CD1C+ REGULATORY B CELL THERAPY FOR GRAFT-VERSUS-HOST DISEASE: A TALE OF MESENCHYMAL STROMAL CELL-BASED INDUCTION SYSTEM

Abstract:Background & AimBackground & Aim: Graft versus host disease (GVHD) remains a potentially life-threatening complication post-hematopoietic stem cell transplantation, largely owing to limited therapeutic options. IL-10+ regulatory B cells (Bregs) show promising potential for treating GVHD. However, how to obtain the high-quality human IL-10+ Bregs in vitro is still a major issue limiting their translational applications.Methods, Results & Conclusion: MethodsKey pathways or regulatory molecules for MSC induction of Bregs were determined via RNA-seq expression profiling data, and an MSC-based Bregs induction system was created. Using spectral flow cytometry and high-dimensional data analysis, the characteristic surface markers of induced Bregs were identified, and a Bregs enrichment strategy was developed. In vitro and in vivo models were used to explore the immunosuppressive function of induced Bregs and evaluate their application in the treatment of GVHD.ResultsAccording to RNA-seq expression profiling data analysis, we established an efficient Bregs induction system in vitro based on mesenchymal stromal cells (MSCs) and Wnt signaling pathway activator (CHIR-99021), which had a robust capacity to induce IL-10+ Bregs while suppressed their TNF-α expression. Furthermore, we found that these induced Bregs could be identified and enriched by specific CD1c+ phenotype via high-dimensional immunophenotyping assays using spectral flow cytometry. Importantly, induced CD1c+ Bregs suppressed the proliferation of PBMC and the inflammatory cytokines secretion of T cells. When adoptive transferred to a humanized mouse model of GVHD, induced CD1c+ Bregs effectively alleviated GVHD through the improvement of survival, weight loss, clinical score and histopathologic damages. Mechanistically, cAMP response element binding protein (CREB) was the pivotal modulator for inducing Bregs in the system. MSC-derived IGF-1 and cell-to-cell contact promoted the phosphorylation of CREB in B cells and enhanced the IL-10+ Bregs induction.ConclusionOverall, we proposed an efficient ex vivo induction system for human Bregs, which has implications in developing novel Bregs-based therapies for GVHD.
