Peritoneal Tube Graft for the Reconstruction of Inferior Vena Cava (IVC): A Viable Alternative Option in a Resource-Constrained Setup

Abstract:Inferior vena cava (IVC) leiomyosarcoma is a rare and aggressive tumor. Radical surgery with margin-negative resection followed by vascular reconstruction remains the treatment of choice. Options for vascular restoration include primary repair, patch repair, synthetic grafts (PTFE), cryopreserved veins, and bovine pericardium. Peritoneal patches and grafts are gaining popularity for venous reconstruction in the last few years because of their easy availability, few associated postoperative complications, and cost-effectiveness. The present report highlights the use of peritoneal tube graft for IVC reconstruction with excellent outcomes. A 50-year-old lady diagnosed with right infrarenal IVC leiomyosarcoma underwent R0 resection, and an approximately 8 cm IVC defect was reconstructed successfully with an autologous peritoneal tube graft in an end-to-end manner. The postoperative course was uneventful. Radical surgery with vascular reconstruction remains the treatment of choice for IVC leiomyosarcoma. Autologous peritoneal tube grafts are an excellent option for major vascular reconstruction in a resource-constrained setup.
