Abstract:Objective. The purpose of this study was to describe the sonographic findings of soft tissue diffuse neurofibromas. Methods. Seven soft tissue diffuse neurofibromas proven pathologically were retrospectively evaluated. Medical records and histologic examinations were reviewed, and sonograms were analyzed for the echogenicity, location, margin, and vascularity. Results. All identified lesions were hyperechoic masses permeated by multiple interconnecting irregular hypoechoic tubular or nodular structures. All lesions were located in the subcutaneous fat zone, and all had poorly defined margins with adjacent normal subcutaneous fat. The margins between the lesions and the adjacent muscles were well defined in 5 and poorly defined in the other 2. The vascularity was extensive in 5 lesions and scarce in the other 2. Conclusions. Soft tissue diffuse neurofibromas have a characteristic sonographic appearance; therefore, sonography should be considered a useful tool in the initial screening of this disease.
High-frequency Sonography in the Diagnosis of Peripheral Nerve Neurofibromas

Abstract:Objective The purpose of this study was to explore the value of high-frequency sonography in the diagnosis of three different types of peripheral nerve neurofibromas.Methods Twenty-one peripheral nerve neurofibromas pathologically proven were retrospectively evaluated.Sonograms were analyzed for the size,location,margin,echogenicity and vascularity.Results(1)Nodular neurofibromas were in 10 cases.Most of the lesions were oval,well-defined,hypoechoic masses with posterior acoustic enhancement and inner increased vascularity.Four lesions arose from and conneeted with the large nerve trunks were visualized.(2)Plexiform neurofibromas were in 3 cases.All lesions had a single or multiple hypoechoic nodules and conne cted with multiple peripheral strip-like hypoechoic lines.The vascularity was extensive in all lesions.(3)Diffuse neurofibromas were in 8 cases.All lesions located in the subcutaneous fat layer with ill-defined margins.7 lesions were hyperechoic masses with multiple irregular hypoechoic tubular and/or nodular structures,and the other one was hypoechoic heterogeneous mass.The vascularity was extensive in 6 lesions and rare in the other two.Conclusions Plexiform neurofibromas and diffuse neurofibromas have characteristic sonographic appearances,therefore,sonography is of great value in the diagnosis of these diseases;but in nodular neurofibromas,sonographic characteristics were limited,unless these lesions arise from the large nerve trunks.

Abstract:This study aimed to describe the characteristics of superficial desmoid fibromatosis (DF) using two-dimensional and contrast-enhanced ultrasonography, intending to improve diagnostic accuracy. We retrospectively analyzed 19 cases of superficial DF confirmed by surgery or core-needle biopsy in our hospital from January 2018 to August 2020. All patients underwent two-dimensional and contrast-enhanced ultrasound (CEUS) examination. Nineteen patients included 15 women and 4 men, with an average age of 33.37±12.13 years old. The mean size of lesions was 4.78±1.99 cm. On ultrasound, all lesions presented as solitary heterogeneous hypoechoic masses; 13 presented with ill-defined margins. Ten lesions (52.63%) presented with fusiform shapes, and 11 lesions (57.89%) presented with the “fascial tail” sign. CEUS suggested the tumors were hyperenhanced, with an enhanced pattern of rapid wash-in and slow wash-out. Four lesions (21.05%) showed an enlarged scope in the CEUS image compared with the grayscale ultrasound image. In conclusion, an ill-defined heterogeneous hyperechoic appearance with fusiform-shaped and “fascial tail” signs on US and heterogeneous hyperenhancement with an enlarged scope on CEUS are valuable clues in the diagnosis and treatment of superficial DF.
Clinicopathological features and surgical treatment in patients with central neurocytoma

Abstract:Objective To investigate the pathological characteristics and the treatment of central neurocytoma(CN).Methods Eight patients with CN,together with clinical manifestations,imaging features,and surgical methods,from January 2001 to December 2009 were retrospectively analyzed.The S100 protein,Ki-67,synaptophysin(Syn) and glial fibrillary acidic protein(GFAP) were studied using envision system immunohistochemistry.Results There were 6 cases whose tumors located in the central and anterior part of lateral ventricle,1 in the posterior part of lateral ventricle,and 1 in the pineal region.Their CT images showed equal or slightly high density mass in which there were multiple small low density area and calcification.MR images showed equal or slightly low signal in T1 weighted imaging(T1WI),uneven and slightly high signal in T2WI,and mild to moderate signal after enhancement.Immunohistochemical results showed 7 cases were positive in Syn,1 was positive in S100,6 were negative and 2 were partly positive in GFAP.The positive rate of Ki-67 was 0.3%-5.5%.All patients were treated with microsurgery.Total resections were taken in 6 cases,subtotal resections were taken in 2 cases and postoperative radiotherapy was given to residual tumor.All the cases were followed up from 1 to 4 years,and the results showed only 1 case developed a recurrent tumor and received a total resection again,and the others got good curative effect.Conclusions CN manifests certain characteristic imaging features.High expression of Syn,low expression of GFAP and S100 in CN are valuable for the diagnosis and differentiation.The treatment of microsurgical resection and radiotherapy to residual tumor can achieve good curative effect.

