present. Historically, dilation and curettage (D&C) was the main diagnostic procedure in patients with PMB; however, newer methods of investigation have replaced D&C. The aim of this review is to present an up-to-date analysis of the current evidence for the clinical management of vaginal bleeding in postmenopausal women.
[A randomized case-control study on the influence of percutaneous estradiol and progestin in cyclic combined regimen on endometrium and vaginal bleeding pattern in Chinese early postmenopausal women]

Abstract:Objective: To observe the influence of percutaneous estradiol + micronized progesterone or medroxyprogesterone on endometrium and vaginal bleeding in Chinese post-menopausal women. Methods: A 3-year open randomized clinical study was designed. The estrogen and progestin were used continuous-combined together in a cyclic regimen. The percutaneous estradiol was regarded as estrogen. The microprogesterone (MP) and medroxyprogesterone (MPA) were served as progestin. Sixty healthy women (naturally menopausal for 1 to 5 years) were recruited and divided into four groups. The regimen as follows: (1) percutaneous estradiol gel (gel) containing 1.5 mg of 17 beta-estradiol (E2)/d plus micronized progesterone (MP) 100 mg/d in group 1 (G1, E2 1.5 + MP); (2) gel, 1.5 mg E2/d plus medroxyprogesterone acetate (MPA), 2 mg/d in group 2 (G2, E2 1.5 + MPA); (3) gel, 0.75 mg E2/d plus MP, 100 mg/d in group 3 (G3, E2 0.75 + MP); and (4) gel, 0.75 mg E2/d plus MPA, 2 mg/d in group 4 (G4, E2 0.75 + MPA). All were given for 25 d/month, and 5 days free. The transvaginal sonography was performed 0, 12, 24 and 36 months. The thickness of endometrium were measured. The endometrium biopsy by Pipelle tube was conducted at 24th or 36th months. The vaginal bleeding was recorded. Results: Fifty-nine patients (98.3%) finished 1 year and 56 patients (93.3%) for 2 years, 51 (85%) for 3 years. There was no significant changes of endometrium. The thickness of endometrium before treatment in four groups were 0.25 cm, 0.27 cm, 0.22 cm, 0.28 cm and after treatment 0.37 cm, 0.30 cm, 0.326 7 cm and 0.3 cm respectively. No significant differences was found in four groups (P > 0. 05). Fifty -three patients had the Pipelle biopsy. The pathological result showed: 16 cases (25.8%) proliferated, 1 case (1.6%) secretary; 11 cases (17.7%) atrophy; 23 cases (37.1%) mixed cell; tendency to hyperplasia in 2 cases (3.2%). During the treatment, the vaginal irregular bleeding rate was highest in G2 and lowest in G3. Conclusions: Daily estrogen containing 0.75 mg and 1.5 mg E2 combined with 100 mg progesterone and 2 mg medroxyprogesterone do not increase the thickness of endometrium significantly. The vaginal bleeding rate was low when combined with 100 mg micronized progesterone. Addition of natural progesterone will low down the bleeding rate and improve the HRT compliance.

Abstract:Objective To investigate the changes in the level of endocrine hormone in plasma in menopausal patients following radio frequency heat coaglulation(RFHC) which was used to abolish emdometrium for the treatment of dysfunctional uterine bleeding(DUB).Methods One hundred and fifty five patients 43-53(44.5±3.5) years old,who failed to respond to some medicine and resulted in amenorrhea,were recuirted randomly in Jan.2009 to Jan.2011 in the Department of Gynecology of Jinan Miltary General Hospital.They were included in the study group and treated with RFHC.Menstrual score(pictorial blood loss assessment chart,PBAC),hemoglobin(Hb) data,and endometrial currettage pathology were collected.In addition,six serum endocrine markers were tested before and after RFHC between 12 months with 24 months,including estradiol(E2),progesterone(P),testosterone(T),pituitary prolactin(PRL),corpus luteum erythropoietin(LH),and follicle stimulating hormone(FSH) examination.The control group(44-51,45.5±4.5 years old) was 38 cases with DUB who were treated with drugs.Results ①The efficacy of RFHC in the study group for bleeding cessation was 96.2%(150/156) and 100% after 12 and 24 months,respectively.Statistically,PBAC and Hb in the study group within follow-up months were obviously improved(P0.05),compared to the treatment group prior treatement and the control group.The rate of irregularly minor bleeding in 6 to 12 months was 3.8%(6/156).Those patients with irregularly minor bleeding received another RFHC treatment.None underwent hysterectomy in this group.②The serum E2 P,T,PRL,LH,FSH of the study group within 12 and 24 months after RFHC treatment showed no statistical difference(P0.05).③In thirty three patients in the study group(21.2%,33/156),with an average age of 48.0±3.7 years old,the level of serum E2,LH,FSH reached the level during menopause in 24 months after RFHC in the study group.And in the control group it was 18.3%(7/38),there was no significant difference between the study and the control groups(P0.05).Conclusion RFHC is a safe,effective and minimally invasive method in the treatment for DUB without postoperative gynecological endocrine hormone change.

Abstract:Unscheduled bleeding on hormone replacement therapy (HRT) can affect up to 40% of users. In parallel with the increase in HRT prescribing in the UK, there has been an associated increase in referrals to the urgent suspicion of cancer pathway for unscheduled bleeding. On behalf of the British Menopause Society (BMS) an expert review panel was established, including primary and secondary care clinicians with expertise in the management of menopause, with representatives from key related organisations, including the Royal College of Obstetricians & Gynaecologists, the British Gynaecological Cancer Society, British Society for Gynaecological Endoscopy, Royal College of General Practitioners and Faculty of Sexual and Reproductive Health, and service development partners from NHS England and GIRFT (Getting it Right First Time). For each topic, a focused literature review was completed to develop evidence led recommendations, where available, which were ratified by consensus review within the panel and by guideline groups.
Endometrial pathology in postmenopausal women with no bleeding

Abstract:Objective: The aim of this study was to determine the rate of unexpected uterine pathology in postmenopausal women admitted to a gynecology clinic with symptoms other than vaginal bleeding and who were scheduled to undergo hysterectomy. Materials and methods: We reviewed retrospectively the medical records of 283 postmenopausal patients who had gynecological surgery between September 2007 and January 2014. We reviewed their presenting symptoms on admission, the indications for surgery, and their transvaginal ultrasonographic findings. Postoperative histopathological results based on uterine specimens were also recorded. The results were analyzed statistically. Results: Of 283 patients who had surgery, 209 had no vaginal bleeding at the time of admission. From this group, 75.6% were found to have unsuspected pathology, including endometrial hyperplasia, endometrial polyps, uterine fibroids, adenomyosis, and one case of endometrial carcinoma (0.5%). The remaining 74 patients had experienced postmenopausal bleeding and in 87.8% there were pathological findings including 13 cases (17.6%) of endometrial cancer (p = 0.0001). Conclusion: Vaginal bleeding in postmenopausal women is indicative of a wide array of gynecological pathologies, including endometrial carcinoma. However, uterine fibroids, pelvic masses, or even endometrial cancer may develop without co-morbid vaginal bleeding. Therefore we advocate that postmenopausal women should undergo yearly screening and consultation, without waiting for an episode of vaginal bleeding.
