Abstract:Background Traumatic haemarthrosis was hypothesized to be the etiology of temporomandibular (TMJ) ankylosis. Here, taking haematoma absorbance as a control, we aimed to reveal the molecular mechanisms involved in haematoma organizing into ankylosis using transcriptome microarray profiles. Material/methods Disk removal was performed to building haematoma absorbance (HA) in one side of TMJ, while removal of disk and articular fibrous layers was performed to induced TMJ ankylosis through haematoma organization (HO) in the contralateral side in a sheep model. Haematoma tissues harvested at days 1, 4 and 7 postoperatively were examined by histology, and analyzed by Affymetrix OviGene-1_0-ST microarrays. The DAVID were recruited to perform the Gene Ontology and Kyoto Encyclopedia of Genes and Genomes pathway analysis for the different expression genes (DEGs). The DEGs were also typed into protein-protein interaction (PPI) networks to get the interaction data. Six significant genes screened from PPI analysis, were confirmed by real-time PCR. Results We found 268, 223 and 17 DEGs at least twofold at days 1, 4 and 7, respectively. At day 1, genes promoting collagen ossification (POSTN, BGN, LUM, SPARC), cell proliferation (TGF-beta), and osteogenic differentiation of mesenchymal stem cells (BMP-2) were up-regulated in the HO side. At day 4, several genes involved in angiogenesis (KDR, FIT1, TEK) shower higher expression in the HO side. While HA was characterized by a continuous immune and inflammatory reaction. Conclusions Our results provide a comprehensive understanding of the role of haematoma in the onset and progress of TMJ ankylosis. The study will contribute to explaining why few injured TMJs ankylose and most do not from the molecular level.
Microarray Analysis of Differential Gene Expression Between Traumatic Temporomandibular Joint Fibrous and Bony Ankylosis in a Sheep Model

Abstract:BACKGROUND The type of traumatic temporomandibular joint (TMJ) ankylosis depends on the degree of severity of TMJ trauma. Here, we performed comprehensive differential molecular profiling between TMJ fibrous and bony ankylosis. MATERIAL AND METHODS Six sheep were used and a bilateral different degree of TMJ trauma was performed to induce fibrous ankylosis in one side and bony ankylosis in the other side. The ankylosed calluses were harvested at days 14 and 28 postoperatively and analyzed by Affymetrix OviGene-1_0-ST microarrays. DAVID was used to perform the Gene Ontology and Kyoto Encyclopedia of Genes and Genomes pathway analysis for the different expression genes (DEGs). The DEGs were also typed into protein-protein interaction (PPI) networks to get the interaction data. Ten DEGs, including 7 hub genes from PPI analysis, were confirmed by real-time PCR. RESULTS We found 90 and 323 DEGs at least 2-fold at days 14 and 28, respectively. At day 14, bony ankylosis showed upregulated DEGs, such as TLR8, SYK, NFKBIA, PTPRC, CD86, ITGAM, and ITGAL, indicating a stronger immune and inflammatory response and cell adhesion, while genes associated with anti-adhesion (PRG4) and inhibition of osteoblast differentiation (SFRP1) had higher expression in fibrous ankylosis. At day 28, bony ankylosis showed increased biological process related to new bone formation, while fibrous ankylosis was characterized by a prolonged immune and inflammatory reaction. CONCLUSIONS This study provides a differential gene expression profile between TMJ fibrous and bony ankylosis. Further study of these key genes may provide new ideas for future treatment of TMJ bony ankylosis.

