Abstract:Abstract Background Ventilator-induced diaphragm dysfunction (VIDD) is a common complication of life support by mechanical ventilation observed in critical patients in clinical practice and may predispose patients to severe complications such as ventilator-associated pneumonia or ventilator discontinuation failure. To date, the alterations in microRNA (miRNA) expression in the rat diaphragm in a VIDD model have not been elucidated. This study was designed to identify these alterations in expression. Results Adult male Wistar rats received conventional controlled mechanical ventilation (CMV) or breathed spontaneously for 12 h. Then, their diaphragm tissues were collected for RNA extraction. The miRNA expression alterations in diaphragm tissue were investigated by high-throughput microRNA-sequencing (miRNA-seq). For targeted mRNA functional analysis, gene ontology (GO) analyses and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analyses were subsequently conducted. qRT-PCR validation and luciferase reporter assays were performed. We successfully constructed a model of ventilator-induced diaphragm dysfunction and identified 38 significantly differentially expressed (DE) miRNAs, among which 22 miRNAs were upregulated and 16 were downregulated. GO analyses identified functional genes, and KEGG pathway analyses revealed the signaling pathways that were most highly correlated, which were the MAPK pathway, FoxO pathway and Autophagy–animal. Luciferase reporter assays showed that STAT3 was a direct target of both miR-92a-1-5p and miR-874-3p and that Trim63 was a direct target of miR-3571. Conclusions The current research supplied novel perspectives on miRNAs in the diaphragm, which may not only be implicated in diaphragm dysfunction pathogenesis but could also be considered as therapeutic targets in diaphragm dysfunction.

Abstract:Background Bronchopulmonary dysplasia is a chronic lung disease of premature neonates characterized by arrested pulmonary alveolar development. There is increasing evidence that microRNAs (miRNAs) regulate translation of messenger RNAs (mRNAs) during lung organogenesis. The potential role of miRNAs in the pathogenesis of BPD is unclear. Results Following exposure of neonatal mice to 80% O 2 or room air (RA) for either 14 or 29 days, lungs of hyperoxic mice displayed histological changes consistent with BPD. Comprehensive miRNA and mRNA profiling was performed using lung tissue from both O 2 and RA treated mice, identifying a number of dynamically regulated miRNAs and associated mRNA target genes. Gene ontology enrichment and pathway analysis revealed that hyperoxia modulated genes involved in a variety of lung developmental processes, including cell cycle, cell adhesion, mobility and taxis, inflammation, and angiogenesis. MiR-29 was prominently increased in the lungs of hyperoxic mice, and several predicted mRNA targets of miR-29 were validated with real-time PCR, western blotting and immunohistochemistry. Direct miR-29 targets were further validated in vitro using bronchoalveolar stem cells. Conclusion In newborn mice, prolonged hyperoxia induces an arrest of alveolar development similar to that seen in human neonates with BPD. This abnormal lung development is accompanied by significant increases in the levels of multiple miRNAs and corresponding decreases in the levels of predicted mRNA targets, many of which have known or suspected roles in pathways altered in BPD. These data support the hypothesis that dynamic regulation of miRNAs plays a prominent role in the pathophysiology of BPD.

