Abstract:Objective: The purpose of this study was to understand the health-related behaviors in children of ethnic minorities and Han nationality so as to provide a basis for formulating a health promotion plan, reasonably allocating health resources, and improving health conditions of the entire population of children.Methods: The selection and processing of study subjects, as well as health-related behaviors, were based on the 2009 Chinese Health and Nutrition Survey (CHNS) data. A total of 867 children were involved in this study, including 762 Han children and 105 minority children. Comparative analysis was conducted on the reported ratio of health-related behaviors, including society and family variation, as well as dietary habit variation, and health condition scores.Results: A comparison on health-related behaviors between ethnic minority and Han children indicated the following: with respect to society and family variation, statistical significance (P<0.05) existed between the two groups in health-related behaviors influenced by parents who did or did not stay at home, level of education, and medical insurance status; and with respect to dietary habit variation, statistical significance (P<0.05) existed in the differences of dietary habits between the two groups. Moreover, differences in the weight-for-age Z score (WAZ), weight-for-height Z score (WHZ), and body mass index-for-age Z score (BAZ) between the two groups were statistically significant (P<0.05).Conclusion: Health-related behavioral norms and health conditions of ethnic minority children should be further improved. Increased health awareness of families with children and health care system development should be stressed to elevate the health level of the entire population of children in China.
The impact of environmental, parental and child factors on health-related behaviors among low-income children

Abstract:Multi-level factors act in concert to influence child weight-related behaviors. This study examined the simultaneous impact of variables obtained at the level of the home environment (e.g., mealtime ritualization), parent (e.g., modeling) and child (e.g., satiety responsiveness) with the outcomes of practicing healthy and limiting unhealthy child behaviors (PHCB and LUCB, respectively) in a low-income U.S. Sample: This was a cross sectional study of caregivers of preschool children (n = 432). Caregivers were interviewed using validated scales. Structural equation modeling was used to examine associations with the outcomes. Adjusting for study region, demographics and caregiver's body mass index, we found significant associations between PHCB and higher mealtime ritualizations (β: 0.21, 95% confidence interval [CI]: 0.11; 0.32, more parental modeling (β: 0.39, 95% CI: 0.27; 0.49) and less parental restrictive behavior (β: -0.19, 95% CI: -0.29; -0.10). More parental covert control (β: 0.44, 95% CI: 0.35; 0.54), more parental overt control (β: 0.14, 95% CI: 0.03; 0.25) and less parental permissive behavior (β: -0.25, 95% CI: -0.34; -0.09) were significantly associated with LUCB. Findings suggest the synergistic effects of mealtime ritualizations and covert control at the environmental-level and parental modeling, overt control, restrictive and permissive behavior at the parent-level on the outcomes. Most factors are modifiable and support multidisciplinary interventions that promote healthy child weight-related behaviors.
Social determinants of health behaviors in primary school children: A cross-sectional study of both migrant and resident children in Beijing, China

Abstract:Childhood risky behaviors always result in adverse health outcomes. These behaviors are greatly affected by social environment and individual factors. However, few studies explored the social determinants of schoolchildren’s health behaviors. In this cross-sectional study, the social determinants of health behaviors were examined among both migrant and resident students in Beijing, China. Data of 967 children from six urban and suburban primary schools were analyzed using multiple linear regression analysis. The results showed that less than 60% of children performed some healthy behaviors, although their health knowledge was generally good. Children’s health behavior scores were greatly determined by school characteristics, health knowledge, demographic characteristics, and family context. Therefore, improving health education and conditions in primary schools is the priority to promote children’s healthy behaviors, especially for disadvantaged children.

