Abstract:Journal of Telemedicine and Telecare, Ahead of Print. ObjectiveThe aim of this study was to assess the precision of a web-based tool in measuring visual acuity (VA) in ophthalmic patients, comparing it to the traditional in-clinic evaluation using a Snellen chart, considered the gold standard.MethodsWe conducted a prospective and in-clinic validation comparing the Eyecare Visual Acuity Test® to the standard Snellen chart, with patients undergoing both tests sequentially. Patients wore their standard spectacles as needed for both tests. Inclusion criteria involved individuals above 18 years with VA equal to or better than +1 logMar (20/200) in each eye. VA measurements were converted from Snellen to logMAR, and statistical analyses included Bland–Altman and descriptive statistics.ResultsThe study, encompassing 322 patients and 644 eyes, compared Eyecare Visual Acuity Test® to conventional methods, revealing a statistically insignificant mean difference (0.01 logMAR, P = 0.1517). Bland–Altman analysis showed a narrow 95% limit of agreement (0.22 to −0.23 logMAR), indicating concordance, supported by a significant Pearson correlation (r = 0.61, P < 0.001) between the two assessments.ConclusionThe Eyecare Visual Acuity Test® demonstrates accuracy and reliability, with the potential to facilitate home monitoring, triage, and remote consultation. In future research, it is important to validate the Eyecare Visual Acuity Test® accuracy across varied age cohorts, including pediatric and geriatric populations, as well as among individuals presenting with specific comorbidities like cataract, uveitis, keratoconus, age-related macular disease, and amblyopia.

Abstract:Amblyopia is a visual disorder that originates from the brain. It exhibits no pathology in the eye. Studies have shown that measuring both visual acuity and binocular balance for assessing amblyopia could be more helpful. However, tests that measure binocular balance are time-consuming, often exceeding 30 min. Their long test durations prevent them from being used in the clinic. For this reason, we have developed a quick (i.e., about 7 min) and precise tool that quantitatively measures binocular balance of patients with amblyopia. The new test can capture binocular imbalance that is typically exhibited at high spatial frequency in amblyopes. In addition, it has an excellent test-retest reliability and repeatability between two experimental sessions. We hope that our newly developed test can pave the road for physicians and researchers to better assess and diagnose amblyopia and other visual disorders that disrupt binocular balance beyond the laboratory.

Developing a Novel Pediatric Eye Chart Assessing Visual Acuity by Minimum Separable Threshold

Abstract:The purpose of this study was to develop a new pediatric acuity chart that can assess the minimum separation threshold by incorporating the minimum separation threshold into the picture. To overcome the design limitations of the Landolt ring, two designs of highly versatile minimum separable thresholds that can be easily incorporated into a picture were created: a black, filled circle (the “Circle”) and a segment (the “Square”), both with the same break as in the Landolt ring. The three designs—the Landolt ring, Circle, and Square—were used to evaluate and compare the differences in the visual acuity of 21 healthy adults. No significant differences were observed between the results of the visual acuity tested with the Landolt ring, Circle, and Square (Landolt ring vs. Circle: p = 0.92, Landolt ring vs. Square: p = 0.31, Circle vs. Square: p = 0.40). The Bland–Altman analysis revealed no fixed errors between the Landolt ring and Circle and between the Landolt ring and Square (95% CI: −0.09–0.08, −0.09–0.12). Proportional errors were also not observed (p = 0.68, p = 0.41). The Landolt ring, Circle, and Square designs obtained equal results in visual acuity, thus achieving the successful development of a novel pediatric visual acuity chart using these designs.

