Abstract:Fiber Optic Shape Sensing is an innovative Optical Fiber Sensing Technology that uses a fiber optic cable to continuously track the 3D shape and position of a dynamic object (with unknown motion) in real-time without visual contact. This technology offers a valid alternative to existing shape sensing methods, thanks to a combination of advantages (including ease of installation, intrinsic safety, compactness, flexibility, electrically passive operation, resistance to harsh environments and corrosion, no need for proximity or computational or numerical models to reconstruct shape) that can bring remarkable improvements to the fields of Civil, Mechanical and Aerospace Engineering, Biomedicine and Medicine. A considerable research effort has been dedicated to this subject in the last twenty years. This paper presents an ambitious review of the current state of the art of Fiber Optic Shape Sensors (FOSS) based on Optical Multicore Fibers (MCF) or multiple optical single-core fibers with embedded strain sensors and provides a comprehensive analysis of a wide range of aspects, comprising: (1) existing alternative technologies; (2) an overview of optical fiber sensors (3) characteristics and advantages of fiber optic shape sensors; (4) historical achievements; (5) applications; (6) performance and error analysis; and (7) present and future perspectives.

Abstract:<h3 class="Heading">Purpose</h3><p class="Para">Endovascular aortic repair procedures are currently conducted with 2D fluoroscopy imaging. Tracking systems based on fiber Bragg gratings are an emerging technology for the navigation of minimally invasive instruments which can reduce the X-ray exposure and the used contrast agent. Shape sensing of flexible structures is challenging and includes many calculations steps which are prone to different errors. To reduce this errors, we present an optimized shape sensing model.</p><h3 class="Heading">Methods</h3><p class="Para">We analyzed for every step of the shape sensing process, which errors can occur, how the error affects the shape and how it can be compensated or minimized. Experiments were done with one multicore fiber system with 38 cm sensing length, and the effects of different methods and parameters were analyzed. Furthermore, we compared 3D shape reconstructions with the segmented shape of the corresponding CT scans of the fiber to evaluate the accuracy of our optimized shape sensing model. Finally, we tested our model in a realistic endovascular scenario by using a 3D printed vessel system created from patient data.</p><h3 class="Heading">Results</h3><p class="Para">Depending on the complexity of the shape, we reached an average error of 0.35–1.15 mm and maximal error of 0.75–7.53 mm over the whole 38 cm sensing length. In the endovascular scenario, we obtained an average and maximal error of 1.13 mm and 2.11 mm, respectively.</p><h3 class="Heading">Conclusion</h3><p class="Para">The accuracies of the 3D shape sensing model are promising, and we plan to combine the shape sensing based on fiber Bragg gratings with the position and orientation of an electromagnetic tracking to obtain the located catheter shape.</p>

Abstract:This paper reviews our recent developments of ultrathin fiber-optic endomicroscopy technologies for transforming high-resolution noninvasive optical imaging techniques to in vivo and clinical applications such as early disease detection and guidance of interventions. Specifically we describe an all-fiber-optic scanning endomicroscopy technology, which miniaturizes a conventional bench-top scanning laser microscope down to a flexible fiber-optic probe of a small footprint (i.e. similar to 2-2.5 mm in diameter), capable of performing two-photon fluorescence and second harmonic generation microscopy in real time. This technology aims to enable real-time visualization of histology in situ without the need for tissue removal. We will also present a balloon OCT endoscopy technology which permits high-resolution 3D imaging of the entire esophagus for detection of neoplasia, guidance of biopsy and assessment of therapeutic outcome. In addition we will discuss the development of functional polymeric fluorescent nanocapsules, which use only FAD approved materials and potentially enable fast track clinical translation of optical molecular imaging and targeted therapy.

Abstract:Lensless fiber endomicroscopy, an emergent paradigm shift for minimally-invasive microscopic optical imaging and targeted light delivery, holds transformative potential, especially in biomedicine. Leveraging holographic detection and physical or computational wavefront correction, it enables three-dimensional imaging in an unprecedentedly small footprint, which is crucial for various applications such as brain surgery. This perspective reviews the recent breakthroughs, highlighting potential emerging applications, and pinpointing gaps between innovation and real-world applications. As the research in this realm accelerates, the novel breakthroughs and existing frontiers highlighted in this perspective can be used as guidelines for researchers joining this exciting domain.

