Abstract:Previous studies on liquid biopsy-based detection of advanced colorectal adenoma (advCRA) and early stage adenocarcinoma (CRC) were limited by low sensitivity. To this end, we performed a prospective study and established an integrated model using multiple fragmentomic profiles of plasma cell-free DNA (cfDNA) for accurately and cost-effectively detecting stage 0/I CRC and advCRA. This study enrolled a total of 621 participants, including 298 CRC patients (34 stage 0, 264 stage I), 92 advCRA patients and 231 healthy controls. Plasma cfDNA samples were prepared for low coverage whole-genome sequencing (∼5X). Participants were randomly divided into a training cohort (N = 310) and a test cohort (N = 311). An ensemble stacked model differentiating healthy controls from advCRA/CRC patients was trained using four machine learning models and five cfDNA fragmentomic features, including fragment size distribution and ratio, end and breakpoint motif, and copy number alteration, which was then validated in the test cohort. Our model showed an Area Under the Curve (AUC) of 0.988 for differentiating advCRA/CRC patients from healthy individuals. The model performed even better for identifying CRC patients (AUC 0.990) compared to advCRA patients (AUC 0.982). At 94.8% specificity, the sensitivities for detecting advCRA and CRC reached 95.7% and 98.0% (stage 0: 94.1%; stage I: 98.5%), respectively. Promisingly, the detection sensitivity has reached 100% and 97.6% in CRC patients with negative fecal occult or CEA blood test results, respectively. In advCRA subgroup, our model showed high sensitivities for detecting different grades of dysplasia (high: 91.3%, low: 100%) and for pedunculated (92.9%) or sessile adenoma (96.9%). Finally, our model maintained promising performances (mean AUC: 0.985, 92.6% sensitivity at 94.8% specificity) even when sequencing depth was downsampled to 1X. Our integrated predictive model using plasma cfDNA fragmentomic profiles demonstrated an unprecedented detection sensitivity for advCRA and very early-stage CRC, shedding light on more accurate non-invasive colorectal cancer screening in clinical practice.

Abstract:3038 Background: Non-small-cell lung cancer (NSCLC) is the leading cause of worldwide cancer-related deaths. Currently, ̃30-55% of the NSCLC patients develop recurrence due to minimal residual disease (MRD) after receiving surgical resection of the tumor. Therefore, there is an urgent clinical need to accurately predict the MRD risk in post-surgical NSCLC patients. However, the current targeted ctDNA mutation profiling method is limited by the cost of design and synthesis of patient-specific panels and relatively low sensitivity. Cell-free DNA (cfDNA) fragmentomics have recently shown great accuracy and affordability in early cancer detection. This retrospective study aims to develop an ultra-sensitive and affordable fragmentomic assay for MRD detection in NSCLC patients. Methods: A total of 116 NSCLC patients (54 stage I, 21 stage II, 40 stage III and 1 stage IV), who received curative surgical resections (32 patients relapsed during follow-up, median relapse time: 315.5 days), enrolled in the Lung Cancer Tempo-spatial Heterogeneity (LuCaTH) cohort. A total of 231 plasma samples, collected at 7 days post-surgery and 3 months thereafter, were used for whole-genome sequencings (̃5X). The cfDNA fragment size profile was used to fit a Regularized Cox Regression model. A leave-one-out cross-validation (LOOCV) was used to evaluate the model's predictive performance, and the optimal cutoff for predicting relapse was determined by identifying the Youden index to the predicted relative risk for all samples. Results: Our machine learning model showed an excellent performance in detecting patients with a high risk of recurrence. At 7 days post-surgery, the high-risk patients detected by our model showed an increased risk of 3 times compared to the low-risk patients (hazard ratio [HR] = 3.2, p < 0.001). Multivariate analysis confirmed that the association between model-determined high-risk status and patient relapses was not affected by the baseline clinical variables (age, sex, smoking, stage, etc.). Furthermore, the longitudinal analysis showed that our model was capable of detecting high-risk patients who were 11 times more likely to develop recurrence, independent from other clinical factors (multivariate HR = 11.8, p < 0.001). Overall, our model was able to identify high-risk status in 26 of 32 relapsed patients (81.2% sensitivity), preceding radiographic relapse by a median of 216 days. Furthermore, the sensitivity for MRD detection reached 91% while combining the model prediction with mutation-based ctDNA results. Conclusions: We have developed a predictive model for patient recurrence by detecting the fragmentomic profiling of plasma cfDNA contributed by the MRD. Despite being limited by the relatively small cohort size, our model has shown great sensitivity in predicting patient recurrence, therefore exhibiting a great potential to guide adjuvant therapy decisions.