An interactive framework for the evaluation and detection of stereoacuity threshold under ambient lighting

Abstract:Objective: Our study aims to provide a novel framework for the continuous evaluation of stereoacuity under ambient lighting conditions using Bayesian inference. Methods: We applied a combination of psychophysical and expected entropy minimization procedures for the computation of a continuous stereoacuity threshold. Subsequently, we evaluated the effect of ambient lighting during stereoacuity testing (ST) by adopting a bisection-matching based adaptive gamma calibration (AGC). Participants ($N=187$) including visually healthy controls ($N=51$), patients with Intermittent Divergent Squint (IDS; $N=45$), and controls with induced anisometropia (IA; $N=91$) performed ST with and without AGC under two lighting conditions: completely dark (20 cd/m$^2$) and normally lit (130 cd/m$^2$) rooms. Results: Our framework demonstrated "excellent" reliability ($> 0.9$) and a positive correlation with TNO (a clinical stereo test), regardless of whether AGC was conducted. However, when AGC is not performed, significant differences (Friedman $X_{r}^{2} = 28.015$; $p<0.00001$; Bland-Altman bias: 30 arc-sec) were found in stereoacuity thresholds between dark and light conditions for participants with IDS and IA. Controls are unaffected by AGC and yield a similar stereoacuity threshold under both lighting conditions. Conclusion: Our study proves that stereoacuity threshold is significantly deviated particularly in participants with IDS or IA stereo-deficits if ambient lighting is not taken into consideration. Moreover, our framework provides a quick (approximately 5-10 minutes) assessment of stereoacuity threshold and can be performed within 30 ST and 15 AGC trials. Significance: Our test is useful in planning treatments and monitoring prognosis for patients with stereo-deficits by accurately assessing stereovision.

Abstract:We describe a novel method for testing a visual field that employs a computer monitor with displays of varying contrast that permits unprecedented resolution and characterization of the structure of scotomas in three dimensions. Patients are placed in front of a touch-sensitive computer screen at a fixed distance. With one eye covered, they focus on a central fixation marker and trace with their finger the areas on an Amsler grid that are missing from their field of vision. Increasing degrees of contrast of the Amsler grid are simulated by repeating the test at different gray-scale levels. The results are recorded and then displayed as topographical contour rings by the computer test program. The results can also be rendered as an immediate 3-D depiction of the central hill-of-vision. Several clinical pilot studies have been conducted at the Doheny Eye Institute and more than 200 patients have been examined with this system so far. Conditions such as optic neuritis, anterior ischemic optic neuropathy (AION), age-related macular degeneration (AMD), glaucoma, and ocular hypertension have been successfully assessed by this test. Each condition provides unique patterns that are most evident in 3-D. The 3-D computer-automated threshold Amsler grid test is an innovative and noninvasive visual field test. It provides several advantages over state-of-the-art standard automated perimetry, including: (1) additional information through 3-D depiction of scotomas, such as location, extent, slope, depth, and shape; (2) high angular resolution (1 deg compared with typically 6 deg); (3) a simple test setup (merely a touch-sensitive computer monitor and the test software); (4) excellent patient compliance (spending 4 to 5 min per eye). In light of its promising initial tests, the 3-D visual field test appears to have the potential for the early detection and monitoring of various diseases over time.

Abstract:Purpose To explore the mechanisms of cone sensitivity loss in retinitis pigmentosa by combining two‐colour perimetry with threshold versus intensity (tvi) testing. Methods Seven subjects with autosomal recessive retinitis pigmentosa and 10 normal subjects were recruited and underwent perimetric testing of one eye using 480‐ and 640‐nm Goldman size V targets presented under scotopic conditions (no background illumination) and against a white background ranging in luminance from −1.5 to 2 log cd m−2 in 0.5 log cd m−2 steps. Data were fitted with tvi functions of the form logT = logT0 + log ((A + A0)/A0)n, where T is the threshold, T0 is the absolute threshold, A is the background intensity, A0 is the 'dark–light' constant and n is a gain constant. Results Reliable tvi functions could not be obtained within the region of the visual field corresponding to loss of the ellipsoid zone on optical coherence tomography. At fixation, changes in both T0 and A0 were observed, consistent with a d1 mechanism loss, which resulted in an upwards and rightwards shift of the tvi function. Losses at [±3°, ±3°] demonstrated changes in T0, consistent with a d3 mechanism loss, resulting in an upwards translation of the tvi curve. Conclusions Although the absolute cone threshold was elevated at each location, shifts in the tvi function (so‐called d1 mechanism loss) at fixation minimise threshold elevation in the presence of white adapting backgrounds, such as those typically employed in standard two‐colour perimetry. At more peripheral testing locations, changes in threshold occurred independent of background luminance (so‐called d3 mechanism loss). These findings suggest that backgrounds which selectively adapt rods while maintaining cones at, or near, absolute threshold may be preferable to conventional two‐colour perimetry for assessing loss of cone sensitivity, especially at the point of fixation.

