Abstract:Naturally occurring mutations in the skeletal muscle Ca2+ release channel/ryanodine receptor RyR1 are linked to malignant hyperthermia (MH), a life-threatening complication of general anesthesia. Although it has long been recognized that MH results from uncontrolled or spontaneous Ca2+ release from the sarcoplasmic reticulum, how MH RyR1 mutations render the sarcoplasmic reticulum susceptible to volatile anesthetic-induced spontaneous Ca2+ release is unclear. Here we investigated the impact of the porcine MH mutation, R615C, the human equivalent of which also causes MH, on the intrinsic properties of the RyR1 channel and the propensity for spontaneous Ca2+ release during store Ca2+ overload, a process we refer to as store overload-induced Ca2+ release (SOICR). Single channel analyses revealed that the R615C mutation markedly enhanced the luminal Ca2+ activation of RyR1. Moreover, HEK293 cells expressing the R615C mutant displayed a reduced threshold for SOICR compared with cells expressing wild type RyR1. Furthermore, the MH-triggering agent, halothane, potentiated the response of RyR1 to luminal Ca2+ and SOICR. Conversely, dantrolene, an effective treatment for MH, suppressed SOICR in HEK293 cells expressing the R615C mutant, but not in cells expressing an RyR2 mutant. These data suggest that the R615C mutation confers MH susceptibility by reducing the threshold for luminal Ca2+ activation and SOICR, whereas volatile anesthetics trigger MH by further reducing the threshold, and dantrolene suppresses MH by increasing the SOICR threshold. Together, our data support a view in which altered luminal Ca2+ regulation of RyR1 represents a primary causal mechanism of MH.

The Impact of Myosin Heavy Chain Isoforms on Contractile Behavior of the Heart

Abstract:In mammalian hearts, two distinct isoforms of myosin heavy chain (MHC), α-MHC and β-MHC, have been identified in cardiac myocytes. Previous studies have shown that the composition of the two myocardial MHC isoforms may change under pathophysiological conditions. Despite 93% similarity in amino acids, the two isoforms exhibit different biochemical and mechanical properties. In vitro studies have shown that increased expression of β-MHC can significantly slow the rate of cross-bridge recruitment and detachment in skinned myocardium [2]. However, the impact of this change on in vivo ventricular function has yet to be determined. In the present study, we evaluated changes in contractile behavior of rats with shifted MHC expression induced by thyroid-deficiency. Our results show that increased expression of β-MHC not only reduced the magnitude of peak systolic strain and torsion, but also altered the timing when the myocardium reached peak systole, leading to deteriorated cardiac function in hypothyroid rat hearts.
An Association between OXPHOS-Related Gene Expression and Malignant Hyperthermia Susceptibility in Human Skeletal Muscle Biopsies

Abstract:Malignant hyperthermia (MH) is a pharmacogenetic condition of skeletal muscle that manifests in hypermetabolic responses upon exposure to volatile anaesthetics. This condition is caused primarily by pathogenic variants in the calcium-release channel RYR1, which disrupts calcium signalling in skeletal muscle. However, our understanding of MH genetics is incomplete, with no variant identified in a significant number of cases and considerable phenotype diversity. In this study, we applied a transcriptomic approach to investigate the genome-wide gene expression in MH-susceptible cases using muscle biopsies taken for diagnostic testing. Baseline comparisons between muscle from MH-susceptible individuals (MHS, n = 8) and non-susceptible controls (MHN, n = 4) identified 822 differentially expressed genes (203 upregulated and 619 downregulated) with significant enrichment in genes associated with oxidative phosphorylation (OXPHOS) and fatty acid metabolism. Investigations of 10 OXPHOS target genes in a larger cohort (MHN: n = 36; MHS: n = 36) validated the reduced expression of ATP5MD and COQ6 in MHS samples, but the remaining 8 selected were not statistically significant. Further analysis also identified evidence of a sex-linked effect in SDHB and UQCC3 expression, and a difference in ATP5MD expression across individuals with MH sub-phenotypes (trigger from in vitro halothane exposure only, MHSh (n = 4); trigger to both in vitro halothane and caffeine exposure, MHShc (n = 4)). Our data support a link between MH-susceptibility and dysregulated gene expression associated with mitochondrial bioenergetics, which we speculate plays a role in the phenotypic variability observed within MH.

