Abstract:Purpose: The primary objective of this study was to develop and validate an AI algorithm as a screening tool for the detection of retinopathy of prematurity (ROP). Participants: Images were collected from infants enrolled in the KIDROP tele-ROP screening program. Methods: We developed a deep learning (DL) algorithm with 227,326 wide-field images from multiple camera systems obtained from the KIDROP tele-ROP screening program in India over an 11-year period. 37,477 temporal retina images were utilized with the dataset split into train ( n = 25,982, 69.33%), validation ( n = 4,006, 10.69%), and an independent test set ( n = 7,489, 19.98%). The algorithm consists of a binary classifier that distinguishes between the presence of ROP (Stages 1–3) and the absence of ROP. The image labels were retrieved from the daily registers of the tele-ROP program. They consist of per-eye diagnoses provided by trained ROP graders based on all images captured during the screening session. Infants requiring treatment and a proportion of those not requiring urgent referral had an additional confirmatory diagnosis from an ROP specialist. Results: Of the 7,489 temporal images analyzed in the test set, 2,249 (30.0%) images showed the presence of ROP. The sensitivity and specificity to detect ROP was 91.46% (95% CI: 90.23%–92.59%) and 91.22% (95% CI: 90.42%–91.97%), respectively, while the positive predictive value (PPV) was 81.72% (95% CI: 80.37%–83.00%), negative predictive value (NPV) was 96.14% (95% CI: 95.60%–96.61%) and the AUROC was 0.970. Conclusion: The novel ROP screening algorithm demonstrated high sensitivity and specificity in detecting the presence of ROP. A prospective clinical validation in a real-world tele-ROP platform is under consideration. It has the potential to lower the number of screening sessions required to be conducted by a specialist for a high-risk preterm infant thus significantly improving workflow efficiency.

Abstract:Retinopathy of prematurity (ROP) is an abnormal blood vessel development in the retina of a prematurely-born infant or an infant with low birth weight. ROP is one of the leading causes for infant blindness globally. Early detection of ROP is critical to slow down and avert the progression to vision impairment caused by ROP. Yet there is limited awareness of ROP even among medical professionals. Consequently, dataset for ROP is limited if ever available, and is in general extremely imbalanced in terms of the ratio between negative images and positive ones. In this study, we formulate the problem of detecting ROP in retinal fundus images in an optimization framework, and apply state-of-art convolutional neural network techniques to solve this problem. Experimental results based on our models achieve 100 percent sensitivity, 96 percent specificity, 98 percent accuracy, and 96 percent precision. In addition, our study shows that as the network gets deeper, more significant features can be extracted for better understanding of ROP.

Abstract:Retinopathy of prematurity (ROP) is a proliferative vascular disease, which is one of the most dangerous and severe ocular complications in premature infants. Automatic ROP detection system can assist ophthalmologists in the diagnosis of ROP, which is safe, objective, and cost-effective. Unfortunately, due to the large local redundancy and the complex global dependencies in medical image processing, it is challenging to learn the discriminative representation from ROP-related fundus images. To bridge this gap, a novel attention-awareness and deep supervision based network (ADS-Net) is proposed to detect the existence of ROP (Normal or ROP) and 3-level ROP grading (Mild, Moderate, or Severe). First, to balance the problems of large local redundancy and complex global dependencies in images, we design a multi-semantic feature aggregation (MsFA) module based on self-attention mechanism to take full advantage of convolution and self-attention, generating attention-aware expressive features. Then, to solve the challenge of difficult training of deep model and further improve ROP detection performance, we propose an optimization strategy with deeply supervised loss. Finally, the proposed ADS-Net is evaluated on ROP screening and grading tasks with per-image and per-examination strategies, respectively. In terms of per-image classification pattern, the proposed ADS-Net achieves 0.9552 and 0.9037 for Kappa index in ROP screening and grading, respectively. Experimental results demonstrate that the proposed ADS-Net generally outperforms other state-of-the-art classification networks, showing the effectiveness of the proposed method.