Abstract:Robot-assisted minimally invasive surgery (MIS) using flexible endoscopy has emerged as a groundbreaking technology for improving traditional surgical approaches. However, a major challenge in advancing this technology is the lack of shape sensing, which leads to inaccurate navigation and control of ultra-long flexible endoscopy within the narrow and tortuous lumen environment. The unique characteristics of flexible endoscopy, including large slenderness ratio, high bending angles, and non-symmetric, multi-channel configurations, pose significant challenges to accurate shape sensing. To address this challenge, we propose a novel shape-sensing scheme based on distributed fiber optic strain measurement, which incorporates a complete and applicable multi-interface strain transfer model adapted to large deformation and multiple sensing points. To validate the theoretical model, a shape sensor with a diameter of 2.84mm and a length of 500mm is fabricated. Both 2D and 3D shape sensing experiments are conducted on predefined templates, and the results highlight a significant improvement in the precision of sensor measurements through the utilization of the proposed model. Specifically, the 3D experiment results show a mean absolute error (MAE) of 5.38mm for complex geometrical shapes and the proposed model reduces the MAE by approximately 52.9 compared to the unmodified case.

Abstract:Precise manipulation of flexible surgical tools is crucial in minimally invasive surgical procedures, necessitating a miniature and flexible robotic probe that can precisely direct the surgical instruments. In this work, we developed a polymer-based robotic fiber with a thermal actuation mechanism by local heating along the sides of a single fiber. The fiber robot was fabricated by highly scalable fiber drawing technology using common low-cost materials. This low-profile (below 2 millimeters in diameter) robotic fiber exhibits remarkable motion precision (below 50 micrometers) and repeatability. We developed control algorithms coupling the robot with endoscopic instruments, demonstrating high-resolution in situ molecular and morphological tissue mapping. We assess its practicality and safety during in vivo laparoscopic surgery on a porcine model. High-precision motion of the fiber robot delivered endoscopically facilitates the effective use of cellular-level intraoperative tissue identification and ablation technologies, potentially enabling precise removal of cancer in challenging surgical sites.

Abstract:In this letter, we propose a novel 3D shape sensing algorithm for flexible endoscopic surgery using multi-core fiber Bragg grating (FBG) sensors. Considering the signal noises and environmental perturbations, the direct use of FBG measurements for shape sensing and position estimation is regarded as far from accurate and stable, especially when utilized in the sensing of long and flexible surgical instruments. To solve this problem, a novel and generic model-based filtering technique for the iterative curvature/twist estimation by taking advantage of the configurations of the multi-core FBGs in the optical fiber is introduced to remedy the sensory noises. Besides, we introduce an enhanced moving average approach to smooth the estimated curvatures and twists spatially on the fiber. We extensively validate our algorithm by conducting shape sensing tasks in simulations under varying conditions, and in experiments using a robotic-assisted colonoscope system integrated with a multi-core FBG fiber. The results prove our method substantially outperforms the conventional approach in the aspect of estimation accuracy and robustness, showing the superiority and application feasibility.

Abstract:A novel approach for fiber optics 3D shape sensing, applicable to mini-invasive bio-medical devices, is presented. The approach exploits the optical backscatter reflectometry (OBR) and an innovative setup that permits the simultaneous spatial multiplexing of an optical fibers parallel. The result is achieved by means of a custom-made enhanced backscattering fiber whose core is doped with MgO-based nanoparticles (NP). This special NP-doped fiber presents a backscattering-level more than 40 dB higher with respect to a standard SMF-28. The fibers parallel is built to avoid overlap between NP-doped fibers belonging to different branches of the parallel, so that the OBR can distinguish the more intense backscattered signal coming from the NP-doped fiber. The system is tested by fixing, with epoxy glue, 4 NP-doped fibers along the length of an epidural needle. Each couple of opposite fibers senses the strain on a perpendicular direction. The needle is inserted in a custom-made phantom that simulates the spine anatomy. The 3D shape sensing is obtained by converting the measured strain in bending and shape deformation.

Abstract:Continuum robots provide inherent structural compliance with high dexterity to access the surgical target sites along tortuous anatomical paths under constrained environments and enable to perform complex and delicate operations through small incisions in minimally invasive surgery. These advantages enable their broad applications with minimal trauma and make challenging clinical procedures possible with miniaturized instrumentation and high curvilinear access capabilities. However, their inherent deformable designs make it difficult to realize 3-D intraoperative real-time shape sensing to accurately model their shape. Solutions to this limitation can lead themselves to further develop closely associated techniques of closed-loop control, path planning, human-robot interaction, and surgical manipulation safety concerns in minimally invasive surgery. Although extensive model-based research that relies on kinematics and mechanics has been performed, accurate shape sensing of continuum robots remains challenging, particularly in cases of unknown and dynamic payloads. This survey investigates the recent advances in alternative emerging techniques for 3-D shape sensing in this field and focuses on the following categories: fiber-optic-sensor-based, electromagnetic-tracking-based, and intraoperative imaging modality-based shape-reconstruction methods. The limitations of existing technologies and prospects of new technologies are also discussed.