Abstract:OBJECTIVE:To identify social determinants of chronic diseases related behaviors in primary school students in Beijing, China.METHODS:Cross-sectional study was designed and a two-stage (districts and schools) stratified (school type) cluster (classes) sampling method was adopted. Haidian and Fengtai districts were chosen firstly. One state and one private school with more migrant children were chosen in each district, paired with two state primary schools with more permanent resident children. Two classes from grades 4, 5 and 6 from each primary school were selected, all students in which and their parents were included as study subjects. A self-designed questionnaire was adopted to collect demographic information, chronic diseases related behaviors and knowledge from students and their parents. Overall, a total of 1604 paired samples of students and parents were obtained from eight primary schools. Eleven self-reported behaviors were grouped into three, which were trying smoking, dietary intake frequency (vegetables, fruit, milk, and breakfast intake, salty foods, fatty diets), sedentary life style (reading, watching TV and playing PC continuously more than one hour, and having physical activities both in school and at home). According to the number of risk behaviors, three groups were classified as low (0 - 1), medium (2 - 3), and high (4 + ). ANOVA, Chi-square test and ordinal logistic regression analysis were adopted to find the influence of demographic characteristics, school types, social economic status (SES) , students' health knowledge, and parents' health knowledge and behaviors on students' number of risk behaviors.RESULTS:Ordinal logistic regression analysis showed boys (OR = 1. 26, 95% CI 1. 03 - 1. 53) and older (OR = 1. 25, 95% CI 1. 14 - 1. 37) were risk factors for more risk behaviors. Higher SES scores (OR = 0. 84,95% CI 0. 73 - 0. 95) and higher students' health knowledge scores (OR = 0. 87, 95% CI 0. 83 - 0. 91), and better parents' health behaviors scores (OR = 0. 87, 95% CI 0. 83 - 0. 91) were protective factors for the number of risk behaviors. Household registration status and school types had no significant impact yet. Boys (OR = 1. 39, 95% CI 1. 08 -1.79) and lower SES scores (OR =0. 85, 95% CI 0. 73 -1. 00) were significant risk factors for more risk behaviors among migrant students, compared with those in residents students, while other factors had similar impact between two groups.CONCLUSION:Family social and economic status, students' health knowledge and parents' behaviors would have great impact on students' health behaviors cultivation.

Abstract:INTRODUCTION:Reducing childhood obesity remains a public health priority given its high prevalence and its association with increased risk of adult obesity and chronic diseases. The objective of this study was to examine the joint influence of multiple risk factors on adolescent overweight status.METHODS:We conducted a random-digit-dialed telephone survey of adolescents aged 12 to 19 years in fall 2008 in a Midwestern city in Nebraska. On the basis of survey data for 791 youths aged 12 to 18 years, we conducted latent class analysis to group youths by the joint occurrence of dietary behavior, physical activity, parenting practices, and physician advice. We then examined the association between the groups and overweight status by using logistic regression, controlling for age, sex, race/ethnicity, and parent and family information.RESULTS:Youths were clustered into 3 groups. Group I (52%) were youths with healthy dietary behavior and physical activity, less permissive parenting practices, and physician advice; Group II (30%) were youths with moderately healthy dietary behavior and physical activity, less permissive parenting practices, and no physician advice; and Group III (18%) were youths with unhealthy dietary behavior and physical activity, permissive parenting practices, and physician advice. Youths in Groups I and II were less likely to be overweight than youths in Group III.CONCLUSIONS:Youths with healthier behavior and less permissive parenting practices were less likely to be overweight. Study findings highlight the need to address obesity risk factors among youths with unhealthy dietary behavior, inadequate exercise, permissive parenting practices, and some physician advice. Tailored interventions should be used to target youths with different obesity risk factors.
Parental Adherence to Healthy Lifestyles in Relation to the Risk of Obesity in Offspring: A Prospective Cohort Study in China

