The Discovery of Penicillin—New Insights After More Than 75 Years of Clinical Use

Abstract:After just over 75 years of penicillin’s clinical use, the world can see that its impact was immediate and profound. In 1928, a chance event in Alexander Fleming’s London laboratory changed the course of medicine. However, the purification and first clinical use of penicillin would take more than a decade. Unprecedented United States/Great Britain cooperation to produce penicillin was incredibly successful by 1943. This success overshadowed efforts to produce penicillin during World War II in Europe, particularly in the Netherlands. Information about these efforts, available only in the last 10–15 years, provides new insights into the story of the first antibiotic. Researchers in the Netherlands produced penicillin using their own production methods and marketed it in 1946, which eventually increased the penicillin supply and decreased the price. The unusual serendipity involved in the discovery of penicillin demonstrates the difficulties in finding new antibiotics and should remind health professionals to expertly manage these extraordinary medicines.

Abstract:Antibiotics have been developed for more than 80 years since the investigation of the sulfonamides and beta-lactams in 1930s. Meanwhile, the pathogenic bacteria developed resistance to antibiotics rapidly and have caused urgent threat to public health. In addition, many antibiotics are used commercially, or are potentially useful, in medicine for activities other than their antimicrobial action. They are used as anti-tumour agents, enzyme inhibitors including powerful hypocholesterolemic agents, immunosuppressive agents, and anti-migraine agents, etc. The purpose of this chapter is to focus on the application of antibacterials, antifungals, and anti-cancers with their clinical use to date, including the development history, side-effects, etc. The antibiotics summarized herein were classified by their uses, structure types, and molecular mechanisms.

Abstract:Antibiotics are one of the greatest inventions in human history and are used worldwide on an enormous scale. Besides its extensive usage in medical and veterinary arenas to treat and prevent the infection, its application is very prominent in other fields, including agriculture, aquaculture, and horticulture. In recent decades, the increased consumption of antibiotics in China saw a vast increase in its production and disposal in various environments. However, in this post-antibiotic era, the abuse and misuse of these valuable compounds could lead to the unreversible consequence of drug resistance. In China, antibiotics are given a broad discussion in various fields to reveal their impact on both human/animals health and the environment. To our knowledge, we are the first paper to look back at the development trend of antibiotic-related studies in China with qualitative and quantitative bibliometric analysis from the past decades. Our study identified and analyzed 5559 papers from its inception (1991) to December 6, 2021, from the Web of Science Core Collection database. However, with few authors and institutions focusing on long-term studies, we found the quality of contributions was uneven. Studies mainly focused on areas such as food science, clinical research, and environmental studies, including molecular biology, genetics and environmental, ecotoxicology, and nutrition, which indicate possible primary future trends. Our study reports on including potentially new keywords, studies' milestones, and their contribution to antibiotic research. We offer potential topics that may be important in upcoming years that could help guide future research.

Abstract:The principal point of this paper is that the discovery of penicillin and the development of the supporting technologies in microbiology and chemical engineering leading to its commercial scale production represent it as the medicine with the greatest impact on therapeutic outcomes. Our nomination of penicillin for the top therapeutic molecule rests on two lines of evidence concerning the impact of this event: (1) the magnitude of the therapeutic outcomes resulting from the clinical application of penicillin and the subsequent widespread use of antibiotics and (2) the technologies developed for production of penicillin, including both microbial strain selection and improvement plus chemical engineering methods responsible for successful submerged fermentation production. These became the basis for production of all subsequent antibiotics in use today. These same technologies became the model for the development and production of new types of bioproducts (i.e., anticancer agents, monoclonal antibodies, and industrial enzymes). The clinical impact of penicillin was large and immediate. By ushering in the widespread clinical use of antibiotics, penicillin was responsible for enabling the control of many infectious diseases that had previously burdened mankind, with subsequent impact on global population demographics. Moreover, the large cumulative public effect of the many new antibiotics and new bioproducts that were developed and commercialized on the basis of the science and technology after penicillin demonstrates that penicillin had the greatest therapeutic impact event of all times.

