Abstract:According to the strategic aim of establishing the health care system through Universal Coverage,the status of medical assistance will be weakened.Medical assistance is designed only for people whose poverty caused by diseases and other special group.However,the medical security system which is dominated by social insurance cannot solve the problem of medical expenditure for the poor radically.Therefore,medical assistance will still play an important role until the health care system through universal coverage is established.At present,the medical assistance does not fully defuse disease risks,because of the unclear concept of medical assistance.Based on the aim of establishing health care system through universal coverage and medical reform,this paper defines some basic concepts,such as the basic risk,the minimum demand and government responsibility.Then starting with the social risk defused by medical assistance,it analyses the basic risk which still should be defused by medical assistance when realizing the aim.Taking the new social assistance system which covers urban and rural in Zhejiang for example,this paper discusses two key problems when the concept of medical assistance is positioned,for promoting equity and efficiency.

Abstract:Health equity is the basic theory of universal health insurance and the objective of reform of the medical welfare system. Based on health equity theory with the perspective of giving priority to disadvantaged groups, this article takes Zhejiang province as an example and adopts a mixed-method approach to explore the effects of the reform of the medical welfare system on health equity for older people. The structural equation model (SEM)-based nonrecursive path analysis demonstrates an improvement of health equity for insured elders of the Basic Medical Insurance for Employees (BMIE) and the Urban Basic Medical Insurance for Residents (UBMIR) as a result of an increased utilization of healthcare. However, there is little difference in terms of the utilization of healthcare and health outcomes between old people insured by the New Rural Cooperative Medical System (NRCMS) and those without medical insurance. This results from the low-level benefits of the insurance policy and defective policy design. A qualitative analysis further discovers the insufficient protection of health equity for several types of vulnerable aged groups in the process of healthcare delivery. Finally, this study advocates the perspective of health equity for vulnerable groups, a transition from emphasizing opportunity equity to stressing outcome equity, and the construction of universal health insurance and long-term care system for the disabled elderly.

Abstract:This study introduced medical insurance system for urban and town residents in some cities,and analyzed adverse selection problem caused by high risk of the safeguarded object and forms taken by the voluntary participation. It is thought that the gradual integration between the methodology of risk avoidance and the medical insurance system of urban and town residents is easy to take mandatory measures for dispersing risk.Therefore,the following suggestions were put forward: firstly,the national and integrated suggestions on implementing and guiding are introduced to improve level of overall planning;secondly,the government strengthens investment to public health and community health services.

Abstract:Nowadays, the shortage of the binary systems of society security is becoming more obvious in its innovation and practice. This paper is based on the expatiation of the strategic sense of the construction of urban-rural Harmonious social security system, and the referencing the development experience and revelation of foreign social security systems. With synthetically utilizing theories and technology means, such as social security basic theory, demography, actuarial science, life-table method and system simulation, the "Three-Step" covering strategy theory from urban to rural area of social security system is presented in this paper. The research explains how to alter the actual binary system to the future Harmonious system of social security. "The First Step" is that the social security system covering urban and rural area will be realized in 2020. It adheres to the principle of "Wide-Covering, Levels-Dividing, Different and Continual" and its backbone is to establish new rural social security system. "The Second Step" will be taken during from 2021 to 2035. The main task of this step will aim at founding a social security linking urban and county in grand regions divided into three parts: East, Middle and West. It takes the concept of "making overall plans while taking all factors into consideration, and dynamically linking" and the parameters of pension substitute rates and yield ratios. "The Third Step" is to try to realize the harmony social security system stably before the century celebration of PRC in 2049. The security system will link the region in the range of the whole state in stead of the three grand regions.

Abstract:This paper explores the necessities and feasibilities of establishing a system of basic social medical insurance by urban-rural integration in regions whose economic development is at different levels.Based on the survey data in 2011,this paper analyzes and compares the residents' willingness to choose medical insurance and its influential demographic characters and calculates the change of government health expenditure after the establishment of the system of medical insurance by urban-rural integration in Liaoning and Shanxi Provinces with the methods of contingency table,linear regression and discriminant analysis.The common demographic characters that influenced the residents ' choice of medical insurance in the above-mentioned two regions included household,age,commercial insurance,occupation and education.The deductible lines of insurances for working and non-working urban residents were expected to be reduced,and the premium and deductible lines of the new type of rural cooperative medical care system could be increased.Shanxi provincial government could save 10.9815 billion yuan and Liaoning provincial government could save 3.6214 billion yuan,if the medical insurances in each region reached the current average level of Beijing Municipality and the residents were allowed to choose medical insurances at their own will.It is necessary and feasible to integrate the current three basic medical insurances into three different levels of medical insurance under a unified system and allow the residents to be free to make choices in the regions where the economic development is at different levels.

