Abstract:Despite the challenges associated with the coronavirus pandemic, the last 2 years have been active periods for hypertension research and initiatives in Asia. There are new hypertension guidelines from the World Health Organization that can be interpreted and applied locally. This is also the case for data from the latest Blood Pressure Lowering Treatment Trialists' Collaboration meta-analysis, which showed that greater reductions in systolic blood pressure (BP) are associated with lower risks of cardiovascular events. The randomized controlled Strategy of Blood Pressure Intervention in the Elderly Hypertensive study and the Salt Substitute and Stroke Study provide local data to inform practice. Other initiatives to help reduce high salt intake in Asia are also underway. Both drug-resistant and nocturnal hypertension are appropriate areas of focus in Asia, and there are an increasing number of pharmacological and non-pharmacological treatment options for these conditions. Digital therapeutics to promote uptake and implementation of lifestyle interventions are showing promise, and other digital-based strategies such as telemedicine, wearable BP monitors to detect beat-by-beat BP and artificial intelligence will no doubt become integral parts of future strategies to reduce the burden of hypertension and hypertension-related disease. A number of initiatives from the Hypertension Cardiovascular Outcome Prevention and Evidence in Asia Network and Japanese Society of hypertension are underway, and there is good reason for optimism regarding the ongoing and future management of hypertension in Asia based on these and the active research activities in the region.

Home blood pressure control and prescribing patterns of anti-hypertensive medications in a home blood pressure-based hypertension-specialized clinic in Japan: a sub-analysis of the Ohasama study

Abstract:Although the benefits of anti-hypertensive treatment are well known, the proportion of hypertensive patients with controlled blood pressure (BP) remains suboptimal. The present study aimed to compare BP control conditions in a hypertension-specialized clinic and non-hypertension-specialized clinics. This cross-sectional study used data from 379 treated patients who measured home BP in the Ohasama study between 2016 and 2019 (men: 43.0%, age: 71.6 years). Of those, 172 patients were managed at the hypertension-specialized clinic where physicians distributed home BP devices to each patient, evaluated the home BP data, and adjusted medications to maintain home BP values according to the recent Japanese guidelines. When we set morning home systolic/diastolic BP of <135/ < 85 mmHg as controlled BP, 93.6% of patients fulfilled the controlled home BP range, compared to 43.0% in non-specialized clinics ( n = 207). The proportion of the patients with home morning BP < 125/ < 75 mmHg was 73.3% in the hypertension-specialized clinic and 20.8% in the non-hypertension-specialized clinics. Hypertension-specialized clinics prescribed three or more anti-hypertensive drug classes to 41.9% of patients, compared to 15.2% in non-specialized clinics. In the hypertension-specialized clinic, angiotensin II receptor blockers were most commonly prescribed (86.6%), followed by dihydropyridine calcium channel blockers (77.9%), thiazide (including thiazide-like) diuretics (30.2%), mineralocorticoid receptor blockers (23.8%), and beta- and alpha-beta blockers (10.5%). In conclusion, the proportion of patients with controlled home BP was excellent in the hypertension-specialized clinic. Home BP-based hypertension practices, as recommended in the current Japanese guidelines, may be the key to achieving sufficient BP control.

Abstract:Raised blood pressure is the leading cause of preventable death in the world. Yet, its global prevalence is increasing, and it remains poorly detected, treated, and controlled in both high and low resource settings. From the perspective of members of the International Society of Hypertension based in all regions, we reflect on the past, present, and future of hypertension care, highlighting key challenges and opportunities, which are often region-specific. We report that most countries failed to show sufficient improvements in blood pressure control rates over the past three decades, with greater improvements mainly seen in some high-income countries, also reflected in substantial reductions in the burden of cardiovascular disease and deaths. Globally there are significant inequities and disparities based on resources, sociodemographic environment, and race with subsequent disproportionate hypertension-related outcomes. Additional unique challenges in specific regions include conflict, wars, migration, unemployment, rapid urbanisation, extremely limited funding, pollution, COVID-19-related restrictions and inequalities, obesity and excessive salt and alcohol intake. Immediate action is needed to address suboptimal hypertension care and related disparities on a global scale. We propose a Global Hypertension Care Taskforce including multiple stakeholders and societies to identify and implement actions in reducing inequities, addressing social, commercial and environmental determinants, and strengthening health systems implement a well-designed customised quality of care improvement framework.

