Abstract:BackgroundDiabetic kidney disease (DKD) is the leading cause of end-stage kidney disease (ESKD) in the world. Emerging evidence has shown that urinary mRNAs may serve as early diagnostic and prognostic biomarkers of DKD. In this article, we aimed to first establish a novel bioinformatics-based methodology for analyzing the urinary kidney-specific mRNAs and verify their potential clinical utility in DKD.MethodsTo select candidate mRNAs, a total of 127 Affymetrix microarray datasets of diabetic kidney tissues and other tissues from humans were compiled and analyzed using an integrative bioinformatics approach. Then, the urinary expression of candidate mRNAs in stage 1 study (n=82) was verified, and the one with best performance moved on to stage 2 study (n=80) for validation. To avoid potential detection bias, a one-step Taqman PCR assay was developed for quantification of the interested mRNA in stage 2 study. Lastly, the in situ expression of the selected mRNA was further confirmed using fluorescent in situ hybridization (FISH) assay and bioinformatics analysis.ResultsOur bioinformatics analysis identified sixteen mRNAs as candidates, of which urinary BBOX1 (uBBOX1) levels were significantly upregulated in the urine of patients with DKD. The expression of uBBOX1 was also increased in normoalbuminuric diabetes subjects, while remained unchanged in patients with urinary tract infection or bladder cancer. Besides, uBBOX1 levels correlated with glycemic control, albuminuria and urinary tubular injury marker levels. Similar results were obtained in stage 2 study. FISH assay further demonstrated that BBOX1 mRNA was predominantly located in renal tubular epithelial cells, while its expression in podocytes and urothelium was weak. Further bioinformatics analysis also suggested that tubular BBOX1 mRNA expression was quite stable in various types of kidney diseases.ConclusionsOur study provided a novel methodology to identify and analyze urinary kidney-specific mRNAs. uBBOX1 might serve as a promising biomarker of DKD. The performance of the selected urinary mRNAs in monitoring disease progression needs further validation.

Abstract:Background Type 2 diabetic kidney disease is the most common cause of chronic kidney diseases (CKD) and end-stage renal diseases (ESRD). Although kidney biopsy is considered as the ‘gold standard’ for diabetic kidney disease (DKD) diagnosis, it is an invasive procedure, and the diagnosis can be influenced by sampling bias and personal judgement. It is desirable to establish a non-invasive procedure that can complement kidney biopsy in diagnosis and tracking the DKD progress. Methods In this cross-sectional study, we collected 252 urine samples, including 134 uncomplicated diabetes, 65 DKD, 40 CKD without diabetes and 13 follow-up diabetic samples, and analyzed the urine proteomes with liquid chromatography coupled with tandem mass spectrometry (LC–MS/MS). We built logistic regression models to distinguish uncomplicated diabetes, DKD and other CKDs. Results We quantified 559 ± 202 gene products (GPs) (Mean ± SD) on a single sample and 2946 GPs in total. Based on logistic regression models, DKD patients could be differentiated from the uncomplicated diabetic patients with 2 urinary proteins (AUC = 0.928), and the stage 3 (DKD3) and stage 4 (DKD4) DKD patients with 3 urinary proteins (AUC = 0.949). These results were validated in an independent dataset. Finally, a 4-protein classifier identified putative pre-DKD3 patients, who showed DKD3 proteomic features but were not diagnosed by clinical standards. Follow-up studies on 11 patients indicated that 2 putative pre-DKD patients have progressed to DKD3. Conclusions Our study demonstrated the potential for urinary proteomics as a noninvasive method for DKD diagnosis and identifying high-risk patients for progression monitoring.

