Abstract:The rapid evolution of neuromodulation techniques includes an increasing amount of research into stimulation paradigms that are guided by patients' neurophysiology, to increase efficacy and responder rates. Treatment personalisation and target engagement have shown to be effective in fields such as Parkinson's disease, and closed-loop paradigms have been successfully implemented in cardiac defibrillators. Promising avenues are being explored for physiologically informed neuromodulation in psychiatry. Matching the stimulation frequency to individual brain rhythms has shown some promise in transcranial magnetic stimulation (TMS). Matching the phase of those rhythms may further enhance neuroplasticity, for instance when combining TMS with electroencephalographic (EEG) recordings. Resting-state EEG and event-related potentials may be useful to demonstrate connectivity between stimulation sites and connected areas. These techniques are available today to the psychiatrist to diagnose underlying sleep disorders, epilepsy, or lesions as contributing factors to the cause of depression. These technologies may also be useful in assessing the patient's brain network status prior to deciding on treatment options. Ongoing research using invasive recordings may allow for future identification of mood biomarkers and network structure. A core limitation is that biomarker research may currently be limited by the internal heterogeneity of psychiatric disorders according to the current DSM-based classifications. New approaches are being developed and may soon be validated. Finally, care must be taken when incorporating closed-loop capabilities into neuromodulation systems, by ensuring the safe operation of the system and understanding the physiological dynamics. Neurophysiological tools are rapidly evolving and will likely define the next generation of neuromodulation therapies.

Abstract:Brain neuromodulation has revolutionized the medical treatment of neurological diseases and injuries; however, existing therapies are limited in their clinical scope of application. Most existing therapies are delivered through implanted macroelectrodes that reside either on top of or directly inside the brain. Estimates of the effective electric field spread from these devices generally span from thousands to millions of individual neurons. Unfortunately, some neurological diseases and injuries require stimulation fields of higher precision. Next-generation microneuromodulation devices ( ̃102 – 103 μm2 surface area) have been developed with hundreds of closely spaced channels. These devices may be able to provide electrical microstimulation in the form of biphasic, charge-balanced small amplitude square waves that provide salient, behaviorally relevant information to human subjects. However, there is a lack of knowledge incorporated into their safety and clinical use. Neuromodulation is a field of science, medicine, and bioengineering that encompasses implantable and non-implantable technologies, electrical or chemical, that act upon neural interfaces to improve life for humanity. Our research groups collaboratively investigate neuromodulation performed via electrical microstimulation. Our primary development target is brain neuromodulation. In this article we highlight the application of electrochemistry to the field of neuromodulation.

Abstract:Bioelectronic Medicine stands as an emerging field that rapidly evolves and offers distinctive clinical benefits, alongside unique challenges. It consists of the modulation of the nervous system by precise delivery of electrical current for the treatment of clinical conditions, such as post-stroke movement recovery or drug-resistant disorders. The unquestionable clinical impact of Bioelectronic Medicine is underscored by the successful translation to humans in the last decades, and the long list of preclinical studies. Given the emergency of accelerating the progress in new neuromodulation treatments (i.e., drug-resistant hypertension, autoimmune and degenerative diseases), collaboration between multiple fields is imperative. This work intends to foster multidisciplinary work and bring together different fields to provide the fundamental basis underlying Bioelectronic Medicine. In this review we will go from the biophysics of the cell membrane, which we consider the inner core of neuromodulation, to patient care. We will discuss the recently discovered mechanism of neurotransmission switching and how it will impact neuromodulation design, and we will provide an update on neuronal and glial basis in health and disease. The advances in biomedical technology have facilitated the collection of large amounts of data, thereby introducing new challenges in data analysis. We will discuss the current approaches and challenges in high throughput data analysis, encompassing big data, networks, artificial intelligence, and internet of things. Emphasis will be placed on understanding the electrochemical properties of neural interfaces, along with the integration of biocompatible and reliable materials and compliance with biomedical regulations for translational applications. Preclinical validation is foundational to the translational process, and we will discuss the critical aspects of such animal studies. Finally, we will focus on the patient point-of-care and challenges in neuromodulation as the ultimate goal of bioelectronic medicine. This review is a call to scientists from different fields to work together with a common endeavor: accelerate the decoding and modulation of the nervous system in a new era of therapeutic possibilities.