Abstract:Retinopathy of Prematurity (ROP) is a fibrovascular proliferative disorder, which affects the developing peripheral retinal vasculature of premature infants. Early detection of ROP is possible in stage 1 and stage 2 characterized by demarcation line and ridge with width, which separates vascularised retina and the peripheral retina. To detect demarcation line/ ridge from neonatal retinal images is a complex task because of low contrast images. In this paper we focus on detection of ridge, the important landmark in ROP diagnosis, using Convolutional Neural Network(CNN). Our contribution is to use a CNN-based model Mask R-CNN for demarcation line/ridge detection allowing clinicians to detect ROP stage 2 better. The proposed system applies a pre-processing step of image enhancement to overcome poor image quality. In this study we use labelled neonatal images and we explore the use of CNN to localize ridge in these images. We used a dataset of 220 images of 45 babies from the KIDROP project. The system was trained on 175 retinal images with ground truth segmentation of ridge region. The system was tested on 45 images and reached detection accuracy of 0.88, showing that deep learning detection with pre-processing by image normalization allows robust detection of ROP in early stages.

Computer-Aided Diagnosis of Early-Stage Retinopathy of Prematurity in Neonatal Fundus Images Using Artificial Intelligence

Abstract:Retinopathy of Prematurity (ROP) is a retinal disorder affecting preterm babies, which can lead to permanent blindness without treatment. Early-stage ROP diagnosis is vital in providing optimal therapy for the neonates. The proposed study predicts early-stage ROP from neonatal fundus images using Machine Learning (ML) classifiers and Convolution Neural Networks (CNN) based pre-trained networks. The characteristic demarcation lines and ridges in early stage ROP are segmented utilising a novel Swin U-Net. 2000 Scale Invariant Feature Transform (SIFT) descriptors were extracted from the segmented ridges and are dimensionally reduced to 50 features using Principal Component Analysis (PCA). Seven ROP-specific features, including six Gray Level Co-occurrence Matrix (GLCM) and ridge length features, are extracted from the segmented image and are fused with the PCA reduced 50 SIFT features. Finally, three ML classifiers, such as Support Vector Machine (SVM), Random Forest (RF), and k- Nearest Neighbor (k-NN), are used to classify the 50 features to predict the early-stage ROP from Normal images. On the other hand, the raw retinal images are classified directly into normal and early-stage ROP using six pre-trained classifiers, namely ResNet50, ShuffleNet V2, EfficientNet, MobileNet, VGG16, and DarkNet19. It is seen that the ResNet50 network outperformed all other networks in predicting early-stage ROP with 89.5% accuracy, 87.5% sensitivity, 91.5% specificity, 91.1% precision, 88% NPV and an Area Under the Curve (AUC) of 0.92. Swin U-Net Convolutional Neural Networks (CNN) segmented the ridges and demarcation lines with an accuracy of 89.7% with 80.5% precision, 92.6% recall, 75.76% IoU, and 0.86 as the Dice coefficient. The SVM classifier using the 57 features from the segmented images achieved a classification accuracy of 88.75%, sensitivity of 90%, specificity of 87.5%, and an AUC of 0.91. The system can be utilised as a point-of-care diagnostic tool for ROP diagnosis of neonates in remote areas.