A self-assembling PCR array consisting of mRNAs with largest fold changes(FC) was constructed for further evalua-tion. 9 randomly-selected urinary mRNA samples of DKD patients were used to test the basic performance of the PCR array. RESULTS: A total of 108 qualiﬁed microarray datasets were included in the ﬁnal analysis. Thirty-ﬁve mRNAs with largest FC, including ALDOB(logFC:8.86,P=1.62E-26,compared with bladder epithelia) and CCL18(logFC:4.31,P=3.42E-7, compared with bladder epithelia), were identiﬁed, most of which haven’t been reported as biomarkers of DKD. The PCR array system achieved a positive call rate > 85% with all 9 samples. Melting curve analysis showed that single peak could be obtained for each reaction. The threshold cycle value of positive PCR control for eachsample indicated good reproducibility of the PCR array. CONCLUSIONS: Our study provided a novel methodology to identify and analyze urinary kidney-speciﬁc mRNAs in DKD, which could also be applied to other kidney diseases. Using the self-assembling PCR array, our future study will fur- ther validate the performance of the selected urinary mRNAs in monitoring DKD progression.
Analysis of Urinary Proteomic Patterns for Diabetic Nephropathy by ProteinChip.

Abstract:Diabetic nephropathy (DN) is the main cause of mortality for diabetic patients. The objective of this work was to develop a proteomic approach to detect proteins or peptides in urine for identifying individuals in the early stage of DN. We obtained urine samples from 106 diabetic patients and 50 healthy subjects. Early stage of DN was defined as urine albumin-to-creatinine ratio between 30 to 299 mg/g. Mass spectra were generated using surface-enhanced laser desorption/ ionization time-of-flight mass spectrometry. Peaks were detected by Ciphergen SELDI software version 3.1. Over 1000 proteins or peptides were obtained using ProteinChip. About 200 of them, the m/z values were in the range from 1008.5 to 79 942.3 Da. These values were significantly differentiated between diabetic patients and control subjects. A mathematical analysis revealed that a cluster of 8 up-regulated proteins and 16 down-regulated proteins was in the diabetic patients, with m/z values from 2197.3 to 79 613 Da. Four top-ranked proteins, with m/z values of 4139.0, 4453.5, 5281.1, and 5898.5 Da, were selected as the potential fingerprints for detection of early stage DN with a sensitivity of 75% and a specificity of 80%. ProteinChip technology may be a novel non-invasive method for detecting early stage DN.

Abstract:Urinary exosomal miRNAs can reflect the physiological and possibly pathophysiological state of cells lining the kidney and participate in the regulation of transcription and translation of proteins, which are playing an important role in the pathogenesis of diabetic kidney disease. In the present study, urine was collected from DM and DKD patients with a duration more than 10 years and urinary exosomal miRNA profiling was conducted in urinary exosomes obtained from three patients with type 2 diabetes (DM) and three patients with type 2 diabetic kidney disease (DKD) using Exiqon's microRNA arrays. In total, the expression of 14 miRNAs (miR-4491, miR-2117, miR-4507, miR-5088-5P, miR-1587, miR-219a-3p, miR-5091, miR-498, miR-4687-3p, miR-516b-5p, miR-4534, miR-1275, miR-5007-3p, and miR-4516) was up-regulated (>2-fold) in DKD patients compared to healthy controls and DM patients. We used qRT-PCR based analysis of these 14 miRNAs in urinary exosomes from 14 DKD to 14 DM patients in confirmation cohort, among which seven miRNAs were consistent with the microarray results. The expressions of miR-4534 and miR-516b-5p correlated with trace proteinuria levels in the confirmation cohort. In conclusion, it has been confirmed that the expression of urinary exosomal miRNA in patients with type 2 diabetes DKD has changed. Mir-4534 might affect the FoxO signaling pathway by targeting BNIP3, and is expected to become a new biomarker for the progression of type 2 DKD disease, which will provide further research on the pathogenesis of DKD.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>As one of the most prevalent diagnostic indicators of diabetic kidney disease (DKD), albumin‐to‐creatinine ratio (ACR) shows considerably limited predictive power in clinical application. We analyzed microarray expression profiling of urine to seek for differentially expressed miRNAs for potential biomarkers of DKD.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Urine samples from type 2 diabetes mellitus (T2DM) patients with (30 mg/g < ACR < 300 mg/g, DKD group) or without DKD (ACR < 30 mg/g, DM group) were collected for miRNA microarray analysis. The differentially expressed miRNAs were screened by bioinformatics analysis and validated by quantitative real‐time PCR. Target genes of differentially expressed miRNAs were predicted in miRDB, Targetscan, and microRNA.org databases. We also conducted the Gene Ontology (GO) and the Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathways analysis to explore for potential mechanisms in DKD.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Nine miRNAs were down‐regulated and seventeen miRNAs were up‐regulated in DKD group, compared to DM group. The levels of miR‐3137 and miR‐4270 in DKD group were 0.670 ± 0.505 and 2.116 ± 1.762 times than those in DM group, respectively, showing great significance. A total of 1076 target genes were simultaneously predicted by miRDB, Targetscan, and microRNA.org databases. According to the GO analysis results, disorders of endomembrane system may be one of the major pathological changes in DKD. In addition, Rap 1 signaling pathway is also altered obviously in DKD, discovered by the KEGG analysis.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>MiR‐3137 and miR‐4270 show the potential for urinary biomarkers of DKD. The pathological changes of DKD may be related to disorders of endomembrane system and alternation of Rap1 signaling pathway.</p></section>

