Abstract:Puberty represents the transition from childhood to adulthood and the attainment of reproductive capacity. The onset and tempo of puberty are impacted by genetic background, epigenetics, race and ethnicity, nutritional status, maternal conditions, and environmental factors. 1 Age at puberty has implications for health throughout the lifespan, and both early and delayed puberty are associated with adverse physical and mental health outcomes. 2 The large, retrospective cohort study by Kubo et al 3 included almost 110 000 children and is, to our knowledge, the first population-based US study that describes the timing of pubertal onset among Asian American, Native Hawaiian, and Pacific Islander ethnic subgroups. Using a comprehensive data set from Kaiser Permanente Northern California, the authors evaluated 11 ethnic subgroups with almost equal representation of females and males in each subgroup. Disaggregating the subgroups demonstrated differences in pubertal timing that were obscured when the groups were analyzed together. The subgroup differences were more pronounced in females, with the difference in median age at thelarche varying by 8 months among subgroups and the difference in pubarche varying by 14 months among subgroups. In males, the differences between earliest and latest subgroups in the median age of gonadarche and pubarche were 4 months and 8 months, respectively. Puberty marks the reactivation of the previously quiescent hypothalamic-pituitary-gonadal axis. The most important initial physical signs of true central puberty are thelarche in females and testicular enlargement (gonadarche) in males. 1 These physical findings are not always practical to document for population-based studies of puberty, particularly male testicular enlargement, but are important to distinguish central puberty from adrenarche, the maturational rise in adrenal androgens, which occurs independently of central puberty but contributes to pubarche. 1 A strength of the study by Kubo et al 3 is the inclusion of thelarche, gonadarche, and pubarche as outcomes of interest. Notably, the study found greater variability in the timing of pubarche in both females and males, 3 implying that genetic background contributes to differences in timing of androgen-mediated hair growth and/or adrenal gland activity. Furthermore, there was more variability in female thelarche than in male gonadarche. This aligns with data suggesting that there is a secular trend toward earlier thelarche in females 4 without concurrent solid evidence for earlier male gonadarche and that there are likely distinct factors affecting male and female central puberty. It has been shown that while age of thelarche is decreasing, the age at menarche is relatively preserved. 4 Kubo et al 3 did not report on age at menarche because of the availability of pediatrician-assessed sexual maturity rating findings. However, it would be interesting to investigate whether the same variations in age at menarche exist; this could shed light on pubertal tempo, which might not be uniform among subgroups. Additionally, considering that thelarche and menarche may be influenced to varying degrees by factors such as environmental exposure and nutrition, understanding their individual timing among subgroups is relevant. Obesity is associated with earlier pubertal timing in girls and possibly boys. 5 The average body mass index (BMI) of each subgroup throughout the study was not reported, but Kubo et al 3 addressed obesity as a potential confounder by performing a sensitivity analysis including only those individuals who had a prepubertal BMI between the 5th and 85th percentiles. The differences in pubertal timing among subgroups persisted after removing individuals with a BMI that fell outside this range, suggesting that the differences are not solely explained by obesity. The authors did not present these raw data, but given the rise in pediatric overweight and obesity broadly, it would be informative to see how many individuals were removed in total and from each subgroup due to a BMI outside the 5th to 85th percentiles. Asian children are reported to have the lowest prevalence of obesity in the US, 6 but little is known about the ethnic subgroups, which are not likely homogeneously at risk for obesity and, more importantly, its sequelae. Kubo et al 3 commented that early pubarche has been found to be associated with risks for cardiometabolic diseases, including type 2 diabetes and gestational diabetes, and that the disparities in pubertal timing described in their study may correspond to disparities in these cardiometabolic outcomes among ethnic subgroups in adults. Abstract:OBJECTIVE:To survey the current status of pubertal development of Chinese children and to compare the precocious puberty prevalence of different regions.METHODS:A cross-sectional epidemiological study was conducted on 18 707 children and adolescents aged 6≊18 y with male/female ratio of 9 812/8 895 from 6 representative geographical areas in China, including Beijing, Tianjin, Hangzhou, Shanghai, Chongqing and Nanning. The height, weight, waist circumference (WC), hip circumference(HC) and sexual maturation states (Tanner stages: breast stages for girls and testicular volume for boys) of children and adolescents were measured. Probit analysis was used to calculate the median age and 95% confidence interval (CI) for onset of breast and testicular development. The prevalence of precocious puberty of different regions and BMI, waist circumference of different groups were compared.RESULTS:Breast development before 8 y was observed in 2.91% of girls, and testicular volume 4 ml or more before 9 y was observed in 1.74% of boys. The median age of onset of Tanner stages 2 for breast development in girls was 9.69 y (95% CI: 9.63≊ 9.75); the median age of onset of puberty as indicated by Tanner stages 2 for testicular development in boys was 11.25 y (95%CI:11.19≊ 11.30). The prevalence of precocious puberty (43 girls and 37 boys) was 0.43% (80/18 707). The prevalence of precocious puberty in northern region was higher than that in southwest region (0.736% compared with 0.282% P<0.05). There was no difference in onset age of precocious puberty in girls among three regions; but the onset age of precocious puberty in boys was earlier in east China [(7.4±0.28)y]. The SD values of BMI and waist-to-hip ratio (W/H) in precocious puberty children were higher than those in the peer normal children. There was no difference in BMI,waist circumference and waist-to-hip ratio in the precocious puberty children among different regions.CONCLUSION:The current diagnostic criteria of precocious puberty are suitable for the children in the survey areas. The prevalence and the onset age of precocious puberty are various in different regions. A positive association between obesity and precocious puberty is found both in boys and girls.

