Abstract:Aims: To determine the pregnancy outcome of early and late onset preeclampsia. Methods: This was a cross sectional analytical study conducted in the department of Obstetrics and Gynaecology, Manipal Teaching Hospital from July to October 2021. All cases of preeclampsia diagnosed according to International Society of Study of Hypertension in Pregnancy 2018, were included. Early and late onset preeclampsia cut-off used was 34 weeks. Preeclampsia diagnosed before 34 were classified as early onset and after 34 weeks as late onset preeclampsia. Maternal and perinatal outcomes were analyzed using Statistical Package for Social Sciences version 21. Results: The prevalence of early onset and late onset preeclampsia were 4.3% and 8.3% of all deliveries. Early to late onset preeclampsia were 1:2. Renal involvement, placental abruption, IUGR, low birth weight, low Apgar and perinatal morbidity were significantly more in early onset; pre-term labor and use of MgSO4 and antihypertensives were also more in early onset. Conclusion: Late onset preeclampsia was more common than early onset preeclampsia but the maternal and perinatal outcome were poor for early onset as compared to late onset preeclampsia.

Abstract:<span>To compare Doppler alterations and perinatal outcomes in pregnant women who evolved with early- and late-onset fetal growth restriction (FGR).A retrospective, observational cohort study with pregnant women who evolved with FGR treated between January 2018 and April 2019, in which all live births from singleton pregnancies, over 24 weeks, with FGR ultrasound diagnosis and under 2700 g weight were included in the study.Pregnancies with early-onset FGR were more associated with hypertensive disorders (<i>p</i> = .00) and placental vascular insufficiency, resulting in a high degree of umbilical artery Doppler involvement (<i>p</i> = .00) in a short period of pregnancy and higher rates of adverse perinatal outcomes (<i>p</i> = .00). The time of prenatal follow-up of early- and late-onset FGR cases was similar, but the degree of prematurity of the former made the evolution more unfavorable.Early-onset FGR had a lower prevalence but was associated with higher maternal and fetal morbidity and mortality than late-onset FGR.</span>

Abstract:OBJECTIVE: To compare perinatal outcomes on fetuses classified as stage I late-onset fetal growth restriction (FGR) depending on the estimated fetal weight (EFW) centile category and the fetal and maternal Doppler study.MATERIAL AND METHODS: Retrospective cohort study on 131 cases of stage I late-onset FGR (diagnosis ≥32+0 weeks), defined as: EFW < 3rd centile and normal Doppler (G1) or EFW < 10th centile and mean uterine artery pulsatility index (PI) > 95th centile (G2) or EFW < 10th centile and mild fetal Doppler alteration: umbilical artery PI > 95th centile, middle cerebral artery PI < 5th centile, or cerebroplacental ratio < 5th centile (G3). All groups were compared to their perinatal results.RESULTS: There were 37, 30, and 64 cases in G1, G2, and G3, respectively. G1 and G2 showed lower percentages of cesarean section when compared with G3 (18.4, 22.5, and 45.3% (p < 0.01), respectively), being attributable to an excess of cesarean sections for non-reassuring fetal status. These differences remained when definitive birth weight centile was above that considered to define FGR, being 5.9, 12.5, and 41.8% (p < 0.01), respectively.DISCUSSION: In stage I late-onset FGR fetuses, abnormal fetal Doppler is associated with a poorer tolerance to vaginal delivery, even when the birth weight is > 10th centile.