Abstract:Objective To investigate the ultrasonographic features corresponding to classification of peripheral neuropathies in neurofibromatosis type Ⅰ.Methods Thirty-six patients with peripheral neuropathies and diagnosis of neurofibromatosis type I were enrolled in this study.Ultrasound appearance of peripheral neuropathies was analyzed and divided into four groups according to their pathological classifications.There were 19 males and 17 females with age between 7-47 years.High frequency or board frequency ultrasound probes were used for scanning.Results The ultrasound findings were varied based on different pathological classifications.Multiple nodular class(n=5) was presented as multiple nodes.Ultrasound demonstrated that tumors were multiple,oval or lobulated,and had well defined margin,and the echogenic of lesions was mainly hypoechoic.Plexiform class (n=12) was presented as cluster nodes along the peripheral nerve trunk.Ultrasound showed trunk nerves in extremities became thicker in a long range and lost the normal structure,and catenular or racemosum in shape.The echogenicity was hypoechoic.Diffuse class (n=14) was diffuse subcutaneous disease.Tumors range was infiltrative.The subcutaneous soft tissue was thicken and echogenicity was increased.The structure of nerve bundle was disturbed and associated with multiple hypoechoic nodes.Mixture class (n=5) was defined as mixture of multiple nodular class and plexiform class.In this group,diffuse disorders were co-existed with multiple nodes disorder.Conclusions The ultrasonographic appearances of peripheral nerve disorders in neurofibromatosis type I was closely corresponded to their pathological characters.And the ultrasonic examination would be helpful to diagnosis and provide important information for therapy.
Characterisation of a Novel Radiological Entity in Neurofibromatosis Type 1 - Diffuse Neurofibromatous Tissue

Abstract:Objectives: To describe the prevalence, demographics and characteristics of a novel radiological entity in neurofibromatosis type 1: diffuse neurofibromatous tissue (DNFT) Design: Aretrospective, descriptive review of MDT and radiology notes. Methods: Of the 1049 patients from the NF1 adult radiology MDT minutes (2009–2021), 77 patients with DNFT were identified and clinical data were collected. MRI scans from 20 DNFT cases were interpreted. Results: Although overall gender distribution of DNFT was roughly even, it was more prevalent in females (73.9%) at the sacroiliac joint—where this entity was most common (29.9%). DNFT often involves the fibrous part of the sacroiliac joint and is seen as diffuse, streaky infiltrating tissues that cause bone erosion without mass effect. The period prevalence of scoliosis and dural ectasia on corresponding spinal levels with spinal DNFT was 62.8 and 51.2%, respectively (n=43). Conclusions: This is the first reported descriptive study of DNFT in NF1 and the first to describe its MRI features in detail. The predilection for the sacroiliac joint and the possible associations with scoliosis and dural ectasia provide important insights that can form the basis for future studies whilst also suggesting the need for active surveillance of this tissue in NF1 patients.

Abstract:Clinical features and ultrasonographic characteristics of 43 lesions of 40 patients pathologically proven as NLCNs at Peking University Shenzhen Hospital from October 2014 to May 2022 were analyzed retrospectively. NLCNs present as fusiform shape in long axis and round shape in short axis. The common ultrasound findings of NLCN are well‐defined, predominately hypoechoic, homogeneous lesion with posterior enhancement, and poor blood supply. Abstract:Background: Nodular fasciitis (NF) has nonspecific clinical manifestations and is often misdiagnosed as sarcoma. The investigations of imaging methods for NF were limited. Objective: To analyze the ultrasound (US) features of NF, and to evaluate the diagnostic value of US for NF. Materials and Methods: A total of 61 NF patients were recruited retrospectively, and 551 lesions in the subcutaneous fat layer were included for comparison. We evaluated the ultrasound features of the patients and divided the NF cases into three types. Chi-square test or Fisher exact test were conducted to detect the potential difference in the distributions of three types in the two groups. Results: Among the 61 NF cases, 65.6% were in the upper extremities (n = 40). The proportion of type 1, 2, and 3 were 57.4%, 24.6%, and 18.0%, respectively. NF were significantly more likely locating in the upper extremities than the other soft tissue tumors (p < 0.001). Type 1 and type 2 of sonographic features were significantly more commonly observed in NF than other soft tissue tumors among the three types (p < 0.001). Conclusion: The type 1 and type 2 of US features can help to distinguish NF from other lesions. US has great potential to improve the diagnostic accuracy and reduce the unnecessary surgery.