Abstract:Importance: A retinopathy of prematurity (ROP) diagnosis currently relies on indirect ophthalmoscopy assessed by experienced ophthalmologists. A deep learning algorithm based on retinal images may facilitate early detection and timely treatment of ROP to improve visual outcomes. Objective: To develop a retinal image-based, multidimensional, automated, deep learning platform for ROP screening and validate its performance accuracy. Design, setting, and participants: A total of 14 108 eyes of 8652 preterm infants who received ROP screening from 4 centers from November 4, 2010, to November 14, 2019, were included, and a total of 52 249 retinal images were randomly split into training, validation, and test sets. Four main dimensional independent classifiers were developed, including image quality, any stage of ROP, intraocular hemorrhage, and preplus/plus disease. Referral-warranted ROP was automatically generated by integrating the results of 4 classifiers at the image, eye, and patient levels. DeepSHAP, a method based on DeepLIFT and Shapley values (solution concepts in cooperative game theory), was adopted as the heat map technology to explain the predictions. The performance of the platform was further validated as compared with that of the experienced ROP experts. Data were analyzed from February 12, 2020, to June 24, 2020. Exposure: A deep learning algorithm. Main outcomes and measures: The performance of each classifier included true negative, false positive, false negative, true positive, F1 score, sensitivity, specificity, receiver operating characteristic, area under curve (AUC), and Cohen unweighted κ. Results: A total of 14 108 eyes of 8652 preterm infants (mean [SD] gestational age, 32.9 [3.1] weeks; 4818 boys [60.4%] of 7973 with known sex) received ROP screening. The performance of all classifiers achieved an F1 score of 0.718 to 0.981, a sensitivity of 0.918 to 0.982, a specificity of 0.949 to 0.992, and an AUC of 0.983 to 0.998, whereas that of the referral system achieved an F1 score of 0.898 to 0.956, a sensitivity of 0.981 to 0.986, a specificity of 0.939 to 0.974, and an AUC of 0.9901 to 0.9956. Fine-grained and class-discriminative heat maps were generated by DeepSHAP in real time. The platform achieved a Cohen unweighted κ of 0.86 to 0.98 compared with a Cohen κ of 0.93 to 0.98 by the ROP experts. Conclusions and relevance: In this diagnostic study, an automated ROP screening platform was able to identify and classify multidimensional pathologic lesions in the retinal images. This platform may be able to assist routine ROP screening in general and children hospitals.

Abstract:Retinopathy of Prematurity (ROP) is an eye disorder primarily affecting premature infants with lower weights. It causes proliferation of vessels in the retina and could result in vision loss and, eventually, retinal detachment, leading to blindness. While human experts can easily identify severe stages of ROP, the diagnosis of earlier stages, which are the most relevant to determining treatment choice, are much more affected by variability in subjective interpretations of human experts. In recent years, there has been a significant effort to automate the diagnosis using deep learning. This paper builds upon the success of previous models and develops a novel architecture, which combines object segmentation and convolutional neural networks (CNN) to construct an effective classifier of ROP stages 1-3 based on neonatal retinal images. Motivated by the fact that the formation and shape of a demarcation line in the retina is the distinguishing feature between earlier ROP stages, our proposed system first trains an object segmentation model to identify the demarcation line at a pixel level and adds the resulting mask as an additional "color" channel in the original image. Then, the system trains a CNN classifier based on the processed images to leverage information from both the original image and the mask, which helps direct the model's attention to the demarcation line. In a number of careful experiments comparing its performance to previous object segmentation systems and CNN-only systems trained on our dataset, our novel architecture significantly outperforms previous systems in accuracy, demonstrating the effectiveness of our proposed pipeline.

Abstract:Retinopathy of prematurity (ROP) is a severe condition affecting premature infants, leading to abnormal retinal blood vessel growth, retinal detachment, and potential blindness. While semi-automated systems have been used in the past to diagnose ROP-related plus disease by quantifying retinal vessel features, traditional machine learning (ML) models face challenges like accuracy and overfitting. Recent advancements in deep learning (DL), especially convolutional neural networks (CNNs), have significantly improved ROP detection and classification. The i-ROP deep learning (i-ROP-DL) system also shows promise in detecting plus disease, offering reliable ROP diagnosis potential. This research comprehensively examines the contemporary progress and challenges associated with using retinal imaging and artificial intelligence (AI) to detect ROP, offering valuable insights that can guide further investigation in this domain. Based on 89 original studies in this field (out of 1487 studies that were comprehensively reviewed), we concluded that traditional methods for ROP diagnosis suffer from subjectivity and manual analysis, leading to inconsistent clinical decisions. AI holds great promise for improving ROP management. This review explores AI's potential in ROP detection, classification, diagnosis, and prognosis.

