Abstract:Objective: To analyze and summarize the clinical features in children with recurrent intussusception. Methods: This retrospective cohort study collected the clinical data of 126 children with recurrent intussusception who were admitted to the Children's Hospital of Zhejiang University School of Medicine due to "abdominal pain, paroxysmal crying, vomiting, bloody stools" from January 1, 2015 to November 30, 2019. The clinical manifestations of recurrent intussusception between ≤3 years old group and >3 years old group were compared, the etiology and age characteristics of pathologic lead points (PLP) were analyzed, and the clinical characteristics of PLP group and non-PLP group were also compared. The χ2 test and Mann-Whitney U test were used to compare the differences between groups. Results: A total of 126 children with recurrent intussusception were included, of whom 76 were males and 50 were females, with the age of 2.9 (1.7, 5.1) years. The proportion of children aged more than 1 year was 87.3% (110/126), and 48.4% (61/126) more than 3 years. Clinical manifestations mostly lacked the typical triad of symptoms. The percentage of paroxysmal crying in ≤ 3 years old group was significantly higher than that in >3 years old group (52.3% (34/65) vs. 24.6% (15/61), χ2=10.17, P=0.001), while the percentage of abdominal pain was significantly lower than that in the >3 years old group (46.1% (30/65) vs. 75.4% (46/61), χ2=11.25, P=0.001). The rate of positive ultrasound examination was 17.5% (22/126), and 63.6% (14/22) of them were diagnosed. The positive rate of CT examination was 4/13, of which 2 cases were diagnosed. In this study, 37 children were diagnosed with PLP by colonoscopy, laparoscopy or laparotomy, and 89 children were found without PLP. The positive rate of PLP in >3 years old group was significantly higher than that in ≤3 years old group (37.7% (23/61) vs. 21.5% (14/65), χ2=3.96, P=0.046). Meckel's diverticulum and juvenile polyp were the main contributors of PLP in ≤3 years old group, accounting for 7/14 and 3/14 respectively, while lymphoma and juvenile polyp accounted for 34.8% (8/23) and 26.1% (6/23), respectively in >3 years old group. Compared with non-PLP group, PLP group had higher age (5.2 (1.6, 6.7) vs. 2.7 (1.8, 4.2) years, Z=-2.26, P=0.01). However, there were no significant differences in gender and recurrence frequency between the two groups (both P>0.05). Conclusions: Recurrent intussusception is more common in children more than 1 year old, and has a wide spectrum of non-specific clinical presentations. Imaging examinations can be used to identify PLP. The most recurrent intussusception is idiopathic, but the presence of PLP should be alerted for, such as Meckel's diverticulum, lymphoma and juvenile polyp. Colonoscopy sometimes is necessary, surgical exploration and treatment should be carried out in time.
Phlebosclerotic colitis: Our clinical experience of 25 patients in China.

Abstract:The aim of the current study was to enhance the awareness of phlebosclerotic colitis (PC) through our clinical experience. A retrospective review of 25 patients who were diagnosed as PC in our 2 affiliated hospitals from January 2013 to October 2017 was conducted. The patients were found at a mean age of 63.5 years, range 47 to 87years. The majority of patients were male (23 cases). Only 4 patients (16%, 4/25) had the history about long-term use of Chinese herbs and medical liquor. The most common symptoms were abdominal pain (40%) and intestinal obstruction (16%), followed by diarrhea (12%), and gastrointestinal bleeding (12%), etc. Three cases (12%) had no symptoms. The varying degrees of calcifications along the colon and mesenteric venous were found in all of their computed tomography (CT) images. The lesions mainly located in transverse and ascending colon (60%, 15/25). The terminal ileum, the whole colon and rectum involvement were also been found. Fourteen patients had the examination of colonoscopy which all presented characteristic dark purple-colored endoscopic findings. Conservative treatment with close follow-up was preferred in our group. Three cases had the surgery of colectomy due to the repeatedly intestinal obstruction, perforation. The PC was a very rare but characteristic entity with unclear etiopathogenesis. Examination of abdomen CT and colonoscopy could help you to make clinical diagnosis.
Clinical Case Analysis of Inflammatory Bowel Diseases in 39 Children