Abstract:Objective To investigate the application value of ultrasonography in evaluating the peripheral neuropathy of neuro-fibromatosis type I (NF1).Methods The ultrasonic manifestations of peripheral neuropathy in 11 cases of NF1 confirmed by clinic or pathology were retrospectively analyzed.The amount, position, range, internal echo, vascular distribution, and the relationship with peripheral nerve and tissue of peripheral nerve lesions in NF1 were observed.Results The ultrasonic characteristics of peripheral neu-ropathy in NF1 showed as neurofibromas of varied sizes, multi focal distribution, and multiform performances.They were divided into three types.5 cases were multiple nodular types.They appeared as round, lobulated and spindly, clear boundary and solid hypoechoic mostly.4 cases were of plexiform type.The involved neural trunks were deformed, distorted, and thickened.The lesions were plexi-form hypoechoic and grew axially along the neural trunks.2 cases were of diffuse type, manifested as subcutaneous fat layer thickening with fuzzy borders, flaked hyperechoic with linear, cord like hypoechoic distribution.3 cases of recurrence in this group were malignant Schwannoma.The sizes of tumor were larger.The internal echoes were heterogeneous.The internal vessels were abundant.The sur-rounding tissues were infiltrated.Conclusions Ultrasonography can clearly display the features of involved peripheral neuropathy in NF1;provides objective and exact evidences to clinical diagnosis.At the same time ultrasonic follow-up also plays an important role in finding recurrence and malignant transformation of peripheral neuropathy in NF1.
Perfusion Mr Imaging and Proton Mr Spectroscopy in A Case of Dysembryroplastic Neuroepithelial Tumor

Abstract:Dysembryoplastic neuroepithelial tumors (DNTs), which were first described by Daumas-Duport in 1988, are one of rare benign tumors usually associated with medically intractable seizures which date from childhood.1 The clinical, pathologic and neuroradiologic findings of DNT have been described.2 Recent advances in magnetic resonance imaging (MRI) technology allow the acquisition of cerebral microcirculation parameters by perfusion weighted imaging (PWI) and brain metabolic indices by MR spectroscopy (MRS). Several studies have shown the utility of PWI and MRS can improve the diagnostic accuracy of brain tumor,3 we combine the two techniques to evaluate a case with DNT and suggest that wider application of these techniques is warranted.

Abstract:Neurofibromatosis (NF) type I is a neuroectodermal and mesodermal dysplasia caused by a mutation of the neurofibromin tumor suppressor gene. Phenotypic features of NF1 vary, and patients develop benign peripheral nerve sheath tumors and malignant neoplasms, such as malignant peripheral nerve sheath tumor, malignant melanoma, and astrocytoma. Multiparametric whole-body MR imaging (WBMRI) plays a critical role in disease surveillance. Multiparametric MRI, typically used in prostate imaging, is a general term for a technique that includes multiple sequences, i.e. anatomic, diffusion, and Dixon-based pre- and post-contrast imaging. This article discusses the value of multiparametric WBMRI and illustrates the spectrum of whole-body lesions of NF1 in a single imaging setting. Examples of lesions include those in the skin (tumors and axillary freckling), soft tissues (benign and malignant peripheral nerve sheath tumors, visceral plexiform, and diffuse lesions), bone and joints (nutrient nerve lesions, non-ossifying fibromas, intra-articular neurofibroma, etc.), spine (acute-angled scoliosis, dural ectasia, intraspinal tumors, etc.), and brain/skull (optic nerve glioma, choroid plexus xanthogranuloma, sphenoid wing dysplasia, cerebral hamartomas, etc.). After reading this article, the reader will gain knowledge of the variety of lesions encountered with NF1 and their WBMRI appearances. Timely identification of such lesions can aid in accurate diagnosis and appropriate patient management.
Nodular Fasciitis: a Retrospective Study of 272 Cases from China with Clinicopathologic and Radiologic Correlation