Abstract:Purpose: Osteoarthritis (OA) is a multifaceted disease that involves pathological changes of multiple tissues in the joint. Most studies on cartilage matrix in OA have focused on changes of glycosaminoglycan and collagen content; very little is known about the alteration of collagen architecture in OA. However, the complex organization of collagen fibers in all layers of articular cartilage requires highly quantifiable imaging tools for visualization and analysis. One such tool is Second Harmonic Generation (SHG) imaging, which is a powerful two-photon confocal imaging technique ideal for imaging non-centrosymmetric molecules, such as collagen fibers. The purpose of this study is to use the SHG imaging method to evaluate collagen fiber thickness and orientation changes in early mouse experimental OA and to correlate these changes with synovial inflammation and joint destruction. Methods: OA was induced in NFκB-luciferase reporter mice (Taconic) via the destabilization of the medial meniscus (DMM) surgery. Mice were then subjected to three types of analysis between 1–10 weeks post-surgery: 1) SHG imaging. Collagen thickness and orientation were visualized using SHG imaging on cryosections. Fiber thickness was calculated as the ratio of forward to backward SHG signal (FSHG/BSHG), and directional orientation was calculated as variance of fiber orientation with respect to each other. 2) Histological analysis. Cartilage damage revealed by safranin O staining was assessed by the modified Mankin system. IHC was used to assess collagen content and macrophage presence. Synovitis in the anterior and posterior compartments of the joint were scored. 3) In vivo imaging of bioluminescence signals for NFκB activity to assess whole joint inflammation. Luciferin (25 μg) was injected intraarticularly, and mice were imaged with the IVIS Biophotonic 200 imager. As controls, sham and non-surgery mice were analyzed. Non-parametric data from Mankin scoring and synovitis analysis were evaluated via Kruskal-Wallis analysis followed by Dunn’s multiple comparison test. All other experiments were evaluated with ANOVA followed by Bonferroni’s multiple comparison test. Spearman’s correlation was performed for correlation analysis. P < 0.05: considered statistically significant. Results: Zonal differences in collagen fiber thickness and organization were observed as soon as OA initiated after DMM surgery, which persisted over time (Fig. A–C). Even at 1 week-post surgery, the DMM joint showed a decrease in collagen fiber thickness in the radial zone and an increase in collagen fiber disorganization in the superficial zone. Interestingly, in the radial zone, where the DMM joint showed decreased directional variance relative to the Sham joint, suggesting that the collagen fibers in the radial zone became more organized in OA. Bioluminescence imaging data showed that DMM and Sham control joints had a similar surge of inflammation at 1-week post-surgery, but the DMM joint exhibited a delay in resolution of inflammation in subsequent weeks. A similar trend was observed with synovitis, which we found to be mainly driven by synovial cell density and inflammatory infiltration rather than synovial thickness (data not shown). Interestingly, we found a correlation between the trend of collagen fiber thickness changes in the deep zone with that of synovitis in both the DMM and Sham knees; however, DMM surgery leads to an overall shift toward higher synovitis and thinner collagen fibers. Additionally, the trend of collagen directional variance in the superficial zone followed that of the Mankin score during OA progression (Fig. D). Conclusions: SHG imaging is a powerful approach that can be used to quantitatively analyze collagen landscape throughout articular cartilage. Combining SHG imaging with bioluminescence imaging and histological approaches, we uncovered a coordinated change between different components of cartilage matrix and inflammation in OA progression.

Abstract:Purpose: The chromatin protein HMGB2 plays important roles in development, remodeling and disruption of the articular cartilage. The objective of present study was to investigate whether the expression of HMGB2 in the condylar cartilage was correlated to the continuing forward mandibular positioning (FMP), and explore its potential relationship with β-catenin pathway. Methods: Fourty-four healthy adult Japanese white rabbits were used in this experiment. The right tempromandibular joints (TMJ) of rabbits were subjected to surgical and sham surgical operation of anterior disc displacement (ADD) (N=8). The TMJ condyles of rabbits were collected and fixed at 1-, 2-, 4-, and 8- weeks after the surgery. Histological micrographs of the condyle cartilage were mearsured by HE staining. The distribution and expression of HMGB2, Col-II, MMP1, MMP13 and β-catenin were evaluated through immunohistochemical staining. Results: Histological results by HE staining showed that the condyle cartilage turned thinner as well as disordered cell arrangement in cartilage with ADD. The expression of HMGB2 chiefly distributed in fibrous layer and proliferative layer of normal condylar cartilage. After ADD treatment, expression of HMGB2 and β-catenin enhanced at the 1 week ADD, and subsequently decreased at 2 and 4 weeks. COL-II and MMP13 expression also increased at the 1–2 weeks after ADD, suggesting osteoarthritis may occur in condylar cartilage. Most of the cartilages were repaired and chondrocytes were arranged in order at 8 weeks, suggesting spontaneous remodeling of condylar cartilage after external force loading. Conclusions: These findings suggest that HMGB2 may play important roles in chondrocytes following exposure to mechanical loading, which may involve in the pathogenesis of osteoarthritis.

