Preservation of the biomedical literature: an overview

Abstract:The National Library of Medicine began to preserve its collection many years ago. This article presents a brief review of NLM's early conservation and microfilming programs, and describes the current activities of the library's new Preservation Section. Also mentioned are the complementary efforts of NLM staff who are involved in research into electronic imaging and the campaign to increase the use of alkaline paper in medical and scientific publishing. Goals of the National Preservation Plan for the Biomedical Literature are summarized and a report on progress in implementing the plan is provided. Results of the preservation needs assessment described in the accompanying article by Kirkpatrick are briefly analyzed. Recent efforts of the Commission on Preservation and Access, the National Endowment for the Humanities, the Research Libraries Group, and several international associations are described in terms of their potential benefit to preservation of the biomedical literature. The need to monitor new preservation technologies and preserve materials in audiovisual and electronic formats is emphasized. It is argued that with enough coordination, cooperation, and willingness among health sciences libraries to share the costs, the goal of preserving all of the important biomedical literature can be accomplished.

Abstract:Advances in telecommunications technology in the last decade have fostered the development of computer networks that allow access to vast amounts of information and services. Of the many computer networks that have been developed, the most prominent is the Internet. Originally intended to be a way to share computing resources among academic and research institutions in the United States, the Internet has gradually evolved into a worldwide network of computers that provides various services reflecting the eclectic nature of its component networks. The recent upsurge in interest in the Internet is due to several mutually reinforcing factors: increased ease and availability of access to the Internet, lower access charges, faster communications, and more organizations offering commercial and noncommercial services over the Internet. Of particular interest to the medical community is the large and increasing number of technical, scientific, and biomedical resources that can be accessed through the Internet. Most large medical centers have publicly accessible information, and some large organizations, such as the National Institutes of Health, have extensive databases and services that can be used by medical researchers, clinicians, and educators. In addition, many medical organizations and some medical journals are advertising their services over the Internet and can be contacted through electronic mail. As the cost of telecommunications decreases and the speed of telecommunications increases, new forms of computer communication, such as long-distance, real-time audio, and video services will become available. Computer networks in general and the Internet in particular are likely to play more important roles in many aspects of medicine in the future.

Abstract:Little is known about the microevolutionary processes shaping within river population genetic structure of aquatic organisms characterized by high levels of homing and spawning site fidelity. Using a microsatellite panel, we observed complex and highly significant levels of intrariver population genetic substructure and Isolation-by-Distance, in the Atlantic salmon stock of a large river system. Two evolutionary models have been considered explaining mechanisms promoting genetic substructuring in Atlantic salmon, the member-vagrant and metapopulation models. We show that both models can be simultaneously used to explain patterns and levels of population structuring within the Foyle system. We show that anthropogenic factors have had a large influence on contemporary population structure observed. In an analytical development, we found that the frequently used estimator of genetic differentiation, FST, routinely underestimated genetic differentiation by a factor three to four compared to the equivalent statistic Jost's Dest (Jost 2008). These statistics also showed a near-perfect correlation. Despite ongoing discussions regarding the usefulness of “adjusted”FST statistics, we argue that these could be useful to identify and quantify qualitative differences between populations, which are important from management and conservation perspectives as an indicator of existence of biologically significant variation among tributary populations or a warning of critical environmental damage.

Abstract:Biomedical researchers often work with massive, detailed and heterogeneous datasets. These datasets raise new challenges of information organization and management for scientific interpretation, as they demand much of the researchers' time and attention. The current study investigated the nature of the problems that researchers face when dealing with such data. Four major problems identified with existing biomedical scientific information management methods were related to data organization, data sharing, collaboration, and publications. Therefore, there is a compelling need to develop an efficient and user-friendly information management system to handle the biomedical research data. This study evaluated the implementation of an information management system, which was introduced as part of the collaborative research to increase scientific productivity in a research laboratory. Laboratory members seemed to exhibit frustration during the implementation process. However, empirical findings revealed that they gained new knowledge and completed specified tasks while working together with the new system. Hence, researchers are urged to persist and persevere when dealing with any new technology, including an information management system in a research laboratory environment.