Abstract:Abstract Background Ventilator-induced diaphragmatic dysfunction (VIDD) is associated with weaning difficulties, intensive care unit hospitalization (ICU), infant mortality, and poor long-term clinical outcomes. The expression patterns of long noncoding RNAs (lncRNAs) and mRNAs in the diaphragm in a rat controlled mechanical ventilation (CMV) model, however, remain to be investigated. Results The diaphragms of five male Wistar rats in a CMV group and five control Wistar rats were used to explore lncRNA and mRNA expression profiles by RNA-sequencing (RNA-seq). Muscle force measurements and immunofluorescence (IF) staining were used to verify the successful establishment of the CMV model. A total of 906 differentially expressed (DE) lncRNAs and 2,139 DE mRNAs were found in the CMV group. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were performed to determine the biological functions or pathways of these DE mRNAs. Our results revealed that these DE mRNAs were related mainly related to complement and coagulation cascades, the PPAR signaling pathway, cholesterol metabolism, cytokine-cytokine receptor interaction, and the AMPK signaling pathway. Some DE lncRNAs and DE mRNAs determined by RNA-seq were validated by quantitative real-time polymerase chain reaction (qRT-PCR), which exhibited trends similar to those observed by RNA-sEq. Co-expression network analysis indicated that three selected muscle atrophy-related mRNAs ( Myog , Trim63 , and Fbxo32 ) were coexpressed with relatively newly discovered DE lncRNAs. Conclusions This study provides a novel perspective on the molecular mechanism of DE lncRNAs and mRNAs in a CMV model, and indicates that the inflammatory signaling pathway and lipid metabolism may play important roles in the pathophysiological mechanism and progression of VIDD.

Abstract:ObjectivesExtracorporeal membrane oxygenation (ECMO) has become an important life support technique during lung transplantation. We aimed to develop a rat model for lung transplantation using venoarterial (VA) ECMO support.MethodsAdult male Sprague-Dawley rats weighing 400 to 450 g were used in this study. ECMO circuits were created by obtaining venous access from the femoral vein with subsequent extracorporeal oxygen exchange, which was then returned to the circulatory system through the left carotid artery (ie, VA-ECMO). Simultaneously, the donor lungs were retrieved and immersed in cold, low-potassium dextran lung preservation solution. Orthotopic left lung transplantation supported by VA-ECMO was performed. Thereafter, a respiratory failure rat model was constructed using ventilation with a hypoxic and hypercapnic gas mixture, consisting of 6% oxygen, 8% carbon dioxide, and 86% nitrogen, before lung transplantation. Similarly, left lung transplantation supported by VA-ECMO was performed in rats with respiratory failure. Arterial blood gas levels were measured at designated time points throughout the experiment.ResultsWe found that VA-ECMO provided sufficient oxygenation and carbon dioxide removal to allow for smooth left lung transplantation in healthy rats and those with respiratory failure.ConclusionsWe established a rat model for lung transplantation using VA-ECMO. Left lung transplantation using VA-ECMO support is also feasible and safe in rat models of respiratory failure. These models provide efficient and economical models for translational medicine for lung transplantation using ECMO. Moreover, it will be invaluable to evaluate the physiological and pathophysiological roles of ECMO during lung transplantation.
Expression of macrophage inflammatory protein-1α in alveolar macrophage in rat subjecting mechanical ventilation

Abstract:Objective To explore the role of alveolar macrophage(AM)activation in ventilator induced lung injury(VILI)by measuring macrophage inflammatory protein-1α(MIP-1α)and nuclear factorkappa B(NF-κB)p65 expressed in AM of rats with different tidal volume ventilations.Methods Thirty-two male Wistar rats were randomly divided into four groups:control group,low tidal volume group,conventional tidal volume group and high tidal volume group.The levels of MIP-1α and NF-κB p65 expressed in AM in BALF were measured by SABC method respectively and the ultrastructures of AM were observed with 100-CX transmission electron microscope.Results The percentages of AM in BALF stained positively with MIP-1α and NF-κB p65 both in high and conventional tidal volume groups were significantly higher than those in control and low tidal volume groups(P＜0.01).The percentages of AM in BALF stained positively with MIP-1α and NF-κB p65 in high tidal volume group were also significantly higher than those in conventional tidal volume group(P＜0.05),but no statistical differences existed between low tidal volume group and control group.Under transmission electron microscope the AM in high and conventional tidal vol ume groups appeared in active state.Conclusions AM is one of the starting cells to inducing VILI and plays an important role in VILI.AM activation and releasing MIP-1α is one of the main reasons leading to neutrophils aggregated and activated in the lungs and so to developing VILI.AM expression and releasing MIP-1α may be regulated by NF-κB to some extent.
Identification of Rat Lung-Specific Micrornas by Microrna Microarray: Valuable Discoveries for the Facilitation of Lung Research