Abstract:CONTEXT:Age of pubertal onset has been decreasing in many countries but there have been no data on pubertal development in Chinese children over the last decade.OBJECTIVE:The primary objective of the study was to evaluate the current status of sexual maturation in Chinese children and adolescents. Secondary objectives were to examine socioeconomic, lifestyle, and auxological associations with pubertal onset.METHODS:In this national, cross-sectional, community-based health survey, a multistage, stratified cluster random sampling method was used to select a nationally representative sample, consisting of 231 575 children and adolescents (123 232 boys and 108 343 girls) between 2017 and 2019. Growth parameters and pubertal staging were assessed by physical examination.RESULTS:Compared to 10 years previously, the median age of Tanner 2 breast development and menarche were similar at 9.65 years and 12.39 years respectively. However, male puberty occurred earlier with a median age of testicular volume ≥4 mL of 10.65 years. Pubertal onset did occur earlier at the extremes, with 3.3% of the girls with breast development at 6.5-6.99 years old, increasing to 5.8% by 7.5-7.99 years old. Early pubertal onset was also noted in boys, with a testicular volume ≥ 4 mL noted in 1.5% at 7.5-7.99 years, increasing to 3.5% at 8.5-8.99 years old. Obesity and overweight increased risk of developing earlier puberty relative to normal weight in both boys and girls.CONCLUSION:Over the past decade, pubertal development is occurring earlier in Chinese children. While the cause is multifactorial, overweight and obesity are associated with earlier puberty onset. The currently used normative pubertal data of precocious puberty may not be applicable to diagnose precocious puberty.