Abstract:Objective To acquire the best diagnostic method by analyzing the features of the children's IBD.Methods According to the diagnostic criteria of IBD that was established in national seminar about inflammatory bowel diseases in 2000,diagnostic key points of CD recommended by WHO,39 children with IBD in hospital were analyzed clinically and comparing UC with CD.Results Among 39 patients with IBD,24 patients were UC,15 patients were CD,the ratio of boys and girls was 2:1.The clinical manifestation of UC and CD was usually abdominal pain,diarrhea and hemafecia.Left hemicolon lesion often presented in UC cases;end piece of ileum lesion developed usually in CD cases.The features of enteroscopy were that diminished and distorted mucosal vascular pattern,friability,hemorrhage and purulent exudates usually found in UC(78.3%)and unspecific sarcomyces and narrowing of the lumen often happened in CD(87.5%).The main histological feature of UC is inflammatory cells infiltration(86.7%),while achroacyte plasma cells and inflammatory cells infiltrating were mainly affected in CD(73.3%).Conclusion At the present stage,the combination of clinical manifestation combine with colon endoscopy and histopathologic examination is the best diagnostic way in children's IBD.
[Clinical and Colonoscopic Characteristics of Pediatric Inflammatory Bowel Disease].

Abstract:OBJECTIVE:To analyze clinical manifestations, endoscopic and histological features for establishing a diagnosis of pediatric inflammatory bowel disease (IBD).METHOD:Thirty-four inpatients with inflammatory bowel disease (ulcerative colitis/UC/: 24; Crohn's disease/CD/:10) were enrolled into this study. Data of clinical manifestations, laboratory values, endoscopic findings and histopathological features of biopsy material were analyzed.RESULT:Four children had mild/moderate active Crohn's disease. Six had severe active disease. The most common presenting symptom in CD was abdominal pain (80%, 8/10). One child had intestinal perforation; 2 had obstruction. Anal fistula was found in 2 patients. There were 5 mild, 14 moderate and 5 severe diseases in UC group. Diarrhea (23/24, 96%) was the most common symptom. Three children with UC suffered from perianal diseases. One had chronic intussusception. ESR and C reactive protein values were significantly higher in patients with CD compared with patients with UC (chi(2) = 15.938, P < 0.01; chi(2) = 11.184, P < 0.01). The pattern of anatomic involvement in CD was: ileocolic 60%, colon 10% and small bowel 30%. Endoscopically, discontinuous lesions, diverse ulcers, proliferative/regenerative patterns and narrowed bowel lumen were observed. Histologically, lymphocytes aggregation in the lamina propria and submucosa were observed. Non-caseating granulomas were found in 22% cases. Twenty-five percent of patients with UC had pancolitis. Colonoscopy showed diffusely distributed multiple erosions and ulcers in UC cases. Twenty-nine percent of children had pseudopolyps. No mucosal bridge was found. Mucosal biopsies showed chronic inflammatory cells, neutrophils and eosinophils diffusely infiltrated in the lamina propria. Crypt abscess was found in 4 cases.CONCLUSION:The clinical manifestations in pediatric inflammatory bowel disease are nonspecific. Colonoscopic examination and biopsy are valuable in establishing the diagnosis of pediatric ulcerative colitis. It is important for colon involved CD children to have a colonoscopic examination. But the mucosal biopsies were short of specificity. Multi-place and deep biopsy are needed to improve the diagnosis.

Abstract:Pediatric-onset colitis and inflammatory bowel disease (IBD) have significant effects on the growth of infants and children, but the etiopathogenesis underlying disease subtypes remains incompletely understood. Here, we report single-cell clustering, immune phenotyping, and risk gene analysis for children with undifferentiated colitis, Crohn's disease, and ulcerative colitis. We demonstrate disease-specific characteristics, as well as common pathogenesis marked by impaired cyclic AMP (cAMP)-response signaling. Specifically, infiltration of PDE4B- and TNF-expressing macrophages, decreased abundance of CD39-expressing intraepithelial T cells, and platelet aggregation and release of 5-hydroxytryptamine at the colonic mucosae were common in colitis and IBD patients. Targeting these pathways by using the phosphodiesterase inhibitor dipyridamole restored immune homeostasis and improved colitis symptoms in a pilot study. In summary, comprehensive analysis of the colonic mucosae has uncovered common pathogenesis and therapeutic targets for children with colitis and IBD.

