Abstract:BACKGROUND:Sleep Apnea Syndrome (SAS) is a common sleep-related breathing disorder, which affects about 4-7% males and 2-4% females all around the world. Different approaches have been adopted to diagnose SAS and measure its severity, including the gold standard Polysomnography (PSG) in sleep study field as well as several alternative techniques such as single-channel ECG, pulse oximeter and so on. However, many shortcomings still limit their generalization in home environment. In this study, we aim to propose an efficient approach to automatically assess the severity of sleep apnea syndrome based on the ballistocardiogram (BCG) signal, which is non-intrusive and suitable for in home environment.METHODS:We develop an unobtrusive sleep monitoring system to capture the BCG signals, based on which we put forward a three-stage sleep apnea syndrome severity assessment framework, i.e., data preprocessing, sleep-related breathing events (SBEs) detection, and sleep apnea syndrome severity evaluation. First, in the data preprocessing stage, to overcome the limits of BCG signals (e.g., low precision and reliability), we utilize wavelet decomposition to obtain the outline information of heartbeats, and apply a RR correction algorithm to handle missing or spurious RR intervals. Afterwards, in the event detection stage, we propose an automatic sleep-related breathing event detection algorithm named Physio_ICSS based on the iterative cumulative sums of squares (i.e., the ICSS algorithm), which is originally used to detect structural breakpoints in a time series. In particular, to efficiently detect sleep-related breathing events in the obtained time series of RR intervals, the proposed algorithm not only explores the practical factors of sleep-related breathing events (e.g., the limit of lasting duration and possible occurrence sleep stages) but also overcomes the event segmentation issue (e.g., equal-length segmentation method might divide one sleep-related breathing event into different fragments and lead to incorrect results) of existing approaches. Finally, by fusing features extracted from multiple domains, we can identify sleep-related breathing events and assess the severity level of sleep apnea syndrome effectively.CONCLUSIONS:Experimental results on 136 individuals of different sleep apnea syndrome severities validate the effectiveness of the proposed framework, with the accuracy of 94.12% (128/136).
Analyzing Sleep Stages In Home Environment Based On Ballistocardiography

Abstract:Currently, a number of people have various sleep disorders, and sleep stages play an important role in assessment of sleep quality and health status. This paper proposes an effective approach of analyzing the sleep stages based on ballistocardiography (BCG), which can be continuously detected with micro-movement sensitive mattress (MSM) in this work, during non-intrusive sleep in home environment. This paper focuses on extracting features from BCG from the following three aspects: multi-resolution wavelet analysis of the heartbeat intervals based time-domain features, Welch's power spectrum estimation based frequency-domain features and the detrended fluctuation analysis (DFA) value for long term correlation based features. Moreover, the support vector machine (SVM) with or without the factor of sleep rhythm, and recurrent neural network (RNN) are adopted to build the classifiers, and both the personal model and self-independent model are investigated for different scenarios. Experimental result of 56 subjects [25 women and 31 men, aged from 16 to 71] was evaluated applying the proposed method and compared to the result provided by professional visual scoring by ECG and EEG. The SVM with the factor of sleep rhythm shows better performance with an average accuracy between 73.21%similar to 83.94% in the personal model, and the self-independent model also achieves a satisfactory level with an average accuracy of 73.611 similar to 78.78% for male and 73.99%similar to 79.46% for female.

