Abstract:This chapter provides an overview of the application of synthetic polymeric biomaterials in musculoskeletal regenerative and reconstructive medicine with selected examples. Hydrogels capable of cell and growth factor delivery were assessed for injectable cartilage tissue engineering. Injectable composite scaffolds incorporating bone morphogenetic protein-2 (BMP-2) were developed to regenerate critical-sized defects in a rat femoral defect model. Advanced scaffold design was achieved by computer aided stereolithography. Nerve tubes with multiple parallel channels were investigated as guidance scaffolds for peripheral nerve and spinal cord regeneration. All of these demonstrated the versatility of synthetic polymers as multifunctional, interactive, and biodegradable bioscaffolds with virtually unlimited possibility for performance optimization through polymer and surface chemistry modification and physical manipulation.

Abstract:Reconstruction of large bone defects resulting from trauma, neoplasm, or infection is a challenging problem in reconstructive surgery. The need for bone grafting has been increasing steadily partly because of our enhanced capability to salvage limbs after major bone loss. Engineered bone graft substitutes can have advantages such as lack of antigenicity, high availability, and varying properties depending on the applications chosen for use. These favorable attributes have contributed to the rise of scaffold-based polymeric tissue regeneration. Critical components in the scaffold-based polymeric regenerative engineering approach often include (1) the existence of biodegradable polymeric porous structures with properties selected to promote tissue regeneration and while providing appropriate mechanical support during tissue regeneration, (2) cellular populations that can influence and enhance regeneration, and (3) the use of growth and morphogenetic factors which can influence cellular migration, differentiation, and tissue regeneration in vivo. Biodegradable polymers constitute an attractive class of biomaterials for the development of scaffolds due to their flexibility in chemistry and their ability to produce biocompatible degradation products. This paper presents an overview of polymeric scaffold-based bone tissue regeneration and reviews approaches as well as the particular roles of biodegradable polymers currently in use.Lay SummaryBiomaterials have become an indispensable tool used in biomedical applications ranging from scaffolds for regenerative engineering to controlled drug delivery and immunomodulation. Regenerative engineering is a developing multidisciplinary field of research that employs the principles of advanced materials science, stem cell science, physics, developmental biology, and clinical translation for the regeneration of damaged tissues. Abstract:This paper reviews recent work involving polymeric biomaterials used for skin, cartilage, bone, vascular, nerve and liver regeneration. Skin trauma involves damage to the epidermal, dermal and/or subdermal tissues. Epithelial, dermal and full-thickness replacements are considered. Cartilage research is mainly focused on replacing hyaline cartilage. Researchers investigate both nondegradable polymers, which must provide mechanical stability, and degradable polymers, which must support cartilage regeneration. Natural healing in large bone defects often fails. Materials for bone reconstruction must be biocompatible, offer mechanical properties similar to those of bone and support tissue regeneration. The area of vascular grafts draws attention as improvements to the patency of existing materials are needed. Studies to improve current vascular graft polymers as well as develop new polymers are reviewed below. The design and testing of materials for nerve regeneration, to repair damage caused by illness or accident, is an active area of research. Directional nerve guidance via tubulation is discussed, as are matrix materials to enhance axonal extension. Finally, liver transplantation remains one of the only options for chronic liver disease and the demand for liver transplants far exceeds the number of available organs. The complexity of parameters involved in liver regeneration is presented here.

Abstract:The past decade has seen the rapid development of new strategies for the design of biodegradable macromolecular compounds, with properly suited architecture and tailored properties, functioning as temporary support for the engineering of living constructs in tissue regeneration applications. The purpose of this paper is to review recent research in the interdisciplinary field of tissue engineering, with particular regard to bone and cartilage tissues, aimed at the design, synthesis, evaluation and characterization of bioactive polymeric scaffolds guiding and promoting new tissue ingrowth. Current strategies in scaffold-guided tissue engineering approach, involving the most employed biodegradable polymers, either of natural or synthetic origin, will be reported underlying the role played by both material structure–property relationship and scaffold architecture. While there are many polymeric materials that may be employed for the regeneration of bone and cartilage tissue, we will focus specifically on those that have been more extensively applied, showing promising outcomes. Commonly exploited and innovative processing techniques for the fabrication of advanced tissue engineering scaffolds will be explored, highlighting theoretical principles and their potential in creating micro-nanostructures suitable for tissue regeneration applications.

Abstract:Tissue Engineering (TE) is an interdisciplinary field that encompasses materials science in combination with biological and engineering sciences. In recent years, an increase in the demand for therapeutic strategies for improving quality of life has necessitated innovative approaches to designing intelligent biomaterials aimed at the regeneration of tissues and organs. Polymeric porous scaffolds play a critical role in TE strategies for providing a favorable environment for tissue restoration and establishing the interaction of the biomaterial with cells and inducing substances. This article reviewed the various polymeric scaffold materials and their production techniques, as well as the basic elements and principles of TE. Several interesting strategies in eight main TE application areas of epithelial, bone, uterine, vascular, nerve, cartilaginous, cardiac, and urinary tissue were included with the aim of learning about current approaches in TE. Different polymer-based medical devices approved for use in clinical trials and a wide variety of polymeric biomaterials are currently available as commercial products. However, there still are obstacles that limit the clinical translation of TE implants for use wide in humans, and much research work is still needed in the field of regenerative medicine.