Abstract:Segmentation of retinal structures, namely optic disc, vessel, demarcation line, and ridge, is essential for describing the characteristics of Retinopathy of Prematurity (ROP). Computerized systems are being developed for automatic segmentation in fundus images to assist the medical experts and bring consistency in the diagnosis. There are multiple challenges in the segmentation task of premature infants’ fundus images. The annotation and ground truth preparation required for the segmentation is complex, challenging, and expensive. Further, ROP datasets are not available publicly, and hence carrying out such a task needs a primary dataset and significant assistance from the domain expert. To address this gap, two primary datasets named HVDROPDB-BV and HVDROPDB-RIDGE were developed. The datasets consist of images captured by two different imaging systems, having different sizes, resolutions, and illumination. This made the trained models generic and robust to data variability and heterogeneity. We propose the modified U-Net architectures by incorporating squeeze and excitation (SE) blocks and attention gates (AG) to segment the demarcation line/ridge and vessel from these datasets. These modifications were tested and validated by ROP experts. The performance of all the three networks (U-Net, AG U-Net, and SE U-Net) was promising, with a variation of 1 to 6% in the dice coefficient for the HVDROPDB datasets. The area under the curve (AUC) obtained for all three networks was above 0.94, indicating them as excellent models. AG U-Net outperformed the other two networks, with a sensitivity of 96% and specificity of 89% for stage detection via new test images.

Novel Fundus Image Preprocessing for Retcam Images to Improve Deep Learning Classification of Retinopathy of Prematurity

Abstract:Retinopathy of Prematurity (ROP) is a potentially blinding eye disorder because of damage to the eye's retina which can affect babies born prematurely. Screening of ROP is essential for early detection and treatment. This is a laborious and manual process which requires trained physician performing dilated ophthalmological examination which can be subjective resulting in lower diagnosis success for clinically significant disease. Automated diagnostic methods can assist ophthalmologists increase diagnosis accuracy using deep learning. Several research groups have highlighted various approaches. Captured ROP Retcam images suffer from poor quality. This paper proposes the use of improved novel fundus preprocessing methods using pretrained transfer learning frameworks to create hybrid models to give higher diagnosis accuracy. Once trained and validated, the evaluations showed that these novel methods in comparison to traditional imaging processing contribute to better and in many aspects higher accuracy in classifying Plus disease, Stages of ROP and Zones in comparison to peer papers.

Abstract:Purpose: To evaluate the screening potential of a deep learning algorithm-derived severity score by determining its ability to detect clinically significant severe retinopathy of prematurity (ROP). Methods: Fundus photographs were collected, and standard panel diagnosis was generated for each examination by combining three independent image-based gradings. All images were analyzed using a deep learning algorithm, and a quantitative assessment of retinal vascular abnormality (DeepROP score) was assigned on a 1 to 100 scale. The area under the receiver operating curve and distribution pattern of all diagnostic parameters and categories of ROP were analyzed. The correlation between the DeepROP score and expert rank ordering according to overall ROP severity of 50 examinations was calculated. Results: A total of 9,882 individual examinations with 54,626 images from 2,801 infants were analyzed. Fifty-six examinations (0.6%) demonstrated Type 1 ROP and 54 examinations (0.5%) demonstrated Type 2 ROP. The DeepROP score had an area under the receiver operating curve of 0.981 for detecting Type 1 ROP and 0.986 for Type 2 ROP. There was a statistically significant correlation between the expert rank ordering of overall disease severity and the DeepROP score (correlation coefficient 0.758, P < 0.001). When hypothetical referral cutoff score of 35 was selected, all cases of severe ROP (Type 1 and Type 2 ROP) was captured and 8,562 eyes (87.6%) with no or mild ROP were excluded. Conclusion: The DeepROP score determined by deep learning algorithm was an objective and quantitative indicator for the severity of ROP, and it had potential in automated detecting clinically significant severe ROP.

Abstract:Abstract Retinopathy of prematurity (ROP) is currently one of the leading causes ofinfant blindness worldwide. In recent years, significant progress has been made incomputer-aided diagnostic methods based on deep learning. However, deeplearning is often data hungry, with its need for large amounts of annotated datafor model optimization, and in clinical scenarios, annotated data requires longhours of effort by experienced physicians. In contrast, a large number ofunlabelled images are relatively easy to obtain. In this paper, we propose a newsemi-supervised learning framework to reduce annotation costs by using partiallylabelled images and a large number of unlabelled images for automatic ROPstaging. Our semi-supervised deep learning framework consists of a studentmodel and a teacher model. We input images that have undergone differentperturbations into the student and teacher models. By encouraging consistencyof predicted output and inter-sample semantic structure between the student andteacher models, useful discriminative information is mined from the unlabelleddata to improve model classification performance. The teacher model parametersare updated by an exponential moving average algorithm. We have conductedextensive experiments on a real clinical dataset. Using only 30% of the labelledimages, our method achieves area under the receiver operating curve (AUC) of0.8630, Accuracy of 0.7895, Sensitivity of 0.7868, and Specificity of 0.8010. Theclassification performance of our method is close to that of using all 100%labelled images when the proportion of labelled images is 50%.
An Interpretable System for Screening the Severity Level of Retinopathy in Premature Infants Using Deep Learning