Abstract:Due to the overuse of antibiotics, bacterial resistance has markedly increased to become a global problem and a major threat to human health. Fortunately, in recent years, various new antibiotics have been developed through both improvements to traditional antibiotics and the discovery of antibiotics with novel mechanisms with the aim of addressing the decrease in the efficacy of traditional antibiotics. This manuscript reviews the antibiotics that have been approved for marketing in the last 20 years with an emphasis on the antibacterial properties, mechanisms, structure-activity relationships (SARs), and clinical safety of these antibiotics. Furthermore, the current deficiencies, opportunities for improvement, and prospects of antibiotics are thoroughly discussed to provide new insights for the design and development of safer and more potent antibiotics.

Abstract:OBJECTIVE: To provide references for clinical rational use of antibiotics.METHODS: The data including the consumption sum of antibiotics and the main pathogens clinically isolated in the first half year of 2010 were collected for statistical analysis with regard to the bacterial resistance to commonly used antibiotics.RESULTS: Cephalosporins,penicillins,macrolides and fluoroquinolones were the main anti-infective therapies in our hospital.The top five pathogens were coagulase negative staphylococcus,escherichia coli,pseudomonas aeruginosa,acinetobacter baumannii and staphylococcus aureus,which were highly resistant to commonly used antibiotics.CONCLUSION: The utilization frequency of antibiotics is positively correlated to bacterial resistance.Rational use and proper choice of categories of antibiotics and drug susceptibility test are of positive value for the anti-infective treatment.

Abstract:Antibiotics, widely used medications that have significantly increased life expectancy, possess a broad range of effects beyond their primary antibacterial activity. While some are recognized as adverse events, others have demonstrated unexpected benefits. These adjunctive effects, which have been defined as "pleiotropic" in the case of other pharmacological classes, include immunomodulatory properties and the modulation of the microbiota. Specifically, macrolides, tetracyclines, and fluoroquinolones have been shown to modulate the immune system in both acute and chronic conditions, including autoimmune disorders (e.g., rheumatoid arthritis, spondyloarthritis) and chronic inflammatory pulmonary diseases (e.g., asthma, chronic obstructive pulmonary disease). Azithromycin, in particular, is recommended for the long-term treatment of chronic inflammatory pulmonary diseases due to its well-established immunomodulatory effects. Furthermore, antibiotics influence the human microbiota. Rifaximin, for example, exerts a eubiotic effect that enhances the balance between the gut microbiota and the host immune cells and epithelial cells. These pleiotropic effects offer new therapeutic opportunities by interacting with human cells, signaling molecules, and bacteria involved in non-infectious diseases like spondyloarthritis and inflammatory bowel diseases. The aim of this review is to explore the pleiotropic potential of antibiotics, from molecular and cellular evidence to their clinical application, in order to optimize their use. Understanding these effects is essential to ensure careful use, particularly in consideration of the threat of antimicrobial resistance.

Abstract:We investigated sociocultural and economic drivers of human antimicrobial use (AMU) in Thailand through ethnographic research, interviews, focus groups and a cross-sectional survey. This community-based study generated findings clustered around three key themes: treatment-seeking practices, medicine use, and interpretation of biomedical constructs. Participants sought care from public health facilities for chronic conditions, but medicines from the private sector were considered more powerful and were preferred for acute complaints. Many antibiotics were unrecognised as such by consumers due to the practice at private healthcare facilities of dispensing repackaged medicines without identifying labels. This unseen use of antibiotics is probably driven by economic drivers including market competition in the private sector, policy implementation drivers whereby rational drug use policies mainly target the public sector, behavioural drivers relating to treatment seeking-practices, and sociocultural drivers that influenced participants' understanding of medical terms and concepts. Participants regarded antibiotics as reducing inflammation and were uncertain about the distinctions between anti-inflammatories, antibiotics, and pain relievers. Antimicrobial Resistance (AMR) was understood as a form of drug tolerance to be remedied by changing the medicine. Community surveys may not provide accurate estimates of AMU where people are unable to distinguish antibiotics reliably from other medicines.

