Abstract:Obliterative portal venopathy (OPV) is a rare cause of portal hypertension in the absence of cirrhosis. It involves a pathologic obliteration of the intrahepatic portal venous branches, leading to increased splanchnic blood flow. It typically manifests with variceal bleeding in young patients without evidence of liver insufficiency or cirrhosis. Differentiation from cirrhosis is paramount since major differences exist in managing both pathologies. OPV is diagnosed by liver biopsy. Since it is a rare condition, at times, it can be wrongly categorized as cryptogenic cirrhosis. There is no established cause of OPV, although it has been associated with HIV infection, Schistosomiasis, prothrombotic states, and medications like Didanosine and Azathioprine.

Abstract:International Journal of Surgical Pathology, Ahead of Print. Aim: The differential diagnosis of intrinsic nonfibrotic conditions that may lead to portal hypertension include hepatoportal sclerosis (HPS), nodular regenerative hyperplasia (NRH), and sinusoidal obstruction syndrome (SOS). In this article, we characterize the clinical features and outcome of these lesions when they manifest as portal hypertension. Methods: Data was collected through retrospective patient medical records. Results: Patients (HPS: 28, NRH: 17, SOS: 11) were identified more frequently in recent years. All groups presented with signs and symptoms of portal hypertension. All patients had complex medical histories. An elevated serum alkaline phosphatase occurred in all groups and an elevated bilirubin with SOS. Imaging of the liver with HPS and NRH suggested cirrhosis, which was not seen with SOS. 11%, 12%, and 9% of patients in the HPS, NRH, and SOS respectively, underwent transjugular intrahepatic portosystemic shunt placement to manage the complications of portal hypertension, while 43%, 24%, and 36% of patients respectively, received a liver transplant. Conclusions: Patients with HPS, NRH, and SOS had complex medical histories, likely contributing to the development of these lesions. They are recognized more frequently now. In contrast to HPS and NRH, SOS occurred in liver transplant recipients, was associated with elevated serum bilirubin, and imaging did not suggest the presence of advanced fibrosis/cirrhosis. Liver transplantation appeared to be a viable treatment for complications related to HPS and NRH. Retransplantation for SOS yielded mixed results. HPS, SOS, and NRH should be considered when evaluating liver specimens from patients with unexplained nonfibrotic portal hypertension. Key message: Intrinsic nonfibrotic causes of portal hypertension appear to be increasing in frequency. The differential diagnosis includes NRH, HPS, and SOS. These conditions are associated with complex diseases and possibly due to treatments. Pathologists need to be aware of this differential diagnosis when presented with liver biopsies performed to assess portal hypertension.

Abstract:Portal hypertensive enteropathy (PHE) describes pathologic mucosal changes observed in the small intestine in a subset of patients with portal hypertension. Polypoid enteropathy is a rare manifestation of PHE with a prevalence of 0.3 %. PHE is commonly a benign finding, however cases with gastrointestinal bleeding, and rare life-threatening outcomes have been reported. No standardized treatment and management exists in the literature.

Abstract:Extrahepatic portal vein obstruction (EHPVO) is a kind of liver vascular disease that causes structural abnormalities in the portal veins, including cavernomatous metamorphosis and obstruction. It is the most common cause of esophageal varices-related hematemesis in youngsters. Significant risk factors include congenital abnormalities, dehydration, sepsis, trauma, hypercoagulable conditions, and multiple transfusions. Acute extrahepatic portal vein blockage is often ignored because patients are usually asymptomatic. Subacute and chronic stages can cause symptoms including splenomegaly and hematemesis without hepatic decompensation. Imaging studies aid in the diagnosis; Doppler imaging is added to ultrasonography to visualize portal vein blood flow. MRI and CT scans are used to visualize portal vein blockage. Prevention of acute bleeding is the cornerstone in the management. Studies have shown that transhepatic thrombolysis is the preferred choice to avoid systemic side effects. Transjugular intrahepatic portosystemic shunt (TIPS) treats extrahepatic portal venous thrombosis and is typically followed by conservative variceal hemorrhage treatment. Liver transplantation is performed when other management measures fail. Here, we present a rare case of EHPVO in a nine-year-old female who was lost to follow-up for a long time and later showed signs of portal biliopathy and non-visualization of a surgically created splenorenal shunt. Re-shunting was performed after detailed conservative management, and the patient responded well to the treatment given.
[A Clinical Analysis of Six Cases of Portal Hypertension Secondary to Primary Myelofibrosis and Review of Literatures].

