Abstract:Objective To investigate expression of thyrotropin receptor(TSHr)in orbital adipocyte of Graves′ ophthalmopathy(GO).Methods Orbital adipose tissues specimens were obtained in the course of orbital becompression surgery for severe GO(n=8).Normal orbital adipose tissues specimens were collected at enucleation of eyebell with other eye disease(n=8)with no history of thyroid disease.Adipocytes were obtained from the orbital tissue specimens by digesting and centrifugation.Adipocytes were stored frozen at-70℃ until processing for RNA isolation.The expression of TSHr in orbital adipocyte was detected with RT-PCR.Results The expression of TSHr in orbital adipocyte was upregulated in GO cases compared to controls,there was significant differences in two groups(P=0.001).Conclusions TSHr might be the autoantigen that was shared between the thyroid gland and the orbital.

Abstract:Thyroid-associated ophthalmopathy, also named Graves′ ophthalmopathy(GO), is an autoimmune inflammatory disease involved in orbital tissue whose pathogenesis has not been fully elucidated. As the most common and difficultly relieved symptom clinically, proptosis is currently believed caused by the increase of orbital contents, such as hyaluronic acid deposition, extraocular muscle thickening and adipose tissue remodeling. Among them, the expansion of the orbital adipose tissue seems to relate closer to it. Thy1 - subtypes in orbital fibroblasts(OF) can differentiate into adipocytes. OF has the potential to differentiate into adipocytes, and the factors affecting the differentiation process may involve in the regulation of signal transduction pathways. Abnormal adipogenesis originated from orbital fibroblasts in GO was reviewed to further understand the adipogenic mechanism, and to intervene it at an early stage, so as to provide new ideas and new action sites for treating GO.
Identification of differentially expressed long non-coding RNAs and mRNAs in orbital adipose/connective tissue of thyroid-associated ophthalmopathy

Abstract:<p>Extracellular matrix remodeling and orbital adipose/connective tissue expansion are two key features of thyroid-associated ophthalmopathy (TAO). Recent studies have indicated the critical role of long non-coding RNAs (lncRNAs) in the pathogenesis of ocular disorders. However, little is known about the roles of lncRNAs in orbital adipose/connective tissue of TAO. In this study, the profiles of lncRNAs and mRNAs in the orbital adipose/connective tissue of TAO were identified by RNA sequencing. A total of 809 differential lncRNAs and 607 differential mRNAs were identified, among which 52 genes were found to be significantly related to the extracellular matrix. Co-expression network analysis suggested that lncRNAs might regulate extracellular matrix remodeling in orbital adipose/connective tissue of TAO. Additionally, the target genes of lncRNAs involved in the lipid metabolism and cytokine-cytokine receptor interaction were also identified. These results may provide potential regulatory mechanisms of lncRNAs in the orbital adipose/connective tissue of TAO.</p>

Abstract:Graves’ disease is always accompanied by thyroid-associated ophthalmopathy (TAO), and both of them shared the same pathophysiological characteristics, as it has been proved that thyroid-stimulating hormone receptor (TSHR) plays an essential role not only in the pathogenesis of Graves’ disease but also in that of TAO. TSHR expresses on orbital fibroblasts in the procedure of TAO, in which many factors such as cellular and humeral immunity, thyrotropin stimulating immunoglobulin (TSI), thyroid-stimulating hormone (TSH), platelet derived growth factor (PDGF) and other thyroid antibodies synergistically initiate the disease via TSHR. The activation of helper T cells recognizing TSHR peptides and ligation of TSHR by TRAb lead to the secretion of inflammatory cytokines and chemokines, and enhanced hyaluronic acid (HA) production and adipogenesis. The resulting connective tissue remodeling results in varying degrees extraocular muscle enlargement and orbital fat expansion. In addition to TSHR, orbital fibroblasts from patients with TAO express high levels of insulin-like growth factors-1 receptor (IGF-1R). Stimulatory autoantibodies direct against the insulin-like growth factor-1 receptor have been proposed to contribute to orbital fibroblast activation in TAO. Abstract:The pathogenesis of thyroid-associated ophthalmopathy (TAO) is still unclear.Researches show that the abnormal expansion of orbital adipose tissue is a leading cause of thyroidassociated ophthalmopathy,of which studies are made by both Chinese and overseas researchers and into which some new insights are gained.This article makes a comprehensive review of the progress of researches in terms of the source of orbital preadipocytes,the regulation of adipogenesis,and the expression and role of peroxisome proliferators-activated receptor gamma (PPARγ) and thyroid stimulating hormone receptor (TSHR) in the orbital adipose tissue of the TAO patients.

