Abstract:Background Rheumatoid arthritis (RA) is a chronic systemic inflammatory disease with a poor prognosis in which synovitis induces progressive destruction of bones and joints, which leads to marked impairment of the activities of daily living.More timely and effective therapy for rheumatoid arthritis has contributed to increasing rates of clinical remission. However, progression of structural damage may still occur in patients who have satisfied remission criteria (1,2).Most RA patients who satisfied the remission criteria with normal findings on clinical and laboratory studies had imaging-detected synovitis may explain the observed discrepancy between disease activity and outcome in RA (3). Objectives To evaluate theeffects of double filtration plasmapheresis (DFPP) in combination with Leflunomide and methotrexate on magnetic resonance imaging (MRI)-detected inflammatory changes (synovitis and bone marrow edema) in early rheumatoid arthritis (RA) patients with high disease activity. Methods Sixty RA patients with high active disease of 6 months to3 years’ duration were randomized to DFPP group and no-DFPP group. All patients received leflunomide10mg, two times daily, plus methotrexate15mg orally once weekly. DFPP was performed in the DFPP group once 1-2 weeks for 3 sessions. The primary end point was the improvement in MRI-detected synovitis from baseline over 6 months. Secondary end point variables included Clinical remission in DAS28 and ACR20, ACR50, ACR70 and ACR90 responses for 6 consecutive months. Results The study achieved a significant reduction in synovitis compared with baseline, witha reduction of 7 in the median synovitis score from baseline to month 6in the DFPP group compared with 0 in the median synovitis score from baseline to month 6for the no-DFPP group (P<0.001). Synovitis scores in 48.39% (15/31)of the Patients in the DFPP group were 0 at month 6.Bone edema scores in 32.26% (10/31) of the Patients in the DFPP group were 0 at month 6. ACR20, ACR50, ACR70 and ACR90 responses for 6 consecutive months had been achieved in 100% of patients in the DFPP group, compared with 37.93%, 20.69%, 10.34%, 0% respectively, of patients in the no-DFPP group (P<0.001). DAS28 remissionfor 6 consecutive months was achieved in 100% of patients in the DFPP group, compared with 6.90%of patients in the no-DFPP group (P<0.001). Conclusions The combination of DFPP and DMARDs is superior to DMARDs alone for reducing MRI-detected signs of synovitis and bone edema in patients with early high active rheumatoid arthritis. The DFPP therapy enables a rapid and more complete suppression of inflammation in patients withearly high active rheumatoid arthritis. Abstract:Penicillamine exists as 2 stereoisomers, but only the D-isomer is used therapeutically. Its chemical reactivity derives from its functional groups, of which the thiol group seems the most important.It is difficult to determine penicillamine in biological fluids because of its instability, the presence of endogenous compounds with a thiol function, and the various chemical forms in which it occurs, namely reduced free penicillamine, penicillamine bound to proteins, and internal (P-S-S-P) and mixed (P-S-S-C) disulphides. The earliest assay methods (colourimetry, isotopic methods, gas-phase chromatography) were neither sensitive nor specific. High performance liquid chromatography with electrochemical detection has led to a more specific assay for D-penkillamine, with detection based on either derivatisation reactions or on electro-oxidisation of the thiol function. With dual-electrode detectors (Au/Hg) disulphides can be assayed directly.D-penicillamine is absorbed rapidly but incompletely (40 to 70%) in the intestine, with wide interindividual variations. Food, antacids and, in particular, iron reduce absorption of the drug. Its bioavailability is also dramatically decreased in patients with malabsorption states. The peak plasma concentration occurs at 1 to 3 hours after ingestion, regardless of dose, and is of the order of 1 to 2 mg/L after an oral dose of 250mg; some investigators have reported a double peak in plasma, which is probably not due to an enterohepatic cycle. The concentration in plasma then decreases rapidly, generally following a biphasic curve. When long term treatment is discontinued, there is a slow elimination phase lasting 4 to 6 days, which suggests that there is a ‘deep compartment’ or ‘slow pool of the drug reversibly bound to tissues’, particularly the skin. This may explain the persistence of its therapeutic effect and the occurrence of undesirable side effects after treatment has been stopped.During long term treatment plasma concentrations are highly variable between individuals. They do not seem to be correlated with the activity or the toxicity of D-penicillamine in patients with rheumatoid arthritis. More than 80% of plasma D-penicillamine is bound to proteins, particularly albumin. The rest is mainly in the free reduced form or as disulphides. Only a small portion of the dose is metabolised in the liver to S-methyl-D-penicillamine. The route of elimination is mainly renal; disulphides represent the main compounds found in the urine. Faecal excretion corresponds mainly to the non-absorbed fraction of the drug.