Abstract:ObjectivesClinical and experimental studies show that the etiology of traumatic temporomandibular joint (TMJ) fibrous ankylosis and bony ankylosis are associated with the severity of trauma. However, how the injury severity affects the tissue differentiation is not clear. We tested the hypothesis that angiogenesis affects the outcomes of TMJ trauma, and that enhanced neovascularization after severe TMJ trauma would promote the development of bony ankylosis.MethodsBilateral condylar sagittal fracture and discectomy were performed for each sheep, with the glenoid fossa receiving either severe trauma to induce bony ankylosis or minor trauma to induce fibrous ankylosis. At days 7, 14, 28, and 56 after surgery, total RNA was extracted from the ankylosed callus. Temporal gene expressions of several molecules functionally important for blood vessel formation were studied by real-time PCR.ResultsHistological examination revealed a prolonged hematoma phase and a lack of cartilage formation in fibrous ankylosis. mRNA expression levels of HIF-1α, VEGF, VEGFR2, SDF1, Ang1, Tie2, vWF, CYR61, FGF2, TIMP1, MMP2, and MMP9 were distinctly lower in fibrous ankylosis compared with bony ankylosis at several time points.ConclusionsOur study indicates that inhibition of angiogenesis after TMJ trauma might be a promising strategy for preventing bony ankylosis in the future.

Abstract:Objective The purpose of the study was to preliminarily explore the differential expressions of a series of genes regulating bone formation in temporomandibular joint (TMJ) fibrous ankylosis, bony ankylosis and condylar fracture healing. Methods The cDNA from either the bony ankylosed callus or fracture callus of the 6 sheep, as described in the part I, were both used in the study. The differences of gene expressions between bony ankylosis and condylar fracture at 1, 3, and 6 months postoperatively were measured by real-time PCR, with 2 samples at each time point. In addition, another 2 sheep were added to have fibrous ankylosis induced on the right TMJ, and 1 sheep was sacrificed at 3 and 6 months after surgery, respectively. The differences of gene expressions between fibrous and bony ankylosis at 3 and 6 months postoperatively were measured by real-time PCR. Results Bony ankylosis showed higher mRNA expression trends in Wnt2b, Wnt5a, β-Catenin, Lef1, CyclinD1, Runx2, Osterix, Sox9, Col10a1, Alp, Ocn, Bmp2, and Bmp7 compared to fibrous ankylosis, although no statistical analysis was performed due to the very small sample size. Whereas bony ankylosis showed a significant lower expression of Wnt5a, β-Catenin, Lef1, Runx2, Osterix, Sox9, Col10a1, Alp, Ocn and Bmp4 compared to condylar fracture at several time points (P < 0.05). Conclusion Our data provided a preliminary molecular evidence for the hypothesis that the development of traumatic TMJ bony ankylosis was the course of delayed bone healing or hypertrophic nonunion, and deserved to be further studied.

Abstract:OBJECTIVE:To preliminarily investigate the temporal patterns of the endogenous mRNA expression for members of the Wnt signaling and a series of genes regulating bone formation during the development of traumatic temporomandibular joint (TMJ) bony ankylosis in a sheep model.METHODS:Six sheep were used for the induction of bony ankylosis of TMJ. We performed a condylar fracture, excision of the lateral 2/3 disc and serious injury to the glenoid fossa to induce bony ankylosis on the right TMJ. An isolated condylar fracture was performed on the left side. Two sheep were sacrificed at 1 month, 3 months, and 6 months after surgery, respectively. The specimens from the ankylosed joint and the condylar fracture were harvested for RNA extraction respectively. In this report (Part I), only the bony ankylosed samples were used for analysis of gene expressions. The specimens 1 month postoperatively were taken as the control, and the changes of expression of target genes over time were examined by real-time PCR.RESULTS:mRNA expression of Wnt1, Wnt2b, Wnt3a, β-catenin, Sfrp1, Lrp6, Lef1, CyclinD1, and Runx2 was up-regulated at 3 and 6 months compared with 1 month. The expression of Wnt5a, Sox9, and Osterix was up-regulated with a peak at 3 months, and then fell back to the basal levels at 6 months. The expression of Ocn began to up-regulate until 6 month postoperatively.CONCLUSION:Our findings suggested that Wnt signaling was involved in the formation of traumatic TMJ bony ankylosis and thus may be a potential therapeutic target for the treatment of the disease in the future.