Abstract:The antibiotics are an essential group of therapeutic drugs used to kill bacteria on various levels in the human body. These antibiotics had played a significant role for the treatment as well as the prevention of bacterial infections. The effectiveness of antibiotics against bacterial infections cannot be denied. However, an overuse and misuse of antibiotics, the current poor hygiene and contamination control mechanisms have lead to the improvement of antibiotic resistance. Anti-microbial or antibiotic resistance is an international public health issue, greatly dominant in the developing countries. Antibiotic resistance is a bacterial adaptation, which allows bacteria to persist regardless of the presence of antibiotics. Antibiotic resistance is a significant risk to human health and is being seen as a global environmental and economic risk. The relationship between bacterial resistance and misuse of antibiotics had been well documented, and was considered to be a major public health problem. Antibiotics are important to treat the bacterial infections but inaccurately prescription, misuse and overuse of antibiotics are elevating the antibiotic resistance. It is still a significant health problem in developing countries where not many hospitals have facilities for microbiology of clinical isolates which can assign to blind treatment. Change in the bacteriological profile due indiscriminate use of antibiotics has been associated with the appearance of multiple drug resistance strains. Information regarding the antibiotic susceptibility profile is essential in the selection of the most appropriate treatment and can minimize the antibiotic resistance. Preventive measures should be implemented in true sense to control antibiotic resistance. “Cleanliness is next to Godliness” and hence, clean surroundings as well as clean hands should be ensured. Soaps should be used for hand washing frequently. Awareness campaigns should be promoted and self-medication should be avoided. Antibiotic susceptibility investigations should be carried out for suspected bacterial infections.
Pharming animals: a global history of antibiotics in food production (1935–2017)

Abstract:Since their advent during the 1930s, antibiotics have not only had a dramatic impact on human medicine, but also on food production. On farms, whaling and fishing fleets as well as in processing plants and aquaculture operations, antibiotics were used to treat and prevent disease, increase feed conversion, and preserve food. Their rapid diffusion into nearly all areas of food production and processing was initially viewed as a story of progress on both sides of the Iron Curtain. However, from the mid-1950s onwards, agricultural antibiotic use also triggered increasing conflicts about drug residues and antimicrobial resistance (AMR). Significantly, antibiotic concerns did not develop evenly but instead gave rise to an international patchwork of different regulatory approaches. During a time of growing concerns about AMR and a post-antibiotic age, this article reconstructs the origins, global proliferation, and international regulation of agricultural antibiotics. It argues that policymakers need to remember the long history of regulatory failures that has resulted in current antibiotic infrastructures. For effective international stewardship to develop, it is necessary to address the economic dependencies, deep-rooted notions of development, and fragmented cultural understandings of risk, which all contribute to drive global antibiotic consumption and AMR.

Abstract:OBJECTIVE To estimate the status quo and existing problem of antibiotic use in Beijing Friendship(Hospital) and provide reference for safe,rational and effective clinic antibiotic use.METHODS The data of all(inpatients)(n=430) between Jan and Dec 2004 were collected randomly and analyzed(retrospectively).RESULTS Of the patients,80.2%(n=345) were receiving antibiotics.The rate of rational(antibiotic) use was 62.6%(n=216).The purpose of antibiotic use for prophylaxis alone was in 40.3%(n=139),for therapy alone was in 24.3%(n=84),for both therapy and prophylaxis was in 34.5%(n=119) and for ambiguous aim was in 0.9%(n=3).The purpose of antibiotic use as an empiric decision was in 95.1% and for pathogenic reasons was in 4.9%.In 56.4%,38.9%,4.2% and 0.6% of the users,were used one to four or more agents,respectively.The most widely used kinds of antibiotics were cephalosporins,quinolones and macrolides.Most(antibiotics) were administered(intravenously)(47.6%),secondly were taken orally(37.5%).CONCLUSIONS There is unreasonable antibiotic usage in clinical practice.It′s very important to control the preventive application of antibiotics and use antibiotics rationally according to the pathogeny and the test of drug sensitivity.

