The TNM stage proportion would be the most important and independent prognostic factor of overall mortality, and no more than 20% of incident gastric cancers in China were diagnosed at an early stage.4Chen X.Z. et al.Med Oncol. 2014; 31: 315Crossref PubMed Scopus (23) Google Scholar Although the stage proportion was unreported among the observations in the trial, the proportion of early-stage cancers might be not significantly different between active eradication and placebo groups by extrapolation. During follow-up in the trial, the interval of endoscopic surveillance was up to 5 years, which was inefficient to detect early disease and improve the proportion of early-stage cancers. Active eradication of H pylori combined with organized screening and surveillance of high-risk populations should be systematic measures to control gastric cancer–specific mortality, especially in high-risk areas. The latest expert consensus5Du Y. et al.Chin J Gastroenterol. 2018; 23: 92-97Google Scholar on early gastric cancer screening in China (2017) defined the candidates as age ≥40 years and at least 1 of the following risk factors: high-prevalence area, H pylori infection, previous stomach disease (atrophic gastritis, gastric ulcer, gastric polyps, gastric stump, Ménétrier disease, and pernicious anemia, etc), first-degree relative of someone with gastric cancer, and other risk factors (ie, excessive salt intake, pickled diet, smoking, heavy drinking, etc). Annual endoscopic screening was suggested in the high-risk subpopulation, together with serologic evaluation.
