Abstract:Although more and more cardiovascular risk factors have been verified in peritoneal dialysis (PD) populations in different countries and regions, it is still difficult for clinicians to accurately and individually predict death in the near future. We aimed to develop and validate machine learning-based models to predict near-term all-cause and cardiovascular death. Machine learning models were developed among 7539 PD patients, which were randomly divided into a training set and an internal test set by five random shuffles of 5-fold cross-validation, to predict the cardiovascular death and all-cause death in 3 months. We chose objectively collected markers such as patient demographics, clinical characteristics, laboratory data, and dialysis-related variables to inform the models and assessed the predictive performance using a range of common performance metrics, such as sensitivity, positive predictive values, the area under the receiver operating curve (AUROC), and the area under the precision recall curve. In the test set, the CVDformer models had a AUROC of 0.8767 (0.8129, 0.9045) and 0.9026 (0.8404, 0.9352) and area under the precision recall curve of 0.9338 (0.8134,0.9453) and 0.9073 (0.8412, 0.9164) in predicting near-term all-cause death and cardiovascular death, respectively. The CVDformer models had high sensitivity and positive predictive values for predicting all-cause and cardiovascular deaths in 3 months in our PD population. Further calibration is warranted in the future.
Machine Learning for the Prediction of First-Year Mortality in Incident Hemodialysis Patients

Abstract:Rationale: The current mortality risk scores for hemodialysis (HD) patients employ regression techniques and have limited applicability and robustness. Objective: We aimed to develop a machine learning model utilizing clinical factors to predict first-year mortality in HD patients that could assist physicians in classifying high-risk patients.Methods: Training and testing cohorts consisted of 5351 incident HD patients from a single center and 5828 incident HD patients from 97 renal centers, respectively. The outcome was all-cause mortality during the first year of dialysis. eXtreme Gradient Boosting (XGBoost) was used for algorithm training and validation. Two models were established based on the data obtained at dialysis initiation (model 1) and data from 0-3 months after dialysis initiation (model 2). Ten-fold cross-validation was applied to each model. The C statistic (area under the receiver operating characteristic curve, AUC) was used to assess the predictive ability of the models.Results: In the training and testing cohorts, 585 (10.93%) and 764 (13.11%) patients, respectively, died during the first-year follow-up. Fifteen most important features were selected from 42 candidate features. The performance of model 1 (C statistic of 0.8318, 95% CI 0.7771 - 0.8374) was similar to that of model 2 (C statistic of 0.8530, 95% CI 0.8057 - 0.8593).Conclusions: We developed and validated two machine learning models to predict first-year mortality in HD patients. CONCLUSIONS:We developed and validated 2 machine learning models to predict first-year mortality in patients undergoing hemodialysis. Both models could be used to stratify high-risk patients at the early stages of dialysis.

Abstract:Abstract Background and Aims Few studies including unplanned dialysis starts have used machine learning for the prediction of death in dialysis patients. The objective of the study was to use R software algorithms to develop machine learning predictive models for the identification of death-related factors in patients undergoing hemodialysis (HD) and peritoneal dialysis (PD). Method This study included adult patients undergoing HD and PD started in a planned or urgent manner in a dialysis center between January 2014 and January 2019. Epidemiological, clinical and laboratory data were collected. Univariate analysis was followed by ML-based analyses. Then, multivariate regressions were obtained using stepwise and Cox regression analyses. Finally, a Random Forest predictive model was generated after variables with missing values >30% were removed. Results Of 581 patients included, 170 died (29,2%). On univariate analysis death was associated with age, number of comorbidities, dialysis modality switching, creatinine, PTH and albumin values at dialysis initiation, presence of diabetes (DM), hospitalization, function recovery and central venous catheter (CVC) for dialysis access. Patients who started dialysis with a CVC had a worse survival (p = 0.0034) than those who did not use CVC, started HD with AVF, or received PD (Figure 1). Data were split into 20% for testing the regression model, and 80% for training the model. Data preprocessing for Cox regression included imputing some values using bag impute (decision trees), creating dummy variables, and removing collinear variables. Death was associated with older age (p < 0.001), fewer ESI-free months (p < 0.001) and lower initial creatinine (p = 0.008) (Table 1). The model C-index was 0.8099. Random forest ranked the following variables predictive of death in descending order of importance: ESI-free months; age; initial levels of creatinine, PTH and albumin; number of comorbidities; dialysis-related infection; initial phosphorus and hemoglobin; hospitalizations; male gender; modality switching (Figure 2). The agreement of the model obtained was 0.8110. Conclusion ESI-free months, age and initial levels of creatinine were associated with death on both multivariate and ML-based analyses.