Abstract:Thyroid-associated ophthalmopathy is a typical autoimmune disease of orbital tissues. Alternative splicing significantly influences many diseases progression, including cancer, age-related macular degeneration, and multiple sclerosis, by modulating the expression of transcripts. However, its role in thyroid-associated ophthalmopathy is still unclear. In this study, differential expression transcripts and differential alternative splicing genes in orbital adipose/connective tissues of thyroid-associated ophthalmopathy patients were detected using RNA sequencing, Cuffdiff, and replicate multivariate analysis of transcript splicing. Three thousand ninety six differential expression transcripts and 2355 differential alternative splicing genes were screened out, while functional enrichment analysis indicated that differential expression transcript and differential alternative splicing genes were associated with immune modulation, extracellular matrix remodeling, and adipogenesis. The expression of the SORBS1, SEPT2, COL12A1, and VCAN gene transcripts was verified by qRT-PCR. In conclusion, prevalent alternative splicing is involved in the disease development in thyroid-associated ophthalmopathy. More attention should be paid to the mechanism of alternative splicing to explore more potential therapeutic targets in thyroid-associated ophthalmopathy.
Regulation of Orbital Fibrosis and Adipogenesis by Pathogenic Th17 Cells in Graves Orbitopathy

Abstract:Context: T helper (Th)17 cells are correlated with many human autoimmune disorders, including Graves disease, and may play key roles in the pathogenesis of Graves orbitopathy (GO). Objective: To study the phenotype of Th17 cells in patients with GO and healthy subjects, investigate the fibrosis and adipogenesis in orbital fibroblasts (OFs) modulated by interleukin (IL)‐17A, and determine the interaction between Th17 cells and OFs. Design/Setting/Participants: Blood samples and orbital tissues from GO patients and healthy controls were collected. Main Outcome Measures: We conducted multicolor flow cytometry, immunohistochemical and immunofluorescent stainings, Western blotting, a PathScan intracellular signaling assay, Luminex and enzyme‐linked immunosorbent assays, and protein mass spectrum. Results: Interferon‐&ggr;‐ and IL‐22‐expressing Th17 cells are increased in GO patients, which are positively related to clinical activity score. Costimulatory molecules are highly expressed in GO orbits and most GO OFs are CD90+. IL‐17A promotes TGF‐&bgr;‐induced fibrosis in CD90+ OFs but impedes 15‐deoxy‐&Dgr;12,14‐prostaglandin J2‐induced adipogenesis in CD90− OFs. Th17 cells promote proinflammatory cytokine secretion in both CD90+ and CD90− OFs. Meanwhile, both CD90+ and CD90− OFs contribute to Th17 cell differentiation through prostaglandin E2 production, which can be attenuated by indomethacin. Furthermore, Th17 cells upregulate costimulatory molecule expression on OFs. Conclusion: Our findings unravel the pathogenicity of IL‐17A in the initiation and progression of GO. In‐depth interpretation of the molecular basis of OFs delineated by CD90 and Th17‐OF interaction will help to afford a novel approach to better therapeutic strategies for GO.

Abstract:To investigate the expression of resistin mRNA and protein in orbital adipose tissues and adipocytes in the patients with and without thyroid associated ophthalmopathy(TAO).【Methods】 The preadipocytes were cultured from the orbital adipose tissues.The preadipoytes stimulated by a differentiation media were differentiated into adipocytes in vitro.The expression of both resistin mRNA and resistin protein in preadipocytes,adipocytes,and adipose tissues were measured by RT-PCR and immunohistochemistry.【Results】 There was no expression of resistin mRNA and protein in preadipocytes,but they were expressed in mature adipocytes and adipose tissues.Immunohistochemistry of mature adipocytes and orbital adipose tissues showed that the resistin protein was abundant in adipocyte cytoplasm.【Conclusion】 The resistin is an adipocytokine secreted by mature adipocytes.In our results,the expression of resistin mRNA and resistin protein were abundant in the orbital adipose tissue of TAO patients,which implicated that the orbital adipose tissue of TAO patients was mostly consisted of mature adipocytes,the diffentiation of preadipocytes was enhanced.