Abstract:Abstract Non-invasive approaches for cell-free DNA (cfDNA) assessment provide an opportunity for cancer detection and intervention. Here, we use a machine learning model for detecting tumor-derived cfDNA through genome-wide analyses of cfDNA fragmentation in a prospective study of 365 individuals at risk for lung cancer. We validate the cancer detection model using an independent cohort of 385 non-cancer individuals and 46 lung cancer patients. Combining fragmentation features, clinical risk factors, and CEA levels, followed by CT imaging, detected 94% of patients with cancer across stages and subtypes, including 91% of stage I/II and 96% of stage III/IV, at 80% specificity. Genome-wide fragmentation profiles across ~13,000 ASCL1 transcription factor binding sites distinguished individuals with small cell lung cancer from those with non-small cell lung cancer with high accuracy (AUC = 0.98). A higher fragmentation score represented an independent prognostic indicator of survival. This approach provides a facile avenue for non-invasive detection of lung cancer.

Abstract:Rationale: Cell-free DNA (cfDNA) analysis holds promise for early detection of lung cancer and benefits patients with higher survival. However, the detection sensitivity of previous cfDNA-based studies was still low to suffice for clinical use, especially for early-stage tumors. Objectives: Establish an accurate and affordable approach for early-stage lung cancer detection by integrating cfDNA fragmentomics and machine learning models. Methods: This study included 350 participants without cancer and 432 participants with cancer. The participants' plasma cfDNA samples were profiled by whole-genome sequencing. Multiple cfDNA features and machine learning models were compared in the training cohort to achieve an optimal model. Model performance was evaluated in three validation cohorts. Measurements and Main Results: A stacked ensemble model integrating five cfDNA features and five machine learning algorithms constructed in the training cohort (cancer: 113; healthy: 113) outperformed all the models built on individual feature-algorithm combinations. This integrated model yielded superior sensitivities of 91.4% at 95.7% specificity for cohort validation I (area under the curve [AUC], 0.984), 84.7% at 98.6% specificity for validation II (AUC, 0.987), and 92.5% at 94.2% specificity for additional validation (AUC, 0.974), respectively. The model's high performance remained consistent when sequencing depth was down to 0.5× (AUC, 0.966-0.971). Furthermore, our model is sensitive to identifying early pathological features (83.2% sensitivity for stage I, 85.0% sensitivity for <1 cm tumor at the 0.66 cutoff). Conclusions: We have established a stacked ensemble model using cfDNA fragmentomics features and achieved superior sensitivity for detecting early-stage lung cancer, which could promote early diagnosis and benefit more patients.

Abstract:Abstract Colorectal cancer is frequently diagnosed in advanced stages, highlighting the need for developing approaches for early detection. Liquid biopsy using cell-free DNA (cfDNA) fragmentomics is a promising approach, but the clinical application is hindered by complexity and cost. This study aimed to develop an integrated model using cfDNA fragmentomics for accurate, cost-effective early-stage colorectal cancer detection. Plasma cfDNA was extracted and sequenced from a training cohort of 360 participants, including 176 patients with colorectal cancer and 184 healthy controls. An ensemble stacked model comprising five machine learning models was employed to distinguish patients with colorectal cancer from healthy controls using five cfDNA fragmentomic features. The model was validated in an independent cohort of 236 participants (117 patients with colorectal cancer and 119 controls) and a prospective cohort of 242 participants (129 patients with colorectal cancer and 113 controls). The ensemble stacked model showed remarkable discriminatory power between patients with colorectal cancer and controls, outperforming all base models and achieving a high area under the receiver operating characteristic curve of 0.986 in the validation cohort. It reached 94.88% sensitivity and 98% specificity for detecting colorectal cancer in the validation cohort, with sensitivity increasing as the cancer progressed. The model also demonstrated consistently high accuracy in within-run and between-run tests and across various conditions in healthy individuals. In the prospective cohort, it achieved 91.47% sensitivity and 95.58% specificity. This integrated model capitalizes on the multiplex nature of cfDNA fragmentomics to achieve high sensitivity and robustness, offering significant promise for early colorectal cancer detection and broad patient benefit. Significance: The development of a minimally invasive, efficient approach for early colorectal cancer detection using advanced machine learning to