Impaired Interaction Between Skeletal Ryanodine Receptors In Malignant Hyperthermia

Abstract:Functional coupling between clustered membrane receptors has been identified as a novel mechanism to improve signaling performance in a number of physiological processes. The potential role of defective inter-receptor coupling in the pathogenesis of disease, however, has not previously been explored. Ryanodine receptors (RyRs), the primary calcium release channel of muscle, usually form ordered two-dimensional arrays in the sarcoplasmic reticulum membranes. Mutations in RyRs are known to cause a number of severe diseases both in skeletal muscle and in heart. Here we present a model demonstrating how impaired functional coupling between neighboring mutant RyR1(R615C) channels may contribute to the pharmacogenetic skeletal muscle sensitivity, malignant hyperthermia (MH). We find that purified RyR1(R615C) from MH susceptible porcine skeletal muscle shows significantly reduced oligomerization when compared to RyR1(WT), indicating a potential loss of intrinsic intermolecular control. The MH-triggering volatile anesthetic, halothane, activates RyR1(R615C) and RyR1WT to a similar extent, using [H-3] ryanodine binding as a measure of activation. Modeling RyR1 array function with parameters modified to simulate the loss of functional inter-RyR coupling recapitulates the MH molecular phenotype-RyR1 channels leaky to Ca2+ at rest and long open-times following exposure to halothane. Our work suggests that a defect in inter-RyR1 coupling is a novel direction for research into the pathogenesis of MH.
Heat- and anesthesia-induced malignant hyperthermia in an RyR1 knock-in mouse.

Abstract:Malignant hyperthermia (MH) is a life-threatening disorder characterized by skeletal muscle rigidity and elevated body temperature in response to halogenated anesthetics such as isoflurane or halothane. Mutation of tyrosine 522 of RyR1 (the predominant skeletal muscle calcium release channel) to serine has been associated with human malignant hyperthermia. In the present study, mice created harboring this mutation were found to represent the first murine model of human malignant hyperthermia. Mice homozygous for the Y522S mutation exhibit skeletal defects and die during embryonic development or soon after birth. Heterozygous mice, which correspond to the human occurrence of this mutation, are MH susceptible, experiencing whole body contractions and elevated core temperatures in response to isoflurane exposure or heat stress. Skeletal muscles from heterozygous mice exhibit increased susceptibility to caffeine- and heat-induced contractures in vitro. In addition, the heterozygous expression of the mutation results in enhanced RyR1 sensitivity to activation by temperature, caffeine, and voltage but not uncompensated sarcoplasmic reticulum calcium leak or store depletion. We conclude that the heterozygous expression of the Y522S mutation confers susceptibility to both heat- and anesthetic-induced MH responses.

Abstract:Temperature has a significant impact on the performance of the neuromuscular system and motor control processes. The smallest functional components of these systems are motor units (MUs), which may differ significantly between different muscles. The influence of temperature on the contractile properties of slow-twitch (S) MUs from soleus (SOL) muscles in rats was investigated under hypothermia (25 °C), normothermia (37 °C), and hyperthermia (41 °C). Hypothermia prolonged the twitch time parameters, decreased the rate of force development, increased the twitch-to-tetanus ratio, enhanced twitch force, and abolished post-tetanic depression. In contrast, hyperthermia did not alter twitch time parameters. Moreover, there was no effect on force despite the noted increase in post-tetanic depression and the twitch-to-tetanus ratio. Therefore, hypothermia induced more profound changes in S MUs compared with hyperthermia. The temperature effects in SOL MUs were compared to the effects previously reported for S MUs in the medial gastrocnemius (MG). The major differences between the S MUs of both muscles were the effects of temperature on twitch force, post-tetanic force modulation, twitch-to-tetanus ratio, and the slope of the force-frequency curve under hypothermia. Hyperthermia shortened twitch time parameters solely in the MG. In contrast, post-tetanic depression, twitch-to-tetanus ratio, and the slope of the force-frequency curve were influenced by hyperthermia only in SOL MUs. The different temperature effects of S MUs probably corresponded to differences in muscle architecture and their diverse functional tasks and enzyme activity. In summary, S MUs in SOL are more thermal-sensitive than their counterparts in MG.

