Abstract:<span>Asymptomatic SARS-CoV-2-infected individuals are thought to play major roles in virus transmission. This study aimed to analyze the characteristics of asymptomatic carriers with COVID-19 to control the spread of the virus. We retrospectively investigated the clinical characteristics of 648 consecutive subjects who were enrolled in the study and were divided into asymptomatic carriers, mild cases, ordinary cases, severe or critical cases, and evaluated their impact on disease severity by means of Spearman correlation and multiple regression analyses. Receiver operating characteristic curve analysis was conducted to determine the optimum cutoff levels of laboratory findings for diagnostic predictors of asymptomatic carriers of COVID-19. In our study, a total of 648 subjects on admission with a mean age of 45.61 y including 345 males and 303 females were enrolled in our study. The leukocyte, lymphocyte, eosinophil, platelet, C-reactive protein, interleukin-6, CD3+, CD4+, and CD8 + T lymphocyte levels, and the erythrocyte sedimentation rate differed significantly among the groups (all <i>p</i> ≤ 0.05). Disease severity was negatively associated with the CD3+ (<i>r</i> = −0.340; <i>p</i> &lt; 0.001), CD4+ (<i>r</i> = −0.290; <i>p</i> = 0.001) and CD8+ (<i>r</i> = −0.322; <i>p</i> &lt; 0.001) T lymphocyte levels. The significant diagnostic predictors of asymptomatic carriers of COVID-19 included the blood cell, cytokine, and T lymphocyte subset levels. Inflammation and immune response may play important roles in disease progression. Hence, the laboratory parameters identified should be considered in clinical practice, which provide new insights into the identification of asymptomatic individuals and the prevention of virus transmission.</span>

What problem does this paper attempt to address?
The main problem that this paper attempts to solve is to analyze the clinical characteristics of asymptomatic carriers of Coronavirus Disease 2019 (COVID - 19) in order to control the spread of the virus. Specifically, the study aims to evaluate the impact of these characteristics on the disease severity by retrospectively investigating the clinical characteristics of 648 consecutive subjects (divided into asymptomatic carriers, mild, moderate, severe or critical cases) in Jiangsu province. The study used Spearman correlation analysis and multiple regression analysis to evaluate the relationship between disease severity and laboratory test results, and also carried out Receiver Operating Characteristic (ROC) curve analysis to determine the optimal cut - off values of diagnostic predictors, thus helping to identify and screen asymptomatic infected people. ### Main research objectives: 1. **Analyze the clinical characteristics of asymptomatic carriers**: Study the clinical manifestations, laboratory test results and other characteristics of asymptomatic carriers to understand the specific situation of this group of people. 2. **Evaluate the influencing factors of disease severity**: Through statistical analysis, explore how different clinical characteristics and laboratory indicators affect the severity of the disease. 3. **Determine diagnostic predictors**: Use ROC curve analysis to find the optimal cut - off values of laboratory indicators that can effectively distinguish asymptomatic carriers, providing a reference for clinical diagnosis. ### Research methods: - **Data collection**: From January 23, 2020 to March 11, 2020, medical records of 648 patients from 25 hospitals in Jiangsu province were collected, including epidemiological data, demographic data, clinical characteristics, imaging characteristics and key laboratory parameters. - **Patient classification**: According to the sixth edition of the "Diagnosis and Treatment Plan for Coronavirus Pneumonia" issued by the National Health Commission of China, patients were divided into asymptomatic carriers, mild, moderate, severe or critical cases. - **Statistical analysis**: Data analysis was carried out using SPSS software, including descriptive statistics, analysis of variance (ANOVA), Kruskal - Wallis H test, chi - square test, Spearman correlation analysis and multiple regression analysis. ### Key findings: - **Differences in clinical characteristics**: There are significant differences in symptoms such as age, admission body temperature, fever, nasal congestion, headache, cough, sore throat, expectoration, fatigue, dyspnea, myalgia or arthralgia, chills and pharyngeal congestion between asymptomatic carriers and other types of patients. - **Differences in laboratory indicators**: There are significant differences in white blood cell count, lymphocyte count, eosinophil count, platelet count, C - reactive protein, interleukin - 6, CD3 +, CD4 + and CD8 + T - lymphocyte levels and erythrocyte sedimentation rate among different groups. - **Inflammation and immune response**: Changes in the levels of inflammatory markers and T - lymphocyte subsets play an important role in disease progression. - **Diagnostic predictors**: ROC curve analysis determined the optimal cut - off values of indicators such as white blood cell count, lymphocyte count, eosinophil count and platelet count, which are helpful for identifying asymptomatic carriers. ### Conclusion: This study provides important information on the clinical characteristics of asymptomatic carriers, emphasizes the role of inflammation and immune response in disease progression, and proposes laboratory parameters that can be used to identify asymptomatic carriers in clinical practice. These findings are of great significance for preventing virus spread and controlling the epidemic.
