Analysis of 32 Cases of Ovarian Malignancies in Childhood and Adolescence

Abstract:Objective To study the presentation,treatment and outcome of ovarian malignancies in childhood and adolescence.Methods A total of 32 cases of ovarian malignancies in young women less than 20 years old between Jan 1997 and Dec 2002 were retrospectively analyzed.Results The median age at the time of diagnosis was 17 years(range 4~20 years).The main presenting symptoms were abdominal pain,tympanites and dysmenorrheal.The mean maximal tumor diameter was 13.2 cm.12 patients had stage I disease,8 had stage Ⅲ disease,and 1 had stage Ⅳ disease at the time of diagnosis.56% patients had malignant germ cell tomor,41% had epithelial malignancies,and only 3% had sex-cord stroma tumor.Most tumors were well differentiated,with 71% grade 1,16% grade 2 and 13% grade 3 disease.Optimal cytoreduction was achieved in 66%(21 /32) of patients in primary treatment.17 patients were treated with conservative surgery,and 27 patients with chemotherapy.The median follow-up was 42 months(range,2~120 months).8 patients died during the follow-up period,3 were alive with tumor,17 alive free of tumor,and 4 were lost to follow-up.The overall 3-year and overall 5-year survival rate was 92.8%,and 85.7%,respectively.Conclusion Young patients with ovarian carcinoma appear to have favorable stage and histologic grade and good survival.