Abstract:OBJECTIVE:The aim of this study is to analyze and visualize blood pressure (BP) patterns during continuous hemodialysis (HD) sessions, referred to as multiple-session patterns (MSPs), and explore whether deep learning models with MSPs have better performance.MATERIAL AND METHODS:Data from 3.79 million hemodialysis BP records collected from July 30, 2007, to August 25, 2016, were obtained from the health system's electronic health records. We analyzed BP patterns during 36 continuous HD sessions (approximately 3 months) and selected 1311 (survival: 1246; death: 65) end-stage renal disease patients to classify 1-year outcomes (survival or death). Convolution kernels of different sizes were used to construct convolutional neural networks to recognize MSPs and BP patterns during a single HD session, referred to as single-session patterns (SSPs). BP patterns corresponded to convolution kernels and were represented and visualized as the input patches that activate the feature maps most. We used global average pooling (GAP) to measure the overall response of the inputs to each convolution kernel (pattern). The weights of the fully connected layers after GAP can measure the correlations between the convolution kernels (patterns) and the classification results. We solved the problem of data imbalance with a two-phase training strategy.RESULTS:The F1_score was 0.782 ± 0.058 (95% CI) in the models with SSPs and was approximately 19.5% higher (0.977 ± 0.014, 95% CI) in the models with MSPs.CONCLUSIONS:The results indicated that consistent with previous studies, patients with lower BPs and longer HD sessions have better prognoses. BP patterns during continuous HD sessions can represent patients' 1-year mortality risk better than BP patterns during a single HD session and therefore improve the performance of prediction models.
Deep Learning Identifies Intelligible Predictors of Poor Prognosis in Chronic Kidney Disease

Abstract:Early diagnosis and prediction of chronic kidney disease (CKD) progress within a given duration are critical to ensure personalized treatment, which could improve patients' quality of life and prolong survival time. In this study, we explore the intelligibility of machine-learning and deep-learning models on end-stage renal disease (ESRD) prediction, based on readily-accessible clinical and laboratory features of patients suffering from CKD. Eight machine learning models were used to predict whether a patient suffering from CKD would progress to ESRD within three years based on demographics, clinical,and comorbidity information. LASSO, random forest, and XGBoost were used to identify the most significant markers. In addition, we introduced four advanced attribution methods to the deep learning model to enhance model intelligibility. The deep learning model achieved an AUC-ROC of 0.8991, which was significantly higher than that of baseline models. The interpretation generated by deep learning with attribution methods, random forest, and XGBoost was consistent with clinical knowledge, whereas LASSO-based interpretation was inconsistent. Hematuria, proteinuria, potassium, urine albumin to creatinine ratio were positively associated with the progression of CKD, while eGFR and urine creatinine were negatively associated. In conclusion, deep learning with attribution algorithms could identify intelligible features of CKD progression. Our model identified a number of critical, but under-reported features, which may be novel markers for CKD progression. This study provides physicians with solid data-driven evidence for using machine learning for CKD clinical management and treatment.