Abstract:Hypertension remains a major global healthcare issue. Considering that most Japanese patients with hypertension are managed by general practitioners, hypertension specialists should be involved in actual clinical practice. We investigated the blood pressure (BP), guidelines recommended for achievement rate of the target BP, and clinical variables of patients with hypertension treated by hypertension specialists and those treated by non-specialists in a real-world setting. Factors associated with the target BP achievement in this population were also investigated. Outpatients with hypertension from 12 medical facilities in Okinawa Prefecture were enrolled (n = 1469 [specialist group, 794; non-specialist group, 675]; mean age, 64.2 years; females, 45.8%). For all patients, BP and rate of the target BP achievement were 129.0 ± 15.5/74.6 ± 10.6 mmHg, and 51.8%, respectively. BP and the rate of target of BP achievement were 128.0 ± 15.1/73.4 ± 10.4 mmHg and 56.7% in the specialist group, and they were 130.1 ± 15.9/76.0 ± 10.8 mmHg and 46.1% in the non-specialist group. The urinary salt excretion and obesity rates were comparable between the specialist and non-specialist groups. Multivariable logistic analyses indicated that hypertension specialists and good medication adherence were positive factors, whereas obesity, chronic kidney disease, diabetes mellitus, and urinary salt excretion were inverse factors associated with target BP achievement in this population. Initiatives for salt reduction, medication adherence, and proper obesity management are crucial to improving BP management in patients with hypertension. Hypertension specialists are expected to play an essential role in them. For all patients, the target blood pressure (BP) achievement rate were 51.8%. Hypertension specialists and good medication adherence were positive factors in achieving target BP; conversely, obesity, diabetes mellitus, chronic kidney disease, and high urinary salt excretion were inverse factors in achieving target BP among patients with hypertension.

Abstract:A self‐measured home blood pressure (BP)‐guided strategy is an effective practical approach to hypertension management. The Asia BP@Home study is the first designed to investigate current home BP control status in different Asian countries/regions using standardized home BP measurements taken with the same validated home BP monitoring device with data memory. We enrolled 1443 medicated hypertensive patients from 15 Asian specialist centers in 11 countries/regions between April 2017 and March 2018. BP was relatively well controlled in 68.2% of patients using a morning home systolic BP (SBP) cutoff of <135 mm Hg, and in 55.1% of patients using a clinic SBP cutoff of <140 mm Hg. When cutoff values were changed to the 2017 AHA/ACC threshold (SBP <130 mm Hg), 53.6% of patients were well controlled for morning home SBP. Using clinic 140 mm Hg and morning home 135 mm Hg SBP thresholds, the proportion of patients with well‐controlled hypertension (46%) was higher than for uncontrolled sustained (22%), white‐coat (23%), and masked uncontrolled (9%) hypertension, with significant country/regional differences. Home BP variability in Asian countries was high, and varied by country/region. In conclusion, the Asia BP@Home study demonstrated that home BP is relatively well controlled at hypertension specialist centers in Asia. However, almost half of patients remain uncontrolled for morning BP according to new guidelines, with significant country/regional differences. Strict home BP control should be beneficial in Asian populations. The findings of this study are important to facilitate development of health policies focused on reducing cardiovascular complications in Asia.

Abstract:It is widely accepted that hypertension constitutes a significant cardiovascular risk factor and that treating high blood pressure (BP) effectively reduces cardiovascular risk. An important issue in Asia is not just the high prevalence of hypertension, particularly in some countries, but also the low level of awareness and treatment rates in many regions. The 2017 update of the American College of Cardiology/American Heart Association hypertension guidelines raised the question about which BP threshold should be used to diagnose and treat hypertension. Although there is a theoretical rationale for a stricter BP criterion in Asia given the ethnic-specific features of hypertension in the region, the majority of countries in Asia have retained a diagnostic BP threshold of >= 140/90 mm Hg. Although lowering thresholds might make theoretical sense, this would increase the prevalence of hypertension and also markedly reduce BP control rates. In addition, there are currently no data from robust randomized clinical trials of the benefits of the lower targets in preventing cardiovascular disease and reducing cardiovascular risk, particularly in high-risk patients and especially for Asian populations. There is also no defined home BP treatment target level for an office BP treatment target of 130/80 mm Hg. However, in this regard, in the interim, lifestyle modifications, including reducing body weight and salt intake, should form an important part of hypertension management strategies in Asia, while studies on treating at lower BP threshold level in Asians and getting to lower BP targets will be helpful to inform and optimize the management of hypertension in the region.