Abstract:Our aim is to describe the largest series of nodular fasciitis (NF) with emphasis on the clinicopathologic and radiologic correlation. A total of 272 cases of NF were diagnosed between 2004 and 2014 at our institution. There were 160 males and 112 females with age ranging from newborn to 77 years (mean, 36 years). The upper extremity was the most common location (34%), followed by the head and neck region (24%), trunk (21%), and lower extremity (14%). By radiology, the lesion appeared as well-defined homogeneous mass with low or isodensity on computed tomography, homogenous hypointense or isointense on T1-weighted sequences, and heterogeneous intermediate-to-high signal on T2-weighted sequences. Although all cases were composed of short intersecting fascicles of uniform plump spindle cell, the cellularity and stromal components varied considerably between different cases. In intramuscular or deeply seated NFs, extension into adjacent skeletal muscles or structures was often noted. Immunohistochemically, all cases showed diffuse staining for smooth muscle actin and calponin, with consistent negativity for desmin, h-caldesmon, and β-catenin. Of patients with available followed up information, only 1 experienced local recurrence due to incomplete excision. Our comprehensive study further demonstrated that NF had a wide clinicopathologic spectrum. Correlation with the radiologic features may help pathologists in arriving at an accurate diagnosis.

Abstract:Abstract BACKGROUND AND PURPOSE To evaluate the differences in the imaging findings of intracranial solitary fibrous tumor (SFT) and meningioma on dynamic susceptibility contrast (DSC) perfusion MRI, apparent diffusion coefficient (ADC), and conventional sequences. METHODS Between January 2016 and April 2022, the study enrolled 14 (median, 46 years [range, 25–74]; six females; seven with DSC-MRI) and 27 patients (median, 53 years [21–83]; 23 females; all with DSC-MRI) with pathologically proven SFT and meningioma, respectively. The normalized relative cerebral blood volume and flow (nrCBV and nrCBF), percentage signal recovery (PSR), normalized mean ADC (nADCmean), and conventional imaging features, including T1-weighted hyperintensity, were evaluated and statistically compared using Bonferonni correction. RESULTS PSR was significantly lower in SFT compared with that in meningioma (median, 22.5 [range, 6.4–48.8] vs. 70.1 [range, 18.6–104.2], p< 0.001). The majority of SFT showed mainly T1-weighted hyperintensities compared to the gray matter, while only one meningioma showed the finding (8/14 [57.1%] vs. 1/27 [3.7%], p< 0.001). No significant difference was observed in nrCBV, nrCBF, or nADCmean. CONCLUSIONS PFS was significantly lower in intracranial SFT compared with that in meningioma, indicating the difference in capillary permeability between the tumors. Comparing the hyperintensity on T1WI of the solid components with that of the cerebral cortex was a simple and useful characteristic to differentiate SFT from meningioma.

Abstract:In Neurofibromatosis type 1 (NF1), peripheral nerve sheaths tumors are common, with cutaneous neurofibromas resulting in significant aesthetic, painful and functional problems requiring surgical removal. To date, determination of adequate surgical resection margins–complete tumor removal while attempting to preserve viable tissue–remains largely subjective. Thus, residual tumor extension beyond surgical margins or recurrence of the disease may frequently be observed. Here, we introduce Shifted-Excitation Raman Spectroscopy in combination with deep neural networks for the future perspective of objective, real-time diagnosis, and guided surgical ablation. The obtained results are validated through established histological methods. In this study, we evaluated the discrimination between cutaneous neurofibroma (n = 9) and adjacent physiological tissues (n = 25) in 34 surgical pathological specimens ex vivo at a total of 82 distinct measurement loci. Based on a convolutional neural network (U-Net), the mean raw Raman spectra (n = 8,200) were processed and refined, and afterwards the spectral peaks were assigned to their respective molecular origin. Principal component and linear discriminant analysis was used to discriminate cutaneous neurofibromas from physiological tissues with a sensitivity of 100%, specificity of 97.3%, and overall classification accuracy of 97.6%. The results enable the presented optical, non-invasive technique in combination with artificial intelligence as a promising candidate to ameliorate both, diagnosis and treatment of patients affected by cutaneous neurofibroma and NF1.