Abstract:Photonics has spurred a myriad of diagnostic and therapeutic applications for defeating cancer owing to its superiority in spatiotemporal maneuverability and minimal harm. The limits of light penetration depth and elusiveness of photosensitizer utilization, however, impede the implementation of the photodiagnostic and -therapy for determining and annihilating the deep-situated tumor. Herein, a promising strategy that harnesses functional optical fibers is developed and demonstrated to realize an in vivo endoscopic cancer sensing and therapy ensemble. Tumor detection is investigated using hypoxia-sensitive fluorescent fibers to realize fast and accurate tumor recognition and diagnosis. The tumor treatment is further performed by exploiting the endogenous photothermal effect of rare-earth-doped optical fibers. The eradication of orthotopic and subcutaneous xenografts significantly validates the availability of tumoricidal fibers. The strategy opens horizons to inspire the design of optical fiber-mediated "plug and play" precise tumor theranostics with high safety, which may intrigue broader fields, such as fiber optics, materials, chemistry, medicine, and clinics.

Abstract:Purpose With the increasing development of the surgical robots, the opto-mechatronic technologies are more potential in the robotics system optimization. The optic signal plays an important role in opto-mechatronic systems. This paper aims to present a review of the research status on fiber-optic-based force and shape sensors in surgical robots. Design/methodology/approach Advances of fiber-optic-based force and shape sensing techniques in the past 20 years are investigated and summarized according to different surgical requirement and technical characteristics. The research status analysis and development prospects are discussed. Findings Compared with traditional electrical signal conduction, the phototransduction provides higher speed transmission, lower signal loss and the immunity to electromagnetic interference in robot perception. Most importantly, more and more advanced optic-based sensing technologies are applied to medical robots in the past two decades because the prominence is magnetic resonance imaging compatibility. For medical robots especially, fiber-optic sensing technologies can improve working security, manipulating accuracy and provide force and shape feedback to surgeon. Originality/value This is a new perspective. This paper mainly researches the application of optical fiber sensor according to different surgeries which is beneficial to learn the great potential of optical fiber sensor in surgical robots. By enumerating the research progress of medical robots in optimization design, multimode sensing and advanced materials, the development tendency of fiber-optic-based force and shape sensing technologies in surgical robots is prospected.

Abstract:Optical endoscopy has become an essential diagnostic and therapeutic approach in modern biomedicine for directly observing organs and tissues deep inside the human body, enabling non-invasive, rapid diagnosis and treatment. Optical fiber endoscopy is highly competitive among various endoscopic imaging techniques due to its high flexibility, compact structure, excellent resolution, and resistance to electromagnetic interference. Over the past decade, endoscopes based on a single multimode optical fiber (MMF) have attracted widespread research interest due to their potential to significantly reduce the footprint of optical fiber endoscopes and enhance imaging capabilities. In comparison with other imaging principles of MMF endoscopes, the scanning imaging method based on the wavefront shaping technique is highly developed and provides benefits including excellent imaging contrast, broad applicability to complex imaging scenarios, and good compatibility with various well-established scanning imaging modalities. In this review, various technical routes to achieve light focusing through MMF and procedures to conduct the scanning imaging of MMF endoscopes are introduced. The advancements in imaging performance enhancements, integrations of various imaging modalities with MMF scanning endoscopes, and applications are summarized. Challenges specific to this endoscopic imaging technology are analyzed, and potential remedies and avenues for future developments are discussed.
Design and analysis of a fiber-optic sensing system for shape reconstruction of a minimally invasive surgical needle

Abstract:Abstract This paper presents the performance analysis of the system for real-time reconstruction of the shape of the rigid medical needle used for minimally invasive surgeries. The system is based on four optical fibers glued along the needle at 90 degrees from each other to measure distributed strain along the needle from four different sides. The distributed measurement is achieved by the interrogator which detects the light scattered from each section of the fiber connected to it and calculates the strain exposed to the fiber from the spectral shift of that backscattered light. This working principle has a limitation of discriminating only a single fiber because of the overlap of backscattering light from several fibers. In order to use four sensing fibers, the Scattering-Level Multiplexing (SLMux) methodology is applied. SLMux is based on fibers with different scattering levels: standard single-mode fibers (SMF) and MgO-nanoparticles doped fibers with a 35–40 dB higher scattering power. Doped fibers are used as sensing fibers and SMFs are used to spatially separate one sensing fiber from another by selecting appropriate lengths of SMFs. The system with four fibers allows obtaining two pairs of opposite fibers used to reconstruct the needle shape along two perpendicular axes. The performance analysis is conducted by moving the needle tip from 0 to 1 cm by 0.1 cm to four main directions (corresponding to the locations of fibers) and to four intermediate directions (between neighboring fibers). The system accuracy for small bending (0.1–0.5 cm) is 90 $$\%$$ % and for large bending (0.6–1 cm) is approximately 92 $$\%$$ % .