Abstract:ImportanceVisual acuity (VA) is one of the most important clinical data points in ophthalmology. However, few options for validated at-home VA assessments are currently available.ObjectiveTo validate 3 at-home visual acuity tests in comparison with in-office visual acuity.Design, Setting, and ParticipantsBetween July 2020 and April 2021, eligible participants with VA of 20/200 or better were recruited from 4 university-based ophthalmology clinics (comprehensive, cornea, glaucoma, and retina clinics). Participants were prospectively randomized to self-administer 2 of 3 at-home VA tests (printed chart, mobile phone app, and website) within 3 days before their standard-of-care clinic visit. Participants completed a survey assessing usability of the at-home tests. At the clinic visit, best-corrected Snellen distance acuity was measured as the reference standard.Main Outcomes and MeasuresThe at-home VA test results were compared with the in-office VA test results using paired and unpairedttests, Pearson correlation coefficients, analysis of variance, χ2tests, and Cohen κ agreement. The sensitivity, specificity, positive predictive value, and negative predictive value of each at-home test were calculated to detect significant VA changes (≥0.2 logMAR) from the in-office baseline.ResultsA total of 121 participants with a mean (SD) age of 63.8 (13.0) years completed the study. The mean in-office VA was 0.11 logMAR (Snellen equivalent 20/25) with similar numbers of participants from the 4 clinics. Mean difference (logMAR) between the at-home test and in-office acuity was −0.07 (95% CI, −0.10 to −0.04) for the printed chart, −0.12 (95% CI, −0.15 to −0.09) for the mobile phone app, and −0.13 (95% CI, −0.16 to −0.10) for the website test. The Pearson correlation coefficient for the printed chart was 0.72 (95% CI, 0.62-0.79), mobile phone app was 0.58 (95% CI, 0.46-0.69), and website test was 0.64 (95% CI, 0.53-0.73).Conclusions and RelevanceThe 3 at-home VA test results (printed chart, mobile phone app, and website) appeared comparable within 1 line to in-office VA measurements. Older participants were more likely to have limited access to digital tools. Further development and validation of at-home VA testing modalities is needed with the expansion of teleophthalmology care.

Abstract:The standard visual acuity measurements rely on stationary stimuli, either letters (Snellen charts), vertical lines (vernier acuity) or grating charts, processed by those regions of the visual system most sensitive to the stationary stimulation, receiving visual input from the central part of the visual field. Here, an acuity measurement is proposed based on discrimination of simple shapes, that are defined by motion of the dots in the random dot kinematograms (RDK) processed by visual regions sensitive to motion stimulation and receiving input also from the peripheral visual field. In the motion-acuity test, participants are asked to distinguish between a circle and an ellipse, with matching surfaces, built from RDKs, and separated from the background RDK either by coherence, direction, or velocity of dots. The acuity measurement is based on ellipse detection, which with every correct response becomes more circular until reaching the acuity threshold. The motion-acuity test can be presented in negative contrast (black dots on white background) or in positive contrast (white dots on black background). The motion defined shapes are located centrally within 8 visual degrees and are surrounded by RDK background. To test the influence of visual peripheries on centrally measured acuity, a mechanical narrowing of the visual field to 10 degrees is proposed, using opaque goggles with centrally located holes. This easy and replicable narrowing system is suitable for MRI protocols, allowing further investigations of the functions of the peripheral visual input. Here, a simple measurement of shape and motion perception simultaneously is proposed. This straightforward test assesses vision impairments depending on the central and peripheral visual field inputs. The proposed motion-acuity test advances the capability of standard tests to reveal spare or even strengthened vision functions in patients with injured visual system, that until now remained undetected.