Abstract:It has been suggested that the field of biomedical engineering has been instrumental in creating new therapeutic modalities in rehabilitation of neurological disorders as well as motor dysfunction. Much of this can be attributed to advancements in the scientific research of the cranial nerves. This paper has an objective which is to combine all the efforts including and not limited to the use of electrical stimulation, imaging technique and various cranial imaging techniques in the repair of the cranial nerves. The narrative of this article will look at the synergistic effects of the use of such technology including how it may enhance certain working parameters in the patient through electrical stimulation, autologous nerve and bio-mimic engineered/derived recell scaffolds and Brain-Computer interfaces. Multidisciplinary approaches other than comprehensive examination and analysis of peer reviewed articles, clinical experiments, tests etc. which were relevant to seizures were also used for seaching of the topics of study. Qualitative and Quantitative methods were deployed to evaluate the outcome of applying electrical stimuli, BCIs as well as tissue-engineered scaffolds in the improvement of patient performances. The results consequence of the studies state that for instance electrical stimulation of cranial nerves can improve and restore cognitive abilities and also can be used for treatment of the neurologic disorders that are in existence. Neuromodulation is one of the applications that has some effects of improvement in the brain of the individual and direct treatment for disorders of neurology. There are thus many therapies that are directed for treatment of neurologicTissue engineering methods showed an effective strategy for restoring function in motor cranial nerves; however, the challenges of therapy are due to problems such as misdirection of reinnervation. Brain computer interfaces on the other hand offer a solution for the restoration of lost motor function with significant improvements. It is possible to achieve an increase in the recovery rate by up to 25% using the techniques of motor imagery and neurofeedback. Although the said positive attributes are present in reality, the document credences that more work still needs to be done in the investigation of new technologies, free from the bias on these existing technologies.
Energy efficient neural stimulation: coupling circuit design and membrane biophysics

Abstract:The delivery of therapeutic levels of electrical current to neural tissue is a well-established treatment for numerous indications such as Parkinson's disease and chronic pain. While the neuromodulation medical device industry has experienced steady clinical growth over the last two decades, much of the core technology underlying implanted pulse generators remain unchanged. In this study we propose some new methods for achieving increased energy-efficiency during neural stimulation. The first method exploits the biophysical features of excitable tissue through the use of a centered-triangular stimulation waveform. Neural activation with this waveform is achieved with a statistically significant reduction in energy compared to traditional rectangular waveforms. The second method demonstrates energy savings that could be achieved by advanced circuitry design. We show that the traditional practice of using a fixed compliance voltage for constant-current stimulation results in substantial energy loss. A portion of this energy can be recuperated by adjusting the compliance voltage to real-time requirements. Lastly, we demonstrate the potential impact of axon fiber diameter on defining the energy-optimal pulse-width for stimulation. When designing implantable pulse generators for energy efficiency, we propose that the future combination of a variable compliance system, a centered-triangular stimulus waveform, and an axon diameter specific stimulation pulse-width has great potential to reduce energy consumption and prolong battery life in neuromodulation devices.

Abstract:As the pace of research in implantable neurotechnology increases, it is important to take a step back and see if the promise lives up to our intentions. While direct electrical stimulation applied intracranially has been used for the treatment of various neurological disorders, such as Parkinson's, epilepsy, clinical depression, and Obsessive-compulsive disorder, the effectiveness can be highly variable. One perspective is that the inability to consistently treat these neurological disorders in a standardized way is due to multiple, interlaced factors, including stimulation parameters, location, and differences in underlying network connectivity, leading to a trial-and-error stimulation approach in the clinic. An alternate view, based on a growing knowledge from neural data, is that variability in this input (stimulation) and output (brain response) relationship may be more predictable and amenable to standardization, personalization, and, ultimately, therapeutic implementation. In this review, we assert that the future of human brain neurostimulation, via direct electrical stimulation, rests on deploying standardized, constrained models for easier clinical implementation and informed by intracranial data sets, such that diverse, individualized therapeutic parameters can efficiently produce similar, robust, positive outcomes for many patients closer to a prescriptive model. We address the pathway needed to arrive at this future by addressing three questions, namely: (1) why aren't we already at this prescriptive future?; (2) how do we get there?; (3) how far are we from this Neurostimulationist prescriptive future? We first posit that there are limited and predictable ways, constrained by underlying networks, for direct electrical stimulation to induce changes in the brain based on past literature. We then address how identifying underlying individual structural and functional brain connectivity which shape these standard responses enable targeted and personalized neuromodulation, bolstered through large-scale efforts, including machine learning techniques, to map and reverse engineer these input-output relationships to produce a good outcome and better identify underlying mechanisms. This understanding will not only be a major advance in enabling intelligent and informed design of neuromodulatory therapeutic tools for a wide variety of neurological diseases, but a shift in how we can predictably, and therapeutically, prescribe stimulation treatments the human brain.