Abstract:Chronic kidney disease (CKD) can be diagnosed by chronic deterioration in kidney function associated with renal parenchymal damage and may progress to end-stage renal disease (ESRD), [ 1 ] which has become a major public health concern. More than 800 million individuals are affected by CKD in 2017, and CKD was predicted to be the fifth leading cause of years of life lost. [ 2 , 3 ] CKD patients had higher mortality rates, [ 4 ] and CKD represents a major public health issue in Taiwan. Wen et al revealed that more than 10% of the Taiwanese population may be affected by CKD. [ 5 ] Comorbidities, such as cardiovascular disease (CVD) and congestive heart failure, not only exacerbate CKD progression but also impact the quality of life and mortality in CKD patients. [ 1 ] Common treatment options for ESRD patients include kidney transplantation and dialysis. [ 6 ] Despite advances in dialysis technology and cardiovascular therapies, cardiovascular-related mortality due to cardiac arrhythmias remains the largest cause of death among patients undergoing general dialysis. In ESRD patients treated with hemodialysis, the incidence of congestive heart failure had increased to 77.3% in 2021. [ 7 ] Given the high morbidity and mortality of these intertwined diseases, the use of hospice palliative care for patients with advanced CKD and CVD is critical to ensure that patients could make informed medical decisions and receive optimal treatment for their needs. Similar to CVD and congestive heart failure, the progression from pre-ESRD to ESRD is highly variable and can follow an unpredictable trajectory. [ 8 , 9 ] Consequently, it is important to understand the progression trajectory to properly initiate and integrate palliative care services, such as the management of patient symptoms and discussion of renal replacement therapy options, including dialysis and non-dialysis management for advanced CKD patients. [ 10 , 11 ] There are already numerous studies on the epidemiology and short-term survival of ESRD patients, [ 12–14 ] but the research on the long-term survival of non-cancer patients with advanced CKD is scarce. Machine learning (ML) has been applied successfully to different chronic diseases, such as chronic liver diseases, CVD, chronic obstructive pulmonary disease, and metabolic syndrome. [ 15–23 ] Furthermore, several studies in the past 2 years have utilized time-series ML techniques to predict the COVID-19 pandemic trend. [ 20 ] Previous studies also used unsupervised learning and network visualization of big data for chronic disease diagnoses. [ 24 ] Machine learning techniques were used in several research about renal problems such as CKD early detection in the EHR database or prediction for acute kidney injury. Several studies have been conducted on the use of artificial intelligence (AI) in ESRD and CKD. [ 20 , 25–29 ] However, these studies mostly focused on CKD prevention or identification. [ 21 , 30–33 ] For mortality prediction in ESRD patients, most studies have focused on populations with only kidney transplant therapy or hemodialysis. [ 34–36 ] Therefore, this study intends to construct a mortality prediction model with AI for pre-ESRD and ESRD patients.

Abstract:Approximately half of all patients with advanced CKD who progress to kidney failure initiate dialysis in an unplanned fashion, which is associated with high morbidity, mortality, and health care costs. A novel prediction model designed to identify patients with advanced CKD who are at high risk for developing kidney failure over short time frames (6–12 months) may help reduce the rates of unplanned dialysis and improve the quality of transitions from CKD to kidney failure.

Abstract:The purpose of this study was to assess the feasibility of machine learning (ML) in predicting the risk of end-stage kidney disease (ESKD) from patients with chronic kidney disease (CKD). Data were obtained from a longitudinal CKD cohort. Predictor variables included patients’ baseline characteristics and routine blood test results. The outcome of interest was the presence or absence of ESKD by the end of 5 years. Missing data were imputed using multiple imputation. Five ML algorithms, including logistic regression, naïve Bayes, random forest, decision tree, and K-nearest neighbors were trained and tested using fivefold cross-validation. The performance of each model was compared to that of the Kidney Failure Risk Equation (KFRE). The dataset contained 748 CKD patients recruited between April 2006 and March 2008, with the follow-up time of 6.3 ± 2.3 years. ESKD was observed in 70 patients (9.4%). Three ML models, including the logistic regression, naïve Bayes and random forest, showed equivalent predictability and greater sensitivity compared to the KFRE. The KFRE had the highest accuracy, specificity, and precision. This study showed the feasibility of ML in evaluating the prognosis of CKD based on easily accessible features. Three ML models with adequate performance and sensitivity scores suggest a potential use for patient screenings. Future studies include external validation and improving the models with additional predictor variables.
Data-driven, two-stage machine learning algorithm-based prediction scheme for assessing 1-year and 3-year mortality risk in chronic hemodialysis patients