Urinary Podocyte-Associated Mrna Profile in Various Stages of Diabetic Nephropathy

Abstract:BACKGROUND: Podocyte injury and subsequent excretion in urine play a crucial role in the pathogenesis and progression of diabetic nephropathy (DN). Quantification of messenger RNA (mRNA) expression in urinary sediment by real-time PCR is emerging as a noninvasive method of screening DN-associated biomarkers. We hypothesized that the urinary mRNA profile of podocyte-associated molecules may provide important clinical insight into the different stages of diabetic nephropathy. METHODS: DN patients (N = 51) and healthy controls (N = 13) were enrolled in this study. DN patients were divided into a normoalbuminuria group (UAE<30 mg/g, n = 17), a microalbuminuria group (UAE 30∼300 mg/g, n = 15), and a macroalbuminuria group (UAE>300 mg/g, n = 19), according to their urinary albumin excretion (UAE). Relative mRNA abundance of synaptopodin, podocalyxin, CD2-AP, α-actin4, and podocin were quantified, and correlations between target mRNAs and clinical parameters were examined. RESULTS: The urinary mRNA levels of all genes studied were significantly higher in the DN group compared with controls (p<0.05), and mRNA levels increased with DN progression. Urinary mRNA levels of all target genes positively correlated with both UAE and BUN. The expression of podocalyxin, CD2-AP, α-actin4, and podocin mRNA correlated with serum creatinine (r = 0.457, p = 0.001; r = 0.329, p = 0.01; r = 0.286, p = 0.021; r = 0.357, p = 0.006, respectively). Furthermore, podocalyxin mRNA was found to negatively correlate with eGFR (r = -0.349, p = 0.01). CONCLUSION: The urinary mRNA profiles of synaptopodin, podocalyxin, CD2-AP, α-actin4, and podocin were found to increase with the progression of DN, which suggested that quantification of podocyte-associated molecules will be useful biomarkers of DN.
Urinary Sediment CCL5 Messenger RNA As a Potential Prognostic Biomarker of Diabetic Nephropathy