Abstract:D-penicillamine (D-pen) is a copper-chelating drug and has immune-modulatory properties. D-pen is used to treat rheumatoid arthritis, Wilson's disease, and kidney stones (cystinuria). However, associated adverse events (AEs) of D-pen treatment are frequent and often serious. Therefore, a comprehensive assessment of the safety profile of D-pen is urgently needed.We identified and analyzed AEs associated with D-pen between April-1970 to July-2020 from the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) databases and calculated the reported odds ratio (ROR) with 95% confidence intervals (CI) using the disproportionality analysis.A total of 9,150,234 AEs related to drugs were reported in the FAERS database, of which 542 were related to D-Pen. We report that D-pen was associated with dystonia (ROR: 20.52; 95%CI: 12.46–33.80), drug hypersensitivity (ROR: 5.42; 95%CI: 3.72–7.90), pancytopenia (ROR: 10.20; 95%CI: 5.61–18.56), joint swelling (ROR: 9.07; 95%CI: 5.51–14.94), renal-impairment (ROR: 6.68; 95%CI: 3.67–12.15), dysphagia (ROR: 5.05; 95%CI: 2.76–8.89), aggravation of condition (ROR: 4.16; 95%CI: 2.60–6.67), congestive cardiac failure (ROR: 4.04; 95%CI: 2.22–7.35), peripheral edema (ROR: 3.77; 95%CI: 2.17–6.55), tremor (ROR: 3.46; 95%CI: 2.00–6.01), pyrexia (ROR: 3.46; 95%CI: 2.00–6.01), and gait disturbance (ROR: 2.41; 95%CI: 1.29–4.52).Patients taking D-pen require close monitoring of renal function, blood counts, immunity, liver, cardiac function, and neurological function. D-pen suppresses immune system which maximizes the risk of infection.

Abstract:The ability to "mentalize," that is to understand and manipulate other people's behavior in terms of their mental states, is a major ingredient in successful social interactions. A rudimentary form of this ability may be seen in great apes, but in humans it is developed to a high level. Specific impairments of mentalizing in both developmental and acquired disorders suggest that this ability depends on a dedicated and circumscribed brain system. Functional imaging studies implicate medial prefrontal cortex and posterior superior temporal sulcus (STS) as components of this system. Clues to the specific function of these components in mentalizing come from single cell recording studies: STS is concerned with representing the actions of others through the detection of biological motion; medial prefrontal regions are concerned with explicit representation of states of the self. These observations suggest that the ability to mentalize has evolved from a system for representing actions.

Abstract:Elastosis perforans serpiginosa (EPS) is a rare complication of chronic therapy with a high-dose of D-penicillamine (1 g daily for more than 5 years), characterized by the elimination of abnormal elastic fibers from the upper dermis through the epidermis. D-penicillamine (DPA) is a heavy metal chelator primarily used for disorders such as cystinuria and Wilson disease. This therapy can lead to induction of EPS through a still unknown mechanism. We report the follow-up of a D-penicillamine-induced EPS in patient with Wilson disease, which prompted us to switch the therapy with trientine (another metal chelator). After 14 years the cutaneous lesions are still visible; therefore, we conclude that the DPA-induced cutaneous damage is irreversible.

Restrictions on antidepressant medications for children: a review of Medicaid policy.

Abstract:OBJECTIVE This study evaluated Medicaid's prior-authorization policies restricting the prescribing of antidepressant medications for children. METHODS Medicaid program prior-authorization policies for antidepressant medications were obtained for all available states. All criteria needed for authorization were recorded, with a focus on policies that applied specifically to children. RESULTS Data from 49 states and the District of Columbia revealed that 30 states (60%) required prior authorization for antidepressants, of which eight (27%) made specific provisions for children. These provisions varied across states. In most states fluoxetine could be prescribed for children with minimal restrictions, and two states had prior-authorization policies that strongly encouraged pediatric patients to use fluoxetine. State policies regarding other selective serotonin reuptake inhibitors varied widely. CONCLUSIONS Although relatively few states included provisions for children in prior-authorization requirements for antidepressants, in states that did, the policies implemented varied widely. These findings raise important questions about the rational development of prescription drug reimbursement policy.
[Perioperative management of treatment of patients with inflammatory rheumatic diseases : Updated recommendations of the German Society of Rheumatology]