Abstract:The establishment of a multi-level medical security policy system have a more important impact on poverty alleviation in the post-poverty era. This study uses the third-party assessment survey data from Hubei and Jiangxi provinces and the 2018 National Sixth National Health Service Statistical Survey Report. From the perspective of disease risk and medical burden, assess the risk of poverty due to illness and medical security needs of the poor, and sort out the transition path and policy system of medical security policies for difficult groups in the post-poverty era, and deeply analyze the implementation effect and existing problems of current medical security poverty alleviation policies. and on this basis, the construction of the multi-level medical security poverty alleviation policy system is expounded from the aspects of social medical insurance and medical assistance, public health prevention and commercial medical insurance.
Perceptions of the benefits of the basic medical insurance system among the insured: a mixed methods research of a northern city in China

Abstract:Background The perceptions of the benefits of the basic medical insurance system among the insured not only reflect the system's performance but also the public's basic medical insurance policy literacy, valuable information for countries that have entered the stage of deepening reform. This study aims to examine the factors that affect the perceptions of the benefits of the basic medical insurance system in China, diagnose the key problems, and propose corresponding measures for improvement. Methods A mixed method design was used. Data for the quantitative study were obtained from a cross-sectional questionnaire survey ( n = 1,045) of residents of Harbin who had enrolled for basic medical insurance system. A quota sampling method was further adopted. A multivariate logistic regression model was then employed to identify the factors influencing the perceptions of the benefits of the basic medical insurance system, followed by semi-structured interviews with 30 conveniently selected key informants. Interpretative phenomenological analysis was used to analyze the interview data. Results Approximately 44% of insured persons reported low perceptions of benefits. The logistic regression model showed that low perceptions of the benefits of the basic medical insurance system was positively correlated with the experience of daily drug purchases (OR = 1.967), perceptions of recognition with basic medical insurance system (OR = 1.948), perceptions of the financial burden of participation costs (OR = 1.887), perceptions of the convenience of using basic medical insurance for medical treatment (OR = 1.770), perceptions of the financial burden of daily drug purchases costs (OR = 1.721), perceptions of the financial burden of hospitalization costs (OR = 1.570), and type of basic medical insurance system (OR = 1.456). The results of the qualitative analysis showed that the key problem areas of perceptions of the benefits of the basic medical insurance system were: (I) system design of basic medical insurance; (II) intuitive cognition of the insured; (III) rational cognition of the insured; and (IV) the system environment. Conclusions Improving the perceptions of the benefits of the basic medical insurance system of the insured requires joint efforts in improving system design and implementation, exploring effective publicity methods of basic medical insurance system information, supporting public policy literacy, and promoting the health system environment.

The integrations between the two schemes could relieve the current"difficult and costly access to healthcare services for the poor",and it has a great realistic significance to improve the accessibility of healthcare services and equality. Based on the status quo of the two schemes integrations,this paper makes the analyses on the necessities and possibilities of the integrations.Moreover,the current existed difficulties and problems of the integrations are studied with relative suggestions put forward.

Abstract:Background: Based on CSS 2019 data, this article analyzes the impact of Basic Medical Insurance for Urban and Rural Residents (BMIUR) on perceived social equity of residents. Method: Using the CSS data of 2019, this article analyzes the influence of BMIUR on the perception of social equity of residents, on the basis of 2SLS model and mediating effect model. Results: We find that BMIUR has a significantly positive impact on the perception of social equity. That is to say, BMIUR can improve residents' evaluation of social equity and further promote the level of social equity, which makes residents more happiness. The conclusion remains valid after using robustness test and propensity score matching to conduct counterfactual reasoning. The discussion of mechanism indicates that the influence of BMIUR is mediated by enhancing social capital, improving satisfaction of income distribution and reducing self-paid medical expenses. This study also finds that the influence of BMIUR is more obvious in the low-income, low-skilled and mid-west groups. Conclusion: The reform of the medical security system should be regarded as a foothold for improving people's well-being and promoting social equity; Expanding people's social capital through multiple channels and improving income distribution mechanisms; Strengthen vocational skills training, especially to provide more public services and social security for low-income groups, low-skilled groups, and groups in the western region of China, in order to improve the welfare and policy effectiveness of China's social security reform.