Abstract:BACKGROUND:An important mechanism for gene regulation utilizes small non-coding RNAs called microRNAs (miRNAs). These small RNAs play important roles in tissue development, cell differentiation and proliferation, lipid and fat metabolism, stem cells, exocytosis, diseases and cancers. To date, relatively little is known about functions of miRNAs in the lung except lung cancer.RESULTS:In this study, we utilized a rat miRNA microarray containing 216 miRNA probes, printed in-house, to detect the expression of miRNAs in the rat lung compared to the rat heart, brain, liver, kidney and spleen. Statistical analysis using Significant Analysis of Microarray (SAM) and Tukey Honestly Significant Difference (HSD) revealed 2 miRNAs (miR-195 and miR-200c) expressed specifically in the lung and 9 miRNAs co-expressed in the lung and another organ. 12 selected miRNAs were verified by Northern blot analysis.CONCLUSION:The identified lung-specific miRNAs from this work will facilitate functional studies of miRNAs during normal physiological and pathophysiological processes of the lung.
MicroRNA and mRNA expression profiling in rat acute respiratory distress syndrome

Abstract:Background Acute respiratory distress syndrome (ARDS) is characterized by pulmonary epithelial injury and extensive inflammation of the pulmonary parenchyma. Systematic analyses of microRNA (miRNA) and mRNA expression profiling in ARDS provide insights into understanding of molecular mechanisms of the pathogenesis of ARDS. The objective of this study was to identify miRNA and mRNA interactions in a rat model of ARDS by combining miRNA and mRNA microarray analyses. Methods Rat model of ARDS was induced by saline lavage and mechanical ventilation. The expression profiles of both mRNAs and miRNAs in rat ARDS model were performed by microarray analyses. Microarray data were further verified by quantitative RT-PCR. Functional annotation on dys-regulated mRNAs and miRNAs was carried out by bioinformatics analysis. Results The expression of 27 miRNAs and 37 mRNAs were found to be significantly changed. The selected miRNAs and genes were further verified by quantitative real-time PCR. The down-regulated miRNAs included miR-24, miR-26a, miR-126, and Let-7a, b, c, f. The up-regulated miRNAs were composed of miR-344, miR-346, miR-99a, miR-127, miR-128b, miR-135b, and miR-30a/b. Gene ontology and functional annotation analyses indicated that up-regulated mRNAs, such as Apc, Timp1, and Sod2, were involved in the regulation of apoptosis. Bioinformatics analysis showed the inverse correlation of altered miRNAs with the expression of their predicted target mRNAs. While Sod2 was inversely correlated with Let-7a, b, c, f., Ebf1 and Apc were inversely correlated with miR-24 and miR-26a, respectively. miR-26a, miR-346, miR-135b, miR-30a/b, miR-344, and miR-18a targeted multiple altered mRNAs. Gabrb1, Sod2, Eif2ak1, Fbln5, and Tspan8 were targeted by multiple altered miRNAs. Conclusion The expressions of miRNAs and mRNAs were altered in a rat model of ARDS. The identified miRNA-mRNA pairs may play critical roles in the pathogenesis of ARDS.
Expression of Matrix Metalloproteinases in Lung Tissue in a Rat Model of Ventilator-Induced Lung Injury