Abstract:ABSTRACT Background Urinary sediment messenger RNAs (mRNAs) have been shown as novel biomarkers of kidney disease. We aimed to identify targeted urinary mRNAs in diabetic nephropathy (DN) based on bioinformatics analysis and clinical validation. Methods Microarray studies of DN were searched in the GEO database and Nephroseq platform. Gene modules negatively correlated with estimated glomerular filtration rate (eGFR) were identified by informatics methods. Hub genes were screened within the selected modules. In validation cohorts, a quantitative polymerase chain reaction assay was used to compare the expression levels of candidate mRNAs. Patients with renal biopsy–confirmed DN were then followed up for a median time of 21 months. End-stage renal disease (ESRD) was defined as the primary endpoint. Multivariate Cox proportional hazards regression was developed to evaluate the prognostic values of candidate mRNAs. Results Bioinformatics analysis revealed four chemokines (CCL5, CXCL1, CXLC6 and CXCL12) as candidate mRNAs negatively correlated with eGFR, of which CCL5 and CXCL1 mRNA levels were upregulated in the urinary sediment of patients with DN. In addition, urinary sediment mRNA of CXCL1 was negatively correlated with eGFR (r = −0.2275, P = 0.0301) and CCL5 level was negatively correlated with eGFR (r = −0.4388, P < 0.0001) and positively correlated with urinary albumin:creatinine ratio (r = 0.2693, P = 0.0098); also, CCL5 and CXCL1 were upregulated in patients with severe renal interstitial fibrosis. Urinary sediment CCL5 mRNA was an independent predictor of ESRD [hazard ratio 1.350 (95% confidence interval 1.045–1.745)]. Conclusions Urinary sediment CCL5 and CXCL1 mRNAs were upregulated in DN patients and associated with a decline in renal function and degree of renal interstitial fibrosis. Urinary sediment CCL5 mRNA could be used as a potential prognostic biomarker of DN.
Urinary Mrna Markers of Epithelial–mesenchymal Transition Correlate with Progression of Diabetic Nephropathy

Abstract:SummaryObjective Epithelial–mesenchymal transition (EMT) plays an important role in the pathogenesis and progression of diabetic nephropathy (DN). Quantification of messenger RNA (mRNA) expression in urinary sediment is emerging as a noninvasive method of screening DN‐associated biomarkers. The aim of our study was to examine whether urinary mRNA profile of EMT‐associated genes may provide valuable clinical insight into the different stages of DN.Design and methods Diabetic nephropathy patients (n = 44) and healthy controls (n = 12) were enrolled in this study. DN patients were divided into three groups according to the levels of estimated glomerular filtration rate (eGFR): Group A (eGFR > 60 ml/min per 1·73 m2, n = 27), Group B (45 < eGFR < 60 ml/min per 1·73 m2, n = 9), and Group C (eGFR < 45 ml/min per 1·73 m2, n = 8). Relative mRNA abundance of α‐smooth muscle actin (α‐SMA), fibronectin, FSP1 and matrix metalloproteinase‐9 (MMP‐9) were quantified, and correlations between target mRNAs and clinical parameters were examined.Results The urinary mRNA levels of α‐SMA, fibronectin and MMP‐9 were significantly higher in the DN group compared with controls (P < 0·05), and mRNA levels increased with DN progression. Urinary mRNA levels of all target genes positively correlated with both urinary albumin excretion (UAE) and blood urea nitrogen (BUN). Moreover, the expression of α‐SMA, fibronectin and MMP‐9 mRNA correlated with serum creatinine levels (r = 0·514, r = 0·53 and r = 0·469, all P < 0·001) and eGFR levels (r = −0·374, r = −0·392 and r = −0·487, all P < 0·01).Conclusions Quantification of EMT‐associated genes in urinary sediment may be a novel approach for searching new biomarkers of DN.
Urinary Mirna-29A-3p Levels Are Associated with Metabolic Parameters Via Regulation of IGF1 in Patients with Metabolic Syndrome.