Abstract:Accurate evaluation of retinopathy of prematurity (ROP) severity is vital for screening and proper treatment. Current deep-learning-based automated AI systems for assessing ROP severity do not follow clinical guidelines and are opaque. The aim of this study is to develop an interpretable AI system by mimicking the clinical screening process to determine ROP severity level. A total of 6100 RetCam Ⅲ wide-field digital retinal images were collected from Guangdong Women and Children Hospital at Panyu (PY) and Zhongshan Ophthalmic Center (ZOC). A total of 3330 images of 520 pediatric patients from PY were annotated to train an object detection model to detect lesion type and location. A total of 2770 images of 81 pediatric patients from ZOC were annotated for stage, zone, and the presence of plus disease. Integrating stage, zone, and the presence of plus disease according to clinical guidelines yields ROP severity such that an interpretable AI system was developed to provide the stage from the lesion type, the zone from the lesion location, and the presence of plus disease from a plus disease classification model. The ROP severity was calculated accordingly and compared with the assessment of a human expert. Our method achieved an area under the curve (AUC) of 0.95 (95% confidence interval [CI] 0.90-0.98) in assessing the severity level of ROP. Compared with clinical doctors, our method achieved the highest F1 score value of 0.76 in assessing the severity level of ROP. In conclusion, we developed an interpretable AI system for assessing the severity level of ROP that shows significant potential for use in clinical practice for ROP severity level screening.

Abstract:BACKGROUND AND OBJECTIVE:Retinopathy of prematurity (ROP), a proliferative vascular eye disease, is one of the leading causes of blindness in childhood and prevails in premature infants with low-birth-weight. The recent progress in digital image analysis offers novel strategies for ROP diagnosis. This paper provides a comprehensive review on the development of digital diagnosing systems for ROP to software researchers. It may also be adopted as a guide to ophthalmologists for selecting the most suitable diagnostic software in the clinical setting, particularly for the remote ophthalmic support.METHODS:We review the latest literatures concerning the application of digital diagnosing systems for ROP. The diagnosing systems are analyzed and categorized. Articles published between 1998 and 2020 were screened with the two searching engines Pubmed and Google Scholar.RESULTS:Telemedicine is a method of remote image interpretation that can provide medical service to remote regions, and yet requires training to local operators. On the basis of image collection in telemedicine, computer-based image analytical systems for ROP were later developed. So far, the aforementioned systems have been mainly developed by virtue of classic machine learning, deep learning (DL) and multiple machine learning. During the past two decades, various computer-aided systems for ROP based on classic machine learning (e.g. RISA, ROPtool, CAIER) became available and have achieved satisfactory performance. Further, automated systems for ROP diagnosis based on DL are developed for clinical applications and exhibit high accuracy. Moreover, multiple instance learning is another method to establish an automated system for ROP detection besides DL, which, however, warrants further investigation in future.CONCLUSION:At present, the incorporation of computer-based image analysis with telemedicine potentially enables the detection, supervision and in-time treatment of ROP for the preterm babies.

Abstract:Retinopathy of Prematurity (ROP) is a critical eye disorder affecting premature infants, characterized by abnormal blood vessel development in the retina. Plus Disease, indicating severe ROP progression, plays a pivotal role in diagnosis. Recent advancements in Artificial Intelligence (AI) have shown parity with or surpass human experts in ROP detection, especially Plus Disease. However, the success of AI systems depends on high-quality datasets, emphasizing the need for collaboration and data sharing among researchers. To address this challenge, the paper introduces a new public dataset, FARFUM-RoP (Farabi and Ferdowsi University of Mashhad's ROP dataset), comprising 1533 ROP fundus images from 68 patients, annotated independently by five experienced childhood ophthalmologists as "Normal," "Pre-Plus," or "Plus." Ethical principles and consent were meticulously followed during data collection. The paper presents the dataset structure, patient details, and expert labels.