Abstract:Life expectancy is likely to be substantially reduced in patients undergoing chronic hemodialysis (CHD). However, machine learning (ML) may predict the risk factors of mortality in patients with CHD by analyzing the serum laboratory data from regular dialysis routine. This study aimed to establish the mortality prediction model of CHD patients by adopting two-stage ML algorithm-based prediction scheme, combined with importance of risk factors identified by different ML methods. This is a retrospective, observational cohort study. We included 800 patients undergoing CHD between December 2006 and December 2012 in Shin-Kong Wu Ho-Su Memorial Hospital. This study analyzed laboratory data including 44 indicators. We used five ML methods, namely, logistic regression (LGR), decision tree (DT), random forest (RF), gradient boosting (GB), and eXtreme gradient boosting (XGB), to develop a two-stage ML algorithm-based prediction scheme and evaluate the important factors that predict CHD mortality. LGR served as a bench method. Regarding the validation and testing datasets from 1- and 3-year mortality prediction model, the RF had better accuracy and area-under-curve results among the five different ML methods. The stepwise RF model, which incorporates the most important factors of CHD mortality risk based on the average rank from DT, RF, GB, and XGB, exhibited superior predictive performance compared to LGR in predicting mortality among CHD patients over both 1-year and 3-year periods. We had developed a two-stage ML algorithm-based prediction scheme by implementing the stepwise RF that demonstrated satisfactory performance in predicting mortality in patients with CHD over 1- and 3-year periods. The findings of this study can offer valuable information to nephrologists, enhancing patient-centered decision-making and increasing awareness about risky laboratory data, particularly for patients with a high short-term mortality risk.

Identifying Patients With Heart Failure Who Are Susceptible to De Novo Acute Kidney Injury: Machine Learning Approach

Abstract:Background: Studies have shown that more than half of patients with heart failure (HF) with acute kidney injury (AKI) have newonset AKI, and renal function evaluation markers such as estimated glomerular filtration rate are usually not repeatedly tested during the hospitalization. As an independent risk factor, delayed AKI recognition has been shown to be associated with the adverse events of patients with HF, such as chronic kidney disease and death. Objective: The aim of this study is to develop and assess of an unsupervised machine learning model that identifies patients with HF and normal renal function but who are susceptible to de novo AKI. Methods: We analyzed an electronic health record data set that included 5075 patients admitted for HF with normal renal function, from which 2 phenogroups were categorized using an unsupervised machine learning algorithm called K-means clustering. We then determined whether the inferred phenogroup index had the potential to be an essential risk indicator by conducting survival analysis, AKI prediction, and the hazard ratio test. Results: The AKI incidence rate in the generated phenogroup 2 was significantly higher than that in phenogroup 1 (group 1: 106/2823, 3.75%; group 2: 259/2252, 11.50%; P<.001). The survival rate of phenogroup 2 was consistently lower than that of phenogroup 1 (P<.005). According to logistic regression, the univariate model using the phenogroup index achieved promising performance in AKI prediction (sensitivity 0.710). The generated phenogroup index was also significant in serving as a risk indicator for AKI (hazard ratio 3.20, 95% CI 2.55-4.01). Consistent results were yielded by applying the proposed model on an external validation data set extracted from Medical Information Mart for Intensive Care (MIMIC) III pertaining to 1006 patients with HF and normal renal function. Conclusions: According to a machine learning analysis on electronic health record data, patients with HF who had normal renal function were clustered into separate phenogroups associated with different risk levels of de novo AKI. Our investigation suggests that using machine learning can facilitate patient phengrouping and stratification in clinical settings where the identification of high-risk patients has been challenging.
Predicting early mortality in hemodialysis patients: a deep learning approach using a nationwide prospective cohort in South Korea

Abstract:Early mortality after hemodialysis (HD) initiation significantly impacts the longevity of HD patients. This study aimed to quantify the effect sizes of risk factors on mortality using various machine learning approaches. A cohort of 3284 HD patients from the CRC-ESRD (2008–2014) was analyzed. Mortality risk models were validated using logistic regression, ridge regression, lasso regression, and decision trees, as well as ensemble methods like bagging and random forest. To better handle missing data and time-series variables, a recurrent neural network (RNN) with an autoencoder was also developed. Additionally, survival models predicting hazard ratios were employed using survival analysis techniques. The analysis included 1750 prevalent and 1534 incident HD patients (mean age 58.4 ± 13.6 years, 59.3% male). Over a median follow-up of 66.2 months, the overall mortality rate was 19.3%. Random forest models achieved an AUC of 0.8321 for first-year mortality prediction, which was further improved by the RNN with autoencoder (AUC 0.8357). The survival bagging model had the highest hazard ratio predictability (C-index 0.7756). A shorter dialysis duration (< 14.9 months) and high modified Charlson comorbidity index scores (7–9) were associated with hazard ratios up to 7.76 (C-index 0.7693). Comorbidities were more influential than age in predicting early mortality. Monitoring dialysis adequacy (KT/V), RAAS inhibitor use, and urine output is crucial for assessing early prognosis.

