Abstract:BackgroundTo evaluate the computed tomographic features and create a prediction model for clinical diagnosis of adenoid cystic carcinoma (ACC) in the palate with intact mucosa.MethodsFrom March 2016 to May 2018, 102 patients with palatal tumors and intact mucosa, including 28 patients with a pathological diagnosis of ACC after surgery, were enrolled in this study. The patients' clinical symptoms, computed tomographic features and pathological diagnoses were recorded and analyzed. Independent predictors of ACC were determined by using univariate analysis and multivariate logistic regression, and the discrimination and calibration of the prediction model was evaluated, and internal validation was performed.ResultsUnivariate analysis of patients showed that ACC patients were more likely than non-ACC patients to be older (P=0.019); to have palatine bone destruction (P<0.001) and greater palatine foramen (GPF) enlargement (P<0.001); to have involvement of the pterygopalatine fossa (P<0.001), foramen rotundum (P<0.001), nasal cavity (P<0.001) and maxillary bone (P<0.001); and to have numbness (P=0.007) and pain (P<0.001). Multivariate logistic analysis showed that age and GPF enlargement were independent predictors of ACC in palatal tumors. The diagnostic prediction model showed good discrimination and calibration, as evaluated by the area under the receiver operating characteristic curve (0.98) and the Hosmer-Lemeshow goodness-of-fit test (P=0.927).ConclusionsThe palate ACC prediction model based on age and GPF enlargement shows excellent discrimination with no evidence of poor calibration. Older patients with palatal tumors and intact mucosa should be considered for ACC when they have GPF enlargement.

Abstract:Objective To investigate the proportions and features of tumors,cysts and tumor-like lesions in the oral-maxillofacial region.Methods 9 056 cases of primary tumors,cysts and tumor-like lesions in the oral-maxillofacial region which were confirmed by pathological diagnosis(from January 1955 to December 2006 at the Department of Oral and Maxillofacial Surgery,Second Affiliated Hospital,School of Medicine,Zhejiang University) were retrospectively analyzed about their clinical pathological features,histological types,gender proportions and primary locations.Results There were 6 337 cases of tumors,2 363 cysts and 356 tumor-like lesions in the oral-maxillofacial region.Among the tumors,the benign tumor accounted 56.7%(3 596/6 337) while the malignant tumor took up 43.3%(2 747/6 337).Pleomorphic adenoma,hemangioma and ameloblastoma were the most common benign tumors.While the principle malignant tumor was carcinoma(86.2%,2 367/2 747),most of which were squamous cell carcinoma(50.0%,1 184/2 367),majorly in tongue,gingiva and cheek,with a gender ratio of 2.34∶1.Meanwhile,the most common benign and malignant salivary gland tumors were pleomorphic adenoma and adenocarcinoma,respectively,commonly sited in parotid gland,submandibular gland,palatine gland and parotid gland,palatine gland,submandibular gland,respectively.The dentigerous cyst(33.9%,408/1 202) and the sublingual gland cyst(41.1%,477/1 161) were the most common types of the odontogenic cysts and non-odontogenic cysts in the oral-maxillofacial region separately.The most frequent tumor-like lesion was epulis (54.8%,195/356).Conclusion Within this research,tumor is the most common lesion,with a number of begnin tumor more than malignant tumor,and the amount of male patients is larger than that of female patients with tumors,cysts or tumor-like lesions in the oral-maxillofacial region(261 words).

Abstract:We describe the case of a 37-year old male who presented with a schwannoma in his right palatine tonsil. The tumour and tonsil were surgically removed transorally. Tonsillar schwannomas are slow growing, and the main symptoms were a foreign body feeling in the throat, dysphagia and snoring. The presence of a round, well-defined mass lesion with heterogeneous density on computed tomography (CT) may aid in differentiation from other tonsillar tumours. The CT findings in the reported case were different to those of schwannomas in other locations; the particular CT manifestations of tonsillar schwannomas seem to relate to the various pathological changes seen within the tumour.