Abstract:A practical way of detecting sleep stages has become more necessary as we begin to learn about the vast effects that sleep has on people's lives. The current methods of sleep stage detection are expensive, invasive to a person's sleep, and not practical in a modern home setting. While the method of detecting sleep stages via the monitoring of brain activity, muscle activity, and eye movement, through electroencephalogram in a lab setting, provide the gold standard for detection, this paper aims to investigate a new method that will allow a person to gain similar insight and results with no obtrusion to their normal sleeping habits. Ballistocardiography (BCG) is a non-invasive sensing technology that collects information by measuring the ballistic forces generated by the heart. Using features extracted from BCG such as time of usage, heart rate, respiration rate, relative stroke volume, and heart rate variability, we propose to implement a sleep stage detection algorithm and compare it against sleep stages extracted from a Fitbit Sense Smart Watch. The accessibility, ease of use, and relatively-low cost of the BCG offers many applications and advantages for using this device. By standardizing this device, people will be able to benefit from the BCG in analyzing their own sleep patterns and draw conclusions on their sleep efficiency. This work demonstrates the feasibility of using BCG for an accurate and non-invasive sleep monitoring method that can be set up in the comfort of a one's personal sleep environment.
SleepABP: Noninvasive Ambulatory Blood Pressure Monitoring Based on Ballistocardiogram in Sleep State

Abstract:Blood pressure (BP) is one of the vital signssignals used to evaluate the health status of the human body, a. And continuous ambulatory blood pressure (ABP) monitoring can accurately and comprehensively reflect the physiological status of the cardiovascular system. ABP is mainly measured using ambulatory sphygmomanometer, by regularly collecting the pressure changes in blood vessels. However, this method is usually intrusive, inconvenient, and expensive. In this paper, we develop SleepABP, a novel natural, continuous and accurate ABP monitoring system for daily sleep scenarios. SleepABP is based on the Ballistocardiogram (BCG) signal, which relies on measuring the tiny fluctuations (impulses) in the body caused by the continuous beating of the heart. Specifically, the proposed BCG sensing system monitors the pulses from the user's head applying the piezoelectric ceramic sensor. The BCG signals are recorded simultaneously, and the BCG cycle restoration algorithm is used to extract the continuous and complete BCG cycles for constructing the multi-dimensional features of the blood pressure prediction. SleepABP was evaluated on 16 participants under various sleeping conditions, positions, and personal internal factors. The RMSE results of diastolic blood pressure 3.22 mmHg and systolic blood pressure 3.5 mmHg demonstrates the system's effectiveness and feasibility in a daily sleeping environment.
Identifying Sleep Apnea Syndrome Using Heart Rate And Breathing Effort Variation Analysis Based On Ballistocardiography

Abstract:Sleep apnea syndrome (SAS) is regarded as one of the most common sleep-related breathing disorders, which can severely affect sleep quality. Since SAS is usually accompanied with the cyclical heart rate variation (HRV), many studies have been conducted on heart rate (HR) to identify it at an earlier stage. While most related work mainly based on clinical devices or signals (e. g., polysomnography (PSG), electrocardiography (ECG)), in this paper we focus on the ballistocardiographic (BCG) signal which is obtained in a non-invasive way. Moreover, as the precision and reliability of BCG signal are not so good as PSG or ECG, we propose a fine-grained feature extraction and analysis approach in SAS recognition. Our analysis takes both the basic HRV features and the breathing effort variation into consideration during different sleep stages rather than the whole night. The breathing effort refers to the mechanical interaction between respiration and BCG signal when SAS events occur, which is independent from autonomous nervous system (ANS) modulations. Specifically, a novel method named STC-Min is presented to extract the breathing effort variation feature. The basic HRV features depict the ANS modulations on HR and Sample Entropy and Detrended Fluctuation Analysis are applied for the evaluations. All the extracted features along with personal factors are fed into the knowledge-based support vector machine (KSVM) classification model, and the prior knowledge is based on dataset distribution and domain knowledge. Experimental results on 42 subjects in 3 nights validate the effectiveness of the methods and features in identifying SAS (90.46% precision rate and 88.89% recall rate).