analyze cfDNA fragment patterns could expedite diagnosis and improve treatment outcomes for patients. Abstract:Abstract Background: Lung cancer is one of the deadliest types of cancer in China. Its 5-year survival rates at early stages are significantly higher than advanced stages. LDCT has been adopted for lung cancer screening in high-risk individuals; however, debate regarding its accuracy is still ongoing. Mutation detection on cell free DNA (cfDNA) has traditionally been used to monitor DNA molecular changes derived from lung cancer cells in blood, while recently fragmentation pattern profiling of cfDNA has been shown as a promising alternative for early cancer detection. We aimed to combine mutation detection and fragmentation pattern analysis on cfDNA to develop a non-invasive assay to screen early lung cancer. Methods: Candidate DNA mutations were curated from literature and public databases, including COSMIC and TCGA. DNA fragmentation markers were collected from literature and whole genome sequencing (WGS) datasets. A panel of 407 primers covering selected mutation and fragmentation markers was developed to distinguish lung adenocarcinoma (ADC) plasma and normal plasma. We enrolled a total of 122 plasma samples (64 normal, 58 ADC) for this study. 49 normal samples and 44 ADC samples were used to construct a mutation-based classification model and a fragmentation-based classification model separately. The tuning parameters and features were determined by inner 4-fold cross validation. For the mutation-based model, baseline was set using normal samples in training set. Maximum allele frequency was calculated for each sample in test data (15 normal, 14 ADC), which was filtered by the background baseline. For the fragmentation-based model, we used the DELFI fragment score to construct fragmentation profiles, which was the ratio between short fragments (100-150bp) and long fragments (151-220bp). After optimization, the two models were integrated by Logistic Regression to create a combined model, which was validated by 4-fold nested cross validation. Results: ADC and normal plasma were sequenced by the aforementioned panel at an average depth of 2,000X to ensure the reliability of model construction and classification results. In classifying normal and ADC plasma, the mutation model alone is only modestly accurate as it produced an AUC of 0.69. But the fragmentation model demonstrated significantly higher accuracy, achieving AUC of 0.85. Furthermore, the combined model performed better than either model along, achieving an elevated AUC of 0.87. Conclusions: We demonstrate that DNA mutation and fragmentation pattern of cfDNA can classify lung cancer and normal plasmas separately, but fragmentation pattern are more accurate than mutation in this task. Combining the two models further improved prediction accuracy, suggesting they complement each other. Although this is a pilot study of limited cases, it demonstrated the potential of combining markers for accurate lung ADC detection in plasma. Abstract:Objective: Identifying minimal residual disease (MRD) in postoperative lung cancer patients is crucial for timely cancer intervention. However, current fixed panel methods suffer from low sensitivity, whereas personalized panels face challenges as long yield time and require patient tissue samples. Our objective was to develop an accurate and efficient method for detecting circulating-tumor DNA (ctDNA) in post-surgery lung cancer patients. Methods: The study retrospectively enrolled 39 lung cancer patients (14 relapse, 25 non-relapse) with 63 post-surgical plasma samples. The mutational profiles of cell-free DNA (cfDNA) were obtained using SHIELDINGTM ULTRA Panel by Geneseeq, covering hotspots in 2365 cancer-associated genes. A comprehensive error suppressing algorithm, which combines intra-run sampling adaptive noise control, cfDNA fragment profiling, normal pool, and white blood cell background, was utilized to eliminate the background artifacts. The plasma ctDNA status were identified under both tumor-informed and tumor-naïve scenarios. Results: Our method yielded results on par with personalized panels, achieving a sensitivity of 85.7% at the specificity of 92.0%, with a negative predictive value (NPV) of 92.0% in longitudinal analysis. The ctDNA positive patients had a 21-fold increased risk (Hazard Ratio [HR] = 20.7, p = 9.2 × 10^(-5)) compared to the ctDNA negative patients. Remarkably, the landmark analysis using 39 samples (⇐ 7 days post-surgical) showed a 57.1% sensitivity at 100.0% specificity. Patients with a landmark (⇐ 7 days post-surgical) ctDNA positive status showed an increased risk of 16.2 times (HR = 16.2, p = 3.5 × 10^(-6)). Furthermore, we found that our approach could maintain its level of performance under tumor-naïve scenario, showing HRs of 12.7 (p = 1.3 × 10^(-4)) for longitudinal ctDNA positive patients and 12.3 (p = 3.7 × 10^(-5)) for landmark ctDNA positive