Abstract:Background While maternal adherence to a healthy lifestyle was shown to be associated with a lower risk of obesity in offspring, the potential role of overall parental lifestyles has not yet been explored. We aimed to address this gap by exploring whether parental adherence to an overall healthy lifestyle was associated with a lower risk of obesity in offspring.Methods We included 5881 children and adolescents aged 6-15 years at enrolment in the 2010, 2012, and 2014 waves of the China Family Panel Studies (CFPS) who were free of obesity and followed them until 2020. Parental healthy lifestyle score at study baseline was composed of five modifiable lifestyle factors (0-5; 1 for each): never smoking, non-habitual drinking, weekly exercise, modified dietary diversity score >= 5 points, and body mass index (BMI) of 18.5-23.9 kg/m2. We defined obesity according to the age-and gender-specific cutoffs by the BMI percentile curves for Chinese children aged 6-18 years. We used multivariable Cox proportional hazard models to examine the association between parental healthy lifestyle score (both as continuous and categorical variables) and risk of offspring obesity.Results Overall, 597 (10.2%) offspring developed obesity during a median follow-up of 6 years. Compared to the lowest tertile of parental healthy lifestyle score, participants in the highest tertile had a 42% (hazard ratio (HR) = 0.58; 95% confidence interval (CI) = 0.45-0.74) lower risk of obesity. Both maternal (HR = 0.75; 95% CI = 0.61-0.92) and paternal (HR = 0.73; 95% CI = 0.60-0.89) healthy lifestyle scores were associated with lower risks of obesity in offspring. For specific lifestyle factors, we observed beneficial associations for paternal diverse diet (HR = 0.73; 95% CI = 0.60-0.88) and healthy BMI (HR = 0.65; 95% CI = 0.55-0.78).Conclusions Adherence to an overall parental healthier lifestyle was associated with a lower risk of obesity in childhood and adolescence. This finding highlights the potential benefits of promoting a healthy lifestyle among parents for the primary prevention of offspring obesity.
Compliance with Health-Related Behaviors Guidelines and Its Relationship with Multiple Factors in Preschool Children Aged 3–6 Years: A National Cross-Sectional Survey in China

Abstract:Purpose: To investigate the compliance rates of health-related behaviors among Chinese preschool children, and to explore how supportive family environment, parental behavior, sociodemographic and community factors affect children’s health-related behavior comprehensively. Method: Preschool children aged 3 to 6 years were chosen from 5760 villages (residential) committees from 471 counties (districts) of 31 provinces by use of a stratified random sampling procedure, with 10,967 preschool children aged 3–6 years old included. The survey was conducted from September 2020 to November 2020. Results: The proportion of Chinese preschool children who met the moderate to vigorous physical activity (MVPA), screen time behavior (ST), and sleep behavior (SLP) guidelines were 62.3%, 52.8%, and 53.8%. Among the supportive family environment factors, parents’ time with their children on weekends had the most significant impact on children’s MVPA, ST, and SLP, with the odds ratio (OR) values of 2.18 (95%CI:1.97, 2.40), 0.69 (0.63, 0.76), and 1.62 (1.48, 1.79), respectively. Among the parental behavior factors, the mother’s exercise frequency had a strong association with the children’s MVPA and SLP, with OR values of 1.65 (1.50, 1.83) and 1.24 (1.13, 1.37), respectively; the mother’s screen time was inversely associated with the children’s ST with an OR value of 0.47 (0.44, 0.51). Conclusions: Different types of family environments were associated with the different levels of MVPA, ST and SLP among Chinese preschool children. In addition to the influence of parents’ education and family income, parents could also improve their children’s behaviors by providing a supportive family environment. The more of these factors presented in a family, the more likely it was for children to meet the guidelines. Therefore, for those families whose children’s health-related behaviors needed to be improved, the parents should create supportive family environments, such as by playing less on mobile phone and spending more time with children.