Abstract:Collagen is commonly used as a regenerative biomaterial due to its excellent biocompatibility and wide distribution in tissues. Different kinds of hybridization or cross-links are favored to offer improvements to satisfy various needs of biomedical applications. Previous reviews have been made to introduce the sources and structures of collagen. In addition, biological and mechanical properties of collagen-based biomaterials, their modification and application forms, and their interactions with host tissues are pinpointed. However, there is still no review about collagen-based biomaterials for tissue engineering. Therefore, we aim to summarize and discuss the progress of collagen-based materials for tissue regeneration applications in this review. We focus on the utilization of collagen-based biomaterials for bones, cartilages, skin, dental, neuron, cornea, and urological applications and hope these experiences and outcomes can provide inspiration and practical techniques for the future development of collagen-based biomaterials in related application fields. Moreover, future improving directions and challenges for collagen-based biomaterials are proposed as well.

Abstract:Native tissues possess unparalleled physiochemical and biological functions, which can be attributed to their hybrid polymer composition and intrinsic bioactivity. However, there are also various concerns or limitations over the use of natural materials derived from animals or cadavers, including the potential immunogenicity, pathogen transmission, batch to batch consistence and mismatch in properties for various applications. Therefore, there is an increasing interest in developing degradable hybrid polymer biomaterials with controlled properties for highly efficient biomedical applications. There have been efforts to mimic the extracellular protein structure such as nanofibrous and composite scaffolds, to functionalize scaffold surface for improved cellular interaction, to incorporate controlled biomolecule release capacity to impart biological signaling, and to vary physical properties of scaffolds to regulate cellular behavior. In this review, we highlight the design and synthesis of degradable hybrid polymer biomaterials and focus on recent developments in osteoconductive, elastomeric, photoluminescent and electroactive hybrid polymers. The review further exemplifies their applications for bone tissue regeneration.

Biomimetic Polyphosphate Materials: Toward Application in Regenerative Medicine

Abstract:In recent years, inorganic polyphosphate (polyP) has attracted increasing attention as a biomedical polymer or biomaterial with a great potential for application in regenerative medicine, in particular in the fields of tissue engineering and repair. The interest in polyP is based on two properties of this physiological polymer that make polyP stand out from other polymers: polyP has morphogenetic activity by inducing cell differentiation through specific gene expression, and it functions as an energy store and donor of metabolic energy, especially in the extracellular matrix or in the extracellular space. No other biopolymer applicable in tissue regeneration/repair is known that is endowed with this combination of properties. In addition, polyP can be fabricated both in the form of a biologically active coacervate and as biomimetic amorphous polyP nano/microparticles, which are stable and are activated by transformation into the coacervate phase after contact with protein/body fluids. PolyP can be used in the form of various metal salts and in combination with various hydrogel-forming polymers, whereby (even printable) hybrid materials with defined porosities and mechanical and biological properties can be produced, which can even be loaded with cells for 3D cell printing or with drugs and support the growth and differentiation of (stem) cells as well as cell migration/microvascularization. Potential applications in therapy of bone, cartilage and eye disorders/injuries and wound healing are summarized and possible mechanisms are discussed.

Abstract:Abstract Nowadays, biomaterials have evolved from the inert supports or functional substitutes to the bioactive materials able to trigger or promote the regenerative potential of tissues. The interdisciplinary progress has broadened the definition of ‘biomaterials’, and a typical new insight is the concept of tissue induction biomaterials. The term ‘regenerative biomaterials’ and thus the contents of this article are relevant to yet beyond tissue induction biomaterials. This review summarizes the recent progress of medical materials including metals, ceramics, hydrogels, other polymers and bio-derived materials. As the application aspects are concerned, this article introduces regenerative biomaterials for bone and cartilage regeneration, cardiovascular repair, 3D bioprinting, wound healing and medical cosmetology. Cell-biomaterial interactions are highlighted. Since the global pandemic of coronavirus disease 2019, the review particularly mentions biomaterials for public health emergency. In the last section, perspectives are suggested: (i) creation of new materials is the source of innovation; (ii) modification of existing materials is an effective strategy for performance improvement; (iii) biomaterial degradation and tissue regeneration are required to be harmonious with each other; (iv) host responses can significantly influence the clinical outcomes; (v) the long-term outcomes should be paid more attention to; (vi) the noninvasive approaches for monitoring in vivo dynamic evolution are required to be developed; (vii) public health emergencies call for more research and development of biomaterials; and (viii) clinical translation needs to be pushed forward in a full-chain way. In the future, more new insights are expected to be shed into the brilliant field—regenerative biomaterials.