Abstract:<h3 class="article-section__sub-title section1"> Objective</h3><p>Pregnancies complicated by late‐onset fetal growth restriction (FGR) are at increased risk of short‐ and long‐term morbidities. Despite this, identification of cases at higher risk of adverse perinatal outcome when FGR is diagnosed is challenging. The aims of this study were to elucidate the strength of association and predictive accuracy of maternal and fetal Dopplers in predicting perinatal outcome of fetuses affected by late‐onset FGR at the time of diagnosis.</p><h3 class="article-section__sub-title section1"> Methods</h3><p>Prospective study of consecutive singleton pregnancies complicated by late‐onset FGR. Estimated fetal weight (EFW), pulsatility index (PI) in the uterine, umbilical (UA), and middle cerebral (MCA) arteries, cerebro‐placental ratio (CPR) and umbilical vein flow (normalized for fetal abdominal circumference) (UVBF/AC) were recorded at the time of the diagnosis of FGR. Composite adverse outcome was defined as the occurrence of either emergency caesarean section (CS) for fetal distress, 5' Apgar score < 7, umbilical artery pH < 7.10 and neonatal admission to special care unit. Logistic regression and area under the curve (AUC) analyses were used to elucidate the strength of association and diagnostic accuracy of different ultrasound parameters for the prediction of composite adverse outcome.</p><h3 class="article-section__sub-title section1"> Results</h3><p>243 consecutive singleton pregnancies complicated by late‐onset FGR were included in the study. Composite adverse perinatal outcome occurred 32.5% (95% CI 26.7‐38.8) of cases. The z values of mean uterine artery PI (2.23±1.34 vs. 1.988±0.89, p= 0.02) were higher, while the z‐values of UVBF/AC (‐1.93±0.88 vs. 0.89±0.94 p≤0.0001), MCA PI (‐1.56±0.93 vs. ‐1.22±0.84, p=0.004) and CPR (‐1.89±1.12 vs. ‐1.44±1.02, p=0.002) were lower in pregnancies who experienced compared to those who did not experience composite adverse outcome. On multivariable logistic regression analysis, mean uterine artery PI (p= 0.04), CPR (p= 0.02) and UVBF/AC (p= 0.001) were independently associated with composite adverse perinatal outcome. UVBF/AC had an AUC of 0.723 (95% CI 0.64‐0.80) for composite adverse outcome and with a better accuracy compared to mean uterine artery (AUC: 0.593), CPR (AUC: 0.615). A multiparametric prediction model including MCA, Uterine arteries and UVBF had an AUC of 0.745 (95% CI 0.66‐0.83) for the prediction of composite adverse outcome.</p><h3 class="article-section__sub-title section1"> Conclusion</h3><p>CPR and uterine artery Dopplers are independently associated with composite adverse perinatal outcome in pregnancies complicated by late‐onset FGR at the time of diagnosis. Despite this, their diagnostic performance for adverse perinatal outcome is low. UVBF/AC showed a better accuracy for composite adverse outcome, although its usefulness in clinical practice as standalone test for adverse pregnancy outcome requires further evidence.</p><p>This article is protected by copyright. Abstract:Objective: The aim of this study is to clarify the role of different Doppler parameters such as umbilicocerebral ratio (UCR), cerebroplacentouterine ratio (CPUR), aortic isthmus, renal artery, and umbilical vein flow Doppler in predicting adverse neonatal outcomes in fetuses with late -onset fetal growth restriction.Materials and Methods: The study included all patients diagnosed with fetal growth restriction at 32-39 weeks' gestation between 01/02/2020 and 01/02/2022 and treated at the Department of Obstetrics and Gynecology, Inonu University School of Medicine.Results: Patients included in the study had a median gestational week at delivery of 37 (minimum 33+0-maximum 39+0), median CPR of 1.42 (minimum-maximum 0.43-3.57), and median UCR of 0.7 (minimum-maximum 0.28-2.3). Receiver operating characteristic analysis was performed to determine the performance of the measured obstetric Doppler parameters in predicting the development of adverse neonatal outcomes. Umbilical venous blood flow showed the best performance in predicting adverse neonatal outcomes [area under the curve 0.952, 95% confidence interval (CI) 0.902-0.981, p<0.001]. Multivariate logistic regression analysis showed that fetuses with abnormal CPUR had a 4.5-fold (95% CI 0.084-0.583, p=0.02) increased risk of adverse neonatal outcome, whereas fetuses with abnormal umbilical venous flow had a 1.07-fold (95% CI 0.903-0.968, p<0.001) increased risk of adverse neonatal outcome.Conclusion: The results of this study demonstrate that the use of UCR, CPUR, umbilical venous flow, and aortic isthmus PI Doppler parameters along with umbilical artery PI and CPR are effective in predicting adverse neonatal outcomes in fetuses with late -onset fetal growth restriction.
Maternal Utero-Placental Perfusion Discordance in Monochorionic-Diamniotic Twin Pregnancies with Selective Growth Restriction Assessed by Three-Dimensional Power Doppler Ultrasound