Abstract:Background: Prior to surgical interventions physicians and patients with inflammatory rheumatic diseases remain concerned about interrupting or continuing anti-inflammatory medication. For this reason, the German Society for Rheumatology has updated its recommendations from 2014. Methods: After a systematic literature search including publications up to 31 August 2021, the recommendations on the use of of glucocorticoids, conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) and biologics (bDMARDs) were revised and recommendations on newer drugs and targeted synthetic (ts)DMARDs were added. Results: The glucocorticoid dose should be reduced to as low as possible 2-3 months before elective surgery (in any case <10 mg/day) but should be kept stable 1-2 weeks before and on the day of surgery. In many cases csDMARDs can be continued, exceptions being a reduction of high methotrexate doses to ≤15 mg/week and wash-out of leflunomide if there is a high risk of infection. Azathioprine, mycophenolate and ciclosporin should be paused 1-2 days prior to surgery. Under bDMARDs surgery can be scheduled for the end of each treatment interval. For major interventions Janus kinase (JAK) inhibitors should be paused for 3-4 days. Apremilast can be continued. If interruption is necessary, treatment should be restarted as soon as possible for all substances, depending on wound healing. Conclusion: Whether bDMARDs increase the perioperative risk of infection and the benefits and risks of discontinuation remain unclear based on the currently available evidence. To minimize the risk of a disease relapse under longer treatment pauses, in the updated recommendations the perioperative interruption of bDMARDs was reduced from at least two half-lives to one treatment interval.
Cystic kidney disease in a patient with systemic toxicity from long-term D-penicillamine use

Abstract:D-penicillamine, used to treat cystinuria, is known to cause impaired collagen deposition and dysfunction in elastic fibers. D-penicillamine also has been associated with glomerular abnormalities, typically membranous glomerulonephritis. We describe a patient with severe bilateral cystic kidney disease that developed after long-term D-penicillamine use for treatment of cystinuria. The cysts in the kidneys were noted during an evaluation for acute kidney injury. The patient had no evidence of cysts on prior renal imaging at a time when his kidney function was normal. Simultaneously, he presented with multiorgan manifestations of D-penicillamine toxicity, including the skin findings of cutix laxa and elastosis perforans serpiginosa. Consequently, D-penicillamine treatment was discontinued, after which the progression of cystic kidney disease gradually ceased, along with the other systemic manifestations of toxicity. To our knowledge, this is the first report of cystic kidney disease associated with and perhaps caused by long-term d-penicillamine therapy. The proposed mechanism of cyst formation is the malfunction of the extracellular matrix of the kidney by d-penicillamine that leads to an impaired repair process after kidney injury.

Abstract:Background Rheumatoid arthritis (RA) is a chronic inflammatory bone-destructive disorder with autoimmune features. Apremilast is a novel phosphodiesterases 4 (PDE4) inhibitor suppressing immune and inflammatory responses. Objectives We assessed the anti-inflammatory and bone protection effects of apremilast in collagen CII induced arthritis (CIA)models. Methods Apremilast was given starting from day 14 after immunisation, we investigated whether apremilast (5 mg/kg or 25 mg/kg) can ameliorate arthritis onset. Bone erosion was measured by histological and micro-computed tomographic analysis. Anti–mouse type II collagen (CII) antibody levels were measured by enzyme-linked immunosorbent assay. Human cartilage and rheumatoid arthritis (RA) synovial fibroblasts (RASFs) implantation in the severe combined immunodeficiency (SCID) mouse model of RA were used to study the role of apremilast in suppression of RASFs destroying cartilage in vivo. Results We found that apremilast therapy delayed arthritis onset and reduced arthritis scores in CIA model at a different dose, compared to CIA model and blank vector (figure 1A). Total serum IgG, IgG1, IgG2a, and IgG2b were all decreased in apremilast groups. Furthermore, apremilast can prevent CIA mice from bone erosion by CT analysis. High dose of apremilast (25 mg/kg) was superior to low dose (5 mg/kg) in treating CIA (figure 1B, C). Apremilast treatment can inhibit destroy and migratory ability of RASFs to cartilages. Compared to the model group, Apremilast treatment significantly reduced the invasion scores in both primary implant and contralateral implant. Conclusions Our data suggest that apremilast was effective in preventing arthritis and bone erosion in CIA model, implicating a potential promise of therapy on rheumatoid arthritis. Aggressive treatment in early rheumatoid arthritis: a randomised controlled trial