Normal values for a clinical test of letter-recognition contrast thresholds

Abstract:Purpose: To investigate the contrast thresholds (CTs) in normal subjects using a high-luminance, letter-recognition task under clinically relevant testing conditions. Setting: Texas Tech University Health Sciences System, Lubbock, Texas, USA. Methods: Sixty normal subjects aged 20 to 49 years with a best corrected visual acuity of 20/20 or better in both eyes participated. M & S Technologies software was used to display black-on-white Sloan letters at contrast levels of 25%, 20%, 15%, 12%, and 10% through 1% in 1% decrements. The effects of age, sex, optotype size, eye dominance, ambient illumination level (bright = 625 - 630 lux; dim = <3 lux), and direction of approach to threshold were analyzed using a multivariate, ordinary, least-squares analysis. Results: Age and sex did not influence CTs. Ascending versus descending testing was not statistically significant (P>.5). The effects of room illumination and eye dominance were significant (P<.01). Significant differences were found between 20/30 and 20/50, 20/30 and 20/70, and 20/50 and 20/70 optotype sizes (P<.01 for all comparisons). Conclusions: A commercially available, computer-based test of CTs was easy to administer and apparently easy for inexperienced subjects to perform. The results suggest criteria for detecting visual problems concerned with familiar but complex spatial-image shapes. This information might be used to assess the effects of treatments such as laser refractive surgery on recognition contrast. Further study is warranted.
Cone Spacing Correlates With Retinal Thickness and Microperimetry in Patients With Inherited Retinal Degenerations

Abstract:Purpose: To determine whether high-resolution retinal imaging measures of macular structure correlate with visual function over 36 months in retinal degeneration (RD) patients and normal subjects.Methods: Twenty-six eyes of 16 RD patients and 16 eyes of 8 normal subjects were studied at baseline; 15 eyes (14 RD) and 11 eyes (6 normal) were studied 36 months later. Adaptive Optics Scanning Laser Ophthalmoscopy (AOSLO) was used to identify regions of interest (ROIs) with unambiguous cones at baseline to measure cone spacing. AOSLO images were aligned with spectral-domain optical coherence tomography (SD-OCT) and fundus-guided microperimetry results to correlate structure and function at the ROIs. SD-OCT images were segmented to measure inner segment (IS) and outer segment (OS) thickness. Correlations between cone spacing, IS and OS thickness and sensitivity were assessed using Spearman correlation coefficient ρ with bootstrap analyses clustered by person.Results: Cone spacing (ρ = 0.57, P < 0.001) and macular sensitivity (ρ = 0.19, P = 0.14) were significantly correlated with eccentricity in patients. Controlling for eccentricity, cone spacing Z-scores were inversely correlated with IS (ρ = -0.29, P = 0.002) and OS thickness (ρ = -0.39, P < 0.001) in RD patients only, and with sensitivity in normal subjects (ρ = -0.22, P < 0.001) and RD patients (ρ = -0.38, P < 0.001). After 36 months, cone spacing increased (P < 0.001) and macular sensitivity decreased (P = 0.007) compared to baseline in RD patients.Conclusions: Cone spacing increased and macular sensitivity declined significantly in RD patients over 36 months. High resolution images of cone structure correlated with retinal sensitivity, and may be appropriate outcome measures for clinical trials in RD.
Rapid campimetry - a novel robust kinetic approach for visual field screening in glaucoma