Abstract:OBJECTIVE:To investigate the clinical characteristics of the patients with portal hypertension secondary to primary myelofibrosis (PH-PMF).METHODS:The clinical data of patients with PH-PMF retrieved from Peking University Third Hospital were collected and analyzed.RESULTS:A total of 6 patients [M:F 3:3, average age (52.8 ± 11.7) years] were included in this study. The median duration from illness onset to definite diagnosis was 24 months. The symptoms related to portal hypertension included bloating, abdominal mass, hematemesis and melena. The signs related to portal hypertension included splenomegaly (moderate or severe in all patients), hepatomegaly (mild or moderate in most of patients), shifting dullness positive and varices in abdominal wall. Only a small portion of patients had jaundice, liver palms and spider angioma. The indexes related to liver functions were normal or only mild abnormal in most cases. The average diameters of portal vein and splenic vein were (16.0 ± 3.2) mm and (11.0 ± 3.4) mm, respectively. Thrombosis of portal vein or splenic vein was found in 2 cases. Liver biopsy revealed extramedullary hemopoiesis in one patient. In addition to medicine therapy related to portal hypertension in 5 patients with PH-PMF, 1 patient received splenectomy and another received endoscopic variceal ligation.CONCLUSIONS:The patients with PH-PMF were clinically rare and easy to be misdiagnosed. The main keys to differentiate PH-PMF from cirrhosis included marked splenomegaly, mild or moderate hepatomegaly, good liver functions, lack of liver palms or spider angioma and extramedullary hemopoiesis showed in liver biopsy samples.

Abstract:Portal hypertension (PH) is a common and serious clinical syndrome characterized by hemodynamic abnormalities of the portal venous system. Esophagogastric variceal bleeding, hepatic encephalopathy, and ascites are serious complications of PH that often result in liver transplantation or death. [ 1 ] Liver cirrhosis is the leading cause of portal hypertension and end-stage chronic liver disease. Alcoholic steatohepatitis is the leading cause of cirrhosis in western countries and India, whereas chronic hepatitis B and C are the main causes of cirrhosis in Asia. Non-cirrhotic portal hypertension (NCPH) accounts for approximately 10% to 20% of portal hypertension. [ 2 ] Porto-sinusoidal vascular disease (PSVD) and extrahepatic portal vein thrombosis are the most common etiologies of NCPH and are characterized by obvious symptoms of portal hypertension, but almost normal liver function. Abstract:Idiopathic noncirrhotic portal hypertension is a poorly defined clinical condition of unknown etiology. Patients present with signs and symptoms of portal hypertension without evidence of cirrhosis. The disease course appears to be indolent and benign with an overall better outcome than cirrhosis, as long as the complications of portal hypertension are properly managed. This condition has been recognized in different parts of the world in diverse ethnic groups with variable risk factors, resulting in numerous terminologies and lack of standardized diagnostic criteria. Therefore, although the diagnosis of idiopathic noncirrhotic portal hypertension requires clinical exclusion of other conditions that can cause portal hypertension and histopathologic confirmation, this entity is under-recognized clinically as well as pathologically. Recent studies have demonstrated that variable histopathologic entities with different terms likely represent a histologic spectrum of a single entity of which obliterative portal venopathy might be an underlying pathogenesis. This perception calls for standardization of the nomenclature and formulation of widely accepted diagnostic criteria, which will facilitate easier recognition of this disorder and will highlight awareness of this entity.

Abstract:Background and Aims: Arteriovenous fistula is a rare cause of portal hypertension, however, multiple fistulas is rather peculiar in clinic. To our knowledge, it has not been reported so far. Case report: We presented a 33-year-old man with abdominal distension, midsection pain, with a history of an abdominal surgery due to a penetrating abdominal trauma10 years ago. Esophageal variceal hemorrhage then developed. Ultrasonography and computed tomography revealed localized dilatation of the portal vein, the diameter of which was 4 centimeters, splenomegaly and ascites. Upper endoscopy revealed grade 3 esophageal varices. Computed tomography angiography demonstrated multiple arteriovenous fistulas with arterioportal shunts which led to a volume-induced portal hypertension. The arteriovenous fistulas concluded the fistula between the right gastroepiloic artery and the splenic vein, the fistula between the splenic artery branch and the splenic vein, the fistula between the superior mesenteric artery and the splenic vein, the fistula between the interior mesenteric artery and the splenic vein. The diagnosis was confirmed by angiogram of the abdominal vessels. Transarterial embolization failed because of high shunt velocities, and the coil prolapsed into the vessel, the diameter of which is 8 centimeters. The patient died finally due to bleeding from esophageal varices. Conclusion: Early diagnosis and treatment of arteriovenous fistula can prevent portal hypertension development and its complications.