Abstract:The aims of this 2-year multicentre study are to systematically evaluate and establish a reference baseline for the resistance of clinically relevant bacterial isolates in 14 tertiary hospitals in China and 5 Swedish hospitals and to characterise the current use of antibiotics in these hospitals. We also aimed to assess the impact of national antibiotic administrative policies on antibiotic use and resistance patterns of the leading nosocomial pathogens in the hospitals. The present study was done at a time when Chinese health authorities launched a campaign to discourage over prescription of antibiotics. At present, it seems their efforts are having a meaningful impact. Following the implementation of the intensive control programme in Chinese tertiary hospitals, we found that the consumption levels of parenteral antimicrobials were reduced and accompanied by a significant increase in susceptibility patterns of both Gram-positive and Gram-negative bacteria, including ESBL-producing organisms. During the 2-year study period, no significant change in the level of resistance to commonly used antibiotic groups could be noted in the clinical isolates from the five Swedish tertiary hospitals.
[Antibiotic resistance : A challenge for society]

Abstract:Without doubt, drug resistance is now one of the greatest health threats of our time. Not even 100 years after the discovery of the first antibiotics that saved human lives, we find ourselves threatened by the thought of a post-antibiotic era. Currently it is estimated that around 700,000 people die annually as a consequence of drug-resistant infections. Antibiotic resistance is a natural phenomenon with bacteria having devised several ways of combating the antibiotic attack. Rather than being a spontaneous event, resistance becomes an integral part of the microbe's genetic make-up, spreading further between and across species. However, the misuse and overuse of antibiotics over the years has in fact allowed for resistant bacteria to thrive, while slowly wiping out sensitive bacteria. Spreading awareness and proper information in the community about the risks and consequences of a rapidly developing antibiotic resistance is essential in tackling this global problem. In working together as an entire community, we can find long-lasting solutions. The One Health concept includes human and veterinary medical sectors, the environmental sector, as well as various decision-making bodies that include individual action as well as national and international policymakers. The role of stakeholders like pharmaceutical companies and agriculturists must be given importance, too, thinking all the while in a global context. Only this way will we find solutions for sustainable healthcare and together help slow down the process of antibiotic resistance.

Abstract:BACKGROUND To design better antimicrobial stewardship programs, detailed data on the primary drivers and patterns of antibiotic use are needed. OBJECTIVE To characterize the indications for antibiotic therapy, agents used, duration, combinations, and microbiological justification in 6 acute-care US facilities with varied location, size, and type of antimicrobial stewardship programs. DESIGN, PARTICIPANTS, AND SETTING Retrospective medical chart review was performed on a random cross-sectional sample of 1,200 adult inpatients, hospitalized (>24 hrs) in 6 hospitals, and receiving at least 1 antibiotic dose on 4 index dates chosen at equal intervals through a 1-year study period (October 1, 2009–September 30, 2010). METHODS Infectious disease specialists recorded patient demographic characteristics, comorbidities, microbiological and radiological testing, and agents used, dose, duration, and indication for antibiotic prescriptions. RESULTS On the index dates 4,119 (60.5%) of 6,812 inpatients were receiving antibiotics. The random sample of 1,200 case patients was receiving 2,527 antibiotics (average: 2.1 per patient); 540 (21.4%) were prophylactic and 1,987 (78.6%) were therapeutic, of which 372 (18.7%) were pathogen-directed at start. Of the 1,615 empirical starts, 382 (23.7%) were subsequently pathogen-directed and 1,231 (76.2%) remained empirical. Use was primarily for respiratory (27.6% of prescriptions) followed by gastrointestinal (13.1%) infections. Fluoroquinolones, vancomycin, and antipseudomonal penicillins together accounted for 47.1% of therapy-days. CONCLUSIONS Use of broad-spectrum empirical therapy was prevalent in 6 US acute care facilities and in most instances was not subsequently pathogen directed. Fluoroquinolones, vancomycin, and antipseudomonal penicillins were the most frequently used antibiotics, particularly for respiratory indications. Infect. Control Hosp. Epidemiol. 2015;37(1):70–79