Abstract:BACKGROUND: Most epidemiologic studies of puberty have only 1 source of pubertal development information (maternal, self or clinical). Interpretation of results across studies requires data on reliability and validity across sources. METHODS: The LEGACY Girls Study, a 5-site prospective study of girls aged 6 to 13 years (n = 1040) collected information on breast and pubic hair development from mothers (for all daughters) and daughters (if ≥10 years) according to Tanner stage (T1–5) drawings. At 2 LEGACY sites, girls (n = 282) were also examined in the clinic by trained professionals. We assessed agreement (κ) and validity (sensitivity and specificity) with the clinical assessment (gold standard) for both the mothers’ and daughters’ assessment in the subcohort of 282. In the entire cohort, we examined the agreement between mothers and daughters. RESULTS: Compared with clinical assessment, sensitivity of maternal assessment for breast development was 77.2 and specificity was 94.3. In girls aged ≥11 years, self-assessment had higher sensitivity and specificity than maternal report. Specificity for both mothers and self, but not sensitivity, was significantly lower for overweight girls. In the overall cohort, maternal and daughter agreement for breast development and pubic hair development (T2+ vs T1) were similar (0.66, [95% confidence interval 0.58–0.75] and 0.69 [95% confidence interval 0.61–0.77], respectively), but declined with age. Mothers were more likely to report a lower Tanner stage for both breast and pubic hair compared with self-assessments. CONCLUSIONS: These differences in validity should be considered in studies measuring pubertal changes longitudinally when they do not have access to clinical assessments.

Characterizing differences in outpatient dermatologic care utilization among disaggregated Asian American subgroups

Abstract:Despite having significantly higher rates of atopic dermatitis, psoriasis, and pigmentary disorders compared to White patients, studies suggest that Asian Americans are underrepresented in outpatient dermatology clinics. In this study, we utilize the National Health Interview Survey (NHIS) and prioritize disaggregated analyses to evaluate differences between the most populous Asian American subgroups (Chinese, Filipino, Indian, and "Other") in utilization of outpatient dermatologic care. We utilized multivariable logistic regression to compare outpatient dermatologic care use between each Asian American subgroup and Non-Hispanic Whites. Out of 96,559 adults, our study included 5264 self-identified Asian American and 91,295 non-Hispanic White adults. Most Asian participants were female, had health insurance, and had incomes > 2 times above the federal poverty line. We found that, compared to 21.4% for NH whites, lifetime prevalence of total body skin exam was highest among Filipino Americans (12.3%) and lowest among Indian Americans (7%). Additionally, all Asian American subgroups had a significantly lower odd than NH Whites of ever having a total body skin exam, with Indian Americans having the lowest odds. While the benefit of TBSEs in Indian Americans is unclear, it is possible that differing cultural perceptions about dermatologic needs, barriers to care, or immigration status may be contributing to the observed difference. Furthermore, the Indian diaspora encapsulates a range of skin tones, risk factors, and behaviors that may differentially influence dermatologic disease risk, similar to trends identified among Hispanic patients (Trepanowski et al. in J Am Acad Dermatol 88:1206–1209, 2023). Additional research utilizing the seven national databases that have been identified as providing disaggregated Asian racial information (Kamal et al. in J Am Acad Dermatol, 2023) may be useful to further illuminate avenues for intervention.

Abstract:Early pubertal timing is associated with youth mental health problems, with association amplified or mitigated by characteristics of the residential neighborhood. Yet, limited research simultaneously examines the roles of neighborhood context and biological sex in this association. This study fills this research gap by examining sex-specific associations between pubertal timing and neighborhood income with youth mental health problems (internalizing and externalizing symptoms) in a longitudinal cohort of early adolescents in the United States (US). Participants were 9201 youth aged 9 or 10 years from the Adolescent Brain Cognitive Development Study. Pubertal timing was the average of parent- and youth-reported pubertal status standardized within sex and age. Outcomes variables were youths' internalizing and externalizing symptoms assessed at 1-year follow-up via parent survey. We evaluated interaction effects between pubertal timing and neighborhood income in a series of sex-stratified linear mixed effect models, adjusted for family and personal sociodemographic characteristics. In girls, earlier pubertal timing was associated with more internalizing ( β = 0.06, p < 0.001) and externalizing problems ( β = 0.07, p < 0.001) at 1-year follow-up, not moderated by neighborhood income. In boys, earlier pubertal timing was associated with more externalizing problems among youth living in high-income neighborhoods, but not among those in low-income neighborhoods (interaction- p = 0.006). Results suggest that pubertal timing may affect youth mental health differentially in boys and girls, depending on the neighborhood contexts. These findings highlight the importance of both biological and social forces in shaping adolescent mental health and, thus, have public health and clinical implications for health promotion.