The caffeine test of isolated human muscle in relation to malignant hyperthermia

Abstract:Summaryo| li]1.|Pharmacological tests of isolated fibers of skeletal muscle proved to be a means of clarifying the occurrence of certain genetic defects. li]2.|Muscle specimens from 74 subjects were investigated. Of these, 14 had recovered from an episode of malignant hyperthermia (MH), an often fatal complication of general anaesthesia which is known to occur on the basis of a genetic predisposition. The others were 25 relatives of MH patients, and 35 subjects unrelated to malignant hyperthermia. li]3.|The tests consisted in measuring the contracture tensions of isolated fibres of skeletal muscle when exposed to various concentrations of caffeine, once in the presence, once in the absence of the anaesthetic agent halothane. The method is described in detail. li]4.|Muscle specimens which in the presence of halothane required more than 1.3 millimolar concentration of caffeine to produce a tension increase of 1 gram were defined as controls. The 45 control data thus defined were normally distributed. Age, sex, type of muscle, and size of the specimen did not account for individual difference within this control population. The control group did not contain any MH patient who had developed rigidity during the attack. On the other hand, it did contain all but one of the specimens from MH-unrelated subjects. li]5.|The acceptance of the control data as a standard left a deviant group of 29 subjects. This group could be subdivided according to the caffeine-contracture in the absence of halothane.All those MH patients who had displayed some rigidity during their attack were in this deviant group; they were characterized by high sensitivity to caffeine. The deviant group included two subjects who had suffered an MH episode without rigidity; they differed from all others by a high resistance to caffeine contracture and thereby seemed to represent a separate category of MH which could not be further analyzed during the studies reported here.The deviant group contained 10 relatives of MH patients who differed in their caffeine response from the patients themselves. Hence there are at least three kinds of relatives of MH patients: those that are indistinguishable from patients, those that fall into the control group, and an intermediate group. One of the MH-unrelated subjects reacted like an intermediate, suggesting that this represents a defect which is not rare in the general population. li]6.|When these data were seen in the context of family groups, there was evidence of a recessively inherited predisposition to malignant hyperthermia. However, part of the data were also compatible with the repeatedly published evidence of dominant inheritance of this predisposition. Since multigenic inheritance appears to be excluded, the simplest genetic models which can reasonably account for the observations are either a two-gene case or the assumption of one gene locus with three alleles. Improvement of the test methods or an increase of the data base should allow a choice between genetic theories.

Abstract:To the Editor: Infection with Salmonella enterica serovar Agbeni is rare. In Canada, it was reported 8 times during 2000–2010 and never in the province of British Columbia (2011 population 4.5 million) (Public Health Agency of Canada, unpub. data). In June 2011, an outbreak of S. enterica ser. Agbeni affecting 8 persons was identified in British Columbia; pulsed-field gel electrophoresis patterns for all isolates were identical. Although no specific source was identified, 2 features were noted: 1) diagnosis through urine specimens for 3 of 8 persons and 2) a longer than typical incubation period for Salmonella spp. infection.

Abstract:Historical data, epidemiological surveys, laboratory and animal experiments, clinical studies and observations in ordinary dental practice all illustrate that starchy fibrous foods have a low cariogenic potential. The recommended substitution of sucrose in Western diets by starchy foods would lead to a reduction of caries provided that these other carbohydrates replace the between meal consumption of sugar-rich products. The low cariogenic potential of a diet containing carbohydrates with a fibrous character is probably not solely due to its low sucrose content. Such a diet promotes mastication and saliva secretion which tend to reduce the caries risk. Furthermore, unrefined foods may contain caries-protective substances like phytates, trace elements and antibacterial factors. Lectins, which are found in many plants could be of importance by influencing the attachment of micro-organisms to the teeth in a favourable manner. The tradition to end a meal with fruit may thus have other oral effects than hitherto considered. In addition, a diet containing carbohydrates with a fibrous character and a low amount of sucrose probably has a low pathogenic potential for the periodontal tissues. However, no epidemiological data are available to support this assumption.
Influence of environmental temperature and energy intake on skeletal muscle respiratory enzymes and morphology

Abstract:Influence of a cold (10 degrees C) or warm (35 degrees C) environment and a high or low level of energy intake on respiratory enzyme activities has been investigated in porcine skeletal muscle. Scanning microdensitometry was used to measure the reaction products from mitochondrial enzymes in individual slow- and fast-twitch muscle fibres. A cold environment was found to increase the activity of succinate dehydrogenase in both types of muscle fibre (P less than 0.001 for dark fibres, P less than 0.01 for light fibres) from young growing animals. Enzyme activity was also increased in animals on a low compared with a high energy intake (P less than 0.01) when living at 10 degrees C but not at 35 degrees C. Similar findings were obtained for NADH diaphorase and cytochrome oxidase aa3. The numbers of slow-twitch muscle fibres also increased after exposure to cold (P less than 0.01) and as a result of a low energy intake (P less than 0.01). These results are similar to those obtained in other species after exercise or as a result of peripheral arterial insufficiency. The extent to which they could be related to local tissue hypoxia or to changes in metabolic hormones is discussed.