Abstract:Objective To investigate the balance between matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMPs) in the lung tissue in a rat model of ventilator-induced lung injury. Methods Thirty healthy male SD rats weighing 200-250g were randomly divided into 3 groups( n = 10 each) : group A spontaneous breathing; group B small tidal volume (VT = 7 ml·kg-1, RR = 40 bpm, FiO2=0.21) and group C large tidal volume (VT = 40 ml·kg-1, RR = 20 bpm, FiO2=0.21). The animals were mechanically ventilated for 4 h in group B and C. The lung injury was assessed by measurement of PaO2 /FiO2 . At the end of the experiment the animals were killed and the lungs were removed. The right lung was used for determination of the expression of mRNAs of MMP-2, MMP-9, TIMP-1 and TTMP-2 using RT-PCR and histologic examination. The left lung was weighed and then lavaged. The broncho-alveolar lavage fluid (BALF) was collected for total WBC and neutrophil counts and determination of total protein content and gelatinase activity. After lavage the left lung was heated at 60℃. The W/D lung weight ratio was calculated. Results PaO2/FiO2 was significantly lower after 4h mechanical ventilation in group C than in group A and B. The total WBC count and total protein content in BALF were significantly higher in group C than in group A and B. The W/D ratio of the left lung was significantly higher in group C than in group A and B. Microscopic examination showed that there were marked WBC infiltration and destructive changes of alveolar walls in group C. The levels of gelatinase (MMP-2 and MMP-9) activities in BALF were significantly higher in group C than in group A and B. The expression of the mRNA of MMP-2 and MMP-9 was significantly higher in group C than in group A and B, but there was no significant difference in the expression of the mRNA of TIMP-1 and TIMP-2 between group C and group A and B. Conclusion Large tidal volume ventilation can induce acute lung injury. MMP-2 and MMP-9 play an important role in the development of ventilator induced lung injury and the imbalance between MMPs and TIMPs contributes to the mechanism of ventilator-induced lung injury.
Expression of Micrornas in Lung Homogenates in Rats with Chronic Obstructive Pulmonary Disease

Abstract:Objective To determine the expression spectrum of microRNA(miRNA)in the lung homogenate of rats with chronic obstructive pulmonary disease(COPD)or healthy rats,to look for differentially expressed miRNA, and to explore their potential role in pathogenesis of COPD. Methods Twenty male Sprague-Dawley(SD)rats were randomly divided into normal control group and COPD model group,with 10 rats in each group. COPD rat models were reproduced by smoke inhalation as well as intratracheal instillation of lipopolysaccharide(LPS). The samples of the lung were harvested,and the histopathological examination of the right lung was carried out to evaluate the degree of lung injury. Total RNAs were isolated from the left lung. The miRNA expressions in lung tissue of rats with COPD or normal rats were determined by miRNA chip eichnology to screen the miRNA with differential expression, and hierarchical clustering analysis was made. The data were analyzed to study the expression difference of miRNAs between the two groups,and to construct the miRNA-Target network. Results Compared with normal control group, 20 miRNA with increased expression were found in COPD model group,the top 10 were miR-30c-2,miR-199a-5p, miR-30a, miR-145, miR-151, miR-674-5p, miR-214, miR-423, miR-28 and miR-181b, and only miR-376b-3p showed down-regulation. Hierarchical clustering analysis showed that significant differences in individual miRNA in lung tissue between rats of two groups were found. However,similar samples could not be gathered well,thus it may give rise to inconsistency during the sample preparation. Many miRNAs had multiple target genes in miRNA-Target network, such as miR-30c-2, miR-145, miR-181b, miR-181a, miR-181d, miR-199. Conclusions The study reveals that there is a variance of miRNAs expression profile in lung homogenates between COPD group and the normal control group of rats,and many miRNAs have multiple target genes,such as miR-30c-2,miR-145,miR-181b,miR-181a,miR-181d,and miR-199. Therefore,the results may be valuable to centain extent for the interpretation of COPD pathogenesis.
Differential Expression Profile of microRNAs and Tight Junction in the Lung Tissues of Rat With Mitomycin-C-Induced Pulmonary Veno-Occlusive Disease