Abstract:Across the world, healthcare costs are projected to continue to increase, and the pressure on the healthcare system is only going to grow in intensity as the rate of growth of elderly population increases in the coming decades. As an example, when people age one possible condition that they may experience is sleep-disordered breathing (SDB). SDB, better known as the obstructive sleep apnea (OSA) syndrome, and associated cardiovascular complications are among the most common clinical disorders. The gold-standard approach to accurately diagnose OSA, is polysomnography (PSG), a test that should be performed in a specialist sleep clinic and requires a complete overnight stay at the clinic. The PSG system can provide accurate and real-time data; however, it introduces several challenges such as complexity, invasiveness, excessive cost, and absence of privacy. Technological advancements in hardware and software enable noninvasive and unobtrusive sensing of vital signs. An alternative approach which may help diagnose OSA and other cardiovascular diseases is the ballistocardiography. The ballistocardiogram (BCG) signal captures the ballistic forces of the heart caused by the sudden ejection of blood into the great vessels with each heartbeat, breathing, and body movement. In recent years, BCG sensors such as polyvinylidene fluoride film-based sensors, electromechanical films, strain Gauges, hydraulic sensors, microbend fiber-optic sensors as well as fiber Bragg grating sensors have been integrated within ambient locations such as mattresses, pillows, chairs, beds, or even weighing scales, to capture BCG signals, and thereby measure vital signs. Analysis of the BCG signal is a challenging process, despite being a more convenient and comfortable method of vital signs monitoring. In practice, BCG sensors are placed under bed mattresses for sleep tracking, and hence several factors, e.g., mattress thickness, body movements, motion artifacts, bed-partners, etc. can deteriorate the signal. In this paper, we introduce the sensors that are being used for obtaining BCG signals. We also present an in-depth review of the signal processing methods as applied to the various sensors, to analyze the BCG signal and extract physiological parameters such heart rate and breathing rate, as well as determining sleep stages. Besides, we recommend which methods are more suitable for processing BCG signals due to their nonlinear and nonstationary characteristics.

Abstract:Monitoring sleep quality and status is important to learn health condition for improvement and prevent sleep apnea. Sleep stage classification based on Ballistocardiography (BCG) has attracted more attention due to its simplicity in equipment usage and high positioning accuracy. Firstly, this paper tries to reconstruct Electrocardiogram (ECG) signals from BCG signals using an improved Deep Convolutional Generative Adversarial Networks (DCGAN) model. Then an optimal Support Vector Machine (SVM) model is exploited for sleep stage classification, in which a Particle Swarm Optimization (PSO) is combined with a Genetic Algorithm (GA) to train the classifier. Finally, the accuracy of four-stage and six-stage SVM models are analyzed and compared using Heart Rate Variability (HRV), incorporating HRV and Respiratory Variability (RV) features. The results show that the accuracy of the four-stage and six-stage SVM models using RV features for sleep stage classification reaches 76.38% and 71.11%, respectively.

Abstract:Sleeping posture is an important factor in the quality of sleep. In this paper, we present a study on the feasibility of the automatic detection of personal sleeping posture from ballistocardiograms (BCGs) recorded by unobtrusive loadcell-embedded mattress. The proposed system is intended as a screening and monitoring tool in home-healthcare applications and provides reference for patients with chronic disease to adjust sleeping pattern. This work extracts the features of a heartbeat waveform pattern obtained from BCG and use the Random Forest classifier to classify various sleeping postures. The experimental results of this work indicate that the proposed algorithm can achieve high accuracy when sleep is stable and in the stage of sleep instability it can also make most of correct decisions about sleeping posture.

Abstract:We present a ballistocardiogram(BCG)measurement system for obtaining the BCG signal.The performance of the BCG measurement system was tested,including the dynamic response frequency of the platform(32 Hz with a 75 kg load),and the signal to noise ratio of the system(minimum 23 dB).Factors that could influence BCG recording during measurement were investigated by collecting data from 48 healthy subjects(28 males and 20females).The basic features of the BCG signal,such as the amplitude and time interval,were extracted,and differences by gender were analyzed.A correlation between the IJ amplitude(amplitude difference between I-and J-peak)and the respiration phase was found(R2=0.49).The BCG signals for both sitting(SiBCG)and standing(StBCG)postures were obtained using this system.These showed that the StBCG amplitudes were larger than those of SiBCG but there was no significant difference in the time intervals.The robustness of our system was verified,and its potential for non-invasive cardiac activity monitoring was demonstrated.
Automatic Detection of Atrial Fibrillation from Ballistocardiogram (BCG) Using Wavelet Features and Machine Learning.