Abstract:Introduction: Cardiovascular disease (CVD) is the leading cause of death in patients with chronic kidney disease (CKD). This study aimed to develop CVD risk prediction models using machine learning to support clinical decision making and improve patient prognosis. Methods: Electronic medical records from patients with CKD at a single center from 2015 to 2020 were used to develop machine learning models for the prediction of CVD. Least absolute shrinkage and selection operator (LASSO) regression was used to select important features predicting the risk of developing CVD. Seven machine learning classification algorithms were used to build models, which were evaluated by receiver operating characteristic curves, accuracy, sensitivity, specificity, and F1-score, and Shapley Additive explanations was used to interpret the model results. CVD was defined as composite cardiovascular events including coronary heart disease (coronary artery disease, myocardial infarction, angina pectoris, and coronary artery revascularization), cerebrovascular disease (hemorrhagic stroke and ischemic stroke), deaths from all causes (cardiovascular deaths, non-cardiovascular deaths, unknown cause of death), congestive heart failure, and peripheral artery disease (aortic aneurysm, aortic or other peripheral arterial revascularization). A cardiovascular event was a composite outcome of multiple cardiovascular events, as determined by reviewing medical records. Results: This study included 8,894 patients with CKD, with a composite CVD event incidence of 25.9%; a total of 2,304 patients reached this outcome. LASSO regression identified eight important features for predicting the risk of CKD developing into CVD: age, history of hypertension, sex, antiplatelet drugs, high-density lipoprotein, sodium ions, 24-h urinary protein, and estimated glomerular filtration rate. The model developed using Extreme Gradient Boosting in the test set had an area under the curve of 0.89, outperforming the other models, indicating that it had the best CVD predictive performance. Conclusion: This study established a CVD risk prediction model for patients with CKD, based on routine clinical diagnostic and treatment data, with good predictive accuracy. This model is expected to provide a scientific basis for the management and treatment of patients with CKD.

Abstract:Background: Cardiac surgery-associated acute kidney injury (CSA-AKI) is a major complication that results in short- and long-term mortality among patients. Here, we adopted machine learning algorithms to build prediction models with the overarching goal of identifying patients who are at a high risk of such unfavorable kidney outcomes. Methods: A total of 1686 patients (development cohort) and 422 patients (validation cohort), with 126 pre- and intra-operative variables, were recruited from the First Medical Centre and the Sixth Medical Centre of Chinese PLA General Hospital in Beijing, China, respectively. Analyses were performed using six machine learning techniques, namely K-nearest neighbor, logistic regression, decision tree, random forest (RF), support vector machine, and neural network, and the APPROACH score, a previously established risk score for CSA-AKI. For model tuning, optimal hyperparameter was achieved by using GridSearch with 5-fold cross-validation from the scikit-learn library. Model performance was externally assessed via the receiver operating characteristic (ROC) and decision curve analysis (DCA). Explainable machine learning was performed using the Python SHapley Additive exPlanation (SHAP) package and Seaborn library, which allow the calculation of marginal contributory SHAP value. Results: 637 patients (30.2%) developed CSA-AKI within seven days after surgery. In the external validation, the RF classifier exhibited the best performance among the six machine learning techniques, as shown by the ROC curve and DCA, while the traditional APPROACH risk score showed a relatively poor performance. Further analysis found no specific causative factor contributing to the development of CSA-AKI; rather, the development of CSA-AKI appeared to be a complex process resulting from a complex interplay of multiple risk factors. The SHAP summary plot illustrated the positive or negative contribution of RF-top 20 variables and extrapolated risk of developing CSA-AKI at individual levels. The Seaborn library showed the effect of each single feature on the model output of the RF prediction. Conclusions: Efficient machine learning approaches were successfully established to predict patients with a high probability of developing acute kidney injury after cardiac surgery. These findings are expected to help clinicians to optimize treatment strategies and minimize postoperative complications. The Prediction of In-Hospital Mortality in Chronic Kidney Disease Patients with Coronary Artery Disease Using Machine Learning Models