Abstract:Objectives: To evaluate the performance of texture anal-ysis (TA) of diffusion kurtosis imaging (DKI) in differen-tiating malignant from benign sinonasal lesions, and its added value to the conventional imaging features.Methods: Fifty -eight patients with malignant and 40 patients with benign sinonasal lesions were retro-spectively enrolled. Conventional CT and MRI features were reviewed. Texture parameters were obtained and compared between two groups. Multivariate logistic regression analysis was used to identify the most valu-able variables. Receiver operating characteristic curves were performed to assess the differentiating perfor-mance of independent variables and their combination.Results: There were significant differences in tumor necrosis, bone erosion and soft tissue invasion between the two groups (all p < 0.05). There were significant differences in the 10th and entropy of Apparent diffu-sion coefficient map, the mean, 10th and entropy of D map, the mean and 90th of K map between the two groups (all p < 0.002). The bone erosion, entropy of D, and mean of K were independent variables associated with malignant tumors. Receiver operating character-istic analyses indicated that the combination of three features possessed better differentiating performance than bone erosion alone (p = 0.003).Conclusion: TA of DKI could supply incremental value to conventional imaging features for pre-operative differ-ential diagnosis between benign and malignant sinon-asal lesions. Advances in knowledge: The present study is the first to combine conventional imaging features and the TA of DKI in the differential diagnosis between benign and malignant sinonasal lesions. Our findings suggest that TA of DKI could supply incremental value to conven-tional imaging features.

Abstract:Abstract Objective The purpose of this retrospective study was to describe the clinicopathological characteristics of primary adenoid cystic carcinoma (ACC) of the palate and to identify the factors affecting prognosis. Methods The medical records of 85 patients with primary ACC of the palate treated with surgery, with or without adjuvant radiotherapy/chemotherapy, from 2009 to 2019 were reviewed. The relationship of different clinical parameters with locoregional recurrence (LR), distant metastasis (DM), and overall survival (OS) were analyzed. Results Median follow‐up time was 44.6 months. LR and DM rates were 24.7% and 25.9%, respectively, and the 5‐year OS and disease‐free survival (DFS) rates were 85.9% and 55.1%, respectively. Multivariate analysis showed that positive margins were independently associated with the risk of LR ( p < .001). Positive margins ( p = .001) and high histological grade ( p = .031) were significantly associated with shorter OS. Conclusion Positive surgical margins are a strong adverse prognostic factor affecting LR and OS in patients with ACC; apart from that, high histopathological grade is an independent predictor of poor OS. Abstract:A 30-year-old woman presented with a sizable swelling on the left side of the hard palate, experiencing pressure sensations behind the left eyeball and within the left maxillary sinus. Additionally, she reported numbness in adjacent structures. These symptoms had been progressing over the course of 18 months before seeking the specialist's attention. The patient had previously undergone endodontic treatment on a left upper molar in an attempt to address the issue. Imaging examinations revealed a large area of soft tissue radiodensity enveloping the left eye orbit, nasal fossae, and extending into the medial and anterior left cranial fossa.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Objectives</h3><p class="a-plus-plus">Accurate preoperative differentiation between squamous cell carcinoma (SCC) and non-Hodgkin's lymphoma (NHL) in the palatine tonsil is crucial because of their different treatment. This study aimed to construct and validate a contrast-enhanced CT (CECT)–based radiomics nomogram for preoperative differentiation of SCC and NHL in the palatine tonsil.</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">This study enrolled 135 patients with a pathological diagnosis of SCC or NHL from two clinical centers, who were divided into training (n = 94; SCC = 50, NHL = 44) and external validation sets (n = 41; SCC = 22, NHL = 19). A radiomics signature was constructed from radiomics features extracted from routine CECT images and a radiomics score (Rad-score) was calculated. A clinical model was established using demographic features and CT findings. The independent clinical factors and Rad-score were combined to construct a radiomics nomogram. Performance of the clinical model, radiomics signature, and nomogram was assessed using receiver operating characteristics analysis and decision curve analysis.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">Eleven features were finally selected to construct the radiomics signature. The radiomics nomogram incorporating gender, mean CECT value, and radiomics signature showed better predictive value for differentiating SCC from NHL than the clinical model for training (AUC, 0.919 vs. 0.801, <em class="a-plus-plus">p</em> = 0.004) and validation (AUC, 0.876 vs. 0.703, <em class="a-plus-plus">p</em> = 0.029) sets. Decision curve analysis demonstrated that the radiomics nomogram was more clinically useful than the clinical model.</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusions</h3><p class="a-plus-plus">A CECT-based radiomics nomogram was constructed incorporating gender, mean CECT value, and radiomics signature. Abstract:Purpose: The aim of this study was to evaluate the volumetric characteristics of mucous retention cysts (MRCs) in the maxillary sinus and to analyze potential associations of MRCs with dentoalveolar pathologies. Materials and Methods: Cone-beam computed tomography (CBCT) scans exhibiting bilateral maxillary sinuses that were acquired from January 2016 to February 2019 were initially screened. A total of 227 scans (454 sinuses) that fulfilled the inclusion criteria were included. The presence, location, and volumetric characteristics of the diagnosed MRCs were evaluated on CBCT images using the 3D-Slicer software platform. The presence of MRCs was correlated with potential influencing factors including age, sex, and dentoalveolar pathology. For MRCs located on the sinus floor, factors with a potential impact on the volume, surface, and diameter were analyzed. Results: An MRC was present in 130 (28.6%) of the 454 sinuses. Most MRCs were located on the sinus walls and floor. The mean MRC volume, surface, and diameter were 551.21 +/- 1368.04 mm(3), 228.09 +/- 437.56 mm(2), and 9.63 +/- 5.40 mm, respectively. Significantly more sinuses with associated endodontically treated teeth/periapical lesions were diagnosed with an MRC located on the sinus floor. For MRCs located on the sinus floor, endodontic status exhibited a significant association with increased volume, surface, and diameter. Conclusion: Periapical lesions might be a contributing factor associated with the presence and volume of MRCs located on the sinus floor. The 3D-Slicer software platform was found to be a useful tool for clinicians to analyze the size of MRCs before surgical interventions such as sinus floor elevation procedures.