Abstract:Diabetic retinopathy (DR), a long-term complication of diabetes, is notoriously hard to detect in its early stages due to the fact that it only shows a subset of symptoms. Standard diagnostic procedures for DR now include OCT and digital fundus imaging. If digital fundus images alone could provide a reliable diagnosis, then eliminating the costly optical coherence tomography would be beneficial for all parties involved. Optometrists and their patients will find this useful. Using deep convolutional neural networks, we provide a novel approach to this problem. Our approach deviates from standard DCNN methods by exchanging typical max-pooling layers with fractional max-pooling ones. In order to collect more subtle information for categorisation, two such DCNNs, each with a different number of layers, are trained. To establish these limits, we use deep convolutional neural networks (DCNNs) and features extracted from picture metadata to train a support vector machine classifier. In our experiments, we used information from Kaggle's open DR detection database. We fed our model 34,124 training images, 1,000 validation examples, and 53,572 test images to train and test it. Each of the five classes in the proposed DR classifier corresponds to one of the steps in the DR process and is given a numeric value between 0 and 4. Experimental results show a higher identification rate (86.17%) than those found in the existing literature, indicating the suggested strategy may be effective. We have jointly developed an algorithm for machine learning and accompanying software, and we've named it Deep Retina. Images of the fundus acquired by the typical person using a portable ophthalmoscope may be instantly analyzed using our technology. This technology might be used for self-diagnosis, at-home care, and telemedicine.

Abstract:Importance Retinopathy of prematurity (ROP) is a leading cause of blindness in children, with significant disparities in outcomes between high-income and low-income countries, due in part to insufficient access to ROP screening. Objective To evaluate how well autonomous artificial intelligence (AI)–based ROP screening can detect more-than-mild ROP (mtmROP) and type 1 ROP. Design, Setting, and Participants This diagnostic study evaluated the performance of an AI algorithm, trained and calibrated using 2530 examinations from 843 infants in the Imaging and Informatics in Retinopathy of Prematurity (i-ROP) study, on 2 external datasets (6245 examinations from 1545 infants in the Stanford University Network for Diagnosis of ROP [SUNDROP] and 5635 examinations from 2699 infants in the Aravind Eye Care Systems [AECS] telemedicine programs). Data were taken from 11 and 48 neonatal care units in the US and India, respectively. Data were collected from January 2012 to July 2021, and data were analyzed from July to December 2023. Exposures An imaging processing pipeline was created using deep learning to autonomously identify mtmROP and type 1 ROP in eye examinations performed via telemedicine. Main Outcomes and Measures The area under the receiver operating characteristics curve (AUROC) as well as sensitivity and specificity for detection of mtmROP and type 1 ROP at the eye examination and patient levels. Results The prevalence of mtmROP and type 1 ROP were 5.9% (91 of 1545) and 1.2% (18 of 1545), respectively, in the SUNDROP dataset and 6.2% (168 of 2699) and 2.5% (68 of 2699) in the AECS dataset. Examination-level AUROCs for mtmROP and type 1 ROP were 0.896 and 0.985, respectively, in the SUNDROP dataset and 0.920 and 0.982 in the AECS dataset. At the cross-sectional examination level, mtmROP detection had high sensitivity (SUNDROP: mtmROP, 83.5%; 95% CI, 76.6-87.7; type 1 ROP, 82.2%; 95% CI, 81.2-83.1; AECS: mtmROP, 80.8%; 95% CI, 76.2-84.9; type 1 ROP, 87.8%; 95% CI, 86.8-88.7). At the patient level, all infants who developed type 1 ROP screened positive (SUNDROP: 100%; 95% CI, 81.4-100; AECS: 100%; 95% CI, 94.7-100) prior to diagnosis. Conclusions and Relevance Where and when ROP telemedicine programs can be implemented, autonomous ROP screening may be an effective force multiplier for secondary prevention of ROP.