Abstract:This paper presents an unobtrusive method for automatic detection of atrial fibrillation (AF) from single-channel ballistocardiogram (BCG) recordings during sleep. We developed a remote data acquisition system that measures BCG signals through an electromechanical-film sensor embedded into a bed's mattress and transmits the BCG data to a remote database on the cloud server. In the feasibility study, 12 AF patients' data were recorded during entire night of sleep. Each BCG recording was split into nonoverlapping 30s epochs labeled either AF or normal. Using the features extracted from stationary wavelet transform of these epochs, three popular machine learning classifiers (support vector machine, K-nearest neighbor, and ensembles) have been trained and evaluated on the set of 7816 epochs employing 30% hold-out validation. The results showed that all the trained classifiers could achieve an accuracy rate above 91.5%. The optimized ensembles model (Bagged Trees) could achieve accuracy, sensitivity, and specificity of 0.944, 0.970 and 0.891, respectively. These results suggest that the proposed BCG-based AF detection can be a potential initial screening and detection tool of AF in home-monitoring applications.

Abstract:To facilitate unobtrusive and continuous sleep monitoring and promote intelligent sleep quality assessment, we present a dataset that includes multiple nights of continuous ballistocardiogram (BCG) data collected using piezoelectric film sensors from 32 subjects in their regular sleep environments. Besides, the referenced heart rate and respiratory data are also recorded by reference sensors to validate the accuracy of the cardiac and respiratory components extracted from the BCG signals. The dataset serves as a foundation for research on unobtrusive vital sign monitoring based on BCG signals, offering data support for the evaluation and optimization of sleep quality.

Identifying Obstructive Sleep Apnea by Exploiting Fine-Grained BCG Features Based on Event Phase Segmentation

Abstract:Obstructive sleep apnea (OSA) is regarded as one of the most common sleep-related breathing disorders, which causes various diseases and affects people's daily life severely. Up to now, massive efforts have been devoted to identifying OSA events during sleep based on different signals (e.g., PSG, ECG, nasal airflow and EMG, etc.). However, there still are more or less shortcomings in current studies. In this paper, we propose a novel framework to improve the performance of identifying OSA events. Particularly, the key idea of our framework is to divide each potential event segment (i.e., a data segment that may or may not contain an OSA event) into different phases, from which we further extract fine-grained features to characterize respiratory pattern comprehensively. Concretely, we first automatically locate potential event segments from raw ballistocardiography (BCG) data by identifying arousals. Afterwards, each potential event segment is divided into three phases (i.e., Apnea Phase, Respiratory Effort Phase and Arousal Phase) by an adaptive threshold-based division algorithm. Based on these phases, we further extract and select efficient features that can characterize respiratory pattern from different aspects. Finally, these potential event segments are classified into OSA events or non-OSA events using BP neural network. Experimental results based on a real BCG dataset that contains 3,790 OSA events and 2,556 non-OSA events show that our framework outperforms the baselines and the precision, recall and AUC reach 94.6%, 93.1%, and 0.951, respectively.