Applying Life History Theory to Understand Earlier Onset of Puberty: An Adolescent Brain Cognitive Development Cohort Analysis

Abstract:Purpose: Life history theory posits that multigenerational exposure to adversity and deprivation influences childhood growth and development, including pubertal maturation. We applied this ecological, evolutionary theory to examine the contributions of distal and proximal adversity on early puberty, a potentially important marker for population health. Methods: Baseline data from 5,645 girls in the adolescent brain cognitive development study were included. Early puberty was defined as midlate/post pubertal development by age 9-11 years. The contributions of multigenerational Black/Indigenous (Black, Indigenous and People of Color [BIPOC]) or Hispanic identities, intergenerational mental health, economic deprivation, personal trauma exposure and mental health, and proximal biological factors of premature birth and body mass index on early puberty were examined with hierarchical modeling. Results: 1,225 girls (21.7%) had early puberty. BIPOC/Hispanic identity, familial adversity, economic deprivation, personal trauma, depression, and a higher body mass index contributed significantly toward early puberty. The effect of multigenerational adversity remained significant across models, but the likelihood of early puberty decreased sequentially for BIPOC and Hispanic youth as proximal adversities were added (e.g., OR decreased from 2.93 to 2.38 for BIPOC youth), supporting a synergistic effect of layered adversity on early puberty. Discussion: This analysis supports life history theory as a coherent framework to understand early puberty among girls. Findings suggest monitoring pubertal timing as a population health indictor, like birth weight, prematurity, or life expectancy. Addressing early puberty may require policy and social changes to mitigate the negative impact of multiple layers of adversity including racial/ethnic disadvantage, family, and individual mental health and trauma, as well as economic insecurity.
Racial/ethnic differences in correlates of psychological distress among five Asian-American subgroups and non-Hispanic Whites

Abstract:Objective: Despite their vastly different historical backgrounds, unique languages and variable pre- and post-immigration experiences, Asian-Americans are considered to share stressors surrounding immigration, but there is a gap in describing manifestations of possible mental distress. Thus, the purpose of this study was to explore and compare differences in factors associated with psychological distress among Asian subgroups including Chinese, Filipino, Vietnamese, Korean, Japanese, and non-Hispanic Whites. Design: Using a cross-sectional study design, California Health Interview Survey (CHIS) 2011/2012 data were analyzed. The sample consisted of 29,142 participants: 25,645 non-Hispanic Whites, 3497 non-Hispanic Asian-Americans, 1156 Chinese, 471 Filipinos, 864 Vietnamese, 704 Koreans, and 302 Japanese. Sociodemographic characteristics included gender, age group, marital status, education, poverty level, working status, health insurance, level of acculturation, social cohesion, neighborhood safety, and civic engagement. Physical health status included disability and chronic illness. Psychological distress was evaluated using the Kessler 6 (K6) scale. Results: Results showed that psychological distress levels ranged between 1.96 and 4.52 (p < .05) out of 24 and associated factors were significantly different among the five Asian subgroups and non-Hispanic Whites. Conclusions: The current study highlights the differences in characteristics of psychological distress among Asian subgroups. It underscores the significance of understanding individualized cultural and historical background in each Asian subgroup and subsequently developing and applying appropriate interventions for those groups. In addition, different influencing factors should be applied to assess and prioritize the needs of Asian subgroups to improve psychological distress. The study also warrants further investigation and careful description of each Asian subgroups.
Disparities in childhood leukemia survival for Asian, Native Hawaiian, and Pacific Islanders in the United States

