Abstract:Abstract Background Human factors (HF) integration can improve patient safety in the operating room (OR), but the depth of current knowledge remains unknown. This study aimed to explore the content of HF training for the operative environment. Methods We searched six bibliographic databases for studies describing HF interventions for the OR. Skills taught were classified using the Chartered Institute of Ergonomics and Human Factors (CIEHF) framework, consisting of 67 knowledge areas belonging to five categories: psychology; people and systems; methods and tools; anatomy and physiology; and work environment. Results Of 1851 results, 28 studies were included, representing 27 unique interventions. HF training was mostly delivered to interdisciplinary groups (n = 19; 70 per cent) of surgeons (n = 16; 59 per cent), nurses (n = 15; 56 per cent), and postgraduate surgical trainees (n = 11; 41 per cent). Interactive methods (multimedia, simulation) were used for teaching in all studies. Of the CIEHF knowledge areas, all 27 interventions taught ‘behaviours and attitudes’ (psychology) and ‘team work’ (people and systems). Other skills included ‘communication’ (n = 25; 93 per cent), ‘situation awareness’ (n = 23; 85 per cent), and ‘leadership’ (n = 20; 74 per cent). Anatomy and physiology were taught by one intervention, while none taught knowledge areas under work environment. Conclusion Expanding HF education requires a broader inclusion of the entirety of sociotechnical factors such as contributions of the work environment, technology, and broader organizational culture on OR safety to a wider range of stakeholders.

Abstract:Patient safety has become a major public concern. Human factors research in other high-risk fields has demonstrated how rigorous study of factors that affect job performance can lead to improved outcome and reduced errors after evidence-based redesign of tasks or systems. These techniques have increasingly been applied to the anesthesia work environment. This paper describes data obtained recently using task analysis and workload assessment during actual patient care and the use of cognitive task analysis to study clinical decision making. A novel concept of "non-routine events" is introduced and pilot data are presented. The results support the assertion that human factors research can make important contributions to patient safety. Information technologies play a key role in these efforts.

Abstract:During the past 70 years, patient safety science has evolved through four organizational frameworks known as Safety-0, Safety -1, Safety-2, and Safety-3. Their evolution reflects the realization over time that blaming people, chasing errors, fixing one-offs, and regulation would not create the desired patient safety. In Safety-0, the oldest framework, harm events arise from clinician failure; event prevention relies on better staffing, education, and basic standards. In Safety-1, used by hospitals, harm events arise from individual and/or system failures. Safety is improved through analytics, workplace culture, high reliability principles, technology, and quality improvement. Safety-2 emphasizes clinicians' adaptability to prevent harm events in an everchanging environment, using resilience engineering principles. Safety-3, used by aviation, adds system design and control elements to Safety-1 and Safety-2, deploying human factors, design-thinking, and operational control or feedback to prevent and respond to harm events. Safety-3 represents a potential way for anesthesia and perioperative care to become safer.

Abstract:Human factors engineering involves the study and development of methods aimed at enhancing performance, improving safety, and optimizing user satisfaction. The focus of human factors engineering encompasses the design of work environments and an understanding of human mental processes to prevent errors. In this review, we summarize the history, applications, and impacts of human factors engineering on the healthcare field. To illustrate these applications and impacts, we provide several examples of how successful integration of a human factors engineer in our pediatric radiology department has positively impacted various projects. The successful integration of human factors engineering expertise has contributed to projects including improving response times for portable radiography requests, deploying COVID-19 response resources, informing the redesign of scheduling workflows, and implementation of a virtual ergonomics program for remote workers. In sum, the integration of human factors engineering insight into our department has resulted in tangible benefits and has also positioned us as proactive contributors to broader hospital-wide improvements.

Abstract:Opinion statementHuman factors engineering (HFE), with its focus on studying how humans interact with systems, including their physical and organizational environment, the tools and technologies they use, and the tasks they perform, provides principles, tools, and techniques for systematically identifying important factors, for analyzing and evaluating how these factors interact to increase or decrease the risk of Healthcare-associated infections (HAI), and for identifying and implementing effective preventive measures. We reviewed the literature on HFE and infection prevention and control and identified major themes to document how researchers and infection prevention staff have used HFE methods to prevent HAIs and to identify gaps in our knowledge about the role of HFE in HAI prevention and control. Our literature review found that most studies in the healthcare domain explicitly applying (HFE) principles and methods addressed patient safety issues not infection prevention and control issues. In addition, most investigators who applied human factors principles and methods to infection prevention issues assessed only one human factors element such as training, technology evaluations, or physical environment design. The most significant gap pertains to the limited use and application of formal HFE tools and methods. Every infection prevention study need not assess all components in a system, but investigators must assess the interaction of critical system components if they want to address latent and deep-rooted human factors problems.