Abstract:OBJECTIVES: To compare three therapeutic strategies using slow acting antirheumatic drugs (SAARDs) in early rheumatoid arthritis (RA), for their disease modifying properties, toxicity, and lag time until treatment effect.METHODS: Patients with recent onset RA from six hospitals were randomly assigned to immediate initiation of one of three treatment strategies: (I) a "mild SAARD with a long lag time" (hydroxychloroquine, if necessary replaced by auranofin); (II) a "potent SAARD with a long lag time" (intramuscular gold, if necessary replaced by D-penicillamine); (III) a "potent SAARD with a short lag time" (methotrexate, if necessary replaced by sulfasalazine). Comparisons included two years of follow up.RESULTS: All SAARD strategies reduced mean disease activity. A greater percentage of patients improved clinically with strategies II and III than with strategy I: percentages of patients improved on joint score with strategies II and III (79% and 82%, respectively), which was statistically different from strategy I (66%). The same was true for remission percentages: 31% and 24% v 16%, respectively). Longitudinal analysis showed significantly less disability with strategy III, and a lower erythrocyte sedimentation rate with strategy II than with strategy I. In addition, radiological damage after one and two years, was significantly lower in strategies II and III (at two years median scores were 11 and 10 v 14 in strategy I, p<0.05). Toxicity was increased in strategy II compared with the other strategies.CONCLUSION: Strategy III, comprising methotrexate or sulfasalazine, produced the best results weighing effectiveness and toxicity. Strategy I (hydroxychloroquine or auranofin) was slightly less effective, and strategy II (intramuscular gold or D-penicillamine) was associated with increased toxicity.

Abstract:Objectives Chronic inflammation promotes cardiovascular risk in rheumatoid arthritis (RA). Biological disease-modifying antirheumatic drugs (bDMARDs) improve disease activity and cardiovascular disease outcomes. We explored whether bDMARDs influence the impact of disease activity and inflammatory markers on long-term cardiovascular risk in RA. Methods We studied 4370 participants without cardiovascular disease in a 10-country observational cohort of patients with RA. Endpoints were (1) major adverse cardiovascular events (MACE) encompassing myocardial infarction, stroke and cardiovascular death; and (2) any ischaemic cardiovascular events (iCVE) including MACE plus revascularisation, angina, transient ischaemic attack and peripheral arterial disease. Results Over 26 534 patient-years, 239 MACE and 362 iCVE occurred. The interaction between 28-joint Disease Activity Score with C-reactive protein (DAS28-CRP) and bDMARD use was significant for MACE (p=0.017), suggesting the effect of DAS28-CRP on MACE risk differed among bDMARD users (n=515) and non-users (n=3855). DAS28-CRP (per unit increase) is associated with MACE risk in bDMARD non-users (HR 1.21 (95% CI 1.07 to 1.37)) but not users (HR 0.69 (95% CI 0.40 to 1.20)). The interaction between CRP (per log unit increase) and bDMARD use was also significant for MACE (p=0.011). CRP associated with MACE risk in bDMARD non-users (HR 1.16 (95% CI 1.04 to 1.30)), but not users (HR 0.65 (95% CI 0.36 to 1.17)). No interaction was observed between bDMARD use and DAS28-CRP (p=0.167) or CRP (p=0.237) for iCVE risk. Conclusions RA activity and inflammatory markers associated with risk of MACE in bDMARD non-users but not users suggesting the possibility of biological-specific benefits locally on arterial wall independently of effects on systemic inflammation.

