Abstract:Multiplexed imaging offers great potential for precise and accurate diagnosis and therapy. Integrating multiplexed imaging with theranostics is promising to develop cancer therapeutics and diagnostics. However, the lack of versatile platforms with inherent different modalities and various probes hinders the potential of multiplexing imaging for cancer diagnosis and therapy. Here, we demonstrate that multiplexed imaging with advanced theranostic nanomaterials provides a potent method for cancer diagnostics and therapeutics both in vitro and in vivo. Unlike previous theranostic nanoplatforms, the prepared coordination nanomaterials have designable functionalities that are capable of imaging multiple intracellular biomarkers, multiple modality imaging, and photothermal ablation of tumor. Strikingly, this nanoplatform could specifically detect and image different tumor-related mRNAs simultaneously; allow infrared thermal, photoacoustic, and magnetic resonance imaging; and enable effective photothermal therapy to inhibit tumor growth in the MCF-7-derived mice xenograft tumor model. This study suggests that the nanoplatforms with designable functionalities may ultimately enable monitoring cancer processes, therapeutic mechanisms, and treatment outcomes. Exploring and exploiting multiplexed imaging with versatile theranostic nanoparticles may offer a powerful framework for precision medicine.

Abstract:Molecular imaging agents and a parallel progress in instrumentation ofimaginingtechnology have demonstrated to be effective in improving diagnosis, prognosis, planning, and monitoring of personalized medication. Molecular imaging modalities include positron emission tomography (PET), single photon emission computed tomography (SPECT), magnetic resonance imaging (MRI), computed tomography (CT), ultrasound (US), and optical (Raman, quantum dots, bioluminescence). Among these imaging modalities, PET and SPECT agents could provide target specific information as well as function, pathway activities, and cell migration in the intact organism. Furthermore, the radiotracer could noninvasively assess diseases treatment endpoints which used to rely almost exclusively on biopsies and histopathological assays. New leads on the development of personalized theranostic (image and treat) agents would allow more accuracy in the selection of patients who may respond to treatment. Topics covered in this special issue include advances in biomarkers in preclinical drug discovery, PET/CT and SPECT/CT in disease management, image-guided therapy approach of diseases, imaging technology in drug development, and progress in instrumentation ofimagingtechnology. For instance, upregulation of transporter expressions has often been observed in tumor cells to facilitate their accelerated rates of uptake. Advances in biomarkers in transporter system-based imaging in oncology and neurological diseases such as amino acid (FL. Kong et al.), glucose (YH. Zhang et al.) and dopaminergic (LH. Shen et al.; HF. Hou et al.) transporters were reported. These biomarkers showed that specific uptake in lesions compared to clinic imaging agent. In addition, these biomarkers were capable to be labeled with theranostic isotopes for personalized medication. Most tumors have a considerable proportion of hypoxic cells that are resistant to radio/chemotherapy, with a high propensity to metastasize, and result in worse therapeutic outcome. The contribution by M. Ali et al. and H. Fuji report their newly developed 99mTc-N4-NIM and 125I-IPOS probes for tumor hypoxia. The preclinical findings showed that these biomarkers could image tumor hypoxia by SPECT. The ability to quantify tissue hypoxia would allow the physicians to select patients for additional or alternative treatment regimens that would circumvent the ominous impact of hypoxia. Along this line, L. Jiang and coworkers report the peptide based radiotracer targeting tumor angiogenesis via VEGF integrin alpha(v) beta3. J. Sims-Mourtada and co-workers report 131I-labeled derivatives of the sonic hedgehog (HH) protein for detection of cancer cells via HH receptors. Molecular imaging of cancer stem cell trafficking was reported by T. Xia and co-workers. Hybrid molecular imaging modalities (PET/CT, SPECT/CT) provide high-sensitivity functional and high resolution anatomical imaging, which are important in design personalized treatment. To avoid radiation exposure from multiple slices-CT, K. Tang and co-workers report a reduction of CT tube voltage from 120 kv to 80 kv, the radiation dose could be reduced by 32–42% without losing low-contrast detectability. F. Chao and H. Zhang report the value of using PET/CT in the staging of nonsmall cell lung cancer. Y. Hu and co-workers report the application of PET/CT in assessment of treatment outcome. The application of SPECT/CT in the differential diagnosis of bone metastasis (Y. Zhang et al.) and Parkinson's disease (L. Wang et al.) was reported. Miyake K and co-workers report the application of FDG, MET, and FLT-PET/CT for the management of gliomas. L. Yang and co-workers report the value of using myocardial contrast echocardiography quantitative analysis during adenosine stress over visual analysis before therapy in acute coronary pain. The quantitative analysis correlates well with thrombosis in myocardial infarction. In summary, molecular imaging could integrate metabolomics and chemical biology. Molecular imaging agents could characterize target expressions, understand the disease progression, prediction for drug response and toxicity, staging, grading, and micrometastasis, and support animal studies. This special issue provides a platform of efficacy of personalized medication from molecular imaging technology which may have high impact on drug discovery, delivery, and development. This special issue provides original research and review of new trends in molecular imaging agent development. Among clinic molecular imaging modalities, radionuclide imaging technique is the most sensitive and could provide target-specific information. The radiotracer could non-invasively assess disease treatment endpoints which used to rely almost exclusively on biopsies and histopathological assays. L.