Abstract:BackgroundPortal hypertension is a key complication of portal hypertension, which is responsible for the development of varices, ascites, bleeding, and hepatic encephalopathy, which, in turn, cause a high mortality and requirement for liver transplantation.AimThis review deals with the present day state-of-the-art preventative treatments of portal hypertension in cirrhosis according to disease stage. Two main disease stages are considered, compensated and decompensated cirrhosis, the first having good prognosis and being mostly asymptomatic, and the second being heralded by the appearance of bleeding or non-bleeding complications of portal hypertension.ResultsThe aim of treatment in compensated cirrhosis is preventing clinical decompensation, the more frequent event being ascites, followed by variceal bleeding and hepatic encephalopathy. Complications are mainly driven by an increase of hepatic vein pressure gradient (HVPG) to values ≥10 mmHg (defining the presence of Clinically Significant Portal Hypertension, CSPH). Before CSPH, the treatment is limited to etiologic treatment of cirrhosis and healthy life style (abstain from alcohol, avoid/correct obesity…). When CSPH is present, association of a non-selective beta-blocker (NSBB), including carvedilol should be considered. NSBBs are mandatory if moderate/large varices are present. Patients should also enter a screening program for hepatocellular carcinoma. In decompensated patients, the goal is to prevent further bleeding if the only manifestation of decompensation was a bleeding episode, but to prevent liver transplantation and death in the common scenario where patients have manifested first non-bleeding complications. Treatment is based on the same principles (healthy life style..) associated with administration of NSBBs in combination if possible with endoscopic band ligation if there has been variceal bleeding, and complemented with simvastatin administration (20-40 mg per day in Child–Pugh A/B, 10–20 mg in Child C). Recurrence shall be treated with TIPS. TIPS might be indicated earlier in patients with: 1) Difficult/refractory ascites, who are not the best candidates for NSBBs, 2) patients having bleed under NSBBs or showing no HVPG response (decrease in HVPG of at least 20% of baseline or to values equal or below 12 mmHg). Decompensated patients shall all be considered as potential candidates for liver transplantation.ConclusionTreatment of portal hypertension has markedly improved in recent years. The present day therapy is based on accurate risk stratification according to disease stage.

Abstract:Cruveilhier-Baumgarten syndrome presents a rare manifestation of portal hypertension characterized by a portosystemic shunt through a dilated paraumbilical vein, typically accompanied by classical signs such as caput medusae and a venous hum. We report a compelling case of a 41-year-old male presenting with portal hypertension, exhibiting clinical and radiological features of Cruveilhier-Baumgarten syndrome but notably lacking the characteristic venous hum. Clinical examination revealed moderate splenomegaly with prominent dilated veins and venous thrill but no caput medusae. Laboratory investigations indicated thrombocytopenia and esophageal varices on upper GI endoscopy. Imaging studies confirmed portal hypertension with findings consistent with Cruveilhier-Baumgarten syndrome, including a dilated paraumbilical vein and splenic artery aneurysms, along with the unexpected absence of a venous hum. Despite the classical radiological features, our patient did not present with hematemesis, possibly attributed to the presence of paraumbilical veins. This case highlights the diagnostic challenges and atypical presentations of Cruveilhier-Baumgarten syndrome, emphasizing the importance of comprehensive clinical evaluation and imaging modalities in its diagnosis and management. Management strategies primarily focus on addressing portal hypertension and underlying liver disease. This case underscores the need for further research to elucidate the varied clinical presentations and pathophysiology of Cruveilhier-Baumgarten syndrome variants, enhancing our understanding and management of this rare entity.
Pseudosclerosing cholangitis in extrahepatic portal venous obstruction

Abstract:Biliary changes secondary to portal hypertension have rarely been described in published reports. Twenty consecutive patients with extrahepatic portal venous obstruction, all of whom showed a variable degree of abnormalities in the biliary tract suggestive of sclerosing cholangitis, are described. These biliary abnormalities were: focal narrowing, dilatations, and cholangitic changes affecting the main bile ducts and hepatic ducts. The left hepatic duct and its branches were affected in all patients. Only one patient had clinical or biochemical evidence of cholestasis. The mechanism of these abnormalities in the biliary tract of these patients is perhaps the development of portal collaterals.