Abstract:Health Education Journal, Ahead of Print. Objective:Despite public health efforts, most children in the United States do not meet nutrition and physical activity guidelines. Moreover, there are nutrition-related health disparities between non-Hispanic and Hispanic communities. Caregivers are critical role models that shape health behaviours in children. To promote healthy behaviours, health promotion interventions must consider the entire family unit. This study aimed to assess the impact of parent's health behaviours on children's health behaviours in a sample of non-Hispanic and Hispanic low-income parents.Design:A mixed-methods design was used to identify how parent health behaviours including fruit and vegetable intake, beverage intake, physical activity, and screen time influence corresponding child health behaviours. In addition, the study aimed to investigate whether the strength of association between parent and child health behaviours varied as a function of ethnicity. Survey data and semi-structured qualitative interviews were utilised for the analyses.Setting:The study was a part of the Culture of Wellness in Preschools Programme, a multi-level obesity prevention programme. The data were obtained from the parent wellness workshops, a direct education intervention aimed at increasing fruit and vegetable consumption and physical activity among parents and their children.Data Analysis:The quantitative analyses utilised survey data from the Family Wellness pre-survey. Hierarchical linear regression models assessed the relationship between parent–child-health behaviours. Interaction terms were created to understand if this relationship varied as a function of ethnicity. Data from qualitative interviews were analysed using thematic analysis to gain an in-depth understanding of the relationship between parent and child health behaviours.Results:Parent health behaviours significantly predicted child health behaviours across all models, and these relationships did not vary as a function of ethnicity. The quantitative data were supported by the qualitative analysis: parents modelled health behaviours and thus influenced their child's health behaviour through a variety of mechanisms.Conclusion:Future efforts to promote a healthy lifestyle in children should engage with both family practices and the wider social context.

Prevalence and Associated Lifestyle Factors of Suboptimal Health Status among Chinese Children Using a Multi-Level Model

Abstract:Chinese children are facing health challenges brought by chronic non-communicable diseases, such as physical problems and psychological related health problems. Childhood represents a critical life period when the long-term dietary and lifestyle behaviors are formed. It is necessary to survey the prevalence of suboptimal health status (SHS) among Chinese children and to research the relationship between SHS and lifestyles. This study aimed to examine the prevalence of SHS among Chinese children using a large-scale population survey sample covering school students and nonstudent children, and clarified the relationships between SHS and lifestyle factors using multi-level models controlled for the cluster effect of location and the confounding effect of demographics. Multi-level generalized estimating equation models were used to examine the relationships between SHS and lifestyle factors. Prevalence ratios (PR) and 95% confidence intervals (CI) were used to assess the strength of these relationships. Of the 29,560 children, 14,393 reported one or more SHS symptoms, giving a SHS prevalence of 48.69%. The prevalence of SHS for boys (46.07%) was lower than that for girls (51.05%). After controlling for the cluster effect of living areas and confounding effect of demographic characteristics, lifestyle factors associated with SHS were: less sleep duration, current smokers (PR = 1.085, 95%CI: 1.027-1.147), current drinkers (PR = 1.072, 95%CI: 1.016-1.131), children' parents suffering from chronic diseases (PR = 1.294, 95%CI: 1.179-1.421), poor sleep quality (PR = 1.470, 95%CI: 1.394-1.550), stress (PR = 1.545, 95%CI: 1.398-1.707), negative life events (PR = 1.237, 95%CI: 1.088-1.406), hypertension (PR = 1.046, 95%CI: 1.009-1.084), unhealthy diet choice (PR = 1.091, 95%CI: 1.051-1.133) and irregular meal time (PR = 1.210, 95%CI: 1.163-1.259). Children who could exercise regularly (PR = 0.897, 95%CI: 0.868-0.927) and those with regular medical checkup (PR = 0.891, 95%CI: 0.854-0.929) were associated with lower prevalence probability of SHS. SHS has become a serious public health challenge for Chinese children. Unhealthy lifestyles were closely associated with SHS. Implementation of preventative strategies are needed to reduce the potential SHS burden associated with these widespread high-risk unhealthy lifestyle behaviors.
Associations of health-risk behaviors with mental health among Chinese children