-H. Shen et al. (Taiwan) reviewed molecular imaging in the diagnosis and management of CNS disorders. F.-L. Kong et al. (USA), N. Tsao et al. (USA), and C.-L. Ho et al. (Taiwan) report pharmacokinetics and dosimetry of molecular imaging compounds in oncology. M.-H. Liao et al. (Taiwan), X. Wang et al. (China), C. Wu et al. (China) and M. Tian et al. (USA) reported that molecular imaging techniques play a major role in the development of novel therapies since they measure target expressions (apoptosis, glycolysis) as well as function, pathway activities, and cell migration in the intact organism. Y.-F. Wang et al. and M.-Y. Chao et al. (Taiwan) described the kit probe for 99mTc-labeling which is crucial for centralized drug process. The kit probe may allow theranostic concept. A theranostic agent would be more accurate in the evaluation of patient response to treatment. The radiotracer could also be used for radionuclide therapy as reported by C. Wang et al. (China). Hybrid molecular imaging modalities (PET/CT, SPECT/CT) provide high-sensitivity functional and high-resolution anatomical imaging which are important in design-personalized treatment. M.-C. Chen et al. (Taiwan) reported the value of using I-131 NP-59 SPECT/CT to diagnose patients with subclinical or atypical features of primary aldosteronism. To proof of concept for molecular biology findings using different imaging modalities (optical, sonography) were described by Z. Pu et al. (China). In summary, molecular imaging enables the comprehensive characterization of therapeutic intervention and can be used in preclinical studies, pharmacokinetic studies, dose-finding studies, and proof-of-concept studies. Abstract:At present, cancer is obviously a major threat to human health worldwide. Accurate diagnosis and treatment are in great demand and have become an effective method to alleviate the development of cancer and improve the survival rate of patients. A large number of theranostic probes that combine diagnosis and treatment methods have been developed as promising tools for tumor precision medicine. Among them, fluorescent theranostic probes have developed rapidly in the frontier research field of precision medicine with their real time, low toxicity, and high-resolution merit. Therefore, this review focuses on recent advances in the development of fluorescent theranostic probes, as well as their applications for cancer diagnosis and treatment. Initially, small-molecule fluorescent theranostic probes mainly including tumor microenvironment-responsive fluorescent prodrugs and phototherapeutic probes were introduced. Subsequently, nanocomposite probes are expounded based on four types of nano-fluorescent particles combining different therapies (chemotherapy, photothermal therapy, photodynamic therapy, gene therapy, etc.). Then, the capsule-type "all in one" probes, which occupy an important position in theranostic probes, are summarized according to the surface carrier type. This review aims to present a comprehensive guide for researchers in the field of tumor-related theranostic probe design and development.
Engineering Molecular Self-Assembly of Theranostic Nanoprobes for Dual-Modal Imaging-Guided Precise Chemotherapy

Abstract:Uniting dual-modality of fluorescence and photoacoustic (PA) imaging into theranostic nanoprobes is imperative for spatio-temporally tracking of drug delivery, distribution, and release. Herein, we present a rational design strategy of molecularly precise amphiphilic prodrugs BPn-Cy-S-CPT (n=0, 5, and 20, refers to the degree of polyethylene glycol (PEG) polymerization; CPT=camptothecin) to tune their self-assembly behaviour, innovatively integrating dual-modal PA and near-infrared (NIR) fluorescence imaging in a single-molecular framework. Among these elaborately designed prodrugs, it is found that only BP20-Cy-S-CPT could form uniform and highly stable self-assemblies, especially in showing synergistically enhanced PA and dual-channel NIR signals. In detail, PA signal is employed to trace the in vivo delivery with high spatial resolution, meanwhile the glutathione (GSH)-triggered dual-channel fluorescence response could real-timely monitor drug distribution and release without “blind spot”. The results of in vivo dual-modal PA/NIR imaging have verified that BP20-Cy-S-CPT displayed synergistic targeting (including passive, active, and activatable targeting) for tumor-specific delivery, and thereby executed CPT release in the tumor site. Consequently, our molecularly precise BP20-Cy-S-CPT self-assemblies could make a breakthrough to spatio-temporally track the in vivo drug release profile, expanding the intelligent theranostic toolbox for precise cancer treatment.

