Are Hispanic Subarachnoid Hemorrhage Patients in Border States Treated Differently Than Those Admitted in Non-Border States? (P2.134)

Abstract:Objective_ To determine if there are treatment disparities in subarachnoid hemorrhage patients in Border and non-Border States and if this correlates to higher hospital charges. Background_ Recent studies have found an underutilization of hospital in-patient care for coronary artery disease in Hispanics living in Border States. Methods _ We identified Hispanic and non-Hispanic subarachnoid hemorrhage patients from the Nationwide Inpatient Sample-2011. We determined the rate of utilization of endovascular and surgical treatments, hospital charges, and outcomes according to patient's demographics and whether or not they lived in a Border state (defined as California, Arizona, New Mexico and Texas). Results- A total of 18,368 patients were admitted with subarachnoid hemorrhage including 2,310 (12.6%) Hispanic patients; of those 1525 were admitted in Border states and 785 were in non-Border states. Patient demographics, risk factors, complications and discharge disposition are included in the Table below. There was a significant difference in Border states in rates of surgical treatment between Hispanic patients (21.9%) versus non-Hispanic (13.9%), p=0.02. In the non-Bordered states, the rates of surgical treatment was similar among Hispanic and non-Hispanic patients (14% versus 15.6%, p=0.6. When comparing the hospital charges between Border state Hispanics and non-border state Hispanics, the difference was significant ($219, 260 versus $192,418; p<0.001). Conclusions_ There is a unique pattern of utilization of surgical treatment in Hispanic patients residing in Border states, which leads to a significant increase in hospital charges in this patient population.