Abstract:It's an electrifying time to be in neuroscience. Using implanted devices that send pulses of electricity through the nervous system, physicians are learning how to influence the neural systems that control people's bodies and minds. These devices give neurologists new ways to treat patients with a wide range of disorders, including epilepsy, chronic pain, depression, and Parkinson's disease. · So far, these stimulators have been oneway devices that deliver a steady sequence of pulses to the nervous system but can't react to changes in the patient's body. Now, at last, medical device companies are coming out with dynamic neural stimulators that have a bit of "brain" themselves. These smart systems can detect changes in a physiological signal and then respond by delivering a therapy or adjusting the patient's treatment in real time.

New modalities of cortical electrophysiology, perspectives in medical research and human physiology

Abstract:Recent advances in material technology and in micro- and nano-electronics have profoundly changed the design of intracranial electrophysiology electrodes. It is now possible to manufacture electrodes that record cortical activity at a spatial resolution that was previously unthinkable. This high spatial resolution enables recording of the functional structures of the brain, and differentiation of the activity of the different types of neurons composing them. In this paper, we present a review of the different types of electrodes now available, and then suggest one of the first applications for such high resolution electrodes, namely a means to better characterise the mechanisms that generate focal seizures in epileptics. Finally, we reflect more broadly on prospects for their future use.

Abstract:In the name of electroceuticals, bioelectronic devices have transformed and become essential for dealing with all physiological responses. This significant advancement is attributable to its interdisciplinary nature from engineering and sciences and also the progress in micro and nanotechnologies. Undoubtedly, in the future, bioelectronics would lead in such a way that diagnosing and treating patients’ diseases is more efficient. In this context, we have reviewed the current advancement of implantable medical electronics (electroceuticals) with their immense potential advantages. Specifically, the article discusses pacemakers, neural stimulation, artificial retinae, and vagus nerve stimulation, their micro/nanoscale features, and material aspects as value addition. Over the past years, most researchers have only focused on the electroceuticals metamorphically transforming from a concept to a device stage to positively impact the therapeutic outcomes. Herein, the article discusses the smart implants’ development challenges and opportunities, electromagnetic field effects, and their potential consequences, which will be useful for developing a reliable and qualified smart electroceutical implant for targeted clinical use. Finally, this review article highlights the importance of wirelessly supplying the necessary power and wirelessly triggering functional electronic circuits with ultra-low power consumption and multi-functional advantages such as monitoring and treating the disease in real-time.

Abstract:BACKGROUND AND OBJECTIVES: Recent advances in stereotactic and functional neurosurgery have brought forth the stereo-electroencephalography approach which allows deeper interrogation and characterization of the contributions of deep structures to neural and affective functioning. We argue that this approach can and should be brought to bear on the notoriously intractable issue of defining the pathophysiology of refractory psychiatric disorders and developing patient-specific optimized stimulation therapies. METHODS: We have developed a suite of methods for maximally leveraging the stereo-electroencephalography approach for an innovative application to understand affective disorders, with high translatability across the broader range of refractory neuropsychiatric conditions. RESULTS: This article provides a roadmap for determining desired electrode coverage, tracking high-resolution research recordings across a large number of electrodes, synchronizing intracranial signals with ongoing research tasks and other data streams, applying intracranial stimulation during recording, and design choices for patient comfort and safety. CONCLUSION: These methods can be implemented across other neuropsychiatric conditions needing intensive electrophysiological characterization to define biomarkers and more effectively guide therapeutic decision-making in cases of severe and treatment-refractory disease.

Abstract:The challenge of treating neurological disorders has motivated the development of implantable devices that can deliver treatment when and where it is needed. This study presents a novel brain implant capable of electrophoretically delivering drugs and recording local neural activity on the surface of the brain. The drug delivery is made possible by the integration of a microfluidic ion pump (µFIP) into a conformable electrocorticography (ECoG) device with recording cites embedded next to the drug delivery outlets. The µFIP ECoG device can deliver a high capacity of several biologically important cationic species on demand. The therapeutic potential of the device is demonstrated by using it to deliver neurotransmitters in a rodent model while simultaneously recording local neural activity. These developments represent a significant step forward for cortical drug-delivery systems.