Abstract:Breast cancer has become a huge burden with continued rise of incidence and death rate worldwide. Various methods for diagnosis and therapy of breast cancer have met the challenges of lack of complete information about the tumor location and limited therapy efficacy. Although aggregation-induced emission luminogens (AIEgens) have shown great promise for various cancer treatment applications, they may be incompetent for deep-seated tumor diagnosis due to the limited penetration depth. Herein, we designed and prepared a radiolabeled AIEgen-based organic photothermal agent for bimodal PET/fluorescence imaging-guided breast tumor photothermal therapy. The prepared multifunctional nanoparticles (Ga-68-TPA-TTINC NPs) with NIR-II fluorescence, gamma irradiation and photothermal conversion property could be efficiently taken up by tumor cells and induce reactive oxygen species burst in vitro, further boosting the photothermal treatment of tumor in vivo. More importantly, the nanoprobe could target and clearly visualize 4T1 tumor xenografts through PET and NIR-II fluorescence imaging with high tumor/muscle ratio up to 4.8, which provides a promising tool and solution for breast tumor theranostics.
Radiotheranostics in oncology: Making precision medicine possible

Abstract:A quintessential setting for precision medicine, theranostics refers to a rapidly evolving field of medicine in which disease is diagnosed followed by treatment of disease‐positive patients using tools for the therapy identical or similar to those used for the diagnosis. Against the backdrop of only‐treat‐when‐visualized, the goal is a high therapeutic index with efficacy markedly surpassing toxicity. Oncology leads the way in theranostics innovation, where the approach has become possible with the identification of unique proteins and other factors selectively expressed in cancer versus healthy tissue, advances in imaging technology able to report these tissue factors, and major understanding of targeting chemicals and nanodevices together with methods to attach labels or warheads for imaging and therapy. Radiotheranostics—using radiopharmaceuticals—is becoming routine in patients with prostate cancer and neuroendocrine tumors who express the proteins PSMA (prostate‐specific membrane antigen) and SSTR2 (somatostatin receptor 2), respectively, on their cancer. The palpable excitement in the field stems from the finding that a proportion of patients with large metastatic burden show complete and partial responses, and this outcome is catalyzing the search for more radiotheranostics approaches. Not every patient will benefit from radiotheranostics; but, for those who cross the target‐detected line, the likelihood of response is very high.

Abstract:Development of novel imaging probes for cancer diagnosis is critical for early disease detection and management. The past two decades have witnessed a surge in the development and evolution of radiolabeled nanoparticles as a new frontier in personalized cancer nanomedicine. The dynamic synergism of positron emission tomography (PET) and nanotechnology combines the sensitivity and quantitative nature of PET with the multifunctionality and tunability of nanomaterials, which can help overcome certain key challenges in the field. In this review, we discuss the recent advances in radionanomedicine, exemplifying the ability to tailor the physicochemical properties of nanomaterials to achieve optimal in vivo pharmacokinetics and targeted molecular imaging in living subjects. Innovations in development of facile and robust radiolabeling strategies and biomedical applications of such radionanoprobes in cancer theranostics are highlighted. Imminent issues in clinical translation of radiolabeled nanomaterials are also discussed, with emphasis on multidisciplinary efforts needed to quickly move these promising agents from bench to bedside.