Abstract:Graves’ Ophthalmopathy (GO) is an organ-specific autoimmune disease that is often characterized by infiltration of orbital tissues and is considered as the most common extra-thyroid manifestation of Graves’ disease (GD). Although genetic susceptibility has been found to be critical for the phenotype of GO, the associated risk alleles in a single gene are generally insufficient to cause the disease. Accruing evidence has shown that epigenetic disorders can act as the potentially missing link between genetic risk and clinically significant disease development. Abnormal epigenetic modifications can lead to pro-inflammatory cascades and activation of orbital fibroblasts (OFs) by promoting the various inflammatory response pathways and regulating the diverse signaling molecules that are involved in the fibrogenesis and adipogenesis, thereby leading to the significant expansion of orbital tissues, fibrosis and inflammation infiltration. Additionally, emerging evidence has shown that the gut microbiome can possibly drive the pathogenesis of GO by influencing the secretion of Thyrotropin receptor antibody (TRAb) and T-helper 17 (Th17)/regulatory T cells (Treg) imbalance. This paper describes the latest epigenetic research evidence and progress made in comprehending the mechanisms of GO development, such as DNA methylation, histone modification, non-coding RNAs, and the gut microbiome.

Abstract:In response to pathological stimulation, methylation status conversion of the genome drives changes of cell feature and is able to promote disease development. Yet the role of methylation in the development of thyroid-associated ophthalmopathy (TAO) remains to be evaluated. Overexpansion of orbital tissue is the key feature of TAO. In this study, the methylation profile of orbital adipose/connective tissue from TAO patients and normal individuals were compared. After screening 3,739 differentially methylated probes, the distribution and properties of these probes were analyzed. Furthermore, enriched biological functions of these genes associated with differential methylation and the relationship between their methylation status and expression profile were also identified, including PTPRU and VCAM-1. According to our results, methylation was involved in disregulated immune response and inflammation in TAO and might contribute to activation of fibroblast and adipogenesis, leading to the expansion of orbital tissue. Neuropathy and neurobehavioral symptoms were also potentially associated with methylation. These results may help to extend the understanding of methylation in TAO and provide more insights into diagnosis and treatment of patients.

The Role of Sphingosine-1-Phosphate in Adipogenesis of Graves' Orbitopathy

Abstract:Purpose: To investigate the action of sphingosine-1-phosphate (S1P) in adipocyte differentiation of orbital fibroblasts and determine its putative role in the pathogenesis of Graves' orbitopathy (GO). Methods: Primary preadipocyte orbital fibroblast cultures were stimulated for adipogenesis. Real-time PCR was performed to evaluate the expression of S1P receptor mRNA. To evaluate the effect of S1P and S1P receptor blockers (W146 and FTY720) on adipocyte differentiation, cultures were exposed to each receptor blocker for the first 4 days of the differentiation period. Differentiated cells were stained with Oil Red O, and the production of peroxisome proliferator activator gamma (PPARγ) and CCAAT-enhancer-binding proteins (C/EBP) α and β were determined by Western blot analysis. Results: Sphingosine-1-phosphate receptor 1, 2, and 3 mRNA expression levels were significantly higher in GO tissue samples than non-GO. Sphingosine-1-phosphate receptor 1 through 5 mRNA expression was significantly increased during the 10 days of adipogenesis. Sphingosine-1-phosphate treatment increased the size and number of adipocytes, and increased the expression of adipogenic transcriptional regulators. Treatment with S1P1 receptor inhibitor (W146) for 4 days after induction of adipogenesis attenuated adipocyte differentiation. Sphingosine-1-phosphate receptor blocker also decreased reactive oxygen species (ROS) production in GO orbital fibroblasts and H2O2-stimulated HO-1 production in GO orbital fibroblasts. S1P1 receptor inhibitor reduced the number of adipocytes and suppressed the accumulation of lipid droplets induced by 10 μM H2O2 or 2% cigarette smoke extract (CSE) treatment. Conclusions: Sphingosine-1-phosphate could play a role in orbital adipocyte differentiation of GO. Modulation of S1P actions may provide a therapeutic target for the treatment of GO.
A combined transcriptomics and proteomics approach reveals S100A4 as a potential biomarker for Graves' orbitopathy