Abstract:What's New? Survival of childhood acute lymphoid leukemia (ALL) may vary significantly among Asians, Native Hawaiians, and Pacific Islanders (ANHPI). However, these populations often are investigated as a single group, which ignores socioeconomic and racial/ethnic heterogeneity. Here, the authors disaggregated the ANHPI population to explore disparities in childhood ALL survival. Compared to Non‐Hispanic Whites, five‐year survival for Southeast Asians and East Asians was poorer. Patients in these subgroups experienced better survival when they lived in urban areas or were more recently diagnosed or diagnosed between ages 1 and 9. The results provide insight into differences in childhood ALL outcomes for ANHPI subgroups. While some previous studies disaggregated the Asian, Native Hawaiian, and Pacific Islander (ANHPI) population to investigate survival for childhood leukemia, further studies are needed to understand the differences between subpopulations. The aim of our study was to estimate 5‐year relative survival for patients with childhood leukemia and to investigate disparities in prognostic factors with disaggregation of the ANHPI population. We used the Surveillance, Epidemiology, and End Results Program 17 database and included 1881 ANHPI patients with childhood leukemia and 8772 non‐Hispanic White (NHW) patients with childhood leukemia. The Cox proportional hazards model was used to estimate hazard ratios for the risk of death. We observed lower 5‐year relative survival rates for Southeast Asian and East Asian compared to NHW patients with childhood leukemia for acute lymphoid leukemia (ALL). The survival rates were higher for patients diagnosed at 1–9 years of age, more recent years of diagnosis, and patients residing in urban areas. The risk of death was 42% higher for East Asian patients and 50% higher for Southeast Asian patients compared to NHW patients for childhood ALL. For prognostic factors among East Asian patients with childhood leukemia, higher risks of death were observed for patients diagnosed at <12 months old and for acute myeloid leukemia compared to ALL. Further studies are needed to elucidate the reasons behind the disparities in survival rates for Southeast Asian and East Asian patients with childhood leukemia, including socioeconomic and genetic contributions to leukemia risk and clinical responses to different therapeutic modalities.

Examination of the "model minority" stereotype through ethnicity and sexual orientation heterogeneity among Asian American youth

Abstract:Introduction: The "model minority" stereotype disguises heterogeneity among Asian American youth, many of whom are harmed by policies and attitudes that assume this population to be uniformly high achieving and "problem free." The current study uses an intersectional lens to disaggregate this population by ethnicity and sexual orientation subgroups to demonstrate differences in academic performance and substance use behavior among Asian American youth. This study also investigates the extent to which racial/ethnicity and sexual orientation-based bullying may explain such links. Methods: Participants included 65,091 Asian American youth (46.41% Southeast Asian; 37.01% East Asian; 16.58% South Asian) in grades 6-12, who were part of the California Healthy Kids Survey (2015-2017). Participants were 49.4% female and about a third each were in grades 6-8, grades 9-10, and grades 11-12. School-based surveys were administered. Youth reported on substance use, grades, and bias-based bullying experiences in the past 12 months. Results: Generalized linear mixed-effects model results showed that outcomes varied widely across youth ethnicity and sexual orientation subgroups. Inclusion of racial/ethnic and sexual orientation bullying in these models attenuated the direct effects between ethnic and sexual identities and academic performance and substance use outcomes. Conclusions: Implications of this work suggest that research and policy should not treat Asian American students as uniformly high-performing and low-risk, because the experiences of those who deviate from these assumptions will be left undetected. Interventions targeting bias-based bullying may be able to reduce disparities in academic and substance use outcomes among Asian American youth.
A Pathway to Positive Youth Development: Unpacking the Asian American Youth Paradox and Cultural Orientations among Filipino American and Korean American Youth

Abstract:This study used longitudinal survey data of Filipino American and Korean American youth in the Chicago Metropolitan area (N = 786, MAGE = 15.00, SD = 1.91 at Wave 1 in 2014) to examine whether and how a set of organized predictors (i.e., universal predictors of youth outcomes and cultural orientations) independently and collectively explains internalizing and externalizing problems and academic performance. The results were that universal predictors such as youth antisocial beliefs, peer antisocial behaviors, and the quality of parent-child relations, were extensively predictive of youth outcomes in the expected directions. The magnitudes of universal predictors were largely unchanged when bilinear and multidimensional cultural orientation variables were accounted for together. The magnitudes of cultural orientation variables were slightly attenuated in full models but showed independent associations with youth outcomes. Specifically, English and heritage language proficiencies were protective of externalizing and internalizing problems. Behavioral practices in respective cultures increased youth problems. In addition, ethnic identity, although beneficial to mental health, can increase externalizing problems. The findings of this study provide insights into understanding the mixed outcomes among Asian Americans and important empirical evidence that can inform intervention programs to prevent youth problems, ultimately toward a pathway to positive youth development among Asian American youth.