Abstract:With the barrier materials, guided tissue regeneration materials and hybrid artificial organs as the representafive materials, the applications of polymer skeletal materials in biomedical fields are introduced in this paper. Based on the results of the induced carcinoma, collagenous encapsulation and chronic inflammation around implanted polymers, the effects of polymers topography on biocompatibility are discussed, and the importance of topographical compatibility of materials is emphasized.

Abstract:The introduction of new materials for the production of various types of constructs that can connect directly to tissues has enabled the development of such fields of science as medicine, tissue, and regenerative engineering. The implementation of these types of materials, called biomaterials, has contributed to a significant improvement in the quality of human life in terms of health. This is due to the constantly growing availability of new implants, prostheses, tools, and surgical equipment, which, thanks to their specific features such as biocompatibility, appropriate mechanical properties, ease of sterilization, and high porosity, ensure an improvement of living. Biodegradation ensures, among other things, the ideal rate of development for regenerated tissue. Current tissue engineering and regenerative medicine strategies aim to restore the function of damaged tissues. The current gold standard is autografts (using the patient's tissue to accelerate healing), but limitations such as limited procurement of certain tissues, long operative time, and donor site morbidity have warranted the search for alternative options. The use of biomaterials for this purpose is an attractive option and the number of biomaterials being developed and tested is growing rapidly.

Abstract:The advancement in material science and fabrication techniques has led to the booming of tissue engineering in recent years, covering tissue culture, repair, regeneration, and the rest. Among the aforementioned, considering the indispensable roles of central/peripheral nerve and soft connective tissues playing in life-sustaining activities, their regenerations have attracted intense research attention, especially using polymeric scaffolds, an easy-to-access, cost efficient, biocompatible and diverse family of engineering materials. Herein, commonly used natural and synthetic polymeric materials for tissue scaffolds are outlined. Specially, polymer-based hybrids, being able to provide both structural support and bio-microenvironments, are discussed. Additionally, representative manufacturing approaches for polymeric scaffolds, including freeze-drying, electrospinning, 3D printing, soft lithography among others are highlighted. Notably, combined techniques (e.g., electrospinning, 3D printing, etc.) allowing tailorable structural configurations of composite polymeric scaffolds are also reviewed. Furthermore, recent achievements in central/peripheral nerve and soft connective tissues regenerations using polymeric scaffolds are discussed in detail. In the end, conclusions and outlooks are provided to draw a roadmap for future research.
Advanced biocompatible polymers for cartilage tissue engineering

Abstract:Biocompatible polymer hydrogels had high potential for applications in cartilage tissue engineering. This chapter introduced cartilage disease and current clinical treatments. The requirements, advantages, drawbacks, and methods for polymer hydrogels applied in cartilage regeneration were summarized. In addition, we focused on the latest progress and clinical application of polymer hydrogels for cartilage tissue engineering, including silk fibroin, alginate, chitosan, hyaluronate, γ-PGA, PEG, PCL, and PVA. The advances of simulation models of hydrogels were also discussed. Furthermore, perspectives and some key challenges on future hydrogels for cartilage regeneration are also proposed.

Abstract:Polymer membranes have been widely used in guided tissue regeneration (GTR) and guided bone regeneration (GBR). In this review, various commercially available membranes are described. Much attention is paid to the recent development of biodegradable polymers applied in GTR and GBR, and the important issues of biodegradable polymeric membranes, including their classification, latest experimental research and clinical applications, as well as their main challenges are addressed. Herein, natural polymers, synthetic polymers and their blends are all introduced. Pure polymer membranes are biodegradable and biocompatible, but they lack special properties such as antibacterial properties, osteoconductivity, and thus polymer membranes loaded with functional materials such as antibacterial agents and growth factors show many more advantages and have also been introduced in this review. Despite there still being complaints about polymer membranes, such as their low mechanical properties, uncontrollable degradation speed and some other drawbacks, these problems will undoubtedly be conquered and biodegradable polymers will have more applications in GTR and GBR.

Abstract:Periodontal tissue regeneration is the ultimate goal of the treatment for periodontitis-affected teeth. The success of regenerative modalities relies heavily on the utilization of appropriate biomaterials with specific properties. Poly (lactic-<i>co</i>-glycolic acid) (PLGA), a synthetic aliphatic polyester, has been actively investigated for periodontal therapy due to its favorable mechanical properties, tunable degradation rates, and high biocompatibility. Despite the attractive characteristics, certain constraints associated with PLGA, in terms of its hydrophobicity and limited bioactivity, have led to the introduction of modification strategies that aimed to improve the biological performance of the polymer. Here, we summarize the features of the polymer and update views on progress of its applications as barrier membranes, bone grafts, and drug delivery carriers, which indicate that PLGA can be a good candidate material in the field of periodontal regenerative medicine.