Abstract:Background: There are no reliable biomarkers to identify Graves' disease patients who will develop severe Graves' orbitopathy (GO). We hypothesize that integrating various omics platforms can enhance our understanding of disease mechanisms and uncover potential biomarkers. This study aimed to (1) elucidate the differential gene expression profile of orbital fibroblasts in GO during early adipogenesis to better understand disease mechanisms and (2) compare tear protein profiles from our earlier study and the transcriptome profiles of orbital fibroblasts (OFs) to identify possible biomarkers of the disease. Methods: OFs were grown from orbital adipose tissue obtained from nine GO patients (three for discovery and six for validation experiments). Total RNA was extracted from OFs on day 0 as the baseline for each sample and from differentiated OFs on days 4 and 8. Protein-protein interaction (PPI) analysis and functional enrichment analysis were also carried out. The differentially expressed genes (DEGs) from the RNA sequencing experiments were then compared to the full tear proteome profile from the author's previous study, which examined the tear protein changes of GO patients based on fold change > 1.6 or < -1.6. FDR < 0.05 was applied within all datasets. Further validation of S100 calcium-binding protein A4 (S100A4) downregulation in GO was performed via quantitative real-time PCR (qPCR). Results: The whole transcriptomic analysis revealed 9 upregulated genes and 15 downregulated genes in common between the discovery and validation experiments. From the PPI network analysis, an interaction network containing six identified DEGs (ALDH2, MAP2K6, MT2A, SOCS3, S100A4, and THBD) was observed. The functional enrichment network analysis identified a set of genes related to oxysterol production. S100A4 was found to be consistently downregulated in both our transcriptome studies and the full-tear proteome profile from the author's previous study. Conclusion: Our study identified several DEGs and potential gene pathways in GO patients, which concurred with the results of other studies. Tear S100A4 may serve as a biomarker for the propensity to develop thyroid eye disease (TED) in patients with autoimmune thyroid disease (AITD) before clinical manifestation and should be confirmed in future studies.

Abstract:Graves’ orbitopathy (GO), also known as thyroid-associated ophthalmopathy, is the most common ocular abnormality of Graves’ disease. It is a disfiguring, invalidating, and potentially blinding orbital disease mediated by an interlocking and complicated immune network. Self-reactive T cells directly against thyroid-stimulating hormone receptor-bearing orbital fibroblasts contribute to autoimmune inflammation and tissue remodeling in GO orbital connective tissues. To date, T helper (Th) 1 (cytotoxic leaning) and Th2 (antibody leaning) cell subsets and an emerging role of Th17 (fibrotic leaning) cells have been implicated in GO pathogenesis. The potential feedback loops between orbital native residential CD34 - fibroblasts, CD34 + infiltrating fibrocytes, and effector T cells may affect the T cell subset bias and the skewed pattern of cytokine production in the orbit, thereby determining the outcomes of GO autoimmune reactions. Characterization of the T cell subsets that drive GO and the cytokines they express may significantly advance our understanding of orbital autoimmunity and the development of promising therapeutic strategies against pathological T cells.

Abstract:Depot specific expansion of orbital-adipose-tissue (OAT) in Graves’ Orbitopathy (GO) is associated with lipid metabolism signaling defects. We hypothesize that the unique adipocyte biology of OAT facilitates its expansion in GO. A comprehensive comparison of OAT and white-adipose-tissue (WAT) was performed by light/electron-microscopy, lipidomic and transcriptional analysis using ex vivo WAT, healthy OAT (OAT-H) and OAT from GO (OAT-GO). OAT-H/OAT-GO have a single lipid-vacuole and low mitochondrial number. Lower lipolytic activity and smaller adipocytes of OAT-H/OAT-GO, accompanied by similar essential linoleic fatty acid (FA) and (low) FA synthesis to WAT, revealed a hyperplastic OAT expansion through external FA-uptake via abundant SLC27A6 (FA-transporter) expression. Mitochondrial dysfunction of OAT in GO was apparent, as evidenced by the increased mRNA expression of uncoupling protein 1 (UCP1) and mitofusin-2 (MFN2) in OAT-GO compared to OAT-H. Transcriptional profiles of OAT-H revealed high expression of Iroquois homeobox-family (IRX-3&5), and low expression in HOX-family/TBX5 (essential for WAT/BAT (brown-adipose-tissue)/BRITE (BRown-in-whITE) development). We demonstrated unique features of OAT not presented in either WAT or BAT/BRITE. This study reveals that the pathologically enhanced FA-uptake driven hyperplastic expansion of OAT in GO is associated with a depot specific mechanism (the SLC27A6 FA-transporter) and mitochondrial dysfunction. We uncovered that OAT functions as a distinctive fat depot, providing novel insights into adipocyte biology and the pathological development of OAT expansion in GO.