Abstract:Circulating microRNAs (miRNAs or miRs) have been demonstrated to serve as diagnostic and prognostic biomarkers in metabolic syndrome (MetS). The role of urinary miRNAs in MetS diagnosis remains unknown. Here, elevated miR-29a-3p levels were observed in urine samples of patients with MetS compared with control subjects using a microarray analysis (n=4/group) and validation via reverse transcription-quantitative polymerase chain reaction (n=40/group). Associations between urinary miR-29a-3p levels and parameters associated with metabolism, such as adiposity, insulin resistance, lipid profiles and hepatic enzymes were further assessed. Multiple linear regression analyses revealed that urinary miR-29a-3p levels were independently correlated with fasting insulin (β=0.561; P<0.001), high density lipoprotein-cholesterol (β=0.242; P<0.001) and body mass index (β=-0.141; P<0.05). The area under the receiver operating characteristic curve was 0.776 and miR-29a-3p had a diagnostic value for MetS with 68.2% sensitivity and 77.3% specificity. Furthermore, insulin-like growth factor 1 was identified as a target of miR-29a-3p by searching bioinformatics databases and was validated by dual-luciferase reporter and western blot assays. In conclusion, elevated urinary miR-29a-3p levels were positively associated with MetS and demonstrated to have a potential value as biomarkers in the diagnosis of MetS. The findings provided a better understanding of the role of urinary miRNAs in pathogenesis of MetS.

Abstract:Abstract Background and Aims In recent years, it has been found that targeted mRNA detection of sediment cells in urine can be used as novel biomarkers for the diagnosis of diabetic nephropathy (DN). However, its value in predicting the progression of DN is not clear. The purpose of this study is to seek for urinary mRNA markers that evaluate the prognosis of DN through systems biological screening, clinical verification and prospective studies. Method GEO database and “Nephroseq” platform were searched, and the transcriptome data of DN glomeruli and tubules and their clinical information were obtained. Weighted gene co-expression network analysis (WGCNA) combined with gene ontology (GO) annotation and KEGG pathway enrichment analysis were used to screen the hub genes negatively related to eGFR, and the hub genes were used as candidate markers for mRNA detection in urine sediment in DN patients. A total of 91 patients with DN diagnosed by renal biopsy were included, and 60 patients with type 2 diabetes and 61 healthy people were selected as the control groups. The mRNA expression of candidate molecules was detected by Taqman probe quantitative PCR, and the correlation between mRNA expression and eGFR and urinary protein levels were analyzed. Patients with DN were followed up for a median time of 21 months, and the primary end point was defied as end stage renal disease or eGFR decreasing by more than 50%. Multivariate Cox regression was used to evaluate the value of mRNA in predicting DN progression. Results GSE30528 and GSE30529 datasets were selected for analysis, including mRNA expression data of 9 cases of DN and 13 cases of normal glomeruli; and 10 cases of DN and 12 cases of normal tubules respectively. The clinical data of the patients in this study, including gender, race, age and eGFR, were searched on the Nephroseq platform. The gene modules negatively related to eGFR were screened by WGCNA. GO and KEGG analysis showed that the main function of the gene modules in both datasets were related to the activation of inflammatory cells and chemokines pathway. Through the screening of hub genes and the comparison of expression levels, CCL5, CXCL1, CXCL6 and CXCL12 were finally obtained as candidate genes. Quantitative PCR showed that the levels of CCL5 and CXCL1 was significantly increased in DN group, CCL5 was negatively correlated with eGFR and positively correlated with urinary protein level, while CXCL1 was negatively correlated with eGFR, but had no significant correlation with urinary protein level. Multivariate Cox regression showed that eGFR, urinary protein level, degree of renal fibrosis and urinary CCL5 were independent risk factors of primary end point. Conclusion The activation of chemokine signal pathway in renal tissue is involved in the progression of DN. Urinary CCL5 mRNA can independently predict the prognosis of DN and may be served as a novel biomarker for the progression of DN.

