Abstract:Introduction: Health profession schools have acknowledged the need for a diverse workforce by increasing diversity in recruitment, but little has been done to build inclusive excellence in learning environments. Microaggressions and other forms of mistreatment can increase stress levels and depression and negatively impact academic performance. To increase student performance, retention, and wellness, mitigating microaggressions is needed to promote an inclusive culture. Methods: We designed this workshop as a framework to think critically about microaggressions, how they impact the health professions academic environment, and how administrators, faculty, and students can promote inclusion excellence. The workshop included a presentation discussing microaggression theory, seven cases describing microaggressions in the health professions education environment, and discussion and facilitator guides. Cases were based on prior research conducted by the primary author and upon interactions authors shared from their professional experience. Participants completed pre- and postsurveys. Results: During six workshops at three different institutions, 138 out of 190 participants (73% response rate), including nursing and medicine faculty, students, and leadership, completed the pre- and postsurveys. Pre- and posttraining measurements found statistically significant improvements in participants' knowledge of the impact of microaggressions, self-efficacy in responding to microaggressions, and commitment to being an active bystander in the face of microaggressions. Participants were highly satisfied with the training. Discussion: This humanistic, case-based learning curriculum allows facilitators to guide faculty, student, and leadership conversations to build skills to promote inclusion excellence through preventing microaggressions, repairing and reestablishing relationships, and restoring reputations once microaggressions occur.

Abstract:Purpose: Increased attention to improving a culture of belonging in clinical learning environments has led to various approaches to addressing microaggressions. However, most approaches in the literature focus on responding or reacting to microaggressions with insufficient attention to building trust before microaggressions might occur. Research on microaggressions in clinical learning environments suggests anticipatory or pre-emptive conversations about microaggressions may foster greater trust. In this study, the authors explored how diverse participants perceived the experience of anticipatory conversations about potential microaggressions. Overall, the authors sought to gain a deeper understanding of how pre-emptive and anticipatory conversations may influence an organization's approach to addressing microaggressions in clinical learning environments. Methods: The authors utilized constructivist grounded theory methodology and conducted individual qualitative interviews with 21 participants in an academic department within a larger health sciences center in the United States. Results: Findings suggest that anticipatory conversations about microaggressions were challenging due to existing norms in dynamic clinical learning and working environments. Participants shared that the idea of anticipating microaggressions elicited dissonance. Conversations about microaggressions could potentially be facilitated through leaders who role model vulnerability, organizational supports, and an individualized approach for each team member and their role within a complex hierarchical organization. Discussion: Anticipating and addressing microaggressions in clinical learning environments holds tremendous potential, however, any conversations about personal identity remain challenging in medical and healthcare environments. This study suggests that any attempts to address microaggressions requires attention to cultural norms within healthcare environments and the ways that hierarchical organizations can constrain individual agency.
Microaggression Interactions Among Healthcare Professionals, Trainees and Students in the Clinical Environment: A Mixed-Studies Review

Abstract:This review aims to comprehensively consolidate and synthesize the existing body of empirical research on the perceptions and experiences of healthcare professionals, trainees, and students concerning microaggression interactions within diverse clinical settings. The review protocol was registered with the international prospective register of systematic reviews (PROSPERO CRD42024546443). We employed the Joanna Briggs Institute's mixed-methods systematic review approach. Comprehensive electronic database searches were conducted across eight databases (Medline, Embase, CINAHL, PsycINFO, Scopus, Web of Science, Social Science Database, and ProQuest Dissertation and Theses) from their respective inception dates up to October 1, 2023. The selection criteria included studies reporting instances of interprofessional or intra-professional microaggressions occurring in clinical environments among healthcare professionals, trainees, and students identifying as heterosexual and without disabilities. In total, 37 studies met the eligibility criteria and were included in our review. Our analysis revealed three primary themes: Racial Microaggressions in the Clinical Environment, Gender Microaggressions in the Clinical Environment, and Religious Microaggressions in the Clinical Environment. Research must delve deeply into these experiences to better understand their long-term consequences, while policy initiatives should be designed to establish equitable, inclusive, and respectful healthcare environments. Collaborative endeavors bridging research and policy are paramount to dismantling barriers and nurturing an inclusive healthcare landscape.
The Need for Understanding and Addressing Microaggressions in the Workplace