Enhanced orbital adipogenesis in a mouse model of T-cell-mediated autoimmunity, zymosan A-treated SKG mice: Implications for Graves’ ophthalmopathy

Abstract:Abstract Inflammation and remodelling of orbital tissue associated with enhanced adipogenesis commonly occur in Graves’ ophthalmopathy (GO), however, the underlying mechanisms that link immune cells and adipocytes in orbital inflammation are not well-known. The primary aim of this study was to elucidate how a genetically determined shift in the T-cell repertoire toward self-reactive T-cells could drive orbital adipogenesis. To induce the T-cell-mediated autoimmune response, SKG mice were intraperitoneally injected with zymosan A once at 8 weeks of age. After three months, orbital magnetic resonance imaging (MRI), histopathologic studies, and in vitro analyses were performed to evaluate inflammation and adipogenesis. The eyes of the zymosan A-treated SKG mice displayed proptosis and blepharitis. A detailed analysis of orbital adipose tissue showed enhanced orbital adipogenesis and cellular infiltration compared to controls. In addition, increased secretion of adipokines and other cytokines in the periorbital tissue was observed, together with elevated serum concentration of inflammatory cytokines. Orbital adipogenesis was enhanced in zymosan A-treated SKG mice, a novel mouse model for GO-like inflammatory adipose phenotypes most likely induced by T-cell mediated autoimmune responses. This mouse model gives us the opportunity to examine the underlying molecular mechanisms of enhanced adipogenesis in GO, ultimately providing a potential therapeutic target alternative to conventional GO treatment.

Culture and Differentiation of Human Orbital Preadipocytes in Thyroid-Associated Ophthalmopathy.

Abstract:PURPOSE:To identify whether there are preadipocytes in the orbits of thyroid-associated (TAO) patients and whether they possess the capacity to undergo ophthalmopathy adipocytic differentiation.METHODS:Orbital fat tissue from TAO patients was placed in primary culture and proliferating cells were sub-passaged. The preadiocytes were identified with immunohistochemistry. Confluent preadipocytes were subjected to a differentiation protocol. And Peroxisome proliferators-associated receptory (PPARgamma) transcript levels were determined before and after differentiation.RESULT:In primary culture, cells were spindly and showed positive immunostaining for preadipocyte factor-1 (pref-1) protein. During the process of differentiation, orbital cells showed evidence of adipogenesis and relatively greater PPARy gene expression with differentiation.CONCLUSION:There are active preadipocytes in the orbital tissue of TAO patients which might be able to undergo adipocyte differentiation. These observations suggest that the preadipocytes differentiation contributed to excess orbital adipose tissue volume in TAO.
Analysis of N6-methyladenosine modification and immune infiltration characterization in thyroid associated ophthalmopathy