Abstract:The study aimed to investigate the individual and combined association of health-risk behaviors with mental health among Chinese children. A cross-sectional study was conducted in Wuhan, China, from May to June 2018. Participants self-reported the information on physical activity (PA), screen time (ST), fruit and vegetable (FV) intake, and sleep duration. Mental health, including depression, social anxiety and self-esteem, was assessed using standard questionnaires. A total of 1296 children (704 males and 592 females) aged 9.2 ± 0.4 years were included in the present study. The prevalence of low PA, high ST, low FV intake, and inadequate sleep duration was 45.6%, 18.0%, 69.7%, and 64.7%, respectively. Overall, significant associations were found between individual health-risk behavior and increased risks of mental health. Furthermore, children with three or four health-risk behaviors showed significantly increased risks of anxiety (OR: 3.18, 95%CI: 1.63-6.21), depression (OR: 4.55, 95%CI: 2.28-9.09) and low self-esteem (OR: 3.59, 95%CI: 2.20-5.88) compared with those without health-risk behavior. Results of this study revealed a high prevalence of health-risk behaviors among Chinese children. Furthermore, the clustering of health-risk behavior was associated with significantly increased risks of mental health in this population. Considering these findings, it is important to perform early interventions to reduce children's health-risk behavior and prevent mental health problems.
Intergenerational transmission of parental risky health behaviors in Chinese children: Are there socioeconomic status differences?

Abstract:Background Risky health behaviors in childhood, including smoking, alcohol consumption, and having a poor diet, are the major sources of non-communicable diseases in adulthood. This study aimed to examine how parents affect children's risky health behaviors and whether intergenerational transmission differs based on socioeconomic status (SES). Methods Data were extracted from the 1991–2015 China Health and Nutrition Survey (CHNS). Smoking ( n = 5,946), alcohol consumption ( n = 7,821), and sugar-sweetened beverages (SSBs) consumption ( n = 3,537) were used as proxies for risky health behaviors in children. A binary choice model for panel data with a random-effect specification was employed to examine whether risky health behaviors can be transmitted from parents to their children. Subsequently, we conducted a seemingly unrelated estimation test (SUEST) to explore the differences in parental transmission between the different SES groups. Results We found strong intergenerational persistence of smoking, alcohol drinking, and SSBs drinking behaviors, except for the mothers' smoking behavior. Mothers had a greater influence on children's alcohol drinking and SSBs drinking behaviors than fathers both in urban and rural areas and in different SES groups. The intergenerational transmission of SSBs drinking behavior exhibited a decreasing trend with increasing SES for both urban and rural families. In urban areas, mothers' alcohol drinking behavior has a decreasing trend with increasing education level, occupation, and income; however, in rural areas, the influence of mothers' alcohol drinking behavior occurred in the same direction with increasing education level and occupation type. In rural areas, the influence of fathers' drinking and smoking behaviors on children appears to mostly increase with increasing SES. Meanwhile, the influence of such behaviors among urban fathers would decrease with increasing SES. Conclusion Parents' behaviors and SES can influence the initiation of risky health behaviors in their offspring. Thus, to promote healthy behaviors, policymakers can introduce health education programs for parents, particularly for those living in rural areas and with a low SES.

Abstract:Objective To investigate the current status of health-related behaviors and their influencing factors among preschool children in Longhua District,Shenzhen.Methods A survey was conducted to investigate the health-related behaviors among preschool children of the kindergartens in Longhua District,Shenzhen and the cognition and practices of parents on training children's health-related behaviors.Multivariable linear regression was used to analyze the relationships between scores of preschool children's health-related behaviors and gender,household registration,only child or not,grade,parental education level,the cognition and the practices of parents on training children's health-related behaviors.Results A total of 62 922 preschool children were surveyed.Of them,54.5％ were boys,45.5％ were girls;23.7％ were in primary class,35.1％ in middle class,41.2％ in top class;30.4％ had Shenzhen household registrations and 69.6％ did not.The correctness percentages of performing 32 health-related behaviors were between 12.2％ and 98.0％.Among 6 dietary behaviors,98.0％ of the children performed correctly in "less intake of fried foods",while 58.8％ preformed correctly in "not playing while eating".Among 12 health behaviors,89.1％ of the children performed correctly in "not shitting everywhere",while only 12.2％ preformed correctly in "gargling after meals".Among 11 safety behaviors,96.5％ of the children performed correctly in "not playing with fire",while 40.3％ preformed correctly in "walking up and down stairs slowly on the right".Among 3 resting and recreation behaviors,64.8％ of the children performed correctly in "watching TV/computer for less than I h",while 33.5％ preformed correctly in "going to bed early and getting up early".Multivariable linear regression showed that the scores of preschool children's health-related behaviors were related to children's gender,household registration,grade,educational background of the parents,the cognition and practices of parents on training children's health-related behaviors.Conclusion Some health-related behaviors among preschool children in Longhua District,Shenzhen need to be improved.It is suggested that the kindergartens and parents work together to strengthen the cultivation of preschool children health-related behaviors.
Effect of poor dietary behaviors on the overweight and obesity of school-aged children]

