Abstract:Question: A 63-year-old man with a past medical history of hypertension presented with increasing lower extremity edema, persistent hypokalemia, and back pain for >2 months. Despite administration of spironolactone together with intravenous and oral potassium supplement, his clinical symptoms progressed, prompting admission to our institution. Physical examination showed prominent bilateral pitting lower extremity edema up to his knees. Laboratory results showed potassium 2.9 mmol/L (normal, 3.5–5.3), Na+ 150 mmol/L (normal, 137–147), Ca2+ 1.89 mmol/L (normal, 2.1–2.6), alanine transaminase 145U/L, blood sugar 7.5 mmol/L, and lipase 110.8 U/L. Cytokeratin 19 fragment 3.55 ng/ml (normal, <3.30), neuron-specific enolase 24.03 ng/mL (normal, <17.00), and other tumor markers were within normal limits. Insulin was 11.4 mU/L, (normal, 2.6-11.1) and glucagon 71 pg/mL (normal, <200). Magnetic resonance imaging of the brain revealed no pituitary lesions. Thoracic computed tomography scanning revealed no abnormality, and abdominal computed tomography and magnetic resonance imaging scans showed a mass in the pancreatic head with several scattered liver lesions (Figure A, B). Radiographs and bone densitometry confirmed severe spinal osteoporosis with first lumbar (L1) vertebral compression fracture. Endoscopic ultrasound examination demonstrated an irregular shaped, hypoechoic heterogeneous mass in the head of the pancreas (Figure C) with multiple intrahepatic solid nodules (Figure D). Positron emission tomography scanning displayed a high accumulation of 18fluorodeoxyglucose in the pancreatic head, distribution at multiple sites in the liver, and bilateral adrenal enlargement (Figure E). Somatostatin receptor scintigraphy using 99mTc-TOC showed significant uptake within lesions in the left hepatic lobe, peripancreatic lymph nodes, and right adrenal gland. What is your diagnosis? The cortisol level was 51.63 μg/dL (normal, 6.2-19.4), adrenocorticotropic hormone (ACTH) was 358 pg/mL (normal, <46), and 24-hour urine cortisol was 4005.2 μg (normal, 30.15–129.13). Suppression of serum cortisol failed after both low- and high-dose dexamethasone tests. These findings highly suggested an ectopic ACTH syndrome (EAS). Endoscopic ultrasound fine needle aspiration of pancreatic and hepatic lesions both indicated a well-differentiated neuroendocrine tumor. We implemented a multidisciplinary team approach for the management of this patient. Treatment with octreotide was started, followed by pancreaticoduodenectomy combined with radiofrequency ablation of liver metastases plus right adrenalectomy. Histology showed that the tumor was composed of small round-to-oval cells with a few mitoses (Figure F), and a diagnosis of pancreatic neuroendocrine tumor (PNET) with multiple liver and lymph node metastases was made. There was extension into peripancreatic tissue and duodenum. Perineural spread was seen, but the adrenal gland was free of tumor. Immunohistochemical results showed that the tumor cells were positive for chromogranin A, synaptophysin, and focally positive for ACTH (Figure G), with a Ki-67 index of 3% and 4 mitoses per 10 high-power fields. His cortisol and ACTH levels returned to normal 1 month after surgery. However, the Cushing’s symptoms returned 4 months later with liver metastases detected. He was treated with chemotherapy and a somatostatin analogue, and was alive with disease 8 months after the initial operation. PNET is a rare cause of EAS. EAS-PNET is usually malignant with metastases frequently detected at the initial diagnosis. It has a poor prognosis owing to the rapid progression of the disease.1Kondo T. Matsuyama R. This case highlights the feasibility of endoscopic ultrasound–guided fine needle aspiration in accurate diagnosis of EAS-PNET preoperatively. Comprehensive treatment, consisting of surgery, somatostatin analogues, chemotherapy, and targeted therapy are helpful to control hypercortisolemia in EAS-PNET patients, and reduce tumor burden, prolonging survival.3Vaduganathan M.