Abstract:A portal vein aneurysm, first reported by Barzilai and Kleckner in 1956,1 is a rare clinical entity caused by portal hypertension or the malformation of veins. The frequent application of radiological imaging for diagnosis and screening of abdominal disorders accounts for the increasing number of case reports over the past 10 years; however, fewer than 60 cases have been reported in the English language literature to date. Herein we report a woman with an extrahepatic portal vein aneurysm, and present a review of literature relating to etiology, clinical significance and management strategies.
Histopathological features of idiopathic portal hypertension: A systematic review and meta-analysis

Abstract:Background: Portal hypertension (PH) is a clinically significant entity that could present with life-threatening gastrointestinal bleeding. Cirrhosis is the most common cause of PH, with well-documented histopathology and etiology. However, in idiopathic portal hypertension (IPH), no single histopathologic finding is associated with PH. Our systematic review aims to identify and summarize the prevalence of the common histological findings of IPH. Methods: We systematically searched PubMed, Cochrane CENTRAL, Web of Science, and Scopus till 1ST March 2022 for studies describing the histopathological features of IPH. Data were extracted from eligible studies and pooled as events rate and 95% confidence interval (CI) using binary random-effects model by open meta-analyst software. Results: We included 23 retrospective studies with a total sample size of 813 patients. The overall incidence of nodular regenerative hyperplasia was 38.6%, 59.8% for portal fibrosis, 51.3% for periportal fibrosis, 39.3% for perisinusoidal fibrosis, 89.8% for portal vein sclerosis, 42.2% for portal inflammation, 53.3% for mega-sinusoids, 39.5% for thickening of portal vein branches, 93.8% for narrowing of portal veins, 53.3% for hepatic veins/venous outflow obstruction, 51.4% for aberrant portal/periportal vessels, 42.4% for shunt vessel, 50.9% for ductular proliferation, and 16.3% for steatosis. Conclusion: Due to the relatively non-pathognomonic and non-specific nature of IPH, a combination of different histological features such as the portal and periportal fibrosis, portal vein sclerosis, mega-sinusoids, narrowing of portal veins, hepatic venous outflow obstruction, aberrant portal or periportal vessels, and ductular proliferation may be of value in diagnosing IPH as the incidence rate of these features was at approximately 50%.

Abstract:This paper presents the case of a 73-year-old female, consented to the publication of case details and images, who presented with a two-month history of progressively worsening abdominal distension. During hospitalization, she complained of vomiting and pain in the left lower abdomen, and intermittent melena was detected. Abstract:Esophageal varices develop as a sequalae of portal hypertension and have a high risk of fatality. Causes of portal hypertension can be divided into pre-hepatic, hepatic, and post-hepatic. Pre-hepatic causes include portal vein thrombosis and post-hepatic causes include heart failure. Hepatic causes can be pre-sinusoidal, sinusoidal, and post-sinusoidal. Of these, cirrhosis, which occurs at the level of sinusoids, is most common; 80%-90% of cases of portal hypertension are due to cirrhosis. Obliterative portal venopathy (OPV) is a non-cirrhotic cause of portal hypertension that occurs at the level of the pre-sinusoids. Despite its serious complications, OPV is not well understood and is sparingly reported in the literature. Here, we discuss a case of esophageal variceal bleed in setting of biopsy proven OPV due to 6-Mercaptopurine (6MP) use.

Abstract:AbstractArterioportal fistulas (APFs) are defined as vascular communications between the systemic arteries and portal circulation. APFs are classified as congenital or acquired, large or small, intrahepatic or extrahepatic, and traumatic or spontaneous. The consistent abnormal connection may result in severe presinusoidal portal hypertension, leading to gastrointestinal bleeding (GIB), ascites, elevated liver function enzymes, heart failure, diarrhea, and even pancreatitis or hematobilia in some unusual cases. Indeed, less than half reported cases have GIB, predominately caused by variceal rupture. Peptic ulcer bleeding has rarely been reported in the setting of APFs. Herein, we describe a case in which intrahepatic APF manifested as gastric ulcer bleeding, progressive portal thrombosis, and massive ascites, with these symptoms successfully relieved by endovascular treatment.

Abstract:Portal hypertension is a common disease in surgical practice, a condition which occurs rarely in patients with myeloproliferative disorders (MPD). Portal hypertension secondary to MPD is often misdiagnosed. Three patients with portal hypertension secondary to MPD were admitted to Renji Hospital, Shanghai Jiaotong University School of Medicine from 1989 to 2006. The medical records of the three patients were reviewed for clinical manifestations, laboratory tests, angiography, bone marrow biopsy and operative findings. Abstract:Intrahepatic arterioportal fistula (IAPF) can be caused by many secondary factors. We report four cases of portal hypertension that were eventually determined to be caused by congenital hepatic arterioportal fistula. The clinical manifestations included ascites, variceal hemorrhage and hepatic encephalopathy. Computed tomography scans from all of the patients revealed the early enhancement of the portal branches in the hepatic arterial phase. All patients were diagnosed using digital subtraction angiography (DSA). DSA before embolization revealed an arteriovenous fistula with immediate filling of the portal venous radicles. All four patients were treated with interventional embolization. The four patients remained in good condition throughout follow-up and at the time of publication. IAPF is frequently misdiagnosed due to its rarity; therefore, clinicians should consider IAPF as a potential cause of non-cirrhotic portal hypertension.