Abstract:Background Pulmonary veno-occlusive disease (PVOD) is characterized by increased pulmonary vascular resistance. Currently, there is a lack of effective treatment. It is of great significance to explore molecular targets for treatment. This study investigated the differential expression profile of miRNAs and tight junction in the lung tissues of rats with mitomycin-C (MMC)-induced PVOD. Methods A total of 14 rats were divided into the control group and he PVOD group. We measured mean pulmonary arterial pressure (mPAP) and right ventricular hypertrophy index (RVHI). Pathological changes including those in lung tissues, pulmonary venules, and capillary were detected by H&E and orcein staining. Western blot was used to detect GCN2, ZO-1, occludin, and claudin-5 expression. We analyzed the miRNAs profile in the rat lung tissues by high-throughput sequencing. The top differentially expressed miRNAs were validated by using real-time polymerase chain reaction (RT-PCR). Results There were severe pulmonary artery hypertrophy/hyperplasia, thickening, and occlusion in the small pulmonary veins, pulmonary edema, and dilated capillaries in MMC-induced rats with PVOD. In addition, mPAP and RVHI were significantly increased ( P < 0.05). The expression of GCN2 was significantly decreased ( P < 0.05). A total of 106 differentially expressed miRNAs were identified. According to the fold changes, the top ten upregulated miRNAs were miRNA-543-3p, miRNA-802-5p, miRNA-493-3p, miRNA-539-3p, miRNA-495, miRNA-380-5p, miRNA-214-5p, miRNA-539-5p, miRNA-190a-3p, and miRNA-431. The top 10 downregulated miRNAs were miRNA-201-3p, miRNA-141-3p, miRNA-1912-3p, miRNA-500-5p, miRNA-3585-5p, miRNA-448-3p, miRNA-509-5p, miRNA-3585-3p, miRNA-449c-5p, and miRNA-509-3p. RT-PCR confirmed that miRNA-214-5p was upregulated, while miRNA-141-3p was downregulated ( P < 0.05). Functional analysis showed various signaling pathways and metabolic processes, such as fatty acid biosynthesis, tight junction, and the mTOR signaling pathway. In addition, the expression of the tight junction-related protein of ZO-1, occludin, and claudin-5 was significantly decreased in rats with PVOD ( P < 0.05). Conclusion miRNAs may be involved in the pathogenesis of PVOD. Furthermore, ZO-1, occludin, and claudin-5 verification confirmed that the tight junction may be involved in the development of the disease.

Effect of High Volume Mechanical Ventilation on Acute Lung Injury in Rats in vivo

Abstract:Objective To establish the model of ventilator-induced lung injury(VILI)in rats and investigate the pathophysiological changes and the underlying mechanisms.Methods Forty Sprague-Dawley rats were randomly divided into the following experimental groups(10 rats in each group):(1)group A:rats were unventilated;(2)group B:rats were ventilated with 40 mL/kg tidal volume room air for one h;(3)group C:rats were ventilated with 40 mL/kg tidal volume room air for two h;(4)group D:rats were ventilated with 40 mL/kg tidal volume room air for four h.The samples of pulmonary tissue and lung lavage fluid were collected after experiments.Apoptosis of the lung cells was assayed by using terminal deoxynucleodityl transferase-mediated nick-end labeling(TUNEL)method.NF-κB activity was assayed by using EMSA.NF-κB P65 was detected by Western blot.Lung pathological changes were examined under an optical microscopy.Total protein,wet/dry ratios(W/D),MDA,SOD and myeloperoxidase(MPO)activity,and counts of white blood cells(WBC)in the lung tissue or lavage fluid were measured.Results After mechanical ventilation,groups B,C and D showed high levels of intra-alveolar exudates,hyaline membrane formation,inflammatory cell infiltration,intra-alveolar hemorrhage and interstitial edema.The degree of ALI in groups B,C,and D was remarkably aggravated with the duration of mechanical ventilation.As compared with group A,ALI scores,W/D ratios,MPO,total proteins,WBC counts and MDA were significantly elevated in groups B,C and D(all P<0.01),and remarkably increased with the duration of mechanical ventilation(all P<0.05).As compared with group A,SOD in groups B,C,and D was significantly decreased(P<0.01).As compared with group B,SOD in group C had no remarkable difference but group D showed a significant decreases(P<0.05).As compared with group C,group D showed a significant decrease in SOD(P<0.05).As compared with group A,NF-κB activity and NF-κB P65 in groups B,C and D were significantly elevated(P<0.01),and remarkably increased with the duration of mechanical ventilation(P<0.05).Conclusion High volume ventilator produced significant pathological changes of ALI in rats,which includes lung edema,leukocytic infiltration,acute inflammatory reaction,oxidative stress and extensive lung cell apoptosis.