Using an 8-year longitudinal study of 444 (54% female; initial Mage = 13-year-old) Chinese American adolescents, we identified two (well- vs. poorly adjusted) concurrent and three (stably well-adjusted, improved, and risk) latent transition profiles from early adolescence to emerging adulthood based on six indicators of academic, behavioral, and socioemotional functioning. Multilevel modeling revealed higher levels of family economic stress or discrimination independently associated with higher likelihood to be in the subgroup of adolescents with poorly adjusted patterns across development. Within an individual, Chinese American adolescents were more likely to have worse adjustment patterns in the year when they experienced higher economic stress or discrimination relative to other study years, suggesting that their adjustment are sensitive to the fluctuation of stress level during development. Longitudinally, those who experienced higher family economic stress or discrimination at early adolescence were more likely to become at risk when they reached adulthood. Higher discrimination predicted the likelihood to be in the improved groups but only among adolescents with low (but not high) economic stress, suggesting that the influence of discrimination may vary by family socioeconomic status. Findings (a) challenge the model minorities assumption, (b) highlight the importance of capturing multiple ecological contexts associated with heterogeneous development among Chinese American adolescents, and (c) provide longitudinal evidence to social policies aiming to reduce wealth disparities and anti-Asian racism. Abstract:Acculturation/enculturation has been found to impact childhood health and obesity status. The objective of this study is to use cross-sectional data to examine the association between proxies of adult/caregiver acculturation/enculturation and child health status (Body Mass Index [BMI], waist circumference [WC], and acanthosis nigricans [AN]) in the U.S.-Affiliated Pacific Islands (USAPI), Alaska, and Hawaiʻi. Study participants were from the Children's Healthy Living (CHL) Program, an environmental intervention trial and obesity prevalence survey. Anthropometric data from 2–8 year olds and parent/caregiver questionnaires were used in this analysis. The results of this study (n = 4121) saw that those parents/caregivers who identified as traditional had children who were protected against overweight/obesity (OWOB) status and WC > 75th percentile (compared to the integrated culture identity) when adjusted for significant variables from the descriptive analysis. AN did not have a significant association with cultural classification. Future interventions in the USAPI, Alaska, and Hawaiʻi may want to focus efforts on parents/caregivers who associated with an integrated cultural group as an opportunity to improve health and reduce child OWOB prevalence.

Abstract:To provide clinically meaningful, normative reference data that describe pubertal timing indicators among urban and rural Chinese girls.Girls of Chinese Han ethnicity from grade 1 in elementary schools through grade 12 in junior high schools in eight regions participated in China Puberty Research Collaboration from October 2010 to May 2011. At examination, stages of breast and pubic hair development were assessed by trained physicians. Data on menses were collected through questionnaire survey.A total of 15,388 girls aged 6.0-18.9 years participated in this collaboration study. Among them, approximately 53.5% (8,238) came from rural areas. The median ages at stage 2 of breast development of Chinese girls were 9.18 (95% confidence interval: 8.91-9.44) years and at stage 2 for pubic hair growth were 11.65 (95% confidence interval: 11.41-11.90) years. Girls with body mass index ≥85th percentile had earlier age for B2 (Tanner stage 2 for breast development), B3 (Tanner stage 3 for breast development), PH2 (Tanner stage 2 for pubic hair development), PH3 (Tanner stage 3 for pubic hair development), and menarche compared with girls with body mass index < 85th percentile. Urban girls achieved PH2, PH3, and menarche at a much earlier age than rural girls, except for B2 and B3 attainment. Mean menarcheal age is 12.43 years.This study presented up-to-date data on female pubertal timing in a national sample of Chinese Han ethnicity girls and separately for urban and rural areas.