Abstract:To explore the rate of overweight and obesity, and the association between unhealthy dietary behaviors and overweight and obesity among Chinese school-aged children from 9 areas.By multiple stage stratified cluster sampling, 814 children aged 7 - 12 years old were chosen, provided with questionnaire and anthropometric measurements. According to the definition from Working Group on Obesity in China, children,were divided into different group, univariate logistic regression and multivariate logistic regression were used to analyze the associations between unhealthy dietary behaviors and overweight and obesity.The rates of overweight and obesity of school-aged children were 22. 88% and 9. 90%, respectively. The rates were higher in urban areas, and second-tier cities were similar with first-tier city. Influence factor analysis revealed skipping breakfast behavior was the influence factor (OR =1. 65, P<0.05, OR= 2. 22, P <0. 01), picky eating behavior was negative correlation (OR =0.43, P<0.01 ; OR = 0. 32, P <0. 01) , and eating fact food and drinking carbonated beverage were picked out by the multivariate logistic regression model.School-aged children’s overweight and obesity has been an increasingly serious health problem, which were correlated with genetic factor, environmental factor, and dietary behaviors and so on, poor dietary behavior might be influence factor for overweight and obesity, so it is necessary to provide earlier intervention strategies for parents to promote children’s good dietary habits.

Abstract:Research on the prevalence of behavior problems in preschool children from low-income families, and the risk factors associated with these behaviors, was reviewed. A systematic search of studies conducted between 1991 and 2002 yielded a total of 30 research reports that met all of the preestablished criteria. These studies yielded several findings. Children from low socioeconomic status (SES) backgrounds were found to have a higher incidence of behavior problems as compared to the general population. Behavior problems were associated with multiple risk factors found in these children's lives related to child, parent, and socioeconomic characteristics. The results are discussed in terms of implications for early identification and intervention and directions for future research.

Abstract:Poor nutrition or insufficient physical activity (PA) are risk factors for obesity and chronic diseases. This 2019 cross-sectional study from the school health survey examined the dietary and PA behaviors of Chinese adolescents. A total of 12,860 adolescents aged 11-18 participated through multistage and stratified cluster random sampling. A questionnaire collected data on weight, PA, sedentary lifestyle, and eating habits. Unhealthy behaviors were identified and summed up for each behavior. Participants were then classified into high and low amounts of risk behaviors. Weight status was defined using Body Mass Index (BMI) cutoff points for Chinese individuals aged 6-18. Multinomial logistic regression was used to assess effects of lifestyle behaviors on weight status. The prevalence of overweight and obesity was 22.3% among all participants (30.6% in boys, 13.2% in girls). Females engaged in more risk physical activities (4.12 vs. 3.80, p < 0.05), while males engaged in more risk dietary activities (2.20 vs. 2.02, p < 0.05). Higher number of risk dietary, PA, and sedentary behaviors were all significantly correlated with higher BMI (dietary: r = 0.064; PA: r = 0.099; sedentary: r = 0.161; p < 0.001 for all) and body weight (dietary: r = 0.124; PA: r = 0.128; sedentary: r = 0.222; p < 0.001 for all). Risk sedentary behaviors was a significant risk factor for overweight/obesity (Adjusted Odds Ratio AOR = 1.30, 95% Confidence Interval CI 1.11-1.52). Obesity and unhealthy lifestyle behaviors remain a concern among Chinese adolescents. These results provide an update on the factors contributing to overweight/obesity among adolescents and call for efforts to address obesity among adolescents.
A Look Back at Healthy Lifestyle Behavioral Patterns among School-Age Children: Are We Neglecting Healthy Weight for Overweight Kids?