Inflammatory Bowel Disease in Chinese Children: A Retrospective Analysis of 49 Cases

Abstract:To present study reports the clinical characteristics, diagnosis and management of pediatric patients with inflammatory bowel disease (IBD) in China. Clinical records of 49 pediatric patients with IBD at Fudan University Children's Hospital (Shanghai, China) between July 2001 and May 2012 were reviewed. Of 49 patients (30 males and 19 females), 8 patients had ulcerative colitis (UC) and 41 patients had Crohn's disease (CD). The mean age was 10.4 years in patients with UC and 10.1 years in patients with CD. The percentages of patients with UC with abdominal pain, diarrhea or rectal bleeding were 62.5, 100 and 87.5%, respectively, and that of CD patients were 75.6, 61 and 39%, respectively. Patients with UC were underweight (37.5%), and had oral ulcers (12.5%) and arthritis (12.5%), and patients with CD were underweight (34.1%), and had oral ulcers (26.8%), anal fistulas (22%) and arthritis (19.5%). Patients with CD had a higher proportion of immunoglobulin G, C-reactive protein and erythrocyte sedimentation rate compared with patients with UC. Histologic lesions of patients with UC were primarily located in the sigmoid colon (75%), transverse colon (50%) or pan-colon (50%), and in patients with CD they were located in the distal ileum (51.2%), ileocecum (58.5%) or upper gastrointestinal tract (41.5%). All patients underwent colonoscopies and 13 underwent double-balloon enteroscopies. Standard treatment for IBD, including corticosteroids, 5-aminosalicylate, and immunosuppressants (6-mercaptopurine and azathioprine), were administered to all patients. In addition 3 patients were treated with infliximab and 10 patients were treated with thalidomide. Comprehensive assessment of clinical, laboratory, endoscopic, and pathohistological data will benefit the timely diagnosis of IBD.
Clinical Presentation of Inflammatory Bowel Disease： a Hospital Based Retrospective Study of 379 Patients in Eastern China

Abstract:Background Numerous studies from Europe and North America have provided a wealth of information regarding the epidemiological and clinical characteristics of inflammatory bowel disease (IBD) in Caucasians. Previous studies in mainland China have been limited by small patient numbers or by lack of detailed information about clinical subgroups of the disease. This study was carried out to assess the demographic and clinical characteristics of IBD in Chinese patients.Methods In the Sir Run Run Shaw Hospital between 1994 and 2003, 379 patients were diagnosed as IBD. Demographic and clinical data were collected and analysed.Results Of 379 patients, 317 had ulcerative colitis (UC) (83.6%, 168 male, 149 female, male-female ratio 1.13:1, age range at diagnosis 14 - 79 years, mean age 44 years) and 62 had Crohn's disease ( CD) (16.4%, 39 male and 23 female, male-female ratio 1.70:1, age range at diagnosis 13 - 70 years, mean age 33 years). In UC, 11.4% of patients had proctitis, 25.2% had proctosigniolditis, 18.6% were diseased to the splenic flexure and 44.8% had extensive colitis. Nine patients with UC (2.8%) had arthritis, three patients (0.9%) had iritis or conjunetivitis. Of the 62 CD patients, 16 (25.8%) had diseases restricted to the terminal ileum; 15 (24.2%) had colonic diseases; 20 (32.3%) had ileocolonic disease and 11 (17.7%) had disease involving the upper gastrointestinal tract.Conclusions This study shows similar caracteristics of IBD to that in the West but there are some differences with respect to severity and extraintestinal manifestations. The ethnic and geographic differences may give important clues to the aetiology of IBD.