An evolution of experts: MEDLINE in the library school

Abstract:Question: What is the real value that medical librarians bring to the health care environment? How have library science educators, frequently former practitioners themselves, responded to the challenge of expert searching? Methods: In this paper, I give an historical introduction to the landscape of medical information retrieval through the development of MEDLINE. I then look at the evolution of training in online searching and its place in the context of library school education and particularly the effect of generalist education on future specialists. Finally, I acknowledge the new role of the informationist as another assertion of our professional expertise. Conclusions: The three interdependent subsystems of our professional machine-our schools, our association, and our professional peers- must all respond to this challenge by asserting our expertise in our curricula, in our continuing education, and in our dialogues with each other. Only by acknowledging the interaction of these subsystems will real and positive changes be effected to benefit our profession and our constituencies.

Abstract:To understand the loss of species, a benchmark is needed, e.g. the status of biodiversity in 1992 when the Convention on Biological Diversity recognized biodiversity crisis to compare to its status in the successive year. Though we are far from knowning how many species there are on planet Earth, we keep track of their descriptions and number through the information kept in our libraries. Each species discovered is represented therein by at least one taxononic treatment. The library includes an estimated 500 million pages and is updated daily with an estimated 17–18,000 new species annually and over 100,000 treatments augmenting the knowledge of existing species. In reality, we do not know how many species exist. We know that the catalogue of life is incomplete and basic knowlege of known species is often lacking and not even updated regularly. On the other hand, the scientific standard to cite previous works and facts is at the same time an ideal prerequisit to build a knowledge graph in the era of digital knowledge. TreatmentBank (TB) and the Biodiversity Literature Repository (BLR), two European Research Infrastructures, provide a service to convert unstructured biodiversity data from scholarly publications into semantically enhanced, digital accessible knowledge (Fawcett et al. 2022) making it findable, accessible, interoperable, reusable (FAIR, Wilkinson et al. 2016) and providing long term access in collaboration with Zenodo. Data sources are either legacy publications or active journals using an XML workflow such as Zookeys or the Biodiversity Data Journal, published by the pioneer of taxonomically enhanced Taxpub XML publishing, Pensoft. As part of the Horizon 2020-funded project Biodiversity Community Integrated Knowledge Library (BiCIKL), bidirectional linking of taxonomic names with the Catalogue of Life, DNA-sequences with the International Nucleotide Sequence Database Collaboration (INSDC) and material citations with their respective natural history collection or their depositions in the Global Biodiversity Information Facility (GBIF) is targeted. As input to allow queries including text and data mining for traits and biotic interactions, the treatments are uploaded to the Swiss Institute of Bioinformatics (SIB) Library Service (SIBiLS) using TaxPub and schema based on Journal Article Tag Suite (JATS) widely used in life science publishing and PubMed Central, allowing the use of the advanced text and data mining tools in the life science community. Another research infrastructure that ensures an RDF representation and conversion into Linked Open Data (LOD) is OpenBiodiv allowing users to explore DAK as part of growing linked open biodiverstiy and related data. All the data fit for GBIF use is reused by GBIF representing close to 60% of all published data sets, which makes TreatmentBank and BLR, for example, a sole provider of data for approximately 90,000 species not recorded in GBIF by other publishers. The long-term strategy is to build up the momentum to be an infrastructure relevant for global biodiversity conservation and research. Long-term support will be achieved by negotiations with respective agencies and publishers, and through building up a global user community using the biodiversity digital acccessible knowledge.