Abstract:Objective: To study the expression of ADAMTs-5 and TIMP-3 in temporomandibular joint osteoarthritis (TMJOA) model rats, to explore and confer the possible effects of ADAMT5-5 and TIMP-3 involved in the degradation of the early stage of OA.Design: 32 SD rats were divided into four groups: 2-week control group (NC1), 2-week OA group (OA1), 4-week control group (NC2) and 4-week OA group (OA2). Each group had 8 rats. Injection of collagenase was used to build up the TMJOA model. HE staining was used to analyze the structural change of condyle cartilage. Western blot and RT-PCR were used to measure the expression of ADAMTs-5 and TIMP-3 in protein and mRNA levels respectively.Results: HE analysis revealed that no significant changes were observed in NC1, NC2 and OA1 groups, while mild damages appeared in OA2 group. No significant differences were achieved in the expression of ADAMTs-5 in protein levels between NC1 and OA1, but the expression of ADAMTs-5 in 4-week group increased significantly compared to that in the NC2 group. On mRNA level, the expression of ADAMTs-5 in 2-week and 4-week OA groups increased significantly compared to that in the matched control group. Meanwhile, the expression of TIMP-3 decreased significantly, showing a completely different trend.Conclusions: The expression of ADAMTs-5 and TIMP-3 changed significantly in the early stage of TMJOA, which indicated that ADAMTs-5 and TIMP-3 may be play an important part in the initial stage of condylar cartilage degradation. Early Subchondral Bone Changes in an Osteoarthritis Model in Wild Type and Tgf-Alpha Knockout Mice

Abstract:Purpose: To examine early subchondral bone changes in the DMM (destabilization of medial meniscus) model of injury-induced osteoarthritis (OA) in wild type (Wt) as well as in TGFalpha knockout (KO) mice. Methods: C57/Bl6 Wt and TGFalpha KO mice were subjected to DMM or SHAM surgery at 12-weeks of age and harvested at 2-, 5- and 10-weeks post-surgery. Catwalk gait analyses, Micro-Computed Tomography (μCT), Toluidine Blue (TB), Picrosirius Red (PR) and Tartrate-Resistant Acidic Phosphatase (TRAP) staining of paraffin-embedded sections were used to investigate gait patterns, 3D joint morphology, bone mineral density (BMD) of subchondral bone, histology, collagen organization and activity of osteoclasts. Results: In Wt mice, OA severity progressed from mild or moderate (5-week post-surgery) to severe (10-week post-surgery) according to OARSI histopathology scoring. Gait disparity occurred only at 10-week post-surgery in DMM mice but not at earlier time-points. Osteophyte formation around the surgical joint was noticed as early as 2-week following DMM and became more severe at 5- and 10-week by histology and μCT. BMD of the subchondral bone of the medial side of the surgical knees (tibia plateau and femoral condyle) increased significantly 5-week after surgery, but not at 10-week, compared to the sham knees. TRAP staining showed increased osteoclast activity in the subchondral bone of surgical knees at both time points but not in the sham knees. In KO mice, our preliminary data showed protective effects of TGFalpha KO for cartilage following DMM. However, μCT showed more severe osteophyte formation around the surgical joint in DMM mice compared to Wt mice. Conclusions: BMD of subchondral bone increased at the early stages of OA and reverted to baseline at latter stages. Gait disparity in mice only occurred at later stages of OA. Osteoclast activity increased during OA progression. TGFalpha KO might be protective for articular cartilage but detrimental for bone.

Abstract:Objective To explore the changes in gene expression and biological process of the osteoarthritis (OA) induced by anterior cruciate ligament (ACL) transection and partial medial meniscectomy,so as to provide bioinformatic basis for further studying the molecular mechanism of OA.Methods The gene chip datasets of a rat model of early 0A induced by ACL transection and partial medial meniscectomy were downloaded from GEO databases (submitted by Appleton,et al.).The differential expression genes (DEGs)were identified,and the Gene ontology(GO) as well as the Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses for DEGs were conducted using bioinformatic methods.Results A total of 170 DEGs including 97 up-regulated genes and 73 down-regulated genes were identified.The up-regulated genes were mainly enriched in the extracellular matrix (ECM) and were closely related to the ECM-receptor interaction,while the down-regulated genes were mainly enriched in the biological function of muscle contraction and were linked with the peroxisome proliferators-activated receptor (PPAR) signaling pathway.Conclusion The changes of ECM and muscle contraction play a key role in the occurrence and development of OA.The ECM-receptor interaction and PPAR signaling pathway are strongly associated with OA and worthy of further study.
Establishment and Histological Evaluation of a Goat Traumatic Temporomandibular Joint Model.