patients. The ctDNA status determined by the tumor-informed and tumor-naïve approaches showed a substantial agreement, yielding a concordance of 90.5% (Cohen's Kappa = 0.738). Conclusion: Herein, we report a comprehensive error suppressing approach for removing background artifacts by fixed MRD panel, allowing an enhanced MRD detection in post-surgical lung cancer patients. Our approach, which accurately identified ctDNA status at a cost-effective rate, outperformed other fixed panels while showing comparable performance with personalized panels. Furthermore, our method can be utilized under tumor naïve scenarios when personalized panels are unfeasible in the absence of tumor information. Finally, the faster turnout time of fixed panel makes our approach more suitable for clinical landmark MRD detection than a personalized panel. Abstract:Background: Lung cancer is one of most common cancers worldwide, with a 5-year survival rate of less than 20%, which is mainly due to late-stage diagnosis. Noninvasive methods using 5-hydroxymethylation of cytosine (5hmC) modifications and fragmentation profiles from 5hmC cell-free DNA (cfDNA) sequencing provide an opportunity for lung cancer detection and management. Results: A total of 157 lung cancer patients were recruited to generate the largest lung cancer cfDNA 5hmC dataset, which mainly consisted of 62 lung adenocarcinoma (LUAD), 48 lung squamous cell carcinoma (LUSC) and 25 small cell lung cancer (SCLC) patients, with most patients (131, 83.44%) at advanced tumor stages. A 37-feature 5hmC model was constructed and validated to distinguish lung cancer patients from healthy controls, with areas under the curve (AUCs) of 0.8938 and 0.8476 (sensitivity = 87.50% and 72.73%, specificity = 83.87% and 80.60%) in two distinct validation sets. Furthermore, fragment profiles of cfDNA 5hmC datasets were first explored to develop a 48-feature fragmentation model with good performance (AUC = 0.9257 and 0.822, sensitivity = 87.50% and 78.79%, specificity = 80.65% and 76.12%) in the two validation sets. Another diagnostic model integrating 5hmC signals and fragment profiles improved AUC to 0.9432 and 0.8639 (sensitivity = 87.50% and 83.33%, specificity = 90.30% and 77.61%) in the two validation sets, better than models based on either of them alone and performing well in different stages and lung cancer subtypes. Several 5hmC markers were found to be associated with overall survival (OS) and disease-free survival (DFS) based on gene expression data from The Cancer Genome Atlas (TCGA). Conclusions: Both the 5hmC signal and fragmentation profiles in 5hmC cfDNA data are sensitive and effective in lung cancer detection and could be incorporated into the diagnostic model to achieve good performance, promoting research focused on clinical diagnostic models based on cfDNA 5hmC data.

Abstract:Background: Early screening using low-dose computed tomography (LDCT) can reduce mortality caused by non-small-cell lung cancer. However, ∼25% of the 'suspicious' pulmonary nodules identified by LDCT are later confirmed benign through resection surgery, adding to patients' discomfort and the burden on the healthcare system. In this study, we aim to develop a noninvasive liquid biopsy assay for distinguishing pulmonary malignancy from benign yet 'suspicious' lung nodules using cell-free DNA (cfDNA) fragmentomics profiling. Methods: An independent training cohort consisting of 193 patients with malignant nodules and 44 patients with benign nodules was used to construct a machine learning model. Base models using four different fragmentomics profiles were optimized using an automated machine learning approach before being stacked into the final predictive model. An independent validation cohort, including 96 malignant nodules and 22 benign nodules, and an external test cohort, including 58 malignant nodules and 41 benign nodules, were used to assess the performance of the stacked ensemble model. Results: Our machine learning models demonstrated excellent performance in detecting patients with malignant nodules. The area under the curves reached 0.857 and 0.860 in the independent validation cohort and the external test cohort, respectively. The validation cohort achieved an excellent specificity (68.2%) at the targeted 90% sensitivity (89.6%). An equivalently good performance was observed while applying the cut-off to the external cohort, which reached a specificity of 63.4% at 89.7% sensitivity. A subgroup analysis for the independent validation cohort showed that the sensitivities for detecting various subgroups of nodule size (<1 cm: 91.7%; 1-3 cm: 88.1%; >3 cm: 100%; unknown: 100%) and smoking history (yes: 88.2%; no: 89.9%) all remained high among the lung cancer group. Conclusions: Our cfDNA fragmentomics assay can provide a noninvasive approach to distinguishing malignant nodules from radiographically suspicious but pathologically benign ones, amending LDCT false positives.
