What exercise programme is the most appropriate to mitigate anterior cruciate ligament injury risk in football (soccer) players? A systematic review and network meta-analysis

Abstract:Objectives To examine the effectiveness of different exercise-based interventions to mitigate the risk of ACL injury in football players, and to determine which is the most appropriate for them, specifically for female football players. Design Four databases (PubMed, Scopus, Web of Science and PEDro) were accessed in July 2023 using the keywords football, soccer, athletic injuries, ACL, knee injuries, injury prevention, exercise-based programme, risk factor. Methods Randomised controlled trials (RCTs) that evaluated any exercise-based injury prevention intervention compared with a control group on the prevention of ACL injury in football players were included. The intervention parameters, population characteristics and count data related to number of ACL injuries and exposure hours for each group were extracted and summarised. Results Eleven studies were included. The majority of the studies were ranked low-to-high risk of bias. The overall certainty of evidence included in this meta- analysis was low due to serious imprecision (i.e., 95 % credible intervals (CrI) crossed zero, and with wide CrI suggesting uncertainty in the estimates) and some network inconsistencies. Data were presented as logarithm hazard ratio (log HR), credible intervals (CrI) and standard deviation (SD). FIFA 11 + was the most effective in reducing ACL injury risk in football players (log HR = − 1.23 [95 % CrI: − 2.20, − 0.35]; SD = 0.47), followed by the Knäkontroll programme (log HR = − 0.76 [95 % CrI: − 1.60, − 0.03]; SD = 0.42). For females, only Knäkontroll had a significant impact on reducing the risk of ACL injury (log HR = − 0.62 [95 % CrI: − 1.71, 0.62]; SD = 0.58). Conclusions Our results support the use of FIFA 11 + and Knäkontroll to mitigate injury incidence at overall level. However, the effectiveness of these interventions changed when adjusting for females, making it less clear which intervention is more protective. Knäkontroll is postulated as the program with the greatest preventie nature, although these results should be interpreted with caution due to the lack of the sample.