Abstract:Objective Chronic kidney disease (CKD) patients with coronary artery disease (CAD) in the intensive care unit (ICU) have higher in-hospital mortality and poorer prognosis than patients with either single condition. The objective of this study is to develop a novel model that can predict the in-hospital mortality of that kind of patient in the ICU using machine learning methods. Methods Data of CKD patients with CAD were extracted from the Medical Information Mart for Intensive Care IV (MIMIC-IV) database. Boruta algorithm was conducted for the feature selection process. Eight machine learning algorithms, such as logistic regression (LR), random forest (RF), Decision Tree, K-nearest neighbors (KNN), Gradient Boosting Decision Tree Machine (GBDT), Support Vector Machine (SVM), Neural Network (NN), and Extreme Gradient Boosting (XGBoost), were conducted to construct the predictive model for in-hospital mortality and performance was evaluated by average precision (AP) and area under the receiver operating characteristic curve (AUC). Shapley Additive Explanations (SHAP) algorithm was applied to explain the model visually. Moreover, data from the Telehealth Intensive Care Unit Collaborative Research Database (eICU-CRD) were acquired as an external validation set. Results 3590 and 1657 CKD patients with CAD were acquired from MIMIC-IV and eICU-CRD databases, respectively. A total of 78 variables were selected for the machine learning model development process. Comparatively, GBDT had the highest predictive performance according to the results of AUC (0.946) and AP (0.778). The SHAP method reveals the top 20 factors based on the importance ranking. In addition, GBDT had good predictive value and a certain degree of clinical value in the external validation according to the AUC (0.865), AP (0.672), decision curve analysis, and calibration curve. Conclusion Machine learning algorithms, especially GBDT, can be reliable tools for accurately predicting the in-hospital mortality risk for CKD patients with CAD in the ICU. This contributed to providing optimal resource allocation and reducing in-hospital mortality by tailoring precise management and implementation of early interventions.

Abstract:Background Patients with end-stage renal disease (ESRD) undergoing hemodialysis (HD) exhibit a high mortality risk, particularly at the onset of treatment. Conventional risk assessment models, dependent on extensive temporal data accumulation, frequently encounter issues of data incompleteness and lengthy collection periods. Objective This study addresses the imbalance in short-term HD data and the issue of missing data features, achieving a robust assessment of mortality risk for HD patients over the subsequent 30 to 450 days. Methods An autoencoder-based mortality prediction model for HD patients is proposed. Leveraging the manifold structure of the non-missing features and the intrinsic relationship between the features in the high-dimensional data space, the model infers the values of the missing features. Noise and redundant information typically distort the manifold structure, impacting the accuracy of inferences about missing features. Consequently, we generate feature dropping masks to simulate the missing data distribution in the deep learning framework and design an autoencoder, forming an adaptive feature extraction module. The module utilizes readily available short-term data for unsupervised learning, enabling the encoder to reconstruct missing features and derive latent representations. Finally, a classifier based on the latent representations achieves the mortality prediction. Results Over a 30-day observation window, the model demonstrated superior mortality prediction performance compared to other models across all prediction windows. Feature importance analysis showed that creatinine and age are consistently the most critical features across all prediction windows. Glucose (fasting) and platelet count also remain significant, with their correlation with mortality risk increasing over time. Serum albumin, international standard ratio, and phosphate are particularly important for short-term predictions, while conjugated bilirubin and prothrombin time gain prominence in mid- and long-term predictions. Conclusion The proposed model proficiently leverages short-term HD data to provide precise mortality risk evaluations in HD patients, with particular efficacy in the short-term. Its application holds considerable value for clinical decision-making and risk management in this patient population.