Abstract:Objective: To describe prevalence of healthy lifestyle behaviors (HLBs) between two school-age cohorts of racial-ethnically diverse children. Methods: Using two Early Childhood Longitudinal Study cohorts (ECLS-K 1999 and 2010), we compared percentage change in HLBs (sleep, physical activity, screen time, and family meals) by child weight groups and within racial-ethnic groups. Weight groups of interest included healthy weight (HW; BMI 5th-84th percentile), overweight (OW; ≥ 85th-94th), obese (OB; ≥ 95th-99th percentile), and severely obese (SO; ≥ 99th percentile). Results: OW children within the 2010 cohort reported greater percentage change (range: 2.0%-15.1% increase) in HLBs, whereas HW children demonstrated lower percentage change (range: -6.2% to 8.7% increase). OB and SO children showed significant lower percentage change in reducing screen time (range: -11.0% and -12.7%, respectively). HW Latino children demonstrated the least favorable trends with overall declines noted for 4/5 HLBs. Screen time was noted to have the greatest degree of favorable change (2%-14%) across weight groups, whereas adequate sleep duration demonstrated the lowest favorable percentage change (2%-2.8%). Conclusion: Fewer HW and OB children were participating in recommended HLBs compared with OW children in 2010. Recent health campaigns may have helped providers prioritize the identification and counseling of OW children, yet our data suggest that we may be neglecting the role of HLB counseling in OB and HW children. Continued analysis on the social determinants that impact HLBs in school-age and HW/OB minority children should inform how we can best tailor counseling and messages in the coming decades.

Health-Related Quality of Life in Relation to Health Behaviour Patterns among Canadian Children

Abstract:Poor health behaviours in childhood, including sedentary behaviour, low physical activity levels, inadequate sleep, and unhealthy diet, are established risk factors for both chronic diseases and mental illness. Scant studies have examined the importance of such health behaviour patterns for health-related quality of life (HRQoL). This study aimed to examine the association of health behaviour patterns with HRQoL among Canadian children. Data from 2866 grade five students were collected through a provincially representative school-based survey of the 2014 Raising Healthy Eating and Active Living Kids in Alberta study. Latent class analysis was used to identify health behaviour patterns based on 11 lifestyle behaviours: sedentary behaviour (using a computer, playing video games, watching TV), physical activity (with and without a coach), sleep (bedtime on weekdays and weekends), and diet (fruit and vegetables intake, grain products, milk and alternatives, meat and alternatives). Multivariable multilevel logistic regression was applied to examine the associations of health behaviour patterns with HRQoL. Three groupings with distinct health behaviour patterns were identified: the first grouping (55%) is characterized by relatively healthy levels of sedentary behaviour, physical activity, and sleep, but a less healthy diet (“activity-focused” group). The second grouping (24%) is characterized by a relatively healthy diet, but moderately healthy levels of sedentary behaviour, physical activity, and sleep (“diet-focused” group). The third grouping (21%) is characterized by mostly unhealthy behaviours (“not health-focused” group). Students in the third and second groupings (“not health-focused” and “diet-focused”) were more likely to report lower HRQoL relative to students in the first grouping (“activity-focused”). The findings suggest that health promotion strategies may be more effective when considering the patterns of health behaviours as distinct targets in the efforts to improve HRQoL. Future research should include prospective observational and intervention studies to further elucidate the relationship between health behaviour patterns and HRQoL among children.