Abstract:The special issue on microaggressions highlighted how subtle interpersonal bias is complex, harms its targets, and reinforces established systems of inequity. The aim of this commentary is to contribute an organizational science perspective to this insightful and important dialogue. Given that the workplace is a microcosm of the broader society where adults spend most of their waking hours, studying microaggressions in this context can shed light on their unique manifestations and consequences, as well as methods to address this unique source of workplace adversity. Unlike other social contexts, many employees do not have complete autonomy over whom they interact with (e.g., choosing one's supervisor, officemates, or clients) and what they are able to convey authentically and safely in those interactions (i.e., choices about how one responds to microaggressions). As a result, people from minoritized backgrounds must often maintain professional relationships with colleagues or supervisors who harbor bias and (un)consciously convey it via microinvalidations, microassaults, and microinsults. Further, reward systems in the world of work (e.g., wages, promotions) may not only reflect but also reinforce systems of disadvantage (e.g., who experiences social mobility). In addition to understanding microaggressions in the workplace, there is a need to actively address them. We engage with Ong et al., Johnson et al., and Spanierman et al. to suggest that organizational understanding and remediation of microaggressions offers a viable avenue for challenging systems of oppression and fostering employee and organizational resilience to adversity.
Impact of racial microaggressions in the clinical learning environment and review of best practices to support learners

Abstract:Microaggressions are brief, commonplace, and daily verbal, behavioral, and environmental slights towards marginalized groups. Underrepresented in medicine (UiM) learners experience a barrage of microaggressions in the pediatric clinical learning environment. Literature demonstrates that microaggressions are detrimental to the mental and physical well-being of the recipient. There is an urgent need to bring greater awareness and understanding of how microaggressions operate in the clinical learning environment, the impact they have on UiM learners and the educational and institutional strategies needed to best support learners. Several databases were searched using combinations of subject headings and keywords as described. While "microaggressions" is not yet an official medical subject heading (Mesh term) for the National Library of Medicine's databases, nonetheless there are numerous studies using the term microaggressions in the research literature about health care, medical education and learning environments. Items were limited to English language publications from the past 10 years. Microaggressions are rooted in power differentials and indirect assertion of power. The clinical learning environment contains learners at all levels along with patients and families. The impact of microaggressions leads to (1) othering (2) stunted professional identity formation and (3) racial battle fatigue. Both educational initiatives targeted at recognizing and responding to microaggressions and institutional anonymous reporting systems and policies are best practices to combat the negative impact of microaggressions. Institutions-at-large and graduate medical education need to create safe spaces for anonymous reporting, robust policies for addressing bias and educational initiatives for responding to microaggressions. Together these will begin to create safe spaces for our UiM learners in a system that so often perpetuates they do not belong.

Abstract:This paper proposes a victim-centered account of microaggressions within the context of clinical medicine. In so doing, it argues that microaggressions can undermine physician-patient relationships, preclude relationships of trust, and therefore compromise the kind and quality of care that patients deserve. Ultimately, by focusing on the experiences of victims of microaggressions, the paper demonstrates how harmful microaggressions in clinical medical contexts can be, and thus provides strong reasons why healthcare providers ought to know about them and actively work to avoid committing them.

Microaggressions in Medicine: A Pilot Study on Differences and Determinants among Doctors and Nurses