Abstract:Objective To investigate clinical Characteristics of ulcerative colitis(UC). Methods Clinical data of 52 cases UC was analysed retrospectivety. Results Clinical,barium enema,enteroscopeand and biochemistry detection were used for diagnosing 52 cases with UC,chronical relapse 22 cases(42.3%),initial type 15 cases(22.8%),acute fulminating type 2 cases(3.8%),chronical persistence type 13 cases(25.0%). Present symptoms:abdominal pain 41 cases(78.8%),hemafecia 6 cases (11.5%),diarrhea 9 cases(17.3%),muco-bloody purulent stool 43 cases(82.7%),fever 4 cases(7.7%). State of illness:varmitis 26 cases(50.0%),medium-sized 16 cases(30.8%) and gravis type 10 cases(19.2%). Extent of disease:rectum type 4 cases(7.7%),vertical sigmoid colon type 14 cases(26.9%),left hemicolon type 19 cases(36.5%),right hemicolon type 2 cases(3.8%),pan-colon type 13 cases(25.0%). Complication of extra-bowels 5 cases(9.6%),complication 2 cases(3.8%). The different illness with body temperature,sedimentation rate,white blood cell,hemoglobin and α2 globulin were significant difference(P < 0.05 or P <0.01). Salicylazosulfapyridine was effective for most of the patients. Conclusion UC can be diagnosed and treated correctly by mastering its clinical specialities.
Leveraging Human Microbiome Features to Diagnose and Stratify Children with Irritable Bowel Syndrome

Abstract:Accurate diagnosis and stratification of children with irritable bowel syndrome (IBS) remain challenging. Given the central role of recurrent abdominal pain in IBS, we evaluated the relationships of pediatric IBS and abdominal pain with intestinal microbes and fecal metabolites using a comprehensive clinical characterization and multiomics strategy. Using rigorous clinical phenotyping, we identified preadolescent children (aged 7 to 12 years) with Rome III IBS (n = 23) and healthy controls (n = 22) and characterized their fecal microbial communities using whole-genome shotgun metagenomics and global unbiased fecal metabolomic profiling. Correlation-based approaches and machine learning algorithms identified associations between microbes, metabolites, and abdominal pain. IBS cases differed from controls with respect to key bacterial taxa (eg, Flavonifractor plautii and Lachnospiraceae bacterium 7_1_58FAA), metagenomic functions (eg, carbohydrate metabolism and amino acid metabolism), and higher-order metabolites (eg, secondary bile acids, sterols, and steroid-like compounds). Significant associations between abdominal pain frequency and severity and intestinal microbial features were identified. A random forest classifier built on metagenomic and metabolic markers successfully distinguished IBS cases from controls (area under the curve, 0.93). Leveraging multiple lines of evidence, intestinal microbes, genes/pathways, and metabolites were associated with IBS, and these features were capable of distinguishing children with IBS from healthy children. These multi-omics features, and their links to childhood IBS coupled with nutritional interventions, may lead to new microbiome-guided diagnostic and therapeutic strategies.
Detection of biomarkers and its clinical signiifcance in the inlfammatory bowel disease in Children

Abstract:Objective To investigate the biological markers and their clinical significance in diagnosis and differential diagnosis of inlfammatory bowel disease (IBD) in children.Methods The study had 22 cases of IBD including 6 cases of ulcerative colitis (UC) and 16 cases of Crohn’s Disease (CD). Twenty-four children without IBD were selected as controls. The serum perinuclear anti-neutrophil cytoplasmic antibody (pNACA) was measured by indirect immune lfuorescence method. The serum anti-saccharomyces cerevisiae antibody (ASCA) IgG and IgA, anti-B mannose glycoside antibody (AMCA) IgG, anti-B glycoside sugar shell antibody (ACCA) IgA, Anti-bacterial lfagellin antibody (Anti-cBir1) IgG, and the fecal calprotectin (FC) were determined by Enzyme linked immunosorbent assay (ELISA). Results The positive rate of serum pANCA was 100% in 6 cases of UC while it was negative in CD cases and control, and there was significant difference among three groups (P<?0.01). In CD cases, both positive rate of serum ACCA IgA and that of Anti-cBir1-IgG were 62.5% and the positive rate of ACCA IgA was 37.5%. Meanwhile, all of them were negative in UC cases and control. There were signiifcant differences among three groups (P<0.01). The positive rate of FC was 100% in children with IBD. It was signiifcantly higher than the positive rate in control group, 54.2% (P<?0.01).Conclusion The serum pANCA is a speciifc index for the diagnosis of UC. The serum ACCA IgA, AMCA IgG, ASCA IgG and IgA, and Anti-cBir1 IgG were speciifc to some extent in the diagnosis of CD. Increased FC can relfect the activity of IBD, but cannot be used for the differential diagnosis of IBD and non IBD.
Fecal proteolytic profiling of pediatric inflammatory bowel disease: A pilot study