Abstract:Fiber optic shape sensing has a great potential for diverse medical and industrial applications to measure curvatures and even shapes. Featuring small footprint, strong immunity to radiation and high flexibility integration, fiber optic shape sensing opens up a new era in the fields of position tracking, human wearable devices, catheter navigation, bending detection and deformation monitoring. This paper focuses on a branch of fiber optic shape sensing techniques, with an emphasis on shape sensing based on fiber Bragg gratings (FBGs). Key technologies of shape sensing based on FBG are introduced in detail together with a critical view of its evolutionary trend. In addition, the major problems that exist in FBG shape sensing have been discussed in the end.

Abstract:Over the last decade, optical fiber-based forms of microscopy and endoscopy have extended the realm of applicability for many imaging modalities. Optical fiber-based imaging modalities permit the use of remote illumination sources and enable flexible forms supporting the creation of portable and hand-held imaging instrumentations to interrogate within hollow tissue cavities. A common challenge in the development of such devices is the design and integration of miniaturized optical and mechanical components. Until recently, microelectromechanical systems (MEMS) sensors and actuators have been playing a key role in shaping the miniaturization of these components. This is due to the precision mechanics of MEMS, microfabrication techniques, and optical functionality enabling a wide variety of movable and tunable mirrors, lenses, filters, and other optical structures. Many promising results from MEMS based optical fiber endoscopy have demonstrated great potentials for clinical translation. In this article, reviews of MEMS sensors and actuators for various fiber-optical endoscopy such as fluorescence, optical coherence tomography, confocal, photo-acoustic, and two-photon imaging modalities will be discussed. This advanced MEMS based optical fiber endoscopy can provide cellular and molecular features with deep tissue penetration enabling guided resections and early cancer assessment to better treatment outcomes.

A miniaturised one-degree-of-freedom joint-angle sensor is devised, using a single light emitting/receiving optical fibre with a coupler connected to a Keyence (FS-N11MN, Osaka, Japan) sensor that supplies and detects light through the optical fibres. A curvature-varying reflective surface integrated in the joint demonstrates non-contact light intensity-based sensing. Various reflector geometries and surface colours are designed to compare sensor output for achieving a large angle range and improved sensitivity for the proposed miniaturised robotic shape-sensing applications.

Shape Sensing for Continuum Robotics using Optoelectronic Sensors with Convex Reflectors

Abstract:Three-dimensional shape sensing in soft and continuum robotics is a crucial aspect for stable actuation and control in fields such as Minimally Invasive surgery, as the estimation of complex curvatures while using continuum robotic tools is required to manipulate through fragile paths. This challenge has been addressed using a range of different sensing techniques, for example, Fibre Bragg grating (FBG) technology, inertial measurement unit (IMU) sensor networks or stretch sensors. Previously, an optics-based method, using optoelectronic sensors was explored, offering a simple and cost-effective solution for shape sensing in a flexible tendon-actuated manipulator in two orientations. This was based on proximity-modulated angle estimation and has been the basis for the shape-sensing method addressed in this paper. The improved and miniaturized technique demonstrated in this paper is based on the use of a spherically shaped reflector with optoelectronic sensors integrated into a tendon actuated robotic manipulator. Upgraded sensing capability is achieved using optimization of the spherical reflector shape in terms of sensor range and resolution, and improved calibration is achieved through the integration of spherical bearings for friction-free motion. Shape estimation is achieved in two orientations upon calibration of sensors, with a maximum Root Mean Square Error (RMS) of 3.37°.

Their size is one of the most critical aspects, because smaller and less rigid endoscopes enable higher agility, facilitate larger accessibility, and induce less stress on the surrounding tissue. In all existing endoscopes, the size of the optics poses a major limitation in miniaturization of the imaging system. Not only is making small optics difficult, but their performance also degrades with downscaling. Meta-optics have recently emerged as a promising candidate to drastically miniaturize optics while achieving similar functionalities with significantly reduced size. Herein, we report an inverse-designed meta-optic, which combined with a coherent fiber bundle enables a 33% reduction in the rigid tip length over traditional gradient-index (GRIN) lenses. We use the meta-optic fiber endoscope (MOFIE) to demonstrate real-time video capture in full visible color, the spatial resolution of which is primarily limited by the fiber itself. Our work shows the potential of meta-optics for integration and miniaturization of biomedical devices towards minimally invasive surgery.