Abstract:Multiple perspectives on the nonlinear processes of medical innovations can be distinguished and combined using the M edical S ubject H eadings ( MeSH ) of the MEDLINE database. Focusing on three main branches—“diseases,” “drugs and chemicals,” and “techniques and equipment”—we use base maps and overlay techniques to investigate the translations and interactions and thus to gain a bibliometric perspective on the dynamics of medical innovations. To this end, we first analyze the MEDLINE database, the MeSH index tree, and the various options for a static mapping from different perspectives and at different levels of aggregation. Following a specific innovation ( RNA interference) over time, the notion of a trajectory which leaves a signature in the database is elaborated. Can the detailed index terms describing the dynamics of research be used to predict the diffusion dynamics of research results? Possibilities are specified for further integration between the MEDLINE database on one hand, and the S cience C itation I ndex and S copus (containing citation information) on the other.
Effect of 1st-trimester loss on restoration of the hypothalamic-pituitary-ovarian axis.

Abstract:This randomized prospective study was conducted to determine the length of time required for re-establishment of the reproductive axis following a 1st-trimester spontaneous abortion. The spontaneous gonadotropin secretion was significantly depressed during the first menstrual cycle after pregnancy loss, while the estradiol levels had normalized. Provocative testing revealed blunted gonadotropin release in the first menstrual cycle with return to normal during the first menstrual cycle after a spontaneous abortion. Endometrial biopsy specimens were also abnormal during the first menstrual cycle with normal histological characteristics by the second menstrual cycle. Therefore, restoration of the hypothalamic-pituitary- ovarian axis after a 1st-trimester loss is achieved within two menstrual cycles, as determined by return of normal pituitary function.
Systems of evidence-based healthcare and personalised health information: some international and national trends

Abstract:In Europe, North America and elsewhere, growing interest has focussed on evidence-based healthcare systems, incorporating the deployment of practice guidelines, as a field of application for health telematics. The clinical benefit and technical feasibility of common European approaches to this task has recently been demonstrated. In Europe it is likely that, building on recent progress in electronic health record architecture (EHRA) standards, a sufficient state of maturity can be reached to justify initiation within CEN TC251 of a prestandards process on guideline content formats during the current 5th Framework of EC RT&D activity. There is now a similar impetus to agree standards for this field in North America. Thanks to fruitful EC-USA contacts during the 4th Framework programme, there is now a chance, given well-planned coordination, to establish a global consensus optimally suited to serve the world-wide delivery and application of evidence-based medicine. This review notes three factors which may accelerate progress to convergence: (1) revolutionary changes in the knowledge basis of professional/patient/public healthcare partnerships, involving the key role of the Web as a health knowledge resource for citizens, and a rapidly growing market for personalised health information and advice; (2) the emergence at national levels of digital warehouses of clinical guidelines and EBM knowledge resources, agencies which are capable of brokering common mark-up and interchange media definitions between knowledge providers, industry and healthcare organizations; (3) the closing gap in knowledge management technology, with the advent of XML and RDF, between approaches and services based respectively on text mark-up and knowledge-base paradigms. A current project in the UK National Health Service (the National electronic Library of Health) is cited as an example of a national initiative designed to harness these trends.

Abstract:Medical librarians work collaboratively across all units and missions of academic medical centers. One area where librarians can provide key expertise is in the building and maintenance of Research Information Management Systems (RIMS). At Penn State, the RIMS implementation team has included a medical librarian, research administrators and marketing staff from the College of Medicine (CoM) since its inception in 2016. As our peer institutions implemented or expanded their own RIMS systems, the CoM team has responded to their questions regarding details about the Penn State RIMS instance. The goal of this commentary is to describe how the CoM team has worked collaboratively within Penn State to address questions related to research output, with special emphasis on details pertaining to questions from other institutions.

Abstract:When Dr. Lindberg was sworn in as Director, the National Library of Medicine (NLM) was providing few resources with information useful to the public, having concentrated efforts towards health professionals and scientists. With his arrival, and that of the Internet in the 1990s, NLM embarked on a research and user-focused path towards providing authoritative health information for patients, families and the public. MedlinePlus, NIHSeniorHealth, and MedlinePlus en espanol delivered health information in a variety of formats using text, still images, audio and video. These resources were supported by NLM advisors and Dr. Lindberg's strong belief that patients and families needed easy access to medical information to be able to effectively care for themselves in illness and maintain the best health possible throughout their lives.