Abstract:OBJECTIVES/GOALS: This study aimed to characterize urinary exosomal miRNA content in African American adults with diabetic kidney disease. METHODS/STUDY POPULATION: Male and female participants between the ages of 18 and 65 were recruited from the Diabetes Treatment Center and the Nephrology Clinic at the Howard University Hospital. Exosomes were isolated from cleared urine of healthy controls (n=3), type 2 diabetics (n=3), and participants with chronic kidney disease (n=3). The purity and size of isolated microparticles was evaluated using NanoSight technology (30nm to 120nm size range) and western blot analysis for exosome-specific markers (TSG101 and CD81) RESULTS/ANTICIPATED RESULTS: Expression of 5 selected microRNAs, miR-4534, miR-320c, miR-451, miR-362-3p and miR-877-3p were evaluated by qRT-PCR. miR-4534 and miR-451 was increased between healthy controls and the type diabetic group. MiR-320c was increased in the CKD group, in comparison to healthy controls. Conversely, there was no difference in miR-877-5p between the three groups. DISCUSSION/SIGNIFICANCE: These findings will provide insight into the use of circulating miRNAs as early markers of DKD, ultimately creating more effective treatments and preventive measures.

Abstract:With a steep increase in the incidence of type 1 and 2 diabetes globally, diabetic nephropathy (DN) has now become the leading cause of renal failure in the world. There are no suitable biomarkers for the diagnosis of early stages of DN. In recent years, tremendous efforts are being made worldwide to delineate the role of micro RNAs in the pathogenesis of DN. Circulating miRNAs in serum, plasma, urine and other body fluids, which reflect a response to various pathophysiological stresses, are being investigated in the context of diabetic nephropathy. Delineation of the changes in miRNA levels in patients with DN may lead to a better understanding of the progression of the disease. We present here an exhaustive survey of the miRNA literature, highlighting various studies performed over the last decade. The aim is to assess if changes in various miRNAs could correlate with the progression of diabetic nephropathy. Based on the survey, we found that miRNA-377, miRNA-192, miRNA-216/217 and miRNA-144 are increased in body fluids of patients with DN, while miRNA-21 and miRNA-375 are decreased. Overall, there are a very few miRNAs that are kidney specific, and although significant differences were observed in the urinary excretion of certain miRNAs, they were not correlative to their levels in the blood or plasma. Thus, it is completely plausible that urine-specific miRNAs could serve as novel biomarkers for the diagnosis of early stages of diabetic nephropathy.
Bioinformatics prediction and experimental verification of key biomarkers for diabetic kidney disease based on transcriptome sequencing in mice

Abstract:Background Diabetic kidney disease (DKD) is the leading cause of death in people with type 2 diabetes mellitus (T2DM). The main objective of this study is to find the potential biomarkers for DKD. Materials and Methods Two datasets ( GSE86300 and GSE184836 ) retrieved from Gene Expression Omnibus (GEO) database were used, combined with our RNA sequencing (RNA-seq) results of DKD mice (C57 BLKS-32w db/db) and non-diabetic (db/m) mice for further analysis. After processing the expression matrix of the three sets of data using R software “Limma”, differential expression analysis was performed. The significantly differentially expressed genes (DEGs) (—logFC— > 1, p -value < 0.05) were visualized by heatmaps and volcano plots respectively. Next, the co-expression genes expressed in the three groups of DEGs were obtained by constructing a Venn diagram. In addition, Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were further analyzed the related functions and enrichment pathways of these co-expression genes. Then, qRT-PCR was used to verify the expression levels of co-expression genes in the kidney of DKD and control mice. Finally, protein-protein interaction network (PPI), GO, KEGG analysis and Pearson correlation test were performed on the experimentally validated genes, in order to clarify the possible mechanism of them in DKD. Results Our RNA-seq results identified a total of 125 DEGs, including 59 up-regulated and 66 down-regulated DEGs. At the same time, 183 up-regulated and 153 down-regulated DEGs were obtained in GEO database GSE86300 , and 76 up-regulated and 117 down-regulated DEGs were obtained in GSE184836 . Venn diagram showed that 13 co-expression DEGs among the three groups of DEGs. GO analysis showed that biological processes (BP) were mainly enriched inresponse to stilbenoid, response to fatty acid, response to nutrient, positive regulation of macrophage derived foam cell differentiation, triglyceride metabolic process. KEGG pathway analysis showed that the three major enriched pathways were cholesterol metabolism, drug metabolism–cytochrome P450, PPAR signaling pathway. After qRT-PCR validation, we obtained 11 genes that were significant differentially expressed in the kidney tissues of DKD mice compared with control mice. (The mRNA expression levels of Aacs, Cpe, Cd36, Slc22a7, Slc1a4, Lpl, Cyp7b1, Akr1c14 and Apoh were declined, whereas Abcc4 and Gsta2 were elevated). Conclusion Our study, based on RNA-seq results, GEO databases and qRT-PCR, identified 11 significant dysregulated DEGs, which play an important role in lipid metabolism and the PPAR signaling pathway, which provide novel targets for diagnosis and treatment of DKD.