Abstract:Background Numerous studies support a bidirectional association between rheumatoid arthritis (RA), a chronic autoimmune degenerative inflammatory joint disease, and periodontitis, a chronic inflammatory disease caused by the immune reaction to bacteria organized in biofilms. RA and periodontitis are both multifactorial chronic inflammatory diseases that share common modifiable and non‐modifiable risk factors. There is no cure for RA; treatment is based on lifestyle modifications and a variety of medications: nonsteroidal anti‐inflammatory drugs (NSAID), glucocorticoids, and disease‐modifying antirheumatic drugs (DMARDs, e.g., conventional synthetic DMARDs [csDMARDs]; biological DMARDs [bDMARD] and targeted synthetic DMARDs). There are molecular pathways of inflammation that are common to both RA and periodontitis. Thus, there is a potential effect of RA treatments on periodontitis. This systematic review aims to assess the impact of antirheumatic agents on periodontal conditions of patients suffering from both RA and periodontitis. Methods PubMed/MEDLINE, Cochrane Library, and Embase online databases were systematically explored, and a manual search was performed to identify relevant studies published until January 2023. This review is registered in the PROSPERO database (CRD42023409006). Results A total of 2827 articles were identified, and 35 fulfilled the inclusion criteria. The included studies generally show a consensus that, at normal dosage, NSAID and corticosteroids have negligible impact on periodontium. Similarly, csDMARD alone or in combination with other csDMARD demonstrated no adverse effect on periodontium. Monotherapy with bDMARD had a positive effect on periodontal pocket depths and gingival inflammation in the longitudinal studies up to 6 months but showed negligible effect on the periodontium in interventional studies with a longer follow‐up (9 months and 15.1 months). However, the combination of tumor necrosis factor (TNF)‐α inhibitors + methotrexate (MTX) was associated with a rise in gingival inflammation. Due to the considerable heterogeneity of the study designs, a meta‐analysis could not reasonably be performed. Conclusion Within the limitations of the available studies, there is evidence to suggest that bDMARD monotherapy may improve the periodontal condition of RA patients with periodontal disease to a certain extent; the concomitant medication of TNF inhibitor + MTX could worsen gingival inflammation. More data are required to understand the impact of RA therapies on periodontal health.

Implementation of a penicillin allergy protocol in open abdominal wall reconstruction: Preoperative optimization program

Abstract:Introduction: Beta-lactam prophylaxis is the first-line preoperative antibiotic in open abdominal wall reconstruction. However, of the 11% patients reporting a penicillin allergy (PA), most receive second-line, non-β-lactam prophylaxis. Previously, abdominal wall reconstruction research from our institution demonstrated increased wound complications, readmissions, and reoperations with non-β-lactam prophylaxis. Therefore, a collaborative quality improvement initiative was developed with the infectious disease service, and a penicillin allergy protocol was instituted that stratified patients' risk of allergic reaction with a goal to increase β-lactam prophylaxis use. The effect of the penicillin allergy protocol on open abdominal wall reconstruction outcomes was prospectively evaluated. Methods: Patients with penicillin allergy undergoing open abdominal wall reconstruction were identified and grouped according to penicillin allergy protocol implementation. Pre-penicillin allergy protocol underwent open abdominal wall reconstruction before January 1, 2020, predominantly receiving non-β-lactam prophylaxis; post-penicillin allergy protocol underwent open abdominal wall reconstruction between January 1, 2020-November 1, 2023, predominantly receiving β-lactam prophylaxis. Incidence of surgical site infection was the primary outcome. Standard and inferential statistical analyses were performed. Results: Of 315 patients with penicillin allergy, 250 underwent open abdominal wall reconstruction pre-penicillin allergy protocol and 65 post-penicillin allergy protocol. Pre- and post-penicillin allergy protocol were similar in allergic reaction severity history, sex, race, age, diabetes, American Society of Anesthesiologists score, hernia defect size, and mesh type (P > .05). Post-penicillin allergy protocol had lower body mass index (33.4 ± 7.9 vs 29.8 ± 5.3 kg/m2; P = .002) and fewer active smokers (12.4% vs 1.5%; P = .019). Expectedly, post-penicillin allergy protocol received more β-lactam prophylaxis (22.8% vs 83.1%; P < .001) and no antibiotic-induced allergic reactions. Post-penicillin allergy protocol had significantly fewer surgical site infections (24.4% vs 3.1%; P < .001), wound breakdown (16.0% vs 3.1%; P = .004), reoperations (19.2% vs 0.0%; P < .001), and readmissions (25.3% vs 9.2%; P = .006) but no statistically significant reduction in recurrence (8.4% vs 1.5%; P = .057). Conclusions: The penicillin allergy protocol safely increased the number of patients with penicillin allergy undergoing open abdominal wall reconstruction receiving β-lactam prophylaxis and decreased the rate of surgical site infections, wound complications, reoperations, and readmissions. These data supported the systemwide implementation of the penicillin allergy protocol for both general and orthopedic surgery, which has been incorporated into the electronic medical record of 13 hospitals within the system.
Retrospective Study of Adverse Events in Patients with Rheumatoid Arthritis Treated with Second-Line Drugs

