Intestinal Dysmotility in Congenital Small Intestinal Atresia

Abstract:Objective To investigate the pathohistological alterations of enteric nerves system (ENS), smooth muscle and interstitial cells of Cajal (ICCs) in congenital small intestinal atresia (IA) as a the reference for of surgical resection, and to elucidate the pathogenesis of postoperative intestinal dysmotility in IA cases. Methods Fifteen patients with IA were studied. Specimens were taken from the resected intestinal segments proximal and distal to the atresia. Six controls were obtained from postmortem neonates without gastro-intestinal or nervous system diseases. All specimens were immunochemically stained with a monoclonal antibody to S-100 protein as a neuroglia cell marker, to α-smooth muscle actin (α-SMA) as a smooth muscle marker and to c-kit protein as a marker of ICCs. Results S-100, α-SMA and c-kit positive expressions in the dilated proximal segments of IA were significantly lower than that of the controls. Overall, these alterations became less obvious at 16 cm proximal and 4 cm distal to the atresia. Conclusions The pathohistological alterations of enteric nerve, smooth muscle and ICCs of small intestine in IA patients occur in the proximal and distal segments of the atresia. This may contribute to the postoperative intestinal dysmotility. The length of the intestinal resection should include 16 cm of proximal and 4 cm of distal small bowel.