Abstract:Patient safety remains a significant concern even in light of some improvements, such as reduction of healthcare-associated complications for hospitalized patients. We argue that renewed attention for systems approaches is necessary in order to more effectively tackle complex patient safety problems. We propose the concept of patient journey as a new perspective for patient safety. The spatio-temporal interactions and care transitions are the essence of the patient journey. The patient journey perspective emphasizes patients’ interactions with multiple care settings over time.We review different process modeling approaches aimed at describing the patient journey. This includes methods based on the SEIPS (Systems Engineering Initiative for Patient Safety) model of work system and patient safety. Examples described in this chapter include modeling of the medication management process and modeling of care coordination for elderly patients with chronic conditions. The examples show how human factors and systems engineering can contribute to analyzing processes in the patient journey and, therefore, provide opportunities for addressing patient safety in a systematic manner.

Abstract:Healthcare relies on high levels of human performance, as described by the 'human as the hero' concept. However, human performance varies and is recognised to fall in high-pressure situations, meaning that it is not a reliable method of ensuring safety. Other safety-critical industries embed human factors principles into all aspects of their organisations to improve safety and reduce reliance on exceptional human performance; there is potential to do the same in anaesthesia. Human factors is a broad-based scientific discipline which aims to make it as easy as possible for workers to do things correctly. The human factors strategies most likely to be effective are those which 'design out' the chance of an error or adverse event occurring. When errors or adverse events do happen, barriers are in place to trap them and reduce the risk of progression to patient and/or worker harm. If errors or adverse events are not trapped by these barriers, mitigations are in place to minimise the consequences. Non-technical skills form an important part of human factors barriers and mitigation strategies and include: situation awareness; decision-making; task management; and team working. Human factors principles are not a substitute for proper investment and appropriate staffing levels. Although applying human factors science has the potential to save money in the long term, its proper implementation may require investment before reward can be reaped. This narrative review describes what is known about human factors in anaesthesia to date.

Abstract:Despite progress within the past 15 years, improving patient safety in health care remains an important public health issue. The history of safety policies, research, and development has revealed that this issue is more complex than initially perceived and is pertinent to all health care settings. Solutions, therefore, must be approached at the systems level and supplemented with a change in safety culture, especially in higher risk fields such as surgery. To do so, health care agents at all levels have started to prioritize the improvement of nontechnical skills such as teamwork, communication, and accountability, as reflected by the development of various checklists and safety campaigns. This progress may be sustained by adopting teamwork training programs that have proven successful in other high-risk industries, such as crew resource management in aviation. These techniques can be readily implemented among surgical teams; however, successful application depends heavily on the strong leadership and vigilance of individual surgeons.
Beyond error: A qualitative study of human factors in serious adverse events

Abstract:The field of healthcare quality and safety has been informed by the study of Human Factors contributing to adverse events. Hitherto, much of the study of Human Factors has been focused on a narrow lens of human error, identifying cognitive-based or knowledge-based errors and cognitive processes such as loss of situational awareness contributing to error. While these factors are important, this narrow approach fails to consider the complexity of relational and systemic factors that also contribute to adverse events. We aimed to explore the relational and systemic human factors, including shared clinician attitudes and behavior, that contribute to serious adverse patient events in a public health setting. The study, set in a metropolitan local health district in New South Wales, Australia, was conducted using a retrospective qualitative multi-incident content analysis design. Serious adverse event reviews (SAER) over 6 months (2022-2023) were subject to qualitative content analysis until data saturation was reached. Data saturation reached at 20 reports. Emergent themes related to human factors in serious adverse events included: (i) delays and inertia-with a subtheme of inertia of ageism; (ii) "All-or-nothing" approach to end-of-life care and planning; (iii) communication lapses; and (iv) implementation gap between standards and practice. Error-based incidents accounted for only 35% of the serious adverse events examined. The sample studied involved mostly (65%) male patients, with a mean age of 69 (70% aged >65), managed across the gamut of specialties, with the most common incident being the management of acutely deteriorating patients. In conclusion, there is more to Human Factors in adverse events than cognitive or knowledge-based error. While identifying and correcting errors is absolutely essential, we need adjunctive "soft measures" to address clinical attitudes, behaviors, and relationships in health care, particularly in increasingly complex, fraught, and stressful health care environments.
The Application of Human Factors Approaches to Improve Safety, Efficiency and Well-being in Urology: A Systematic Scoping Review