Evaluation of cfDNA fragmentation characteristics in plasma for the diagnosis of lung cancer: A prospective cohort study

Abstract:Lung cancer is one of the most prevalent cancers worldwide, yet only approximately 16% of patients are diagnosed in early stage, highlighting the urgent need for novel, highly accurate detection models. In our study, patients with suspected lung cancer or lung disease, as identified through radiographic imaging, along with healthy individuals, were consecutively recruited from Beijing Chest Hospital. Circulating free DNA (cfDNA) was extracted from plasma samples, and low-depth whole-genome sequencing was performed to identify fragmentomic features for model construction. A total of 265 participants were prospectively enrolled, comprising 124 lung cancer patients and 141 noncancer individuals. The model we developed was based on four cfDNA fragmentation characteristics, including 20-bp breakpoint nucleotides motif, fragmentation size coverage, fragmentation size distribution, and copy number variation. In an independent test cohort, the model achieved an area under the curve (AUC) of 0.861 (95% CI: 0.781-0.942) and demonstrated a sensitivity of 70% (95% CI: 53.5%-83.4%) at a specificity of 89.4% (95% CI: 76.9%-96.5%). Notably, the model was also effective in detecting early-stage cancer, with an AUC of 0.808 (95% CI: 0.69-0.925). In summary, our lung cancer detection model shows strong screening capabilities by leveraging four cfDNA fragmentation characteristics, exhibiting robust performance in early cancer diagnosis.

An equivalently good performance was observed while applying the cutoff to the external cohort, which reached a specificity of 63.4% (95% CI: 46.9-77.9%) at 89.7% sensitivity (95% CI: 78.8-96.1%). Subgroup analysis for the independent validation cohort showed that the sensitivities for detecting various subgroups of nodule size ( 3cm: 100%; Unknown: 100%) and smoking history (Yes: 88.2%; No: 89.9%) all remained high among the lung cancer group. Additionally, the specificities for successfully identifying the benign nodules among different subgroups decrease as the size increase ( 3cm: 33.3%). Conclusions: Our cfDNA fragmentomics assay can provide a non-invasive approach to distinguish malignant nodules from the radiographically suspicious but pathologically benign nodules. Abstract:Colorectal cancer is frequently diagnosed in advanced stages, highlighting the need for developing approaches for early detection. Liquid biopsy using cell-free DNA (cfDNA) fragmentomics is a promising approach, but the clinical application is hindered by complexity and cost. This study aimed to develop an integrated model using cfDNA fragmentomics for accurate, cost-effective early-stage colorectal cancer detection. Plasma cfDNA was extracted and sequenced from a training cohort of 360 participants, including 176 patients with colorectal cancer and 184 healthy controls. An ensemble stacked model comprising five machine learning models was employed to distinguish patients with colorectal cancer from healthy controls using five cfDNA fragmentomic features. The model was validated in an independent cohort of 236 participants (117 patients with colorectal cancer and 119 controls) and a prospective cohort of 242 participants (129 patients with colorectal cancer and 113 controls). The ensemble stacked model showed remarkable discriminatory power between patients with colorectal cancer and controls, outperforming all base models and achieving a high area under the receiver operating characteristic curve of 0.986 in the validation cohort. It reached 94.88% sensitivity and 98% specificity for detecting colorectal cancer in the validation cohort, with sensitivity increasing as the cancer progressed. The model also demonstrated consistently high accuracy in within-run and between-run tests and across various conditions in healthy individuals. In the prospective cohort, it achieved 91.47% sensitivity and 95.58% specificity. This integrated model capitalizes on the multiplex nature of cfDNA fragmentomics to achieve high sensitivity and robustness, offering significant promise for early colorectal cancer detection and broad patient benefit. Significance: The development of a minimally invasive, efficient approach for early colorectal cancer detection using advanced machine learning to analyze cfDNA fragment patterns could expedite diagnosis and improve treatment outcomes for patients. Abstract:Lung cancer screening via annual low-dose computed tomography (LDCT) has poor adoption. We conducted a prospective case-control study among 958 individuals eligible for lung cancer screening to develop a blood-based lung cancer detection test that when positive is followed by an LDCT. Changes in genome-wide cell-free DNA (cfDNA) fragmentation profiles (fragmentomes) in peripheral blood reflected genomic and chromatin characteristics of lung cancer. We applied machine learning to fragmentome features to identify individuals who were more or less likely to have lung cancer. We trained the classifier using 576 cases and controls from study samples, and then validated it in a held-out group of 382 cases and controls. The validation demonstrated high sensitivity for lung cancer, and consistency across demographic groups and comorbid conditions. Applying test performance to the screening eligible population in a five-year model with modest utilization assumptions suggested the potential to prevent thousands of lung cancer deaths.