Abstract:Abstract BACKGROUND AND AIMS Diabetic nephropathy (DN) is the common cause of end-stage renal disease. However, the early and specific diagnostic biomarkers are still lacking. Urinary exosomes are a potential source of biomarkers for kidney disease due to their renal cell origination, stability, and noninvasive collection. Long non-coding RNAs (lncRNAs) are increasingly recognized to be involved in diverse pathophysiological processes including kidney disease. Here we set to characterize the lncRNAs profiles in urinary exosomes of DN with aims to identify novel non-invasive biomarkers of DN. METHOD First-morning urine was collected from 11 healthy people, 9 patients with diabetes mellitus (DM), and 8 patients with biopsy-proven DN. Urinary exosomes were pelleted by ultracentrifugation and total RNA was extracted for next generation sequencing (NGS). The lncRNA profile of urinary exosomes was first characterized in terms of the detectable RNA numbers, expression levels in different groups. Then differential gene expression analysis was performed to facilitate the discovery of potential biomarkers. RESULTS Similar tremendous number of lncRNAs was detected with a total number of 6439 transcripts present in urinary exosomes in each group. In terms of the expression levels, the percentage of transcripts with TPM above 1 was 31.4%, 32.8%, 33.2% in the healthy group, DM and DN patients respectively, suggesting that no significant alteration of lncRNAs numbers and distribution in DN and diabetes conditions. Next, the differential expression analysis was performed to identify the distinct differential lncRNAs. Compared with healthy people, 18 lncRNAs were upregulated and 67 lncRNAs were downregulated in DM group, while 37 unregulated and 80 downregulated lncRNAs were identified in DN group. Moreover, the profile of lncRNAs in urinary exosomes was significantly different in DN patients compared to DM group with 59 lncRNAs upregulated and 36 lncRNAs downregulated. To explore the candidate biomarkers, 12 upregulated lncRNAs and 4 downregulated lncRNAs from the two comparison sets (DN versus Healthy, DN versus Healthy) were identified, among which FLJ16779 was present with the most remarkable difference. Further analysis showed that the sensitivity and specificity of FLJ16779 for distinguishing DN from healthy people are both 87.5%, and the area under the curve (AUC) was 0.9531. Additionally, the sensitivity and specificity of FLJ16779 for distinguishing DN from DM were 75% and 87.5% respectively, and AUC was 0.9063. CONCLUSION Urinary exosomes contain rich lncRNAs information with differential expression profiles in DN patients compared to DM and healthy controls, suggesting the possible involvement of lncRNAs in the pathogenesis of DN. lncRNA FLJ16779 might be a promising non-invasive biomarker of DN which requires further validation.