Abstract:Purpose To investigate the robustness and variability of a novel kinetic visual field (VF) screening method termed rapid campimetry (RC). Methods In RC visual field (VF) screening is enabled via kinetic-based testing on any computer (10°/4.7 s at 40-cm viewing distance) and high contrast in a dark room (1 cd/cm 2 ). In experiment (1): 30 participants [20 healthy participants (HC), 5 glaucoma patients (GLA) and 5 patients with cataract (CAT)] were included to test the intra-session variability (fatigue effect) and the following effects on RC: room illumination (140 cd/m 2 ), ±3 D refractive errors, media opacity. In experiment (2): Inter-session variability (1–3 weeks apart) was assessed in 10 HC and 10 GLA. Since RC detects absolute scotomas, the outcome measure was the size of physiological (blindspot) and pathological (glaucoma) scotomas in degrees. A repeated measures ANOVA was employed in experiment 1 and intraclass correlation (ICC) in experiment 2. Results Neither the size of the blindspot nor the VF defects differed significantly between the different testing conditions. For intra-session variability, the average bias of blindspot size was −0.6 ± 2.5°, limits of agreement (LOA), in comparison to 0.3 ± 1.5° for VF defects, both with ICC of 0.86 and 0.93, respectively. For the inter-session repeatability, the average bias and LOA for blindspot size was 0.2 ± 3.85° in comparison 1.6 ± 3.1° for VF defects, both with ICC of 0.87 and 0.91, respectively. Conclusion RC was robust to suboptimal testing VF conditions and showed good-to-excellent reliability between VF testing visits holding high potential for teleophthalmology.

Choice of Stimulus Range and Size Can Reduce Test-Retest Variability in Glaucomatous Visual Field Defects.

Abstract:PURPOSE To develop guidelines for engineering perimetric stimuli to reduce test-retest variability in glaucomatous defects. METHODS Perimetric testing was performed on one eye for 62 patients with glaucoma and 41 age-similar controls on size III and frequency-doubling perimetry and three custom tests with Gaussian blob and Gabor sinusoid stimuli. Stimulus range was controlled by values for ceiling (maximum sensitivity) and floor (minimum sensitivity). Bland-Altman analysis was used to derive 95% limits of agreement on test and retest, and bootstrap analysis was used to test the hypotheses about peak variability. RESULTS Limits of agreement for the three custom stimuli were similar in width (0.72 to 0.79 log units) and peak variability (0.22 to 0.29 log units) for a stimulus range of 1.7 log units. The width of the limits of agreement for size III decreased from 1.78 to 1.37 to 0.99 log units for stimulus ranges of 3.9, 2.7, and 1.7 log units, respectively (F = 3.23, P < 0.001); peak variability was 0.99, 0.54, and 0.34 log units, respectively (P < 0.01). For a stimulus range of 1.3 log units, limits of agreement were narrowest with Gabor and widest with size III stimuli, and peak variability was lower (P < 0.01) with Gabor (0.18 log units) and frequency-doubling perimetry (0.24 log units) than with size III stimuli (0.38 log units). CONCLUSIONS Test-retest variability in glaucomatous visual field defects was substantially reduced by engineering the stimuli. TRANSLATIONAL RELEVANCE The guidelines should allow developers to choose from a wide range of stimuli.