Abstract:Objective To evaluate the effects of mechanical ventilation preconditioning on the expression of Rho-associated kinase 1 (ROCK1) in the lung tissue in a rat model of ventilator-induced lung injury.Methods Forty-eight male Sprague-Dawley, aged 8-12 weeks, weighing 250-300 g, were randomly assigned into 6 groups (n =8 each) using a random number table: control group (group C);normal mechanical ventilation group (group N);mechanical ventilation with large tidal volume (VT) group (group L);preconditioning with mechanical ventilation with different VT groups (P1, P2 and P3 groups).The animals were anesthetized with intraperitoneal pentobarbital sodium, and tracheostomized.In group C, the animals kept spontaneous breathing after intubation.In group N, the animals were mechanically ventilated for 3 h with the VT of 8-10 ml/kg.In group L, the animals were mechanically ventilated for 3 h with the VT of 40 ml/kg.In P1, P2 and P3 groups, the animals were mechanically ventilated for 30 min with the VT of 6-7, 20 and 30 ml/kg, respectively, and then were mechanically ventilated for 3 h with the VT of 40 ml/kg.The respiratory rate was 40 breaths/min, and inspiratory/expiratory ratio was 1 : 1.The animals were sacrificed at the end of ventilation.Broncho-alveolar lavage fluid was collected, and pulmonary permeability index (PPI) was calculated.Lungs were removed for examination of pathological changes which were scored,and for detection of the wet to dry lung weight ratio (W/D ratio), apoptosis in alveolar epithelial cells, and expression of ROCK1 (by immunohistochemistry and Western blot).Apoptosis index (AI) was calculated.Results Compared with group C, the pathological scores, W/D ratio, PPI and AI were significantly increased, and the expression of ROCK1 was up-regulated in L, P1, P2 and P3 groups (P＜0.05) , and no significant change was found in the parameters mentioned above in group N (P＞0.05).Compared with group L, the pathological scores, W/D ratio, PPI and AI were significantly decreased, and the expression of ROCK1 was down-regulated in group P1 (P＜0.05) , and no significant change was found in the parameters mentioned above in P2 and P3 groups (P＞0.05).Conclusion The mechanism by which mechanical ventilation preconditioning decreases ventilator-induced lung injury may be related to inhibition of the expression of ROCK1 in lung tissues of rats.
The changes of microRNAs expression in rat cardiac hypertrophy

Abstract:Objective:To analyze the changes of microRNAs(miRNAs) expression in rat cardiac hypertrophy.Methods:To understand the roles of miRNAs in hypertrophy,a rat model of cardiac hypertrophy was established with abdominal aortic constriction(AAC).Real-time quantitative PCR was used to detect the changes of miRNAs expression in myocardium.Results:The heart weight/body weight,Nppa and myh7,the coding genes of atrial natriuretic peptide and β-myosin heavy chain(which were the markers of cardiac hypertrophy) were significant up-regulated on week 1 after AAC,it was meant the success of cardiac hypertrophy model.One week after aortic banding,miR-1,miR-133,miR-181a and miR-499 were significantly down-regulated,whereas miR-199a was up-regulated.Four weeks after AAC,miR-21,miR-24 and miR-214 were up-regulated,whereas miR-181a was significantly down-regulated,while miR-23a,miR-133,miR-145,miR-199a and miR-499 were unchanged.Conclusions:The expression of miRNAs was changed in rat cardiac hypertrophy,the changes of miRNAs maybe played an important role in regulation of cardiac hypertrophy pathological process.
Non-coding RNA alterations in extracellular vesicles from bronchoalveolar lavage fluid contribute to mechanical ventilation-induced pulmonary fibrosis