Abstract:Objective: To elucidate the application of HF approaches within urology to improve clinical work-system function via a systematic scoping review. Human Factors (HF) plays an integral role to improving safety, efficiency, and well-being by optimizing work-system interactions. Despite its established application across various high-risk industries, the systematic exploration of HF methods applied within urology remains limited. Methods: A scoping review of HF interventions implemented within urologic care from 1980 to 2023 was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology. Web of Science, PubMed, and OVID Medline databases were searched. Database searching resulted in 10,850 articles, after systematic review, 22 studies were included. Results: The included studies predominantly originated from the United States and covered diverse areas of urology, including surgical procedures, diagnostics, and patient care. Interventions varied widely from ergonomic equipment implementations to modifications in clinical processes and team dynamics. Most studies utilized predetermined interventions based on prior literature or experiential anecdotes (15, 65.22%), while a smaller subset employed data-driven strategies to tailor interventions (8, 34.7%). The HF methods employed included questionnaires, retrospective reviews, observations, and physical measurements, targeting improvements in patient experiences, operational efficiencies, and clinical outcomes. Conclusion: This review underscores the emergent role of HF in urology, highlighting a broad spectrum of interventions and methodological approaches that contribute to system optimization. Future endeavors should focus on collaborative efforts to develop standardized HF applications in urology, promoting a safer, more efficient, and clinician-friendly environment.

Abstract:Healthcare practitioners, patient safety leaders, educators and researchers increasingly recognise the value of human factors/ergonomics and make use of the discipline's person-centred models of sociotechnical systems. This paper first reviews one of the most widely used healthcare human factors systems models, the Systems Engineering Initiative for Patient Safety (SEIPS) model, and then introduces an extended model, 'SEIPS 2.0'. SEIPS 2.0 incorporates three novel concepts into the original model: configuration, engagement and adaptation. The concept of configuration highlights the dynamic, hierarchical and interactive properties of sociotechnical systems, making it possible to depict how health-related performance is shaped at 'a moment in time'. Engagement conveys that various individuals and teams can perform health-related activities separately and collaboratively. Engaged individuals often include patients, family caregivers and other non-professionals. Adaptation is introduced as a feedback mechanism that explains how dynamic systems evolve in planned and unplanned ways. Key implications and future directions for human factors research in healthcare are discussed.

Patient-centered care in the OR: is this possible?

Abstract:Patient-centered care can be practiced in high technology areas such as the operating room if care-givers develop an attitude of seeing their care delivery through the patient's eyes. Through caring interactions with our patients, we can find out what matters most to them. It seems that the more complex our technology and the more bureaucratic our hospitals have become, the less respect and positive regard is being communicated to patients. In addition, time is becoming increasingly scarce in our busy operating rooms. However, it is not how much time we spend with our patients. Instead, it is how we are with our patients in these stressful moments before surgery. Using concepts of relational care discussed in current nursing literature, nurses can evolve from a technically dominant care-giving paradigm towards a humanistic paradigm that focuses on the patient's expressed needs. This enables our clients to experience health and healing in their context.

Abstract:Keeping patients safe while they receive medical care is essential. Yet current systems designed to ensure patient safety are not enough, because medical error is the third leading cause of preventable deaths in the United States. Clinicians can partner with the patient to enhance patient safety. Pulse Center for Patient Safety proposes patient- and family-driven processes designed to improve a patient's chances of avoiding harm. This article discusses highlights of the role of patient safety through a grassroots lens, summarizes the factors that influence the patient's role in patient safety and reviews recommendations on how clinicians can partner with patients.