Abstract:The Rapid Campimetry (RC), a kinetic visual field test proved to reliably detect visual field defects within the central 10° degrees, the most crucial part for visual acuity and quality of life, affected even at very early stages of glaucoma, within a short measurement epoch, ~ 1 min. This study aims to further investigate RC correspondence with standard tests in glaucoma, namely standard automated perimetry (SAP) and optical coherence tomography (OCT) within 10° of visual field (VF). For this purpose, we included 41 participants, [21 glaucoma (GLA, mean age: 65.9 ± 12.4; 12 preperimetric eyes and 11 with VF defects) and 20 healthy controls (HC; mean age: 65.0 ± 10.3); 20 eyes]. At first, we compared the rate of detection/exclusion of VF defects in RC vs. SAP. Then, for those with VF defects (11 eyes), we investigated the 68-pointwise correspondence of 10 − 2 layout of RC and SAP. For functional and structural (SF) correspondence, pointwise correspondence of RC, SAP vs. OCT macula [ macular ganglion cell thickness (GCL)] was also performed. Further, we compared sector-based correspondence of RC, SAP vs. OCT macula accounting for GCL displacement at the fovea as well as sector-based correspondence with OCT disc [peripapillary retinal nerve fiber layer thickness (pRNFL)]. Agreement estimates were reported along with Cohen Kappa ( ) statistic. For overall performance, RC and SAP showed 100% agreement ( ) for the exclusion of VF defects (HC and preperimetric GLA) and for detection of VF defects [11 eyes of 9 GLA, ( ]. Further, RC outperformed SAP in detection of arcuate scotomas, 7 vs. 5, respectively. Pointwise correspondence of VF defects (11 eyes), RC-SAP agreement reached 90% accuracy ( ). For SF correspondence, RC [SAP] showed 62% [69%] pointwise agreement with OCT macula , ( ). For macular sector-based correspondence, SF correspondence improved and reached 83% [83%] agreement, ( ). For OCT disc sector-based analysis, SF correspondence was highest, 100% [100%] agreement, ( ). Rapid Campimetry gave reproducible results in comparison to SAP and OCT with high potential as a screening VF method given its short testing duration, ~ 1 min in screening mode, and compatibility with telemedicine technologies upon future optimization and automation.

Abstract:Traditional visual acuity test depends on the subjective evaluation, such as the naming of Snellen letters, which is difficult to perform with preverbal children and even adults with suspected malingering or low intellectual abilities. Although steady-state motion visual evoked potentials (SSMVEPs) can provide an objective visual acuity assessment method (Zheng et al., 2019), the time spent on the visual acuity test is a bit long and the spatial frequencies of stimulus are limited, easily leading to visual fatigue and affecting the precision of results. Hence, the combination of the Freiburg Visual Acuity and Contrast Test (FrACT) and SSMVEPs provides an alternative method for rapid and precise visual acuity assessment (Bach, 1996). In this study, we replaced the Landolt-Cs of eight orientations of FrACT with SSMVEP paradigms of eight different temporal frequencies, and the manual response by pressing the buttons of the response box was also replaced by automatic online SSMVEP response by canonical correlation analysis (CCA) method. The visual acuity was defined as the corresponding spatial frequency threshold of the paradigm at the steepest part of the psychometric function of a logistic function (Bach, 2006), and the best PEST (parameter estimation by sequential testing) was used to estimate threshold by using previous response results (Lieberman & Pentland, 1982). Ten subjects participated in this experiment for two minutes of one eye’s test, and Bland-Altman analysis found that the agreement between objective SSMVEP and subjective FrACT visual acuity was pretty good. Our study proved that the visual acuity assessment based on the combination of SSMVEPs and FrACT can be an objective, rapid and precise method to measure visual acuity, especially for infants and preverbal children.

Abstract:The Swedish Interactive Thresholding Algorithm (SITA) is the main measurement acquisition algorithm used on the Humphrey Field Analyser, the most commonly used instrument for visual field (VF) assessment worldwide. We compare the sensitivity outputs and reliability parameters of the three currently available SITA algorithms—SITA Standard (SS), Fast (SF), and Faster (SFR), with a focus on the newly released SFR and the 24-2C test grid. SFR displays similar sensitivity outputs to SS and SF, but may not be interchangeable with SS in eyes with more severe VF loss. The reliability metric with the greatest impact on VF reliability is the level of false positives, although the recommended 15% false positive cut off may be inappropriate as a threshold for judging whether a test is reliable and should be included for use in SFR. Finally, the 24-2C grid may be useful in flagging the presence of a clustered central VF defect, while the 10-2 grid can be used to more comprehensively characterize central field defects. We also discuss strategies to improve testing frequency in clinical practice.