Abstract:Introduction "Microaggression", encapsulates the notion that subtle and commonplace instances of discrimination and bias, can result in psychological and emotional distress, further entrenching inequality and cultivating a hostile social atmosphere for marginalized individuals or collectives. Studies endeavors to shed light on illuminating the impact of microaggressions on healthcare workers have found that they have consistently underscored their pervasive detrimental effects. This study aims to investigate the current status of microaggression encounters among healthcare workers, alongside an examination of the contributing risk factors associated with the occurrence of such microaggressions. Methods A total of 190 aged 18–60 years clinical healthcare practitioners were recruited from March to April 2023. Questionnaires including the Everyday Discrimination Scale-9 items (EDS). Results A total of 83 nurses [82(98.8%) female] and 107 doctors [54(50.5%) female] participated. Among the participants, 40(37.4%) doctors and 50(60.2%) nurses reported encountering microaggressions. Notably, the prevalence of microaggressions among nurses was significantly higher than that among doctors (P = 0.002). Binary logistic regression analysis provided insights into the independent factors influencing the experience of microaggressions. For doctors, the department emerged as a significant influencer (reference level = internal; ORauxiliary=6.138, Pauxiliary=0.016), for nurses, age (reference level = 18 ~ 35y; OR36 ~ 60=3.497, P36 ~ 60=0.037), department (reference level = internal; ORauxiliary=0.072, Pauxiliary=0.007), and family structure (reference level = nuclear family; ORbig family=0.242, Pbig family=0.012) demonstrated significant influence of experience of microaggressions. Conclusions Healthcare professionals have encountered a significant prevalence of microaggressions, with a distinct impact observed among nurses. The encounters with microaggressions within the healthcare workforce have exhibited a robust connection with symptoms of anxiety and depression. Specifically, doctors employed in auxiliary departments have been identified as being at a heightened risk of encountering microaggressions in comparison to their peers in internal medicine. Conversely, nurses stationed in auxiliary departments face an elevated risk in contrast to their counterparts in internal medicine. Moreover, among nurses, an advanced age and living in a nuclear family (as opposed to big family) have been identified as factors contributing to an increased vulnerability to microaggressions.

Abstract:Introduction: Although the ACGME and other accrediting organizations are increasingly emphasizing the importance of clinical learning environments that value diversity, equity, and inclusion, faculty development surrounding behavioral skills that promote inclusivity in the learning environment still needs cultivation. We designed a virtual longitudinal faculty development curriculum focused on direct observation, feedback, and practice of behavioral skills to acknowledge and address microaggressions in the learning environment. Methods: We used Kern's six steps of curriculum development to create four voluntary virtual workshops offered twice throughout the academic year, with topics including: (1) recognizing and naming microaggressions, (2) apologizing when harm has been experienced, (3) setting expectations surrounding microaggressions, and (4) debriefing microaggressions. Participant learners included residency program directors, associate program directors, and other leaders across all medical and surgical departments from one institution. Results: Thirty-one faculty from 10 departments participated in this yearlong curriculum. Pre- and postworkshop surveys analyzed participants' self-assessments of confidence and comfort in applying learned skills. Participants were more confident in openly naming bias, delivering expectations surrounding microaggressions, and debriefing microaggressions with learners. Participants also reported greater comfort in apologizing to learners when harm has occurred in public, in person, and electronically. Discussion: To create an inclusive learning environment, faculty can increase their comfort and confidence with addressing bias and microaggressions through practice and feedback. Our curriculum demonstrates how experiential learning allows for continual practice to solidify a new skill.

Abstract:Background This pilot study, conducted at a public univerisity in an urban community, collected the experiences of 42 social work students enrolled in a required course on diversity. Objective This research utilizes Critical Race Theory as a framework and expands on the themes experienced by recievers of microaggression insults, assaults, and invalidation (Sue et al., 2007). Methods Participants in this qualitative study completed three survey questions about: their microaggression experiences, immediate responses, and how they felt. Results Student feelings provided beginning insights as to how they coped with their microaggression experiences. Conclusion Social work educators may find this research supports the importance of cultural awareness and culturally responsive pedagogy.

Abstract:Introduction: Graduate trainees from diverse backgrounds may experience discrimination, mistreatment, and microaggressions. While the ability to identify and respond to microaggressions is a much-needed skill for all emerging trainees, limited training workshops exist for residents, especially within perioperative medicine. To embody the principles of diversity, equity, inclusion, and anti-racism (DEIA), we aimed to empower trainees in the perioperative environment with several strategies for addressing microaggressions to bridge this training gap. Methods: Based on critical race theory, transformative learning, minority stress theory, and the structural theory of gender and power, this workshop was developed with the primary aim of educating trainees on microaggressions, amplifying the role of allyship, and providing tools to respond to microaggressions as an ally. We used a mixed methods approach to examine participants' pre/post self-evaluations of microaggression intervention tools and the overall effectiveness of the workshop. Results: The postsurvey captured the experiences of 54 trainees, including 37 of 44 (84%) first-year clinical anesthesia residents and 14 of 24 (58%) surgery residents. The facilitator and course feedback was remarkably positive. Paired t test analyses on participants' pre- and postsurvey responses demonstrated a statistically significant increase in knowledge of microaggressions. This workshop also significantly increased learners' self-reported tools for responding to microaggressions. Discussion: Overall, these promising findings suggest that the strategies presented in this workshop could be applied across other graduate medical education programs. Institutions may wish to customize workshop elements, such as the case scenarios, and the workshop can also be incorporated within a DEIA curriculum.
Faculty Physician and Trainee Experiences with Micro- and Macroaggressions: a Qualitative Study