Abstract:For physiological processes in the vital organs of eutherian mammals to function, it is important to maintain constant core body temperature at ∼37°C. Mammals generate heat internally by thermogenesis. The focus of this review is on heat generated in resting skeletal muscles, using the same cellular components that muscles use to regulate cytoplasmic calcium concentrations [Ca2+] and contraction. Key to this process, known as muscle-based nonshivering thermogenesis (MB-NST), are tiny Ca2+ movements and associated ATP turnover coordinated by the plasma membrane, sarcoplasmic reticulum (SR), and the mitochondria. MB-NST has made mammals with gain-of-function SR ryanodine receptor (RyR) variants vulnerable to excessive heat generation that can be potentially lethal, known as malignant hyperthermia. Studies of RyR variants using recently developed techniques have advanced our understanding of MB-NST.

Abstract:Susceptibility to malignant hyperthermia is diagnosed by contracture test, which requires an open muscle biopsy sample. We postulated that intramuscular injection of caffeine increases local carbon dioxide pressure (pCO(2)) in individuals susceptible to malignant hyperthermia but not in those who are non-susceptible or in healthy individuals. We measured pCO(2) in the rectus femoris muscle during local stimulation with 500 microL caffeine 80 mmol/L in 12 patients susceptible to malignant hyperthermia, eight non-susceptible individuals, and seven healthy controls. In susceptible individuals, pCO(2) temporarily increased to a maximum of 63 mm Hg (IQR 61-69) compared with 44 mm Hg (43-47) in non-susceptible participants (p=0.003) and 42 mm Hg (41-46) in controls (p=0.003). There were no systemic side-effects. Metabolic monitoring of the highly diffusible CO(2) stimulated by caffeine might allow a minimally invasive test for susceptibility to malignant hyperthermia.

Transcriptomic Response of Differentiating Porcine Myotubes to Thermal Stress and Donor Piglet Age

Abstract:Climate change is a current concern that directly and indirectly affects agriculture, especially the livestock sector. Neonatal piglets have a limited thermoregulatory capacity and are particularly stressed by ambient temperatures outside their optimal physiological range, which has a major impact on their survival rate. In this study, we focused on the effects of thermal stress (35 °C, 39 °C, and 41 °C compared to 37 °C) on differentiating myotubes derived from the satellite cells of Musculus rhomboideus, isolated from two different developmental stages of thermolabile 5-day-old (p5) and thermostable 20-day-old piglets (p20). Analysis revealed statistically significant differential expression genes (DEGs) between the different cultivation temperatures, with a higher number of genes responding to cold treatment. These DEGs were involved in the macromolecule degradation and actin kinase cytoskeleton categories and were observed at lower temperatures (35 °C), whereas at higher temperatures (39 °C and 41 °C), the protein transport system, endoplasmic reticulum system, and ATP activity were more pronounced. Gene expression profiling of HSP and RBM gene families, which are commonly associated with cold and heat responses, exhibited a pattern dependent on temperature variability. Moreover, thermal stress exhibited an inhibitory effect on cell cycle, with a more pronounced downregulation during cold stress driven by ADGR genes. Additionally, our analysis revealed DEGs from donors with an undeveloped thermoregulation capacity (p5) and those with a fully developed thermoregulation capacity (p20) under various cultivation temperature. The highest number of DEGs and significant GO terms was observed under temperatures of 35 °C and 37 °C. In particular, under 35 °C, the DEGs were enriched in insulin, thyroid hormone, and calcium signaling pathways. This result suggests that the different thermoregulatory capacities of the donor piglets determined the ability of the primary muscle cell culture to differentiate into myotubes at different temperatures. This work sheds new light on the underlying molecular mechanisms that govern piglet differentiating myotube response to thermal stress and can be leveraged to develop effective thermal management strategies to enhance skeletal muscle growth.
Identification of Metabonomics Changes in Longissimus Dorsi Muscle of Finishing Pigs Following Heat Stress through LC-MS/MS-Based Metabonomics Method