Abstract:Abstract Background Thyroid-associated ophthalmopathy (TAO) is a potentially sight-threatening ocular disease that affects 50% of patients with Graves’ disease. N6-methyladenosine (m6A) modification is post-transcriptional RNA modification in mammalian mRNA. Exploring the role of m6A regulators and the immune infiltration characterization in TAO is essential to understand its pathogenesis. Methods We excavated differentially expressed m6A regulators from the TAO microarray data set (GSE58331) in the Gene Expression Omnibus (GEO). Based on R software (v4.2.2), we performed bioinformatics analyses including differential expression analysis, training models construction such as support vector machine (SVM), random forest (RF), and nomogram models for the evaluation of TAO occurrence, consensus clustering algorithm for the evaluation of clusters quantity, principal component analysis (PCA) for quantification of m6A modification level in individual patients, Gene Ontology (GO) functional annotation, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis, and single sample gene set enrichment analysis (ssGSEA) for functional enrichment analysis, etc. Results Four significant m6A regulators (IGFBP2, IGFBP3, RBM15B, and FTO) were screened by differential expression analysis between the TAO group and the healthy group. RM identified that the four m6A regulators above were associated with the risk of TAO. Furthermore, a nomogram was conducted to provide benefits to patients. Based on filtered m6A regulators, we divided the TAO group into two m6A subtypes (cluster A/B) by consensus clustering. The PCA algorithm calculated the m6A score of each sample to quantify m6A patterns. GO functional annotation proved that m6A-related differentially expressed genes (DEGs) enriched in biological processes (BP), cellular components (CC), and molecular functions (MF). KEGG enrichment analysis showed m6A-related DEGs were particularly abundant in the endocytosis, protein processing in endoplasmic reticulum, ubiquitin mediated proteolysis, and protein export. ssGSEA presented the difference in immune infiltration between m6Acluster A/B and gene cluster A/B. In addition, we also analyzed the relationship between m6Aclusters or gene clusters and cytokines. Conclusions This article analyzed the effect of m6A modification in the pathogenesis of TAO, focusing on immune infiltration. These findings may be able to guide the future strategy of immunotherapy.

Abstract:Thyroid-associated ophthalmopathy (TAO) is an autoimmune disease. Recent studies have found the aberrant epigenetics in TAO, including DNA methylation, non-coding RNAs, and histone modification. Many genes have an aberrant level of methylation in TAO. For example, higher levels are found in CD14, MBP, ANGLE1, LYAR and lower levels in DRD4 and BOLL. Non-coding RNAs are involved in the immune response (miR-146a, miR-155, miR-96, miR-183), fibrosis regulation (miR-146a, miR-21, miR-29), adipogenesis (miR-27) and are thought to play roles in TAO. MicroRNA is also related to the clinical activity score (miR-Let7d-5p) and may be a predictor of glucocorticoid therapy (miR-224-5p). The quantities of H4 in TAO are increased compared with euthyroid control subjects, and the role of histone modifications in Graves' disease may lead to better understanding of its role in TAO. More studies are needed to explain the role of epigenetics in TAO and provide potential therapeutic strategies.

Traditional Chinese medicine in thyroid-associated orbitopathy

Abstract:Purpose: Orbital fibroblasts (OF) are considered the central target cells in the pathogenesis of thyroid-associated orbitopathy (TAO), which comprises orbital inflammation, orbital tissue edema, adipogenesis, fibrosis, oxidative stress and autophagy. Certain active ingredients of traditional Chinese medicine (TCM) demonstrated inhibition of TAO-OF in pre-clinical studies and they could be translated into novel therapeutic strategies. Methods: The pertinent and current literature of pre-clinical studies on TAO investigating the effects of active ingredients of TCM was reviewed using the NCBI PubMed database. Results: Eleven TCM compounds demonstrated inhibition of TAO-OF in-vitro and three of them (polydatin, curcumin, and gypenosides) resulted in improvement in TAO mouse models. Tanshinone IIA reduced inflammation, oxidative stress and adipogenesis. Both resveratrol and its precursor polydatin displayed anti-oxidative and anti-adipogenic properties. Celastrol inhibited inflammation and triptolide prevented TAO-OF activation, while icariin inhibited autophagy and adipogenesis. Astragaloside IV reduced inflammation via suppressing autophagy and inhibited fat accumulation as well as collagen deposition. Curcumin displayed multiple actions, including anti-inflammatory, anti-oxidative, anti-adipogenic, anti-fibrotic and anti-angiogenic effects via multiple signaling pathways. Gypenosides reduced inflammation, oxidative stress, tissue fibrosis, as well as oxidative stress mediated autophagy and apoptosis. Dihydroartemisinin inhibited OF proliferation, inflammation, hyaluronan (HA) production, and fibrosis. Berberine attenuated inflammation, HA production, adipogenesis, and fibrosis. Conclusions: Clinical trials of different phases with adequate power and sound methodology will be warranted to evaluate the appropriate dosage, safety and efficacy of these compounds in the management of TAO.