Abstract:BACKGROUND AND OBJECTIVES:Obesity has become a global epidemic. In China, 42% of adults and about onefifth of children are overweight or obese. In major cities, about one-third of boys are overweight or obese. This study aimed to understand how children and parents in China make eating and physical activity (PA) decisions, considering individual, family, community, social, and environmental factors, and to collect parents' recommendations for interventions to promote healthy eating and physical activity.METHODS AND STUDY DESIGN:Children (n=41, aged 10-15 years) and their parents (n=41) participated in eight semi-structured focus groups (FGs) in Beijing (in North China) and Nanjing (South China). Each site conducted two FGs with children and two FGs with parents. A framework analysis of FG data was conducted with NVivo.RESULTS:Three main themes were identified: Children chose food based on flavor, and consumption of unhealthy snacks was prevalent; there were inconsistent standards and practices of school lunch services across schools; students had limited PA time due to academic demand. Students favored high-calorie snacks over fruits or vegetables. Students' and parents' perceptions of school lunch services varied among schools in terms of operation, price, quality, nutritious options, and food taste. Most students reported enjoying PA but spent little time in PA, due to study burdens. Parents made recommendations for improving school food services and increasing PA during and after school.CONCLUSIONS:These findings will help develop family- and school-targeted health promotion interventions. Intervention framing must consider the unique Chinese social and cultural context.
Association between dietary behaviors and overweight or obesity in preschool children

Abstract:BackgroundOverweight/obesity in children are a global health issue. Preschool age is an important stage of children's growth and development and dietary behaviors, so it is crucial to explore dietary behavior characteristics and childhood overweight/obesity.ObjectiveTo explore the association between dietary behavior characteristics and overweight/obesity in preschool children in Shanghai, and the consistency of children's eating behavior characteristics with their parents'.MethodsThe children aged 3 to 6 years in 5 kindergartens in Shanghai in 2015 were selected by convenience sampling and surveyed. Their height and weight were measured. Children's and parents' dietary behavior characteristics were self-reported by their parents. According to the weight-for-height recommended by the World Health Organization, excess weight was calculated by the following formula: excess-weight%=[(measured weight - standard weight)/standard weight×100%]; obese is determined at excess-weight% > 20%, overweight: excess-weight% at 10%-20%, normal weight: excess-weight% at −10%-<10%. Chi-squared test and t test were used to compare the differences of demographic and dietary behavior characteristics between overweight/obese and normal weight children. Multiple logistic regression models were used to evaluate the association between dietary behaviors and overweight/obesity. The consistency of dietary behavior characteristics was tested between children and their parents.ResultsAmong the 897 preschool children, 217 (24.2%) were overweight/obese and 680 (75.8%) were of normal weight. The birth weight [(3439±472) g vs. (3361±418) g] and proportion of monthly household income < 10000 yuan (17.1% vs. 10.7%) in the overweight/obese children were significantly higher than those in the normal weight children, and there were no statistical differences in sex, age, gestational age, time of adding complementary foods, outdoor activities after school every day, and parental BMI. After adjusting for potential confounders such as sex, age, household income, outdoor activities after school every day, and parental overweight/obesity, frequently having puffed food (OR=2.46, 95%CI: 1.20-4.95), western-style fast food (OR=3.52, 95%CI: 1.41-8.79), and carbonated drinks (OR=2.92, 95%CI: 1.37-6.13), often eating a lot (OR=1.51, 95%CI: 1.09-2.08) and having a good appetite (OR=2.60, 95%CI: 1.87-3.62), having regular meals (OR=1.48, 95%CI: 1.07-2.03), and fast eating (OR=3.37, 95%CI: 2.10-5.42) were positively associated with children's overweight/obesity (P<0.05). Children's partial/picky eating was inconsistent with fathers' behavior (P<0.05), while the other dietary behaviors were similar with both mothers' and fathers'.ConclusionEating behavior characteristics of preschool children are associated with overweight/obesity, and affected by their parents. Early identifying dietary behavior problems and improving dietary habits of children might be an effective strategy in preventing and controlling the occurrence and development of obesity in childhood.
