Prediction for post-ERCP pancreatitis in non-elderly patients with common bile duct stones: a cross-sectional study at a major Chinese tertiary hospital (2015–2023)

LASSO regression was used to screen 22 out of 28 potential predictive variables, and a multivariable logistic regression model was used for modeling. The internal and external validity of the model was verified by evaluating the receiver operating characteristic (ROC) curve and the area under the curve (AUC). Additionally, an interactive web application for risk prediction was developed using the "shiny" package in R software. - **Results**: The incidence of PEP was 5.22% (48/919), significantly higher in non-elderly patients who were female, had hypertension, a history of pancreatitis, difficult cannulation, underwent endoscopic sphincterotomy (EST), had lower alkaline phosphatase levels, and smaller common bile duct diameters. The predictive performance in the test set and external validation set was 0.915 (95% CI, 0.858–0.972) and 0.838 (95% CI, 0.689–0.986), respectively. The results of the multivariable restricted cubic spline showed that the incidence of PEP increased in patients aged 33 to 50 years, with a neutrophil percentage greater than 58.90%, hemoglobin greater than 131 g/L, platelets less than 203.04 or greater than 241.40 × 10^9/L, total bilirubin greater than 18.39 umol/L, aspartate aminotransferase less than 36.56 IU/L, alkaline phosphatase less than 124.92 IU/L, and albumin less than 42.21 g/L. - **Conclusion**: The study established a regression model based on 22 non-zero coefficient variables that can accurately predict the risk of PEP in non-elderly patients with common bile duct stones. Additionally, a visual web version was developed to allow clinicians to assess the PEP risk of patients in real-time based on the above results and take preventive measures early.