Abstract:Chronically implantable neurostimulation devices are becoming a clinically viable option for treating patients with neurological disease and psychiatric disorders. Neurostimulation offers the ability to probe and manipulate distributed networks of interacting brain areas in dysfunctional circuits. Here, we use tools from network control theory to examine the dynamic reconfiguration of functionally interacting neuronal ensembles during targeted neurostimulation of cortical and subcortical brain structures. By integrating multimodal intracranial recordings and diffusion-weighted imaging from patients with drug-resistant epilepsy, we test hypothesized structural and functional rules that predict altered patterns of synchronized local field potentials. We demonstrate the ability to predictably reconfigure functional interactions depending on stimulation strength and location. Stimulation of areas with structurally weak connections largely modulates the functional hubness of downstream areas and concurrently propels the brain towards more difficult-to-reach dynamical states. By using focal perturbations to bridge large-scale structure, function, and markers of behavior, our findings suggest that stimulation may be tuned to influence different scales of network interactions driving cognition.

Abstract:A multidisciplinary and international convergent working group (neural stem cell biology, functional electrical stimulation, materials engineering, electrical engineering, neurosurgery, neurology, biomedical device, and commercialization) met in Canada to create a call to action for Electrical Stimulation for Neural Repair. Electrical stimulation, in the form of deep brain stimulation (DBS), is an approved treatment for various neurological disorders such as essential tremor, Parkinson's disease, and epilepsy. Here, DBS works through disrupting neural circuits; however, electrical stimulation may also effectively promote neural repair due to the activation of electrosensitive resident neural stem cells. Activating neural stem cells has great promise for enhancing neuroplasticity to treat damaged brains. To realize this therapy's potential, multidisciplinary experts met to identify barriers, gaps, and next steps. Neurological disorders are a leading cause of death and disabilities worldwide and represent an enormous public health challenge (Feigin and Vos, 2019). A conservative estimate of the global financial burden is more than two trillion US dollars/year, and these costs are expected to double by 2030. For those afflicted, they have a decreased quality of life, and their dependence on others places a heavy burden on our society and healthcare networks. The major neurological disorders contributing to the overall burden are stroke and dementias (including Alzheimer's disease; Feigin et al., 2020). There are presently no cures and limited treatment options for stroke or neurodegenerative diseases. There is a clear need and incentive for therapeutic interventions to promote neural repair and functional recovery. Patterned Electrical Brain Stimulation by a Wireless Network of Implantable Microdevices

Abstract:Transmitting meaningful information into brain circuits by electronic means is a challenge facing brain-computer interfaces. A key goal is to find an approach to inject spatially structured local current stimuli across swaths of sensory areas of the cortex. Here, we introduce a fully wireless approach to multipoint patterned electrical microstimulation by a spatially distributed epicortically implanted network of silicon microchips to target specific areas of the cortex. Each sub-millimeter-sized microchip harvests energy from an external radio-frequency source and converts this into biphasic current injected focally into tissue by a pair of integrated microwires. The amplitude, period, and repetition rate of injected current from each chip are controlled across the implant network by implementing a pre-scheduled, collision-free bitmap wireless communication protocol featuring sub-millisecond latency. As an in vivo demonstration, a network of 30 wireless stimulators was chronically implanted into motor and sensory areas of the cortex in a freely moving rat for three months. We explored the effects of patterned intracortical electrical stimulation on trained animal behavior at average RF powers well below safety limits.

Abstract:The development of future neural prostheses involves much more than connecting commercially available stimulators to disabled individuals. Safe and effective operation of prostheses requires fundamental studies of the electrode-tissue interface. The electrochemistry of the interface must be controlled to prevent toxic byproducts. Histopathological studies of stimulated tissue are necessary to establish safe limits of stimulation and to determine mechanisms of neural damage when it does occur. Electrophysiological studies elucidate which neural pathways are excited and help in the design of more selective electrode arrays. Biomaterials are required that protect the implant from the hostile environment of the body. Presently available materials are being improved and totally new materials are being developed. One of the goals of neural prostheses developers is a nonhermetic packaging material that can be applied to miniature implants without appreciably increasing their size. The techniques used to make integrated circuits on silicone substrates are ideally suited to making ultraminiature electrodes with self-contained electronic signal processing. Both integrated circuit stimulating and recording electrodes are being designed and fabricated.