Abstract:Objective:To detect urinary polypeptide patterns in stages of the type 2 diabetic nephropathy(DN) in order to screen and find out the potential biomarkers.Methods:A total of 68 urine samples from type 2 diabetic patients with normoalbuminuria(DM1,n = 24),microalbuminuria(DM2,n = 22)and macroalbuminuria(DM3,n = 22),and healthy controls(C,n = 20) were condensed by magnetic beads based weak cation exchange chromatography(MB-WCX),and analyzed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry(MALDI-TOF-MS).ClinProtTM software was used to profile and screen the polypeptide patterns in urine of the patients.Results:When the relative molecular mass < 12 000,there were significant differences in 15 protein peaks between group DM1 and group C(P < 0.01).There was one protein peak of significantly low expression in the group DM2,when compared to group C(P = 0.029 8).And there were significant differences in 10 protein peaks between group DM3 and group C(P < 0.01).Conclusion:MB-WCX coupled to MALDI-TOF-MS is a fast,convenient and high throughput analyzing method capable of screening some relatively specific and potential biomarkers from the urine of DN with different stages.
Urinary FBP1 as a novel biomarker for the diagnosis and progression of diabetic nephropathy

Abstract:Abstract Background: The pathogenesis of diabetic nephropathy (DN) is complex, and its onset is occult. It should be considered from multiple levels and angles using the concept of "system biology". New technologies, such as omics and high-throughput screening, provide more strategies for the screening of ideal markers of DN. Methods: we used data-independent acquisition (DIA) mass spectrometry for urine proteomic analysis of DN patients at different pathological stages and healthy controls. Results: Fructose-1,6-bisphosphatase 1 (FBP1) was specifically found in the urine of DN patients but not in that of healthy controls. Then, ELISA demonstrated that the urinary level of FBP1 in DN patients was significantly higher than that in healthy controls. We further verified the higher levels of urine FBP1 in association with DN than in association with type 2 diabetes mellitus (T2DM) and nondiabetic nephropathy. Receiver operating characteristic (ROC) curve analysis showed that FBP1 can distinguish DN and T2DM patients, and the AUC was 0.889 (sensitivity: 80%, specificity: 85%). Correlation analysis revealed that the level of urinary FBP1 was positively correlated with the progression of DN, as reflected by the fact that urinary FBP1 was negatively correlated with e-GFR and positively correlated with urea nitrogen, the interstitial inflammation score, and the interstitial fibrosis and tubular atrophy (IFTA) score. Conclusions: The source of urinary FBP1 was traced, and it was revealed that it was specifically reduced in proximal renal tubules with the progression of DN, supporting the view that urinary FBP1 can be used as a marker for the progression of diabetic nephropathy. Together, our results indicate that FBP1 might not only be a novel marker for the diagnosis of DN but also be a new marker for the progression of DN.

Abstract:Diabetic kidney disease is a growing health burden that lacks specific early non-invasive diagnostic procedures. To approach a solution for this clinical need, we sequenced microRNAs of urinary extracellular vesicles and performed biomarker discovery by small RNA sequencing in a type 1 diabetes cohort including males with and without albuminuria. The results were replicated by sequencing or qPCR in two independent cohorts and four previously published datasets including type 1 and 2 diabetes as well as both sexes. Non-diabetic and prostate cancer cohorts were used as additional controls and miRNAs changed due to preanalytical urine collection variables were excluded. Using these data, we additionally validated previously identified reference candidate miRNAs. Correlations with clinical parameters, receiver operating characteristic analysis, targeted mRNAs and pathways including integrated single cell data, and targeted circulating proteins from type 1 and 2 diabetes cohorts were analyzed. We pinpointed 6 stable microRNAs, 11 differentially expressed microRNAs, 9 target proteins and 16 DKD-associated pathways in individuals with diabetic kidney disease. Replication showed that the differentially expressed miRNAs in DKD were partly shared between diabetes subtypes and sexes with overall strongest evidence for miR-192-5p, miR-146a-5p, miR-486-5p, and miR-574-5p. Combination of these miRNAs with clinical variables showed potential to classify individuals with the fastest kidney function decline (sensitivity 0.75-1.00 and specificity 0.83-1.00) even in the normoalbuminuria group, thus holding the potential as early diagnostic markers. Altogether, the candidate microRNAs show specificity for diabetic kidney disease, identify declining kidney function, and target key kidney cell types, mRNAs, proteins, and pathogenic mechanisms.