Abstract:A 66-year-old female patient attended the oral stomatology clinic in 2021, complaining of discomfort in the palate due to the presence of a lesion, noticed 60 days ago, which prevented the use of the prosthesis but did not cause painful symptoms. Clinically, an extensive lesion was observed on the hard palate, on the left side, with a sessile base, an ulcerated and smooth surface on palpation, pink and purple in color. Radiographically, there was no observed alteration of the bone tissue in the region.

The genetic profiling of bilateral ACC of unknown primary site and application in postoperative decision-making are less reported. Case Presentation: A 57-year-old male with a smoking history of over 30 years and multiple nodules in both lungs was present to our department. After assessing the bilateral solid nodules in his Positron Emission Tomography–Computed Tomography (PET/CT) scan, malignant lesions at the left lower lung, right lower lung, and right middle lung are suspected. Sequential selective video-assisted thoracoscopic surgeries (VATS) were performed. A genetic alteration test of 425 cancer-related genes and global gene expression profile of the specimens revealed intrapulmonary metastasis existed. The patient was followed up for three years without recurrence and tissue mutations in liquid biopsy. Conclusion: We present a way of omics-based multiple pulmonary lesions origin assessment, facilitating post-operative differential diagnosis and treatment decision for difficult cases. Abstract:Pulmonary adenoid cystic carcinoma (PACC) of the lung is a malignant tumor arising in the tracheobronchial glands distributed in the airway submucosa. The aim of this study is to investigate the molecular characteristics of PACC. From July 2013 to December 2016, 15 PACC patients received treatment. All the patients were diagnosed by pathology. We retrospectively reviewed the clinical data and genetic state. EGFR mutation rate was 6.67% (1/15), and it was 19del, the relationship between EGFR gene status and gender (P=1.000), age (P=1.000), smoking status (P=1.000) and stage (P=1.000) were no significant, and ALK fusion and ROS1 fusion gene was not detected. Gene change exists PACC, and the gene detection cannot be ignored in PACC.

Abstract:Objectives Lung adenocarcinoma associated with cystic airspaces (LACA) is a unique entity with limited understanding. Our aim was to evaluate the radiological characteristics of LACA and to study which criteria were predictive of invasiveness. Methods A retrospective monocentric analysis of consecutive patients with pathologically confirmed LACA was performed. The diagnosed adenocarcinomas were classified into preinvasive (atypical adenomatous hyperplasia, adenocarcinoma in situ, or minimally invasive adenocarcinoma) and invasive adenocarcinomas. Eight clinical features and twelve CT features were evaluated. Univariable and multivariable analyses were performed to analyse the correlation between invasiveness, and CT and clinical features. The inter-observer agreement was evaluated using κ statistics and intraclass correlation coefficients. The predictive performance of the model was evaluated using the area under the receiver operating characteristic curve (AUC). Results A total of 252 patients with 265 lesions (128 men and 124 women; mean age, 58.0 ± 11.1 years) were enrolled. Multivariable logistic regression indicated that multiple cystic airspaces (OR, 5.599; 95 % CI, 1.865–16.802), irregular shape of cystic airspace (OR, 3.236; 95 % CI, 1.073–9.761), entire tumour size (OR, 1.281; 95 % CI, 1.075–1.526), and attenuation (OR, 1.007; 95 % CI, 1.005–1.010) were independent risk factors for invasive LACA. The AUC of the logistic regression model was 0.964 (95 % CI, 0.944–0.985). Conclusion Multiple cystic airspaces, irregular shape of cystic airspace, entire tumour size, and attenuation were identified as independent risk factors for invasive LACA. The prediction model gives a good predictive performance, providing additional diagnostic information.