Abstract:Purpose: To establish a goat model for traumatic temporomandibular joint (TMJ) and describe the gross appearance and histopathologic changes for the prognostic comparison of different types of TMJ trauma.Materials and Methods: According to clinical traumatic types, 16 adult goats were randomly divided into 4 groups based on outcome of surgical induction: 1) disc displacement without reduction (DD), 2) condylar cartilage defect (CCD), 3) DD plus CCD, and 4) control. TMJs were removed and observed 2 weeks and 1 month after the operation. These specimens were compared and evaluated based on gross appearance and histopathologic observations with hematoxylin and eosin and toluidine blue stains. The histopathologic score of condylar cartilage was observed and compared across the 4 groups using SPSS 17.0 software.Result: Three traumatic TMJ goat models were successfully established. At 2 weeks and 1 month after surgical induction, the DD group showed slightly irregular cartilage compared with the C group, with disordered cartilage composition, little angiogenesis, and proteoglycan degradation. The CCD group showed fibrous adhesion between the condyle and the disc without reparative cartilage tissues; the remaining cartilage was normal in composition and proteoglycan content. The DD + CCD group showed fibrous adhesions and irregular condylar cartilage with disordered cartilage composition and decreased matrix proteoglycans. The scores of cartilage degeneration among the 4 groups showed significant differences at 2 weeks and 1 month (DD + CCD > DD > CCD; P < .05).Conclusion: Among the 3 traumatic TMJ models, DD plus CCD can cause osteoarthritis (OA) and adhesions in the joint, DD can cause only OA, and CCD with the disc in normal position merely affects the remnant cartilage. Early Changes of Articular Cartilage and Subchondral Bone in The DMM Mouse Model of Osteoarthritis

Abstract:To examine the early changes of articular cartilage and subchondral bone in the DMM mouse model of osteoarthritis, mice were subjected to DMM or SHAM surgery and sacrificed at 2-, 5- and 10-week post-surgery. Catwalk gait analyses, Micro-Computed Tomography, Toluidine Blue, Picrosirius Red and Tartrate-Resistant Acid Phosphatase (TRAP) staining were used to investigate gait patterns, joint morphology, subchondral bone, cartilage, collagen organization and osteoclasts activity, respectively. Results showed OA progressed over 10-week time-course. Gait disparity occurred only at 10-week post-surgery. Osteophyte formed at 2-week post-surgery. BMDs of DMM showed no statistical differences comparing to SHAM at 2 weeks, but BV/TV is much higher in DMM mice. Increased BMD was clearly found at 5- and 10-week post-surgery in DMM mice. TRAP staining showed increased osteoclast activity at the site of osteophyte formation of DMM joints at 5- and 10-week time points. These results showed that subchondral bone turnover might occurred earlier than 2 weeks in this mouse DMM model. Gait disparity only occurred at later stage of OA in DMM mice. Notably, patella dislocation could occur in some of the DMM mice and cause a different pattern of OA in affected knee.
Comparative transcriptomic analysis of articular cartilage of post-traumatic osteoarthritis models

Abstract:Animal models of post-traumatic osteoarthritis (PTOA) recapitulate the pathological changes observed in human PTOA. Here, skeletally mature C57Bl6 mice were subjected to either rapid-onset non-surgical mechanical rupture of the anterior cruciate ligament (ACL) or to surgical destabilisation of the medial meniscus (DMM). Transcriptome profiling of micro-dissected cartilage at day 7 or day 42 following ACL or DMM procedure, respectively, showed that the two models were comparable and highly correlative. Gene ontology (GO) enrichment analysis identified similarly enriched pathways that were overrepresented by anabolic terms. To address the transcriptome changes more completely in the ACL model, we also performed small RNA sequencing, describing the first microRNA profile of this model. miR-199-5p was amongst the most abundant, yet differentially expressed, microRNAs, and its inhibition in primary human chondrocytes led to a transcriptome response that was comparable to that observed in both human 'OA damaged vs intact cartilage' and murine DMM cartilage datasets. We also experimentally verified CELSR1, GIT1, ECE1 and SOS2 as novel miR-199-5p targets. Together, these data support the use of the ACL rupture model as a non-invasive companion to the DMM model.