Abstract:Out-of-office blood pressure measurement is an essential part of diagnosing and managing hypertension. In the era of advanced digital health information technology, the approach to achieving this is shifting from traditional methods (ambulatory and home blood pressure monitoring) to wearable devices and technology. Wearable blood pressure monitors allow frequent blood pressure measurements (ideally continuous beat-by-beat monitoring of blood pressure) with minimal stress on the patient. It is expected that wearable devices will dramatically change the quality of detection and management of hypertension by increasing the number of measurements in different situations, allowing accurate detection of phenotypes that have a negative impact on cardiovascular prognosis, such as masked hypertension and abnormal blood pressure variability. Frequent blood pressure measurements and the addition of new features such as monitoring of environmental conditions allows interpretation of blood pressure data in the context of daily stressors and different situations. This new digital approach to hypertension contributes to anticipation medicine, which refers to strategies designed to identify increasing risk and predict the onset of cardiovascular events based on a series of data collected over time, allowing proactive interventions to reduce risk. To achieve this, further research and validation is required to develop wearable blood pressure monitoring devices that provide the same accuracy as current approaches and can effectively contribute to personalized medicine.

Abstract:BACKGROUND: Patients with hypertension remain at increased risk of micro- and macrovascular complications unless their elevated blood pressure (BP) can be adequately controlled. However, helping patients with hypertension to get to, and stay at, target BP goals can be difficult in everyday clinical practice.SCOPE: The present study describes the magnitude of this problem in various regions and attempts to analyse possible underlying reasons. For this purpose, a non-systematic literature review using Medline database was performed.FINDINGS: Reasons for suboptimal blood pressure control include factors under the control of the physician, such as insufficient education and motivation of the patient, reluctance to initiate lifestyle changes or drug treatment; overlooking the importance of controlling systolic BP (especially in the elderly) and failure to modify and expand therapy when it is indicated (e.g. use of combination therapy if monotherapy is proving inadequate for BP lowering). Patient-related factors that may give rise to inadequate BP control include a lack of awareness of the relevance of hypertension, failure to comply with recommended lifestyle modifications and poor medication compliance (e.g. because of forgetfulness, tolerability problems, or incomplete understanding of the long-term nature of therapy).CONCLUSIONS: These issues can be addressed by ensuring that doctors adhere more closely to national or international guidelines and that patients are well-informed about the need for long-term therapy. Further approaches include simplifying the treatment regimen (e.g. use of well-tolerated, fixed-dose combinations) and using various measures to reduce forgetfulness. With properly directed therapy and consideration of potential physician- and patient-related barriers to poor BP control, more hypertensive patients in everyday clinical practice can achieve target BP goals and thereby realise the proven benefits of antihypertensive therapy for decreasing their cardiovascular risk.

Unique Approaches to Hypertension Control in China

Abstract:According to the China nationwide hypertension survey in 2012-2015, the prevalence of hypertension in adults (>= 18 years) was 27.9%, which increased approximately by 50% from 2002 (18.8%). The management of hypertension was improved, but still at low level, with the awareness, treatment and control rates of 46.9%, 40.7% and 15.3%, respectively. The challenge was still the low awareness and the low control rate in treated hypertension (37.5%). Several actions are currently being taken to improve hypertension control in China. In collaboration with the International Society of Hypertension May Measurement Month (MMM) Program, hundreds of volunteers across the country measured blood pressure in a large number of people in summer since 2017. In addition, a nationwide Web-based and WeChat-linked blood pressure measurement system is being built, with thousands of devices installed in public places within easy-reach of people. To improve blood pressure control in treated hypertension, two initiatives are being undertaken. One is the expanding use of ambulatory blood pressure monitoring to guide antihypertensive treatment. Another is primary aldosteronism screen in resistant hypertension. It is hoped that in the framework of the Healthy China 2030 Program the control rate of hypertension may increase in the next 10 years to 50% or higher by increasing both the awareness rate in the population and control rate in treated hypertensive patients to 70% or higher.

Abstract:Objectives: A self-HomeBP-guided strategy is an effective, practical approach for managing hypertension. However, there is no standardized cross-sectional evaluation of home BP measurement across Asia. [email protected] is the first study to evaluate home BP control status simultaneously in 11 countries/regions, utilising the same validated home BP monitoring device and following the same BP measurement schedule. Methods: Patients with hypertension living in Asia, receiving antihypertensive therapy, were provided BP monitoring devices and instructed to measure their BP twice in the morning and twice at bedtime for at least 7 days in a 15-day period. Results: In 1443 patients who were attending 15 specialist hypertension centers in Asia, 68.2 % were well-controlled for morning home systolic BP (SBP) < 135 mmHg, and 55.5% were well-controlled for clinic SBP < 140 mmHg. The finding of well-controlled hypertension for both clinic and morning home SBP was the highest (45.9%) among all the 4 classifications and their thresholds for clinic (140 mmHg) and morning home (135 mmHg) SBP. Even with the new 2017AHA/ACC thresholds, 53.6% were well-controlled for morning home SBP. In the Japanese nationwide general practitioner-based home BP study (J-HOP) with baseline data from 2005 to 2012, 46% were well-controlled for morning home SBP by using the conventional thresholds and 31% were well-controlled with the new thresholds. Conclusion: The finding that home BP status was well controlled in patients attending specialist hypertension centers is the “Hope of Asia”, giving hope that strict morning home BP control outside these centers could perhaps be achieved by transferring the expertise of hypertension management from the hypertension specialists to general practitioners.