Abstract:Health insurance was prioritized as one of the four key areas for reform in China’s Health Care Reform Plan released in April 2009. The Plan aimed to expand coverage, enhance security, and improve payment methods, but implementation was sluggish. In 2011, the government announced that social health insurance (SHI) schemes had reached a coverage rate of 95%, but official statistics did not match the claim. Universal coverage is still an unfinished mission, while inadequate financing and benefit design remain major challenges. Despite a substantial increase in government funding over the past decade, SHI schemes have so far never accounted for more than 54% of national health care expenditure. Surplus has also become a serious policy issue for SIH funds. Finally, payment methods still require further reform and improvement. Traditional fee-for-service payments prevail in many localities, and new payment methods, such as capitation, global budgets, diagnosis-related groups (DRGs), per diem payments, and mixed payments, are only in the stage of experimentation. The health care delivery system still lacks sufficient governance structure and incentives. Health insurance reform is an important part of welfare-state building, and China still has a long path ahead before it reaches its goals in this respect.

But with the development of economics and society and the deepening of system reform, the new situation and new issues have brought about new requirements for the construction of medical insurance laws. Combined with the "Social Insurance Law", this article discussed issues of the combination of the urban and rural residents' medical insurance, the urban and rural residents' critical illness insurance, the cumulative length of payment for the retired, the transfer of medical insurance relationship, and the pre-payment and repayment of medical insurance fund. Based on the discussions, some suggestions for law revision or policy support were made.

Abstract:This paper explores the necessities and feasibilities of establishing an urban-rural coordinated basic social medical insurance in regions with different economic development levels by analyzing residents ’ willingness of choosing medical insurances and calculating the government health expenditure in Beijing Municipal and Shanxi Province.Based on the survey data in 2011,this paper analyzed and compared residents’willingness of choosing basic medical insurances and its influential demographic characters and calculated the government health expenditure after built urban-rural coordinated medical insurance system in Shanxi and Beijing by using the method of contingency table and logistic regression.The current social medical insurances which based on household couldn’t meet the needs of all residents;household,commercial medical insurance, household size and family annual income were the common demographic characters that influenced choice of residents on medical insurances in the two regions;Beijing Municipal government needed to spend another 11992.9 million Yuan and Shanxi Province government could save 10981 .5 million Yuan,if the medical insurances in each region reached the current average level of Beijing Municipal and the residents were allowed to choose medical insurances freely.It is necessary and feasible to integrate the current three basic medical insurances into three different medical insurance levels under a unified system and allow residents to choose freely in Beijing and Shanxi which are the regions with different economic development levels.

Abstract:Establishing an equitable and effective basic medical insurance system is the primary goal of China's current healthcare reform.Although some achievements have been made in this regard,the unreasonable measures taken by the government to prevent the widespread moral hazard behavior in the medical field have led to serious problems such as heavy burden of outpatient expenses,insufficient reimbursement to inpatient care,harsh cost control on medical providers and misappropriation of health insurance funds.This paper explores the possible solutions for these problems.After analyzing the nature of the basic medical insurance,we focus on investigating the moral hazard associated with the use of common resources and the principle-agent relationship.We propose that the government,confronted with two different dilemmas,should abolish the restrictions on the insured and the medical institutions,and establish an incentive based supervision and regulation mechanism in order to achieve a more effective basic medical insurance system.
Does Medical Insurance Improve Health? an Empirical Analysis from China.

Abstract:BACKGROUND:Medical insurance is considered to be closely related to individual health status, while their relationship are left to be clarified. This article aims to examine the relationship between medical insurance and the health status of residents in China.METHODS:The data is from a nationally representative sample of CGSS2015, and the estimation methods of ordered logit model, generalised ordered logit model and instrumental variable (IV) were used.RESULTS:Both public medical insurance (PMI) and commercial medical insurance (CMI) had a positive correlation with residents' self-assessed physical and mental health, while PMI was more statistically significant and substantively important than CMI. The basic results estimated with the generalised ordered logit model and IV model still remained robust. Further analysis found that medical insurance, whether public or commercial, had more or less weakened the importance of income to personal health, showing a substitute effect for income.CONCLUSION:PMI has been proven to help promote the physical and mental health of residents and moderate the importance of residents' income to health. Besides, CMI also plays a beneficial supplementary role in promoting residents' health.