Abstract:Colonoscopy is the gold standard for diagnosing inflammatory bowel disease (IBD). However, this invasive procedure has a high burden for pediatric patients. Previous research has shown elevated fecal amino acid concentrations in children with IBD versus controls. We hypothesized that this finding could result from increased proteolytic activity. Therefore, the aim of this study was to investigate whether fecal protease-based profiling was able to discriminate between IBD and controls. Protease activity was measured in fecal samples from patients with IBD (Crohn's disease (CD) n = 19; ulcerative colitis (UC) n = 19) and non-IBD controls (n = 19) using a fluorescence resonance energy transfer (FRET)-peptide library. Receiver operating characteristic (ROC) curve analysis was used to determine the diagnostic value of each FRET-peptide substrate. Screening the FRET-peptide library revealed an increased total proteolytic activity (TPA), as well as degradation of specific FRET-peptides specifically in fecal samples from IBD patients. Based on level of significance (p < .001) and ROC curve analysis (AUC > 0.85), the fluorogenic substrates W-W, A-A, a-a, F-h, and H-y showed diagnostic potential for CD. The substrates W-W, a-a, T-t, G-v, and H-y showed diagnostic potential for UC based on significance (p < .001) and ROC analysis (AUC > 0.90). None of the FRET-peptide substrates used was able to differentiate between protease activity in fecal samples from CD versus UC. This study showed an increased fecal proteolytic activity in children with newly diagnosed, treatment-naïve, IBD. This could lead to the development of novel, noninvasive biomarkers for screening and diagnostic purposes.
Analysis of Skin Manifestations in 50 Children with Inflammatory Bowel Disease

Abstract:Objective:To analyze skin manifestations of pediatric inflammatory bowel disease (IBD) .Methods:Children with IBD were collected from pediatric wards in Peking University Third Hospital from January 2010 to January 2022, and their skin manifestations were retrospectively analyzed.Results:A total of 50 children with IBD were included, including 27 with Crohn′s disease and 23 with ulcerative colitis. Twenty-five (50%) patients had skin manifestations, including specific skin manifestations in 11 (22%) and relevant skin manifestations in 11 (22%) . Specific skin manifestations included cutaneous perianal Crohn′s disease in 2 cases, and anal fistula and/or perianal abscess in 9 cases; relevant skin manifestations included erythema nodosum in 5 cases, aphthous stomatitis in 3 cases, psoriasis in 1 case, polyarteritis nodosa in 1 case, and Henoch-Sch?nlein purpura in 1 case. Compared with the ulcerative colitis group, the Crohn′s disease group was more prone to suffer from specific skin manifestations and relevant skin manifestations, and there were significant differences in the prevalence of specific and relevant skin manifestations between the two groups (both P < 0.05) . Of the 27 children with Crohn′s disease, 19 (70%) had one or more skin manifestations, 2 of whom successively presented with 4 different skin manifestations. One child with Crohn′s disease and 1 with ulcerative colitis had 3 different skin manifestations in different periods. The fecal calprotectin level was elevated in all children with skin manifestations, and in 12 (48%) children without skin manifestations. The skin lesions of 5 children were improved or subsided after dose adjustment (1 case) or switch (4 cases) of biological agents. Conclusions:Half of the children with IBD have skin manifestations, and children with Crohn′s disease are more prone to have specific and relevant skin manifestations. Different skin manifestations could be observed in the same child in different periods. Multidisciplinary teamwork is conducive to the overall control of this disease.