Abstract:We describe different sources of hazards from cardiovascular operating room (CVOR) technologies, how hazards propagate in the CVOR and their impact on cognitive processes. Previous studies have examined hazards from poor design of a specific CVOR technology. However, the impact of different CVOR technologies functioning in context is not clearly understood. In addition, the impact of non-design hazards in technology devices is unclear. Our study identified hazards from organisational, physical/environmental elements, in addition to design of technology in a CVOR. We used observations, follow-up interviews and photographs. With qualitative analyses, we categorised the different hazard sources and their potential impact on cognitive processes. Patient safety can be built into technologies by incorporating user needs in design, decision-making and implementation of medical technologies. Practitioner summary: Effective design and implementation of technology in a safety-critical system requires prospective understanding of technology-related hazards. Our research fills this gap by studying different technologies in context of a CVOR using observations. Qualitative analyses identified different sources for technology-related hazards besides design, and their impact on cognitive processes.
Improving Patient Safety Through Collaboration Between Clinical Staff and Engineering Staff in Hospitals

Abstract:Clinicians and biomedical engineers each provide their unique skill sets to the common goal of improving patient safety. Increased communication between the two can improve the medical device experience of caregivers and patients at many points in the lifespan of a medical device. This includes purchasing, training, using, and adverse event reporting and investigation. The Food and Drug Administration’s (FDA’s) Medical Product Safety Network (MedSun), a network of 250 US hospitals trained to partner with FDA and device manufacturers to report and resolve problems with medical devices, works with participating hospitals to encourage communication and collaboration to improve adverse event reporting and promote patient safety. A cursory look at reports submitted through MedSun shows that hospitals with strong collaborations between engineering and clinical staff are more likely to submit higher-quality reports to FDA with both the clinical scenario and engineering evaluation necessary to appropriately identify and act on medical device issues. The purpose of this article was to discuss the various ways strong collaboration between clinicians and engineers can benefit hospitals and share collaboration strategies as reported by hospitals participating in MedSun.

Abstract:The interest in employing Human Factors (HF) in healthcare is increasing. There are very few papers that outline an approach to embedding the practical application of HF in healthcare and the lessons that can be learned. The SCReaM HF and Team Resource Management (TRM) programme is not only aimed at raising the awareness and understanding of the science of HF within healthcare, but also to embed its practical application to support staff improve their safety and wellbeing and that of their patients. The programme is divided into three strands: rolling training, HF Projects and HF Engineering. Each strand of the programme supports the use of HF by teaching and applying systems thinking methodology. The programme has been embedded into an NHS Trust and this paper provides a reflection on the approach used to help guide future attempts to integrate HF into an NHS trust. Interest in employing Human Factors (HF) in healthcare is increasing. The SCReaM HF and Team Resource Management (TRM) programme has been embedded into an NHS Trust and provides a good model for how HF can be introduced and utilised within an NHS trust to improve staff and patient safety and wellbeing.

Abstract:The aim of this study was to analyse the design of anaesthetic rooms using human factors and ergonomics (HFE) methods. The methods used were hierarchical task analysis, link analysis and anthropometry. The study found several latent design errors which negatively affected drug and patient preparation tasks. Recommendations include anaesthetic room layout design modifications and system level changes. HFE principles provide generic recommendations but specific design details may not be generalizable. Further research is needed to explore the implementation of system changes.

We review examples of modern and forward-looking applications of simulation and describe where human factors and ergonomics can be leveraged to ensure simulations are effective at minimizing harm and improving patient outcomes, as well as future directions of simulation-based systems improvement that can enrich and improve patient care.

Abstract:Objective: This article provides a review of areas that present significant challenges in infection prevention and control and describes human factors engineering (HFE) approaches that have been applied successfully to these areas. In addition, implications and recommendations for HFE use in future research are discussed. Background: Infection prevention and control aims to prevent patients and health care personnel from acquiring preventable infections in healthcare. Effective infection control practices of healthcare-associated infections have recently become even more critical with the emergence of life-threatening infections. HFE could benefit infection prevention and control in addressing older and more recent challenges, but uptake has been limited. Method/Results: This literature review is an integration and synthesis of recently published research that describes HFE-based approaches in infection prevention and control to address the challenges for three specific topics. The results of the review suggests that HFE is in a position to support work in infection prevention and control and improve overall healthcare safety. Conclusion: HFE provides conceptual frameworks and methods that have significant potential to improve infection prevention and control. Application: The work reviewed can provide potential solutions for current infection prevention and control challenges by applying HFE based recommendations.