Abstract:Background: The aim of this study was to assess the correlation between selective growth restriction (sGR) and co-twin utero-placental perfusion discordance by using three-dimensional power Doppler (3DPD). Material/Methods: We prospectively recruited 60 sGR and 64 normal monochorionic-diamniotic (MCDA) twin pregnancies. Vascularization index (VI), flow index (FI), and vascularization flow index (VFI) were assessed by 3DPD, while umbilical artery pulsatility index (UA-PI), middle cerebral artery peak systolic velocity (MCA-PSV), pulsatility index (MCA-PI), and cerebroplacental ratio (CPR) were assessed by conventional Doppler imaging. Results: In sGR co-twins, the VI, FI, VFI, MCA-PI, and CPR were significantly lower, while the UA-PI and MCA-PSV were significantly greater, in the smaller fetuses compared with the larger fetuses; significant differences were also observed in the VI, FI, VFI, CPR, and UA-PI in normal co-twins. Compared with the appropriately grown twins, the discordances of the VI, FI, VFI, UA-PI, MCA-PI, and CPR were increased in the sGR cohort. The discordances of the VI, FI, VFI, UA-PI, MCA-PI, and CPR were associated with birthweight discordance, and the FI discordance and CPR discordance were independently associated with sGR. The combination of the FI and CPR discordance showed a higher predictive accuracy for sGR, with an area under the ROC curve of 0.813, and a sensitivity and specificity of 68.33% and 85.94%, respectively. Conclusions: MCDA twin pregnancies with birthweight discordance presented utero-placental perfusion deterioration assessed by 3DPD prior to sGR diagnosis. Co-twin utero-placental perfusion discordance was significantly correlated with growth discordance, and this correlation was more predictive of sGR when 3DPD was combined with conventional Doppler imaging.

Abstract:INTRODUCTION: Fetal growth abnormalities are among the main drivers of perinatal mortality and morbidity and a cornerstone of prenatal care. However they are defined based on arbitrary definitions. Our objective was to define fetal growth abnormalities based on increase risk of stillbirth. METHODS: In a prospective cohort of 35,251 singleton pregnancies, stillbirth was defined as fetal demise at > 20 weeks without chromosomal or structural abnormalities. Fetal growth was percentile of birth weight for gestational age at delivery. The risk of stillbirth as a function of percentile of fetal growth and gestational age was determined using logistic regression. The threshold in the relationship adjusted for gestational age at delivery was determined using the Bayesian information criterion to identify the point where the relationship’s strength changes. RESULTS: There were 61 (0.2%) non-anomalous stillbirths. At the 8.0th percentile of fetal growth the risk of stillbirth increased from 1.5 to 6.5 per 1,000 pregnancies. Risk of stillbirth at or below 8th percentile was 0.0065 (95% CI 0.0060-0.0070) and above the 8.0th percentile the risk of stillbirth was 0.0015 (95% CI 0.0013-0.0017); P<.0001 for both associations. CONCLUSION: Definition of fetal growth impairment based on the risk of stillbirth will result in fewer small but normal pregnancies to be misclassified as abnormally grown and thus decreasing unnecessary testing and interventions. Additionally, lower threshold 8th vs 10th percentile for defining fetal growth restriction would decrease by 2% of all pregnancies (80,000 per year in the US) the number of pregnancies requiring further testing and intervention.