Abstract:Background: Micro- and macroaggressions are often stereotype threats that can have detrimental effects on the recipients. Survey data shows that these aggressions are happening. However, there are few qualitative studies on trainees and faculty physicians' experiences with such aggressions and their impact. Objective: Explore how micro- and macroaggressions impact physician trainees and faculty. Design, setting, participants: Virtual, one-on-one, semi-structured interviews were conducted between February and September 2021, among 14 physicians and trainees (medical students, residents, fellows, and faculty) at a tertiary, urban, US academic medical center and its associated hospitals. Participants shared their experiences with micro- and macroaggressions in training and the workplace, as well as their thoughts on intervention and education. Approach: Qualitative interviews; grounded theory approach KEY RESULTS: A total of 14 physicians and trainees (5 faculty, 2 fellows, 5 residents, 2 students; 11 [79%] women) participated. Four themes with multiple subthemes surfaced: definition, the moment an aggression is experienced, aftereffect of an aggression, and education and training. While general definitions of micro- and macroaggressions were similar among participants, some may have overlooked the inclusion of a marginalized group as central to each term. Both types of aggressions had a range of effects on participants, with faculty noting a cumulative effect. Institutional diversity was identified as a key source of support. Ideas on how to combat such acts included mandatory educational programs and policies, with the acknowledgment that much effort and time are necessary to change mindset and culture. Conclusions: Faculty physicians and medical trainees shared their personal experiences with micro- and macroaggressions during work and training. Participants described various emotions in the moment but also noted that these aggressions often had lasting impacts. They recognized the challenges of finding a solution to micro- and macroaggressions. Institution-wide education was favored by many as a first step.
Simulation Training to Interrupt Microaggressions

Abstract:Microaggressions in health care occur frequently and negatively impact the well-being of trainees. High-realism simulation can effectively train health care providers to communicate in emotionally difficult situations. In 2023, we developed and piloted 4 simulation scenarios for pediatric residents centered on addressing microaggressions in the clinical environment that built on an existing didactic curriculum. These scenarios included single and intersecting forms of oppression including racism, sexism, ableism, ethnocentrism, and weight bias. We also trained faculty, who had no prior simulation debriefing experience, to facilitate and debrief the simulation sessions. Thirty-three residents participated and reported an increase in confidence immediately following the simulation training; this increase was sustained at 3 months. Faculty participants reported increased empathy for residents, recognition of microaggressions, and confidence facilitating conversation after microaggressions that occur both in the simulated setting and in real life. High-realism simulation holds promise as a way to bridge the gap between classroom and real-life interruption of microaggressions, a necessary skill to improve the health care environment for learners and patients.

Abstract:What do "microbes" have to do with social equity? These microorganisms are integral to our health, that of our natural environment, and even the "health" of the environments we build. The loss, gain, and retention of microorganisms—their flow between humans and the environment—can greatly impact our health. It is well-known that inequalities in access to perinatal care, healthy foods, quality housing, and the natural environment can create and arise from social inequality. Here, we focus on the argument that access to beneficial microorganisms is a facet of public health, and health inequality may be compounded by inequitable microbial exposure.