Abstract:Background:The purpose was to estimate the incidence and characteristics of childhood inflammatory bowel disease (IBD) during 2000–2010 in Shanghai, China. Methods:IBD patients between the ages of 0 and 18 years old were identified by survey of computerized medical information. Relevant data were extracted from their corresponding medical records. Results:A total of 153 IBD cases were included in the study. Among them, 107 were males and 46 were females (male/female ratio, 2.3:1.0). Eighty-two had Crohn's disease (CD) and 71 had ulcerative colitis (UC). The peak prevalence of IBD was observed in the 10–14-year-old age group. The annual incidence of IBD in the 0 to 14 years age group of Shanghai residents steadily increased from 2000 to 2010. The most common symptoms of IBD were diarrhea (68.6%), bloody stool (68.6%), and abdominal pain (61.4%). More CD than UC patients had anemia and raised erythrocyte sedimentation rate and C-reactive protein levels. Ileocolonic type disease was more common in CD patients, and left-side colon involvement was more common in UC. Of all CD patients, 33 had mild active disease and 49 had moderate/severe disease. In UC patients, 34 were mild and 37 were moderate/severe disease. Conclusions:This retrospective, multicenter hospital-based study over a decade shows a steadily increasing trend of childhood IBD in China. This suggests a need for population-based epidemiological studies to explore the risk factors.

Abstract:BackgroundEarly diagnosis and treatment of pediatric Inflammatory bowel disease (PIBD) is challenging due to the complexity of the disease and lack of disease specific biomarkers. The novel machine learning (ML) technique may be a useful tool to provide a new route for the identification of early biomarkers for the diagnosis of PIBD. MethodsIn total, 66 treatment naive PIBD patients and 27 healthy controls were enrolled as an exploration cohort. Fecal microbiome profiling using 16S rRNA gene sequencing was performed. The correlation between microbiota and inflammatory and nutritional markers was evaluated using Spearman's correlation. A random forest model was used to set up an ML approach for the diagnosis of PIBD using 1902 markers. A validation cohort including 14 PIBD and 48 irritable bowel syndrome (IBS) was enrolled to further evaluate the sensitivity and accuracy of the model. ResultCompared with healthy subjects, PIBD patients showed a significantly lower diversity of the gut microbiome. The increased Escherichia-Shigella and Enterococcus were positively correlated with inflammatory markers and negatively correlated with nutrition markers, which indicated a more severe disease. A diagnostic ML model was successfully set up for differential diagnosis of PIBD integrating the top 11 OTUs. This diagnostic model showed outstanding performance at differentiating IBD from IBS in an independent validation cohort. ConclusionThe diagnosis penal based on the ML of the gut microbiome may be a favorable tool for the precise diagnosis and treatment of PIBD. A study of the relationship between disease status and the microbiome was an effective way to clarify the pathogenesis of PIBD.
Importance of early detection of infantile inflammatory bowel disease with defective IL-10 pathway: A case report

Abstract:Rationale: Infantile inflammatory bowel disease (IBD) is an extremely rare subgroup of IBD that includes patients whose age of onset is younger than 2 years old. These patients can have more surgical interventions, and a severe and refractory disease course with higher rates of conventional treatment failure. Monogenic defects play an important role in this subgroup of IBD, and identification of the underlying defect can guide the therapeutic approach. Patient concerns: In 2007, a 4-month-old girl from a nonconsanguineous family presenting with anal fistula, chronic diarrhea, and failure to thrive. She underwent multiple surgical repairs but continued to have persistent colitis and perianal fistulas. Diagnosis: Crohn's disease was confirmed by endoscopic and histologic finding. Intervention: Conventional pediatric IBD therapy including multiple surgical interventions and antitumor necrosis factor alpha agents were applied. Outcomes: The patient did not respond to conventional pediatric IBD therapy. Interleukin-10 (IL-10) receptor mutation was discovered by whole-exome sequencing and defective IL-10 signaling was proved by functional test of IL-10 signaling pathway by the age of 12. The patient is currently awaiting hematopoietic stem cell transplantation. Lessons: Early detection of underlying genetic causes of patients with infantile-IBD is crucial, since it may prevent patients from undergoing unnecessary surgeries and adverse effects from ineffective medical therapies. Moreover, infantile-IBD patients with complex perianal disease, intractable early onset enterocolitis and extraintestinal manifestations including oral ulcers and skin folliculitis, should undergo genetic and functional testing for IL-10 pathway defect.