Abstract:Developing effective antibacterial solutions has become paramount in maintaining global health in this era of increasing bacterial threats and rampant antibiotic resistance. Traditional antibiotics have played a significant role in combating bacterial infections throughout history. However, the emergence of novel resistant strains necessitates constant innovation in antibacterial research. We have analyzed the data on antibacterials from the CAS Content CollectionTM, the largest human-curated collection of published scientific knowledge, proven valuable for quantitative analysis of global scientific knowledge. Our analysis focuses on mining the CAS Content Collection data for recent publications (since 2012). This article aims to explore the intricate landscape of antibacterial research while reviewing the advancement from traditional antibiotics to novel and emerging antibacterial strategies. By delving into the resistance mechanisms, this paper highlights the need to find alternate strategies to address the growing concern.

Abstract:BACKGROUND Disruptions in gut microbiota have been implicated in cardiometabolic disorders and other health outcomes. Antibiotics are known gut microbiota disruptors, but their long-term consequences on taxonomic composition of the gut microbiome remain underexplored. METHODS We investigated associations between register-based oral antibiotic use over 8 years and gut microbiota composition assessed with fecal shotgun metagenomics in 15,131 adults from the Swedish population-based studies SCAPIS, MOS, and SIMPLER. We applied multivariable regression models with the number of prescriptions in three pre-specified periods before fecal sampling (<1 year, 1-4, 4-8 years) as the main exposures and adjusted for sociodemographics, lifestyle, and comorbidities. Secondary analyses included participants with only one antibiotic course or none. RESULTS Antibiotic use <1 year before fecal sampling was associated with the greatest reduction in gut microbiota species diversity; however, antibiotic use 1-4 years and 4-8 years earlier was also associated with decreased diversity. Clindamycin, fluoroquinolones, and flucloxacillin accounted for most of the associations between antibiotic use and the abundance of individual species across all periods. Use of these three antibiotics 4-8 years earlier was associated with altered abundance of 10-14% of the species studied; use of penicillin V, extended-spectrum penicillins, and nitrofurantoin were associated with altered abundance of only a few species. Similar results were found when comparing one antibiotic course 4-8 years before sampling vs. none in the past 8 years. CONCLUSION Commonly prescribed antibiotics like clindamycin, fluoroquinolones, and the narrow-spectrum flucloxacillin appear to have long-lasting consequences for the gut microbiota.

At present, it is accepted that antibiotics have been one of the most common drugs in the world and early-life antibiotic use is associated with increased risk for several diseases. Antibiotic use during infancy will induce imbalances in gut microbiota, which is called dysbiosis. Therefore, more and more researches have been known about the impact of antibiotics on gut microbiota of infants. Here, we discuss some effects of antibiotics on gut microbiota and health of infants, including four types: structure of gut flora, metabolic capacity, diversity and stability of gut microbiota, risk of diseases. We also profile antibiotic resistance gene carried by gut microbiota and mechanism of intestinal drug-resistant bacteria. This article will also help provide recommendations for antibiotic use during infancy.

Abstract:Antibiotics are the super drugs that have revolutionized modern medicine by curing many infectious diseases caused by various microbes. They efficiently inhibit the growth and multiplication of the pathogenic microbes without causing adverse effects on the host. However, prescribing suboptimal antibiotic and overuse in agriculture and animal husbandry have led to the emergence of antimicrobial resistance, one of the most serious threats to global health at present. The efficacy of a new antibiotic is high when introduced; however, a small bacterial population attains resistance gradually and eventually survives. Understanding the mode of action of these miracle drugs, as well as their interaction with targets is very complex. However, it is necessary to fulfill the constant need for novel therapeutic alternatives to address the inevitable development of resistance. Therefore, considering the need of the hour, this article has been prepared to discuss the mode of action and recent advancements in the field of antibiotics. Efforts has also been made to highlight the current scenario of antimicrobial resistance and drug repurposing as a fast-track solution to combat the issue.