Abstract:This article examines the history, complexity, and psychotherapeutic approaches to microaggressions, everyday verbal, nonverbal, or environmental slights that convey hostile messages to marginalised individuals. The concept traces back to the works of Derald Wing Sue, with microaggressions often related to discrimination, stereotypes, and prejudices, causing psychological harm. The Positive Psychotherapy (PPT) approach uniquely addresses microaggressions by transforming negative experiences into opportunities for personal development. The article further delves into the interconnectedness of aggression and microaggressions in contemporary society, highlighting the influence of power dynamics, capitalism, and critical social theories. Methodological recommendations and mindfulness-based interventions are also discussed, emphasising self-awareness, non-judgmental acceptance, emotional regulation, empathy, and cognitive flexibility. Addressing microaggressions necessitates a collaborative effort to promote understanding, empathy, and respect for diversity and difference in all aspects of life.
Is microaggression an oxymoron? A mixed methods study on attitudes toward racial microaggressions among United States university students

Abstract:To best understand the possible negative health and social consequences associated with racial microaggression, in-depth understanding of how people judge these events is needed. People of Color (POC) and White participants ( N = 64) were recruited for a mixed-methods study that incorporated quantitative attitude ratings and focus group interviews. Participants read and discussed their attitudes toward five vignettes that reflected microassault, microinsult, and microinvalidation scenarios. Semantic differential ratings showed that participants judged microassaults to be most unacceptable, followed by microinsults and then microinvalidations. Using a grounded theory approach, our qualitative analysis of interview data revealed five thematic categories. First, participants judged receivers’ psychological harm to be a critical consideration for their attitudes toward microaggression scenarios; they discussed factors associated with individual differences in appraisals, prior exposures to discrimination, and sensitivity to race. Second, participants were less consistent in their opinion about the role of the deliverers’ intent on their judgment of microaggressions; many considered microaggression events to be results of deliverers’ cultural ignorance and racial insensitivity. Third, our analysis revealed the central importance of contexts that shaped participants’ attitudes toward microaggression. Fourth, participants also discussed the notion that receivers of microaggression were racist for calling attention to race issues. Finally, POC participants tended to relate to the vignettes and use their lived experiences to contextualize their opinions about racial microaggression. The current results raise concerns regarding the conceptualization and utility of the word “microaggression,” especially within the broader contexts of racism and major discrimination. Other empirical and practical implications are discussed.

Abstract:Background The increasing pursuit of enhanced educational opportunities has led to a significant rise in international student enrollment in various fields, including nursing. Nursing is currently in its early stages and faces challenges related to racial microaggression. Understanding the dynamics of racial microaggression is crucial in countries like Turkey, where students from diverse ethnic backgrounds are accommodated. Objective To explore and achieve a more profound insight into the lived encounters of ethnic minority nursing students confronting racial microaggression. Method The study employed a descriptive phenomenological approach. Data collection involved conducting in-depth interviews from February 1, 2023, to June 1, 2023. Analysis was performed utilizing Colaizzi's analysis method. Results Each participant in the study encountered at least one type of microaggression. The analysis identified three clear themes: "challenges in social interactions," "unfavorable learning atmosphere," and "aspirations for the future." Conclusion This study highlights the crucial need to establish secure and inclusive environments that foster authentic discussions within academic settings. Faculty and educators should strengthen their ability to consider diverse perspectives in various scenarios. Moreover, integrating an up-to-date and comprehensive curriculum, along with the adoption of inclusive language, into the nursing program is essential for effectively addressing these concerns.

Abstract:Background and purpose: This paper describes the context and experiences of a workshop to raise knowledge and awareness of a college of pharmacy's faculty and staff about microaggressive behaviors and implicit biases. The workshop was intended to provide a non-threatening, interactive, and informative professional development program to demonstrate the cumulative marginalizing effects on students, faculty, and staff who may perceive themselves as targets. Educational activity and setting: A half-day workshop was conducted during July 2018. Participants were initially provided with definitions and categories of microaggression and implicit bias. To bring the subject matter "alive" and foster receptivity, interactive videos were shown with scenarios depicting situations reflective of microaggressions and implicit biases. College faculty, staff, and students made these relatable. To foster objectivity, an outside consultant was hired to facilitate the ensuing roundtable and plenary discussions. Findings: Sixty-eight participants responded to a pre-survey designed by the workshop team, and 78% indicated never having attended a training/seminar on microaggression and/or implicit bias. Sixty-two individuals responded to the post-survey with 92% indicating increased knowledge gained from workshop. Anecdotal reports suggested that the workshop had an ongoing impact, as faculty and staff continued the discussions in subsequent months and requested additional training sessions. Summary: The workshop heightened awareness and increased faculty and staff knowledge on microaggressive behaviors, implicit biases, and the potential consequences thereof. It also demonstrated the importance of addressing conversations that are perceived as difficult, in order to create a diverse and inclusive workplace and learning environment for all.