Abstract:Pediatric ulcerative colitis (UC) is a distinct type of inflammatory bowel disease with severe disease activity and rapid progression, which can lead to detrimental life-long consequences. The pathogenesis of pediatric UC remains unclear, although dysbiosis of the gut microbiota has been considered an important factor. In this study, we collected intestinal mucosal-luminal interface microbiota samples from a cohort of treatment-naïve pediatric UC or control patients and used two different mass spectrometry-based glycomic approaches to examine the N-glycans that were associated with the microbiota. We observed abundant small N-glycans that were associated with the microbiota and found that the pediatric UC microbiota samples contained significantly higher levels of these atypical N-glycans compared to those of controls. Four paucimannosidic or other truncated N-glycans were identified to successfully segregate UC from control patients with an area under the ROC curve of ≥0.9. This study indicates that the aberrant metabolism of glycans in the intestinal by gut microbiota may be involved in the pathogenesis of UC and intestinal N-glycans, including small glycans, can act as novel biomarker candidates for pediatric UC.SignificanceThere is no cure for pediatric ulcerative colitis (UC) due to its unclear pathogenesis and the diagnosis of UC in children still largely depends on invasive colonoscopic examination. Recent evidence suggests that the dysbiosis of intestinal microbiota is associated with the onset and development of UC, however how the microbiota interact with the host remains unclear. This study used two different mass spectrometry-based glycomic approaches to quantitatively examine N-glycans that are associated with colonic mucosal-luminal interface microbiota of pediatric UC or control patients. To the best of our knowledge, this is the first comprehensive glycomic study of intestinal microbiota samples in UC, which demonstrated that intestinal microbiota was associated with abundant atypical small N-glycans with elevated levels in UC than controls. This study also identified four intestinal paucimannosidic or other truncated N-glycans as promising biomarker candidates for pediatric UC. These findings shed light on the mechanism study of host-microbiome interactions in UC and indicate that atypical glycans present in the gut can be a source for UC biomarker discovery.

Abstract:Objective To investigate the features of clinic and genetic of Chinese children with very early onset inflammatory bowel disease (VEO-IBD).Methods A total of 30 Chinese children diagnosed with VEO-IBD definitely were enrolled in the study.The onset age,clinical manifestation,body development,blood test and drug efficacy were investigated.Next generation sequencing (NGS) based on target gene sequencing panel and Sanger sequencing were used to analyze the 50 candidate genes reported.Results Among the 30 VEO-IBD children,the meaningful gene mutations were found in 15 cases,of whom 13 had mutations in IL-10RA gene,1 had hemizygote mutation in CYBB gene and 1 had compound heterozygote mutation in SLC37A4 gene.A total of 8 mutation sites of IL-10RA were found,of which p.R101W was found in 10 cases,p.K173NfsX7 in 5 cases,p.R165X(414) in 2 cases,p.R117H in 2 cases,p.P146fs in 1 case,p.G141R in 1 case,p.V100G in 1 case and p.Y64C in 1 case.As compared to other 15 patients without mutation or without meaningful mutation,those with meaningful mutations had earlier onset age [0.5 (0.03,2) months vs 4 (0.23,9) months,P ＜ 0.05],higher proportion of cases with onset age less than 1 month old (66.7％ vs.26.7％,P ＜ 0.05),higher proportion of cases with perianal lesions (73.3％vs.36.4％,P ＜ 0.05) and lower remission rate after drug therapy (0 vs 46.7％,P＜ 0.01).Conclusions The frequency of monogenic mutation in Chinese VEO-IBD pediatric patients is quite high,and the most common mutation is IL-10RA gene mutation.VEO-IBD patients with gene mutation have earlier onset age,higher incidence of perianal lesion and lower remission rate after drug therapy.