Abstract:The authors historically review bibliographic research concepts and define globalization in time. Moreover, recent free online data exchange is important in terms of medical progress, scientific updating and patient-care improving. In the author's opinion, data globalization is favoring medical knowledge flow even more. The concept of a traditional library has radically changed over time, gradually missing their pivotal role in research activity. To date, the birth of the Internet and its sudden development has given a great boost to the spread of worldwide information, quickly and cheaply. Nevertheless, besides the advantages, the Internet also hides misleading risks. In this paper, the professional Medline source is compared to common Internet sources. The authors state that Internet sources have a great importance in the spread of medical knowledge. They conclude, however, that the risk of too much available information could lead to a decrease in quality.

What impact do healthcare digital libraries have? An evaluation of national resource of infection control at the point of care using the Impact-ED framework

Abstract:Over the last decade billions of dollars’ worth of investments have been directed into ICT solutions for healthcare. In particular, new evidence-based digital libraries and web portals designed to keep busy clinicians up to date with the latest evidence were created in the UK and US. While usability and performance of digital libraries were widely researched, evaluation of impact did not seem to be sufficiently addressed. This is of major concern for healthcare digital libraries as their success or failure has a direct impact on patients’ health, clinical practice, government policies and funding initiatives. In order to fill this gap, we developed the Impact-ED evaluation framework measuring impact on four dimensions of digital libraries—content, community, services and technology. Applying a triangulation technique we analysed pre- and post-visit questionnaires to assess the clinical query or aim of the visit and subsequent satisfaction with each visit, mapped it against weblogs analysis for each session and triangulated with data from semi-structured interviews. In this paper, we present the complete description of the Impact-ED framework, a definition of the comparative Impact score and application of the framework to a real-world medical digital library, the National Resource of Infection Control (NRIC, http://www.nric.org.uk), to evaluate its impact at the point of care and demonstrate the generalisability of this novel methodology. We analysed the data from a cohort of 53 users who completed the registration questionnaire, of which 32 completed pre- and post-visit questionnaires of which 72 sets were matched for analysis and five users out of these were interviewed using Dervin’s method. NRIC is generally perceived to be a useful resource with 93 % of users reporting it provides relevant information regularly or occasionally ($$n=28$$) and provided relevant information in over 65 % of visits ($$n=47$$). NRIC has a positive impact on user knowledge in over half of visits to the library (52.8 %), NRIC actual impact score$$I_{\text{ a}} = 0.65$$ and the study revealed several areas for potential development to increase its impact.

Abstract:Background The National Library of Medicine's Integrated Academic/Advanced Information Management Systems (IAIMS) initiative played a central role in the evolution of health informatics over the project's lifetime (1983–2009) and continues to do so. Aim Our objective is to demonstrate how IAIMS and two key IAIMS concepts, integration and outreach, have functioned at very different times during this evolutionary process.Approach.Using a framework drawn from Lorenzi and Stead's 2021 history of IAIMS, we examine the role of integration and outreach in work at the University of Maryland Baltimore (UMB) in the early 1980 s and at the University of Texas Arlington (UTA) in 2020. Results Guided by these concepts, UMB implemented a campus-wide information utility, while UTA established a center to accelerate research and innovation. Conclusions Outreach and integration have been formative in the evolution of health informatics and will continue to hold their power.