Abstract:Most health care professionals spend a substantial amount of their time at the workplace. Our interactions with team members can define our daily experiences, impact our work performance, and influence our overall job satisfaction. Over the last years, how we interact with colleagues and patients has changed with the introduction of social media, a tenser political climate, and an evolving health care system. In oncology, a team can be composed of medical students, clinicians, and support and administrative staff within a heavy emotional environment where some of our patients are facing the risk of early mortality and most are dealing with the unmeasurable burden of cancer. Many of these factors can increase the risk for professionalism lapses. We discuss common challenges faced in the practice of cancer care, including the generational gap between medical trainees and senior members, gender disparities, and microaggressions. Microaggressions represent verbal, behavioral, and environmental indignities that communicate hostile, derogatory, and negative slights that insult a target person or group. Microaggressions should not be accepted as the norm in the workplace. It is essential to recognize these negative behaviors and manage them effectively to reduce or even prevent the long-term toxicities that these behaviors can bring to the workplace environment. Ultimately, we must acknowledge that these issues exist and remember that education and collaboration are the pillars of an inclusive workplace. We owe such efforts to our patients who deserve good care, to our partners in the care of patients so that they feel supported and included, and to ourselves.

Abstract:Cet article vise à sensibiliser les psychologues, psychiatres, psychothérapeutes, travailleurs sociaux et autres professionnels de la santé mentale aux microagressions raciales vécues par les patients noirs, autochtones et de couleur, ainsi qu’à leur fournir des stratégies pour mieux les évaluer, les réduire et les éliminer dans la pratique clinique. Les microagressions raciales se définissent comme des formes subtiles ou cachées de racisme, souvent involontaires, dirigées contre les personnes de couleur. Les microagressions peuvent être des actions, des commentaires ou même des objets dans l’environnement. Qu’elle soit intentionnelle ou non, les microagressions sont une forme néfaste de racisme. Elles ont été corrélées à de nombreux problèmes de santé mentale chez les patients et lorsqu’elles sont perpétrées par des cliniciens elles peuvent aggraver les problèmes en créant un obstacle dans la relation clinique et à la réussite du traitement. Les patients qui subissent des microagressions de la part des prestataires de soins peuvent être moins engagés, moins enclins de suivre les recommandations du traitement et moins susceptibles de revenir pour des soins de suivi. Les microagressions sont l’un des facteurs pouvant expliquer la sous-utilisation et l’interruption prématurée des soins de santé mentale par les personnes de couleur. En plus de décrire les microagressions et des mécanismes sous-jacents, cet article fournit des exemples tirés de la pratique clinique visant à mieux guider et former les professionnels de la santé. Sont également incluses des mesures d’évaluation de la fréquence et de l’impact des microagressions raciales chez les patients de couleur, disponibles en français. This article is intended to raise awareness among psychologists, psychiatrists, psychotherapists, social workers, and other mental health care professionals about the problem of racial microaggressions in clinical settings. Racial microaggressions are subtle or covert forms of racism, often unintentional, directed toward people of colour. Microaggressions may be actions, comments, or even things in the environment. Regardless of the intention, racial microaggressions are a harmful form of racism. Microaggressions have been linked to a number of mental health problems in patients, and when committed by clinicians, they can further compound problems by creating a barrier to treatment. Patients experiencing microaggressions from providers may be less engaged, less amenable to treatment recommendations, and less likely to return for follow-up care. As such, this may account for patterns of underutilization and premature termination of mental health care for people of colour. In addition to describing microaggressions and their underlying mechanisms, this article provides examples from clinical practice to better help health care professionals eliminate microaggressions in clinical settings. Also included are measures to evaluate the frequency and impact of racial microaggressions on patients of colour which are available in French.