Abstract:AIMS/INTRODUCTION: To investigate the current status of achieved blood pressure levels in association with the number of antihypertensive drug classes as of 2013, and to explore the clinical correlates with achievement of target blood pressure in a large-scale cohort of Japanese subjects with type 2 diabetes.MATERIALS AND METHODS: A nationwide survey was conducted including 12,811 subjects with type 2 diabetes. Subjects were divided by achieved blood pressure, <130/80 or 140/90 mmHg, and the number of drug classes taken.RESULTS: The percentages achieving a blood pressure of <130/80 or 140/90 mmHg were 52.0% and 86.1%, respectively. The prevalence of hypertension, if defined as ≥130/80 mmHg or treated, became 67.9%. Among subjects taking antihypertensive drugs, a blood pressure of <130/80 or <140/90 mmHg was 46.7% and 83.2%, respectively. The percentages of <130/80 mmHg were 55.9% without drugs, 47.1% on 1, 42.5% on 2, 47.2% on 3, and 56.8% on ≥4 drugs, respectively. The most prescribed drugs were renin-angiotensin system inhibitors, followed by calcium channel blockers, diuretics, and β-blockers. The multiple logistic regression analysis indicated that a blood pressure <130/80 mmHg was associated with lower values in age, body mass index, albuminuria, and glomerular filtration rate, higher proportions on targets for HbA<sub>1C</sub> and lipids, and less retinopathy.CONCLUSIONS: In type 2 diabetes, hypertension is common and only 52% achieved <130/80 mmHg, indicating a difficulty in blood pressure lowering. This was correlated with difficulties in glycemic and lipid management, obesity, and vascular complications, implying these clustering to be a serious problem. This article is protected by copyright. Abstract:Hypertension is one of the most powerful modifiable risk factors for cardiovascular disease. It is usually asymptomatic and therefore essential to measure blood pressure regularly for the detection of hypertension. Home blood pressure monitoring (HBPM) is recognized as a valuable tool to monitor blood pressure and facilitate effective diagnosis of hypertension. It is useful to identify the masked or white‐coat hypertension. There is also increasing evidence that supports the role of HBPM in guiding antihypertensive treatment, and improving treatment compliance and hypertension control. In addition, HBPM has also shown prognostic value in predicting cardiovascular events. Despite these benefits, the use of HBPM in many parts of Asia has been reported to be low. An expert panel comprising 12 leading experts from 10 Asian countries/regions convened to share their perspectives on the realities of HBPM. This article provides an expert summary of the current status of HBPM and the key factors hindering its use. It also describes HBPM‐related initiatives in the respective countries/regions and presents strategies that could be implemented to better support the use of HBPM in the management of hypertension.

Abstract:We thank Dr. Walter Espeche et al. [1] for their interest in our article regarding the diagnostic agreement between masked uncontrolled hypertension detected by ambulatory blood pressure (BP) monitoring (ABPM) and that detected by home BP monitoring (HBPM) using an all-in-one BP monitoring device [2]. Although our study's main results were based on the BP control status derived with the use of only systolic BP (SBP), the correspondence by Dr. Espeche et al. raises the important question of whether both systolic and diastolic BP (DBP) values should be included when classifying the BP control status. We would first like to re-emphasize that our study's main purpose was to investigate the diagnostic agreement between masked uncontrolled hypertension detected by ABPM indices with that detected by HBPM indices. We observed that the prevalence of masked uncontrolled hypertension was increased when assessed by the consideration of multiple BP indices both when assessed using SBP values alone and when assessed using SBP and DBP values. In the study's report, we discussed mainly the BP control status obtained with SBP values alone because of the older age of our study population (69.2 ± 11.5 years) [2]. The association between hypertension and cardiovascular risk has been well established in all age groups, but a gradual shift in the BP elevation from DBP to SBP with increasing age has been demonstrated [3]. In patients <50 years old, DBP was a significant risk factor for coronary heart disease but SBP was not, whereas in those ≥60 years old, SBP was a significant risk factor for coronary heart disease but DBP was not [3]. In addition, in the IDACO (International Database on Ambulatory Blood Pressure in Relation to Cardiovascular Outcomes) study, isolated diastolic hypertension was not a risk factor for cardiovascular disease in adults aged ≥50 years [4].