Abstract:The purpose of this research is to propose a new method for identifying diabetic retinopathy using retinal fundus images. Currently, identifying diabetic retinopathy from computerized fundus images is a challenging task in medical image processing and requires new strategies to be developed. The manual analysis of the retinal fundus is time-consuming and requires a significant amount of skill. To assist clinicians, this research develops a graphical user interface that integrates imaging algorithms to assess whether the patient's fundus image is affected by diabetic retinopathy. The diagnosis is made using a deep neural network, specifically the Resnet152-V2, which has been shown to have 100% accuracy in all evaluation criteria including accuracy, recall, precision, and F1 Score. The severity of the disease is displayed on the graphical user interface and the patient's information is stored in a local database. This proposed method can also be used by ophthalmologists as a backup option to support in disease detection, reducing the necessary processing time.

Abstract:Retinopathy of prematurity (ROP) is a retinal disorder that occurs in preterm infants with low birth weight and is the leading cause of preventable blindness in children. Early identification of high-risk patients and early diagnosis and timely treatment of ROP can substantially improve patients' visual outcomes. However, manual screening consumes both time and resources. Telescreening using retinal fundus images has the potential to reduce the burden engendered by the necessity of on-site screening. Recently, substantial progress has been made in using computer-aided diagnosis with retinal fundus images, and this approach has attracted considerable attention for the diagnosis of eye diseases. Abnormalities of and alterations in retinal blood vessels may relate to the occurrence and progression of ROP. In this study, we examined the hypothesis that ROP severity may be associated with the angle and width of arteries and veins. We computationally determined the artery–artery and vein–vein angles in the temporal quadrants—the temporal artery angle (TAA) and temporal vein angle (TVA)—under normal conditions and in different ROP stages. We also estimated retinal vessel width—temporal artery width (TAW) and temporal vein width (TVW)—by applying the Radon transform method to fundus images. Our results revealed significant decreases in TAA and TVA and increases in TAW and TVW with increasing ROP severity (all P < 0.0001).In addition, we observed positive TAA–TVA and TAW–TVW correlations (both P < 0.0001). The TAA was negatively correlated with the TAW ( r = −0.162, P = 0.0314). These retinal vessel features may be useful in assisting ophthalmologists in the early detection of ROP and its progression.

Abstract:Diabetic Retinopathy (DR) is a consequence of diabetes mellitus that results in damage to the retina's blood vessel networks. It is now the major cause of irreversible blindness among diabetics. It is more prevalent in those with a history of diabetes mellitus. A high blood glucose level results in the loss of blood and other fluids from the retina's blood vessels. The importance of early detection and treatment of this illness cannot be overstated. In medical image processing, reliably detecting diabetic retinopathy from digital fundus pictures is considered an open challenge that requires the exploration of innovative approaches. An automated computerized system accomplishes this goal via a series of processes, including the identification and classification of lesions in fundus images. We are motivated by the researchers who chose to use deep learning for the detection of many diseases, like glaucoma, due to its recent breakthroughs and significant success over typical machine learning techniques in a variety of applications. The goal of this article is to develop reformed networks for detecting and categorizing diabetic retinopathy into five classes. Through this work, three novel Convolutional Neural Networks(CNNs) based on deep learning have been proposed for the prediction of diabetic retinopathy. The first proposed network is built from scratch, the second network is the ensemble of five best-performing networks, and the last one is the Convolutional Neural Network-Long Short Term Memory (CNN-LSTM) model. The performance of three proposed networks is compared with twenty-one well-accepted image nets. The effectiveness of the proposed reformed networks is validated using multiple performance indicators on seven experiments of three Kaggle datasets (separately and in combination of these). The presented work achieves the maximum accuracy of 0.9545 (highest average-wise 0.9368), a maximum F1 score of 0.9685 (highest average-wise 0.9374), the maximum sensitivity of 0.9566 (highest average-wise 0.9420),and a maximum AUC score of 0.9769 (highest average-wise 0.9395). The results of the experiments showed that the provided model outperforms the many current models in terms of classification. So, the results of the calculations show that with the proposed networks, retinal problems in diabetic patients can be effectively found so that they can get a better diagnosis and avoid losing their sight.