Abstract:Sleep state classification is vital in managing and understanding sleep patterns and is generally the first step in identifying acute or chronic sleep disorders. However, it is essential to do this without affecting the natural environment or conditions of the subject during their sleep. Techniques such as Polysomnography(PSG) are obtrusive and are not convenient for regular sleep monitoring. Fortunately, The rise of novel technologies and advanced computing has given a recent resurgence to monitoring sleep techniques. One such contactless and unobtrusive monitoring technique is Ballistocradiography(BCG), in which vitals are monitored by measuring the body's reaction to the cardiac ejection of blood. In this study, we propose a Multi-Head 1D-Convolution based Deep Neural Network to classify sleep-wake state and predict sleep-wake time accurately using the signals coming from a BCG sensor. Our method achieves a sleep-wake classification score of 95.5%, which is on par with researches based on the PSG system. We further conducted two independent studies in a controlled and uncontrolled environment to test the sleep-wake prediction accuracy. We achieve a score of 94.16% in a controlled environment on 115 subjects and 94.90% in an uncontrolled environment on 350 subjects. The high accuracy and contactless nature of the proposed system make it a convenient method for long term monitoring of sleep states.

Abstract:A ballistocardiogram (BCG)unobtrusively records the weight variation caused by the reaction force of blood ejection and blood flow in the aorta. A BCG system was developed to record BCG signals and detect the heart beats in the BCG signal. Data from 24 healthy subjects was collected to investigate the system with simultaneous electrocardiograms(ECG)as a reference. The results show no significant difference between the heartbeats detected by the ECG and BCG system (p =0. 95)with the BCG system having a sensitivity of 99. 56％ and an accuracy of 99. 77％.

Abstract:Ballistocardiogram (BCG) could reflect mechanical activity of cardiovascular system instead of ECG. And it is often acquired by sensitive mattress or chair without any constraints and limitations, but it contains many noise because of the impact of body and acquired equipment, those questions make heart rate detection difficult from the original BCG. In the paper, we propose an adaptive method which is used to extract heartbeat intervals (RR), and the method acquire automatically input parameters of Ensemble Empirical Mode Decomposition (EEMD) algorithm, and then decompose BCG signal using EEMD algorithm, and select adaptively decomposition component of BCG signal, whose periodicity is in accordance with the cardiac cycle completely as the target signal. Furthermore we detect the peak points and calculate the heartbeat intervals series using the target signal. In the result, the proposed method is tested using the BCG datasets from 18 subjects, including 8 females and 10 males (age 20–72). Finally, the heart rate from BCG will be compared with ECG, and the results are satisfactory and have a high accuracy.

Abstract:Sleep plays an important role in human life activities, and Obstructive sleep apnea (OSA) is a very important factor. Sleep apnea is a common sleep-related respiratory disease. Polysomnography (PSG) is the gold standard for detecting sleep apnea, but PSG is a contact device, there will be a first night effect, and some people may even be disturbed by long-term incompatibility. This study proposes to extract the cardiopulmonary coupling (CPC) strength based on the Ballistocardiogram (BCG) signal to achieve the purpose of further improving the accuracy of OSA detection. We extract heart rate and breathing through the BCG signal. Then, the time domain features and frequency domain features of the heartbeat interval sequence over a fixed length of time are extracted. The coupling strength of the two signals is further analyzed to generate cardiopulmonary coupling characteristics. A classification model of sleep apnea is used to determine whether sleep apnea occurs within a fixed length of time. And, the accuracy can be further improved by adding the cardiopulmonary coupling feature.
A Non-contact and Unconstrained Sleep Health Monitoring System

Abstract:Clinically, polysomnography (PSG) is used to assess sleep quality by monitoring various parameters, such as Electroencephalogram (EEG), electrocardiogram (ECG), Electrooculography (EOG), Electromyography (EMG), pulse, oxygen saturation, and respiratory rate. However, in order to assess these parameters, PSG requires a variety of sensors that must make direct contact with patients’ bodies, which can affect patients’ quality of sleep during testing. Thus, the use of PSG to assess sleep quality can yield invalid and inaccurate results. To address this gap, this paper proposes a sleep health monitoring system that has no restraints and does not interfere with sleep. This method collects ballistocardiogram (BCG) due to ejection by placing a piezoelectric film sensor under a sleeping cushion and evaluates three indicators: heart rate variability (HRV), respiration, and body movements. The ECG and BCG of 10 subjects were collected synchronously while the subjects were lying flat. Specifically, power-line interference was eliminated by adaptive digital filtering with a minimum mean square. A paired t-test revealed that there were no significant differences between BCG and standard ECG signals in the time-domain, frequency-domain, and nonlinear parameters of HRV. Respiration and body motion were extracted from the BCG in order to effectively monitoring of sleep apnea and nighttime bed-off times. Compared with other non-contact monitoring methods, such as acceleration sensors, coupling electrodes, Doppler radar and camera, the system presented in this paper is superior, as it has high signal quality, strong anti-interference ability, and low cost. Moreover, it does not interfere with normal sleep.