Abstract:Cutaneous neurofibromas (cNFs) are characteristic of neurofibromatosis 1 (NF1), yet their immune microenvironment is incompletely known. A total of 61 cNFs from 10 patients with NF1 were immunolabeled for different types of T cells and macrophages, and the cell densities were correlated with clinical characteristics. Eight cNFs and their overlying skin were analyzed for T cell receptor CDR domain sequences, and mass spectrometry of 15 cNFs and the overlying skin was performed to study immune-related processes. Intratumoral T cells were detected in all cNFs. Tumors from individuals younger than the median age of the study participants (33 years), growing tumors, and tumors smaller than the data set median showed increased T cell density. Most samples displayed intratumoral or peritumoral aggregations of CD3-positive cells. T cell receptor sequencing demonstrated that the skin and cNFs host distinct T cell populations, whereas no dominant cNF-specific T cell clones were detected. Unique T cell clones were fewer in cNFs than in skin, and mass spectrometry suggested lower expression of proteins related to T cell-mediated immunity in cNFs than in skin. CD163-positive cells, suggestive of M2 macrophages, were abundant in cNFs. Human cNFs have substantial T cell and macrophage populations that may be tumor-specific.

Abstract:OBJECTIVE:To determine the clinical value of multiparametric whole-body (WBMRI) over whole-spine magnetic resonance imaging (WSMRI) in patients with neurofibromatosis type 1 (NF1).METHODS:A consecutive series of 30 patients with known NF1 underwent WBMRI screening using anatomic, diffusion, and contrast imaging over a 30-month period. Thirteen of 30 patients also had WSMRI. Tumors were classified per location and morphology and were software segmented to determine numbers and volumes. Extra tumor burden detected by WBMRI was assessed. The comparison was made between WBMRI and WSMRI in 13 patients who had both types of scans. Enhancement characteristics were noted and 2 readers recorded apparent diffusion coefficient (ADC) in 30 patients with WBMRI scans. Interobserver performance was assessed using intraclass correlations. A 2-sample test was used for testing mean differences between tumors.RESULTS:The age of 30 patients with WBMRI and 13 patients with WSMRI were 39.4 ± 14.4 and 41.54 ± 10.79 years (mean ± standard deviation) and male/female ratio was 1:1.73 and 1:2.25, respectively. Only 1 patient was found to have a heterogeneously enhancing lumbar paraspinal malignant peripheral nerve sheath tumor, seen on both WBMRI and WSMRI. The additional total number of tumors on WBMRI was 2766 and 2602 tumors were missed on WSMRI. The volume of tumors was 16,053 cm3 and 15,614 cm3 of tumor burden was incrementally detected on WBMRI. Mean ADC of superficial tumors was significantly lower than that of deep tumors (1.93 ± 0.39 × 10-3 mm2/second and 2.26 ± 0.56 × 10-3 mm2/second, respectively; P = 0.009), whereas no ADC differences were seen in plexiform versus discrete tumors (P = 0.64). Interobserver performance for ADC was excellent (intraclass correlation, 0.84).CONCLUSIONS:Multiparametric WBMRI provides superior determination of tumor burden and should be considered as a preferred method for evaluation of patients with NF1.

Abstract:Objective To evaluate MRI and dynamic enhancement scanning in the diagnosis of nerve sheath tumors(NSTs) in soft tissue. Methods Thirty histopathologically proved soft-tissue NSTs were analyzed by MRI in 23 patients. They were divided into 15 benign NSTs which included 13 Schwannomas, 2 neurofibromas and 15 malignant peripheral nerve sheath tumors(MPNSTs). Scan sequences included SE T_1WI, FSE T_2WI, Fast multiplanar spoiled gradient recalled technique(FMPSPGR) before and after injection of Gd-DTPA. Results There were 30 NSTs in 23 patients. Soft-tissue NSTs presented as an fusiform mass(15/30),“target” signs(9/30) , the tubular entering and exiting nerve (10/30), the split-fat sign(10/30). Two MPNSTs showed associated surrounding edema. Twelve benign NSTs manifested delayed enhancement(n=9)and no enhancement(n=3). All MPNSTs and 3 benign NSTs demonstrated early enhancement with slow washout or without obvious washout. Conclusion Fusiform shape, “target” sign, the relationship to the nerve and “split-fat” sign were characteristic signs of soft-tissue NSTs and dynamic contrast-enhanced MRI may be useful and helpful to differentiate malignant from benign NSTs.
Schwannomatosis: a New Member of Neurofibromatosis Family.