Here, a flexible electronic device with tentacle-like channels radiating from a central wireless circuit is presented. The device is constructed by different organic and inorganic materials that have been made into thin-film or nanoparticle formats. All channels have been equipped with flexible components for distributed and synchronized opto-electrical stimulation, biopotential sensing, and ion concentration monitoring. They can be implanted into different brain regions through adaptive bending and individually addressed to follow programmable working sequences. Experimental results conducted in vitro and in vivo have demonstrated the capability in generating optical or electrical stimulation, while sensing 16-channels biopotential and concentration of Ca2+, Na+, and K(+)ions in distributed regions. Behavior and immunohistochemistry studies suggest potential applications in regulating brain functions for freely moving animals. In combination with various functional materials, the device can serve as a comprehensive research platform that can be modularized to accommodate different needs for brain studies, offering numerous possibilities and combinations to yield sophisticated neuromodulation and behavior regulation.

Abstract:Electrical neuromodulation plays a pivotal role in enhancing patient outcomes among individuals suffering from neurological disorders. Implantable neural interfaces are vital components of the electrical neuromodulation system to ensure desirable performance. However, conventional devices are limited to a single function and are constructed with bulky and rigid materials, which often leads to mechanical incompatibility with soft tissue and an inability to adapt to the dynamic and complex 3D structures of biological systems. In addition, current implantable neural interfaces utilized in clinical settings primarily rely on wire‐based techniques, which are associated with complications such as increased risk of infection, limited positioning options and movement restrictions. Here, the state‐of‐art applications of electrical neuromodulation are presented. Material schemes and device structures that can be employed to develop robust and multifunctional neural interfaces, including flexibility, stretchability, biodegradability, self‐healing, self‐rolling, or morphing are discussed. Furthermore, multimodal wireless neuromodulation techniques, including optoelectronics, mechano‐electrics, magnetoelectrics, inductive coupling and electrochemically based self‐powered devices are reviewed. Abstract:Electroceuticals provide promising opportunities for peripheral nerve regeneration, in terms of modulating the extensive endogenous tissue repair mechanisms between neural cell body, axons and target muscles. However, great challenges remain to deliver effective and controllable electroceuticals via bioelectronic implantable device. In this review, the modern fabrication methods of bioelectronic conduit for bridging critical nerve gaps after nerve injury are summarized, with regard to conductive materials and core manufacturing process. In addition, to deliver versatile electrical stimulation, the integration of implantable bioelectronic device is discussed, including wireless energy harvesters, actuators and sensors. Moreover, a comprehensive insight of beneficial mechanisms is presented, including up-to-datein vitro, in vivoand clinical evidence. By integrating conductive biomaterials, 3D engineering manufacturing process and bioelectronic platform to deliver versatile electroceuticals, the modern biofabrication enables comprehensive biomimetic therapies for neural tissue engineering and regeneration in the new era.

Abstract:Abstract Precise neurostimulation can revolutionize therapies for neurological disorders. Electrode-based stimulation devices face challenges in achieving precise and consistent targeting due to the immune response and the limited penetration of electrical fields. Ultrasound can aid in energy propagation, but transcranial ultrasound stimulation in the deep brain has limited spatial resolution caused by bone and tissue scattering. Here, we report an implantable piezoelectric ultrasound stimulator (ImPULS) that generates an ultrasonic focal pressure of 100 kPa to modulate the activity of neurons. ImPULS is a fully-encapsulated, flexible piezoelectric micromachined ultrasound transducer that incorporates a biocompatible piezoceramic, potassium sodium niobate [(K,Na)NbO 3 ]. The absence of electrochemically active elements poses a new strategy for achieving long-term stability. We demonstrated that ImPULS can i) excite neurons in a mouse hippocampal slice ex vivo, ii) activate cells in the hippocampus of an anesthetized mouse to induce expression of activity-dependent gene c-Fos, and iii) stimulate dopaminergic neurons in the substantia nigra pars compacta to elicit time-locked modulation of nigrostriatal dopamine release. This work introduces a non-genetic ultrasound platform for spatially-localized neural stimulation and exploration of basic functions in the deep brain.