Abstract:Background: There are limited data regarding modifiable lifestyle behaviors and prediabetes among US Asian/Pacific Islander (API) children. This study characterizes the association of obesogenic behaviors and weight with prediabetes among API adolescents with obesity receiving pediatric care. Methods: We used retrospective data from 3,233 API adolescents aged 13-17 years in a US healthcare system who had well child visits in 2016-2019, provided well-check information on health behaviors, had BMI ≥95th percentile, and HbA1c level (within 1 year). Age, sex, self-reported daily obesogenic behaviors (eating <5 servings of fruits/vegetables, drinking ≥12 oz juice/soda, exercising for <60 minutes, >2 hours screen time), obesity level (Class I,2,3) and neighborhood deprivation index (NDI) were examined. Prediabetes was defined as HbA1c 5.7-6.4%; those with HbA1c ≥6.5% or diabetes diagnosis were excluded. Multivariable logistic regression was used to examine the association of these factors and prediabetes. Results: Among 3,233 API adolescents (mean age 14.6 ± 1.4 y; 63.8% male) with obesity, 22.4% had prediabetes (84.4% had HbA1c 5.7-5.9%). Prediabetes did not differ by age or sex but increased with obesity level (20.1%, 25.2%, 33.2% for Class 1, 2, 3 obesity, respectively). In unadjusted analyses, low exercise was marginally associated with prediabetes (p=0.07) but not diet or screen time. In fully adjusted multivariable analyses, obesity severity (OR 1.98 [95% CI 1.47-2.64] for class 3, OR 1.34 [1.10-1.63] for class 2 vs 1), and low exercise (OR 1.20 [1.01-1.44]) were associated with increased odds of prediabetes, whereas other modifiable behaviors were not. Conclusion: About 1 in 5 API adolescents with obesity had prediabetes, associated with obesity severity and low exercise. These findings support efforts that address obesity and promote physical activity in API children to reduce the risk of diabetes. Further studies should examine potential differences by ethnic group. Comparison of physician- and self-assessed pubertal onset in Japanese children

Abstract:Introduction: Physical examinations to assess pubertal development are challenging in large epidemiological surveys. This study aimed to assess the reliability of judgment of pubertal onset in Japanese children by the original pubertal self-assessment sheet. Methods: A total of 144 children aged 10 or 12 years were recruited between March 2019 and September 2020 from the pediatric endocrine outpatient clinics of participating institutions. Agreement between the physician- and participantassessed pubertal onsets was determined using unweighted kappa (UK) and Gwet's agreement coefficient (AC1). Results: The physician's assessment of pubertal onset was in slight agreement with that of the self-assessment sheet in 10-year-old boys (UK: 0.23 and AC1: 0.14), whereas the agreement between the physician's assessment and self-assessment sheet results was good and the physician's assessment was fair (UK: 0.64 and AC1: 0.94) in 12-year-old boys. The physician's assessment of pubertal onset were in good and moderate agreement with the self-assessment sheet in 10-year-old girls (UK/AC1: 0.74/0.78, respectively). In 12-year-old girls, although it showed poor agreement with UK (0.46), there was a very good agreement with AC1 (0.88). Conclusions: Although self-assessment of breast development was in good agreement with that of the physician's assessment for determining pubertal onset in girls, large-scale epidemiological studies are difficult to conduct for adolescent boys, especially for those in the early pubertal stage.