Abstract:The diagnosis and treatment of cancer or tumor at the cellular level will be greatly improved with the development of techniques that enable the delivery of analyte probes and therapeutic agents into cells and cellular compartments. Organic and inorganic nanoparticles that interface with biological systems have recently attracted widespread interest in the fields of biology and medicine. The new term nanomedicine has been used recently. Nanoparticles are considered to have the potential as novel intravascular or cellular probes for both diagnostic (imaging) and therapeutic purposes (drug/gene delivery), which is expected to generate innovations and play a critical role in medicine. Target-specific drug/ gene delivery and early diagnosis in cancer treatment is one of the priority research areas in which nanomedicine will play a vital role. Some recent breakthroughs in this field recently also proved this trend. Nanoparticles for drug delivery and imaging have gradually been developed as new modalities for cancer therapy and diagnosis. In this article, we review the significance and recent advances of gene/drug delivery to cancer cells, and the molecular imaging and diagnosis of cancer by targeted functional nanoparticles. Abstract:The last decade has witnessed the growing development of optical imaging using near-infrared (NIR) light (wavelength range: 700–1700 nm) in a wide range of applications, from fundamental research of live biological and physiological processes to clinical applications such as disease screening and image-guide surgical interventions. The low-energy NIR light offers reduced tissue scattering, absorption as well as autofluorescence background compared to traditional wavelengths in the visible (400–700 nm) range, thus dramatically enhancing imaging resolution, signal-to-noise ratio, and penetration depth. This Research Topic calls for advances in new fluorescent probes, imaging instrumentation, analytical methods and their applications for improvement of diagnosis and therapy in this “tissue transparent window,” aiming to provide a timely update of this continuously evolving filed. Imaging and diagnosis agents are the cornerstone of preclinical applications. In their study, Tan et al. report lycosin-I peptide modified gold nanoclusters (LGNCs), which exhibit strong NIR fluorescence, for imaging of specific cancer cell nuclei. The lycosin-I peptide not only possesses highly efficient and selective intracellular translocation against several cancer cells, but also induces aggregation of gold nanoclusters to further enhance their brightness. After uptake by tumor cells, the LGNCs aggregates can be depolymerized into ultrasmall nanoclusters by the high level GSH in the tumor microenvironment, facilitating nuclear targeting translocation in cancer cells. Another study by Huang et al. reports a novel biomimetic nanoprobe for photoacoustic (PA) imaging of cervical carcinoma. This probe is constructed by coating indocyanine green (ICG)-loaded mesoporous silica nanoparticles with the cancer cell membrane, providing small particle size, excellent dispersion, large loading efficiency, good biocompatibility and homologous targeting ability to Hela cells in vitro. The biomimetic nanoprobe can specifically accumulate into tumor and display a superior PA imaging efficacy compared with free ICG, allowing for non-invasive deep tissue imaging of cervical carcinoma in vivo. Another article by Gu et al. focuses on a particular type of NIR fluorophores, semiconducting polymer nanoparticles (SPNs). SPNs are constructed by encapsulating hydrophobic semiconducting polymers (SPs) with amphiphilic copolymers, and have shown great promise for phototheranostics of cancer. The authors review recent advances of thiadiazoloquinoxaline (TQ)-based SPNs from their synthesis and hydrophilic functionalization to NIR-II (1,000–1,700 nm) fluorescence imagingguided cancer photothermal therapy (PTT). To enhance the PTT efficiency, they also discuss methods for constructing SPNs with capability of multimodal imaging. Current limitations are summarized at the end that hamper clinical applications of the SPNs, such as relatively large size and low tumor targeting ability. Abstract:Malignant tumors are the main diseases threatening human life. Using precise theranostics to diagnose and cure tumors has emerged as a new method to improve patient survival. Based on the current development of precise tumor imaging, image-guided tumor therapy has received widespread attention because it is beneficial for developing precise treatment of tumors, has the potential to improve the efficacy of tumor therapy and reduce the incidence of adverse side effects. Nanoprobes, which are nanomaterial functionalized with specific biomolecules, have intrigued intense interest due to their great potential in monitoring biorecognition and biodetection evens. Benefiting from the unique advantages of nanomaterials, including the easy surface functionalization, the unique imaging performances, and the high drug loading capacity, nanoprobes have become a powerful tool to simultaneously realize tumor precise imaging, diagnosis, and therapy. This review introduces the non-invasive tumor precise imaging and highlights the recent advances of image-guided oncotherapy mediated by nanoprobes in anti-tumor drug delivery, tumor precise surgical navigation, chemodynamic therapy, and phototherapy. Finally, a perspective on the challenge and future direction of nanoprobes in imaging-guided tumor theranostics is also discussed.

Abstract:Theranostics provides opportunities for precision cancer therapy. However, theranostic probes that simultaneously turn on their diagnostic signal and pharmacological action only in respond to a targeted biomarker have been less exploited. We herein report the synthesis of a macrotheranostic probe that specifically activates its near-infrared fluorescence (NIRF), photoacoustic (PA), and photothermal signals in the presence of a cancer-overexpressed enzyme for imaging-guided cancer therapy. Superior to the small-molecule counterpart probe, the macrotheranostic probe has ideal biodistribution and renal clearance, permitting passive targeting of tumors, in situ activation of multimodal signals, and effective photothermal ablation. Our study thus provides a macromolecular approach towards activatable multimodal phototheranostics.