Abstract:Background: This study aimed to develop and validate a machine learning (ML) model based on serum Klotho for predicting end-stage kidney disease (ESKD) and cardiovascular disease (CVD) in patients with chronic kidney disease (CKD). Methods: Five different ML models were trained to predict the risk of ESKD and CVD at three different time points (3, 5, and 8-year) using a cohort of 400 non-dialysis CKD patients. The dataset was divided into a training set (70%) and an internal validation set (30%). These models were informed by data comprising 47 clinical features, including serum Klotho. The best-performing model was selected and used to identify risk factors for each outcome. Model performance was assessed using various metrics. Results: The findings showed that the Lasso regression model had the highest accuracy (C-index=0.71) in predicting ESKD. The features mainly included in this model were estimated glomerular filtration rate (eGFR), 24-hour urinary microalbumin, serum albumin, phosphate, parathyroid hormone, and serum Klotho, which achieved the highest area under the curve (AUC) of 0.930 (95% CI: 0.897-0.962). In addition, for the CVD risk prediction, the Random Survival Forest (RSF) model with the highest accuracy (C-index=0.66) was selected and achieved the highest AUC of 0.782 (95% CI: 0.633-0.930). The features mainly included in this model were age, history of primary hypertension, calcium, tumor necrosis factor-alpha, and serum Klotho. Conclusion: We successfully developed and validated Klotho-based ML risk prediction models for CVD and ESKD in CKD patients with good performance, indicating their high clinical utility.

Abstract:Objective: Peritoneal Dialysis (PD) is one of the most widely used life-supporting therapies for patients with End-Stage Renal Disease (ESRD). Predicting mortality risk and identifying modifiable risk factors based on the Electronic Medical Records (EMR) collected along with the follow-up visits are of great importance for personalized medicine and early intervention. Here, our objective is to develop a deep learning model for a real-time, individualized, and interpretable mortality prediction model - AICare. Method and Materials: Our proposed model consists of a multi-channel feature extraction module and an adaptive feature importance recalibration module. AICare explicitly identifies the key features that strongly indicate the outcome prediction for each patient to build the health status embedding individually. This study has collected 13,091 clinical follow-up visits and demographic data of 656 PD patients. To verify the application universality, this study has also collected 4,789 visits of 1,363 hemodialysis dialysis (HD) as an additional experiment dataset to test the prediction performance, which will be discussed in the Appendix. Results: 1) Experiment results show that AICare achieves 81.6%/74.3% AUROC and 47.2%/32.5% AUPRC for the 1-year mortality prediction task on PD/HD dataset respectively, which outperforms the state-of-the-art comparative deep learning models. 2) This study first provides a comprehensive elucidation of the relationship between the causes of mortality in patients with PD and clinical features based on an end-to-end deep learning model. 3) This study first reveals the pattern of variation in the importance of each feature in the mortality prediction based on built-in interpretability. 4) We develop a practical AI-Doctor interaction system to visualize the trajectory of patients' health status and risk indicators.

Abstract:AIMS: Diabetic kidney disease (DKD) is the most common cause of end-stage renal disease (ESRD) and is associated with increased morbidity and mortality in patients with diabetes. Identification of risk factors involved in the progression of DKD to ESRD is expected to result in early detection and appropriate intervention and improve prognosis. Therefore, this study aimed to establish a risk prediction model for ESRD resulting from DKD in patients with type 2 diabetes mellitus (T2DM).METHODS: Between January 2008 and July 2019, a total of 390 Chinese patients with T2DM and DKD confirmed by percutaneous renal biopsy were enrolled and followed up for at least 1 year. Four machine learning algorithms (gradient boosting machine, support vector machine, logistic regression, and random forest (RF)) were used to identify the critical clinical and pathological features and to build a risk prediction model for ESRD.RESULTS: There were 158 renal outcome events (ESRD) (40.51%) during the 3-year median follow up. The RF algorithm showed the best performance at predicting progression to ESRD, showing the highest AUC (0.90) and ACC (82.65%). The RF algorithm identified five major factors: Cystatin-C, serum albumin (sAlb), hemoglobin (Hb), 24-hour urine urinary total protein, and estimated glomerular filtration rate. A nomogram according to the aforementioned five predictive factors was constructed to predict the incidence of ESRD.CONCLUSION: Machine learning algorithms can efficiently predict the incident ESRD in DKD participants. Compared with the previous models, the importance of sAlb and Hb were highlighted in the current model.HighlightsWhat is already known? Identification of risk factors for the progression of DKD to ESRD is expected to improve the prognosis by early detection and appropriate intervention.What this study has found? Machine learning algorithms were used to construct a risk prediction model of ESRD in patients with T2DM and DKD. In contrast with the treatment of participants with early-phase T2DM with or without mild kidney damage, major emphasis should be placed on indicators of kidney function, nutrition, anemia, and proteinuria for participants with T2DM and advanced DKD to delay ESRD, rather than age, sex, and control of hypertension and glycemia.