An Updated Analysis of Pubertal Linear Growth Characteristics and Age at Menarche in Ethnic Chinese

Abstract:Objectives: Concerns regarding change in the onset and tempo of pubertal growth in ethnic Chinese have posed a need for current information on growth characteristics. This study is to update the normative data of pubertal linear growth characteristics and distribution of age at menarche in healthy Chinese adolescents.Methods: A multistage stratified cluster sampling investigation based on age and sex was designed for anthropometric data collection in 2008, including 15,204 healthy boys and 13,047 healthy girls. The PB1 mathematical growth model was utilized to derive biological parameters of the adolescent growth curves. Ages at menarche were estimated in 6,476 girls through probit analysis at the ages at which 10%, 25%, 50%, 75%, and 90% of the girls attained menarche.Results: The peak growth age (PGA) was 12.6 years for boys and 10.6 years for girls. The PHV was 6.91 cm/yr in boys and 6.69 cm/yr in girls. The overall increments of adult height since 1985 were 3.3 cm for males and 2.3 cm for females, yielded rates of 1.4 and 1.0 cm/decade, respectively. Less than 10% of Chinese girls experience onset of menses before 11.38 years, and approximately 90% of all Chinese girls are menstruating by 13.88 years, with a median age of 12.63 years.Conclusions: The current study showed an advancing trend in PGA in both Chinese boys and girls. Decreasing menarche age was also observed for Chinese girls. These updated data would serve as useful reference for interpretation of endocrine and growth status and growth disorders in Chinese during peripubertal period. Am. J. Hum. Biol. 23:132-137, 2011. Abstract:BACKGROUND:Ethnic differences in body fat distribution contribute to ethnic differences in cardiovascular morbidities and diabetes. However few data are available on differences in fat distribution in Asian children from various backgrounds. Therefore, the current study aimed to explore ethnic differences in body fat distribution among Asian children from four countries.METHODS:A total of 758 children aged 8-10 y from China, Lebanon, Malaysia and Thailand were recruited using a non-random purposive sampling approach to enrol children encompassing a wide BMI range. Height, weight, waist circumference (WC), fat mass (FM, derived from total body water [TBW] estimation using the deuterium dilution technique) and skinfold thickness (SFT) at biceps, triceps, subscapular, supraspinale and medial calf were collected.RESULTS:After controlling for height and weight, Chinese and Thai children had a significantly higher WC than their Lebanese and Malay counterparts. Chinese and Thais tended to have higher trunk fat deposits than Lebanese and Malays reflected in trunk SFT, trunk/upper extremity ratio or supraspinale/upper extremity ratio after adjustment for age and total body fat. The subscapular/supraspinale skinfold ratio was lower in Chinese and Thais compared with Lebanese and Malays after correcting for trunk SFT.CONCLUSIONS:Asian pre-pubertal children from different origins vary in body fat distribution. These results indicate the importance of population-specific WC cut-off points or other fat distribution indices to identify the population at risk of obesity-related health problems.

Abstract:Introduction: The HPV vaccination rates among Asian American adolescents were found to be below optimal levels. Understanding the high-risk demographics and the various factors influencing HPV vaccine acceptance is essential for addressing disparities and enhancing vaccination rates in this population. This study aimed to investigate HPV vaccine uptake in adolescents aged 13 to 17 across six major Asian American ethnic groups using nationally representative data. Methods: The study analyzed data from the National Immunization Survey- Teen (2015-2019), comprising 3,968 Asian American adolescents from Asian Indian, Chinese, Filipino, Japanese, Korean, and Vietnamese backgrounds. Provider and practice characteristics were examined alongside HPV vaccine initiation and completion rates within each subgroup. Results: Findings revealed that while the overall initiation rate among Asian American adolescents was 66.65%, significant variations existed among subgroups, ranging from 57.3% in Asian Indians to 71.22% in Chinese adolescents. Similarly, completion rates ranged from 30.89% in Asian Indians to 48.14% in Chinese adolescents, with an aggregated completion rate of 40.37% for the total Asian American sample. Household survey data generally showed lower initiation and completion rates compared to provider-reported data but still reflected subgroup disparities. Conclusion: Across all ethnic groups, a common barrier to HPV vaccination was the lack of physician recommendation. However, slight variations in this barrier were observed among subgroups. These findings underscore the importance of considering socioeconomic, cultural, and acculturation-related factors in designing effective public health and clinical interventions to promote HPV vaccination among Asian American adolescents.