Abstract:A smart and versatile theranostic nanoplatform with single component based on hollow mesoporous Prussian blue nanoparticles (HMPBs) realizes the in vivo highly efficient synergistic chemo-thermal tumor therapy. Abstract:The identification of biomarkers in biological and clinical samples is a critical component of the emerging paradigm of personalized medicine. With the significant rise in novel targeted therapies and immunotherapies, medical professionals require accurate and reliable biomarker data to make informed decisions regarding the most appropriate treatments for individual patients. Furthermore, the discovery of new biomarkers must be supported by the development of fast, easy, and cost-effective methods to ensure their widespread application in clinical practice.This special issue presents a collection of original articles and reviews that highlight the latest advances in biomarker discovery and detection methodologies.In their study, Yanhong Pei and collaborators conducted a bibliometric and visualization analysis focusing on the use of extracellular vesicles as a liquid biopsy tool for diagnostic biomarkers in osteosarcoma [1]. Their work sheds light on the potential of circulating as non-invasive diagnostic techniques in cancer care.Ke Li and colleagues offered another notable contribution by systematically evaluating the efficacy of one-step nucleic acid amplification (OSNA) for diagnosing sentinel lymph node metastasis in CK19-positive cancers. As sentinel lymph node metastasis is a critical indicator of cancer prognosis, the team conducted a bioinformatic analysis based on data from The Cancer Genome Atlas (TCGA) and the Human Protein Atlas, further underscoring the importance of this method in cancer diagnostics [2].Similarly, Liu et al. performed a systematic review and a PRISMA-compliant meta-analysis to assess the diagnostic performance of OSNA in detecting sentinel lymph node metastasis in CK19positive breast cancer. This comprehensive analysis enhances our understanding of OSNA's role in cancer diagnosis and patient management [3].Finally, Regiart and collaborators contributed a systematic review on the latest developments in the electrochemical detection of protein biomarkers in lung cancer. Their review, covering research published in the last five years, provides valuable insights into how electrochemical methods can advance cancer biomarker detection, offering a promising avenue for clinical application of their analysis [4].We extend our sincere gratitude to all the authors and reviewers who have contributed to this special issue, as well as the journal's editorial team for their invaluable support throughout the editorial process.

A Thiol-triggered Croconaine-Chromene Integration to Induce Ferroptosis and Photothermal Synergistic Efficient Tumor Ablation

Abstract:Theranostic probes, combining diagnostic and treatment capabilities, have emerged as promising tools in tumor precision medicine. However, existing probes with constant fluorescence and photothermal activity can result in low signal-to-background ratios and phototoxicity. In this study, we introduced CM-Croc, a novel probe comprised of chromene and croconaine, selectively triggered by thiol. CM-Croc exhibited turn-on fluorescence and released croconaine for photothermal therapy. The croconaine moiety possesses high photothermal conversion efficiency up to 55%. Besides, it demonstrated potent activity against various cancer cell lines at low micromolar, including drug-resistant variants, through enhanced photothermal therapy combined with the ferroptosis effect. What's more, CM-Croc was proved to inhibit the activtity of GPX4 to induce ferroptosis. Finally, CM-Croc was demonstrated to the first croconaine-derived SOP, that targeted tumor and significantly inhibited tumor growth in vivo following intravenous administration with irradiation. This study showcased CM-Croc's potential for enhancing tumor precision medicine.

Nanoscale Radiotheranostics for Cancer Treatment: From Bench to Bedside

Abstract:In recent years, the application of radionuclides-containing nanomaterials in cancer treatment has garnered widespread attention. The diversity of nanomaterials allows researchers to selectively combine them with appropriate radionuclides for biomedical purposes, addressing challenges faced by peptides, small molecules, or antibodies used for radionuclide labeling. However, with advantages come challenges, and nanoradionuclides still encounter significant issues during clinical translation. This review summarized the recent progress of nanosized radionuclides for cancer treatment or diagnosis. The discussion began with representative radionuclides and the methods of incorporating them into nanomaterial structures. Subsequently, new combinations of nanomaterials and radionuclides, along with their applications, were introduced to demonstrate their future trends. The benefits of nanoradionuclides included optimized pharmacokinetic properties, enhanced disease-targeting efficacy, and synergistic application with other treatment techniques. Besides, the basic rule of this section was to summarize how these nanoradionuclides can truly impact the diagnosis and therapy of various cancer types. In the last part, the focus was devoted to the nanoradionuclides currently applicable in clinics and how to address the existing issues and problems based on our knowledge.