Abstract:Small-for-gestational-age (SGA) infants have been associated with increased risk of adverse perinatal outcomes (APOs). In this work, we assess the predictive ability of the ultrasound-estimated percentile weight (EPW) at 35 weeks of gestational age to predict late-onset SGA and APOs, according to six growth standards, and whether the ultrasound–delivery interval influences the detection rate. To this purpose, we analyze a retrospective cohort study of 9585 singleton pregnancies. EPWs at 35 weeks were calculated to the customized Miguel Servet University Hospital (MSUH) and Figueras standards and the non-customized MSUH, Fetal Medicine Foundation (FMF), INTERGROWTH-21st, and WHO standards. As results of our analysis, for a 10% false positive rate, the detection rates for SGA ranged between 48.9% with the customized Figueras standard (AUC 0.82) and 60.8% with the non-customized FMF standard (AUC 0.87). Detection rates to predict SGA by ultrasound–delivery interval (1–6 weeks) show higher detection rates as intervals decrease. APOs detection rates ranged from 27.0% with FMF to 7.9% with the Figueras standard. In conclusion, the ability of EPW to predict SGA at 35 weeks is good for all standards, and slightly better for non-customized standards. The APO detection rate is significantly greater for non-customized standards.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Introduction</h3><p>Early‐onset fetal growth restriction is a pregnancy complication often coinciding with abnormal Doppler flow in the umbilical artery. Absent or reversed end‐diastolic flow in the umbilical artery is associated with adverse perinatal outcome. As the optimal management of this condition is unclear, the objective of this study was to analyze the time interval from admission to delivery of pregnancies with early‐onset fetal growth restriction, while pursuing a policy of postponing delivery unless active management of labor would be required because of fetal distress or maternal condition. We also assessed short‐ and long‐term perinatal outcome.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Material and methods</h3><p>In this historical cohort study, all pregnant women with singleton pregnancies, admitted during the period 2004‐2015 with early‐onset fetal growth restriction were included. Pregnancies with absent or reversed end‐diastolic flow (AREDF) were compared to pregnancies with a positive end‐diastolic Doppler flow (PEDF). Time until delivery was determined, and perinatal outcome was assessed for both groups.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>In our study, 111 women were allocated to the PEDF group and 109 to the AREDF group. In the AREDF group, fetal distress was more often an indication for delivery, in comparison to the PEDF group (<i>P</i>=0.004). Median time until delivery in patients admitted between 26‐28 weeks' gestation was six weeks and five days in the PEDF group and one week and four days in the AREDF group <i>(P</i>= 0.001). No statistically significant difference was found between the Doppler groups in the composite adverse neonatal outcome, which includes at least one of the following outcomes: infant respiratory distress syndrome, sepsis, bronchopulmonary dysplasia, necrotizing enterocolitis, intraventricular hemorrhage >grade 2, periventricular leukomalacia and perinatal death (<i>P</i>= 0.63). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>In this study, comprising pregnancies with early‐onset fetal growth restriction, fetal distress was observed more frequently in the AREDF group with the consequence of delivery at an earlier stage of gestation, compared to the PEDF group. AREDF was not associated with increased perinatal morbidity and mortality compared to PEDF.</p></section>

Abstract:Assessment of fetal growth and wellbeing is one of the major purposes of antenatal care. Some fetuses have smaller than expected growth in utero and while some of these fetuses are constitutionally small, others have failed to meet their growth potential, that is they are growth restricted. While severe growth restriction is uncommon, the consequences of it being undetected may include perinatal death or severe morbidity. It is, therefore, important to have strategies in place to detect the fetus at risk of growth restriction. These would include an assessment of 'prior risk' from maternal history and examination combined with the results of biochemical and ultrasound investigations, the most promising of which are uterine artery Doppler and biochemistry. We discuss some of the factors to consider when stratifying the obstetric population into degrees of likelihood for growth restriction, and discuss aspects of the management and outcome of pregnancies complicated by growth restriction.