Abstract:Abstract Objectives Upper gastrointestinal (UGI) tract involvement is frequently reported in pediatric Crohn disease (CD) and ulcerative colitis (UC). Aside from granulomas, most findings are nonspecific. The aims of this study were to review the prevalence of UGI tract findings in pediatric patients with CD or UC at diagnosis and to describe differences in endoscopic and histologic features. Methods Patients with CD and UC aged 2 to 17 years diagnosed between 2000 and 2015 who had upper and lower endoscopy at diagnosis were randomly chosen from the BC Children’s Hospital inflammatory bowel disease (IBD) registry. Pathology review of the UGI biopsy specimens was blinded to IBD diagnosis. Results Of the 198 patients, 102 with CD and 96 with UC were included, with a mean age of 11.7 years (range, 2.3-17 years). Patients with CD were more likely to have aphthous ulcers (20.4% vs 3.5%, P = .002) and erosions (16.3% vs 3.5%, P =.018), most commonly affecting the antrum. Macroscopically normal UGI endoscopy was present in 60% of patients. Microscopic disease was reported in 100% of patients with CD and 87% of patients with UC. In both groups, nonspecific inflammation was the most common finding. Chronic deep, superficial, and diffuse inflammation were more frequent among patients with CD than UC (42% vs 4%, P < .001; 60% vs 17%, P < .001; 50% vs 34%, P = .04, respectively). Conclusions The UGI tract macroscopic changes were common in pediatric IBD, especially in CD. Despite macroscopically normal endoscopy, histologic abnormalities were frequent. Although chronic inflammation was more often reported in patients with CD, aside from granulomas there were no unique histologic abnormalities unique to CD.

Abstract:Background and Aim: Accumulating evidence have implicated gut microbiota alterations in pediatric and adult patients with inflammatory bowel disease (IBD); however, the results of different studies are often inconsistent and even contradictory. It is believed that early changes in new-onset and treatment-naïve pediatric patients are more informative. We performed a systematic review to investigate the gut microbiota profiles in pediatric IBD and identify specific microbiota biomarkers associated with this disorder. Methods: Electronic databases were searched from inception to 31 July 2020 for studies that observed gut microbiota alterations in pediatric patients with IBD. Study quality was assessed using the Newcastle–Ottawa scale. Results: A total of 41 original studies investigating gut microbiota profiles in pediatric patients with IBD were included in this review. Several studies have reported a decrease in α-diversity and an overall difference in β-diversity. Although no specific gut microbiota alterations were consistently reported, a gain in Enterococcus and a significant decrease in Anaerostipes, Blautia, Coprococcus, Faecalibacterium, Roseburia, Ruminococcus , and Lachnospira were found in the majority of the included articles. Moreover, there is insufficient data to show specific microbiota bacteria associated with disease activity, location, and behavior in pediatric IBD. Conclusions: This systematic review identified evidence for differences in the abundance of some bacteria in pediatric patients with IBD when compared to patients without IBD; however, no clear overall conclusion could be drawn from the included studies due to inconsistent results and heterogeneous methodologies. Further studies with large samples that follow more rigorous and standardized methodologies are needed.

Abstract:Rapid and accurate clinical staging of pediatric patients with inflammatory bowel disease (IBD) is crucial to determine the appropriate therapeutic approach. This study aimed to identify effective, convenient biomarkers for staging IBD in pediatric patients. We recruited cohorts of pediatric patients with varying severities of IBD to compare the features of the intestinal microbiota and metabolites between the active and remitting disease stages. Metabolites with potential for staging were targeted for further assessment in both patients and colitis model mice. The performance of these markers was determined using machine learning and was validated in a separate patient cohort. Pediatric patients with IBD exhibited distinct gut microbiota structures at different stages of disease activity. The enterotypes of patients with remitting and active disease were Bacteroides-dominant and Escherichia-Shigella-dominant , respectively. The bile secretion pathway showed the most significant differences between the two stages. Fecal and serum bile acid (BA) levels were strongly related to disease activity in both children and mice. The ratio of primary BAs to secondary BAs in serum was developed as a novel comprehensive index, showing excellent diagnostic performance in stratifying IBD activity (0.84 area under the receiver operating characteristic curve in the primary cohort; 77% accuracy in the validation cohort). In conclusion, we report profound insights into the interactions between the gut microbiota and metabolites in pediatric IBD. Serum BAs have potential as biomarkers for classifying disease activity, and may facilitate the personalization of treatment for IBD.