Retinal sensitivity changes in early/intermediate AMD: a systematic review and meta-analysis of visual field testing under mesopic and scotopic lighting

Abstract:Visual fields under mesopic and scotopic lighting are increasingly being used for macular functional assessment. This review evaluates its statistical significance and clinical relevance, and the optimal testing protocol for early/intermediate age-related macular degeneration (AMD). PubMed and Embase were searched from inception to 14/05/2022. All quality assessments were performed according to GRADE guidelines. The primary outcome was global mean sensitivity (MS), further meta-analysed by: AMD classification scheme, device, test pattern, mesopic/scotopic lighting, stimuli size/chromaticity, pupil dilation, testing radius (area), background luminance, adaptation time, AMD severity, reticular pseudodrusen presence, and follow-up visit. From 1489 studies screened, 42 observational study results contributed to the primary meta-analysis. Supported by moderate GRADE certainty of the evidence, global MS was significantly reduced across all devices under mesopic and scotopic lighting with large effect size (−0.9 [−1.04, −0.75] Hedge's g, P < 0.0001). The device ( P < 0.01) and lighting ( P < 0.05) used were the only modifiable factors affecting global MS, whereby the mesopic MP-1 and MAIA produced the largest effect sizes and exceeded test-retest variabilities. Global MS was significantly affected by AMD severity (intermediate versus early AMD; −0.58 [−0.88, −0.29] Hedge's g or −2.55 [3.62, −1.47] MAIA-dB) and at follow-up visit (versus baseline; −0.62 [−0.84, −0.41] Hedge's g or −1.61[−2.69, −0.54] MAIA-dB). Magnitudes of retinal sensitivity changes in early/intermediate AMD are clinically relevant for the MP-1 and MAIA devices under mesopic lighting within the central 10° radius. Other factors including pupil dilation and dark adaptation did not significantly affect global MS in early/intermediate AMD.

Abstract:Visual function comprises three principles: light sensation, color sensation, and minimum separable sensation. Although clinical evaluation of light sensation and visual acuity have been remarkably developed through comprehensive application of various methods, the test methods to evaluate color sensation in the clinical field have not reflected these various significant developments after their recommendation at the International Congress of Ophthalmology in 1933. To date, various research methods in color vision have been invented on the basis of clinical evaluation methods, most of which were limited to laboratory investigations and were not applied to the clinical field. In this review, the author focuses on both the currently clinical available evaluation methods and the clinically applicable methods based on the present laboratory research studies.
The cone dysfunction syndromes

Abstract:The cone dysfunction syndromes are a heterogeneous group of inherited, predominantly stationary retinal disorders characterised by reduced central vision and varying degrees of colour vision abnormalities, nystagmus and photophobia. This review details the following conditions: complete and incomplete achromatopsia, blue-cone monochromatism, oligocone trichromacy, bradyopsia and Bornholm eye disease. We describe the clinical, psychophysical, electrophysiological and imaging findings that are characteristic to each condition in order to aid their accurate diagnosis, as well as highlight some classically held notions about these diseases that have come to be challenged over the recent years. The latest data regarding the genetic aetiology and pathological changes observed in the cone dysfunction syndromes are discussed, and, where relevant, translational avenues of research, including completed and anticipated interventional clinical trials, for some of the diseases described herein will be presented. Finally, we briefly review the current management of these disorders.
Visual field screening and the development of a new screening program

Abstract:The diagnosis of chronic open-angle glaucoma requires diligent and careful screening. Intraocular pressure measures and disc evaluations, when used in isolation, give unacceptably high numbers of false positives and false negatives, while full visual field analysis, as conducted with kinetic or full threshold static perimetry, takes too long for routine use. Screening of the visual field may offer a viable alternative. The effect of changing certain parameters of a visual field screening test are evaluated. These include the location of the stimuli, the number of stimuli and their suprathreshold increment. The results of an optimized visual field screening test are then evaluated with respect to the theoretical predictions. The results indicate that high levels of sensitivity and specificity can be achieved with visual field screening tests which are short enough to be included as part of the routine optometric examination.
