METHODS We prospectively collected medical information of 40 consecutive robotic posterior retroperitoneal anatomical adrenalectomies (RPRAA) performed by a single surgeon between December 2014 and July 2015, and compared them with 216 consecutive cases of laparoscopic posterior retroperitoneal anatomical adrenalectomies (LPRAA) performed by the same surgeon between January 2010 to November 2014. RESULTS A total of 256 cases (RPRAA: 40; LPRAA: 216) were included in this study. Though tumor volume in RPRAA group was significantly larger than LPRAA group, estimated blood loss was significantly lower in RPRAA group (mean: 70.6 ml) than in LRPAA group (155.6 ml). No significant difference was observed in operative time, complications, and hospital stay. Postoperative pathology revealed no significant difference in tumor histology between the two groups. CONCLUSIONS We detailed our standardized technique of RPRAA in a step-by-step manner, allowing for smooth implementation of the procedure, consistent surgical outcomes, and optimization of the learning curve. Successful surgery relies on several key steps, including suitable trocar configuration/angulation.