Abstract:Heat stress (HS) negatively affects meat quality by affecting material and energy metabolism, and exploring the mechanism underlying the muscle response to chronic HS in finishing pigs is important for the global pork industry. This study investigated changes in the metabolic profiles of the longissimus dorsi (LD) muscle of finishing pigs under high temperature using ultra-performance liquid chromatography coupled with electrospray ionization quadrupole time-of-flight mass spectrometry (UPLC-ESI-QTOF-MS) and multivariate data analysis (MDA). Castrated male DLY pigs (Duroc × Landrance × Yorkshire pigs, n = 24) from 8 litters were divided into three treatment groups: constant optimal ambient temperature at 22 °C and ad libitum feeding (CR, n = 8); constant high ambient temperature at 30 °C and ad libitum feeding (HS, n = 8); and constant optimal ambient temperature 22 °C and pair-feeding to the control pigs (PF, n = 8). The metabolic profile data from LD muscle samples were analyzed by MDA and external search engines. Nine differential metabolites (L-carnosine, acetylcholine, inosinic acid, L-carnitine, L-anserine, L-α-glycerylphosphorylcholine, acetylcarnitine, thiamine triphosphate, and adenosine thiamine diphosphate) were involved in antioxidant function, lipid metabolism, and cell signal transduction, which may decrease post mortem meat quality and play important roles in anti-HS. Four metabolites (L-carnosine, acetylcholine, inosinic acid, and L-carnitine) were verified, and it was indicated that the muscle L-carnitine content was significantly lower in HS than in CR (p < 0.01). The results show that constant HS affects the metabolites in the LD muscle and leads to coordinated changes in the endogenous antioxidant defense and meat quality of finishing pigs. These metabonomics results provide a basis for researching nutritional strategies to reduce the negative effects of heat stress on livestock and present new insights for further research.

Abstract:Mutations in the skeletal muscle ryanodine receptor gene (RYR1) can cause susceptibility to malignant hyperthermia (MH), a potentially lethal genetic condition triggered by volatile anesthetics. MH is associated with hypermetabolism, which has directed research interest into oxidative phosphorylation and muscle bioenergetics. The most common cause of MH in the United Kingdom is the c.7300G>A RYR1 variant, which is present in ∼16% of MH families. Our study focuses on the MH susceptible G2435R-RYR1 knock-in mouse model, which is the murine equivalent of the human c.7300G>A genotype. Using a combination of transcriptomics, protein expression, and functional analysis, we investigated adult muscle fiber bioenergetics in this mouse model. RNA-Seq data showed reduced expression of genes associated with mitochondria and fatty acid oxidation in RYR1 mutants when compared with WT controls. Mitochondrial function was assessed by measuring oxygen consumption rates in permeabilized muscle fibers. Comparisons between WT and homozygous G2435R-RYR1 mitochondria showed a significant increase in complex I-facilitated oxidative phosphorylation in mutant muscle. Furthermore, we observed a gene-dose-specific increase in reactive oxygen species production in G2435R-RYR1 muscle fibers. Collectively, these findings provide evidence of metabolic defects in G2435R-RYR1 knock-in mouse muscle under basal conditions. Differences in metabolic profile could be the result of differential gene expression in metabolic pathways, in conjunction with mitochondrial damage accumulated from chronic exposure to increased oxidative stress.

Abstract:Structural motifs of pre-nucleation clusters prepared in single, optically levitated supersaturated aqueous aerosol microparticles containing CaBr2 as a model system are reported. Cluster formation is identified by means of X-ray absorption in the Br K-edge regime. The salt concentration beyond the saturation point is varied by controlling the humidity in the ambient atmosphere surrounding the 15-30 μm microdroplets. This leads to the formation of metastable supersaturated liquid particles. Distinct spectral shifts in near-edge spectra as a function of salt concentration are observed, in which the energy position of the Br K-edge is red-shifted by up to 7.1 ± 0.4 eV if the dilute solution is compared to the solid. The K-edge positions of supersaturated solutions are found between these limits. The changes in electronic structure are rationalized in terms of the formation of pre-nucleation clusters. This assumption is verified by spectral simulations using first-principle density functional theory and molecular dynamics calculations, in which structural motifs are considered, explaining the experimental results. These consist of solvated CaBr2 moieties, rather than building blocks forming calcium bromide hexahydrates, the crystal system that is formed by drying aqueous CaBr2 solutions.