Abstract:Biomedical research based on the sharing and use of ever larger volumes of samples and data is increasingly becoming an essential component of scientific discovery. The success of biobanking and genomic research is dependent on the broad sharing of resources for use by investigators. However, important ethical challenges need to be addressed for the sample and data sharing to be successful. Despite low- and middle-income countries (LMICs) carrying a higher burden of disease, biomedical research conducted to date has mainly focused on high-income countries. In order for LMICs to benefit from the advances in such research, normative documents (such as laws and guidelines) play a significant role in allowing LMIC projects to partake and be represented in global biomedical research. The administration and management of the ethical aspects of biobanking, including informed consent, are key components in ensuring that samples and data can legally and ethically be used and shared. As part of its support to the LMIC biobanks, the International Agency for Research on Cancer (IARC) established a biobank and population cohort building network (BCNet) in 2013 with the aims of providing support (including education and training) and facilitating the development and improvement of biobanking infrastructure in LMICs. A comparative analysis of the laws and guidelines in BCNet countries was completed to highlight some of the ethical and legal issues related to biobanking in LMICs and to identify examples of effective systems of governance already in operation.

Abstract:Introduction: Information systems are a key success factor for medical research and healthcare. Currently, most of these systems apply heterogeneous and proprietary data models, which impede data exchange and integrated data analysis for scientific purposes. Due to the complexity of medical terminology, the overall number of medical data models is very high. At present, the vast majority of these models are not available to the scientific community. The objective of the Portal of Medical Data Models (MDM, https://medical-data-models.org) is to foster sharing of medical data models. Methods: MDM is a registered European information infrastructure. It provides a multilingual platform for exchange and discussion of data models in medicine, both for medical research and healthcare. The system is developed in collaboration with the University Library of Münster to ensure sustainability. A web front-end enables users to search, view, download and discuss data models. Eleven different export formats are available (ODM, PDF, CDA, CSV, MACRO-XML, REDCap, SQL, SPSS, ADL, R, XLSX). MDM contents were analysed with descriptive statistics. Results: MDM contains 4387 current versions of data models (in total 10,963 versions). 2475 of these models belong to oncology trials. The most common keyword (n = 3826) is 'Clinical Trial'; most frequent diseases are breast cancer, leukemia, lung and colorectal neoplasms. Most common languages of data elements are English (n = 328,557) and German (n = 68,738). Semantic annotations (UMLS codes) are available for 108,412 data items, 2453 item groups and 35,361 code list items. Overall 335,087 UMLS codes are assigned with 21,847 unique codes. Few UMLS codes are used several thousand times, but there is a long tail of rarely used codes in the frequency distribution. Discussion: Expected benefits of the MDM portal are improved and accelerated design of medical data models by sharing best practice, more standardised data models with semantic annotation and better information exchange between information systems, in particular Electronic Data Capture (EDC) and Electronic Health Records (EHR) systems. Academic health sciences libraries' outreach and engagement with North American Indigenous communities: a scoping review

Abstract:Objective: We sought to identify trends and themes in how academic health sciences libraries in the United States, Canada, and Mexico have supported engagement and outreach with Native Americans, Alaska Natives, First Nations, and Indigenous peoples, in or from those same countries. We also sought to learn and share effective practices for libraries engaging with these communities. Methods: We conducted a scoping review utilizing Arksey and O'Malley's framework for scoping reviews and followed principles from JBI Manual for Evidence Synthesis. We searched seven bibliographic databases, E-LIS (Eprints in Library and Information Science repository), and multiple sources of grey literature. Results were screened using Covidence and Google Sheets. We reported our review according to the PRISMA and PRISMA-S guidelines. We determined types of interventions used by academic health sciences libraries in engagement with our included populations, the level of public participation reached by these interventions, what partnerships were established, and what practices emerged. Results: Database searching returned 2,020 unique results. Additional searching resulted in 211 further unique results. Full text screening of relevant articles found 65 reports meeting criteria for inclusion. Data extraction was conducted on these programs to identify partners, intervention type, and evaluation method. The programs were categorized using the IAP2 Spectrum of Public Participation. Conclusion: Our scoping review found that many programs were health information trainings and did not move beyond informing the public with little further involvement. The need for sustained funding, greater community participation and more publishing on engagement and outreach are discussed.