Abstract:Objective: For respiratory failure patients, mechanical ventilation (MV) is a life-saving therapy to maintain respiratory function. However, MV could also cause damage to pulmonary structures, result in ventilator-induced lung injury (VILI) and eventually progress to mechanical ventilation-induced pulmonary fibrosis (MVPF). Mechanically ventilated patients with MVPF are closely related to increased mortality and poor quality of life in long-term survival. Thus, a thorough understanding of the involved mechanism is necessary. Methods: We used next-generation sequencing to identify differentially expressed non-coding RNAs (ncRNAs) in BALF EVs which were isolated from Sham and MV mice. Bioinformatics analysis was conducted to identify the engaged ncRNAs and related signaling pathways in the process of MVPF. Results: We found 1801 messenger RNAs (mRNA), 53 micro RNAs (miRNA), 273 circular RNAs (circRNA) and 552 long non-coding RNAs (lncRNA) in mice BALF EVs of two groups, which showed significant differential expression. TargetScan predicted that 53 differentially expressed miRNAs targeted 3105 mRNAs. MiRanda revealed that 273 differentially expressed circRNAs were associated with 241 mRNAs while 552 differentially expressed lncRNAs were predicated to target 20528 mRNAs. GO, KEGG pathway analysis and KOG classification showed that these differentially expressed ncRNA-targeted mRNAs were enriched in fibrosis related signaling pathways and biological processes. By taking the intersection of miRNAs target genes, circRNAs target genes and lncRNAs target genes, we found 24 common key genes and 6 downregulated genes were confirmed by qRT-PCR. Conclusions: Changes in BALF-EV ncRNAs may contribute to MVPF. Identification of key target genes involved in the pathogenesis of MVPF could lead to interventions that slow or reverse fibrosis progression.

Cellular senescence contributes to mechanical ventilation-induced diaphragm dysfunction by upregulating p53 signalling pathways

Abstract:Background: Mechanical ventilation can cause acute atrophy and injury in the diaphragm, which are related to adverse clinical results. However, the underlying mechanisms of ventilation-induced diaphragm dysfunction (VIDD) have not been well elucidated. The current study aimed to explore the role of cellular senescence in VIDD. Methods: A total of twelve New Zealand rabbits were randomly divided into 2 groups: (1) spontaneously breathing anaesthetized animals (the CON group) and (2) mechanically ventilated animals (for 48 h) in V-ACV mode (the MV group). Respiratory parameters were collected during ventilation. Diaphragm were collected for further analyses. Results: Compared to those in the CON group, the percentage and density of sarcomere disruption in the MV group were much higher (p < 0.001, both). The mRNA expression of MAFbx and MuRF1 was upregulated in the MV group (p = 0.003 and p = 0.006, respectively). Compared to that in the CON group, the expression of MAFbx and MuRF1 detected by western blotting was also upregulated (p = 0.02 and p = 0.03, respectively). Moreover, RNA-seq showed that genes associated with senescence were remarkably enriched in the MV group. The mRNA expression of related genes was further verified by q-PCR (Pai1: p = 0.009; MMP9: p = 0.008). Transverse cross-sections of diaphragm myofibrils in the MV group showed more intensive positive staining of SA-βGal than those in the CON group. p53-p21 axis signalling was elevated in the MV group. The mRNA expression of p53 and p21 was significantly upregulated (p = 0.02 and p = 0.05, respectively). The western blot results also showed upregulation of p53 and p21 protein expression (p = 0.03 and p = 0.05, respectively). Moreover, the p21-positive staining in immunofluorescence and immunohistochemistry in the MV group was much more intense than that in the CON group (p < 0.001, both). Conclusions: In a rabbit model, we demonstrated that mechanical ventilation in A/C mode for 48 h can still significantly induce ultrastructural damage and atrophy of the diaphragm. Moreover, p53-dependent senescence might play a role in mechanical ventilation-induced dysfunction. These findings might provide novel therapeutic targets for VIDD.
Effects of Mechanical Ventilation on the Expression of Toll Like Receptor 4 in Rat's Alveolar Macrophages