Abstract:BACKGROUND:Schwannomatosis is a recently recognized peripheral nerve polyneoplasm with clinical characteristics and a genetic background that differ from those of neurofibromatosis 2 (NF2). The diagnostic and treatment criteria of this rare disorder are herein discussed.METHODS:The data of 180 patients who underwent operations for benign schwannomas from 2003 to 2012 in our center were reviewed. Eight of them were classified as schwannomatosis according to the diagnostic criteria suggested by MacCollin. The demographic characteristics were documented and compared between the two groups of patients. The patients' clinical presentations, imaging characteristics, histological features, and treatment results were retrospectively investigated and summarized.RESULTS:Of the 180 cases of benign schwannomas we reviewed this time, eight patients presented with schwannomatosis (4.44%). The mean age of the two groups was not significantly different (40.0 vs. 44.7 years, t = 0.88, P = 0.378). However, schwannnomatosis seems to more generally occur in females (75% vs. 48% were females, P = 0.162), although the difference was not statistically significant. The initial main symptom was pain. The neurological examination was otherwise normal. Magnetic resonance imaging (MRI) revealed multiple discrete, well-defined round, or oval lesions distributed along the course of the peripheral nerves in the extremities with low-to-intermediate signal intensity on T1-weighted images and high-signal intensity on T2-weighted images. Vestibular schwannomas were excluded in four patients by cranial MRI. The lesions in all patients were resected and were pathologically proven to be schwannomas. The average follow-up period was 26 months. Six individuals obtained a good result without symptoms or function loss.CONCLUSIONS:Schwannomatosis is characterized by the development of multiple schwannomas without evidence of the vestibular tumors that are diagnostic for NF2. It commonly occurs in middle-aged females. It has similar demographic features to solitary benign schwannoma. Surgical resection always results in a good outcome.
Imaging Features and Pathologic Correlations of Desmoid-Type Fibromatoses

Abstract:Objective To investigate the imaging and pathology features of Desmoid-type Fibromatoses(DF) and to improve the knowledge and diagnostic accuracy of DF.Methods Imaging features of 14 patients with DF confirmed by pathology were reviewed retrospectively.5 of 14 patients underwent CT scan and contrast-enhancement CT scan,9 of 14 patients underwent MRI scan and 8 cases contrast-enhancement MRI scan.Results 8 of 14 cases were located in extra-abdominal(2 chest,2 neck,2 back,2 extremities,1 hip),3 in bone and 3 in napes.Tumors were single in all patients,contours of the tumors were presented round in 4 cases,spindle in 2 cases,lobulated in 4 cases and irregular in 4 cases.The tumors ranged in size from 3 to 21cm in diameter.Well defined borders showed in 5 cases,whereas 9 cases had ill defined margins.6 lesions showed homogeneous density on plain CT scan,8 tumors were isointense or hypointense in relation to muscle on T1WI,whereas T2WI showed predominantly high signal intensity but hypointensity compared with fat.The border of the tumors could be seen clearly on STIR.Tumors showed intense heterogeneous enhancement on postcontrast.The tumors consisted of fibroblasts and myofibroblasts which had no mitotic activity and polymorphism presented infiltrative features,without capsule or intact capsule.Conclusion Compared with CT,MRI can characterize the imaging features such as the shape,border,signal and enhancement pattern of DF,which may have a potential role in differential diagnosis.However,the final diagnosis should be based on histo-pathology.

Abstract:Objective To explore the diagnostic value of 3.0T MRI in neurogenic tumor of soft tissue in the extremities.Methods The MRI appearance of 17 neurogenic tumors with pathological confirmation was retrospectively analyzed.Various imaging characteristics of tumors were evaluated and different imaging findings were compared.The diagnosis value of each MRI features was evaluated with receiver-operating-characteristics (ROC) analysis.Results In the benign tumors significant differences between neurilemmoma and neurofibromas were noted for the position (P =0.044).Heterogenicity on T2-weighted fat suppression images was also significant in differentiating between neurilemmoma and neurofibromas (P =0.020).The shape of tumors,maximum length of tumor short diameter,edem around masses,relationship with adjoining fascia had the best discriminatory ability.The ROC analysis yield the area under curve (AUC) of them was 0.967 (P =0.037),0.923 (P =0.048),0.981 (P =0.034),0.981 (P =0.034),respectively.Conclusion If the neurogenic tumors of soft tissue in the extremities had one or several features of these characteristics (irregular margin,big volume,edem around masses,aggressive behavior with adjoining fascia) on 3.0T MRI,they had more possibility to be malignant.T2-weighted fat suppression series on 3.0TMRI was very important for discrimination of tumor histological characteristics.