Abstract:In this work, we present a ballistocardiographic (BCG) system for the determination of heart and breath rates and activity of a user lying in bed. Our primary goal was to simplify the analog and digital processing usually required in these kinds of systems while retaining high performance. A novel sensing approach is proposed consisting of a white LED facing a digital light detector. This detector provides precise measurements of the variations of the light intensity of the incident light due to the vibrations of the bed produced by the subject's breathing, heartbeat, or activity. Four small springs, acting as a bandpass filter, connect the boards where the LED and the detector are mounted. Owing to the mechanical bandpass filtering caused by the compressed springs, the proposed system generates a BCG signal that reflects the main frequencies of the heartbeat, breathing, and movement of the lying subject. Without requiring any analog signal processing, this device continuously transmits the measurements to a microcontroller through a two-wire communication protocol, where they are processed to provide an estimation of the parameters of interest in configurable time intervals. The final information of interest is wirelessly sent to the user's smartphone by means of a Bluetooth connection. For evaluation purposes, the proposed system has been compared with typical BCG systems showing excellent performance for different subject positions. Moreover, applied postprocessing methods have shown good behavior for information separation from a single-channel signal. Therefore, the determination of the heart rate, breathing rate, and activity of the patient is achieved through a highly simplified signal processing without any need for analog signal conditioning.
[A Mattress System of Recognizing Sleep Postures Based on BCG Signal].

Abstract:Sleep posture recognition is the core index of diagnosis and treatment of positional sleep apnea syndrome. In order to detect body postures noninvasively, we developed a portable approach for sleep posture recognition using BCG signals with their morphological difference. A type of piezo-electric polymer film sensor was applied to the mattress to acquire BCG, the discrete wavelet transform with cubic B-spline was used to extract characteristic parameters and a naive Bayes learning phase was adapted to predict body postures. Eleven healthy subjects participated in the sleep simulation experiments. The results indicate that the mean error obtained from heart rates was 0.04±1.3 beats/min (±1.96 SD). The final recognition accuracy of four basic sleep postures exceeded 97%, and the average value was 97.9%. This measuring system is comfortable and accurate, which can be streamlined for daily sleep monitoring application.

Abstract:Objective: to develop quantitative methods for the clinical interpretation of the ballistocardiogram (BCG). Methods: a closed-loop mathematical model of the cardiovascular system is proposed to theoretically simulate the mechanisms generating the BCG signal, which is then compared with the signal acquired via accelerometry on a suspended bed. Results: simulated arterial pressure waveforms and ventricular functions are in good qualitative and quantitative agreement with those reported in the clinical literature. Simulated BCG signals exhibit the typical I, J, K, L, M and N peaks and show good qualitative and quantitative agreement with experimental measurements. Simulated BCG signals associated with reduced contractility and increased stiffness of the left ventricle exhibit different changes that are characteristic of the specific pathological condition. Conclusion: the proposed closed-loop model captures the predominant features of BCG signals and can predict pathological changes on the basis of fundamental mechanisms in cardiovascular physiology. Significance: this work provides a quantitative framework for the clinical interpretation of BCG signals and the optimization of BCG sensing devices. The present study considers an average human body and can potentially be extended to include variability among individuals.