Abstract:OBJECTIVE:To investigate the effects of mechanical ventilation on Toll like receptor 4 (TLR4) in rat's alveolar macrophages (AM). METHODS:Eighteen clean grade adult male SD rats weighting 230-240 g were randomly divided into three groups (n=6): autonomous breathing group(R group), small tidal volume (V(T)) mechanical ventilation group (M group, V(T)=6 ml/kg) and large V(T) mechanical ventilation group (N group, V(T)=40 ml/kg). The animals were anesthetized with intraperitoneal 20% urethane 8 ml/kg, tracheostomized and mechanically ventilated [inhalation: exhalation (I:E)=1:1, with positive end-expiratory pressure (PEEP)=0, fraction of inspired oxygen (FiO(2))=0.21]. The respiratory rate was adjusted, and end-tidal carbon dioxide partial pressure (P(ET)CO(2)) was maintained at 35-45 mm Hg (1 mm Hg=0.133 kPa). Arterial blood samples were taken for blood gas analysis at the beginning of the experiment and 1, 2 and 3 hours after the experiment. The experiment was terminated, and pathomorphology score, pulmonary tissue wet/dry (W/D) weight ratio, white blood cells count (WBC) in bronchoalveolar lavage fluid (BALF) were determined at 3 hours. The rats were sacrificed by bloodletting after experiment. Then the rats' BALF and pulmonary albumin permeability (PAP) were determined. The expression of AM TLR4 mRNA was assessed by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR). AM TLR4 protein expression was determined by immunohistochemistry. RESULTS:Over ventilation, elevated pH and lower partial pressure of carbon dioxide in arterial blood (PaCO(2)) were observed in N group at 1 hour, and other indexes were normal. The pulmonary pathomorphology score, pulmonary W/D weight ratio, WBC in BALF, PAP, and the mRNA expression of TLR4 and the protein of TLR4 in AM in the N group were greatly increased as compared with the R group (all P<0.01), while they were not significantly changed in the M group (all P>0.05). CONCLUSION:Large V(T) mechanical ventilation induced injury of lungs and up-regulated TLR4 expression in rat's AM. Small V(T) mechanical ventilation avoided above-mentioned changes.

Abstract:Inflammation is involved in ventilator-induced diaphragm dysfunction. Toll-like receptor 4 (TLR4) is an important inflammatory factor, but it remains unclear whether TLR4 contributes to ventilator-induced diaphragm dysfunction. This study aimed to investigate the role of TLR4 signaling in ventilator-induced diaphragm dysfunction. Total 30 adult male SD rats were randomly divided into control group, low tidal volume and high tidal volume group (n = 10). Control group received tracheotomy and endotracheal intubation but no mechanical ventilation; low tidal volume group and high tidal volume group received tracheotomy, mechanical ventilation after intubation, and then received tidal volume 6 ml/kg and 20 ml/kg, respectively. Ventilation rate was 60 beats/min, inspiratory to expiratory ratio was 1:3, FiO2 was 21%, ventilation was 24 h. Diaphragmatic muscle contraction, tumor necrosis factor (TNF-α) and TLR4 expression, and malondialdehyde (MDA) and superoxide dismutase (SOD) contents in the diaphragm tissues were detected. TLR4 and TNF-α expression in diaphragm tissues of high tidal volume group were significantly increased and diaphragm muscle contraction was significantly decreased, compared to other groups. In conclusion, high tidal volume ventilation may activate TLR4 signaling and cause pathological changes in diaphragm tissues. TLR4 is a promising target for the prevention and treatment of ventilator-associated diaphragmatic injury.