Abstract:OBJECTIVE:To evaluate rates on the adverse side effect and discontinuation of second-line drugs frequently used in the treatment of rheumatoid arthritis (RA).METHOD:Eight hundred and sixty-four RA patients were studied in a retrospective program.RESULTS:Upper abdominal discomfort was most commonly seen when using second-line drugs. Rash was often associated with D-penicillamine (20.6%) and Sinomenium therapy (13.7%). Methotrexate (MTX) was uniquely characterized by substantial upper GI toxicity (32.2%) and Tripterygium wilfordii Hook. f. (TWH) (14.4%) by menstrual abnormality. Sulfasalazine users reported adverse events including upper abdominal trouble (39.0%), nausea (7.3%) and anorexia (7.3%) while the risk of GI malaise was greater. Patients taking hydroxychloroquine complained of blurred vision (19.6%) but no one went blind. Toxic side effects seemed to be the most common reasons for stoppages, and the patients taking MTX had the lowest discontinuation rate. Combination of D-penicillamine and Methotrexate did not increase the incidence of adverse events.CONCLUSIONS:Knowledge on these different patterns of toxicity provided choices in the selection of second line agents for particular RA patients. However, long-term monitor are required when drugs are being used.
Effect of Periodontal Therapy on Rheumatoid Disease Activity in Patients on Anti-Rheumatoid Drugs - A Prospective Study

Abstract:Aim: The aim of this clinical trial rheumatoid arthritis-chronic periodontitis (RA-CP) is to assess the effect of non-surgical periodontal therapy in RA patients. Materials and methods: Sixty RA patients taking disease-modifying anti-rheumatoid drugs (DMARD) with CP were selected from the Institute of Rheumatology, Madras Medical College, randomised into the intervention group (IG) and control group (CG). IG received non-surgical periodontal therapy after the screening visit, but CG did not receive treatment until the 3 months study period. Periodontal parameters were recorded at baseline and after 3 months for both groups. Rheumatoid Disease activity (RD activity) was assessed using disease activity score (DAS28), inclusive of tender joints count (TJC), swollen joints count (SJC), visual analog scale (VAS), and erythrocyte sedimentation rate (ESR) for both groups at baseline and after 3 months. Results: At the end of 3 months, IG had significant reduction in RA parameters such as tender joints count (0.0005), swollen joints count (0.0005), ESR (0.003), VAS score (0.0005), and DAS28 values (0.0005) compared to CG. Periodontal parameters at the end of 3 months were also significantly reduced in IG than in CG. Conclusion: Non-surgical periodontal therapy might have reduced the burden of systemic inflammatory markers, thus reducing severity of RA in IG. Reduction in tenderness and swelling of joints in the upper arm might have enabled the patients in IG to perform better oral hygiene maintenance procedures.

Abstract:AIM AND METHODS By hydrogen gas clearance technique to measure gastric mucosal blood flow (GMBF) and a high dose of capsaicin to ablate the capsaicin-sensitive afferent fibers, the roles of capsaicin-sensitive afferent fibers and endogenous NO in the gastric acid secretion and hyperemic response to intragastric distention were studied in rats. RESULTS (1) There was an increase in acid secretion associated with the increase in GMBF to intragastric distention. (2) Pretreatment with a high dose of capsaicin to ablate afferent fibers completely abolished the GMBF and partially inhibited the acid secretion during the intragastric distention. (3) The increase in GMBF to intragastric distention was completely blocked by pretreatment with L-NAME, whereas the acid secretion was significantly attenuated. CONCLUSION Capsaicin-sensitive afferent fibers and endogenous NO are involved in the increases of gastric acid secretion and GMBF.

Abstract:We study numerically the interference resulting from the superposition of two Bessel beams propagating in free space. We discuss how to obtain such beams and show the existence of the self-imaging effect during propagation. The evolution of the superimposed Bessel beams is analyzed on the basis of the evolution of the individual beams. Our exact numerical predictions contradict previous approximated analytical treatments, showing that they can lead to quantitatively wrong results and misleading conclusions.
